Abstract

Background: In Canada, as in other parts of the world, there is geographic maldistribution of the nursing workforce, and insufficient attention is paid to the strengths and needs of those providing care in rural and remote settings. In order to inform workforce planning, a national study, Nursing Practice in Rural and Remote Canada II, was conducted with the rural and remote regulated nursing workforce (registered nurses, nurse practitioners, licensed or registered practical nurses, and registered psychiatric nurses) with the intent of informing policy and planning about improving nursing services and access to care. In this article, the study methods are described along with an examination of the characteristics of the rural and remote nursing workforce with a focus on important variations among nurse types and regions. Methods: A cross-sectional survey used a mailed questionnaire with persistent follow-up to achieve a stratified systematic sample of 3822 regulated nurses from all provinces and territories, living outside of the commuting zones of large urban centers and in the north of Canada. Results: Rural workforce characteristics reported here suggest the persistence of key characteristics noted in a previous Canada-wide survey of rural registered nurses (2001-2002), namely the aging of the rural nursing workforce, the growth in baccalaureate education for registered nurses, and increasing casualization. Two thirds of the nurses grew up in a community of under 10000 people. While nurses’ levels of satisfaction with their nursing practice and community are generally high, significant variations were noted by nurse type. Nurses reported coming to rural communities to work for reasons of location, interest in the practice setting, and income, and staying for similar reasons. Abstract

To investigate family physicians' differential diagnoses of clinical-scenario patients presenting with symptoms of either generalized anxiety disorder (GAD) or a major depressive episode (MDE).Cross-sectional survey.Saskatchewan.A total of 331 family physicians practising in Saskatchewan as of December 2007.Type and number of physicians' differential diagnoses for a GAD-scenario patient and an MDE-scenario patient.The survey response rate was 49.7% (331 of 666 surveys returned). Most physicians suggested a diagnosis of anxiety (82.5%) for the GAD-scenario patient and a diagnosis of depression (84.2%) for the MDE-scenario patient. In descending order, the 5 most frequent differential diagnoses for the GAD-scenario patient were anxiety, hyperthyroidism, depression, panic disorder or attack, and bipolar disorder. The 5 most frequent differential diagnoses for the MDE-scenario patient were depression, anxiety, hypothyroidism, irritable bowel syndrome, and anemia. Neither a diagnosis of anxiety nor a diagnosis of depression was associated with physicians' personal attributes (sex, age, and years in practice) or organizational setting (number of total patient visits per week, private office or clinic, solo practice, Internet access, and rural practice setting). However, physicians in solo practice suggested fewer differential diagnoses for the GAD-scenario patient than those in group practice; physicians in practice 30 years or longer suggested fewer differential diagnoses for the MDE-scenario patient than those in practice fewer than 10 years. On average, physicians suggested 3 differential diagnoses for each of the scenarios.Most family physicians recognize depression and anxiety in patients presenting with symptoms of these disorders and consider an average of 3 differential diagnoses in each of these cases.

Objectives: To document the frequency of self-reported use of dental services within a 2-year period by a sample of dentate Saskatchewan residents aged 18 years and older; explore the correlates associated with dental service use for men and women separately; and examine the age, gender and income distribution of dental insurance coverage. Methods: The 1999-2000 Saskatchewan Population Health and Dynamics Survey provided a representative sample of 5,003 dentate respondents (mean age 45.5 years; 52.3% women), 18 years and older, randomly selected and interviewed by telephone. Results: Of those interviewed, 77% of the women and 67% of the men reported a dental visit within the previous 2 years. For women, logistic regression analysis indicated that when all other factors were held constant, the odds of dental service use within the last 2 years were higher among those who were aged 18-19 years, had post-secondary education or technical certification, were in the highest household income adequacy category, held dental insurance and engaged in such preventive behaviours as regular general checkups, eye checkups, skin self-examination and not smoking daily. For men, the odds of dental service use within the last 2 years were greater if they had education at or beyond the secondary level, were students, were in the upper or highest income adequacy levels, held dental insurance and engaged in preventive health behaviours such as general checkups, eye checkups and not smoking daily. Dental insurance coverage was strongly associated with household income adequacy and peaked among men and women aged 30-49 years. Conclusions: This study indicates that the roles of gender, income, education, age, health-related behaviours and resources such as dental insurance should be considered in future public oral health strategies. Abstract

Background|objective: Nurses provide essential primary care (PC) in rural and remote Canada. We examined the practice context and responsibilities of this little-known understudied workforce., Method: Data from Nursing Practice in Rural and Remote Canada II, a 2014 to 2015 pan-Canadian survey, were analyzed., Results: Of 3,822 respondents, 192 identified that PC was their only practice focus (PC-Only), and for 111, it was one focus among others (PC-Plus). Proportionally more PC-Only than PC-Plus nurses had graduate education, were employed in larger communities and had experienced higher job resources and lower job demands. Proportionally fewer PC-Only than PC-Plus nurses followed protocols/decision support tools, dispensed medications and provided emergency services. Proportionally more PC-Only than PC-Plus nurses ordered advanced diagnostic tests/imaging, and fewer PC-Only than PC-Plus nurses performed and interpreted laboratory tests and diagnostic imaging on site., Conclusion: Contributions of the rural and remote nursing workforce to PC are rendered invisible by contemporary characterizations of the PC workplace, limiting evaluation and improvement efforts., (Copyright © 2020 Longwoods Publishing.)

Abstract

Objective: To investigate family physicians' differential diagnoses of clinical-scenario patients presenting with symptoms of either generalized anxiety disorder (GAD) or a major depressive episode (MDE)., Design: Cross-sectional survey., Setting: Saskatchewan., Participants: A total of 331 family physicians practising in Saskatchewan as of December 2007., Main Outcome Measures: Type and number of physicians' differential diagnoses for a GAD-scenario patient and an MDE-scenario patient., Results: The survey response rate was 49.7% (331 of 666 surveys returned). Most physicians suggested a diagnosis of anxiety (82.5%) for the GAD-scenario patient and a diagnosis of depression (84.2%) for the MDE-scenario patient. In descending order, the 5 most frequent differential diagnoses for the GAD-scenario patient were anxiety, hyperthyroidism, depression, panic disorder or attack, and bipolar disorder. The 5 most frequent differential diagnoses for the MDE-scenario patient were depression, anxiety, hypothyroidism, irritable bowel syndrome, and anemia. Neither a diagnosis of anxiety nor a diagnosis of depression was associated with physicians' personal attributes (sex, age, and years in practice) or organizational setting (number of total patient visits per week, private office or clinic, solo practice, Internet access, and rural practice setting). However, physicians in solo practice suggested fewer differential diagnoses for the GAD-scenario patient than those in group practice; physicians in practice 30 years or longer suggested fewer differential diagnoses for the MDE-scenario patient than those in practice fewer than 10 years. On average, physicians suggested 3 differential diagnoses for each of the scenarios., Conclusion: Most family physicians recognize depression and anxiety in patients presenting with symptoms of these disorders and consider an average of 3 differential diagnoses in each of these cases.

Objective: To explore family physicians' recommendations for treatment of, and number of weeks to first follow-up visit for, clinical-scenario patients presenting with symptoms of either a major depressive episode (MDE) or generalized anxiety disorder (GAD), as well as physicians' perceived barriers to optimal care for these patients., Design: Cross-sectional survey., Setting: Saskatchewan., Participants: A total of 331 family physicians practising in Saskatchewan as of December 2007., Main Outcome Measures: Type of treatment and number of weeks to first follow-up visit recommended for clinical-scenario patients, as well as family physicians' barriers to providing optimal care., Results: The response rate was 49.7% (331 of 666 surveys returned). Most physicians recommended treatment of the GAD-scenario patient (93.7%) and the MDE-scenario patient (90.1%). Most physicians recommended immediate (65.6%) rather than delayed (28.1%) treatment of the GAD-scenario patient, and immediate (55.6%) rather than delayed (34.5%) treatment of the MDE-scenario patient. Pharmacotherapy alone (26.3%) was the most commonly recommended immediate treatment of the GAD-scenario patient; combination pharmacotherapy and counseling (15.8%) was the most commonly recommended immediate treatment of the MDE-scenario patient. Most physicians recommended that the first follow-up visit occur within 2 weeks for the GAD (79.4%) and the MDE (82.5%) clinical-scenario patients. Physicians were more likely to identify themselves rather than patients and the health care system as barriers to providing optimal care to the GAD (39.4%) and the MDE (39.8%) clinical-scenario patients., Conclusion: Most family physicians recommend immediate treatment and early follow-up for patients presenting with symptoms of GAD or MDE. Physician-related barriers outweigh patient and health system barriers to providing optimal care to patients with common psychiatric disorders.

Abstract

Objectives: To document the frequency of self-reported use of dental services within a 2-year period by a sample of dentate Saskatchewan residents aged 18 years and older; explore the correlates associated with dental service use for men and women separately; and examine the age, gender and income distribution of dental insurance coverage., Methods: The 1999-2000 Saskatchewan Population Health and Dynamics Survey provided a representative sample of 5,003 dentate respondents (mean age 45.5 years; 52.3% women), 18 years and older, randomly selected and interviewed by telephone., Results: Of those interviewed, 77% of the women and 67% of the men reported a dental visit within the previous 2 years. For women, logistic regression analysis indicated that when all other factors were held constant, the odds of dental service use within the last 2 years were higher among those who were aged 18-19 years, had post-secondary education or technical certification, were in the highest household income adequacy category, held dental insurance and engaged in such preventive behaviours as regular general checkups, eye checkups, skin self-examination and not smoking daily. For men, the odds of dental service use within the last 2 years were greater if they had education at or beyond the secondary level, were students, were in the upper or highest income adequacy levels, held dental insurance and engaged in preventive health behaviours such as general checkups, eye checkups and not smoking daily. Dental insurance coverage was strongly associated with household income adequacy and peaked among men and women aged 30-49 years., Conclusions: This study indicates that the roles of gender, income, education, age, health-related behaviours and resources such as dental insurance should be considered in future public oral health strategies. We propose a conceptual model to aid in understanding the social structural and individual pathways to oral health.

Abstract

Research on nursing practice issues in rural and remote areas of Canada is very limited. This report describes the method and initial results of a comprehensive survey of registered nurses (RNs) practising outside the commuting zones of large urban centres, designed to determine: who practises nursing in rural and remote Canada; the nature and scope of their nursing practice; and their satisfaction with their work, community, and practice supports. Using a mailed questionnaire with persistent follow-up, the data-collection frame included a stratified random sample of rural RNs and the full population of RNs who worked in the northern territories and outpost ("remote") settings. The analyses focus on regional comparisons of demographics and primary work settings and on provincial comparisons of satisfaction levels related to work and community. The survey is part of a larger multi-method project intended to inform policy on rural nursing practice in Canada.
