Celiac disease (CD) is a chronic gastrointestinal disorder that is most frequently found in Western countries, and the only treatment available today is a lifelong gluten-free diet (GFD). The main aim of the present study is to compare the self-care practices and body mass index (BMI) of adolescents with CD and without CD in different dimensions (physical, psychological, social, and management). The non-probabilistic sample included 236 participants (118 with CD) aged between 12 and 16 years old, who were part of the project "Role of the family in the perception of youth self-care". The data were obtained through the self-administration of the Practice and Management of Youth Self-care questionnaire, while the celiac population also completed the Adherence to a Gluten-Free Diet (GFD) questionnaire. The celiac population analyzed showed significant correlations between the Physical Practices and Management with adherence to a GFD, and between a GFD and Psychological Practices. Differences were observed between the celiac and the non-celiac populations in the dimensions of Physical Practices, Social Practices, and Management. With respect to BMI, age and CD showed a significant influence of a GFD on BMI (p < 0.001). In conclusion, the application of multi-dimensional questionnaires and their relationship with the adherence to a GFD provide valuable information to propose interventions directed to this population. Publicidad de alimentos; Niños; Conducta alimentaria Food advertising; Children; Eating behavior Publicitat d'aliments; Nens; Conducta alimentària Introduction Some important factors influencing and maintaining unhealthy habits are food advertising and products accessibility. In order to develop and support recommendations, an analysis of the available evidence on the impact of food advertising on the health of children and adolescents has been carried out. Methods Literature review of systematic reviews and meta-analyses published up to January 2022 for the term “food advertising” that analyzed the impact of food advertising on weight, body mass index (BMI), adiposity, dietary intake, behavior toward the advertised product, its purchase or consumption in children and adolescents. Results Twenty-one systematic reviews fulfilled the inclusion criteria, including a total of 490 primary studies, 5 of which also contained a meta-analysis. The vast majority of the primary studies evaluate intermediate effects, related to the behavior of children and adolescents in relation to advertised products and their consumption. There is great variety in terms of the type of advertising and effects studied. Most of the studies agree that there is an association between food advertising and effect analyzed, being more evident in children under 12 years of age and in obese children. Most recent systematic reviews are focused on online advertising, noticing the negative effects especially in adolescents. Conclusions Children and adolescents are a particularly vulnerable population to food advertising strategies. Despite the difficulty to demonstrate an independent effect, there is evidence of an association between food advertising and childhood and adolescents’ behavior respect to the announced products, and the increase of consumption at short-term. In Spain unhealthy product advertising are still very common in the media and in the children and adolescent's online environment. The Nutrition and Breastfeeding Committee of the Spanish Association of Pediatrics supports the need for regulation and limitation of unhealthy food advertising, covering all media and marketing strategies. Introducción Entre los factores más importantes que influyen en la aparición y el mantenimiento de malos hábitos de alimentación están la accesibilidad y publicidad de los productos alimentarios menos saludables. Con el objetivo de elaborar y fundamentar recomendaciones, se ha realizado un análisis de la evidencia disponible sobre el impacto de la publicidad de alimentos en la salud de niños y adolescentes. Métodos Se ha realizado una revisión bibliográfica de revisiones sistemáticas y metanálisis publicados hasta enero de 2022 con el término “food advertising”, incluyendo aquellas que analizaban el impacto de la publicidad de alimentos sobre el peso, índice de masa corporal (IMC), adiposidad, ingesta dietética, conducta ante el producto anunciado, su compra o su consumo en niños y adolescentes. Resultados Fueron incluidas 21 revisiones sistemáticas que incluyen un total de 490 artículos, 5 de las cuales contienen además un meta-análisis. La gran mayoría de los estudios primarios evalúan efectos intermedios, relacionados con el comportamiento de niños y adolescentes ante los productos anunciados y su consumo. Existe gran variedad en cuanto al tipo de publicidad y efectos estudiados. La mayoría de trabajos muestra una asociación entre el tipo de publicidad y el efecto concreto analizado, siendo más evidente en menores de 12 años y en niños obesos. Las revisiones más recientes se centran en la publicidad online indicando sus efectos nocivos especialmente en adolescentes. Conclusiones Los niños y adolescentes constituyen una población especialmente vulnerable. A pesar de la dificultad para demostrar un efecto independiente, hay evidencia de asociación entre la publicidad y los comportamientos en la infancia y adolescencia respecto a los productos anunciados y el aumento de su consumo a corto plazo. En España, los anuncios de productos no saludables siguen muy presentes en los medios y en el entorno online de los menores. El Comité de Nutrición y Lactancia Materna de la Asociación Española de Pediatría suscribe la necesidad de una regulación y limitación de la publicidad de alimentos no saludables que abarque todos los medios y todas las estrategias de marketing.

Exclusive enteral nutrition (EEN) has been shown to be more effective than corticosteroids in achieving mucosal healing in children with Crohn´, s disease (CD) without the adverse effects of these drugs. The aims of this study were to determine the efficacy of EEN in terms of inducing clinical remission in children newly diagnosed with CD, to describe the predictive factors of response to EEN and the need for treatment with biological agents during the first 12 months of the disease. We conducted an observational retrospective multicentre study that included paediatric patients newly diagnosed with CD between 2014&ndash, 2016 who underwent EEN. Two hundred and twenty-two patients (140 males) from 35 paediatric centres were included, with a mean age at diagnosis of 11.6 ±, 2.5 years. The median EEN duration was 8 weeks (IQR 6.6&ndash, 8.5), and 184 of the patients (83%) achieved clinical remission (weighted paediatric Crohn&rsquo, s Disease activity index [wPCDAI] <, 12.5). Faecal calprotectin (FC) levels (&mu, g/g) decreased significantly after EEN (830 [IQR 500&ndash, 1800] to 256 [IQR 120&ndash, 585] p <, 0.0001). Patients with wPCDAI &le, 57.5, FC <, 500 &mu, g/g, CRP >, 15 mg/L and ileal involvement tended to respond better to EEN. EEN administered for 6&ndash, 8 weeks is effective for inducing clinical remission. Due to the high response rate in our series, EEN should be used as the first-line therapy in luminal paediatric Crohn&rsquo, s disease regardless of the location of disease and disease activity.

Introduction:Parenteral nutrition (PN) in childhood is a treatment whose characteristics are highly variable depending on the age and pathology of the patient. Material and methods: The Standardization and Protocols Group of the Spanish Society for Parenteral and Enteral Nutrition (SENPE) is an interdisciplinary group formed by members of the SENPE, the Spanish Society of Gastroenterology, Hepatology and Pediatric Nutrition (SEGHNP) and the Spanish Society of Hospital Pharmacy (SEFH) that intends to update this issue. For this, a detailed review of the literature has been carried out, looking for the evidences that allow us to elaborate a Clinical Practice Guide following the criteria of the Oxford Center for Evidence-Based Medicine. Results: This manuscript summarizes the recommendations regarding indications, access routes, requirements, modifi cations in special situations, components of the mixtures, prescription and standardization, preparation, administration, monitoring, complications and home NP. The complete document is published as a monographic number. Conclusions: This guide is intended to support the prescription of pediatric PN. It provides the basis for rational decisions in the context of the existing evidence. No guidelines can take into account all of the often compelling individual clinical circumstances.

Background and objective: there are few studies on the prevalence of metabolic syndrome (MetS) in European adolescent populations, and some have reported a higher prevalence in the Mediterranean basin area. Our objective was to examine the prevalence of MetS in adolescents in a Mediterranean city of Spain, comparing two different definitions of MetS and the associated risk factors. Methods and results: a cross-sectional population-based study was conducted among 379 adolescents aged 12-16.9 years, selected using a random sampling method. Anthropometric measurements and fasting blood samples were obtained. The definitions of MetS used were that of the National Cholesterol Education Program's Adult Treatment Panel III (NCEP-ATPIII) and that of the International Diabetes Federation (IDF). Kappa coefficient was used to measure the agreement between definitions and a multivariate logistic regression model to determine the associated risk factors. Results: the prevalence of MetS was 5.7% (95%CI 3.33-8.07) according to the NCEP-ATPIII definition and 3.8% (95%CI 1.85-5.75) according to the IDF definition. No differences between the sexes or by age groups were found. The agreement between the two definitions was very good (kappa 0.815), especially in the obese subsample, but was lower in normal weight adolescents (kappa 0.497). Insulin resistance and obesity were associated with both definitions. Conclusions: the prevalence of MetS in our adolescent population is higher than the European media. Although the overall agreement between both definitions was very good, the prevalence was higher using the NCEP-ATPIII criteria. Independently of the definition used, obesity and insulin resistance were risk factors for MetS. Introducción y objetivos: existen escasos estudios acerca de la prevalencia de síndrome metabólico (MetS) en la población general adolescente en Europa. Algunos resultados muestran mayor prevalencia en adolescentes del área mediterránea. Nuestro objetivo fue estudiar la prevalencia de MetS en la población general adolescente de una ciudad del área mediterránea en España, comparando dos definiciones de MetS y los factores de riesgo asociados. Material y métodos: estudio epidemiológico observacional sobre una muestra de base poblacional, elegida de forma aleatoria, representativa de los adolescentes de 12 a 16,9 años escolarizados en la ciudad de Almería. Se recogieron variables antropométricas y analíticas. Se compararon dos definiciones de SM para población adolescente: National Cholesterol Education Program (NECP- ATPIII) e International Diabetes Federation (IDF). Estadística: coeficiente kappa para analizar la concordancia entre definiciones y regresión logística múltiple para el estudio de factores de riesgo asociados. Resultados: la prevalencia de MetS fue 5,7% (95%IC 3,33-8,07) con la definición NECP-ATPIII y 3,8% (95%IC 1,85-5,75) con los criterios IDF. No se encontraron diferencias entre sexos ni entre grupos de edad. La concordancia entre ambas definiciones fue muy buena a nivel global (kappa 0,815) y especialmente en el grupo de obesos, pero empeoró entre adolescentes con normopeso (kappa 0,497). Los factores asociados a ambas definiciones fueron obesidad y resistencia insulínica. Conclusiones: nuestros resultados muestran una prevalencia de MetS en adolescentes mayor a la media europea. Aunque la concordancia entre definiciones fue muy buena a nivel global, la prevalencia fue mayor con la definición NECP-ATPIII. Obesidad y resistencia insulínica fueron los factores de riesgo asociados.

Objetivo: Analizar los factores asociados con insulinemia en ayunas en población general adolescente. Métodos: Estudio epidemiológico observacional sobre una muestra poblacional de adolescentes (12-16,9 años) seleccionados mediante muestreo probabilístico polietápico. Se realizaron examen físico, antropometría, encuesta nutricional y se determinó insulinemia en ayunas. Estadística: regresión lineal múltiple. Resultados: Se incluyeron 379 adolescentes con edad media 14,08 ± 1,30 años. Se relacionaron con mayores cifras de insulinemia el desarrollo puberal [β 4,55 (95% IC 0,42-8,69)], obesidad abdominal [β 6,11(95% IC 3,938,29)] y el antecedente de bajo peso para la edad gestacional (BPEG) [β 7,45 (95% IC 2,47-12,44)]. El consumo habitual de aceite de oliva en domicilio mostró una relación inversa con las cifras de insulinemia [β -4,14 (95% IC -7,31- -0,98)]. Conclusión: Los factores de riesgo asociados a mayores cifras de insulinemia en ayunas fueron la obesidad abdominal y el antecedente de BPEG. El consumo habitual de aceite de oliva en domicilio fue un factor protector.
