PurposeWe used quantitative magnetic resonance imaging to prospectively analyze the association between microstructural damage to memory-associated structures within the medial temporal lobe and longitudinal memory performance after brain radiation therapy (RT).Methods and materialsPatients with a primary brain tumor receiving fractionated brain RT were enrolled on a prospective trial (n = 27). Patients underwent high-resolution volumetric brain magnetic resonance imaging, diffusion-weighted imaging, and neurocognitive testing before and 3, 6, and 12 months post-RT. Medial temporal lobe regions (hippocampus; entorhinal, parahippocampal, and temporal pole white matter [WM]) were autosegmented, quantifying volume and diffusion biomarkers of WM integrity (mean diffusivity [MD]; fractional anisotropy [FA]). Reliable change indices measured changes in verbal (Hopkins Verbal Learning Test-Revised) and visuospatial (Brief Visuospatial Memory Test-Revised [BVMT-R]) memory. Linear mixed-effects models assessed longitudinal associations between imaging parameters and memory.ResultsVisuospatial memory significantly declined at 6 months post-RT (mean reliable change indices, -1.3; P = .012). Concurrent chemotherapy and seizures trended toward a significant association with greater decline in visuospatial memory (P = .053 and P = .054, respectively). Higher mean dose to the left temporal pole WM was significantly associated with decreased FA (r = -0.667; P = .002). Over all time points, smaller right hippocampal volume (P = .021), lower right entorhinal FA (P = .023), greater right entorhinal MD (P = .047), and greater temporal pole MD (BVMT-R total recall, P = .003; BVMT-R delayed recall, P = .042) were associated with worse visuospatial memory. The interaction between right entorhinal MD (BVMT-R total recall, P = .021; BVMT-R delayed recall, P = .004) and temporal pole FA (BVMT-R delayed recall, P = .024) significantly predicted visuospatial memory performance.ConclusionsBrain tumor patients exhibited visuospatial memory decline post-RT. Microstructural damage to critical memory regions, including the hippocampus and medial temporal lobe WM, were associated with post-RT memory decline. The integrity of medial temporal lobe structures is critical to memory performance post-RT, representing possible avoidance targets for memory preservation.

Background and purposeExecutive function (EF) decline is common after brain radiation therapy (RT), yet the etiology is unclear. We analyzed the association between longitudinal changes in frontal lobe white matter microstructure and decline in EF following RT in brain tumor patients on a prospective clinical trial.Materials and methodsDiffusion tensor imaging was obtained on 22 patients with brain tumors prior to RT, as well as 3- and 6-months post-RT, in a prospective, observational trial. Fractional anisotropy (FA), mean diffusivity (MD), radial diffusivity (RD), and axial diffusivity (AD) were calculated within the superficial white matter (SWM) of the anterior cingulate (AC) and dorsolateral prefrontal cortex. Measures of cognitive flexibility, verbal fluency, and verbal set-shifting were obtained pre- and post-RT. Reliable change indices were calculated to determine significant baseline to 6-month EF changes.ResultsDecreases in FA and increases in MD were observed in the caudal AC (CAC) at 3-months post-RT. CAC changes were characterized by increased RD bilaterally. From baseline to 6-months post-RT, decreased FA and increased MD and RD of the CAC was associated with decline in verbal set-shifting ability, whereas increased MD in the CAC was associated with a decline in cognitive flexibility.ConclusionWhite matter underlying the AC may be particularly vulnerable to radiation effects. Early microstructural loss within AC SWM represents an important biomarker for EF decline, and dose reduction in this region may represent a possibility for cognitive preservation for patients receiving radiotherapy.

PurposeNeurologic deficits that may be manifested as cognitive impairment contribute to the challenges faced by caregivers of patients with brain metastases. To better address their needs, we examined how caregivers respond to these challenges and explore the relationship between the patient's cognitive impairment and caregiver resilience and coping.MethodsWe conducted a descriptive, cross-sectional study using self-reported data from 56 caregivers of patients with brain metastases. Study participants from a comprehensive cancer center were asked to complete a series of instruments that measured their perception of the patient's cognitive dysfunction (revised memory and behavior problems checklist, RMBC), their own personal resilience (Resilience Scale, RS), and their utilization of a broad range of coping responses (COPE inventory and Emotional-Approach Coping scale).ResultsCaregivers reported that memory-related problems occurred more frequently in the patients they cared for compared to depression and disruptive behavior (mean scores 3.52 vs 2.34 vs. 1.32, respectively). Coping strategies most frequently used by caregivers were acceptance (3.28), planning (3.08), and positive reinterpretation and growth (2.95). Most caregivers scored moderate to high on the RS (77%). The coping strategy acceptance correlated significantly with the memory and disruptive behavior subscales of the RMBC.ConclusionsGiven the protective effect of problem-focused coping and the high rate of caregivers utilizing less effective coping strategies in instances of worsening cognitive dysfunction, healthcare professionals need to systematically assess the coping strategies of caregivers and deliver a more personalized approach to enhance effective coping among caregivers of patients with brain metastases.

Abstract Background In 6-month anti-dementia drug trials, a 4-point change in the Alzheimer's Disease Assessment Scale-Cognitive Subscale (ADAS-Cog) is held to be clinically important. We examined how this change compared with measures of clinical meaningfulness. Methods This is a secondary analysis of a 12 month open-label study of 100 patients (71 women) diagnosed with mild to moderate AD treated with 5–10 mg of donepezil daily. We studied the observed case, 6-month change from baseline on the ADAS-Cog, the Clinician's Interview Based Impression of Change-Plus Caregiver Input (CIBIC-Plus), patient-Goal Attainment Scaling (PGAS) and clinician-GAS (CGAS). Results At 6 months, donepezil-treated patients (n = 95) were more likely to show no change (+/- 3 points) on the ADAS-Cog (56%) than to improve (20%) or decline (24%) by 4-points. ADAS-Cog change scores were little correlated with other measures: from -0.09 for PGAS to 0.27 for the CIBIC-Plus. While patients who improved on the ADAS-Cog were less likely to decline on the clinical measures (26%), 43% of patients who declined on the ADAS-Cog improved on at least two of the clinical measures. Conclusion The ADAS-Cog did not capture all clinically important effects. In general, ADAS-Cog improvement indicates clinical improvement, whereas many people with ADAS-Cog decline do not show clinical decline. The open-label design of this study does not allow us to know whether this is a treatment effect, which requires further investigation.

- Abstract
Following the crash of Asiana flight 214 on July 6, 2013, the National Transportation Safety Board (NTSB) provided a recommendation to the Aircraft Rescue & Fire Fighting Working Group (ARFFWG) to develop best practices to avoid strike/rollover of seriously injured or deceased persons in mass casualty situations. This paper summarizes the research process and the technical committee’s conclusions. The qualitative inquiry used participatory research and evaluation from medico-legal professionals and the expert committee comprised of 21 aircraft rescue and firefighting experts. Key informants were complemented by documentary analysis, including FAA Advisory Circulars, regulations, standards, medical and aircraft rescue industry publications. The research provides specific committee recommendations for establishing control zones in the mass casualty environment, using START for triage, surveillance, transportation, training and certification of both ARFF personnel and incident command, and airport emergency planning strategies. A best practice for ARFF personnel, which is a tactical procedure for task prioritization at aircraft crash incidents, is also proposed. The authors suggest future pilot testing for evaluation of these best practices following NTSB’s consent with the committee’s recommendations.

- Abstract
As the nexus between aviation, fire science, and emergency management becomes increasingly more complex, technology can be used to augment training and avoid risk while maintaining authenticity with situational reality. In Aircraft Rescue & Fire Fighting (ARFF), the manner in which the learning content is effectively imparted to the student-professional can make all the difference in a successful emergency response. The interactive 3-D software developed exclusively for Dallas/Fort Worth Fire Training Research Center (DFW FTRC) is part of a curriculum designed to educated and train ARFF professionals in life-saving strategies and tactics. Through a 2013 Memorandum of Understanding, the world’s leading aviation and aerospace university, Embry-Riddle Aeronautical University (ERAU), and DFW FTRC have created a unique partnership for the conduct of research to explore emerging issues in the discipline. Researchers from ERAU designed this collaborative study to examine how effectively the DFW FTRC virtual technology software in the classroom translated knowledge and accelerated haptic skill transfer to the hands-on portion of ARFF training at the facility. The qualitative approach integrated classroom observations with key informant interviews to create a single-case study exploratory research strategy. Emergent themes led to a richer understanding of experiential learning, particularly key differences among individual student characteristics and instructional culture that influence student proficiency and attitudes. Most noteworthy were: a larger interest among international over domestic students, age/generational difficulties with new technology, and an apprehension among ARFF professionals to transition from traditional tabletop exercises to virtual reality. Variability also existed within the instructional culture, including inconsistencies of software integration in the classroom, instructor proficiency with the software, and adult and group teaching theories. Advances in neuroscience related to developmental disorders could substantially impact individuals with disabilities and the field of special education. However, several challenges impede the current translation of neuroscience research for special education practice, such as misinterpretations of neuroscience findings. An investigation of translational research in medicine and social sciences revealed a common conceptual framework founded by the National Institutes of Health (NIH). Using this model as a guide, the authors introduce a new framework, the Neuroeducation Translational (NET) Research Model, to scaffold neuroscience research from the laboratory to special education practice in four phases.
