1. Spontaneous bilateral tubal pregnancy - case report and review of diagnostic and treatment difficulties

Abstract

Introduction. Spontaneous bilateral ectopic pregnancy is a rare condition easily overlooked or misdiagnosed. We present a case of spontaneous bilateral tubal ectopic pregnancy and discuss the difficulties in diagnosing and treating such patients. Case outline. A 39-years-old patient with a history of irregular and abundant menstrual cycles complained of pelvic pain and light bleeding after 55 days of amenorrhea. Ultrasound revealed enlarged uterus with a myoma and a heterogenic formation with echogenic ring sign beside the left ovary. Free fluid with clots was present in the pelvis. As the patient was hemodynamically unstable and ectopic pregnancy was suspected, emergency laparotomy was performed. During the surgery we found that both tubes were significantly edematous, dilated, and livid in their ampullary regions. On the left tube anterior wall rupture 15 × 8 mm was noticeable, while right tube was intact with bleeding from its abdominal ostium. Even though the patient was not informed about the possibility of bilateral salpingectomy, after thorough consideration and due to the extent of tubal damage bilateral salpingectomy was eventually performed. Histopathological analysis confirmed the presence of decidua, partially viable and partially necrotic chorionic villi, and trophoblastic tissue in both right and left tubes. Conclusions. Careful preoperative and intraoperative examination of both Fallopian tubes as well as the whole abdominal and pelvic cavity should be mandatory during every assessment of patients with ectopic pregnancy.

2. Livestock Protective Fencing (LPF) to protect dairy cattle against vectors in Serbia: Project processes and methodology

Abstract

Lumpy skin disease (LSD) is among a number of vector-borne diseases (VBDs) threatening the Balkans and therefore, preventing feeding of insects on cattle would reduce the spread of VBDs. In order to test the efficiency of Livestock Protective Fences (LPF) in the protection of dairy cattle from insect bites, a case-control study was conducted, in the districts of Nišava, Pirot and Pčinja, Southern Serbia. It consisted in comparing the number of biting flies collected within time, between 10 farms protected with LPF and 10 non protected ones. The insects were collected using two types of traps; the monoconical Vavoua trap set outside in between forested areas or rivers and the actual farm, and the BGsentinel trap baited with CO2, placed in proximity of the cattle but outside the stable. Vectors were collected every 15 days for 48 hours from May to October 2018 and kept in vials containing 70% of ethanol. Catches per trap were separately stored and for each trap, insects were classified according to species and sex and then counted. Data on milk parameters were analyzed separately, on data collected within protected farms, before and after the LPF deployment, and on data without protection at all. It was not possible to detect a direct impact of LPF on vector densities but the number of bacteria colonies (CFU) values were reduced. Some corrections/adaption in the methodology used may lead to better impact.

Abstract

Background and Objectives: Mutations in succinate dehydrogenase (SDH) and fumarate hydratase (FH) give rise to various familial cancer syndromes, with these alterations being characteristic of certain types of histomorphologically specific leiomyomas that hold significant predictive value. Materials and Methods: This study presents two cases of uterine leiomyomas exhibiting rare histomorphological and genetic characteristics, which are crucial for prognosis and further treatment. Results: Distinct histopathological features such as marked nuclear atypia, intracellular eosinophilic globules, and abnormal intratumoral vessels raise suspicion for specific leiomyoma subtypes, which carry predictive significance for additional hereditary cancer syndromes. Immunohistochemical analysis confirmed FH/SDH deficiency in both patients, who underwent careful follow-up. Conclusions: This study describes two cases involving unusual leiomyomas, the histopathological characteristics of which may easily go unrecognized. These features hold predictive significance because their specific mutations point to additional hereditary cancer syndromes, highlighting the need for further examinations. [ABSTRACT FROM AUTHOR]

5. Modeling the Effects of Horizontal Transverse Vibrations on the Thermal Behavior and the Ampacity of Rectangular Bus Bars

Abstract

The purpose of this research is to correctly model steady-state heat transfer in and around rectangular bus bars installed horizontally in an indoor environment and to estimate the corresponding ampacities, considering the effects of horizontal transverse vibrations caused by electromagnetic forces. This thermo-electro-magneto-mechanical problem is solved analytically using correlations determined experimentally by other researchers, while the accuracy of the obtained results is verified numerically using the finite element method (FEM). The novelties of the developed model are as follows. First, modeling the effects of horizontal transverse vibrations on free convection from the top and bottom surfaces of rectangular bus bars via forced convection for different characteristic lengths. Second, modeling the effects of vibration amplitudes and vibration frequencies on the bus bar ampacity. Third, introducing the existing vibration classes (A, B, and C) into the analytical and FEM-based thermal analyses. The results show that with an increase either in the vibration amplitude or the vibration frequency, there is a greater convection-based dissipation of heat from the bus bars and an increase in their ampacity. Finally, for the standard vibration classes, it is found that the effect of horizontal transverse vibrations on the ampacity can be up to 41.99% for Class C.

Abstract

Epitelni tumori jajnika čine oko 90% svih ovarijalnih tumora. Poreklo vode od ćelija mezotela koje oblažu površinu jajnika tj. od epitelnih Mullerian type ćelija jajovoda. U odnosu na sposobnost proliferacije epitela, dele se na benigne, maligne i atipično proliferišuće tumore (ATP) jajnika. Ovarijalni karcinomi spadaju u grupu malignih ginekoloških tumora sa najvećom stopom mortaliteta širom sveta. Uprkos primenjenoj terapiji, pacijentkinje sa uznapredovalim stadijumima bolesti vrlo slabo reaguju na do sada primenjene modalitete lečenja. Ovarijalni karcinomi vrlo efikasno suprimiraju imunološke odgovore domaćina. U karcinomskim ćelijama PD-L1 regulatorni mehanizam može zaštititi tumor od citotoksičnih T-limfocita narušavajući antitumorski imunitet. Autofagija je vrlo bitna u nastanku i razvoju neoplastičnih procesa, kao i u reagovanju tumora na razne oblike antitumorske terapije. Primećeno je da inhibicija PD-1/PD-L1 mehanizma stimuliše proces autofagije u tumorskim ćelijama i omogućava njihovo preživljavanje. Ciljevi: Analiza imunohistohemijske ekspresije PD-L1 molekula i markera autofagije p62, LC3 i Beclin1 u epitelnim tumorima jajnika, u odnosu na histološki tip tumora, stadijum tumora, kao i njihovu regionalnu distribuciju unutar tumorskog tkiva (centralna u odnosu na periferna područja najinvazivnijeg rasta tumorskog tkiva) i u odnosu na postojanje ili nedostatak reaktivne limfocitne infiltracije. Analiza potencijalne međusobne povezanosti karakteristika ekspresije pomenutih markera i njihove povezanosti sa najvažnijim kliničkim i patohistološkim parametrima bolesti... Epithelial ovarian tumors are 2/3 of all ovarian tumors. Their origin is from mesothelial cells that line the surface of the ovaries, ie. of epithelial Mullerian type cells from uterine tube. In relation to the ability of epithelial proliferation, they are divided into benign, malignant and atypically proliferating (APT) epithelial ovarian tumors.Ovarian cancer is the malignant gynecological tumor with the highest mortality rate. Despite the standard therapy protocols, patients in advanced stages of the disease show poor treatment results. Ovarian cancer can very effectively suppress the host immune responses. Tumor cells with expressed PD-L1 protein can activate the imunosupressive regulatory mechanism and protect themselves from the cytotoxic T-lymphocytes. Autophagy is very important mechanism in tumorogenesis, as well as in the response of tumors to various form of antitumor therapy. Inhibition of the PD-1/PD-L1 mechanism has been found to stimulate autophagy in tumor cells and allow them to survive. 9. Prognostic relevance of autophagy-related markers p62, LC3, and Beclin1 in ovarian cancer

Abstract

Aim To analyze the expression of autophagy markers p62, LC3, and Beclin1 in ovarian cancer tissue and evaluate the prognostic potential of these markers. Methods The study enrolled 328 patients: 122 with epithelial ovarian carcinoma, 42 with atypical proliferative tumor, and 164 with benign epithelial ovarian tumor. The expression of p62, LC3, and Beclin1 was analyzed in central and invasive tumor segments with immunohistochemistry combined with tissue microarray. The expression levels of the analyzed markers were correlated with relevant histopathology parameters. Results The expression of all analyzed markers was most remarkable in epithelial ovarian carcinoma. There was a strong positive correlation between the expressions of p62 and LC3, while these two markers negatively correlated with Beclin1. High-grade serous carcinoma had higher p62 and LC3 levels, and lower Beclin1 levels than other tumor types. This expression profile was also observed in more advanced tumor stages. Conclusion Prominent p62 and LC3 expression in combination with weak Beclin1 expression in high-grade serous carcinoma indicates potential for the application of autophagy inhibitors in patients with this tumor subtype.

Abstract

Bolest kvrgave kože (LSD) spada među brojne bolesti koje se prenose vektorskim putem (VBD) koji prete Balkanu, pa bi sprečavanje hranjenja insekata stokom smanjilo širenje VBD-a. Da bi se testirala efikasnost stočnih zaštitnih mreža (LPF) u zaštiti mlečnih goveda od uboda insekata, sprovedena je studija njene efikasnosti. Ogledi su rađeni u Nišavskom, Pirotskom i Pčinjskom okrugu ( južna Srbija). Sastojao se u poređenju broja hematofagih insekata prikupljenih u šestomesečnom periodu , na 10 farmi zaštićenih LPF i 10 nezaštićenih. Insekti su sakupljeni koristeći dve vrste zamki; monokonična zamka Vavoua koja je postavljena napolju između šumovitih područja ili reka i stvarne farme, i BG-sentinel zamka sa CO2, smeštenih u blizini stoke, ali izvan štale. Vektori su prikupljeni svakih 15 dana tokom 48 sati od maja do oktobra 2018. godine i držali su se u bočicama sa 70% etanolom. Ulovi po zamci odvojeno su smešteni i za svaku zamku su insekti klasifikovani prema vrsti i polu, a zatim su brojani. Podaci o parametrima mleka analizirani su odvojeno. Podacima na zaštićenim farmama prikupljani su pre i posle primene LPF i sa farmi gde nije postavljena zaštita. Nije bilo moguće detektovati direktan uticaj LPF na gustinu vektora, ali je broj kolonija bakterija (CFU) u mleku smanjen. 13. Classification of the asphalts and their source rock from the Dead Sea Basin (Israel): the asphaltene/kerogen vanadyl porphyrins

Abstract

The asphaltenes of the asphalts from the Dead Sea Basin were examined for the presence of vanadyl porphyrins. These examinations demonstrated that the asphalts fall into two broad class types, one having a relatively high vanadyl prophyrins content ( GT 300 ppm) and the other characterized by no vanadyl porphyrins ( LT 10 ppm). It is concluded that these asphalts belong to two generic types and have separate origins. The kerogens isolated from the petroleum-source rock of the Dead Sea Basin were also analyzed. Two genetic types of kerogen appeared to exist, a marine one highly enriched with these pigments and a terrestrial one with no vanadyl porphyrins. For comparison, the asphaltenes of typical asphaltic crude oils from Western Venezuela and kerogen from their La Luna source rock were also examined. Previous detailed geochemical studies indicated that La Luna kerogen is derived from marine organic matter source. These materials have higher concentrations of vanadyl porphyrins than the Dead Sea asphaltenes and the kerogens are enriched with these compounds. Our results strengthen the potential of vanadyl porphyrins of both petroleum asphaltenes and source-rock kerogen for use in genetic and correlation studies.
