8. Effectiveness of Non-pharmacological Interventions on Symptom Clusters in Cancer Patients: a Network Meta-analysis

Abstract

Background The presence of symptom clusters is associated with poor treatment outcomes and decreased quality of life in patients with cancer, and holistic interventions for symptom clusters can improve the management efficiency. Numerous non-pharmacologic interventions are available for symptom clusters in patients with cancer, but there is disagreement as to which interventions is the most effective. Objective To explore and compare the effectiveness of different non-pharmacological interventions in reducing the severity of symptom clusters in cancer patients. Methods In September 2021, randomized controlled trials (RCTs) about non-pharmacological interventions in reducing the severity of symptom clusters in cancer patients were searched in SinoMed, CNKI, Wanfang Data, CQVIP, the Cochrane Library, PubMed, Web of Science and Wiley Online Library databases from inception to September 2021. Two researchers independently screened studies based on the inclusion and exclusion criteria, extracted data, and performed risk of bias assessment using the Cochrane handbook for systematic reviews of interventions (version 5.1.0) . RevMan 5.3 was adopted for heterogeneity test using a traditional method and publication bias analysis. Addis 16.6 was used to make a network meta-analysis. Results Twelve RCTs were finally included, involving 1 116 cases, and eight non-pharmacologic interventions: cognitive behavior intervention, mindfulness-based stress reduction, acupoint massage, multiple-modality exercise, exercise and health counseling, auricular point sticking therapy, narrative nursing, and attention control. Network meta-analysis results showed that, for reducing the severity of symptom clusters in cancer patients, acupoint massage was superior to interventions other than mindfulness-based stress reduction, mindfulness-based stress reduction was superior to usual care and narrative nursing, and cognitive behavioral intervention was superior to multiple-modality exercise, usual care and narrative nursing (Pmindfulness-based stress reduction>cognitive behavior intervention>auricular point sticking therapy/attention control>multiple-modality exercise>usual care>narrative nursing>exercise and health counseling. Conclusion Based on the results of network meta-analysis and probability ranking, acupoint massage was recommended as the clinical priority non-pharmacological intervention to alleviate the severity of symptom clusters in cancer patients, followed by mindfulness-based stress reduction. In the future, high-quality, large-sample, multicenter, double-blind RCTs are needed to further validate the effects of different non-pharmacological interventions on improving symptom clusters in cancer patients, and to provide more reliable evidence for supporting the development of interventions for symptom clusters in these patients.

18. A Novel High-Misalignment Tolerant Magnetic Coupler for IPT System With Concentrated Magnetic Flux and Counteracting Coil

Abstract

The capability of misalignment tolerance between the primary and secondary coils in an inductive power transfer system is vital. This paper proposes a novel coupling structure composed of a solenoid and a planar square coil with a third coil (SPS-TC) that can significantly improve the misalignment tolerance of couplers. The novelty of SPS-TC is the combination of concentrated magnetic fields and reverse magnetic fields. The proposed SPS-TC has an excellent characteristic that when coils are misaligned in the horizontal direction, the coupling coefficient varies within a very small range. A 1-kW prototype is fabricated. The experimental results show that when the horizontal misaligned distance does not exceed 37.5% of the coil size, the output voltage variation range is extremely small, and the minimum system efficiency is 90.12%. The benefits of SPS-TC are a smaller outer side length and excellent horizontal and rotational misalignment tolerance. It can be used for wireless charging applications in robots, rockets, satellites, unmanned aerial vehicles, etc.

Abstract

This study presents a cost-effective sensorless whole-body compliance control strategy for collaborative manipulator. The control strategy realizes decoupled adjustable compliant effects, namely, stiffness, damping, and inertia controls, under a single control framework. The inherent position controller is retained, which ensures a smooth transition between normal position operation and compliance control. The two features can greatly simplify the customization of collaborative manipulator control algorithms. A modified sensorless disturbance observer based on generalized momentum is used to estimate the external torque, and this way eliminates the dependence on the force/torque sensors. Only basic motor position sensors are required. The compliant trajectory generated by the external torque is sufficiently smooth owing to the haptic filter. Various experiments prove that the modified sensorless disturbance observer is effective. The necessity of using the position servo loop for sensorless compliance control is discussed through a comparative experiment. The proposed compliance control strategy is further verified using sensorless and sensor-based disturbance observers.

Abstract

Protease biocatalysis in a high-salt environment is very attractive for applications in the detergent industry, the production of diagnostic kits, and traditional food fermentation. However, high-salt conditions can reduce protease activity or even inactivate enzymes. Herein, in order to explore new protease sources, we expressed a salt-tolerant pseudolysin of Pseudomonas aeruginosa SWJSS3 isolated from deep-sea mud in Saccharomyces cerevisiae. After optimizing the concentration of ion cofactors in yeast peptone dextrose (YPD) medium, the proteolytic activity in the supernatant was 2.41 times more than that in the control group when supplemented with 5 mM CaCl2 and 0.4 mM ZnCl2. The extracellular proteolytic activity of pseudolysin reached 258.95 U/mL with optimized expression cassettes. In addition, the S. cerevisiae expression system increased the salt tolerance of pseudolysin to sodium chloride (NaCl)and sodium dodecyl sulfate (SDS) and the recombinant pseudolysin retained 15.19% activity when stored in 3 M NaCl for 7 days. The recombinant pseudolysin was able to efficiently degrade the β-conglycinin from low-denatured soy protein isolates and glycinin from high-denatured soy protein isolates under high temperatures (60 °C) and high-salt (3 M NaCl) conditions.
