Abstract

Background: Hypertension management in older patients represents a challenge, particularly when hospitalized. Objective: The objective of this study is to investigate the determinants and related outcomes of antihypertensive drug prescription in a cohort of older hospitalized patients. Methods: A total of 5671 patients from REPOSI (a prospective multicentre observational register of older Italian in-patients from internal medicine or geriatric wards) were considered; 4377 (77.2%) were hypertensive. Minimum treatment (MT) for hypertension was defined according to the 2018 ESC guidelines [an angiotensin-converting-enzyme-inhibitor (ACE-I) or an angiotensin-receptor-blocker (ARB) with a calcium-channel-blocker (CCB) and/or a thiazide diuretic; if >80 years old, an ACE-I or ARB or CCB or thiazide diuretic]. Determinants of MT discontinuation at discharge were assessed. Study outcomes were any cause rehospitalization/all cause death, all-cause death, cardiovascular (CV) hospitalization/death, CV death, non-CV death, evaluated according to the presence of MT at discharge. Results: Hypertensive patients were older than normotensives, with a more impaired functional status, higher burden of comorbidity and polypharmacy. A total of 2233 patients were on MT at admission, 1766 were on MT at discharge. Discontinuation of MT was associated with the presence of comorbidities (lower odds for diabetes, higher odds for chronic kidney disease and dementia). An adjusted multivariable logistic regression analysis showed that MT for hypertension at discharge was associated with lower risk of all-cause death, all-cause death/hospitalization, CV death, CV death/hospitalization and non-CV death. Conclusions: Guidelines-suggested MT for hypertension at discharge is associated with a lower risk of adverse clinical outcomes. Nevertheless, changes in antihypertensive treatment still occur in a significant proportion of older hospitalized patients.

Abstract

Coronavirus disease 2019 (COVID-19) is a newly recognized infectious disease which can lead to acute respiratory distress syndrome requiring ventilatory support and intensive care unit admission. The aim of our study is to evaluate the performance of two non-invasive respiratory function indices (the ROX index and the SatO2/FiO2 ratio), as compared to the traditional PaO2/FiO2 ratio, in predicting a clinically relevant composite outcome (death or intubation) in hospitalized patients for COVID-19 pneumonia. Four hospital centers in Northern Italy conducted an observational retrospective cohort study during the first wave of COVID-19 pandemic. Four hundred and fifty-six patients with COVID-19 pneumonia admitted to medical or sub-intensive wards were enrolled. Clinical, laboratory, and respiratory parameters, for the calculation of different indices, were measured at hospital admission. In medical wards (Verona and Padua) the PaO2/FiO2 ratio, ROX index and SatO2/FiO2 ratio were able to predict intubation or death with good accuracy (AUROC for the PaO2/FiO2 ratio, ROX index and SatO2/FiO2 ratio of 75%, 75% and 74%, respectively). Regarding sub-intensive wards (Milan and Mantua), none of the three respiratory function indices was significantly associated with the composite outcome. In patients admitted to medical wards for COVID-19 pneumonia, the ROX index and the SatO2/FiO2 ratio demonstrated not only good performance in predicting intubation or death, but their accuracy was comparable to that of the PaO2/FiO2 ratio. In this setting, where repeated arterial blood gas tests are not always feasible, they could be considered a reliable alternative to the invasive PaO2/FiO2 ratio.

Anche se l'opinione pubblica è più colpita dagli aspetti emergenziali del fenomeno migratorio, la presenza di individui di origine non italiana nel nostro Paese sta assumendo un carattere strutturale. Ciò comporta la presenza di un elevato numero di studenti nelle nostre scuole senza cittadinanza italiana. Le scuole italiane svolgono un ruolo fondamentale nel compito di includere i più giovani tra i migranti nella nostra società. Il contributo vuole presentare la complessità del fenomeno dal punto di vista quantitativo, della loro distribuzione, della provenienza, della differenza di genere, dell'anzianità di arrivo e cerca di presentare alcuni approcci al fenomeno da parte degli insegnanti.

Abstract

Introduzione: Il 5-fluorouracile (5-FU) ed il suo profarmaco capecitabina, attivo per via orale, è usato nel trattamento di numerosi tumori solidi, tra cui carcinomi mammari, colo-rettali e della regione testa-collo. Nonostante la sua efficacia, a suo carico sono stati associati fenomeni di grave tossicità correlati all’enzima diidropirimidina deidrogenasi (DPD), responsabile del suo metabolismo,la cui carenza determina un profondo ritardo nell’eliminazione dell’agente chemioterapico.Pertanto la presenza di polimorfismi nella regione codificante del gene DPYD può determinare lo sviluppo di gravi tossicità di III-IV grado, come: nausea, vomito, diarrea, stomatiti, leucopenia, neutropenia, piastrinopenia, anemia e sindrome mano-piede.

Abstract

Aim: Root hydraulic conductivity is one of the main factors that control water flow in the soil-plant system and ultimately affect crop irrigation requirements. This work sets out to estimate root water conductivity for three self-rooted or grafted table grape cultivars.Methods and results: We evaluated root water conductivity of the cultivars ‘Black Magic’, ‘Matilde’ and ‘Victoria’ grafted onto 1103 Paulsen rootstock or self-rooted. Measurements were performed on two-year-old table grapes grown in pots filled with pumice. Root water conductivity was determined by placing the pots in a pressure chamber and increasing pressures from 0.05 to 0.30 MPa, at intervals of 0.05 MPa. Plant growth in terms of shoot and root dry matter was also evaluated, as well as leaf and root area.Conclusion: Root water conductivity differed according to cultivar and grafting. The ‘Victoria’ cultivar had higher root water conductivity than the other two, which differed little between them. The grafted plants showed higher root water conductivity than the self-rooted plants, except the plants of the cultivar ‘Matilde’, whose root water conductivity for the grafted and self-rooted plants was almost the same. It was also observed that the higher root water conductivity in relation to cultivar and grafting changed with increasing water flux and was constant for high water flux.Significance and impact of the study: Estimation of root hydraulic conductivity helps to determine the water consumption of the cultivars investigated, whether self-rooted or grafted, as well as the amount of irrigation water to apply to vineyards.

Abstract

Background & Aims: Bacterial infections can trigger the development of organ failure(s) and acute-on-chronic liver failure (ACLF). Geographic variations in bacteriology and clinical practice could lead to worldwide differences in ACLF epidemiology, phenotypes and associated outcomes. Herein, we aimed to evaluate regional differences in bacterial infection-related ACLF in patients with cirrhosis admitted to hospital. Methods: This post hoc analysis included 1,175 patients with decompensated cirrhosis (with bacterial infection on admission or nosocomial infection) from 6 geographic regions worldwide. Clinical, laboratory and microbiological data were collected from the diagnosis of infection. Patients were followed-up for organ failure(s) and ACLF development according to the EASL-CLIF criteria from enrolment to discharge/death. Results: A total of 333 patients (28%) had ACLF at diagnosis of infection, while 230 patients developed ACLF after diagnosis of infection, resulting in an overall rate of bacterial infection related-ACLF of 48%, with rates differing amongst different geographic regions (38% in Southern Europe vs. 75% in the Indian subcontinent). Abstract

Summary T helper 9 (Th9) cells and interleukin (IL)-9 are involved in the pathogenesis of several autoimmune diseases. The exact role of IL-9 and Th9 cells in patients with systemic sclerosis (SSc) have not yet been studied adequately. IL-9, IL-9R, transcription factor PU.1 (PU.1), IL-4, thymic stromal lymphopoietin (TSLP) and transforming growth factor (TGF)-β expression were assessed in skin and kidney biopsies of SSc patients and healthy controls (HC) by immunohistochemistry (IHC). The cellular source of IL-9 was also analysed by confocal microscopy analysis. Peripheral IL-9-producing cells were also studied by flow cytometry. The functional relevance of IL-9 increased expression in SSc was also investigated. Our results demonstrated a strong expression of IL-9, IL-9R, IL-4, TSLP and TGF-β in skin tissues of patients with both limited and diffuse SSc. IL-9 expression was observed mainly in the context of skin infiltrating mononuclear cells and keratinizing squamous epithelium. IL-9 over-expression was also observed in renal biopsies of patients with SSc. IL-9 producing cells in the skin were identified as Th9 cells. Similarly, Th9 cells were expanded and were the major source of IL-9 among SSc peripheral blood mononuclear cells (PBMC), their percentage being correlated directly with the modified Rodnan skin score. Infiltrating mononuclear cells, mast cells and neutrophils expressed IL-9R. In in-vitro studies stimulation with rIL-9 significantly induced NET (neutrophil extracellular traps) release by dying cells (NETosis) in neutrophils, expansion of mast cells and increase of anti-systemic scleroderma 70 (Scl70) production by B cells. Our findings suggest that Th9 cells and IL-9 could be implicated in the pathogenesis of SSc.

Abstract

Background: The aim of this study was to analyse the nutraceutical properties of table green olives Nocellara del Belice, a traditional Mediterranean food. The Mediterranean Diet has as key elements olives and extra virgin olive oil, common to all Mediterranean countries. Olive oil is the main source of fat and can modulate oxidative stress and inflammation, whereas little is known about the role of olives. Moreover, emerging evidences underline the association between gut microbiota and food as the basis of many phenomena that affect health and delay or avoid the onset of some age-related chronic diseases. Methods: In order to show if table green olives have nutraceutical properties and/or probiotic effect, we performed a nutritional intervention, administering to 25 healthy subjects (mean age 38,3), 12 table green olives/day for 30 days. We carried out anthropometric, biochemical, oxidative stress and cytokines analyses at the beginning of the study and at the end. Moreover, we also collected fecal samples to investigate about the possible variation of concentration of Lactobacilli, after the olives consumption. Result: Our results showed a significant variation of one molecule related to oxidative stress, malondialdehyde, confirming that Nocellara del Belice green olives could have an anti-oxidant effect. In addition, the level of interleukin-6 decreased significantly, demonstrating how this food could be able to modulate the inflammatory response. Moreover, it is noteworthy the reduction of fat mass with an increase of muscle mass, suggesting a possible effect on long time assumption of table olives on body mass variation. No statistically significant differences were observed in the amount of Lactobacilli, although a trend towards an increased concentration of them at the end of the intervention could be related to the nutraceutical effects of olives. Conclusion: These preliminary results suggest a possible nutraceutical effect of daily consumption of green table olives Nocellara del Belice. To best of our knowledge, this is the first study performed to assess nutraceutical properties of this food. Of course, it is necessary to verify the data in a larger sample of individuals to confirm their role as nutraceuticals.

Abstract

Killer Immunoglobulin like Receptors (KIR) are membrane proteins expressed on Natural Killer (NK) cells and on a small subset of CD8 lymphocytes. They influence the activation or inhibition of both cell types through interaction with their ligands, represented by Human Leukocyte Antigen (HLA) class I molecules. Several studies have shown that KIR/HLA interactions are involved in the pathogenesis and progression of different diseases as viral infections, autoimmune-disorders, or cancer, conditioning susceptibly to or protection against the outcome of the disease. HBV infection represents a major health problem with 2 billion people infected and 3 hundred and fifty million people with chronic diseases worldwide. The aim of this study is to assess whether KIR/HLA interaction is involved in immune response against hepatitis B infection. We are conducting a case-control study in Sicilian population, comparing subjects with chronic hepatitis B with subjects with previous HBV infection (HBc-Ab with or without HBeAb or HBsAb) as controls. A second control population was represented by HBcAb negative subjects. Peripheral blood samples were collected and genomic DNA was extracted from leukocytes and typed, using PCR-SSP, for KIR and HLA Preliminary data showed that HLA-A Bw4 was more frequent in cases than controls (1. cases, 23/35, 66%, vs. HBcAb positive controls, 8/35, 23%, OR 6.47, p=0.0003, and 2. cases, 23/35, 66%, vs. HBcAb negative controls, 6/60, 10%, OR 17.25, p=0.00001). No difference have been found in the frequencies HLA-C1, HLA-C2, HLA-Bw4I, and HLA-Bw4T; no difference have also been reported in the number of KIRs, both activating and inhibitory, and haplotype A and B between groups. Our data suggest that the HLA-A Bw4, likely through its interaction with the inhibitory KIR3DL1, is associated to the risk of developing chronic hepatitis B. Data on HBcAb negative subjects suggest also the possibility of selective genetic pressure.

This paper reviews cultural practices to improve fruit quality in table grape during vegetative and reproductive seasons. Summer pruning in table grape (Vitis vinifera L.) has more effects than winter pruning, above all with regard to plant productivity and final number of bunches for harvesting. Abstract

My researches during my PhD were mainly focused on two aspects. The first one, was to study molecular aspects potentially implicated in autoimmunity pathogenesis, in order to identify new potential risk factors useful as therapeutic target. To this end, we focused on two severe and wasting systemic autoimmune diseases: systemic lupus erythematosus and systemic sclerosis. Pathogenesis of these diseases has still not clear and early diagnosis is difficult to identify because of complex and heterogeneous presentation of symptoms. A strong genetic association between HLA and disease susceptibility is well accept, nevertheless, other factors as oxidative stress, KIR and inflammatory cytokines have a relevant role in disease induction and progression, particularly in predisposed individuals. The aim of this thesis was to observe molecular and genetic variants connected to inflammatory and oxidative status, typical of autoimmune disorders, in order to increase our knowledge about pathogenesis and to find possible diagnostic markers and/or therapeutic targets for improving diagnosis and treatment of these severe diseases. The second aspect was to study potential molecular mechanisms to develop new vaccines or the optimization of immunization in elderly. It is well known, that ageing affects the immune system and its ability to respond pathogens. The current immunological strategies are based on triggering of TLRs on DCs to elicit cytokine production and different immune responses. In this thesis, we evaluated cytokine production in DCs populations after stimulation with combined molecular adjuvants, incorporating complementary TLR agonists. This mechanism may be useful in the new development of vaccine strategy both in young than in elderly. Interestingly, it has been found out that similar strategy could be used in AIDs treatment by the use of negative/positive ligands of TLRs, opening future strategic perspective to adopt in autoimmunity. Indeed, if on the one hand triggering of TLRs may induce increased secretion of inflammatory cytokines to enhance T cell response against pathogens; on the other hand, using TLRs regulators could confer an anti-inflammatory polarizing profile to DCs that could be beneficial in patients with AIDs.

Abstract

Gender medicine is a multidisciplinary science and represents an important perspective for pathophysiological and clinical studies in the third millennium. Here, it is provided an overview of the topics discussed in a recent course on the Role of Sex and Gender in Ageing and Longevity. The paper highlights three themes discussed in the course, i.e., the interaction of gender/sex with, i) the pathophysiology of age-related diseases; ii), the role of genetics and epigenetics in ageing and longevity and, iii) the immune responses of older people to pathogens, vaccines, autoantigens, and allergens. Although largely unexplored, it is clear that sex and gender are modulators of disease biology and treatment outcomes. 19. Impact of bacterial infections prior to liver transplantation on post-transplant outcomes in patients with cirrhosis.

Abstract

Background & Aims: Bacterial infections are frequent in patients with cirrhosis and increase the risk of death and drop-out from liver transplant (LT) waiting list. In patients with bacterial infections, LT is frequently delayed because of the fear of poor outcomes. We evaluated the impact of pre-LT infections on post-LT complications and survival., Methods: From 2012 to 2018, consecutive patients transplanted at the Hospital of Padua were identified and classified in two groups: patients surviving an episode of bacterial infection within 3 months before LT (study group) and patients without infections before LT (control group). Post-LT outcomes (complications, new infections, survival) were collected., Results: A total of 466 LT recipients were identified (study group n = 108; control group n = 358). After LT, the study group had a higher incidence of new bacterial (57% vs. 20%, p <0.001) and fungal infections (14% vs. 5%, p  = 0.001) and of septic shock (8% vs. 2%, p  = 0.004) than the control group. Along with the model for end-stage liver disease (MELD) score and alcohol-related cirrhosis, bacterial infection pre-LT was an independent predictor of post-LT infections (odds ratio = 3.92; p <0.001). Nevertheless, no significant difference was found in 1-year (88% vs. 89%, p  = 0.579) and 5-year survival rates (76% vs. 75%, p  = 0.829) between the study group and control group. Within the study group, no association was found between the time elapsed from infection improvement/resolution to LT and post-LT outcomes., Conclusions: Patients with pre-LT infections have a higher risk of new bacterial and fungal infections and of septic shock after LT. However, post-LT survival is excellent. Therefore, as soon as the bacterial infection is improving/resolving, transplant should not be delayed, but patients with pre-transplant bacterial infections require active surveillance for infections after LT., Impact and Implications: Bacterial infections increase mortality and delay transplant in patients with cirrhosis awaiting liver transplantation (LT). Little is known about the impact of adequately treated infections before LT on post-transplant complications and outcomes. The study highlights that pre-LT infections increase the risk of post-LT infections, but post-LT survival rates are excellent despite the risk. Abstract

Background: Prevalence of mobile device addiction has increased over the years; both women and men have assimilated the mobile phone as a central component of their personal existence: integrating it into their lifestyle or becoming so dependent on it that life without it has become unimaginable. Smartphones generate radio-frequency electromagnetic fields. While short-term exposure in adults was considered quite safe, effects of long-term exposure or exposure during pregnancy on fetuses or during breastfeeding on newborns are not well studied yet. The objective of the present study was to investigate the prevalence and usage characteristics of smartphones among a sample of pregnant women, and promote the correct and conscious use of the smartphone., Methods: A cross-sectional study was conducted, with a questionnaire administered during childbirth classes and - after the questionnaire administration - an educational intervention focused on promoting the correct and conscious use of smartphones was carried out by psychologists and psychotherapists., Results: The findings of our study suggest that a significant number of the participants suffered addiction to mobile phone usage, but were not aware of it. More than two third of the sample (67.2%) have not changed their smartphone use habits since the beginning of their pregnancy and even more significant data shows that almost all future moms (98.3%) never speak with their doctor about smartphone use during pregnancy., Conclusions: Data collected suggest a lack of attention to the proposed topic, especially in relation to pregnancy. It seems necessary to sensitize future mothers on this topic. The promotion of a more conscious and controlled use of electronic devices can help reduce the radiation to which the unborn child may be exposed, but has a fundamental role even after birth, to ensure an adequate psychomotor and relational development of the child and do not affect, due to uncontrolled use of smartphones, the mother-child relationship.

21. A scoping review of measures examining the environments of persons with neurodevelopmental disorders in a supported housing setting.

Abstract

Introduction: Having a neurodevelopmental disorder (NDD) can impact the abilities of an individual in many areas of life, including the ability to live independently. The environment of an individual impacts their day-to-day life throughout their lifespan. To improve supported housing experiences, it is important to map the evidence, especially relating to quality and satisfaction with the environment (as defined by the International Classification of Functioning, Disability and Health (ICF) framework). This has been exacerbated by COVID-19 pandemic restrictions, therefore more insight is needed in measuring this. Objective and methods: This scoping review searched 5 health and social science databases with the objective to identify and examine the outcome measures that integrate aspects of the environment that examine supported housing in individuals with NDD. Results and discussion: Fifteen studies met the inclusion criteria. We found that most measures targeted the satisfaction of individuals about their environment, with the ICF Support and Relationships factor of the environment assessed most. Measures were most often completed by a proxy through an interview. 22. Acute-on-Chronic Liver Failure in Cirrhosis.

Abstract

Acute-on-chronic liver failure (ACLF) is a syndrome that develops in patients with acutely decompensated chronic liver disease. It is characterised by high 28-day mortality, the presence of one or more organ failures (OFs) and a variable but severe grade of systemic inflammation. Despite the peculiarity of each one, every definition proposed for ACLF recognizes it as a proper clinical entity. In this paper, we provide an overview of the diagnostic criteria proposed by the different scientific societies and the clinical characteristics of the syndrome. Established and experimental treatments are also described. Among the former, the most relevant are directed to support organ failures, treat precipitating factors and carry out early assessment for liver transplantation (LT). Further studies are needed to better clarify pathophysiology of the syndrome and discover new therapies.

Abstract

Large numbers of experimental toolmarks of screwdrivers are often required in casework of toolmark examiners and in research environments alike, to be able to recover the angle of attack of a crime scene mark and to determine statistically meaningful properties of toolmarks respectively. However, in practice the number of marks is limited by the time needed to create them. In this article, we present an approach to predict how a striated mark of a particular tool would look like, using 3D surface datasets of screwdrivers. We compare these virtual toolmarks qualitatively and quantitatively with real experimental marks in wax and show that they are very similar. In addition we study toolmark similarity, dependent on the angle of attack, with a very high angular resolution of 1°. The results show that for the tested type of screwdriver, our toolmark comparison framework yields known match similarity scores that are above the mean known non-match similarity scores, even for known match differences in angle of attack of up to 40°. In addition we demonstrate an approach to automatically recover the angle of attack of an experimental toolmark and experiments yield high accuracy and precision of 0.618 ± 4.179°. Furthermore, we present a strategy to study the structural elements of striated toolmarks using wavelet analysis, and show how to use the results to simulate realistic toolmarks., (Copyright © 2016 Elsevier Ireland Ltd.
