A total of 24 patients with a mean age of 45.8 +/- 20.8 were included in the study. The patients were grouped as Control (C), Degenerative Syndromes (DS), Degeneration Associated with External Factors (DEF), Degeneration Associated with Focal Neurologic Lesion (DFN) and Demyelinating Disease (DM). Imaging started 15 minutes for early and 4 hours for delayed scans after i.v. infusion of I-123 IMP. The rCBF was calculated by the IMP autoradiographic (ARG) method. The wash-out ratio (WR) was calculated as the ratio of the Delay/Early count. In the rCBF of the various areas of the brain, significant differences were noted between various disease groups. No correlation was noted between rCBF and WR (r = -0.50). The WR of patients grouped according to various disease processes did not show a significant difference between various areas of the brain. In conclusion, the rCBF was effective in separating both various areas of the brain and disease entities. WR from a delayed study is less useful in neurodegenerative diseases. [ABSTRACT FROM AUTHOR]

We determined the effect of retention on the changes in regional biodistribution of Tc-99m ECD in the brain. A total of 14 cases, 7 normal volunteers and 7 patients with various diagnoses but with very minimal radiologic findings or none were included in the study. SPECT images were taken at 30 min, 1, 2, 3, 4 and 6 hrs after an intravenous injection. Retention rates were calculated in various regions and were corrected according to the time decay of technetium. There was a tendency for the retention rate to increase up to three hours of imaging and then a decrease was noted in most regions of the brain. In the thalamus, increasing retention was noted. In conclusion, Tc-99m ECD retention in the different regions of the brain varies with time. These differences should always be considered when planning and interpreting SPECT quantitative studies. To investigate the influence of diabetic myocardial damage (suspected myocardial damage; SMD) diagnosed by201T1-SPECT and diabetic cardiac autonomic neuropathy (AN) on myocardial MIBG uptake in patients with non-insulin-dependent diabetes mellitus (NIDDM). Eighty-seven diabetic patients divided into four subgroups: 23 with SMD (+) AN (+); 19 with SMD (+) AN (−); 27 with SMD (−) AN (+); 18 with SMD (−) AN (−), and 10 controls were studied. Both planar and SPECT images were taken at 30 minutes (early) and 3 hours (delayed) after123I-MIBG injection. The heart to mediastinum uptake ratio (H/M) and washout ratio of123I-MIBG (WR) were obtained from both planar images. On SPECT images, the total uptake score (TUS) was obtained by the 5 point score method by dividing the myocardium into 20 segments on visual analysis. Similarly, the difference between the201Tl image and the123I-MIBG image in TUS was taken as the difference in the total uptake score (ΔTUS) representing cardiac sympathetic denervation without SMD. On both early and delayed planar images, the mean H/M value in the subgroups of diabetic patients was significantly lower in the SMD (+) AN (+) group than in the control group, but among those subgroups, there was statistically significant difference between the SMD (+) AN (+) and SMD (−) AN (−) groups only on the delayed images. Regarding the WR value, there was no statistically significant difference among subjects. On SPECT image analysis, the diabetic sub-group with AN or SMD had statistically significant lower values for TUS than those of the control group. Among diabetics, there was a statistically significant differences between SMD [+] AN [+] and SMD [−] AN [−] on both early and delayed images. Similarly, the SMD [+] AN [−] group also had significantly lower values than those of SMD [−] AN [−] on early images. Regarding ΔTUS, there was a statistically significant differences between AN [+] subgroups and controls. Similarly, the mean value for ΔTUS was much higher in AN [+] subgroups than in AN [−] subgroups with or without SMD in diabetes mellitus. 123I-MIBG myocardial uptake is affected by both SMD and cardiac autonomie neuropathy. Based on the finding that ΔTUS was much higher in AN [+] subgroups and there was no statistically significant difference between SMD [+] AN [+] and SMD [−] AN [+] subgroups, a decrease in myocardial123I-MIBG uptake might progress independently of SMD. The case of a 77-year-old male patient who complained of left upper quadrant pain and progressive vomiting. Laboratory examination showed extremely high lactic acid dehydrogenase (LDH) and adult T-cell leukemia antibody (ATLA). The anatomical studies CT, MRI, US and upper GI series substantiated an omental lymphadenopathy which was causing a circumferential compression of portions of the duodenum and jejunum. Gallium-67 citrate (Ga-67) scintigraphy showed high uptake at LUQ. Ultrasound guided biopsy failed to confirm the diagnosis. Irradiation was performed. Ga-67 scintigraphy had a contributory role in clinical subtyping of the disease, planning of treatment, posttreatment assessment and prognostication of adult T-cell lymphoma. [ABSTRACT FROM AUTHOR]

Objective: This multicenter, phase II clinical trial evaluated the diagnostic performance of 18F-fluciclovine, a novel amino acid for positron-emission tomography (PET), for detection of small lymph node metastases with short-axis diameters of 5-10 mm in patients with prostate cancer., Methods: Patients with prostate cancer were eligible after screening of laboratory tests and pelvic contrast-enhanced computed tomography (CT). Pelvic region 18F-fluciclovine PET/CT was then acquired within 28 days and dissection of regional lymph nodes was performed within 60 days of pelvic contrast-enhanced CT. Diagnostic performance of 18F-fluciclovine-PET/CT was evaluated by comparison with standard histopathology of lymph nodes., Results: In a total of 28 patients, 40 regional lymph nodes with short-axis diameters of 5-10 mm were eligible for efficacy evaluation; seven of these showed metastases confirmed by histopathology. The sensitivity of 18F-fluciclovine PET/CT was 57.1% (4/7). All four true positive lymph nodes detected by 18F-fluciclovine PET/CT had a metastatic lesion with a long-axis diameter of ≥7 mm and a high proportion of cancer volume (60-100%) according to pathology evaluation. The specificity, diagnostic accuracy, positive predictive value, and negative predictive value of 18F-fluciclovine PET/CT in lymph node-based analysis were 84.8% (28/33), 80.0% (32/40), 44.4% (4/9), and 90.3% (28/31), respectively. No clinically significant adverse events occurred., Conclusions: 18F-fluciclovine PET/CT detected small lymph node metastases; however it also showed positive findings in benign lymph nodes. Refinement of the image assessment criteria may improve the diagnostic performance of 18F-fluciclovine PET/CT for small lymph node metastases in patients with prostate cancer., (© The Author(s) 2019. Background/aim: The [(18)F]-fluorodeoxyglucose positron emission tomography (FDG-PET) cancer screening program is defined as a cancer screening for asymptomatic subjects using FDG-PET (including PET/ computed tomography [CT]) with or without combination of other screening tests. The aim of the present study was to analyze the thyroid cancer detection rate in asymptomatic individuals through a FDG-PET cancer screening program in Japan., Materials and Methods: A total of 153,775 asymptomatic individuals underwent FDG-PET cancer screening. We analyzed 1,308 cases for possible thyroid cancer in all screening tests., Results: Among the 1,308 possible cases, 353 were verified as thyroid cancer. The relative sensitivity and positive predictive value (PPV) of FDG-PET were 90.7% and 29.5% for thyroid cancer, respectively. The relative sensitivity was equivalent to thyroid ultrasonography (US) (90.9%) and higher than neck computed tomography (CT) (75.7%, p<0.001), thyroglobulin (36.1%, p<0.001), and carcinoembryonic antigen (5.6%, p<0.001). The sensitivity with a PET/CT scanner was higher than that with a dedicated PET scanner (94.1% vs. 85.0%, p<0.001). Combining thyroid US with FDG-PET increased the relative sensitivity and PPV. The majority of thyroid carcinomas detected by FDG-PET screening were Union for International Cancer Control (UICC) stage I, but a significant number of cases were also detected as stage III or IV., Conclusion: The FDG-PET cancer screening program in Japan detected thyroid cancer at an early stage. Purpose: The aim of this study was to analyze the detection rate for CRC and adenomas for asymptomatic subjects in Japan by FDG-PET cancer screening program carried out between 2006 and 2009., Methods: The "FDG-PET cancer screening program" included both PET and positron emission tomography with computed tomography (PET/CT) with or without other screening tests. A total of 154,783 asymptomatic subjects underwent FDG-PET cancer screening program; we analyzed the 1,808 cases with findings from any detection method that indicated suspected CRC., Results: Among the 1,808 cases, the number of cases verified as CRC and adenoma was 394 and 679, respectively. The sensitivity and positive predictive value (PPV) of FDG-PET were 86.0 and 31.7% for CRC, and 63.6 and 63.8% for CRC and adenoma. The sensitivity and PPV of fecal occult blood test (FOBT) for CRC were lower than those of FDG-PET, but higher for adenoma. Therefore, FDG-PET and FOBT were complementary for screening for CRC, and CRC and adenoma. The majority of CRC detected by the FDG-PET imaging was UICC stage 0 or I, however, detection of smaller or less invasive cancer was limited., Conclusion: The FDG-PET screening program in Japan has detected CRC at an early stage and adenomas as premalignant lesions. A combination of FDG-PET and FOBT yields the best results if the intent is to detect either CRC or adenoma. However, it is absolutely clear that an FDG-PET cancer screening program cannot detect all colon lesions.

Objectives: The aim of this study was to analyze detection rates and effectiveness of (18)F-fluorodeoxyglucose positron emission tomography (FDG-PET) cancer screening program for prostate cancer in Japan, which is defined as a cancer-screening program for subjects without known cancer. It contains FDG-PET aimed at detection of cancer at an early stage with or without additional screening tests such as prostate-specific antigen (PSA) and magnetic resonance imaging (MRI)., Methods: A total of 92,255 asymptomatic men underwent the FDG-PET cancer screening program. Of these, 504 cases with findings of possible prostate cancer in any screening method were analyzed., Results: Of the 504 cases, 165 were verified as having prostate cancer. Of these, only 61 cases were detected by FDG-PET, which result in 37.0% relative sensitivity and 32.8% positive predictive value (PPV). The sensitivity of PET/computed tomography (CT) scanner was higher than that of dedicated PET (44.0% vs. 20.4%). However, the sensitivity of FDG-PET was lower than that of PSA and pelvic MRI. FDG-PET did not contribute to improving the sensitivity and PPV when performed as combined screening., Conclusion: PSA should be included in FDG-PET cancer screening programs to screen for prostate cancer.

Aim: The aim of this study was to analyze the lung cancer detection rate in asymptomatic individuals by the Fluorine-18 fluorodeoxyglucose-positron emission tomography FDG-PET cancer screening program in Japan., Materials and Methods: A total of 153,775 asymptomatic individuals underwent the FDG-PET cancer screening program; the 854 cases with findings that indicated suspected lung cancer by any detection method were analyzed., Results: Among the 854 cases, 319 were verified as lung cancer. The relative sensitivity and positive predictive value (PPV) of FDG-PET were 86.5% and 38.9% for lung cancer, respectively. The sensitivity of PET/computed tomography (CT) scanner was higher than that of dedicated PET (100.0% vs. 63.2%), indicating that CT imaging was effective for lung cancer screening. The majority of lung carcinomas detected by FDG-PET screening were UICC stage IA or IB, but detection of smaller or less invasive carcinomas was limited., Conclusion: The FDG-PET screening program in Japan detected lung cancer at an early stage.

Purpose: The aim of this study was to evaluate the interpretations of incidental colonic 18F-FDG uptake made by 10 experienced readers and to more clearly identify the pattern of suspicious colonic FDG uptake. The potential contributions of delayed FDG-PET scanning and of immune fecal occult blood testing (FOBT) in making a diagnosis were also analyzed., Materials and Methods: Visual interpretations by 10 readers were made for 147 FDG uptake sites from 126 PET scans (cancer, 38 sites; adenoma, 43 sites; and no abnormality, 66 sites) with colonic FDG uptake. Assessments for the early FDG-PET images were (1) FDG uptake pattern, (2) FDG uptake degree, and (3) likelihood of malignancy. For the delayed images, the assessments were (1) change in the FDG uptake position, (2) change in FDG uptake degree, and (3) likelihood of malignancy. The results of FOBT were analyzed independently of the visual interpretations., Results: Interobserver agreement (κ) was 0.501 for assessing FDG uptake patterns, while agreement on assessing changes in uptake degree and changes in uptake position between early and delayed imaging were low (κ = 0.213-0.229). Logistic regression analysis indicated that 'FDG uptake patterns' and 'FDG uptake degree' were significantly related to decide on the suspicion of malignancy (p < 0.001) and the final result (p < 0.001). "Small localized" and "large irregular localized" types had a high probability of a lesion regardless of either (1) FDG uptake degree or (2) variation in the uptake between the early and the delayed image. The delayed image decreased false-positive cases for some FDG uptake patterns, but it had little impact on distinguishing clearly between "cancer or adenoma" and "normal". The addition of FOBT had little impact on the diagnosis., Conclusion: There was highest agreement among readers with respect to the recognition of specified colonic FDG uptake patterns, and this pattern recognition had the most influence on the diagnosis. "Small localized" and "large irregular localized" types had a high probability of a lesion. The addition of delayed imaging and of FOBT results to the early imaging did not have much impact on the diagnosis.

Objective: The aim of this study was to survey the 18F-fluorodeoxyglucose-positron emission tomography (FDG-PET) cancer screening program conducted in Japan., Methods: The "FDG-PET cancer screening program" included both FDG-PET and positron emission tomography with computed tomography (PET/CT) with or without other combined screening tests that were performed for cancer screening in asymptomatic subjects. A total of 155,456 subjects who underwent the FDG-PET cancer screening program during 2006-2009 were analyzed., Results: Of the 155,456 subjects, positive findings suggesting possible cancer were noted in 16,955 (10.9 %). The number of cases with detected cancer was 1,912 (1.23 % of the total screened cases, annual range 1.14-1.30 %). Of the 1,912 cases of detected cancer, positive findings on FDG-PET were present in 1,491 cases (0.96 % of the total number of screened cases). According to the results of further examinations, the true positive rate for subjects with suggested possible cancer (positive predictive value) was 32.3 % with FDG-PET. Cancers of the colon/rectum, thyroid, lung, and breast were most frequently found (396, 353, 319, and 163 cases, respectively) with high PET sensitivity (85.9, 90.7, 86.8, 84.0 %, respectively). Prostate cancer and gastric cancer (165 and 124 cases, respectively) had low PET sensitivity (37.0 and 37.9 %, respectively). The Union for International Cancer Control (UICC) clinical stage of cancer found with the FDG-PET cancer screening program was mainly Stage I., Conclusions: The FDG-PET screening program in Japan has detected a variety of cancers at an early stage. However, several cancers were found in repeated FDG-PET cancer screening program, indicating the limitation of a one-time FDG-PET cancer screening program. The value of the FDG-PET cancer screening program is left to the judgment of individuals with regard to its potentials and limitations.

Objective: The aim of this study was to estimate radiation exposure and evaluate the risks and benefits of 18F-fluorodeoxyglucose positron emission tomography (FDG-PET) in cancer screening., Methods: A nationwide survey of FDG-PET cancer screening was conducted in 2006, and the results were analyzed with a common index, "extension/shortening of the average life expectancy.", Results: The average estimated effective dose was 4.4 mSv (male 4.7 mSv; female 4.0 mSv) for dedicated PET and 13.5 mSv (male 14.2 mSv; female 12.8 mSv) for PET/computed tomography (CT). The risk-benefit break-even age from the viewpoint of radiation exposure was in the 40s for men and 30s for women for dedicated PET and in the 50s for men and 50s (variable injection dose) or 60s (constant injection dose) for women for PET/CT., Conclusions: FDG-PET cancer screening is beneficial for examinees above the break-even ages. The risks and benefits should be explained to examinees because of the larger radiation used in cancer FDG-PET screening compared with other X-ray tests.

We have devised a technique of two-stage tracheal reconstruction using S-shaped skin flaps and an aural conchal cartilage graft. During the first operation, S-shaped skin flaps were elevated before resection of the trachea. A tracheocutaneous fistula was created at the tracheal defect using S-shaped skin flaps while placing the conchal cartilage graft underneath. During the second operation, a skin incision was made around the fistula to elevate the hinge flaps, including the cartilage. The edges of the hinge flaps were sutured to form the tracheal lumen, and the area of the skin defect was then closed with double-rotation skin flaps., (Copyright © 2011 The Society of Thoracic Surgeons. Objective: The most distinctive feature of FDG-PET cancer screening program is the ability to find various kinds of malignant neoplasms in a single test. The aim of this survey is to clarify the range and frequency of various malignant neoplasms detected by FDG-PET cancer screening performed in Japan., Methods: "FDG-PET cancer screening" was defined as FDG-PET or positron emission tomography and computed tomography (PET/CT) scan with or without other tests performed for cancer screening of healthy subjects. This survey was based on a questionnaire regarding FDG-PET cancer screening. We analyzed the situation of 9 less frequently found malignant neoplasms including malignant lymphoma, malignancy of head and neck, esophagus, hepatobiliary and gallbladder, pancreas, kidney, cervical and uterine, ovary, and bladder., Results: The detailed information of subjects with the suspected 9 kinds of malignant neoplasms mentioned above in the FDG-PET cancer screening program was studied in a total of 1,219 cases from 212 facilities. A statistical significance between PET/CT and PET was found in relative sensitivity and PPV for renal cell cancer. Malignant lymphoma was frequently of indolent type, suspected head and neck cancers had many false-positive results, and pancreatic cancer detected in this program was often in the advanced stage even in asymptomatic subjects. The recommendation of combined screening modality to PET or PET/CT was as follows: gastric endoscopy for assessing early esophageal cancer; abdominal ultrasound for screening hepatobiliary and gallbladder cancer; pelvic magnetic resonance imaging for assessing gynecological and pelvic cancers; and the CA125 blood test for screening ovarian cancer. Delayed image was helpful depending on the type of suspected malignant neoplasm., Conclusion: We analyzed various types of malignant neoplasms detected by the FDG-PET cancer screening program and presented recommended combination of examinations to cover FDG-PET and PET/CT.

Objective: The aim of this study is to survey the situation of (18)F-fluorodeoxyglucose-positron emission tomography (FDG-PET) cancer screening in Japan and to describe its performance profile., Methods: "FDG-PET for cancer screening" was defined as FDG-PET or positron emission tomography and computed tomography (PET/CT) scan with or without other tests performed for cancer screening of healthy subjects. We sent questionnaires regarding FDG-PET cancer screening to 99 facilities in which FDG-PET tests were performed during the fiscal year 2005. Replies were obtained from 68 of the 99 facilities, of which 46 facilities performed FDG-PET cancer screening. The total number of subjects who underwent FDG-PET cancer screening was 50 558. From 38 of 46 facilities, reliable results of thorough examinations were obtained for the subjects who were positive by FDG-PET and/or one or more of the combined screening tests was performed and were referred for further evaluation. The total number of subjects in these 38 facilities amounted to 43 996., Results: A total of 50,558 healthy subjects underwent FDG-PET (including PET/CT) scanning with or without other tests for cancer screening in 46 PET centers during the fiscal year of 2005 in Japan. Thorough examination was indicated for 9.8% of the cases as a result of positive findings suggesting possible cancer. On analyzing 43 996 cases from 38 PET centers from which detailed information was obtained, 500 cases of cancers (1.14%) were found, of which 0.90% were PET positive and 0.24% were PET negative, resulting in the relative sensitivity of PET being 79.0%. Cancers of the thyroid, colon/rectum, lung, and breast were most frequently found (107, 102, 79, and 35 cases, respectively) with high PET sensitivity (88%, 90%, 80%, and 92%). PET showed an overall positive predictive value of 29.0%. PET/CT had a better detection rate, sensitivity, and positive predictive value than dedicated PET (P < 0.01)., Conclusions: We were able to clarify the performance profile of "FDG-PET for cancer screening" on the basis of a Japanese nationwide survey. The number of facilities possessing PET is increasing steadily, highlighting the necessity of evaluating the usefulness of "FDG-PET cancer screening" as soon as possible by undertaking long-term investigations of large series of subjects.

Purpose: To determine the usefulness of brain perfusion SPECT for evaluating the severity and progression of Alzheimer's disease (AD)., Methods: Eighty-four AD patients were included. At entry, 99mTc-HMPAO-SPECT, the Mini Mental State Examination (MMSE), Mental Function Impairment Scale (MENFIS), and the Raven Colored Progression Matrix (RCPM) were performed for all 84 patients. During the follow-up periods, two individual MMSE evaluations in 34 patients, two MENFIS evaluations in 30 patients, and two RCPM evaluations in 20 patients were performed. Based on the regions of decreased cerebral blood flow demonstrated on 3D-SSP images of SPECT, the cases were classified as type A (no decrease), type B (decreased blood flow in the parietal or temporal lobe), type C (decreased blood flow in the frontal lobe and parietal or temporal lobe), type Pc (decreased blood flow in posterior cingulate gyrus only), and "other types". The types of decreased blood flow, scores on neuropsychological evaluations, and symptom progression were analyzed., Results: The MENFIS, MMSE, and RCPM scores were poorest in type C patients at entry. The degree of decrease of these scores during the follow-up periods was also greatest in type C. The greatest difference between patients with and without rapid progression in SPECT data of the mild AD patients (MMSE score > or = 24) was in the frontal lobe., Conclusion: Decreased blood flow in the frontal lobe of AD patients is correlated not only with reduced cognitive function at the time of the evaluation but with rapid progression in the subsequent clinical course.

We report a case of an 85-year-old woman with a foreign-body granuloma which accumulated 18F-fluorodeoxyglucose (FDG). Unenhanced computed tomography showed a hyperdense mass with a hypodense rim in the right subphrenic space. FDG PET/CT images showed intense FDG uptake in the hypodense rim and little FDG uptake in the center of the mass, showing a ring-shaped appearance. The fusion imaging of FDG PET/CT represented the metabolic features of the foreign-body granuloma. When a ring-shaped FDG uptake is noted in the abdomen of a patient with a history of abdominal surgery, a foreign-body granuloma should be included in the differential diagnosis.

Unlabelled: The current study is an investigation of alterations in regional cerebral blood flow (rCBF) distribution in patients with transient hypothyroidism after thyroidectomy. In addition, the effects of thyroxine treatment on rCBF changes were studied., Methods: Noninvasive rCBF measurements using 99mTc-HMPAO SPECT were performed on 24 post-thyroidectomy patients who were in a hypothyroidic state. The measurements were conducted before 131I therapy and after thyroid hormone (thyroxine) replacement. We used adjusted rCBF images (normalization of global CBF for each subject to 50 ml/100 g/min with proportional scaling) to compare these data with age-matched normal control groups (n =15) using SPM96. We also compared the absolute rCBF value of hypothyroidic patients with those of normal control groups. In addition, the association between rCBF alteration and the severity of depression was also analyzed. Finally, the effect of thyroid hormone replacement on rCBF was investigated individually using the Jack-knife test, in which patient data were compared with those from healthy volunteers. According to the result of this test, all cases were categorized into three subgroups, namely, improved, unchanged group and normal. To prove the reversibility of rCBF alteration after thyroid hormone replacement, a group comparison test between the normal controls and the improved group was done before and after thyroid hormone replacement. Similarly a group comparison test between the unchanged group and normal controls was also performed., Results: In the hypothyroidic condition, there was a significant decrease in the posterior part of the bilateral parietal lobes and in part of the bilateral occipital lobes, including the cuneus. These decreased rCBF areas extended to the bilateral prefrontal cortices as deterioration became more profound. On individual analysis, 16 of 24 patients (66.7%) demonstrated rCBF reduction, while 8 patient did not show significant rCBF change (33.3%, the normal group). After thyroxine replacement, improvement of rCBF was noted in nine of 16 patients (56.3%, the improved group). In seven of 16 patients (43.7% the unchanged group), the significant low rCBF area remained unchanged. Compared with the normal controls, the improved group showed significantly decreased rCBF of the bilateral parietal lobe and the occipital lobe in the hypothyroic condition. After thyroid hormone replacement, these abnormal rCBF areas disappeared. In contrast, in the unchanged group, the significant hypoperfusion area became localized but remained., Conclusion: 99mTc-HMPAO SPECT and SPM96 analysis demonstrated a significant rCBF decrease in the parietal lobe and part of the occipital lobe in patients with induced transient hypothyroidism after thyroidectomy. This phenomenon might contribute to understanding of the depressive state. Recovery of rCBF after thyroid hormone replacement was confirmed in some patients. However, rCBF improvement did not always occur in every patient during the follow up period. The reversibility of rCBF in transient hypothyroidism may be dependent on individual characteristics during a short-term period.

Purpose: We performed 123I-MIBG (MIBG) myocardial scintigraphy twice in patients with non-insulin-dependent diabetes mellitus (NIDDM) to investigate whether MIBG distribution was improved by pertinent clinical control. To determine the influential factors for MIBG distribution, we investigated the association between various clinical parameters and the serial change in MIBG uptake parameters., Patients and Methods: Twenty NIDDM patients with no cardiac disorders were evaluated. Planar images were taken at 30 minutes (early) and 3 hours (delayed) after MIBG injection. The heart-to-upper-mediastinum uptake ratio (H/M) and washout ratio (WR) were calculated as parameters for estimating cardiac sympathetic function. Patients were divided into two groups, eight in the improved group and twelve in the unimproved group, according to the serial change in H/M. The mean interval between the baseline and the follow up study was 2.1 +/- 0.6 year. Differences between the means of the laboratory data in patients in both groups were compared for the baseline and the follow up study by using the paired t-test. As a means of determining the influential factors for a serial change of MIBG uptake, Fisher's exact test was performed to evaluate the association between the serial change in cardiac MIBG parameters and changes in other clinical parameters, such as blood sugar (BS) control, BS control method (insulin therapy), serum cholesterol control, and severity of diabetic complications. We also analyzed the association between the changes in CV(R-R) (coefficient variance of R-R intervals at rest ECG) or NCV (velocity of posterior tibial nerve) and those of other clinical parameters. Associations among these neurological parameters (MIBG parameters, CV(R-R) and NCV) were also analyzed., Results: Paired t-tests showed a significant decrease in fasting blood sugar and fructosamine in the improved group in the follow up study compared to those in the baseline study. Nevertheless, Fisher's exact test showed no significant association between FBS, HbA1C, fructosamine and the improvement in cardiac MIBG uptake. The only significant association was observed between the serial change in H/M and the BS-control method (insulin therapy). Within the neurological parameters, a significant association was noted between the serial changes in H/M and CV(R-R)., Conclusion: Although BS control was likely to be an important factor, it did not always ameliorate cardiac MIBG uptake. Based on the significant association between the BS-control method (insulin therapy) and MIBG uptake change, the severity of diabetes mellitus was likely to be a more influential factor. It was suggested that cardiac MIBG uptake could improve within the mild stage if controlled by diet therapy or an oral hypoglycemic agent in NIDDM.

We also evaluated the usefulness of linearization correction for acetazolamide challenge test of 99mTc-ECD SPECT., Methods: Twenty patients with unilateral chronic occlusive cerebrovascular disease (10 patients had middle cerebral arterial lesion and 10 had internal carotid lesion) were included in the study. Split-dose (a dose fractioning was 1:2), and sequential SPECT technique was used for 99mTc-ECD SPECT studies while only acetazolamide challenge test studies for 123I-IMP SPECT were performed. Permeability surface area product model (PS model) and back-diffusion model (Lassen's correction) were used for linearization correction of acetazolamide challenge with 99mTc-ECD SPECT., Results: Six of 16 patients with reduced vasodilatory capacity in 123I-IMP SPECT were underestimated by 99mTc-ECD SPECT acetazolamide challenge test. Relative ECD uptake normalized by cerebellar uptake compared with IMP uptake showed a nonlinear relationship, indicating relatively less uptake in high flow range. The underestimations of limited vasodilatory capacity observed in 99mTc-ECD SPECT without linearization correction was modified by linearization algorithm. However, the effect of correction based on PS model was superior than that of Lassen's correction. The corrected 99mTc-ECD uptake ratio, based on PS model, and IMP uptake ratio demonstrated a better linear relationship than that of Lassen's correction., Conclusion: Technetium-99m ECD SPECT corrected based on the PS model is a better method of linearization for evaluating cerebrovascular reserve using acetazolamide challenge.

Unlabelled: Technetium-99m methoxyisobutylisonitrile (MIBI) is a myocardial perfusion imaging agent that has been reported to effectively localize in various tumors (e.g., lung and thyroid carcinomas and osteogenic sarcoma). To determine its usefulness in thyroid tumors, we compared 99mTc-MIBI with 201TI imaging., Method: We evaluated 25 patients with thyroid tumors (papillary carcinoma in 11, follicular carcinoma in 2, follicular adenoma in 7, adenomatous goiter in 5). Fifteen metastatic lesions from differentiated thyroid carcinomas were also evaluated. Early (10 min after injection) and delayed images (120 min after injection) were obtained for both 99mTc-MIBI and 201TI scintigraphy., Results: The early images showed very similar findings for both 99mTc-MIBI and 201TI. However, the delayed images showed that malignant tumors tended to retain more tracer agent than benign nodules. Marked retention was in 61.5% (8 of 13) of 201TI images and 53.8% (7 of 13) of 99mTc-MIBI images. For metastatic lesions from thyroid carcinomas, the findings for 99mTc-MIBI imaging were nearly identical to those for 201TI imaging. A slight difference in clarity was seen that may have been due to the effect of the 99mTc., Conclusion: Although 99mTc-MIBI scintigraphy does not have particularly good results in differentiating malignant from benign thyroid tumors, it may be useful in evaluating metastases or predicting recurrence because of its better imaging characteristics.

Purpose: To investigate cerebral vasodilatory capacity by acetazolamide challenge in healthy subjects and in patients with chronic occlusive cerebrovascular disease by using susceptibility-weighted gradient-echo MR imaging., Methods: Eight patients with chronic occlusive cerebrovascular disease and four healthy volunteers were studied with susceptibility-weighted MR imaging before and after intravenous administration of 1000 mg of acetazolamide. Signal intensities were measured as a function of time in several regions of interest defined on anatomic images. In all patients with chronic occlusive cerebrovascular disease, acetazolamide challenge and resting regional cerebral blood flow were also evaluated with single-photon emission CT (SPECT)., Results: In healthy volunteers, signal intensities began to increase 3 to 4 minutes after acetazolamide administration, with a continuous increase during the subsequent 10 minutes. The effect lasted for approximately 45 minutes after administration. In patients with chronic occlusive cerebrovascular disease, signal changes on susceptibility-weighted MR images of occluded areas with normal vasodilatory capacity on SPECT images did not differ from signal changes of nonocclusive areas. In those patients with changes that reflected diminished vasodilatory capacity, the MR images showed a lower percentage of signal changes after acetazolamide administration than those in normally perfused areas., Conclusion: Susceptibility-weighted MR imaging offers an alternative method for estimating vasodilatory capacity.

Unilateral Moyamoya disease presents as unilateral stenosis or obstruction of the supraclinoid internal carotid artery, which causes cerebral hypoperfusion resulting in seizures or TIA-like attacks. In severe cases, surgical treatment is performed with superficial temporal artery-middle cerebral artery anastomosis. In mild cases, conservative management is the treatment of choice. Flunarizine is a calcium ion anti-blocking agent, whose primary effect is that the cerebral vessels have been used for the treatment of postcerebrovascular disorders. Recently, it has been suggested that flunarizine could be used to treat Moyamoya disease. This report documents the efficacy of flunarizine to improve regional cerebral perfusion in Moyamoya disease.

Unlabelled: The purpose of this study is to clarify the changes of hippocampal perfusion in neuropsychiatric diseases, including dementia, compared with control subjects, and to correlate hippocampal perfusion with the dementia rating scale and the severity of memory disturbance in patients with these diseases., Methods: A total of 45 right-handed patients were investigated (13 with dementia of Alzheimer type, 6 with multi-infarct dementia, 4 with progressive dementia with motor neuron disease (MND), 3 with transient global amnesia, 5 with other diseases and 14 control subjects). Regional cerebral blood flow (rCBF) in the parietal cortex and hippocampus was evaluated by high-resolution SPECT technique with HMPAO in all subjects., Results: The rCBF measurements in the bilateral parietal cortices and hippocampus were lower in dementia of Alzheimer type and multi-infarct dementia patients than in controls. Hypoperfusion in the hippocampus was a more sensitive marker than hypoperfusion in the parietal cortex in diagnosing dementia of Alzheimer type. Hippocampal hypoperfusion was observed in demented patients regardless of etiology and in patients having memory disturbance without dementia, such as transient global amnesia. Finally, hippocampal hypoperfusion reflected the severity of dementia and memory disturbance regardless of etiology., Conclusion: The rCBF image with high-resolution SPECT system may be useful in assessing the extent of dementia and memory disturbance in patients with neuropsychiatric diseases.

Unlabelled: The aim of this study was to review the etiology of CCD and study factors that affect the development and manifestation of CCD., Methods: Three hundred and eleven patients with supratentorial lesions were evaluated for the presence of CCD with SPECT and 123I-IMP. In representative cases, continuous arterial blood sampling was done and rCBF was calculated using Kuhl's method., Results: IMP-SPECT detected an abnormality in 206 patients, of whom 30 had CCD. Of CCD patients, 27 had more than single lobe involvement, 17 had motor impairment, 8 of 11 had rCBF of less than 29.1 +/- 10.9 ml/100 g/min. There was also a significant difference in rCBF between non-CCD and CCD cases., Conclusion: Although CCD can also occur with dementia (mixed or vascular type), it is more common with multilobar lesions. It is also associated with the presence of motor impairment but not related to its severity. It is more likely to develop, however, if rCBF is less than 29.1 +/- 10.9 ml/100 g/min regardless of etiology.

Unlabelled: We studied the usefulness of IMP SPECT with acetazolamide in 16 patients with moyamoya disease. Cerebral angiography was performed for all patients who were classified in three grades according to their angiographic stages., Methods: Techniques used included ring-type emission computed tomography with a minicomputer system. Patients received 111 MBq of 123I-IMP and SPECT images were obtained 20 min postinjection. Nine patients were studied using iodoamphetamine (IMP) SPECT with and without acetazolamide. IMP SPECT with acetazolamide was performed 20 min after each injection of 1 g of acetazolamide., Results: Low perfusion areas in the upper and lower frontal, parietal and temporal regions in grades 2 and 3 using IMP SPECT were observed. The mean cerebral-to-cerebellar activity ratios (C/C ratio) of six regions (upper and lower frontal, temporal, parietal occipital and basal ganglia) in grades 1, 2 and 3 were 0.96 to 1.06, 0.91 to 0.96 and 0.76 to 0.88, respectively., Conclusion: Our results indicate that measurement of regional cerebral blood flow (rCBF) elucidates cerebral hemodynamic factors, including the reactivity of cerebral vessels which cannot be detected angiographically in patients with moyamoya disease, and that the acetazolamide test is useful for detecting cerebral blood flow reserve. The test can be used to detect disease progress prospectively.

Early and delayed 201Tl SPECT studies were performed on 13 patients with intracranial meningiomas, which were classified in three groups according to their histological types: meningothelial (n = 7); transitional and fibroblastic (n = 3); and malignant types (n = 3). The early uptake indices (UI, ratios of average counts/pixel in the lesion to those of the contralateral area on early images) were relatively high in all types: meningothelial meningiomas, 5.75 +/- 2.16 (mean +/- s.d.); transitional and fibroblastic meningiomas, 4.69 +/- 0.54; and malignant meningiomas, 7.10 +/- 3.72. There were no statistical differences in relation to histological type. The delayed uptake indices were 2.65 +/- 0.89, 3.37 +/- 1.02, and 5.16 +/- 1.62, respectively. Statistically, the delayed UI of meningothelial meningiomas were lower than those of malignant types (p < 0.05). The retention indices (RI, ratios of delayed to early UI) were 0.48 +/- 0.08, 0.79 +/- 0.16, and 0.84 +/- 0.16, respectively. The RI of the meningothelial type were also statistically lower than those of the other two groups (p < 0.05). There were no statistically significant differences between transitional plus fibroblastic types and malignant meningiomas. We conclude that 201Tl exhibits early high accumulations in all types of meningiomas, but its retention rates probably differ according to histological types, and a high retention index is predictive of the malignant potential in a meningioma.

Purpose: To compare MR with thallium-201 scintigraphy in the follow-up of patients who have undergone thyroidectomy and modified radical neck dissection for differentiated thyroid cancer., Methods: Both MR imaging and 201Tl scintigraphy were performed in 39 patients after surgery for differentiated thyroid cancer. Ten patients did not have recurrence; 29 patients had 51 recurrent tumors in the neck and mediastinum., Results: Among 51 tumor sites, 39 sites of recurrence were detected by MR and 24 were detected by 201Tl scintigraphy. The true-positive rate, false-negative rate, true-negative rate, and false-positive rate in detecting recurrent thyroid cancer were 76.5, 23.5, 100, and 0%, respectively, for MR and 47.1, 52.9, 100, and 0%, respectively, for 201Tl scintigraphy. MR was more sensitive than 201Tl scintigraphy in detecting recurrent tumors (especially small metastatic nodes). On T2-weighted MR images, recurrent tumor was characterized by high signal intensity. Scarring in the postoperative thyroid bed was characterized by low signal intensity on both T1- and T2-weighted images. Abnormal accumulation of 201Tl in the postoperative thyroid bed was not observed., Conclusions: Although both MR images and 201Tl scintigraphy were effective in distinguishing recurrent tumor from postoperative fibrous tissue, MR images were more sensitive than 201Tl scintigraphy in detecting recurrent tumors. These results suggest that MR imaging is more useful than 201Tl scintigraphy in the follow-up evaluation of patients after neck surgery for differentiated thyroid cancer.

The authors describe two patients with thyroid lymphoma and Hashimoto thyroiditis; T1- and T2-weighted MR sequences were used. In one patient, the region of lymphoma showed a different signal intensity on T2 images; in the other patient, the signal level was identical to the Hashimoto disease. In sum, the processes could not be significantly differentiated using MR.

Regional cerebral blood flow was evaluated by single-photon emission CT (SPECT) with 123I-N-isopropyl-p-iodoamphetamine (123I-IMP) in 11 patients with dementia of the Alzheimer type, three patients with progressive dementia and motor neuron disease, and eight healthy control subjects. Regional blood flow measurements in the bilateral frontal, parietal association, and temporal cortices were lower in the Alzheimer dementia patients than in controls. Flow deficits in the parietal association cortex were demonstrated in all patients with Alzheimer-type dementia; these deficits were correlated with the severity of disease. Lateral hemispheric asymmetry was seen in nine of 11 patients with Alzheimer-type dementia. In all three patients with progressive dementia and motor neuron disease, flow deficits were demonstrated in the bilateral frontal and temporal cortices, but no flow deficits were seen in the parietal association cortex. Brain SPECT with 123I-IMP may be useful in the differential diagnosis and evaluation of the severity of degenerative dementia.

Two patients presenting with progressive dementia coupled with motor neuron disease underwent brain SPECT using N-isopropyl-p iodine-123-iodoamphetamine [( 123I]IMP). The characteristic clinical features of progressive dementia and motor neuron disease were noted. IMP SPECT also revealed reduced uptake in the bilateral frontal and temporal regions, with no reduction of uptake in the parietal, parietal-occipital regions. We conclude that IMP SPECT has potential for the evaluation of progressive dementia with motor neuron disease.

In the scintigraphic diagnosis of diffuse hepatocellular diseases, increased splenic uptake of colloids and splenomegaly are helpful signs. To quantify these patterns, the volume and activity of liver and spleen were measured by using single photon emission computed tomography (SPECT). The spleen-to-liver (S/L) ratios in volume and activity that were calculated from SPECT images were estimated in normal individuals and in patients with diffuse hepatocellular diseases. The maximum normal limits of S/L ratios of volume, activity, and activity/volume were predicted as 0.19, 0.087, and 0.72 (mean +/- 2 s.d.). Twenty-two of twenty-three patients (96%) with liver cirrhosis had at least two elevated S/L ratios, and three elevated S/L ratios could clearly differentiate the patient with liver cirrhosis from normal individuals. On the other hand, only six patients (32%) with chronic hepatitis had elevation of any S/L ratio. Abnormal S/L ratios of activity/volume in the range from 0.72 to 1.05 were not obvious on planar or SPECT images.

Regional cerebral blood flow (rCBF) was assessed in patients with cerebrovascular diseases (CVD) by single photon emission computed tomography (SPECT) using 133Xe and N-isopropyl-p-[123I]iodoamphetamine ([123I]IMP). The purpose of this study was to determine the infarcted and symptomatic blood flow thresholds of cerebral cortex by SPECT. In the 133Xe inhalation method rCBF was calculated by employing the Celsis modification of the Kanno and Lassen algorithm. In the [123I]IMP SPECT quantitation was obtained by counting-rate ratio for the low flow lesion to the contralateral homologous region which was presumed to be normal (Lesion/Normal ratio, L/N ratio). The infarcted and symptomatic blood flow thresholds by 133Xe SPECT was 19-23 and 33-36 ml/100 g/min, respectively. While, those of L/N ratio in the [123I]IMP SPECT were 39-48 and 65-72%, respectively. There was a significant correlation between the ischemic degrees evaluated by 133Xe and [123I]IMP SPECT studies.

Dynamic studies of single photon emission computed tomography (SPECT) were performed with intravenous administration of N-isopropyl-p-[123I]iodoamphetamine ([123I]IMP) in 12 patients with meningiomas. Their vascularities were also assessed by angiography. Regions of interest (ROIs) were placed over the lesion and the contralateral presumed normal area, the symmetrical one if possible. Time-activity curves (TACs) were then constructed and the counting-rate ratio for the lesion to the contralateral presumed normal area (L/N ratio) was calculated in the first image of the dynamic SPECT. TACs were classified into three types: type I, an initial high uptake and rapid washout type, type II, an initial high uptake and slow washout one, and type III, an initial low uptake one. L/N ratios of angioblastic meningiomas were over 300%, while those of many fibroblastic ones were under 100%, at most 105%. We compared L/N ratios with angiographic vascularities and histologic types and conclude that dynamic SPECT with [123I]IMP is the examination of choice for evaluating the vascularity of lesions.

Regional cerebral blood flow (rCBF) was measured by single photon emission computed tomography (SPECT) using 133Xe, N-isopropyl-p-[123I]iodoamphetamine ([123I]IMP) and [99mTc] hexamethylpropyleneamine oxime ([99mTc]HM-PAO) in 24 patients with cerebrovascular diseases. The greatest advantage of 133Xe SPECT was to be able to provide absolute rCBF values without arterial sampling. However, its image quality was very poor. Iodine-123 IMP SPECT provided rCBF images of higher quality and it had good correlation to 133Xe SPECT. Iodine-123 IMP SPECT provided the best images to detect mild ischemic lesions. It could detect obstructive or stenotic changes of large cerebral arteries very well except for a moderate stenosis of internal carotid artery. Technetium-99m HM-PAO SPECT also provided very good rCBF images and it had good correlation to 133Xe SPECT. However, the count-density ratios for the ischemic lesions to the contralateral presumed normal areas of [99mTc] HM-PAO SPECT were significantly higher than those of [123I]IMP SPECT.

Technetium-99m hexamethylpropyleneamine oxime ([99mTc]HM-PAO) has recently been introduced as an alternative to N-isopropyl-p-[123I]iodoamphetamine ([123I]IMP) for measurement of regional cerebral blood flow. This study compares dynamic SPECT studies using the two tracers in seven patients with meningiomas. Regions of interest were placed over the lesion and contralateral homologous presumed normal area. The counting-rate ratio for the lesion to the contralateral homologous area (L/N ratio) was then calculated in the first image. L/N ratios of [99mTc]HM-PAO single photon emission computed tomography (SPECT) were lower than those of [123I])IMP SPECT, particularly in hypervascular meningiomas. Furthermore, time-activity curves showed that the washout of [99mTc]HM-PAO in the tumors was very slow or incomplete, preventing an accurate assessment of vascularity of meningiomas with [99mTc]HM-PAO, as is generally possible with [123I]IMP.
