Introdução: Para avaliação nutricional de pacientes com hepatopatias recomenda-se a combinação de diversos parâmetros nutricionais. Objetivo: Avaliar o estado nutricional de pacientes diagnosticados com hepatite B e/ou C. Materiais e métodos: Estudo transversal, descritivo e analítico, realizado no período de junho a novembro de 2017 em um Centro de Referência da Amazônia Oriental, com pacientes diagnosticados com hepatite B e/ou C. Foi realizada avaliação antropométrica (índice de massa corporal, circunferência do braço, prega cutânea tricipital, circunferência muscular do braço, área muscular do braço corrigida e prega cutânea subescapular) e do consumo alimentar (recordatório de 24 horas). Para análise dos dados, foram realizados teste t de Student ou de Mann-Whitney, além de teste G ou Qui-Quadrado, considerando significativos quando p≤0.05. Resultados: 81 pacientes avaliados. Em relação a avaliação antropométrica, não foi observada diferença estatística significativa entre as médias e as classificações dos grupos avaliados. Patient identification has been recognized as a care practice with the potential to cause nutrition-related safety incidents. Inappropriate or absent nutritional care can result in patient insecurity. The objectives of this study were to characterize the sociodemographic profile of the Nutrition and Dietetic Service (NDS) team, to diagnose the process of patient identification in the diets supply and to verify the team’s knowledge on the subject. This was a quantitative, qualitative, descriptive and cross-sectional study conducted in June 2018 through an interview with NDS employees of a public hospital in Belém, PA. The IRAMUTEQ Platform version 0.7 alpha 2 was employed for qualitative analysis using the Word Cloud and Similarity Analysis methods. Sixty-one employees participated in the study, most of them were Operational Assistants, females, from 31 to 40 years old, with high school education and up to 5 years of experience. The results showed that most did not know the concepts examined and did not use the guidelines in their work routine, indicating the need for educational actions aimed at implementing protocols and routines focused on patient safety.

Soroprevalence for Hepatitis C virus is reported as 2.12% in Northern Brazil, with about 50% of the patients exhibiting a sustained virological response (SVR). Aiming to associate polymorphisms in Killer Cell Immunoglobulin-like Receptors (KIR) with chronic hepatitis C and therapy responses we investigated 125 chronic patients and 345 controls. Additionally, 48 ancestry markers were genotyped to control for population stratification. The frequency of the KIR2DL2 and KIR2DL2+HLA-C Asp80 gene and ligand was higher in chronic infected patients than in controls (p < 0.0009, OR = 3.4; p = 0.001, OR = 3.45). In fact, KIR2DL3 is a weaker inhibitor of NK activity than KIR2DL2, which could explain the association of KIR2DL2 with chronic infection. Moreover, KIR2DS2 and KIR2DS2+HLA-C Asp80 (p < 0.0001, OR = 2.51; p = 0.0084, OR = 2.62) and KIR2DS3 (p < 0.0001; OR = 2.57) were associated with chronic infection, independently from KIR2DL2. No differences in ancestry composition were observed between control and patients, even with respect to therapy response groups. The allelic profile KIR2DL2/KIR2DS2/KIR2DS3 was associated with the chronic hepatitis C (p < 0.0001; OR = 3). Furthermore, the patients also showed a higher mean number of activating genes and a lower frequency of the homozygous AA profile, which is likely secondary to the association with non-AA and/or activating genes. In addition, the KIR2DS5 allele was associated with SVR (p = 0.0261; OR = 0.184).The ancestry analysis of samples ruled out any effects of population substructuring and did not evidence interethnic differences in therapy response, as suggested in previous studies.

METHODS: A retrospective study of 20 patients coinfected with HIV-1/HCV who were treated in the outpatient liver clinic at the Sacred House of Mercy Foundation Hospital of Pará (Fundação Santa Casa de Misericórdia do Pará - FSCMPA) from April 2004 to June 2009. Patients were treated with 180µg PEG interferon-α2a in combination with ribavirin (1,000 to 1,250mg/day) for 48 weeks. The end point was the sustained virological response (SVR) rate (HCV RNA negative 24 weeks after completing treatment). RESULTS: The mean age of the patients was 40±9.5 years, of which 89% (n=17) were male, and the HCV genotypes were genotype 1 (55%, n=11/20), genotype 2 (10%, n=2/20) and genotype 3 (35%, n=7/20). The mean CD4+ lymphocyte count was 507.8, and the liver fibrosis stages were (METAVIR) F1 (25%), F2 (55%), F3 (10%) and F4 (10%). The early virological response (EVR) was 60%, the end-of-treatment virological response (EOTVR) was 45% and the SVR was 45%. CONCLUSIONS: The median HCV viral load was high, and in 85% of cases in which highly active antiretroviral therapy (HAART) was used, none of the patients with F3-F4 fibrosis responded to treatment. Of the twenty patients treated, 45% achieved SVR and 45% achieved EOTVR. Studies that include cases from a wider region are needed to better evaluate these findings.

O Programa de Hepatopatias do Hospital da Fundação Santa Casa de Misericórdia do Pará surgiu pela necessidade de prestar assistência a hepatopatas na região amazônica priorizando assistência qualificada, identificação das etiologias, seguimento clínico, e tratamento direcionado. Este trabalho visa descrever dados relativos à epidemiologia clínica, fatores etiológicos e análise histopatológica. Dos 1469 pacientes avaliados, através de exames clínicos, laboratoriais, endoscópicos e de imagem e/ou histopatológico, foram considerados hepatopatas crônicos 935 (63,6%). Nesta casuística, a média de idade foi 50 anos, 666 (71,2%) do sexo masculino e maior procedência de Belém. Os agentes etiológicos mais prevalentes foram alcoolismo (53,7%) e hepatites virais (39,1%). Biópsia hepática realizada em 403/935 (43,1%), demonstrou hepatite crônica (34%) e cirrose (34%) na maioria das amostras. Conclui-se, portanto, que a doença hepática crônica na região é mais prevalente no sexo masculino, sendo o alcoolismo a principal etiologia e mais da metade dos casos se encontravam em fase avançada no momento do diagnóstico.

Mediante critérios epidemiológicos, clínicos e laboratoriais, foi levantada a casuística de equinococose policística no período de 1962 a 2003, no âmbito da Amazônia oriental brasileira, incluindo casos inéditos e aqueles já publicados. Dessa forma, foram identificados 40 casos da doença no referido período, compreendendo casos procedentes dos estados do Pará e Amapá, Brasil. A amplitude das idades foi de 10 a 72 anos. Do total 47,5% pertenciam ao sexo masculino. O fígado foi o órgão mais acometido (82,5% dos casos). O Echinococcus vogeli (Rausch e Bernstein, 1972), apresentou-se como o principal agente etiológico envolvido. A partir do reconhecimento da importância e das implicações do manejo da equinococose para a região tropical, acredita-se que deverá ocorrer uma implementação do diagnóstico precoce, tratamento adequado e de um melhor registro da doença.
