Aim: In glioma classification, the isocitrate dehydrogenase (IDH) mutation status is crucial, as it significantly influences treatment approaches and prognosis. This study assessed the performance metrics of perfusion and permeability magnetic resonance imaging (MRI) parameters with optimal cut-offs in differentiating IDH genotype and tumor grade in patients with grade 2-4 gliomas., Material and Methods: This retrospective study included 36 patients surgically diagnosed with grade 2-4 glioma (six grade 2, seven grade 3, and 23 grade 4) with known IDH genotypes (23 IDH wild-type, 13 IDH mutant) between November 2021 and August 2023. All patients underwent preoperative perfusion and permeability MRI examinations with a 3.0 Tesla scanner. Parameters were calculated on colored map images. Using the intraclass correlation coefficient, intra- and inter-observer agreement was assessed. Following multiple testing correction, the perfusion parameters with statistically significant differences were subjected to receiver operating characteristic (ROC) analysis., Results: Five MRI parameters (rCBV and rCBF from perfusion; Ktrans, Ve, and Vp from permeability) showed a significant difference between groups in terms of IDH genotype (p 0.001). In ROC analysis, the best parameters in differentiating IDH genotype included rCBV and Ktrans; rCBV with a cut-off of 5.58 achieved an area under the ROC curve (AUC), sensitivity, specificity, and accuracy of 0.883, 95.7%, 76.9%, and 88.8%, respectively. For Ktrans, with a cut-off of 0.0727 min-1, these values were 0.893, 100%, 69.2%, and 88.8%, respectively. In ROC analysis, these two parameters with rCBF and Ve also showed good performance in differentiating low- and high-grade gliomas with an AUC, sensitivity, and accuracy exceeding 0.940, 86%, and 88%, respectively., Conclusion: Perfusion and permeability MRI may provide useful parameters in differentiating the IDH genotype and grade of gliomas.

Aim: To investigate the added value of dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) sequences in predicting somatostatin analog (SSA) responses in patients with acromegaly., Material and Methods: This study included 55 active acromegaly patients with macroadenoma. Mean and maximum signal intensities were measured using region of interests in T2-weighted (T2W) and DCE-MRI sequences. Semi-quantitative values indicating relative signal intensity ratios and contrast-enhanced kinetics were obtained. Bivariate and multivariate analyses were used to determine whether the pathological granulation pattern of adenomas (dense versus others) was associated with patients' demographic variables and semi-quantitative MRI parameters., Results: Three parameters formed the logistic model, x2(3)=23.278, p < 0.0001: age (odds ratio [OR]=1.08), hypointensity of adenomas in T2W images (OR=15.45), and high maximum enhancement ratio in the second interval (ER2max) values (OR=2195.74). The overall accuracy of this model was 85.45% with an area under the curve of 0.880. Sensitivity, specificity, positive predictive, and negative predictive values of the model were 68.75%, 92.31%, 78.58%, and 87.8%, respectively., Conclusion: In patients with newly diagnosed acromegaly, the model created based on the relative T2W signal intensity, patient's age, and ER2 < submax parameter from DCE-MRI sequences might be used to more accurately predict SSA responses.

Objectives: Immunosuppressive treatment generally increases the severity of active infection. Therefore, liver transplant is contraindicated in the presence of active tuberculosis. Despite the importance of supportive treatment, liver transplant is the only treatment for fulminant hepatic failure., Materials and Methods: We report a case of successful liver transplant for fulminant hepatic failure in the presence of active tuberculosis infection., Results: We immediately performed a liver transplant from a live donor. The patient received low-dose immunosuppressive treatment and antituberculosis treatment. The patient was cured and discharged on the 25th day after surgery. We stopped antituberculosis treatment 10 months after discharge. The patient has been followed for 32 months after transplant with normal graft function and has been free of pulmonary tuberculosis infection., Conclusions: Liver transplant can be performed in cirrhotic patients with active infections, such as tuberculosis, as a life-saving procedure.
