Multiple mini interviews (MMIs) were designed in 2002 to assess such domains in prospective healthcare professions. Being a high-stake assessment, the utility and limitations of MMI need to be explored. The purpose of this article is to review the available evidence to establish its utility. The claim of the reliability is verified by the studies assessing the effect of number of stations, duration of stations, format and scoring systems of stations and number of raters assessing the applicants. Similarly, by gathering evidence concerning its content validity, convergent/divergent correlation and predictive ability, validity is ensured. Finally, its acceptability and feasibility along with limitations is discussed. This article concludes by providing recommendations for further work required to deal with the limitations and enhance its utility.Keywords: MMI, utility, reliability, validity, acceptability, feasibility, limitations

The current study scrutinizes heat and mass transfer features of magnetized flow of [Formula: see text] nanolubricant over Riga plate in a Darcy Forchheimer medium. The effects of variable viscosity, thermal radiation, variable thermal conductivity, viscous dissipation and uniform heat source/sink are examined in this study. The diffusion model presented by Cattaneo-Christov is incorporated in this study to enclose heat and mass transport phenomenon. Additionally, the mass transfer rate is inspected subjected to the effects of variable solutal diffusivity and higher order chemical reaction. Heat and mass transfer phenomena have significant applications in the disciplines of science and technology that can be seen everywhere in nature. This simultaneous transportation phenomenon indicates a variety of applications in manufacturing processes, aerodynamics, cooling systems, environmental sciences, oceanography, food industries, biological disciplines, and energy transport systems etc. The modeled system of PDEs is metamorphosed to nonlinear ODEs with the introduction of appropriate transformations. An eminent bvp4c method in MATLAB has been incorporated to execute the resulting system of ODEs numerically. The outcomes of velocity, temperature and concentration profiles corresponding to various emerging parameters have been exposed graphically. The motion of [Formula: see text] nanolubricant tends to enhance significantly with larger modified Hartmann number, whereas converse behavior is reported by increasing porosity parameter and variable viscosity parameter. The greater heat transfer rate is observed for variable thermal conductivity parameter. The rates of heat and mass transfer slow down for thermal and solutal time relaxation parameters respectively. The concentration profile gets enriched by growing the order of the chemical reaction and variable mass diffusivity parameter. It is concluded that by increasing solid volume fraction up to [Formula: see text], the viscosity of the nanolubricant enhances up to [Formula: see text] which consequently slows down motion of the nanolubricant but increases temperature and concentration profiles., (© 2023. This study discusses the flow of hybrid nanofluid over an infinite disk in a Darcy-Forchheimer permeable medium with variable thermal conductivity and viscosity. The objective of the current theoretical investigation is to identify the thermal energy characteristics of the nanomaterial flow resulting from thermo-solutal Marangoni convection on a disc surface. By including the impacts of activation energy, heat source, thermophoretic particle deposition and microorganisms the proposed mathematical model becomes more novel. The Cattaneo-Christov mass and heat flux law is taken into account when examining the features of mass and heat transmission rather than the traditional Fourier and Fick heat and mass flux law. MoS 2 and Ag nanoparticles are dispersed in the base fluid water to synthesize the hybrid nanofluid. PDEs are transformed to ODEs by using similarity transformations. The RKF-45th order shooting method is used to solve the equations. With the use of appropriate graphs, the effects of a number of non-dimensional parameters on velocity, concentration, microorganism, and temperature fields are addressed. The local Nusselt number, density of motile microorganisms and Sherwood number are calculated numerically and graphically to derive correlations in terms of the relevant key parameters. The findings show that as we increase the Marangoni convection parameter, skin friction, local density of motile microorganisms, Sherwood number, velocity, temperature and microorganisms profiles increase, whereas Nusselt number and concentration profile exhibit an opposite behavior. Development of fermented flavour during storage reduces acceptability of Shughri pear. Therefore, the current study was designed to investigate the combined effect of 1-Methylcyclopropene (1-MCP) and hypobaric treatment on stability of Shughri pear during 120 days of storage. Fruit were treated individually or combinedly with 25, 50, and 75 kilo pascal hypobaric treatments for 4 h and 1-MCP (0.3 μLL -1 and 0.6 μLL -1 ) for 24 h, whereas control received no treatment. The pears were stored for 120 days at (0 ± 1 °C, 85 ± 5% RH), and were evaluated after every 30 days. After cold storage, pears were shifted to simulated retail conditions (20 ± 3 °C, 65 ± 5% RH). The combination of 25 kPa + 0.6 μLL -1 1-MCP significantly (P ≤ 0.05) delayed fruit ripening, reduced Alcohol dehydrogenase (ADH), and Pyruvate decarboxylase (PDC) activities, maintained the quality, and led to higher consumers' acceptability of the pear followed by 50 kPa + 0.6 μLL -1 and 25 kPa + 0.3 μLL -1 . The control fruit were marketable for a week after storage with relatively less acceptability due to fermented flavour compared to treated fruit, marketable for more than two weeks. Among all the treatments, the synergy of 1-MCP and hypobaric treatment 25 kPa + 0.6 μLL -1 1-MCP improved the postharvest storage life and quality parameters, preventing development of fermented flavour in the pears. This research aimed to extend the postharvest shelf life of Royal Gala apple during cold storage and maintain its market value in simulated retail conditions. Apples were treated with hypobaric pressure (50 kPa for 4 h) followed by 1-MCP (0.5 µL L -1 , 0.7 µL L -1 , and 1.0 µL L -1 ) treatment for 24 h individually and in combinations, stored at (1 ± 1 °C, 85 ± 3% RH) for 120 d and analyzed for different quality parameters (Peel color, firmness, weight loss, TSS, acidity, ethylene production rate, and respiration rate) at each 30 d interval, followed by a 20 d simulated retail condition at 20 ± 3 °C with 4 d interval. Results indicated that all 1-MCP concentrations were more effective in retaining quality compared to individual hypobaric treatment. However, a synergistic effect was observed by combining 1-MCP with hypobaric treatment. Among the combined treatments, 1.0 µL L -1 1-MCP + 50 kPa more effectively and significantly retained quality during cold storage. Furthermore, the apples were more juicy, tasty, and attractive in color than others in simulated conditions. Addition of hypobaric treatment to 1-MCP might reduce endogenous ethylene in fruit by outward diffusion and blocking further ethylene synthesis by the action of 1-MCP. Background Urinary tract infections (UTIs) are common in patients with diabetes. The use of sodium-glucose cotransporter-2 inhibitors (SGLT2i) to achieve good glycemic control increases glucose levels in urine. This glycosuria further enhances the risk of UTIs. This study aimed to evaluate the frequency of UTIs in patients with type 2 diabetes receiving the SGLT2i dapagliflozin as an add-on therapy. Methods We conducted this cross-sectional study at the Endocrinology Department of Hayatabad Medical Complex in Peshawar from April 2020 to September 2020. A total of 400 patients with diabetes receiving either 5 mg or 10 mg of dapagliflozin as an add-on therapy for the treatment of type 2 diabetes were included in this study. We collected blood and urine samples from participants and measured glycosylated hemoglobin levels. Urine samples were cultured on cysteine lactose electrolyte deficient agar. We used IBM SPSS Statistics for Windows, version 25.0 (IBM Corp., Armonk, NY) to analyze our data. Results The prevalence of UTIs in diabetic patients receiving 5 mg or 10 mg of dapagliflozin was 5.3%. Women were more affected (76.2%) than men (p < 0.05). UTIs were more prevalent in patients older than 50 years (85.7%) than in any other age group. The dose strength of dapagliflozin was not associated with UTIs (p > 0.05). Conclusion This study examined UTIs in patients taking dapagliflozin for the treatment of type 2 diabetes. These infections were mild to moderate and were treated easily. None of these infections caused the patient to discontinue the treatment. Hematological and hematopoietic cells malignancies of the genes and hematopoietic cells are associated with the genetic mutation, often at the chromosomal level. The standard cytogenetic study is widely accepted as one of the main diagnostics and prognostic determinants in patients. Therefore, the current descriptive and cross-sectional study sought to determine the cytogenetic analysis of frequent hematological malignancies in Pakistan. A total of 202 peripheral bone marrow or blood samples from patients with benign and malignant hematological malignancy were taken using a conventional G-banding technique. Among enrolled patients, the mean age was 21.5 years ± 23.4, and gender-wise distribution showed a marked predominance of the male 147 (73%) population compared to the female 55 (27%). Patients in the age group (2-10 years) had the highest frequency, 48 (24%), of hematological neoplasms, followed by age (11-20 years) with 40 (20%). Normal karyotypes (46, XX/46, XY) was found in 51% (n=103) patients. Furthermore, the frequency of complex karyotype was 30 (15%), while normal was seen in 171 (85%) patients. Pre-B Acute Lymphoblastic Leukemia (Pre-B ALL) was the most prevalent malignancy of 66 (33%), followed by Chronic Myelogenous Leukemia (CML) of 41 (20%) and Acute Lymphocytic Leukemia of 29 (14%). Translocation was the most prevalent 50 (25%), followed by hypotriploidy 14 (7%) and monosomy 8 (4%) on chromosome aberration analysis. In addition, t(9:22) translocation was found to be 20 (10%) in CML, with the majority in the age group (31-40 years). This study recommends that karyotyping should be tested frequently in hematological conditions because it may provide insight into the relative chromosomal changes associated with particular malignancies.

The appropriate utilization of entropy generation may provoke dipping losses in the available energy of nanofluid flow. The effects of chemical entropy generation in axisymmetric flow of Casson nanofluid between radiative stretching disks in the presence of thermal radiation, chemical reaction, and heat absorption/generation features have been mathematically modeled and simulated via interaction of slip boundary conditions. Shooting method has been employed to numerically solve dimensionless form of the governing equations, including expressions referring to entropy generation. The impacts of the physical parameters on fluid velocity components, temperature and concentration profiles, and entropy generation number are presented. Simulation results revealed that axial component of velocity decreases with variation of Casson fluid parameter. A declining variation in Bejan number was noticed with increment of Casson fluid constant. Moreover, a progressive variation in Bejan number resulted due to the impact of Prandtl number and stretching ratio constant.

Inflammatory bowel disease (IBD) is a constellation of devastating chronic inflammatory changes in the bowel, either involving the large or small bowel or part of both. As it is widely diagnosed in the fertile age group, this disorder can present itself, very commonly, during pregnancy and thus a better understanding of the disease can be an important factor to influence the maternal and fetal well-being. Medications are what is considered the first line in the management of this disease to control the symptoms or keep the disease in remission. In addition to this, the drugs used to keep the disease in remission can also cause significant adverse effects on the patient and the new nurturing life preparing itself for the outside world. What the fetus gets from the mother will stay for life with the child. We conducted an electronic literature review search which highlights the significance and impact of sustained remission of IBD and the cautious use of various drugs during pregnancy for that purpose. In addition to the influences already mentioned, It is evident that nutritional deficiencies can also prevail with the advancing disease, something to manage as a side note as well. These deficiencies can have a definite effect on the fetus and may cause developmental malformations. In order to avoid this process, a systemic and joint approach should be curtailed. In this study, many bacterial strains were screened for the production of minor ginsenosides, but based on conversion competence among the strains, the strain Niabella ginsenosidivorans BS26 T has the good ginsenoside-transforming ability. Therefore, the strain BS26 T was selected for complete genome sequence analysis to determine the target (glycoside hydrolase) functional genes. Whole genome analysis of strain BS26 T showed 43 glycoside hydrolase genes in total. To determine the target functional gene, 12 sets of six different glycoside hydrolases (3 set of β-glucosidase; 3 set of trehalase; 3 set of arabinofuranosidase; 2 set of xylosidase; and one set of each α-galactosidase and α-fucosidase, respectively) were selected and cloned in E. coli BL21 (DE3) using the pGEX4T-1 vector and were characterized. Among these 12 sets of clones, only one, β-glucosidase (BglNg-767), showed ginsenoside conversion ability. The BglNg-767 comprised 767 amino acids and belonged to glycoside hydrolase family 3 (GH3). The recombinant GST-BglNg-767 was capable of altering the ginsenosides Rb 1 , Rd, and gypenoside XVII (Gyp-XVII) to F 2 ; Rb 2 to C-O; Rb 3 to C-Mx1, and Rc to C-Mc1. Besides, complete genome sequence analysis of strain BS26 T also indicates 30 endopeptidase genes, which may be responsible for self-hydrolysis of the proteins. Therefore, using SDS-PAGE analysis, we predict that the difference between the molecular weight of the expressed protein (around 90 kDa) and the predicted amino-acid sequence (102.7 kDa) is due to self-hydrolysis of the proteins., Competing Interests: Conflict of interestThe authors declare that there are no conflict of interest.

Background: Farm workers and female cotton pickers are exposed to residual impacts of pesticide use in cotton production, in addition to dust, ultraviolet radiation, etc. Cotton picking causes various health hazards among cotton pickers with varied health cost. A soil bacterium known as Bacillus thuringiensis (Bt) is incorporated in cotton seed through genetic modification and it has resistance against certain bollworms of cotton. So it is considered that Bt cotton fields have less pesticide exposure compared to non-Bt cotton fields. This study was designed to examine and compare the impacts and health cost of cotton picking among female cotton pickers working in Bt and non-Bt cotton fields., Methods: The study used the data collected from Vehari district of Pakistani Punjab. Health hazards and associated health cost of the respondents involved in Bt cotton picking were compared with those who harvested non-Bt cotton. Comparative use of the personal protective measures among those respondents was also examined. Health cost function and its determinants were analyzed using ordinary least square method., Results: Findings of the study showed that 61 % cotton pickers from Bt cotton households reported one or more health effects of pesticide during picking season whereas this percentage for non-Bt cotton households was 66 %. Health impacts included skin problems, headache, cough, flu/fever, eye irritation and sleeplessness, however, percentage of these health impacts was comparatively higher among non-Bt cotton households. Health cost from exposure to pesticide use in cotton was US$ 5.74 and 2.91 per season for non-Bt cotton and Bt cotton households, respectively. Education, picking in Bt cotton fields and preventive measures were significantly related with health cost., Conclusion: Cotton pickers working in Bt cotton fields are found to have less occupational health hazards compared to those working in non-Bt cotton fields. Thus generating awareness among cotton pickers for adopting precautionary measures during harvesting and the use of Bt cotton seed can result in a decline in the ill-effects of cotton picking.

Objective: The objective of this study was to investigate the clinical characteristics of patients with dengue viral infection during the 2008 outbreak in Lahore in order to better understand the clinical pattern and severity of disease in Lahore., Methods: We analyzed the clinical characteristics of 110 patients infected with dengue virus; data were collected on standardized data collection sheets at two tertiary care hospitals from September to December 2008. Dengue infection was confirmed serologically or by real-time polymerase chain reaction (RT-PCR)., Results: Out of the total of 110 dengue infected patients, 70 were male and 40 were female. The most common symptoms included fever (100%), myalgia (68.2%), headache (55.5%), nausea (39.1%), skin rash (53.6%), mucocutaneous hemorrhagic manifestations (58.2%), and ocular pain (20%). Classic dengue fever (DF) was seen in 41.8% of the patients, 56.4% had dengue hemorrhagic fever (DHF), and only 1.8% developed dengue shock syndrome (DSS). The mean duration of fever was 6 days. Thrombocytopenia, leukopenia, and abnormal aspartate aminotransferase (AST)/alanine aminotransferase (ALT) were more frequently encountered in DHF and DSS as compared to DF. Viral RNA detection was done by RT-PCR in 17 patients. Ten patients had DEN4, five had DEN2, and two had DEN3 serotypes. The majority of the patients recovered completely without complications., Conclusion: The high frequency of DHF during the 2008 outbreak and the presence of three different dengue serotypes, emphasize the need to prevent and control dengue infection. Health authorities should consider strengthening surveillance for dengue infection, given the potential for future outbreaks with increased severity. The cauterising iris scissors described here are a modification achieved by suitably insulating the standard De Wecker's iris scissors. This modification provides in addition to cutting an advantageous cauterising effect, which could prove useful in the surgery of eye conditions with high risk of bleeding or metastasis. It utilises the optimum electric current range of 20-40 mA.
