This retrospective study aimed to investigate the validity of a 30-sec chair stand test (CS-30) as a simple test to assess exercise tolerance and clinical outcomes in 53 Japanese patients with esophageal cancer. There was a strong correlation between the results of CS-30 and the 6-min walk test (6MWT), the gold standard for assessing exercise tolerance (r=0.759). Furthermore, fewer patients whose CS-30 score was greater than 16 (the cutoff value defined based on 6MWT) experienced pneumonia in their postoperative course. These results suggest that exercise tolerance could be assessed using CS-30, and its cutoff value may be useful in predicting postoperative pneumonia risk.

OBJECTIVE: To assess whether the phosphorylated Fas-associated death domain protein (FADD) at 194 serine (p-FADD) is valuable as a marker of biochemical recurrence in hormone-naive patients who had undergone radical prostatectomy. MATERIALS AND METHODS: We used radical prostatectomy specimens from 106 patients. None of the patients had received neoadjuvant or adjuvant therapy. The percentage of positive p-FADD cells (nuclear staining) was immunohistochemically evaluated. The correlation between FADD phosphorylation and the clinicopathologic parameters was assessed. The correlation between the biochemical recurrence-free rate and the p-FADD expression level was analyzed using the Kaplan-Meier method. RESULTS: Overall, 39 patients developed biochemical recurrence. We investigated the expression of p-FADD in 106 patients with prostate cancer using immunohistochemistry. We compared our findings with the clinicopathologic parameters, including the follow-up data. Patients with a greater positive p-FADD rate had a significantly lower biochemical recurrence rate than those with a lower positive p-FADD rate (P < .001). A significant inverse correlation was found between the positive p-FADD rate and the Gleason score. CONCLUSION: A low expression of p-FADD could be a predictor of biochemical recurrence in hormone-naive patients who have undergone radical prostatectomy., 博士（医学）・乙第1313号・平成25年5月29日

Speech transmission index (STI) is one of the objective measurements that assess speech transmission quality in the room acoustics. This paper proposed a method for blindly estimatingSTI based on the concept of the modulation transferfunction (MTF). In this method, STI can be estimated from the estimated MTFs in the seven octave-bands, by using our previous method for blindy estimating reverberation time (RT). Simulations were carried out to evaluate the proposed method, in realistic environments (SMILE2004 datasets). Results showed that the proposed method can effectively estimate both RT and STI from these reverberant environments.

泌尿器科手術3164件(男2633件, 女531件).5年毎にI～IV期に分けると, 第II期の929件が最も多く, 以後漸減していた.臓器別件数は後腹膜腔・副腎31件, 腎269件, 尿管271件, 膀胱756件, 前立腺820件, 尿道121件, 陰茎291件, 陰嚢内容343件, その他265件で, 主な疾患は悪性腫瘍893件, 前立腺肥大症767件, 尿路結石328件, 先天性奇形376件であった.尿管結石に対しては, 第I期は尿管切石術のみで, 第II期はTULが増加し, 第III期では体外衝撃波結石破砕術(ESWL)を保有する他施設への紹介により激減し, 第IV期にはESWLの導入でTUL症例も増加した.前立腺肥大症に対してはTUR-Pを第一選択としており, 最近減少傾向にあるが, 他の治療法の有効性には疑問があるため, 今後件数回復の見込みがある.陰茎の手術は殆どが包茎であるが, 件数は明らかに減少しており, 少子化や美容形成外科での施行の影響が考えられた, A clinical statistics survey of the operations was performed at the Department of Urology, Nissei Hospital during a 20-year period from 1982 to 2001. The total number of operations was 3, 164, and the number of extracorporeal shock wave lithotripsy procedures was 143. Operations for urogenital malignancies urolithiasis and prostatic hyperplasia numbered 893 (28.2%), 328 (10.4%), and 767 (24.2%), respectively. The total number of operations has been decreasing owing to the development of new diagnostic and therapeutic modalities.

症例は28歳男性で, 4ヵ月ほど前からの右側背部痛と持続的な微熱および右陰嚢内有痛性腫瘤を主訴に近医を受診, 右精巣上体炎の診断で入院となった.腹部CTで右腸腰筋に腫瘍を指摘され, 喀痰培養検査で結核菌陽性を示したため紹介来診し, 肺結核, 右結核性精巣上体炎および右腸腰筋冷膿瘍の診断で入院となった.エタンブトール, リファンピシン(REP), イソニアジド(INH)の3剤併用療法の施行では発熱が持続するためストレプトマイシン, ピラジナミド, REP, INHの4剤併用療法に変更した.さらに右腸腰筋膿瘍に超音波ガイド下で経皮的ドレナージを施行したところ症状が著明に改善した.炎症所見の低下傾向とCT上も膿瘍の消失を認めドレナージチューブを抜去したが, 約2ヵ月後に冷膿瘍再発を認め, 再度ドレナージを施行した.約5ヵ月後の腹部CTで右腸腰筋膿瘍の縮小, 瘢痕化を認め, ドレナージチューブを抜去した.その後も化学療法を継続し, 再燃を認めなかったため入院より約1年半後に退院となり, 外来にて経過観察中である, A 28-year-old man was referred to our hospital with a complaint of painful induration of right epididymis accompanied with right back pain and persistent low-grade fever. He was finally diagnosed with tuberculosis by sputum culture. Abdominal computed tomography (CT) revealed right psoas abscess and vertebral caries. He underwent a percutaneous drainage of the abscess followed by multidrug (streptomycin, pyrazinamide, refanpicin, isoniazide) combination therapy. Immediately after the drainage, symptoms began to improve with these therapies. However, four months later, abdominal CT showed a worsening of the abscess. Recently there is a stagnation in the decline of incidence of tuberculosis. It is still necessary to examine young people carefully bearing urogenital tuberculosis in mind. The pathogenesis and management of this rare condition are discussed.
