The usual PR program for patients with severe COPD consisted of two 8-week sessions (PR session 1: PR1, PR session 2: PR2). The step-FB group was provided a program with step-FB added to PR2 (PR2+step-FB). Furthermore, all patients were evaluated at pre-intervention (baseline), PR1, and PR2. The primary outcome of this study was the number of daily steps (steps) and energy expenditure from activity (energy expenditure), as measured by a pedometer. The secondary outcomes were dyspnea and exercise tolerance.Results: In PR1, dyspnea, exercise tolerance, steps, and energy expenditure were significantly improved as compared to baseline, in both groups. During PR2, dyspnea and exercise tolerance were significantly improved as compared to PR1, in both groups. Steps and energy expenditure were significantly improved in the step-FB group, but not in the control group.Conclusion: PR improved PA by improving physical function in severe COPD patients. Adding step-FB improved PA in severe COPD patients by presenting an activity goal for improving PA. Therefore, pedometer-based step-FB is a viable addition to PR and has the potential to improve PA continuously in these patients.Keywords: dyspnea, exercise tolerance, monitoring, pulmonary rehabilitation, step counter

We investigated the hypothesis that meal consumption may reduce tongue strength and endurance in older adults (OAs).Methods: Tongue–palate pressure, oral diadochokinesis, repetitive saliva swallowing, and surface electromyography activity before and after a meal were measured in 23 young adults (YAs) and 23 OA volunteers.Results: There was a statistically significant difference in both tongue pressure and the number of voluntary swallows between YAs and OAs. Peak tongue pressure was significantly lower in OAs than YAs both before and after meal consumption. The most notable finding was that the first time interval (the time from test initiation to the beginning of the first swallow) was prolonged after meal consumption only in OAs, whereas the first time interval showed no difference between YAs and OAs before meal consumption with reference to the repetitive saliva swallowing test. The initiation of swallowing was prolonged by both meal consumption and aging; there was a significant interaction between these two factors. The number of repetitions of the monosyllable/pa/was statistically similar between YAs and OAs before meal consumption, but it was significantly lower in OAs after meal consumption.Conclusion: Aging leads to declining tongue pressure and motor function of the lips. It is possible that swallowing function declines in older individuals when meal consumption is prolonged, especially at the end of mealtime, as a result of their efforts in mastication and swallowing. Abstract

The Widom line identifies the locus in the phase diagram where a supercritical gas crosses over from gas-like to a more liquid-like behavior. A similar transition exists in correlated electron liquids, where the interplay of Coulomb repulsion, bandwidth and temperature triggers between the Mott insulating state and an incoherent conduction regime. Here we explore the electrodynamic response of three organic quantum spin liquids with different degrees of effective correlation, where the absence of magnetic order enables unique insight into the nature of the genuine Mott state down to the most relevant low-temperature region. Combining optical spectroscopy with pressure-dependent dc transport and theoretical calculations, we succeeded to construct a phase diagram valid for all Mott insulators on a quantitative scale. In the vicinity of the low-temperature phase boundary, we discover metallic fluctuations within the Mott gap, exhibiting enhanced absorption upon cooling that is not present in antiferromagnetic Mott insulators. Abstract

The Mott insulator $\kappa$-(BEDT-TTF)$_2$Ag$_2$(CN)$_3$ forms a highly-frustrated triangular lattice of $S=1/2$ dimers with a possible quantum-spin-liquid state. Our experimental and numerical studies reveal the emergence of a slight charge imbalance between crystallographically inequivalent sites, relaxor dielectric response and hopping dc transport. In a broader perspective we conclude that the universal properties of strongly-correlated charge-transfer salts with spin liquid state are an anion-supported valence band and cyanide-induced quasi-degenerate electronic configurations in the relaxed state. The generic low-energy excitations are caused by charged domain walls rather than by fluctuating electric dipoles. They give rise to glassy dynamics characteristic of dimerized Mott insulators, including the sibling compound $\kappa$-(BEDT-TTF)$_2$Cu$_2$(CN)$_3$., Comment: 5 pages, 6 figures, submitted to Physical Review B Rapid Communications

Abstract

The International Axion Observatory (IAXO) will be a forth generation axion helioscope. As its primary physics goal, IAXO will look for axions or axion-like particles (ALPs) originating in the Sun via the Primakoff conversion of the solar plasma photons. In terms of signal-to-noise ratio, IAXO will be about 4-5 orders of magnitude more sensitive than CAST, currently the most powerful axion helioscope, reaching sensitivity to axion-photon couplings down to a few $\times 10^{-12}$ GeV$^{-1}$ and thus probing a large fraction of the currently unexplored axion and ALP parameter space. IAXO will also be sensitive to solar axions produced by mechanisms mediated by the axion-electron coupling $g_{ae}$ with sensitivity $-$for the first time$-$ to values of $g_{ae}$ not previously excluded by astrophysics. With several other possible physics cases, IAXO has the potential to serve as a multi-purpose facility for generic axion and ALP research in the next decade. In this paper we present the conceptual design of IAXO, which follows the layout of an enhanced axion helioscope, based on a purpose-built 20m-long 8-coils toroidal superconducting magnet. All the eight 60cm-diameter magnet bores are equipped with focusing x-ray optics, able to focus the signal photons into $\sim 0.2$ cm$^2$ spots that are imaged by ultra-low-background Micromegas x-ray detectors. Abstract

The International Axion Observatory (IAXO) is a next generation axion helioscope aiming at a sensitivity to the axion-photon coupling of a few 10^{-12} GeV^{-1}, i.e. 1-1.5 orders of magnitude beyond sensitivities achieved by the currently most sensitive axion helioscope, the CERN Axion Solar Telescope (CAST). Crucial factors in improving the sensitivity for IAXO are the increase of the magnetic field volume together with the extensive use of x-ray focusing optics and low background detectors, innovations already successfully tested at CAST. Abstract

We investigate the dc resistivity and Hall effect of the quasi-two-dimensional organic materials κ–(ET)2X, where X=Ag2(CN)3 and B(CN)4 and compare them with the results for X=Cu2(CN)3. All three compounds are considered to be quantum-spin-disordered Mott insulators. Despite high similarities in chemical composition and crystal structure, large differences in the dc resistivity and Hall coefficient are found. While around room temperature the dc transport properties are dominantly determined by the strength of the electron correlations, upon reducing the temperature, dc transport happens by hopping due to inherent disorder. The most disordered compound with X=Cu2(CN)3 turns out to have the lowest dc resistivity and the highest charge carrier density, i.e., in the phase diagram it is located closest to the metal-insulator transition. The least disordered compound with X=B(CN)4 shows the highest resistivity and the lowest carrier density, i.e., lies farthest from the metal-insulator transition. We explain such counterintuitive behavior within the theory of Mott-Anderson localization as a consequence of disorder-induced localized states within the correlation gap.

Abstract

A 47-year-old woman with previous history of transvaginal uterectomy 4 years before, presented to another hospital complaining of pollakisuria and pain during micturition. She was treated with antibiotics, but symptoms failed to resolve. So she referred to our department for investigations and treatments. On cystoscopy, there was a large mass with edematous mucosa in the anterior wall of bladder. Magnetic resonance image demonstrated an 8 cm irregularly-formed cystic mass which occupied dome of bladder. Resection of the mass including partial cystectomy was performed. This cystic mass contained retained 3 silk sutures surrounded by green color pus. Microscopic examination revealed inflammatory granulations without any malignancy. Because of the previous history, she was diagnosed as paravesical suture abscess due to infected silk materials at transvaginal uterectomy. Paravesical suture abscess is very rare complication of inguinal herniorrhaphy and mimics bladder or urachal neoplasm. In review of previously reported cases, the symotoms were predominantly urological. For avoiding unnecessary examinations and treatments, it is important to consider paravesical suture abscess in cases with histories of inguinal herniorrhaphy or other intra-pelvic operations. To our knowledge, there is no case report of paravesical abscess formation associated with transvaginal uterectomy and the present case is the first one for report.

Abstract

Se propone estudiar la problemática de los pobladores del desierto del noreste de Mendoza, dedicados a la cría de caprinos, en el afán por interpretar y transformar la realidad de estos pobladores. Incluye metodologías interdisciplinarias de proyectos referidos a: profundización del conocimiento de la problemática socio-ambiental y de las necesidades y aspiraciones de los pobladores, cuantificación de la oferta forrajera y su incremento, posibilidades de revegetación con gramíneas peretines nativas, uso adecuado de los bosques de algarrobo, producción caprina diversificada, implementación de huertas familiares y la producción local de energia eléctrica, a partir de energía solar. Los pobladores viven en puestos aislados y por lo general carecen de energía eléctrica, agua potable y tecnologías apropiadas. Existen problemas de salud con características propias, entre ellos patologías orales que son evaluadas y atendidas para lograr la sustentabilidad de la salud bucal. Se contempla una participación interactiva, en la cual la comunidad comparte el análisis, las decisiones y el desarrollo de las acciones. Proposes study problematic of the populations of the desert of the northeast of Mendoza, dedicated to goat raising, in the anxiety for to interpret and to transform the reality of this populations. Includes methodologies interdisciplinaries of projects referred to: deepening the knowledge of the socio-environmental matter and of the needs and aspirations of the inhabitants; qnantification of the forage on offer and its increase, revegetation possibilities with perennial native grasses; adequate use of the Prosopis forests; diversified goat production; possibility of implementing family orchards and the local production of electricity from solar energy. These people live in isolated "puestos" and generally lack electricity, water potable and technologies appropriates. Thy also undergo health problems with their own characteristics, among them buccal pathologies that the assessed and taken cure of in order to attain buccal health sustainability. Abstract

The International Axion Observatory (IAXO) is a new generation axion helioscope aiming at a sensitivity to the axion-photon coupling of gaγ (≳) few × 10−12 GeV−1, i.e. 1–1.5 orders of magnitude beyond the one achieved by CAST, currently the most sensitive axion helioscope. The main elements of IAXO are an increased magnetic field volume together with extensive use of x-ray focusing optics and low background detectors, innovations already successfully tested in CAST. Additional physics cases of IAXO could include the detection of electron-coupled axions invoked to explain the white dwarf cooling, relic axions, and a large variety of more generic axion-like particles (ALPs) and other novel excitations at the low-energy frontier of elementary particle physics.

21. Breathlessness is associated with urinary incontinence in men: A community-based study

Abstract

Background: Urinary incontinence (UI) is a distressing problem for older people. To investigate the relationship between UI and respiratory symptoms among middle-aged and older men, a community-based study was conducted in Japan. Methods: A convenience sample of 668 community-dwelling men aged 40 years or above was recruited from middle and southern Japan. The International Consultation on Incontinence Questionnaire-Short Form, the Medical Research Council's dyspnoea scale and the Australian Lung Foundation's Feeling Short of Breath scale, were administered by face-to-face interviews to ascertain their UI status and respiratory symptoms. Results: The overall prevalence of UI was 7.6%, with urge-type leakage (59%) being most common among the 51 incontinent men. The presence of respiratory symptoms was significantly higher among incontinent men than those without the condition, especially for breathlessness (45% versus 30%, p = 0.025). The odds of UI for breathlessness was 2.11 (95% confidence interval 1.10-4.06) after accounting for age, body mass index, smoking and alcohol drinking status of each individual. Conclusions: The findings suggested a significant association between UI and breathlessness in middle-aged and older men.

24. Soy consumption and risk of COPD and respiratory symptoms: A case-control study in Japan

Abstract

Background: To investigate the relationship between soy consumption, COPD risk and the prevalence of respiratory symptoms, a case-control study was conducted in Japan. Methods: A total of 278 eligible patients (244 men and 34 women), aged 50–75 years with COPD diagnosed within the past four years, were referred by respiratory physicians, while 340 controls (272 men and 68 women) were recruited from the community. All participants underwent spirometric measurements of respiratory function. Information on demographics, lifestyle characteristics and habitual food consumption was obtained using a structured questionnaire. Results: Total soy consumption was positively correlated with observed lung function measures. The mean soy intake was significantly higher among controls (59.98, SD 50.23 g/day) than cases (44.84, SD 28.5 g/day). A significant reduction in COPD risk was evident for highest versus lowest quartile of daily intake of total soybean products, with adjusted odds ratio (OR) 0.392, 95% CI 0.194–0.793, p for trend 0.001. Similar decreases in COPD risk were associated with frequent and higher intake of soy foods such as tofu and bean sprouts, whereas respiratory symptoms were inversely associated with high consumption of soy foods, especially for breathlessness (OR 0.989, 95% CI 0.982–0.996). Conclusion: Increasing soy consumption was associated with a decreased risk of COPD and breathlessness.

Abstract

To examine effects of bestatin on the host immunity of patients with urogenital cancer, 54 patients were randomized into 2 groups: bestatin treated and controls. In each group, the patients were divided into 2 subgroups: one which received basic treatment expected to greatly affect host immunity ('invasive treatment') while the other one received other types of basic treatment ('non-invasive treatment'). Peripheral lymphocyte, OKT 4/8 ratio and purified protein derivative (PPD) skin reaction were used as immunological markers. There were significant differences in the 'invasive' treatment group between bestatin treated patients and controls concerning lymphocyte counts and PPD skin reactions and in the 'non-invasive' group concerning lymphocyte counts and OKT 4/8 ratios. These results suggest that bestatin may potentiate host immunity in patients with urogenital cancer. Further studies on larger materials are, however, needed before more definite conclusions can be drawn.
