Abstract

The first-passage problem of the Ornstein-Uhlenbeck process to a boundary is a long-standing problem with no known closed-form solution except in specific cases. Taking this as a starting-point, and extending to a general mean-reverting process, we investigate the long- and short-time asymptotics using a combination of Hopf-Cole and Laplace transform techniques. As a result we are able to give a single formula that is correct in both limits, as well as being exact in certain special cases. We demonstrate the results using a variety of other models.

Abstract

The Fokker--Planck equation is a key ingredient of many models in physics, and related subjects, and arises in a diverse array of settings. Analytical solutions are limited to special cases, and resorting to numerical simulation is often the only route available; in high dimensions, or for parametric studies, this can become unwieldy. Using asymptotic techniques, that draw upon the known Ornstein--Uhlenbeck (OU) case, we consider a mean-reverting system and obtain its representation as a product of terms, representing short-term, long-term, and medium-term behaviour. A further reduction yields a simple explicit formula, both intuitive in terms of its physical origin and fast to evaluate. We illustrate a breadth of cases, some of which are `far' from the OU model, such as double-well potentials, and even then, perhaps surprisingly, the approximation still gives very good results when compared with numerical simulations. This Study informing treatment Pathway dEcision in bladder cAnceR (SPEAR-Bladder) aimed to provide insight into the optimal sequencing of IO treatments among la/mUC patients treated in the US Oncology Network.Patients and Methods: This was a retrospective analysis of adult patients with la/mUC who initiated first-line chemotherapy followed by either IO therapy (C-IO subgroup) or chemotherapy (C-C subgroup) between 01/01/2015 and 04/30/2017 and included a potential follow-up period through 06/30/2017. Data were sourced from iKnowMed electronic health records. Patient and treatment characteristics were assessed descriptively, with Kaplan–Meier methods used to evaluate time-to-event outcomes, including overall survival (OS).Results: A total of 117 patients were included in this analysis (median age 69 years, 74.4% male, 88.0% Caucasian): 79 and 38 patients were in the C-IO and C-C subgroups, respectively. The median OS was 19.2 months among patients who received the C-IO sequence and 11.9 months among those who received the C-C treatment sequence.Conclusion: These results suggest that patients who received the C-IO treatment sequence had notable improvement in OS compared with those who received the C-C sequence. This analysis resolves the concordance genus of 533 of the 552 prime 11--crossing knots. 10. An exactly solvable self-convolutive recurrence

Abstract

We consider a self-convolutive recurrence whose solution is the sequence of coefficients in the asymptotic expansion of the logarithmic derivative of the confluent hypergeometic function $U(a,b,z)$. By application of the Hilbert transform we convert this expression into an explicit, non-recursive solution in which the $n$th coefficient is expressed as the $(n-1)$th moment of a measure, and also as the trace of the $(n-1)$th iterate of a linear operator. Applications of these sequences, and hence of the explicit solution provided, are found in quantum field theory as the number of Feynman diagrams of a certain type and order, in Brownian motion theory, and in combinatorics.

Efficacy and safety data were sourced from published literature and US package inserts. The analysis was conducted in collaboration with a specialist oncologist who validated clinical assumptions. Costs were based on the number of eligible patients, time-to-treatment failure, overall survival, adverse events (AEs), and projected market shares of various treatments.Results: In a hypothetical commercial health plan of 30,000,000 members, 884 patients were estimated to be eligible for 2L treatment over a three-year time period. Without avelumab, the total cost for treating patients with mUC was estimated to be US$70,268,035. The introduction of avelumab increased total costs by $73,438 (0.10% increase). In a hypothetical Medicare health plan of 30,000,000 beneficiaries, a total of 4,705 patients were estimated to be eligible for 2L treatment. Without avelumab, the total cost for treating patients with mUC was estimated to be $292,923,098 from a Medicare perspective; however, with avelumab, there was an increase of $719,324 (0.25% increase) in total costs. Results of the sensitivity analyses demonstrated a cost-neutral impact across all tested scenarios from both perspectives.Conclusion: The BIM estimated that avelumab would have a cost-neutral impact within a US commercial and a Medicare health plan. Overall, avelumab can be an affordable and valuable treatment option for patients with locally advanced or mUC in the 2L setting. These findings demonstrate a consistently favorable budget impact in both populations. The comparator treatments included in the analysis were pembrolizumab and nivolumab (other immuno-oncology agents); and the chemotherapies routinely used in the eligible mMCC population. Model inputs included market share uptake of avelumab and other comparators, duration of treatments, and costs (drugs, health care resource utilization, adverse events). The model was evaluated from a commercial payer perspective. Sensitivity analyses were conducted to test uncertainties arising from the input values used in the model.Results: In a hypothetical commercial health plan of 30 million members, 285 patients with mMCC were identified over 3 years; 43 patients received avelumab as a 1L treatment over 3 years. In a world without avelumab, the total health care costs of treating patients with mMCC over 3 years were estimated to be US$11,710,115 from a commercial health plan perspective. With avelumab, there were estimated savings of $2,643,173 considering the total costs related to the treatment of mMCC over 3 years (23% reduction in the budget). The incremental cost per member per month over 3 years was −$0.0025.Conclusion: The model results indicate that the adoption of avelumab as a treatment option for mMCC would likely result in minimal budget impact from a US health plan perspective. Abstract

Background Age and comorbidities increase COVID-19 related in-hospital mortality risk, but the extent by which comorbidities mediate the impact of age remains unknown. Methods In this multicenter retrospective cohort study with data from 45 Dutch hospitals, 4806 proven COVID-19 patients hospitalized in Dutch hospitals (between February and July 2020) from the CAPACITY-COVID registry were included (age 69[58–77]years, 64% men). The primary outcome was defined as a combination of in-hospital mortality or discharge with palliative care. Logistic regression analysis was performed to analyze the associations between sex, age, and comorbidities with the primary outcome. The effect of comorbidities on the relation of age with the primary outcome was evaluated using mediation analysis. Results In-hospital COVID-19 related mortality occurred in 1108 (23%) patients, 836 (76%) were aged ≥70 years (70+). Both age 70+ and female sex were univariably associated with outcome (odds ratio [OR]4.68, 95%confidence interval [4.02–5.45], OR0.68[0.59–0.79], respectively;both pp Conclusions Age is the main determinant of COVID-19 related in-hospital mortality, with negligible mediation effect of pre-existing comorbidities. 30. Teachers' digital pedagogies during the emergency remote school lockdowns in Australia

Abstract

This paper examines digital teaching approaches adopted by Australian teachers in an emergency remote schooling period occurring during the pandemic. We examine these practices using a digital pedagogical framework presented in our 2020 chapter that highlights the features of personalisation, authenticity and collaboration. Data were collected from four case schools that were selected because of their reputation for effective management of student learning and wellbeing during this remote teaching period. Findings reveal that despite minimal preparation time, teacher participants from the case schools found ways of designing and implementing digital learning tasks that leveraged student agency and collaboration, and where possible, elements of authentic learning. Teachers’ confidence levels with new digital pedagogies improved during this period, and many practices continued upon return to normal schooling.

39. Professional Characteristics of Canadian Counsellors: Results of a National Survey

Abstract

With statutory regulation of mental health professions clearly at the forefront in many Canadian jurisdictions, counselling appears to be experiencing internal tensions regarding its vision and direction. The goal of this study was to collect data directly from Canadian counsellors to more clearly define the current practices of counselling in Canada. We developed a web-based survey and invited members of the Canadian Counselling and Psychotherapy Association (CCPA) to respond: 511 (22.9%) completed surveys were returned. Survey data indicate that CCPA members do not believe that the professional identity among Canadian counsellors is clear and coherent and they do not perceive that the profession is widely respected among the public for the services it provides. However, respondents expressed a very high degree of satisfaction with their career decision to become counsellors. The findings have implications for counsellor education as well as the practice of counselling in Canada.

41. An examination of teachers’ digital practices during school lockdowns

Abstract

This paper provides insights into how mobile devices were used to support students’ learning (mobile learning) during school lockdowns. It draws on teachers’ views captured in surveys from two international projects. The first project tracks mainstream school teachers’ pedagogical practices with mobile devices (mobile pedagogies) before, during and after the pandemic. The second project examines mobile pedagogies adopted by teachers of students with disabilities in the same period. Both projects use a validated survey instrument to examine practices through the lens of a sociocultural digital framework that highlights the distinctive mobile pedagogies of personalization, authenticity and collaboration. A subset of the data from across the projects is analysed. These data comprise survey responses from teachers (n=141) who reported on their adopted practices during school lockdowns. Findings provide an understanding of mobile pedagogies during this challenging period, particularly practices supporting student agency. Future research and professional learning directions are provided.

Abstract

Aims Patients with cardiac disease are considered high risk for poor outcomes following hospitalization with COVID-19. The primary aim of this study was to evaluate heterogeneity in associations between various heart disease subtypes and in-hospital mortality. Methods and results We used data from the CAPACITY-COVID registry and LEOSS study. Multivariable Poisson regression models were fitted to assess the association between different types of pre-existing heart disease and in-hospital mortality. A total of 16 511 patients with COVID-19 were included (21.1% aged 66-75 years; 40.2% female) and 31.5% had a history of heart disease. Patients with heart disease were older, predominantly male, and often had other comorbid conditions when compared with those without. Mortality was higher in patients with cardiac disease (29.7%; n= 1545 vs. 15.9%; n= 1797). However, following multivariable adjustment, this difference was not significant [adjusted risk ratio (aRR) 1.08, 95% confidence interval (CI) 1.02-1.15; P = 0.12 (corrected for multiple testing)]. Associations with in-hospital mortality by heart disease subtypes differed considerably, with the strongest association for heart failure (aRR 1.19, 95% CI 1.10-1.30; P <0.018) particularly for severe (New York Heart Association class III/IV) heart failure (aRR 1.41, 95% CI 1.20-1.64; P < 0.018). None of the other heart disease subtypes, including ischaemic heart disease, remained significant after multivariable adjustment. Serious cardiac complications were diagnosed in <1% of patients. Conclusion Considerable heterogeneity exists in the strength of association between heart disease subtypes and in-hospital mortality. Of all patients with heart disease, those with heart failure are at greatest risk of death when hospitalized with COVID-19. Serious cardiac complications are rare during hospitalization. 44. Digital pedagogies for future school education: Promoting inclusion

Abstract

This article develops four school education scenarios to help educators consider the role of digital technologies in future teaching and to provoke discussion of how education might be done differently. The context for these scenarios is a world shaped by the COVID19 pandemic. The development of education scenarios is a useful device for thinking about the future and is often used as a methodology to stimulate discussion about possible variations in education. We use our empirical data from a recent study of Australian schools illustrating exemplary remote-teaching, to generate four alternative scenarios for future school education, with a particular focus on learning agency, and the level of educational technology use. We show how different contexts and different student needs are supported by each of the four scenarios. This set of scenarios challenges the common perception that remote learning is only effective through high end use of online technologies, and suggests there is a role for both student-led and teacher-led approaches, depending on the particular contexts. The article discusses the place of innovation in disadvantaged and technology-restricted contexts. It concludes with an analysis of the ways in which school education might respond to future challenges and opportunities.

Abstract

Background Inflammatory adverse events following COVID-19 vaccination are being reported amidst the growing concerns regarding vaccine’s immunogenicity and safety, especially in patients with pre-existing inflammatory conditions. Methods Multinational case series of patients diagnosed with an ocular inflammatory event within 14 days following COVID-19 vaccination collected from 40 centres over a 3 month period in 2021. Results Seventy patients presented with ocular inflammatory events within 14 days following COVID-19 vaccination. The mean age was 51 years (range, 19–84 years). The most common events were anterior uveitis (n = 41, 58.6%), followed by posterior uveitis (n = 9, 12.9%) and scleritis (n = 7, 10.0%). The mean time to event was 5 days and 6 days (range, 1–14 days) after the first and second dose of vaccine, respectively. Among all patients, 36 (54.1%) had a previous history of ocular inflammatory event. Most patients (n = 48, 68.6%) were managed with topical corticosteroids. Final vision was not affected in 65 (92.9%), whereas 2 (2.9%) and 3 (4.3%) had reduction in visual acuity reduced by ≤3 lines and > 3 lines, respectively. Reported complications included nummular corneal lesions (n = 1, 1.4%), cystoid macular oedema (n = 2, 2.9%) and macular scarring (n = 2, 2.9%). Conclusion Ocular inflammatory events may occur after COVID-19 vaccination. The findings are based on a temporal association that does not prove causality. Even in the possibility of a causal association, most of the events were mild and had a good visual outcome.
