"The purpose of this study was to examine the impact of interprofessional student-team home visits on the health outcomes of super-utilizers of the health care system. The cost of health care continues to increase at an unsustainable pace. Innovative educational approaches integrated with existing models of care are an example of a potential strategy aimed at reducing costs and improving outcomes. The model implemented in this study is referred to as hotspotting, an emerging term that describes an intervention during which super-utilizers are the focus of the intervention. Teams of interprofessional health professions students performed home visits on these patients in conjunction with the patients being in the care of one home health agency. This study analyzed 30-day hospital readmission rates and emergency department visits of the super-utilizers. There were twenty patients in the intervention group and twenty patients in the control group. Data analysis revealed a statistically significant difference in 30-day readmission rates between the two groups, with the group receiving interprofessional student-team home visits having fewer 30-day hospital readmissions. The number of emergency department visits were too small to warrant analysis. Home health agencies may find hotspotting an effective intervention to decrease costs and improve patient outcomes."--Abstract from author supplied metadata

"The State of Kansas is changing to a new functional eligibility and client assessment, referral, and evaluation (CARE) tool. The new tool is called the Medicaid Functional Eligibility Instrument-Level of Care (MFEI-LOC). This tool will replace the Functional Assessment Instrument (FAI) used in some Medicaid Long Term Services and Supports (LTSS) programs and CARE assessments used for nursing facility applicants. The MFEI-LOC will be used to determine functional eligibility for LTSS to meet federal Medicaid law eligibility requirements."

"The purposes of the Client Assessment, Referral, and Evaluation (CARE) Program is for data collection, individual assessment, referral to community based services, and appropriate placement in long-term care facilities. ... In order to be compliant with Section 1919(e)(7) of the Social Security Act, all individuals admitting to a Medicaid-certified nursing home need to have a valid proof of PASRR (pre-admission screening and resident review). ... The purpose of PASRR is to determine whether an individual with mental illness, mental retardation, or other developmental disability needs nursing facility services, or specialized mental health or mental retardation services. In Kansas, the PASRR assessment is the CARE assessment."

For many years Ohio's system of long-term services and supports was criticized for having an institutional bias. In 1993, more than nine of ten older people (60 and over) with a severe disability who received Medicaid long-term services did so in a nursing home. With one of the largest aging populations in the United States and growing and unsustainable Medicaid expenditures, state policy makers charted a course to change Ohio's system. This longitudinal study, initiated by the General Assembly in 1993 and continuously funded by the Ohio Department of Aging, tracks how long-term utilization has changed over the past two decades. Recent data indicate that the ratio of institutional care to home and community-based services for older people with severe disability using Medicaid has now changed to 55/45. Over the last 15 years, while the age 85 and older population grew by 50%, Ohio decreased nursing home use for older people by 11%. During this same 15-year time period, the state increased the number of older people receiving home and community-based services by 150%, while holding the Medicaid long-term services budget relatively constant (7% increase over 15 years, in 2011 dollars).

As part of the 2010/2011 budget, the General Assembly asking the Ohio Department of Aging (ODA) to create a diversion and transition program that would target 2300 individuals for diversion from long-stay nursing home placement and transition from nursing homes. The new program was specifically designed to achieve two distinct goals: to prevent unnecessary long-term nursing facility placement (diversion), and to provide community-based alternatives for long-stay nursing home residents who preferred and were able to live in a community setting (transition).
