Tregs prevent autoimmune disorders by protecting the host-cells from an immune response, related to the peripheral tolerance. However, tumor cells use Tregs as a shield to protect themselves against anti-tumor immune response. Thus, Tregs are a hurdle in achieving the complete potential of anti-cancer therapies including immunotherapy. This has prompted the development of novel adjuvant therapies that obviate their negative effects thereby enhancing the therapeutic efficacy. Our earlier studies have shown the efficacy of the glycolytic inhibitor, 2-deoxy-D-glucose (2-DG) by reducing the induced Tregs pool and enhance immune stimulation as well as local tumor control. These findings have suggested its potential for enhancing the efficacy of immunotherapy, besides radiotherapy and chemotherapy. This review provides a brief account of the current status of Tregs as a component of the immune-biology of tumors and various preclinical and clinical strategies pursued to obviate the limitations imposed by them in achieving therapeutic efficacy.Keywords: T-regulatory cells, cyclophosphamide, dendritic cells, immune enhancement, targeted cancer therapy, 2-deoxy-D-glucose, metabolic inhibitor

3. Tissue-specific transcriptome profiling of the Arabidopsis inflorescence stem reveals local cellular signatures

Abstract

Genome-wide gene expression maps with a high spatial resolution have substantially accelerated plant molecular science. However, the number of characterized tissues and growth stages is still small due to the limited accessibility of most tissues for protoplast isolation. Here, we provide gene expression profiles of the mature inflorescence stem of Arabidopsis thaliana covering a comprehensive set of distinct tissues. By combining fluorescence-activated nucleus sorting and laser-capture microdissection with next-generation RNA sequencing, we characterized the transcriptomes of xylem vessels, fibers, the proximal and distal cambium, phloem, phloem cap, pith, starch sheath, and epidermis cells. Our analyses classified more than 15,000 genes as being differentially expressed among different stem tissues and revealed known and novel tissue-specific cellular signatures. By determining overrepresented transcription factor binding regions in the promoters of differentially expressed genes, we identified candidate tissue-specific transcriptional regulators. Our datasets predict the expression profiles of an exceptional number of genes and allow hypotheses to be generated about the spatial organization of physiological processes. Moreover, we demonstrate that information about gene expression in a broad range of mature plant tissues can be established at high spatial resolution by nuclear mRNA profiling. Abstract

Item does not contain fulltext, Arabidopsis thaliana Col-0 plants were used in this study. The plants were grown in soil (Profi Substrat Classic CLT SM fein, Einheitserde, Sinntal-Altengronau, Germany, #10-00306-40) alone or mixed with perlite to 20% (PERLIGRAN Extra, Knauf Performance Materials, Dortmund, Germany) under short-day conditions (10-h light and 14-h darkness) for 3–5 weeks and transferred to long-day conditions (16-h light and 8-h darkness) for 3 weeks to induce reproductive growth. Light was provided by red and blue LED lights (GreenPower LED production DR/B LO, Phlips (Signify), Eindhoven, The Netherlands, #9290 004 871) at 70–120 µM photosynthetically active radiation. The SMXL5pro:H4-GFP (pIL53) line and the PXYpro:H4-GFP (pPS24) lines were described previously (Shi et al., 2019). Other transgenic lines were generated by the floral dip method using Agrobacterium tumefaciens (Clough and Bent, 1998). Analyses were performed using homozygous lines, except for promoter-reporter line analyses, where plants from the T1 or T2 generation were used. DNA vector construction An H4-GFP-containing construct was a gift from Daniel Schubert (Freie Universität Berlin, Germany). A vector containing the ANT promoter region (MT63, Schoof et al., 2000) was used as a template for PCR-based cloning (Supplemental Table 2). NST3pro:H4-GFP (pMS59), VND7pro:H4-GFP (pTOM61), APLpro:H4-GFP (pPS02), SCRpro:H4-GFP (pPS20), and LTP1pro:H4-GFP (pPS16) were cloned using the pGreen0229 vector (Hellens et al., 2000) as a backbone. Cloning of the APL promoter and terminator regions was described previously (Sehr et al., 2010). Primer sequences used to clone promoter and terminator regions of other genes and to amplify the H4-GFP sequence with the appropriate restriction enzyme sites are listed in Supplemental Table 2. Abstract

In this chapter, I consider the claim for pluralism commonly advanced in political philosophy as a claim concerning the standards, methods, and norms for forming belief and judgment about certain kinds of facts. After distinguishing between descriptive and normative epistemic pluralism, I contend that in this context pluralism needs to rest on grounds that are stronger than fallibilism yet weaker than relativism in order to enjoy a distinct standing. The idea of reasonable pluralism seems to devise a variety of normative pluralism designed to meet this demand. I argue, however, that this is an unstable position and suggest that an epistemic view of deliberation may be better suited to making sense of political justification. The latter view, though, is bound to dispense with normative pluralism.

8. A Data-Analytics Approach for Enterprise Resilience

Abstract

© 2001-2011 IEEE. Enterprise resilience plays an important role to prevent business services from disruptions caused by human-induced disasters such as failed change implementations and software bugs. Traditional expert-centric approach has difficulty to maintain continued critical business functions because the disasters can often only be handled after their occurrence. This paper introduces a data-analytics approach, which leverages system monitoring data for the enterprise resilience. With the power of data mining and machine learning techniques, we build an intelligent business analytics system to detect the potential disruptions proactively, and to assist the operational team for enterprise resilience enhancement. We demonstrate the effectiveness of our approach on a real enterprise system monitoring dataset in simulation.

14. A case control study of melatonin with or without coenzyme Q10 in improving oocyte quality and outcomes in in vitro fertilization.

Abstract

Antioxidant therapy is thought to improve oocyte quality through the reduction of oxidative stress. Melatonin and coenzyme Q10 (CoQ10) are known antioxidants. We aimed to investigate the clinical efficacy of exogenous melatonin supplementation with or without CoQ10 in improving oocyte quantity and quality. Fifty-three women undergoing cycles at Monash IVF were supplemented with melatonin 4mg daily, with 19 of those additionally supplemented with 150mg of CoQ10. Controls were matched at a 2:1 ratio to cases. Data were taken from an index (pretherapy) cycle and from a therapeutic (posttherapy) cycle for both cases and controls. Comparisons were made directly between cases and controls in the therapeutic cycle and by comparing the median differences of the change in outcomes from the index to therapeutic cycle. Primary outcomes included follicle, oocyte, and metaphase II oocytes (MII) counts. Results showed higher median absolute oocyte numbers (6.00 vs. 4.00, P = 0.049) in the case group. Cases and controls were significantly different when comparing median change between cycles in oocyte number (0.00 vs. -1.00, P = 0.028) and MII count (0.00 vs. -1.00, P = 0.034). However, this was the result of poorer outcomes in the control group rather than improvements in the case group, and it cannot be concluded that treatment improved oocyte quality.Copyright © the authors, publisher and licensee Libertas Academica Limited.

15. Perinatal complications following IVF in older women: Impact of the use of donor oocytes.

Abstract

Introduction The maternal age has steadily risen over the years. IVF has contributed to this trend, in particular with the introduction of donor oocyte treatment. IVF pregnancy rates are directly correlated with the age of the egg provider. However, there is less reliable data on perinatal outcomes in donor and autologous cycles. Our aim was to analyse the perinatal complication rates in women over 40 years of age who conceived with IVF using donor (DO) or autologous oocytes (AO). Methods This was a retrospective cohort study between 2000 and 2013. Women >=40 years, all pregnant after IVF using AO or DO. A total of 2824 pregnancies were identified for analysis excluding pregnancies with vitrified oocytes/embryos, GIFT, surrogacy and incomplete identifying data. Both fresh and frozen cycles were included. t test and Fisher's exact test were used where appropriate. Adjusted odds ratios (aOR) were calculated using generalised estimating equations to adjust for within-patient correlations and confounding variables. Results 2134 AO and 689 DO pregnancies were analysed. The average age (years) of the AO and DO mothers was 41.0 +/- 1.2 (mean +/- SD) and 44.5 +/-3.0 respectively and that of the oocyte donors was 32.9 +/- 4.6. There was no statistical difference between the two groups for BMI, smoking, parity and the number of embryos transferred. The DO group had a significantly higher number of treatment cycles (6.7 versus 4.8) and a lower miscarriage rate (24.4% versus 37.7%). Compared to the AO group, the aORs (95% CI) for the following obstetric complications in DO were: IUGR 1.4 (0.5-3.9); GDM 1.1 (0.6-2.0); PET 1.1 (0.5-2.6); pregnancy-induced hypertension 2.0 (0.8-4.9); placenta previa 1.8 (0.6-5.9); and surgical delivery rate 1.3 (0.8-2.1). Conclusion As expected, the miscarriage rate is lower in the DO group. However, our results suggest that the risk of obstetric complications in women >=40 years is not reduced with the use of donor oocytes from younger women.

17. Improving endometrial receptivity: Does hysteroscopy in the cycle prior help to improve implantation and clinical pregnanacy.

Abstract

OBJECTIVE: Mechanical endometrial injury is proposed to induce and modulate expression of genes for factors required for improving implantation. Hysteroscopy also provides reassurance and opportunity for treatment of uterine pathology. However, with a lack of large sized RCTs, recommendation is based on meta-analysis, using non randomized or quasi randomized studies, which have used non-matched controls. We sought to identify whether a normal hysteroscopy in the cycle preceding transfer (fresh or frozen) would have a beneficial impact on the implantation and clinical pregnancy rate after matching for variables known to affect pregnancy rates. DESIGN: Case -controlled study. MATERIALSAND METHODS: Case controlled study of women undergoing hysteroscopy, during IVF treatment in the last 4 years in one clinician's practice. All patients with no abnormality detected at hysteroscopy followed by transfer in the next cycle (n= 87) were matched with controls in a 3:1 ratio(no=263). Matching included age, cycle and treatment number, stimulation protocol, IVF/ICSI and Fresh/Frozen embryos transferred, and day of transfer. Statistical analysis was performed with Fischer's exact test. RESULT(S):Mean age in the hysteroscopy and control group was comparable: 36.2 yrs. and 36.3 years respectively. The only biochemical positive pregnancy rate per transfer in the intervention group was 5.9, in controls 7.1 (p=NS), pregnancy rate per transfer 28.2 in the intervention group versus 20.9(p= NS, stratification of fresh versus frozen was non-significant (21.7:21.4; 35.9:20.2). A further comparison stratifying for age subgroups and cycle numbers did not show any benefits in the intervention group. CONCLUSION(S): Mechanical intervention with hysteroscopy did not show a statistical significance in terms of implantation or pregnancy when matched with controls for confounding variables .Although there was a probable trend of more pregnancies in the frozen transfers following hysteroscopy, this s

Use of biopesticides, viz., Heterorhabditls indicu, Metarhizium anisopliae, Beauveria bassiana, Steinernema carpocapsae, S. carpocapsae + B. bassiana, S. carpocapsae + M. anisopliae and H. indica + M. anisoliae provided less protection at earlier stages of seedling growth, whereas at later stages, they were effective and registered higher yield as compared to control. H. indica + M. anisopliae treated plots recorded higher yield as compared to other biopesticides.

Abstract

Biomorphic silicon carbide ceramics are a new class of materials fabricated by reactive infiltration of molten silicon with carbonaceus preforms obtained from wood pyrolysis. The resulting material is a Si/SiC composite, in which the SiC forms a skeleton that replicates the wood microstructure with unreacted Si filling the pores. The silicon can be removed from the composite creating a highly porous SiC material that can be refilled with other products, like metal alloys, to create ceramic/metal composites.

26. Interaction of Arabidopsis Thaliana with Plasmodiophora Brassicae

Abstract

Plasmodiophora brassicae is a protistan pathogen that attacks roots of brassicaceous plant species causing devastating disease. Resistance is characterised by restriction of the pathogen and susceptibility by the development of severely malformed roots (‘clubroots’) and stunting of the plant that is associated with alterations in the synthesis of cytokinin and auxin hormones. We are examining the susceptible response in Arabidopsis and whether suppression of key resistance factors by the pathogen contributes to susceptibility. The interaction is being studied using a number of approaches including microscopy of the infection process and development of the pathogen within roots and host gene expression analysis. Quantitative PCR was used to confirm the timing of infection of roots and showed that infection occurred at day four and colonisation increased thereafter to high levels by 23 days after inoculation by which time roots were showing systemic abnormalities. To investigate the basis of this compatible interaction we have conducted a time course experiment following infection of a susceptible ecotype of Arabidopsis (Col-0) to examine whole genome geneexpression changes in the host. Differential gene expression analysis of inoculated versus control roots showed that a higher number of genes had altered expression levels at day four compared to that at day seven and at day ten. At day four the expression levels of several genes known to be important for recognition and signal transduction in resistant interactions and genes involved in the biosynthesis of lignin, phenylpropanoids and ethylene were suppressed. Suppression by P. brassicae of specific plant defence responses appears to be a key component of susceptibility in this system.

27. Investigation of plasmodiophora brassicae (clubroot disease) in vegetable brassica using arabidopsis thaliana as a model system

Abstract

Clubroot, caused by Plasmodiophora brassicae, is the most devastating soil-borne disease of vegetable brassicas. It occurs all over the world and is responsible for crop losses of up to 10% every year. In Australia, the disease is being managed effectively with chemicals and cultural practices, but ideally control can be improved in the long term by the introduction of resistant cultivars. The life cycle ofP. brassicae and mode of action of plant resistance has not been fully elucidated because of the technical difficulties of working with an obligate, soil-borne plant pathogen. However, Arabidopsis thaliana, which is a host ofP. brassicae, has great potential as a model system for studying the life cycle, the infection process and development of resistance. We have developed a sand-liquid-culture system for growing Arabidopsis that allows easy observation of all life stages and, most importantly, the primary plasmodial stages within the root hair. The method was first optimised for observations of the lifecycle of the pathogen in a susceptible Arabidopsis ecotype (Col-3) where all stages of the lifecycle have now been observed and characterised. Further screening of Arabidopsis ecotypes for disease resistance has utilised one of the most virulent Australian pathotypes of brassica (ECD number 16/19/31). To date, Arabidopsis ecotype Ta-0 has shown a level of tolerance to the disease even though the roots get infected. It has been reported earlier that resistance toP. brassicae in Arabidopsis is due to one or a small number of genes. To examine changes in gene expression during the early, critical stages of infection, RNA was extracted from the susceptible and resistant ecotypes at two time points, 4 days and 17 days after inoculation. Microarray analysis will be used to investigate genome wide changes in gene expression during infection but also to identify candidate genes that may confer resistance to Australian isolates of the pathogen.

Abstract

In this investigation we examined the essential oils of three aromatic plants, Zanthoxylum armatum, Juniperus communis, and Dysphania ambrosioides, which are used by the local population of the western Himalayan region for medicinal purposes. These plants were studied for their antifungal, larvicidal and biting deterrent activities. Additionally, chemical compositions of the oils were determined by GC-MS and their odor evaluated. The main compounds were 2-undecanone (47.7%) and 1,8-cineol (20.5%) from Z. armatum and α-pinene (43.2%) and limonene (34.7%) from J. communis. p-Cymene (36.3%) and ascardiole (31.2%) were found as the main components of D. ambrosioides essential oil. Antifungal activity was evaluated against three Colletotrichum species. Dyspania ambrosioides oil showed the best activity against these strawberry anthracnose causing fungi. It also demonstrated biting deterrent activity against Aedes aegypti similar to DEET as positive control, whereas Z. armatum and J. communis were less effective against Colletotrichum and mosquitoes.

Abstract

In Rhododendron spp. and Ledum groenlandicum a callose wall is laid down around the zygote in the first 2 days after fertilization. The periodic acid/Schiff-positive, aniline blue-fluorescence-positive callosic wall is initiated adjacent to the degenerating synergid, extends to cover the entire zygote surface, and remains visible during the initiation of embryogeny as the zygote elongates before the first proembryonal division. Unfertilized ovules show eventual callose deposition in the ovule wall cells during senescence in undeveloped abscising pistils, but show no development of callose within the embryo sac. Possible roles of a zygote special callose wall are discussed.

34. Secular trends in cholesterol lipoproteins and triglycerides and prevalence of dyslipidemias in an urban Indian population

Abstract

Abstract Background Coronary heart disease is increasing in urban Indian subjects and lipid abnormalities are important risk factors. To determine secular trends in prevalence of various lipid abnormalities we performed studies in an urban Indian population. Methods Successive epidemiological Jaipur Heart Watch (JHW) studies were performed in Western India in urban locations. The studies evaluated adults ≥ 20 years for multiple coronary risk factors using standardized methodology (JHW-1, 1993–94, n = 2212; JHW-2, 1999–2001, n = 1123; JHW-3, 2002–03, n = 458, and JHW-4 2004–2005, n = 1127). For the present analyses data of subjects 20–59 years (n = 4136, men 2341, women 1795) have been included. In successive studies, fasting measurements for cholesterol lipoproteins (total cholesterol, LDL cholesterol, HDL cholesterol) and triglycerides were performed in 193, 454, 179 and 252 men (n = 1078) and 83, 472, 195, 248 women (n = 998) respectively (total 2076). Age-group specific levels of various cholesterol lipoproteins, triglycerides and their ratios were determined. Prevalence of various dyslipidemias (total cholesterol ≥ 200 mg/dl, LDL cholesterol ≥ 130 mg/dl, non-HDL cholesterol ≥ 160 mg/dl, triglycerides ≥ 150 mg/dl, low HDL cholesterol Results Mean levels of various lipoproteins increased sharply from JHW-1 to JHW-2 and then gradually in JHW-3 and JHW-4. Age-adjusted mean values (mg/dl) in JHW-1, JHW-2, JHW-3 and JHW-4 studies respectively showed a significant increase in total cholesterol (174.9 ± 45, 196.0 ± 42, 187.5 ± 38, 193.5 ± 39, 2-stage least-squares regression R = 0.11, p < 0.001), LDL cholesterol (106.2 ± 40, 127.6 ± 39, 122.6 ± 44, 119.2 ± 31, R = 0.11, p < 0.001), non-HDL cholesterol (131.3 ± 43, 156.4 ± 43, 150.1 ± 41, 150.9 ± 32, R = 0.12, p < 0.001), remnant cholesterol (25.1 ± 11, 28.9 ± 14, 26.0 ± 11, 31.7 ± 14, R = 0.06, p = 0.001), total:HDL cholesterol ratio (4.26 ± 1.3, 5.18 ± 1.7, 5.21 ± 1.7, 4.69 ± 1.2, R = 0.10, p < 0.001) and triglycerides (125.6 ± 53, 144.5 ± 71, 130.1 ± 57, 158.7 ± 72, R = 0.06, p = 0.001) and decrease in HDL cholesterol (43.6 ± 14, 39.7 ± 8, 37.3 ± 6, 42.5 ± 6, R = 0.04, p = 0.027). Trends in age-adjusted prevalence (%) of dyslipidemias in JHW-1, JHW-2, JHW-3 and JHW-4 studies respectively showed insignificant changes in high total cholesterol (26.3, 35.1, 25.6, 26.0, linear curve-estimation coefficient multiple R = 0.034), high LDL cholesterol ≥ 130 mg/dl (24.2, 36.2, 31.0, 22.2, R = 0.062), and high low HDL cholesterol < 40 mg/dl (46.2, 53.3, 55.4, 33.7, R = 0.136). Increase was observed in prevalence of high non-HDL cholesterol (23.0, 33.5, 27.4, 26.6, R = 0.026), high remnant cholesterol (40.1, 40.3, 30.1, 60.6, R = 0.143), high total:HDL cholesterol ratio ≥ 5.0 (22.2, 47.6, 53.2, 26.3, R = 0.031) and ≥ 4.0 (58.6, 72.5, 70.1, 62.0, R = 0.006), and high triglycerides (25.7, 28.2, 17.5, 34.2, R = 0.047). Greater correlation of increasing non-HDL cholesterol, remnant cholesterol, triglycerides and total:HDL cholesterol ratio was observed with increasing truncal obesity than generalized obesity (two-line regression analysis p < 0.05). Greater educational level, as marker of socioeconomic status, correlated significantly with increasing obesity (r2 men 0.98, women 0.99), and truncal obesity (r2 men 0.71, women 0.90). Conclusion In an urban Indian population, trends reveal increase in mean total-, non-HDL-, remnant-, and total:HDL cholesterol, and triglycerides and decline in HDL cholesterol levels. Prevalence of subjects with high total cholesterol did not change significantly while those with high non-HDL cholesterol, cholesterol remnants, triglycerides and total-HDL cholesterol ratio increased. Increasing dyslipidemias correlate significantly with increasing truncal obesity and obesity.

Abstract

BACKGROUND: Among patients with stable coronary disease and moderate or severe ischemia, whether clinical outcomes are better in those who receive an invasive intervention plus medical therapy than in those who receive medical therapy alone is uncertain. METHODS: We randomly assigned 5179 patients with moderate or severe ischemia to an initial invasive strategy (angiography and revascularization when feasible) and medical therapy or to an initial conservative strategy of medical therapy alone and angiography if medical therapy failed. The primary outcome was a composite of death from cardiovascular causes, myocardial infarction, or hospitalization for unstable angina, heart failure, or resuscitated cardiac arrest. A key secondary outcome was death from cardiovascular causes or myocardial infarction. RESULTS: Over a median of 3.2 years, 318 primary outcome events occurred in the invasive-strategy group and 352 occurred in the conservative-strategy group. At 6 months, the cumulative event rate was 5.3% in the invasive-strategy group and 3.4% in the conservative-strategy group (difference, 1.9 percentage points; 95% confidence interval [CI], 0.8 to 3.0); at 5 years, the cumulative event rate was 16.4% and 18.2%, respectively (difference, -1.8 percentage points; 95% CI, -4.7 to 1.0). Results were similar with respect to the key secondary outcome. The incidence of the primary outcome was sensitive to the definition of myocardial infarction; a secondary analysis yielded more procedural myocardial infarctions of uncertain clinical importance. There were 145 deaths in the invasive-strategy group and 144 deaths in the conservative-strategy group (hazard ratio, 1.05; 95% CI, 0.83 to 1.32). CONCLUSIONS: Among patients with stable coronary disease and moderate or severe ischemia, we did not find evidence that an initial invasive strategy, as compared with an initial conservative strategy, reduced the risk of ischemic cardiovascular events or death from any cause over a median of 3.2 years. The trial findings were sensitive to the definition of myocardial infarction that was used. Abstract

In this chapter, I consider the claim for pluralism commonly advanced in political philosophy as a claim concerning the standards, methods, and norms for forming belief and judgment about certain kinds of facts. After distinguishing between descriptive and normative epistemic pluralism, I contend that in this context pluralism needs to rest on grounds that are stronger than fallibilism yet weaker than relativism in order to enjoy a distinct standing. The idea of reasonable pluralism seems to devise a variety of normative pluralism designed to meet this demand. I argue, however, that this is an unstable position and suggest that an epistemic view of deliberation may be better suited to making sense of political justification. The latter view, though, is bound to dispense with normative pluralism.

40. Equitable differential privacy.

Abstract

Differential privacy (DP) has been in the public spotlight since the announcement of its use in the 2020 U.S. Census. While DP algorithms have substantially improved the confidentiality protections provided to Census respondents, concerns have been raised about the accuracy of the DP-protected Census data. The extent to which the use of DP distorts the ability to draw inferences that drive policy about small-populations, especially marginalized communities, has been of particular concern to researchers and policy makers. After all, inaccurate information about marginalized populations can often engender policies that exacerbate rather than ameliorate social inequities. Consequently, computer science experts have focused on developing mechanisms that help achieve equitable privacy, i.e., mechanisms that mitigate the data distortions introduced by privacy protections to ensure equitable outcomes and benefits for all groups, particularly marginalized groups. Our paper extends the conversation on equitable privacy by highlighting the importance of inclusive communication in ensuring equitable outcomes for all social groups through all the stages of deploying a differentially private system. We conceptualize Equitable DP as the design, communication, and implementation of DP algorithms that ensure equitable outcomes. Thus, in addition to adopting computer scientists' recommendations of incorporating equity parameters within DP algorithms, we suggest that it is critical for an organization to also facilitate inclusive communication throughout the design, development, and implementation stages of a DP algorithm to ensure it has an equitable impact on social groups and does not hinder the redressal of social inequities. To demonstrate the importance of communication for Equitable DP, we undertake a case study of the process through which DP was adopted as the newest disclosure avoidance system for the 2020 U.S. Census. Drawing on the Inclusive Science Communication (ISC) framework, we examine the extent to which the Census Bureau's communication strategies encouraged engagement across the diverse groups of users that employ the decennial Census data for research and policy making. Abstract

Multi-photon 3D laser printing has gathered much attention in recent years as a means of manufacturing biocompatible scaffolds that can modify and guide cellular behavior in vitro. However, in vivo tissue engineering efforts have been limited so far to the implantation of beforehand 3D printed biocompatible scaffolds and in vivo bioprinting of tissue constructs from bioinks containing cells, biomolecules, and printable hydrogel formulations. Thus, a comprehensive 3D laser printing platform for in vivo and in situ manufacturing of microimplants raised from synthetic polymer-based inks is currently missing. Here, a platform for minimal-invasive manufacturing of microimplants directly in the organism is presented by one-photon photopolymerization and multi-photon 3D laser printing. Employing a commercially available elastomeric ink giving rise to biocompatible synthetic polymer-based microimplants, first applicational examples of biological responses to in situ printed microimplants are demonstrated in the teleost fish Oryzias latipes and in embryos of the fruit fly Drosophila melanogaster. This provides a framework for future studies addressing the suitability of inks for in vivo 3D manufacturing. Abstract

Hemophilia A (HA) is a genetic disorder of hemostasis associated with a deficiency or reduced activity of clotting factor VIII (FVIII). This disorder remains unacceptably underdiagnosed in India. Early diagnosis and appropriate management of HA can substantially prevent morbidity and mortality. Currently, HA is managed with regular replacement therapy using standard or extended half-life FVIII concentrates or non-factor drug products. The challenges associated with FVIII concentrates include plateauing of drug effect, issues with its administration and adherence to treatment, breakthrough bleeds, and the development of inhibiting antibodies against administered clotting factors. Emicizumab is a bispecific antibody, launched in India in April 2019, for managing patients with HA. To investigate the role of emicizumab in Indian patients with HA, opinions were sought from 13 eminent hematologists and experts from India on the effectiveness of emicizumab in preventing all bleeds, spontaneous bleeds, perioperative bleeds, and intracranial hemorrhage; resolving target joints; and reducing the rate of hospitalizations and fatality associated with HA in children and adults, with or without inhibitors. The benefits of emicizumab over traditional FVIII concentrates include the subcutaneous route of delivery, less frequent dosing, and a lack of inhibitor development, in addition to providing sustained hemostasis without in-depth monitoring. It is a safe and effective management option for all HA patients, especially for patients with certain archetypes, such as those with inhibitors, those with high annualized bleed rates, those living far away from hemophilia care centers, pediatric patients and infants with intravenous access challenges, and those with a history of life-threatening bleeding events., Competing Interests: The authors have declared that no competing interests exist., (Copyright © 2024, Gupta et al.)

Abstract

Cervical cancer remains the leading cause of female cancer deaths in sub-Saharan Africa. This is despite cervical cancer being both preventable and curable if detected early and treated adequately. This paper reports on a series of action-research 'cycles' designed to progressively integrate a comprehensive, task-shifted, point-of-care, prevention program in a community-based public health facility in Uganda. The work has been undertaken through a UK-Ugandan Health Partnership coordinated by Knowledge for Change, a UK-registered Charity. The intervention demonstrates the effectiveness of task-shifting responsibility to Community Health Workers combined with the use of Geographic Information Systems to strategically guide health awareness-raising and the deployment of medical devices supporting respectful and sustainable point-of-care screen-and-treat services. The integration of this with public human immunodeficiency virus services demonstrates the ability to engage hard-to-reach 'key populations' at greatest risk of cervical cancer. The findings also demonstrate the impact of external influences including the Results Based Financing approach, adopted by many foreign Non-Governmental Organizations. The model presents opportunities for policy transfer to other areas of health promotion and prevention with important lessons for international Health partnership engagement. Initial Society of Critical Care Medicine Discovery Viral Infection and Respiratory illness Universal Study (VIRUS) Registry analysis suggested that improvements in critical care processes offered the greatest modifiable opportunity to improve critically ill COVID-19 patient outcomes., Objectives: The Structured Team-based Optimal Patient-Centered Care for Virus COVID-19 ICU Collaborative was created to identify and speed implementation of best evidence based COVID-19 practices., Design Setting and Participants: This 6-month project included volunteer interprofessional teams from VIRUS Registry sites, who received online training on the Checklist for Early Recognition and Treatment of Acute Illness and iNjury approach, a structured and systematic method for delivering evidence based critical care. Collaborators participated in weekly 1-hour videoconference sessions on high impact topics, monthly quality improvement (QI) coaching sessions, and received extensive additional resources for asynchronous learning., Main Outcomes and Measures: Outcomes included learner engagement, satisfaction, and number of QI projects initiated by participating teams., Results: Eleven of 13 initial sites participated in the Collaborative from March 2, 2021, to September 29, 2021. A total of 67 learners participated in the Collaborative, including 23 nurses, 22 physicians, 10 pharmacists, nine respiratory therapists, and three nonclinicians. Site attendance among the 11 sites in the 25 videoconference sessions ranged between 82% and 100%, with three sites providing at least one team member for 100% of sessions. The majority reported that topics matched their scope of practice (69%) and would highly recommend the program to colleagues (77%). A total of nine QI projects were initiated across three clinical domains and focused on improving adherence to established critical care practice bundles, reducing nosocomial complications, and strengthening patient- and family-centered care in the ICU. Major factors impacting successful Collaborative engagement included an engaged interprofessional team; an established culture of engagement; opportunities to benchmark performance and accelerate institutional innovation, networking, and acclaim; and ready access to data that could be leveraged for QI purposes., Conclusions and Relevance: Use of a virtual platform to establish a learning collaborative to accelerate the identification, dissemination, and implementation of critical care best practices for COVID-19 is feasible. Our experience offers important lessons for future collaborative efforts focused on improving ICU processes of care., Competing Interests: Drs. Abstract

Since its inception two years ago, the international, multicenter Pancreatic Cancer Early Detection (PRECEDE) Consortium has enrolled high-risk individuals (HRI) undergoing pancreatic ductal adenocarcinoma (PDAC) surveillance. Herein we aim to evaluate enrollment disparities in PRECEDE. Data on HRIs enrolled between May 2020 and March 2022 were collected, with HRIs defined as participants enrolled in PRECEDE meeting guideline-based criteria for PDAC surveillance. Of 1,273 HRIs enrolled, 1,113 were eligible for inclusion, with 47.2% meeting familial pancreatic cancer criteria without a known pathogenic variant (PV) and the remainder having a pathogenic variant in a PDAC-risk gene (CDKN2A, STK11, PRSS1, BRCA1, BRCA2, PALB2, ATM, MLH1, MSH2, MSH6, PMS2, or EPCAM). Study participants were predominantly from the United States (82.7%), the most common age range at enrollment was 60-69 years (37.4%), and a non-PDAC cancer was present in 32.4%. There were racial/ethnic- and sex-based disparities among enrolled subjects, as the majority of participants were female (65.9%) and self-reported white (87.7%), with only 2.9% having Hispanic ethnicity. While more than 97% of participants consented to utilize imaging data and biosamples for research, there was no difference in rate of consent based on race/ethnicity, sex, or age, thereby demonstrating uniform participation in research activities among all subgroups after enrollment. Ensuring that diversity of HRIs in PDAC surveillance programs mirrors the communities served by participating centers is important. Substantial racial/ethnic- and sex-based disparities persist among recently enrolled HRIs undergoing PDAC surveillance, and therefore reducing these disparities will be a major focus of the PRECEDE Consortium moving forward., Prevention Relevance: Pancreatic cancer surveillance is critical to decreasing pancreatic cancer mortality; therefore, it is important that pancreatic cancer surveillance studies enroll diverse patients. We demonstrate that substantial racial/ethnic- and sex-based disparities exist amongst enrollment in the international PRECEDE consortium, highlighting the dire need for future efforts to reduce these disparities. 48. Diffuse Esophageal Hyperkeratosis: A Rare Case Presentation.

Abstract

Diffuse esophageal hyperkeratosis (DEH) is a very intriguing and impressive mucosal finding that is quite easily identified on endoscopy and histology. A distinction must be made between microscopic/focal hyperkeratosis and endoscopically visible DEH. Microscopic hyperkeratosis is not uncommon in histological studies, while diffuse hyperkeratosis is seen very rarely. Over the past century, only a handful of cases have been reported. The endoscopic appearance of hyperkeratosis is of thick, white, piled-up mucosa. On histology, there is a prominent thickening of the stratum corneum, the squamous cells are anuclear, and there is no hyperplasia of the squamous epithelium. These histological characteristics distinguish benign orthokeratotic hyperkeratosis from other premalignant entities such as parakeratosis or leukoplakia where hyperplastic squamous cells retain pyknotic nuclei, lack keratohyalin granules, and also lack complete keratinization in superficial epithelial cells. The clinical presentation of hyperkeratosis includes gastroesophageal reflux, hiatal hernia, and associated symptoms. Our case highlights a very rare endoscopic finding associated with a common clinical presentation. The nearly 10-year follow-up reinforces the benign nature of ortho-hyperkeratosis and our report underscores the features that distinguish DEH from premalignant conditions. It merits additional research into factors that lead to hyperkeratinization of the esophageal mucosa as opposed to the more common columnar metaplasia. The concomitant presence of Barrett's esophagus in some patients is even more intriguing. Animal models with variable pH and content of the refluxate may shed light on the role played by duodenogastric/non-acid reflux in this condition. Abstract

Background: Obstructed total anomalous pulmonary venous connection (TAPVC) typically present with severe cardiovascular decompensation and requires urgent surgical management. Pulmonary arterial hypertension (PAH) is a major risk factor affecting mortality. Perioperative management focuses on providing inotropic support and managing potential pulmonary hypertensive episodes. Milrinone and inhaled nitric oxide (iNO) efficiently reduce pulmonary artery pressure (PAP) and help to improve the outcome. The aim was to determine the outcome of patients with high PAP with milrinone alone and a combination of iNO and milrinone., Material and Method: After ethical committee approval, the study was conducted over a period of 3 years in 80 patients with obstructed TAPVC repair. A total of 80 patients having severe PAH (supra systemic arterial pressure) randomly divided into two groups with 40 patients in each (M & MN). Group M (milrinone) patients received milrinone and Group MN (milrinone & iNO) patients received both milrinone (after opening aortic cross clamp) and iNO (post operative ICU). Ventilation time, hospital stay, ICU stay, complications, in hospital mortality were compared between both groups., Result: Ventilation time, Intensive Care Unit (ICU) stay, hospital stay for group M was 8.02 ± 5.74 days, 11.25 ± 7.33 day, 14.92 ± 8.55 days, respectively, and for group MN was 5.02 ± 1.78 days, 8.27 ± 3.24 days, 10.3 ± 3.18 days, respectively. In hospital mortality for group M and MN was 10% and 2.5%, respectively. P value for each variable was significant < 0.05 (except mortality)., Conclusion: Most of the patients with obstructed TAPVC had severe PAH. Management of severe PAH with a combination of milrinone with iNO had a better outcome than milrinone alone.
