This systematic review and meta-analysis aimed to evaluate the complications of lung biopsy in patients with acute respiratory failure (ARF), including acute respiratory distress syndrome (ARDS).We searched the MEDLINE and Cochrane Central Register of Controlled Trials. The primary outcomes were biopsy-related death, respiratory failure, cardiac complications, bleeding, and other major complications. We used the McMaster Quality Assessment Scale of Harms (McHarm) to evaluate the risk of bias. A random-effects model was used to calculate the pooled frequencies.Thirteen studies (consisting of 574 patients) were included in the meta-analysis. Furthermore, most of the included studies had a high or unclear risk of bias in half of the items in McHarm. All included studies evaluated surgical lung biopsies. The median overall hospital mortality was 53% (range: 17%-90%). The pooled frequencies of biopsy-related death, respiratory failure, cardiac complication, bleeding, and other major complications were 0.00% (95% confidence interval [CI]: 0.00%-0.21%), 1.30% (95% CI: 0.00%-5.69%), 1.03% (95% CI: 0.00%-3.73%), 1.46% (95% CI: 0.16%-3.56%), and 4.26% (95% CI: 0.00%-13.0%), respectively.The results of this study will be valuable information in considering the indications of lung biopsy in patients with ARF, including ARDS.The protocol was registered with the University Hospital Medical Information Network Clinical Trials Registry (UMIN 000040650).

Controlling the reactivity of high-valent Fe(IV)-O catalytic intermediates, Compounds I and II, generated in heme enzymes upon reaction with dioxygen or hydrogen peroxide, is important for function. It has been hypothesized that the presence (wet) or absence (dry) of distal heme pocket water molecules can influence whether Compound I undergoes sequential one-electron additions or a concerted two-electron reduction. To test this hypothesis, we investigate the role of water in the heme distal pocket of a dye-decolorizing peroxidase utilizing a combination of serial femtosecond crystallography and rapid kinetic studies. In a dry distal heme site, Compound I reduction proceeds through a mechanism in which Compound II concentration is low. This reaction shows a strong deuterium isotope effect, indicating that reduction is coupled to proton uptake. The resulting protonated Compound II (Fe(IV)-OH) rapidly reduces to the ferric state, giving the appearance of a two-electron transfer process. In a wet site, reduction of Compound I is faster, has no deuterium effect, and yields highly populated Compound II, which is subsequently reduced to the ferric form. This work provides a definitive experimental test of the hypothesis advanced in the literature that relates sequential or concerted electron transfer to Compound I in wet or dry distal heme sites.

Background The joint committee of the Japanese Society of Intensive Care Medicine/Japanese Respiratory Society/Japanese Society of Respiratory Care Medicine on ARDS Clinical Practice Guideline has created and released the ARDS Clinical Practice Guideline 2021. Methods The 2016 edition of the Clinical Practice Guideline covered clinical questions (CQs) that targeted only adults, but the present guideline includes 15 CQs for children in addition to 46 CQs for adults. As with the previous edition, we used a systematic review method with the Grading of Recommendations Assessment Development and Evaluation (GRADE) system as well as a degree of recommendation determination method. We also conducted systematic reviews that used meta-analyses of diagnostic accuracy and network meta-analyses as a new method. Results Recommendations for adult patients with ARDS are described: we suggest against using serum C-reactive protein and procalcitonin levels to identify bacterial pneumonia as the underlying disease (GRADE 2D); we recommend limiting tidal volume to 4–8 mL/kg for mechanical ventilation (GRADE 1D); we recommend against managements targeting an excessively low SpO2 (PaO2) (GRADE 2D); we suggest against using transpulmonary pressure as a routine basis in positive end-expiratory pressure settings (GRADE 2B); we suggest implementing extracorporeal membrane oxygenation for those with severe ARDS (GRADE 2B); we suggest against using high-dose steroids (GRADE 2C); and we recommend using low-dose steroids (GRADE 1B). The recommendations for pediatric patients with ARDS are as follows: we suggest against using non-invasive respiratory support (non-invasive positive pressure ventilation/high-flow nasal cannula oxygen therapy) (GRADE 2D), we suggest placing pediatric patients with moderate ARDS in the prone position (GRADE 2D), we suggest against routinely implementing NO inhalation therapy (GRADE 2C), and we suggest against implementing daily sedation interruption for pediatric patients with respiratory failure (GRADE 2D). Conclusions This article is a translated summary of the full version of the ARDS Clinical Practice Guideline 2021 published in Japanese (URL: https://www.jsicm.org/publication/guideline.html). The 15th edition of the Workshop on Recent Issues in Bioanalysis (15th WRIB) was held on 27 September to 1 October 2021. Even with a last-minute move from in-person to virtual, an overwhelmingly high number of nearly 900 professionals representing pharma and biotech companies, contract research organizations (CROs), and multiple regulatory agencies still eagerly convened to actively discuss the most current topics of interest in bioanalysis. The 15th WRIB included 3 Main Workshops and 7 Specialized Workshops that together spanned 1 week in order to allow exhaustive and thorough coverage of all major issues in bioanalysis, biomarkers, immunogenicity, gene therapy, cell therapy and vaccines. Moreover, in-depth workshops on biomarker assay development and validation (BAV) (focused on clarifying the confusion created by the increased use of the term “Context of Use – COU”); mass spectrometry of proteins (therapeutic, biomarker and transgene); state-of-the-art cytometry innovation and validation; and, critical reagent and positive control generation were the special features of the 15th edition. This 2021 White Paper encompasses recommendations emerging from the extensive discussions held during the workshop, and is aimed to provide the bioanalytical community with key information and practical solutions on topics and issues addressed, in an effort to enable advances in scientific excellence, improved quality and better regulatory compliance. Due to its length, the 2021 edition of this comprehensive White Paper has been divided into three parts for editorial reasons. This publication (Part 1A) covers the recommendations on Endogenous Compounds, Small Molecules, Complex Methods, Regulated Mass Spec of Large Molecules, Small Molecule, PoC. Tunable high-refractive-index nanostructures are highly desired for realizing photonic devices with a compact footprint. By harnessing the large thermo-optic effect in silicon, we show reversible and wide thermal tuning of both the far- and near-fields of Mie resonances in isolated silicon nanospheres in the visible range. We perform in situ heating in a transmission electron microscope and electron energy-loss spectroscopy to show that the Mie resonances exhibit large spectral shifts upon heating. We leverage the spectral shifts to demonstrate near-field tuning between different Mie resonances. By combining electron energy-loss spectroscopy with energy-dispersive X-ray analysis, we show a reversible and stable operation of single silicon nanospheres up to a temperature of 1073 K. Our results demonstrate that thermal actuation offers dynamic near-field tuning of Mie resonances, which may open up applications in tunable nonlinear optics, Raman scattering, and light emission.

Resonant optical nanomaterials with a high refractive index, such as silicon, have become key elements for controlling free-space light. Here, we show that silicon nanoparticles can manipulate highly confined guided waves in the form of surface plasmon polaritons (SPPs) on a subwavelength scale. Using electron energy-loss spectroscopy in a transmission electron microscope, we demonstrate that SPPs in ultrathin metal films can be efficiently launched due to the strong coupling between the Mie resonances of the nanoparticle and the SPP modes. We find that the SPP excitation wavelength can be tuned across the entire near-infrared by varying the particle size. For insight into the coupling mechanism, we also measure the electron-beam-induced response of the Mie resonances in isolated silicon nanostructures in a broad size range. Finally, we show that the silicon nanoparticles act as scatterers of the SPPs supported by the film. Our results may pave the way for using high-refractive-index dielectric nanoantennas as compact elements for manipulating highly confined SPPs.

Human cytochromes P45011β (CYP11B1) and P450aldo (CYP11B2) are monooxygenases that synthesize cortisol through steroid 11β-hydroxylation and aldosterone through a three-step process comprising 11β-hydroxylation and two 18-hydroxylations, respectively. CYP11B1 also catalyzes 18-monohydroxylation and 11β,18-dihydroxylation. To study the molecular basis of such catalytic divergence of the two enzymes, we examined a CYP11B1 mutant (Mt-CYP11B1) with amino acid replacements on the distal surface by determining the catalytic activities and crystal structure in the metyrapone-bound form at 1.4-Å resolution. Mt-CY11B1 retained both 11β-hydroxylase and 18-hydroxylase activities of the wild type (Wt-CYP11B1) but lacked 11β,18-dihydroxylase activity. Comparisons of the crystal structure of Mt-CYP11B1 to those of Wt-CYP11B1 and CYP11B2 that were already reported show that the mutation reduced the innermost space putatively surrounding the C3 side of substrate 11-deoxycorticosterone (DOC) bound to Wt-CYP11B1, while the corresponding space in CYP11B2 is enlarged markedly and accessible to bulk water through a channel. Molecular dynamics simulations of their DOC-bound forms supported the above findings and revealed that the enlarged space of CYP11B2 had a hydrogen bonding network involving water molecules that position DOC. Thus, upon positioning 11β-hydroxysteroid for 18-hydroxylation in their substrate-binding sites, steric hindrance could occur more strongly in Mt-CYP11B1 than in Wt-CYP11B1 but less in CYP11B2. Our investigation employing Mt-CYP11B1 sheds light on the divergence in structure and function between CYP11B1 and CYP11B2 and suggests that CYP11B1 with spatially-restricted substrate-binding site serves as 11β-hydroxylase, while CYP11B2 with spatially-extended substrate-binding site successively processes additional 18-hydroxylations to produce aldosterone., Graphical abstract Image 1, Highlights • CYP11B1 and CYP11B2 catalyze steroid hydroxylation for syntheses of cortisol and aldosterone, respectively. • Structural basis for their differences in ability of multiple hydroxylations remains unclear. • A CYP11B1 mutant generated is characterized by monohydroxylase activities. • X-ray crystallography and molecular dynamics simulation reveal spatial restriction in substrate-binding site of the mutant. • Spatial dimension of the substrate-binding sites is crucial for differential production of gluco- and mineralocorticoids.

The interaction between negatively charged all-inorganic silicon quantum dots (Si QDs) and bovine serum albumin (BSA) is studied. It is shown that a small difference in the size of Si QDs affects the structure of Si QD–BSA composites significantly. When the diameter of Si QDs is 4 nm, a heterodimer (~20 nm) composed of one Si QD and one BSA molecule is a preferable and stable structure. On the other hand, when the diameter is 7 nm, the size of the composites increases to ~50 nm. The Si QD–BSA composites exhibit stable photoluminescence in the near-infrared range in phosphate-buffered saline.

Obtaining structures of intact redox states of metal centers derived from zero dose X‐ray crystallography can advance our mechanistic understanding of metalloenzymes. In dye‐decolorising heme peroxidases (DyPs), controversy exists regarding the mechanistic role of the distal heme residues aspartate and arginine in the heterolysis of peroxide to form the catalytic intermediate compound I (FeIV=O and a porphyrin cation radical). Using serial femtosecond X‐ray crystallography (SFX), we have determined the pristine structures of the FeIII and FeIV=O redox states of a B‐type DyP. These structures reveal a water‐free distal heme site that, together with the presence of an asparagine, imply the use of the distal arginine as a catalytic base. A combination of mutagenesis and kinetic studies corroborate such a role. Our SFX approach thus provides unique insight into how the distal heme site of DyPs can be tuned to select aspartate or arginine for the rate enhancement of peroxide heterolysis., Femtosecond X‐ray crystallography reveals the pristine structures of the FeIII and FeIV=O redox states of a B‐type dye‐decolorising heme peroxidase. We developed a self-limited self-assembly process to produce red-to-near-infrared luminescent supraparticles made from biocompatible silicon (Si) quantum dots (QDs) for fluorescence bioimaging. A starting material is a methanol solution of boron (B) and phosphorus (P) codoped all-inorganic Si QDs. The Si QDs have a heavily B and P codoped amorphous shell, and the shell induces negative potential on the surface, which prevents agglomeration of QDs in polar solvents. By adding toluene to the methanol solution, controlled agglomeration of Si QDs occurs and spherical supraparticles around 100 nm in diameter with a narrow size distribution are grown. The average diameter of supraparticles was controlled by the growth parameters. We also developed a process to stabilize the supraparticles by coating the surface by polyvinylpyrrolidone (PVP) and then by silica. The photoluminescence spectra of PVP- and silica-coated Si QD supraparticles were very similar to those of Si QDs dispersed in solution.

Inorganic pyrophosphatase (PPase) catalyses the hydrolysis reaction of inorganic pyrophosphate to phosphates. Our previous studies showed that manganese (Mn) activated PPase from the psychrophilic bacterium Shewanella sp. AS-11 (Mn-Sh-PPase) has a characteristic temperature dependence of the activity with an optimum at 5 °C. Here we report the X-ray crystallography and electron paramagnetic resonance (EPR) spectroscopy structural analyses of Sh-PPase in the absence and presence of substrate analogues. We successfully determined the crystal structure of Mn-Sh-PPase without substrate and Mg-activated Sh-PPase (Mg-Sh-PPase) complexed with substrate analogue (imidodiphosphate; PNP). Crystallographic studies revealed a bridged water placed at a distance from the di-Mn centre in Mn-Sh-PPase without substrate. The water came closer to the metal centre when PNP bound. EPR analysis of Mn-Sh-PPase without substrate revealed considerably weak exchange coupling, whose magnitude was increased by binding of substrate analogues. The data indicate that the bridged molecule has weak bonds with the di-Mn centre, which suggests a ‘loose’ structure, whereas it comes closer to di-Mn centre by substrate binding, which suggests a ‘well-tuned’ structure for catalysis. Thus, we propose that Sh-PPase can rearrange the active site and that the ‘loose’ structure plays an important role in the cold adaptation mechanism.

Gel electrophoresis, which is a standard method for separation and analysis of macromolecules such as DNA, RNA and proteins, is applied for the first time to silicon (Si) quantum dots (QDs) for size separation. In the Si QDs studied, boron (B) and phosphorus (P) are simultaneously doped. Codoping induces a negative potential on the surface of a Si QD and makes it dispersible in water. Si QDs with different B and P concentrations and grown at different temperatures (950 °C-1200 °C) are studied. It is shown that native polyacrylamide gel electrophoresis can separate codoped Si QDs by size. The capability of gel electrophoresis to immobilize size-separated QDs in a solid matrix makes detailed analyses of size-purified Si QDs possible. For example, the photoluminescence (PL) studies of the dried gel of Si QDs grown at 1100 °C demonstrate that a PL spectrum of a Si QD solution with the PL maximum around 1.4 eV can be separated into more than 15 spectra with the PL maximum changing from 1.2 to 1.8 eV depending on the migration distance. It is found that the relationship between the PL peak energy and the migration distance depends on the growth temperature of Si QDs as well as the B and P concentration. For all the samples with different impurity concentrations and grown at different temperatures, a clear trend is observed in the relationship between the full width at half maximum (FWHM) and the peak energy of the PL spectra in a wide energy range. The FWHM increases with the increasing peak energy and it is nearly twice larger than those observed for undoped Si QDs. The large PL FWHM of codoped Si QDs suggests that excitons are further localized in codoped Si QDs due to the existence of charged impurities.

Tetraphenylporphyrin (TPP) is a synthetic porphyrin whose properties can be readily modified, endowing it with significant benefits over naturally occurring porphyrins. Yet, their insolubility in water and/or steric bulk have rendered them incompatible with biological systems. Herein, we report the first example of a native biomolecule capturing TPP as well as its derivatives. The haemoprotein HasA, secreted by certain pathogens to scavenge haem from their hosts, can capture various metal- and meso-substituted TPPs. The rapid crystallisation of TPP derivatives captured by HasA revealed the binding mode of TPP at excellent resolutions. A single-site mutation (L85A) of HasA enlarged the binding pocket, allowing the incorporation of a bulkier derivative of TPP. HasA binding TPP derivatives was also demonstrated to inhibit proliferation of the opportunistic pathogen Pseudomonas aeruginosa. This study not only represents a simple method for the complexation of TPP derivatives with a native protein, but also opens the door for the future use of TPP derivatives as biological tools.

In the field of drug development, technology for producing human metabolites at a low cost is required. In this study, we explored the possibility of using prokaryotic water-soluble cytochrome P450 (CYP) to produce human metabolites. Streptomyces griseolus CYP105A1 metabolizes various non-steroidal anti-inflammatory drugs (NSAIDs), including diclofenac, mefenamic acid, flufenamic acid, tolfenamic acid, meclofenamic acid, and ibuprofen. CYP105A1 showed 4'-hydroxylation activity towards diclofenac, mefenamic acid, flufenamic acid, tolfenamic acid, and meclofenamic acid. It should be noted that this reaction specificity was similar to that of human CYP2C9. In the case of mefenamic acid, another metabolite, 3'-hydroxymethyl mefenamic acid, was detected as a major metabolite. Substitution of Arg at position 73 with Ala in CYP105A1 dramatically reduced the hydroxylation activity toward diclofenac, flufenamic acid, and ibuprofen, indicating that Arg73 is essential for the hydroxylation of these substrates. In contrast, substitution of Arg84 with Ala remarkably increased the hydroxylation activity towards diclofenac, mefenamic acid, and flufenamic acid. Recombinant Rhodococcus erythrocyte cells expressing the CYP105A1 variant R84A/M239A showed complete conversion of diclofenac into 4'-hydroxydiclofenac. These results suggest the usefulness of recombinant R. erythropolis cells expressing actinomycete CYP, such as CYP105A1, for the production of human drug metabolites.

An 84-year-old Japanese woman presented to our hospital with a month-long dry cough during the coronavirus disease 2019 (COVID-19) pandemic. She also had skin lesions on her fingers from 3 months prior. A chest computed tomography (CT) scan showed bilateral ground- glass opacities with a subpleural distribution, similar to the findings of COVID-19. The results of COVID-19 tests were negative. The titer of the anti-melanoma differentiation-associated gene 5 (MDA5) antibody was elevated. Consequently, we confirmed the diagnosis of clinically amyopathic dermatomyositis (CADM) and then administered oral prednisolone combined with tacrolimus. After the treatment, her symptoms, skin lesions and CT findings were gradually resolved.

A stand-alone plasmonic nanocomposite into which a metal nanostructure and an emitting material are integrated is a promising building block for optoelectronics and biophotonics devices. Here, we present the plasmonic property of a nanocomposite composed of a Au elongated nanocap and a β-NaYF4 dielectric nanorod. We show that elongation of a Au nanocap results in splitting of the magnetic dipole resonance, and the resonance wavelengths can be controlled in a wide wavelength range by the aspect ratio. As an application of the elongated nanocap, we demonstrate strong enhancement of the near-infrared to visible upconversion of an Er3+ and Yb3+ doped β-NaYF4 nanorod by tuning the resonance wavelength of a Au nanocap placed on it to the excitation wavelength.

The ability to rapidly obtain structures of protein–ligand complexes using X-ray crystallography is central to drug discovery, but the typical cryocooling of samples and the effects of the X-ray beam may distort the observed ligand binding. X-ray free-electron lasers (XFELs) have the promise to solve these issues, but methods to rapidly produce structures of protein–ligand complexes at XFELs have not yet been realized. Here, an efficient solution using high-throughput, fixed-target serial femtosecond crystallography at an XFEL is demonstrated., High-throughput X-ray crystal structures of protein–ligand complexes are critical to pharmaceutical drug development. However, cryocooling of crystals and X-ray radiation damage may distort the observed ligand binding. Serial femtosecond crystallography (SFX) using X-ray free-electron lasers (XFELs) can produce radiation-damage-free room-temperature structures. Ligand-binding studies using SFX have received only modest attention, partly owing to limited beamtime availability and the large quantity of sample that is required per structure determination. Here, a high-throughput approach to determine room-temperature damage-free structures with excellent sample and time efficiency is demonstrated, allowing complexes to be characterized rapidly and without prohibitive sample requirements. This yields high-quality difference density maps allowing unambiguous ligand placement. Crucially, it is demonstrated that ligands similar in size or smaller than those used in fragment-based drug design may be clearly identified in data sets obtained from

Boron (B) and phosphorous (P) co-doped colloidal silicon nanocrystals (Si NCs) have unique size-dependent optical properties, which lead to potential applications in optoelectronic and biomedical applications. However, the microstructure of the B and P co-doped colloidal Si NCs – in particular, the exact location of the dopant atoms in real space, has not been studied. A lack of understanding of this underlying question limits our ability to better control sample fabrication, as well as our ability to further develop the optical properties. To study the microstructure, a process enabling atom probe tomography (APT) of colloidal Si NCs was developed. A dispersion of colloidal Si NCs in a SiO2 sol-gel solution and a low temperature curing are demonstrated as the key sample preparation steps. Our APT results demonstrate that a B-rich region exists at the surface of the Si NCs, while P atoms are distributed within the Si NCs. First principles density functional theory calculations of a Si NC embedded in SiO2 matrix reveal that P atoms, which always prefer to reside inside a Si NC, significantly influence the distribution of B atoms. Specifically, P atoms lower the B diffusion barrier at Si/SiO2 interface and stabilize B atoms to reside within individual Si NCs. We propose that the B-modified surface changes the chemical properties of the Si NCs by (i) offering chemical resistance to attack by HF and (ii) enabling dispersibility in solution without aggregation.

The antimicrobial activity of a hybrid nanoparticle (NP) composed of a silver (Ag) NP core decorated with silicon (Si) nanocrystals (NCs) on the exterior (Ag/Si NPs) is evaluated. The shell of Si NCs effectively protects the surface of Ag NPs, thus the particles are more stable in water and in air compared to conventional organic-capped Ag NPs. The bacterial growth kinetic analysis reveals that the Si NC shell does not suppress the release of Ag ions from the Ag NP surface due probably to the porous structure. For the antimicrobial coating application, a thin film of the hybrid Ag/Si NPs is produced by drop coating the solution on a cover glass. Thanks to the Si NC shell, agglomeration of Ag NPs in the film is prevented and the film shows a very similar optical absorption spectrum to that of the solution. The film exhibits a larger zone of inhibition in an agar diffusion assay of Escherichia coli compared to a film produced from organic-capped Ag NPs.
