Bringing INnovation to onGOing water management – a better future under climate change. The following chapters provide an overview of the hydrological model results for past conditions at the six BINGO research sites in Cyprus, Germany, the Netherlands, Norway, Portugal, and Spain. Each model development/application description is reported per site and includes information on model objectives, model application, model description, data used for modelling, calibration and validation strategies, model results, and model evaluation and discussion.

Abstract

Bringing INnovation to onGOing water management – a better future under climate change. This report D3.6 provides an overview of the benefit of the BINGO modelling approach for 14 sites in 6 countries. Even though the sites are diverse, as they each tackle different water problems that are typical for Europe, the combination of bottom-up knowledge and data with top-down modelling strategies (climate change data, scenarios) has been successful. At the some of the sites, e.g. in the Netherlands and Germany, individual models had already been implemented, but have been further developed or joined by additional models within BINGO.

Abstract

Online citizen science projects have demonstrated their usefulness for research, however little is known about the potential benefits for volunteers. We conducted 39 interviews (28 volunteers, 11 researchers) to gain a greater understanding of volunteers' motivations, learning and creativity (MLC). In our MLC model we explain that participating and progressing in a project community provides volunteers with many indirect opportunities for learning and creativity. The more aspects that volunteers are involved in, the more likely they are to sustain their participation in the project. These results have implications for the design and management of online citizen science projects. It is important to provide users with tools to communicate in order to supporting social learning, community building and sharing.

Abstract

Deliverable 4.1 – “Context for risk assessment at the six research sites, including criteria to be used in risk assessment” was developed by LNEC, AQUALOGY, IWW, NTNU, CyI, KWR and InterSus under WP 4 ("Assessment of the impacts of extreme weather events"), within Task 4.1 - "Establishment of the context for Risk Management Process", led by LNEC. This deliverable aims at formulating the adaptation problems within BINGO and setting up a common and clear understanding of all important variables at each research site (context), taking into consideration the end user’s views for the choice of the criteria and methodologies to be used. This report will be the basis for the future development of the three steps of risk assessment that will be performed in WP4: risk identification (Task 4.2) and risk analysis and risk evaluation (Task 4.3), and provide important input and basic information about the research sites into the work done in WP5.

This deliverable evaluates the state of water resources (surface and groundwater, quantity and quality) at all six research sites. A consistent characterization of the catchments and surface, groundwater, and estuarine water bodies, including land use, pollution sources and anthropogenic water abstractions is included. It includes measured data, results from previous research, literature, knowledge. This deliverable serves as an overview of all six BINGO research sites. According to the comments and suggestions made by the external EC reviewer, this report has been updated on November 15th 2016.

Abstract

In the framework of lifelong and lifewide learning, training for teachers should be aimed to foster their positive attitude towards learning and learning to learn. A systemic, constructivist and auto/biographical approach was used in a course for teachers in mathematics and physics. To propitiate lifelong and lifewide learning, they were invited to taka an active and reflexive position towards their experience at school, teacher/student relationships, and concept of learning. The general aim was to develop a reflexive attitude and explicit ideas about knowledge, learning and the subject matter. Auto/biographical participatory inquiry was used as a method to learn through research, where learners were invited to develop an individual and collective satisfying theory about their choice to become a teacher, their struggles for identity, and the challenges of a difficult social and professional situation.

8. Guest Editorial: Special Issue on Emerging Health Telematics Applications in Europe

Abstract

THROUGHOUT the world, information and communi- cation technologies—telematics technologies—form the basis of a new area in the practice of medicine and the delivery of healthcare. It is characterized as a revolution based on information and the expression of human intelligence targeting to improve the efficiency and efficacy of healthcare delivery. Europe is actively participating in this revolution, both through national strategic plans as well as European initiatives, such as the research and development programs of the European Union (EU). The main objective of this Special Issue of the IEEE TRANSACTIONS ON INFORMATION TECHNOLOGY IN BIOMEDICINE is to provide a snapshot of current European activities in health telematics. A total of 14 papers are published in this Special Issue and the December 1998 Special Issue under the following topics: Electronic Healthcare Records, Collaborative Technologies in Health- care Delivery, Virtual Reality Applications in Medicine, and Healthcare Education . In this editorial, the papers are briefly introduced, together with a short reference on European past and current activities, outlining major challenges for wider implementation of health telematics applications and services in Europe, and the future trends. Rapid advances in information technology and telecommu- nications and their combination (telematics) are leading to the emergence of a new type of information infrastructure that has the potential of supporting an array of advanced services for healthcare. In Europe, during the last decade, information technologies have played a decisive role in the dramatic developments not only in diagnosis, treatment, surveillance, and rehabilitation of patients, but also on the side of the more collective aspects of healthcare and health prevention, such as clinical trials, epidemiology, and health education. In the constant competition for resources in the health sector, these new so-called telematics technologies and applications would not stand much chance if they were not intended to improve the quality of care, directly or indirectly, to help in the cost-containment and better management of the health sector, and to increase competitiveness of the health telematics industry. The case of quality improvement has long been proven by the older part of health telematics, medical informatics, which is the bread and butter of the clinical decision making process [1]. Among the many contributions of telematics applications to the quality of health delivery, we cannot overlook the new possibilities of fast access to vital patient data through sharable and secure electronic healthcare records (EHCR’s), the increasing use of telemedicine services, such as telecon- sultation and emergency telemedicine, and the advancement in medical imaging. In the case of cost containment, it is difficult to argue on a large scale since the telematics expenditure in healthcare and its cost effectiveness have not been properly monitored. There are many examples of hospital information systems that, with the combination of administrative and clinical data, provide managers with vital information leading to significant savings. For example, it has been shown that the use of electronic patient records in countries like the United Kingdom [2] and The Netherlands saves time and money for doctors as well as for healthcare authorities by receiving and processing data electronically. If the cost-reduction argument is substantiated on a large scale, it will be one of the strongest factors, together with the end-user acceptance, for the large deployment of telematics applications. The healthcare telematics industry is on the rise mostly in the developed world. While the overall percentage of the healthcare budget spent on information and telecom- munication technologies in Europe is relatively low in healthcare (approximately $400/employee compared to other sectors, such as manufacturing—$1500/employee—or finance—$5000/employee [3]), it is expected to grow due to the new policies and strategies of Member States or to the structural funds provided to some Member States for computerization of healthcare. In the United States, it is estimated that the growth of information technologies expenditure in the health sector will reach 15–20% per year [4]. The health telematics applications and services are the en- abling mechanism for the continuity of care through all stages of care delivery, namely, prevention, diagnoses, treatment, and rehabilitation, as well as continuity across all points of care, such as hospitals, primary care centers, rehabilitation centers, laboratories, pharmacies, homes, etc., center around the patient. This patient-centered care is a result of a growing trend for more effective management, a need for collaborative work of health professionals (shared care), and a greater involvement of patients or citizens in receiving information, decision making, and responsibility for their own health. The continuity of care scenario is supported by the European Union (EU) through its research and development (R&D) program in health telematics. In Section II, we briefly present the EU's past and present activities and R&D programs in the area of health telematics. In Section III, we outline the major difficulties toward the wider implementation and acceptance of health telematics applications and services. In Section IV, an overview of the papers published in this Special Issue is given. Section V introduces the future trends and plans of the next R&D program of EU, and Section VI covers the conclucions.

12. How LGBT+ Young People Use the Internet in Relation to Their Mental Health and Envisage the Use of e-Therapy: Exploratory Study.

Abstract

Background: Lesbian, gay, bisexual, and transgender (LGBT) youth and other young people diverse in terms of their sexuality and gender (LGBT+) are at an elevated risk of mental health problems such as depression. Factors such as isolation and stigma mean that accessing mental health services can be particularly challenging for LGBT+ young people, and previous studies have highlighted that many prefer to access psychological support on the Web. Research from New Zealand has demonstrated promising effectiveness and acceptability for an LGBT+ focused, serious game-based, computerized cognitive behavioral therapy program, Rainbow Smart, Positive, Active, Realistic, X-factor thoughts (SPARX). However, there has been limited research conducted in the area of electronic therapy (e-therapy) for LGBT+ people., Objective: This study aimed to explore how and why LGBT+ young people use the internet to support their mental health. This study also sought to explore LGBT+ young people's and professionals' views about e-therapies, drawing on the example of Rainbow SPARX., Methods: A total of 3 focus groups and 5 semistructured interviews were conducted with 21 LGBT+ young people (aged 15-22 years) and 6 professionals (4 health and social care practitioners and 2 National Health Service commissioners) in England and Wales. A general inductive approach was used to analyze data., Results: LGBT+ youth participants considered that the use of the internet was ubiquitous, and it was valuable for support and information. However, they also thought that internet use could be problematic, and they highlighted certain internet safety and personal security considerations. They drew on a range of gaming experiences and expectations to inform their feedback about Rainbow SPARX. Their responses focused on the need for this e-therapy program to be updated and refined. LGBT+ young people experienced challenges related to stigma and mistreatment, and they suggested that strategies addressing their common challenges should be included in e-therapy content. Professional study participants also emphasized the need to update and refine Rainbow SPARX. Moreover, professionals highlighted some of the issues associated with e-therapies needing to demonstrate effectiveness and challenges associated with health service commissioning processes., Conclusions: LGBT+ young people use the internet to obtain support and access information, including information related to their mental health. 13. Patient and public involvement in patient safety research: a workshop to review patient information, minimise psychological risk and inform research.

Plain English Summary: Patient safety is a growing research area. However, although patients and the public are increasingly involved in clinical research, there is little guidance on how best to involve patients in patient safety research. Here we focus on how patients can contribute to the design of patient safety research. We conducted a workshop with patients as part of a project exploring errors and safety in the delivery of intravenous medication (medication given via a vein). The workshop was designed to explore how best to engage with hospital inpatients about these issues, to generate research topics, and to inform researchers about patients' experiences. Nine patients participated, each of whom had previously received intravenous medication. Participants advised against using terms such as 'error'; they also advocated caution when using terms such as 'safety' when describing the study to patients as this may worry some who had not thought about these issues before. We received thorough and useful feedback on our patient information sheets to ensure they were clear and understandable to patients. Patients also shared rich experiences with us about their treatment, which emphasised the need to extend our research focus to include a wider range of factors affecting quality and safety., Abstract: Background Patient safety has attracted increasing attention in recent years. This paper explores patients' contributions to informing patient safety research at an early stage, within a project on intravenous infusion errors. Currently, there is little or no guidance on how best to involve patients and the wider public in shaping patient safety research, and indeed, whether such efforts are worthwhile. Method We ran a 3-hour workshop involving nine patients with experience of intravenous therapy in the hospital setting. The first part explored patients' experiences of intravenous therapy. We derived research questions from the resulting discussion through qualitative analysis. In the second part, patients were asked for feedback on patient information sheets considering both content and clarity, and on two potential approaches to framing our patient information: one that focused on research on safety and error, the other on quality improvement. Results The workshop led to a thorough review of how we should engage with patients. Importantly, there was a clear steer away from terms such as 'error' and 'safety' that could worry patients. The experiences that patients revealed were also richer than we had anticipated, revealing different conceptions of how patients related to their treatment and care, their role in safety and use of medical devices, the different levels of information they preferred, and broader factors impacting perceptions of their care. Conclusion Involving patients at an early stage in patient safety research can be of great value. Our workshop highlighted sensitivities around potentially worrying patients about risks that they might not have considered previously, and how to address these. Patient representatives also emphasised a need to expand the focus of patient safety research beyond clinicians and error, to include factors affecting perceptions of quality and safety for patients more broadly.

14. Exploring the Current Landscape of Intravenous Infusion Practices and Errors (ECLIPSE): protocol for a mixed-methods observational study.

Abstract

Introduction: Intravenous medication is essential for many hospital inpatients. However, providing intravenous therapy is complex and errors are common. 'Smart pumps' incorporating dose error reduction software have been widely advocated to reduce error. However, little is known about their effect on patient safety, how they are used or their likely impact. This study will explore the landscape of intravenous medication infusion practices and errors in English hospitals and how smart pumps may relate to the prevalence of medication administration errors., Methods and Analysis: This is a mixed-methods study involving an observational quantitative point prevalence study to determine the frequency and types of errors that occur in the infusion of intravenous medication, and qualitative interviews with hospital staff to better understand infusion practices and the contexts in which errors occur. The study will involve 5 clinical areas (critical care, general medicine, general surgery, paediatrics and oncology), across 14 purposively sampled acute hospitals and 2 paediatric hospitals to cover a range of intravenous infusion practices. Data collectors will compare each infusion running at the time of data collection against the patient's medication orders to identify any discrepancies. The potential clinical importance of errors will be assessed. Quantitative data will be analysed descriptively; interviews will be analysed using thematic analysis., Ethics and Dissemination: Ethical approval has been obtained from an NHS Research Ethics Committee (14/SC/0290); local approvals will be sought from each participating organisation. Findings will be published in peer-reviewed journals and presented at conferences for academic and health professional audiences. Results will also be fed back to participating organisations to inform local policy, training and procurement.
