PURPOSE To develop a novel simultaneous co-registered T1 , T2 , T2∗ , T1ρ , and fat fraction abdominal MR fingerprinting (MRF) approach for fully comprehensive liver-tissue characterization in a single breath-hold scan. METHODS A gradient-echo liver MRF sequence with low fixed flip angle, multi-echo radial readout, and varying magnetization preparation pulses for multiparametric encoding is performed at 1.5 T. The T2∗ and fat fraction are estimated from a graph/cut water/fat separation method using a six-peak fat model. Water/fat singular images obtained are then matched to an MRF dictionary, estimating water-specific T1 , T2 , and T1ρ . The proposed approach was tested in phantoms and 10 healthy subjects and compared against conventional sequences. RESULTS For the phantom studies, linear fits show excellent coefficients of determination (r2 > 0.9) for every parametric map. For in vivo studies, the average values measured within regions of interest drawn on liver, spleen, muscle, and fat are statistically different from the reference scans (p 0.92 for every parameter). CONCLUSION The proposed multi-echo inversion-recovery, T2 , and T1⍴ prepared liver MRF sequence presented in this work allows for quantitative T1 , T2 , T2∗ , T1⍴ , and fat fraction liver-tissue characterization in a single breath-hold scan of 18 seconds. The approach showed good agreement and correlation with respect to reference clinical maps.

Magnetic resonance elastography (MRE) of the liver has emerged as the non-invasive standard for the evaluation of liver fibrosis in chronic liver diseases (CLDs). The utility of MRE in the evaluation of different CLD in both adults and children has been demonstrated in several studies, and MRE has been recommended by several clinical societies. Consequently, the clinical indications for evaluation of CLD with MRE have increased, and MRE is currently used as an add-on test during routine liver MRI studies or as a standalone test. To meet the increasing clinical demand, MRE is being installed in many academic and private practice imaging centers. There is a need for a comprehensive practical guide to help these practices to deliver high-quality liver MRE studies as well as troubleshoot the common issues with MRE to ensure smooth running of the service. This comprehensive clinical practice review summarizes the indications and provides an overview on why to use MRE, technical requirements, system set-up, patient preparation, acquiring the data, and interpretation.

OBJECTIVE. The objective of our study was to compare an abbreviated liver MRI protocol with a standard liver MRI protocol for the posttreatment follow-up of colorectal metastases in assessing disease presence, segmental involvement, and response to chemotherapy and for surgical planning. MATERIALS AND METHODS. This retrospective single-center study reviewed consecutive chemotherapy-naive patients with colorectal liver metastases (April 1, 2011-August 31, 2017) who underwent gadoxetate disodium-enhanced MRI on a 1.5-T unit before and 8-12 weeks after chemotherapy. Two radiologists blinded to outcomes independently reviewed images obtained using standard MRI sequences at baseline and after treatment. The standard MRI sequences were the following: axial T1-weighted, axial T2-weighted, axial DWI (b values = 0-750 s/mm2), axial multiphase contrast-enhanced T1-weighted, and axial and coronal hepatobiliary phase (HBP) T1-weighted sequences. The standard sequences obtained at baseline and the abbreviated protocol sequences (i.e., HBP gadoxetate disodium-enhanced T1-weighted and DWI sequences) obtained after treatment were reviewed. For each image set, reviewers assessed disease presence and segmental involvement; in addition, for images obtained after therapy, treatment response according to RECIST 1.1 was recorded. RESULTS. One hundred thirteen patients (73 men and 40 women; mean age, 61.6 years) were evaluated. The total number of metastases showed high agreement between the standard protocol and abbreviated protocol (intraclass correlation coefficient = 0.97). There was good agreement between the protocols for segmental involvement (weighted κ = 0.73-0.85), and the weighted kappa was 0.82 for all segments. Discrepancies in segmental involvement did not alter potential surgical planning. Categorization of RECIST 1.1 treatment response using the abbreviated protocol versus the standard protocol showed excellent agreement (weighted κ = 0.92). CONCLUSION. An abbreviated liver MRI protocol (i.e., HBP gadoxetate disodium-enhanced T1-weighted and DWI sequences) allows assessment after chemotherapy similar to a standard liver MRI protocol. Use of the abbreviated protocol can reduce imaging time without sacrificing diagnostic performance for the follow-up of colorectal liver metastases.

To assess the feasibility of a CNN-based liver registration algorithm to generate difference maps for visual display of spatiotemporal changes in liver PDFF, without needing manual annotations. This retrospective exploratory study included 25 patients with suspected or confirmed NAFLD, who underwent PDFF-MRI at two time points at our institution. PDFF difference maps were generated by applying a CNN-based liver registration algorithm, then subtracting follow-up from baseline PDFF maps. The difference maps were post-processed by smoothing (5 cm2 round kernel) and applying a categorical color scale. Two fellowship-trained abdominal radiologists and one radiology resident independently reviewed difference maps to visually determine segmental PDFF change. Their visual assessment was compared with manual ROI-based measurements of each Couinaud segment and whole liver PDFF using intraclass correlation (ICC) and Bland-Altman analysis. Inter-reader agreement for visual assessment was calculated (ICC). The mean patient age was 49 years (12 males). Baseline and follow-up PDFF ranged from 2.0 to 35.3% and 3.5 to 32.0%, respectively. PDFF changes ranged from - 20.4 to 14.1%. ICCs against the manual reference exceeded 0.95 for each reader, except for segment 2 (2 readers ICC = 0.86–0.91) and segment 4a (reader 3 ICC = 0.94). Bland-Altman limits of agreement were within 5% across all three readers. Inter-reader agreement for visually assessed PDFF change (whole liver and segmental) was excellent (ICCs > 0.96), except for segment 2 (ICC = 0.93). Visual assessment of liver segmental PDFF changes using a CNN-generated difference map strongly agreed with manual estimates performed by an expert reader and yielded high inter-reader agreement. • Visual assessment of longitudinal changes in quantitative liver MRI can be performed using a CNN-generated difference map and yields strong agreement with manual estimates performed by expert readers.

Objectives To evaluate the clinical feasibility of a new T2 weighted sequence to calculate T2 relaxation times (T2RT) of liver lesions using two-dimensional radial turbo spin echo (2DRTSE) and to evaluate this sequence by performing image quality and relaxation time comparison of multiple liver lesions. Materials and methods This prospective analysis of 2DRTSE sequences (using 22 echoes) was performed in 19 patients with 36 liver lesions. Two radiologists independently obtained T2RTs for liver lesions and scored image quality and image artifacts. Lesions were classified as cyst, hemangioma, solid, or necrotic. T2RT values were compared. Inter-reader agreement was evaluated. Results The 2DRTSE images were considered good quality with few artifacts by both radiologists. Nineteen patients were included in the study, with a total of 36 liver lesions. Two of the liver lesions were classified as cysts, 7 as hemangiomas, 4 as necrotic lesions, and 23 as solid lesions. The concordance correlation coefficient was 0.996 for the calculated T2RT of each liver lesion between the two readers, indicating good agreement. There was statically significant difference of the calculated T2RT for each lesion type. Conclusion The 2DRTSE sequence can be performed and provides good T2W image quality and a quantitative T2RT map of the entire abdomen. The liver lesions can be distinguished based on the calculated T2RT using this technique. 2DRTSE could potentially supplant the current T2-weighted imaging sequence with the benefit of quantitative T2RTs.

Purpose To determine if improvement in imaging reduces the non-resection rate (NRR) among patients with pancreatic ductal adenocarcinoma (PDAC). Methods From 2000 to 2019, 751 consecutive patients with PDAC were considered eligible for a intention-to-treat pancreatectomy and entered the operating room. In April 2011, our institution acquired a dual energy spectral computed tomography (CT) scanner and liver diffusion weighted magnetic resonance imaging (DW-MRI) was included in the imaging workup. We consequently considered 2 periods of inclusion: period #1 (February 2000–March 2011) and period #2 (April 2011–August 2019). Results All patients underwent a preoperative CT scan with a median delay to surgery of 18 days. Liver DW-MRI was performed among 407 patients (54%). Median delay between CT and surgery decreased (21 days to 16 days, P P P P P P Conclusions Due to changes in our therapeutic strategies, the NRR did not decrease during two decades despite imaging improvement.

Microvascular invasion (mVI) is the most significant independent predictor of recurrence for hepatocellular carcinoma (HCC), but its pre-operative assessment is challenging. In this study, we investigate the use of multi-parametric MRI radiomics to predict mVI status before surgery. We retrospectively collected pre-operative multi-parametric liver MRI scans for 99 patients who were diagnosed with HCC. These patients received surgery and pathology-confirmed diagnosis of mVI. We extracted radiomics features from manually segmented HCC regions and built machine learning classifiers to predict mVI status. We compared the performance of such classifiers when built on five MRI sequences used both individually and combined. We investigated the effects of using features extracted from the tumor region only, the peritumoral marginal region only, and the combination of the two. We used the area under the receiver operating characteristic curve (AUC) and accuracy as performance metrics. By combining features extracted from multiple MRI sequences, AUCs are 86.69%, 84.62%, and 84.19% when features are extracted from the tumor only, the peritumoral region only, and the combination of the two, respectively. For tumor-extracted features, the T2 sequence (AUC = 80.84%) and portal venous sequence (AUC = 79.22%) outperform other MRI sequences in single-sequence-based models and their combination yields the highest AUC of 86.69% for mVI status prediction. Our results show promise in predicting mVI from pre-operative liver MRI scans and indicate that information from multi-parametric MRI sequences is complementary in identifying mVI.

Background The impact on survival of gadoxetic acid-enhanced MRI in addition to multiphase contrast material-enhanced CT for initial staging in patients with hepatocellular carcinoma (HCC) is unknown. Purpose To compare all-cause mortality in patients with HCC who underwent CT only, CT plus non-gadoxetic acid-enhanced MRI, or CT plus gadoxetic acid-enhanced MRI as part of their initial diagnostic work-up. Materials and Methods The authors performed a nationwide retrospective cohort study of patients diagnosed with HCC in South Korea between January 2008 and December 2010. Follow-up extended through December 2014. The primary outcome was all-cause mortality. Cox proportional hazards regression model with adjustment of confounding factors was used to estimate hazard ratios (HRs) for all-cause mortality. Results Among 30 023 patients with HCC (mean age ± standard deviation, 58.5 years ± 10.7, 23 978 men), the proportions of patients in whom HCC was diagnosed using CT only, CT plus non-gadoxetic acid-enhanced MRI, and CT plus gadoxetic acid-enhanced MRI were 56.1%, 12.9%, and 31.0%, respectively. In adjusted analysis using CT only as the reference category, the HR for mortality for CT plus gadoxetic acid-enhanced MRI was 0.64 (95% confidence interval [CI]: 0.62, 0.67

MRI is the best imaging modality for detection and characterization of liver masses. There are multiple pulse sequences used in MRI and they can be confusing and difficult to understand. Therefore, four important pulse sequences are introduced in this article in order to simplify the seemingly complex pulse sequences, and allow general radiologists and clinicians of all specialties to approach MRI of liver masses with ease.

Rationale and Objective Primary sclerosing cholangitis, a chronic liver disease causes heterogeneous parenchymal changes and fibrosis. Liver stiffness measurement (LSM) with magnetic resonance Elastography (MRE) may be affected by this heterogeneous distribution. We evaluated interobserver agreement of LSM in primary sclerosing cholangitis (PSC) with manual and automated methods to study the influence of heterogeneous changes. Materials and Methods A total of 79 consecutive patients with PSC who had a liver MRI and MRE formed the study group. Three readers with 1–3 years’ experience in MRE and a MRE expert (11 years’ experience) independently performed LSM. Each reader manually drew free hand (fROI) and average (aROI) on stiffness maps. Automatic liver elasticity calculation (ALEC) was used to generate automated LSM. The expert fROI was the reference standard. Correlation analysis and absolute intra-class correlation coefficient (ICC) analysis was performed. Results LSM data of 79 livers and 315 sections were evaluated. There was excellent ICC between expert and reader fROIs (0.989, 95% confidence interval, and 0.985–0.993) and aROIs (0.971, 95% confidence interval, and 0.953–0.983) and ALEC (0.972, 0.957–0.982) with fROI performing better. The areas measured with fROIs and ALEC had moderate ICC with Expert fROI (0.64 and 0.56, respectively) whereas aROI area had a poor ICC of 0.12. Comparison of multiple methods showed significant differences in LSM between expert fROI and aROI of two readers and no significant differences for fROIs of all three readers. Conclusion LSM with MRE in PSC patients shows excellent interobserver agreement with both fROI and aROI methods with better performance with fROI. fROI may therefore be preferred for LSM measurements in PSC.

Background: Abdominal computed tomography (CT) is the standard imaging method for patients with suspected colorectal liver metastases (CRLM) in the diagnostic workup for surgery or thermal ablation. Diffusion-weighted and gadoxetic-acid-enhanced magnetic resonance imaging (MRI) of the liver is increasingly used to improve the detection rate and characterization of liver lesions. MRI is superior in detection and characterization of CRLM as compared to CT. However, it is unknown how MRI actually impacts patient management. The primary aim of the CAMINO study is to evaluate whether MRI has sufficient clinical added value to be routinely added to CT in the staging of CRLM. The secondary objective is to identify subgroups who benefit the most from additional MRI.Methods: In this international multicentre prospective incremental diagnostic accuracy study, 298 patients with primary or recurrent CRLM scheduled for curative liver resection or thermal ablation based on CT staging will be enrolled from 17 centres across the Netherlands, Belgium, Norway, and Italy. All study participants will undergo CT and diffusion-weighted and gadoxetic-acid enhanced MRI prior to local therapy. The local multidisciplinary team will provide two local therapy plans: first, based on CT-staging and second, based on both CT and MRI. The primary outcome measure is the proportion of clinically significant CRLM (CS-CRLM) detected by MRI not visible on CT. CS-CRLM are defined as liver lesions leading to a change in local therapeutical management. If MRI detects new CRLM in segments which would have been resected in the original operative plan, these are not considered CS-CRLM. It is hypothesized that MRI will lead to the detection of CS-CRLM in >= 10% of patients which is considered the minimal clinically important difference. Furthermore, a prediction model will be developed using multivariable logistic regression modelling to evaluate the predictive value of patient, tumor and procedural variables on finding CS-CRLM on MRI.Discussion: The CAMINO study will clarify the clinical added value of MRI to CT in patients with CRLM scheduled for local therapy. This study will provide the evidence required for the implementation of additional MRI in the routine work-up of patients with primary and recurrent CRLM for local therapy.

PurposeTo investigate the feasibility of a fast liver magnetic resonance imaging (MRI) protocol for lesion detection in adults using 3.0-T MRI.MethodsA fast liver MRI exam protocol was proposed. The protocol included motion-resistant coronal T2-w sequence, axial T2-w fast spin echo sequence with fat suppression, axial in-op phase gradient recalled echo (GRE) T1, axial diffusion weighted imaging (DWI), and axial contrast-enhanced T1 sequences. To evaluate the diagnostic capacity of the proposed protocol, 31 consecutive patients (20 males and 11 females; mean age, 53.2 years) underwent a liver MRI exam with conventional sequences, including the proposed protocol as a subset. Images from the conventional protocol and extracted abbreviated protocol were independently read, and the diagnostic concordance rate was assessed for each patient. The concordance analysis is presented as the proportion of concordant cases between the two protocols.ResultsThe net measurement time of the fast liver MRI protocol without adjustment and waiting time were 4 min and 28 s. In the 31 patients included in this study, 139 suspicious findings were found from both the conventional liver MR protocol and the fast liver MRI protocol. The diagnostic concordance rate was 96.4%.ConclusionsThe fast liver MRI protocol is feasible at 3.0-T, with a shorter exam time and high diagnostic concordance compared to the conventional liver MRI workflow.

Objectives To evaluate gadoxetic acid-enhanced liver MRI (EOB-MRI) versus contrast-enhanced computed tomography (CECT) for preoperative detection of liver metastasis (LM) and reduction of open-close laparotomies for pancreatic ductal adenocarcinoma (PDAC). Methods Sixty-six patients with PDAC had undergone preoperative EOB-MRI and CECT. LM detection by EOB-MRI and CECT and their impact on surgical planning, open-close laparotomies were compared by clinical and radiology reports and retrospective analysis of imaging by two blinded independent readers. Histopathology or imaging follow-up was the reference standard. Statistical analysis was performed at patient and lesion levels with two-sided McNemar tests. Results EOB-MRI showed higher sensitivity versus CECT (71.7% [62.1-80.0] vs. 34% [25.0-43.8]; p = 0.009), comparable specificity (98.6%, [96.9-99.5] vs. 100%, [99.1-100], and higher AUROC (85.1%, [80.4-89.9] vs. 66.9%, [60.9-73.1]) for LM detection. An incremental 7.6% of patients were excluded from surgery with a potential reduction of up to 13.6% in futile open-close laparotomies due to LM detected on EOB-MRI only. Conclusions Preoperative EOB-MRI has superior diagnostic performance in detecting LM from PDAC. This better informs surgical eligibility with potential reduction of futile open-close laparotomies from attempted curative intent pancreatic cancer surgery.

Background Iron overloaded Beta Thalassemia major (β-TM) patients have a high risk of liver problems. In recent years studies revealed that vitamin D level is decreased in chronic liver diseases. The present study was designed to find the association between the serum vitamin D levels and the liver iron deposition in patients with β-TM. Materials and methods A total of 101 patients with a diagnosis of β-TM were included into this study. The patients were divided into four groups according to liver T2* MRI scores (group 1: normal, group 2: mild iron load, group 3: moderate iron load and group 4: severe iron load). Serum vitamin D was measured by chemiluminescence immunoassay method. Results The vitamin D level was median 14 (4–91) ng/mL. There was a positive correlation between vitamin D levels and liver T2* MRI scores (r = 0.31, p Conclusions Vitamin D monitorization and supplementation should be routine in β-TM patients to prevent both skeletal and non-skeletal complications.

Intensity inhomogeneity (IIH), also named as bias field, is an undesired phenomenon of liver magnetic resonance imaging (MRI) which severely affects the quantitative analysis of medical image and decreases the performance of subsequent computer aided diagnosis (CAD) of liver cirrhosis. Many algorithms have been proposed to reduce or eliminate IIH of MRI, and some notable achievements for brain MRI have been obtained. However, IIH correction of abdominal MRI receives less attention and is challenging because of the irregular structure and the wide intensity range of different tissues. In this paper, an automatic method based on the global intensity, the local intensity and the spatial continuity information is presented for reducing IIH of liver MRI. What should be noted is that the gray level should be preserved after correction since it is important for subsequent quantitative image analysis. Therefore, a constraint term is introduced based on the information of bias field intensity for appropriate IIH correction. In addition, the objective function introduces a fuzzy membership mask to remove the background noise and avoid misclassification. Our method is successfully applied to the clinical liver MRI and acquires desirable results. Compared with other approaches, our method obtains the best segmentation with Jaccard similarity (JS) = 0.88 ± 0.06, Dice index (DI) = 0.94 ± 0.03, and accuracy (ACC) = 0.99 ± 0.01.

PURPOSE To analyze the predictive value of ΔT1 of the liver and spleen as well as the extracellular volume fraction (ECV) of the spleen as noninvasive biomarkers for the determination of clinically significant portal hypertension (CSPH) on routine Gd-EOB-DTPA liver MRI. METHOD 195 consecutive patients with known or suspected chronic liver disease from 9/2018 to 7/2019 with Gd-EOB-DTPA liver MRI and abdominal T1 mapping were retrospectively included. Based on the presence of splenomegaly with thrombocytopenia, ascites and portosystemic collaterals, the patients were divided into noCSPH (n = 113), compensated CSPH (cCSPH, ≥1 finding without ascites; n = 55) and decompensated CSPH (dCSPH, ascites ± other findings; n = 27). T1 times were measured in the liver, spleen and abdominal aorta in the unenhanced and contrast-enhanced T1 maps. Native T1 times and ΔT1 of the liver and spleen as well as ECV of the spleen were compared between groups using the Kruskal-Wallis test with Dunn's post hoc test. Furthermore, cutoff values for group differentiation were calculated using ROC analysis with Youden's index. RESULTS ΔT1 of the liver was significantly lower in patients with cCSPH and dCSPH (p 0.29 and > 41.9, respectively. CONCLUSION ΔT1 of the liver and spleen in addition to ECV of the spleen allow for determination of CSPH on routine Gd-EOB-DTPA liver MRI.

Intravoxel incoherent motion (IVIM) theory in MRI was proposed to account for the effect of vessel/capillary perfusion on the aggregate diffusion weighted MR signal. The prevalent IVIM modeling is based on equation-1: SI(b)/SI(0)= (1 -PF) × exp(-b× Dslow) + PF × exp(-b× Dfast) [1] where SI(b)and SI(0)denote the signal intensity of images acquired with the b-factor value ofbandb=0 s/mm2, respectively. We recently reported that, for the liver and likely for other organs as well, IVIM modeling of the perfusion component is constrained by the diffusion component, with a reducedDslowmeasure leading to artificially higher PF andDfastmeasures. With higher b-value associated lower image signal of the targeted tissue, Euqation-1 is focused on describing the signal decay pattern along increasingly higherb-values by three IVIM parameters. Signal intensity at eachb-value (i.e., SI(b)) is normalised by the signal intensity ofb=0 image (i.e., SI(0)). We noted an apparent problem for Euqation-1. For example, if we want to compare the IVIM parameters of the normal liver parenchyma and a liver tumor, following Euqation-1 we will take the assumption that the SI(0)of the normal parenchyma and the tumor are the same and considered equally as 1 (or 100) for the biexponential decay modelling. However, this assumption is invalid for many scenarios. From our liver IVIM database of 27 healthy female subjects, we chose six of the youngest subjects (20-27 yrs) and six of the oldest subjects (58-71 yrs) and measured the signals of the liver and left erector spinae muscle onb=0 and 2 s/mm2images. The results show, while there was no apparent difference of left erector spinae muscle signal among the young and elderly groups, the elderly group’s liver SI(0)is approximately 20 % lower than that of young group. This difference skewed the ratios of various SI(b)/SI(0)and the followed IVIM parameter determination. The general trend is that lower liver SI(0)is associated with lowerDslowand higher PF andDfast. If IVIM bi-exponential decay fitting starts from a very low non-zero b images (such asb=2 s/mm2images), this problem persists. We performed an additional analysis of our IVIM database of five cirrhotic livers and the results show SI(b=2)of cirrhotic right liver is positively associatedDslow(Pearson r=0.687), and negatively associated with PF (Pearsonr=-0.733). Though the examples we used in this letter are on liver aging and liver fibrosis, the points discussed are expected to be generalisable to other pathologies and to other organs.

Chronic liver diseases (CLDs) are becoming increasingly more prevalent in modern society. The use of imaging techniques for early detection, such as magnetic resonance imaging (MRI), is crucial in reducing the impact of these diseases on healthcare systems. Artificial intelligence (AI) algorithms have been shown over the past decade to excel at image-based analysis tasks such as detection and segmentation. When applied to liver MRI, they have the potential to improve clinical decision making, and increase throughput by automating analyses. With Liver diseases becoming more prevalent in society, the need to implement these techniques to utilize liver MRI to its full potential, is paramount. In this review, we report on the current methods and applications of AI methods in liver MRI, with a focus on machine learning and deep learning methods. We assess four main themes of segmentation, classification, image synthesis and artefact detection, and their respective potential in liver MRI and the wider clinic. We provide a brief explanation of some of the algorithms used and explore the current challenges affecting the field. Though there are many hurdles to overcome in implementing AI methods in the clinic, we conclude that AI methods have the potential to positively aid healthcare professionals for years to come.

Reading and interpreting the medical image still remains the most challenging task in radiology. Through the important achievement of deep Convolutional Neural Networks (CNN) in the context of medical image classification, various clinical applications have been provided to detect lesions from Magnetic Resonance Imaging (MRI) and Computed Tomography (CT) scans. In the diagnosis process for the liver cancer from Dynamic Contrast-Enhanced MRI (DCE-MRI), radiologists consider three phases during contrast injection: before injection, arterial phase, and portal phase for instance. Even if the contrast agent helps in enhancing the tumoral tissues, the diagnosis may be very difficult due to the possible low contrast and pathological tissues surrounding the tumors (cirrhosis). Alongside, in the medical field, ontologies have proven their effectiveness to solve several clinical problems such as offering shareable terminologies, vocabularies, and databases. In this article, we propose a multi-label CNN classification approach based on a parallel preprocessing algorithm. This algorithm is an extension of our previous work cited in the International Conference on Pattern Recognition and Artificial Intelligence (ICPRAI) 2020. The aim of our approach is to ameliorate the detection of HCC lesions and to extract more information about the detected tumor such as the stage, the localization, the size, and the type thanks to the use of ontologies. Moreover, the integration of such information has improved the detection process. Background The recently described "macrotrabecular-massive" (MTM) histologic subtype of hepatocellular carcinoma (HCC) (MTM-HCC) represents an aggressive form of HCC and is associated with poor survival. Purpose To investigate whether preoperative MRI can help identify MTM-HCCs in patients with HCC. Materials and Methods This retrospective study included patients with HCC treated with surgical resection between January 2008 and February 2018 and who underwent preoperative multiphase contrast material-enhanced MRI. Least absolute shrinkage and selection operator (LASSO)-penalized and multivariable logistic regression analyses were performed to identify clinical, biologic, and imaging features associated with the MTM-HCC subtype. Early recurrence (within 2 years) and overall recurrence were evaluated by using Kaplan-Meier analysis. Multivariable Cox regression analysis was performed to determine predictors of early and overall recurrence. Due to significant improvements in morbidity and mortality, surgery with curative intent is now the standard of care for patients with colorectal liver metastases who are surgical candidates. As a result, highly accurate preoperative per-lesion diagnosis is crucial. However, in some instances, this remains limited with standard techniques, including magnetic resonance imaging (MRI) with conventional contrast agents. Delayed retention of contrast of fibrotic liver metastases on MRI with extracellular contrast agents may mimic the late retention of contrast in hemangiomas and represent a diagnostic pitfall that limits diagnosis. Early preliminary work suggests that this imaging pitfall may not be seen on MRI with intravascular contrast agents (e.g., gadofosveset). This case report describes a surgical patient with colorectal liver metastases where gadofosveset-enhanced liver MRI was helpful in determining patient management.

Purpose: The aim of this project is to investigate the usefulness of the absolute liver lesion ADC value and ratio of Apparent diffusion coefficient (ADC) values of a liver lesion and liver parenchyma to discriminate between a benign and malignant lesion.Methods: Liver MRI scans performed between January 2009 and June 2015 were retrospectively analysed. Scans were performed on either a 1.5 T or 3 T MRI unit. The type of liver lesion (benign or malignant) was determined by its radiological appearance, histology result and clinical management. Lesions with undetermined diagnosis or MRI studies degraded by artifacts were excluded. Liver cysts were also excluded from the analysis. ADC value of a lesion and liver parenchyma was measured and ADCratio was calculated. The values were analysed using independent samples t-testResults:Data set contained 39 benign lesions and 36 malignant lesions. Mean ADC value for benign lesions was 1678, and the mean value for malignant lesions was 1097 with a statistically significant difference of p 1.5 were benign. ADCratio of 1.1 was identified statistically as the best available cut-off value for differentiating benign from malignant lesions, with sensitivity of 82%, specificity of 86% and an overall accuracy of 92%.Conclusion:Our dataset indicates that lesion to background liver ADCratio is superior in discriminating between benign and malignant focal lesions compared to absolute ADC values of the hepatic lesions. PURPOSE: In this study, we compare an abbreviated screening MRI protocol (aMRI), utilizing only dynamic contrast-enhanced images, to a conventional liver MRI (cMRI) for the characterization of observations in at-risk patients. MATERIALS & METHODS: 164 consecutive HCC screening MRIs were retrospectively analyzed. Two sets of deidentified image sets were created: one with all acquired sequences including T2- and diffusion-weighted sequences (cMRI), and one with only T1-weighted precontrast and dynamic post-contrast images utilizing an extracellular gadolinium contrast agent (aMRI). Three readers assigned a LI-RADS score based on the lesion with the highest LI-RADS category using the aMRI and cMRI data sets during separate reads. RESULTS: There was no change between the aMRI and cMRI LI-RADS categorization in 93%, 96% and 96% of cases for readers 1, 2 and 3 respectively. In the majority of the discrepant cases, the score increased from LI-RADS 3 to LI-RADS 4 due to the presence of ancillary features on T2 and DWI. Kappa values for interobserver variability demonstrated fair to moderate LI-RADS agreement among the 3 readers. CONCLUSION: There was strong agreement between the abbreviated T1-only MRI protocol and a full liver MRI, with only 5% of cases changing LI-RADS categorization due to the inclusion of T2 and DWI. The estimated time to run this abbreviated MRI is approximately 7–10 minutes, possibly allowing for a more cost-effective screening MRI than our cMRIs.

Liver MR is a well-established modality with multiparametric capabilities. However, to take advantage of its full capacity, it is mandatory to master the technique and optimize imaging protocols, apply advanced imaging concepts and understand the use of different contrast media. Physiologic artefacts although inherent to upper abdominal studies can be minimized using triggering techniques and new strategies for motion control. For standardization, the liver MR protocol should include motion-resistant T2-w sequences, in-op phase GRE T1 and T2-w fast spin echo sequences with fat suppression. Diffusion-weighted imaging (DWI) is mandatory, especially for detection of sub-centimetre metastases. Contrast-enhanced MR is the cornerstone of liver MR, especially for lesion characterization. Although extracellular agents are the most extensively used contrast agents, hepatobiliary contrast media can provide an extra-layer of functional diagnostic information adding to the diagnostic value of liver MR. The use of high field strength (3T) increases SNR but is more challenging especially concerning artefact control. Quantitative MR belongs to the new and evolving field of radiomics where the use of emerging biomarkers such as perfusion or DWI can derive new information regarding disease detection, prognostication and evaluation of tumour response. This information can overcome some of the limitations of current tests, especially when using vascular disruptive agents for oncologic treatment assessment. MR is, today, a robust, mature, multiparametric imaging modality where clinical applications have greatly expanded from morphology to advanced imaging. Purpose Varying frequencies (5 – 18 %) of contrast-related transient severe motion (TSM) imaging artifacts during gadoxetate disodium-enhanced arterial phase liver MRI have been reported. Since previous reports originated from the United States and Japan, we aimed to determine the frequency of TSM at a German institution and to correlate it with potential risk factors and previously published results. Materials and Methods Two age- and sex-matched groups were retrospectively selected (gadoxetate disodium n = 89; gadobenate dimeglumine n = 89) from dynamic contrast-enhanced MRI examinations in a single center. Respiratory motion-related artifacts in non-enhanced and dynamic phases were assessed independently by two readers blinded to contrast agents on a 4-point scale. Scores of ≥ 3 were considered as severe motion artifacts. Severe motion artifacts in arterial phases were considered as TSM if scores in all other phases were Results For gadoxetate disodium, the mean score for respiratory motion artifacts was significantly higher in the arterial phase (2.2 ± 0.9) compared to all other phases (1.6 ± 0.7) (p 0.05). Liver cancer is one of the leading causes of cancer-related mortality worldwide. Magnetic resonance imaging (MRI) is a non-invasive imaging technique that is often used by radiologists for diagnosis and surgical planning. Analysis of a large amount of liver MRI data for each patient limits the radiologist's efficiency and may lead to misdiagnoses. The redundant MRI data, especially from dynamic contrast enhanced (DCE) sequences, is also a bottleneck in transmitting the images via the internet or PACS for remote consultancy in a reasonable amount of time. This study included 25 patients (aged between 20 and 70 years) with liver cysts (seven cases), hemangiomas (eight cases), or hepatic cell carcinomas (10 cases). DCE T1WI MRI was performed for all the patients. The diagnosis reference included typical MRI findings and post-surgery pathology. The methods were as follows: (i) MRI sequence pre-processing based on large vessels variation level set method to remove non-liver parts from MRI images; (ii) human visual model features (luminance, motion, and contour) extraction and fusion; (iii) anomaly-based MRI ranking; and (iv) methods assessment with the 25 patients' DCE MRI data. The prioritization methods applied to the DCE images could automatically assimilate and determine the content of the medical images, identifying the liver cysts, hemangiomas, and carcinomas. The average uniformity between radiologists and prioritization with the proposed method was 0.805, 0.838, and 0.818 for cysts, hemangiomas, and carcinomas, respectively, which indicates that the proposed method is an efficient method for liver DCE image prioritization.

MRI-based corrected T1 (cT1) is a non-invasive method to grade the severity of steatohepatitis and liver fibrosis. We aimed to identify genetic variants influencing liver cT1 and use genetics to understand mechanisms underlying liver fibroinflammatory disease and its link with other metabolic traits and diseases.First, we performed a genome-wide association study (GWAS) in 14,440 Europeans, with liver cT1 measures, from the UK Biobank. Second, we explored the effects of the cT1 variants on liver blood tests, and a range of metabolic traits and diseases. Third, we used Mendelian randomisation to test the causal effects of 24 predominantly metabolic traits on liver cT1 measures.We identified 6 independent genetic variants associated with liver cT1 that reached the GWAS significance threshold (p 5×10The association between 2 metal ion transporters and cT1 indicates an important new mechanism in steatohepatitis. Future studies are needed to determine whether interventions targeting the identified transporters might prevent liver disease in at-risk individuals.We estimated levels of liver inflammation and scarring based on magnetic resonance imaging of 14,440 UK Biobank participants. We performed a genetic study and identified variations in 6 genes associated with levels of liver inflammation and scarring. Participants with variations in 4 of these genes also had higher levels of markers of liver cell injury in blood samples, further validating their role in liver health. Two identified genes are involved in the transport of metal ions in our body. Further investigation of these variations may lead to better detection, assessment, and/or treatment of liver inflammation and scarring.

Objective: To test the hypothesis that there is a measureable upward motion of the diaphragm during prolonged breath-holds that could have a detrimental effect on image quality in liver MRI and to identify factor that potentially influence the magnitude of this motion. Methods: 15 healthy volunteers underwent MRI examination using prolonged breath-holds in the maximum inspiratory position and a moderate inspiratory position. Coronal T1 weighted three-dimensional gradient echo sequences of the entire thorax were acquired every 6 s during breath-holding allowing the calculation of total lung volume and the measurement of the absolute position of the dome of the liver. The potential impact of subject’s gender, body mass index, and total lung capacity on the change in lung volume/diaphragmatic motion was assessed using random coefficient regression. Results: All volunteers demonstrated a slow reduction of the total lung volume during prolonged breath-holding up to 123 ml. There was a measurable associated upward shift of the diaphragm, measuring up to 5.6 mm after 24 s. There was a positive correlation with female gender (p = 0.037) and total lung volume (p = 0.005) and a negative association with BMI (p = 0.012) for the maximum inspiratory position only. Conclusion: There is a measureable reduction of lung volumes with consecutive upward shift of the diaphragm during prolonged breath-holding which likely contributes to motion artifacts in liver MRI. Rapid injection of gadoxetic acid has been shown not to increase tumor enhancement in arterial phase liver MRI for unknown reasons. This study aimed to investigate the effect of injection durations on peak contrast concentration in tumors and to correlate it with signal enhancement in gadoxetic acid-enhanced arterial phase MRI.Gadoxetic acid-enhanced arterial phase MRI was obtained using a bolus-tracking technique with injection durations of 1, 3, and 6s in six rabbits with VX2 liver tumors. The peak concentration of gadoxetic acid in the aorta and tumor was estimated by iopromide-enhanced time-resolved CT using the same injection volume and durations with those for MRI. Signal enhancement on MRI and peak enhancement on CT were compared and correlated.There was no significant difference in MR signal enhancement of tumors among the 3 injection durations (p = 0.87). In CT, shorter injection durations significantly increased peak contrast concentration in the aorta (p0.01) but produced equivalent peak contrast concentration in tumors (p = 0.24). The longer injections resulted in the stronger correlation between peak contrast concentration in CT and MR signal enhancement in tumors (r = 0.31, 0.43, and 0.86 with 1s-, 3s-, and 6s-injection, respectively) with a statistical significance only found with 6s-injection (p = 0.03).Estimation of contrast concentration by CT demonstrated that shorter injections did not increase peak contrast concentration in tumors despite increased peak concentration in the aorta. Furthermore, tumor signal enhancement in gadoxetic acid-enhanced arterial phase MRI was less correlated with the peak contrast concentration with shorter injections.

Background Imaging is an essential tool in the management of patients with Colorectal cancer (CRC) by helping evaluate number and sites of metastases, determine resectability, assess response to treatment, detect drug toxicities and recurrences. Although multidetector computed tomography (MDCT) is the first tool used for staging and patient’s surveillance, magnetic resonance imaging (MRI) is the most reliable imaging modality that allows to assess liver metastases. Our purpose is to compare the diagnostic performance of gadoxetic acid-(Gd-EOB) enhanced liver MRI and contrast-enhanced MDCT in the detection of liver metastasis from colorectal cancer (mCRC). Methods One hundred and twenty-eight patients with pathologically proven mCRC (512 liver metastases) underwent Gd-EOB MRI and MDCT imaging. An additional 46 patients without mCRC were included as control subjects. Three radiologists independently graded the presence of liver nodules on a five-point confidence scale. Sensitivity and specificity for the detection of metastases were calculated.
