4. Incidental Findings in Patients with Cleft Lip and Palate: A Case–Control Study.

Abstract

Featured Application: Systematic assessment of diagnostic imaging records should be applied to eliminate the risk of overlooking clinically significant incidental findings and facilitate successful interdisciplinary treatment planning (especially in cleft lip and/or palate cases). Incidental findings (IFs) depicted in imaging tests during the diagnostic evaluation of patients with cleft lip and/or palate (CL/P) can guide clinicians during treatment planning procedures for effective, comprehensive cleft care. Evidence regarding IFs in different anatomical regions distant from the dentition is scarce. The aim of this study was to compare the prevalence of IFs in various anatomical areas between patients with non-syndromic CL/P and unaffected subjects. Orthopantomographs (OPTs), lateral cephalometric (LC) radiographs, and cone beam computed tomography (CBCT) scans of 120 subjects (case group: n = 40, 18 females, age 14.5 ± 5.0 years; control group: n = 80, 36 females, age 14.6 ± 4.9 years) were examined, and IFs were assessed by four observers. A significantly higher prevalence of IFs (p < 0.001) was reported in the case group (97.5%) compared with the control group (62.5%). The most prevalent location of IFs in the case group was the maxilla (92.5%), followed by the nasal cavity (75.8%) and the mandible (63.2%), while in the control group, the highest rates of IFs occurred in the maxilla (43.8%), mandible (41.3%), and temporomandibular joint (TMJ) (14.7%). No gender-dependent pattern for IF occurrence was detected overall. Orofacial clefts may affect the mandible. However, the reported IFs were limited compared to those in the maxilla. In conclusion, individuals with CL/P are more likely to present with a range of IFs than their unaffected peers. Understanding the association between CL/P and IFs is critical for successful interdisciplinary treatment, raising awareness of the potential need for future dental care for cleft patients, and managing extra-dental aberrations. Abstract

The aim of this study was to adapt Mindful Eating Questionnaire (MEQ) in a Greek sample (N = 300), with age ranging from 18 to 95 years old. Exploratory factor analysis (EFA) revealed a three-factor model (Emotional Disinhibition, Awareness, Distraction) retaining 20 items (MEQ-20). Confirmatory factor analysis (CFA) demonstrated a good fit between the three-factor model and the sample data. Cronbach���s alpha and item-total correlations were satisfactory. As regard MEQ-20 validity, there was only weak correlation between the MEQ-20 and EAT-13 subscales and total scores. However, MEQ-20 total score significantly discriminated between participants with and without healthy BMI. The findings suggest that MEQ-20 is a reliable and valid instrument for evaluating mindful eating in the Greek population, for clinical and research applications., Hellenic Journal of Psychology, Vol 19, No 1 (2022)

Abstract

Lifestyle medicine and, in particular, medical nutrition therapy (MNT) is the cornerstone of gestational diabetes mellitus (GDM) management. MNT aims to achieve normoglycemia, prevent ketosis, provide adequate gestational weight gain, and optimize fetal growth. Evidence-based MNT can negate pregnancy complications, improve obstetrical outcomes, and reduce the risk for maternal and fetal adverse events. Although achieving an energy equilibrium is vital for improving all outcomes, specific daily energy intake goals are rarely suggested in clinical practice guidelines. Moreover, despite the plethora of dietary supplements tested in randomized controlled trials in women with GDM, very few have demonstrated beneficial effects in maternal-fetal outcomes. An energy-prudent and nutritionally adequate diet comprises the core of all successful nutrition interventions.

Abstract

The International Society of Pediatric and Adolescent Diabetes (ISPAD) recommends metformin (MET) use for metabolic disturbances and hyperglycemia, either in combination with insulin therapy or alone. A caveat of MET therapy has been suggested to be biochemical vitamin B12 deficiency, as seen mainly in studies conducted in adults. In the present case–control study, children and adolescents of different weight status tiers on MET therapy for a median of 17 months formed the cases group (n = 23) and were compared with their peers not taking MET (n = 46). Anthropometry, dietary intake, and blood assays were recorded for both groups. MET group members were older, heavier, and taller compared with the controls, although BMI z-scores did not differ. In parallel, blood phosphorus and alkaline phosphatase (ALP) concentrations were lower in the MET group, whereas MCV, Δ4-androstenedione, and DHEA-S were higher. No differences were observed in the HOMA-IR, SHBG, hemoglobin, HbA1c, vitamin B12, or serum 25(OH)D3 concentrations between groups. Among those on MET, 17.4% exhibited vitamin B12 deficiency, whereas none of the controls had low vitamin B12 concentrations. Participants on MET therapy consumed less energy concerning their requirements, less vitamin B12, more carbohydrates (as a percentage of the energy intake), and fewer fats (including saturated and trans fats) compared with their peers not on MET. None of the children received oral nutrient supplements with vitamin B12. The results suggest that, in children and adolescents on MET therapy, the dietary intake of vitamin B12 is suboptimal, with the median coverage reaching 54% of the age- and sex-specific recommended daily allowance. This low dietary intake, paired with MET, may act synergistically in reducing the circulating vitamin B12 concentrations.
