Abstract

PREdiction of NOn-LINear soil behavior (PRENOLIN) is an international benchmark aiming to test multiple numerical simulation codes that are capable of predicting nonlinear seismic site response with various constitutive models. One of the objectives of this project is the assessment of the uncertainties associated with nonlinear simulation of 1D site effects. A first verification phase (i.e., comparison between numerical codes on simple idealistic cases) will be followed by a validation phase, comparing the predictions of such numerical estimations with actual strongmotion recordings obtained at well-known sites. The benchmark presently involves 21 teams and 23 different computational codes. We present here the main results of the verification phase dealing with simple cases. Three different idealized soil profiles were tested over a wide range of shear strains with different input motions and different boundary conditions at the sediment/bedrock interface. A first iteration focusing on the elastic and viscoelastic cases was proved to be useful to ensure a common understanding and to identify numerical issues before pursuing the nonlinear modeling. Besides minor mistakes in the implementation of input parameters and output units, the initial discrepancies between the numerical results can be attributed to (1) different understanding of the expression “input motion” in different communities, and (2) different implementations of material damping and possible numerical energy dissipation. The second round of computations thus allowed a convergence of all teams to the Haskell–Thomson analytical solution in elastic and viscoelastic cases. For nonlinear computations, we investigate the epistemic uncertainties related only to wave propagation modeling using different nonlinear constitutive models. Such epistemic uncertainties are shown to increase with the strain level and to reach values around 0.2 (log10 scale) for a peak ground acceleration of 5 m=s2 at the base of the soil column, which may be reduced by almost 50% when the various constitutive models used the same shear strength and damping implementation.

Abstract

Background Biological drugs have improved the management of immune-mediated inflammatory diseases (IMIDs) despite being associated with important safety issues such as immunogenicity, infections, and malignancies in real-world settings. Objective The aim of this study was to explore the potential of a large Italian multi-database distributed network for use in the postmarketing surveillance of biological drugs, including biosimilars, in patients with IMID. Methods A retrospective cohort study was conducted using 13 Italian regional claims databases during 2010–2019. A tailor-made R-based tool developed for distributed analysis of claims data using a study-specific common data model was customized for this study. We measured the yearly prevalence of biological drug users and the frequency of switches between originator and biosimilars for infliximab, etanercept, and adalimumab separately and stratified them by calendar year and region. We then calculated the cumulative number of users and person-years (PYs) of exposure to individual biological drugs approved for IMIDs. For a number of safety outcomes (e.g., severe acute respiratory syndrome coronavirus 2 [SARS-COV-2] infection), we conducted a sample power calculation to estimate the PYs of exposure required to investigate their association with individual biological drugs approved for IMIDs, considering different strengths of association. Results From a total underlying population of almost 50 million inhabitants from 13 Italian regions, we identified 143,602 (0.3%) biological drug users, with a cumulative exposure of 507,745 PYs during the entire follow-up. The mean age ± standard deviation of biological drug users was 49.3 ± 16.3, with a female-to-male ratio of 1.2. The age-adjusted yearly prevalence of biological drug users increased threefold from 0.7 per 1000 in 2010 to 2.1 per 1000 in 2019. Overall, we identified 40,996 users of biosimilars of tumor necrosis factor (TNF)-α inhibitors (i.e., etanercept, adalimumab, and infliximab) in the years 2015–2019. Of these, 46% (N = 18,845) switched at any time between originator and biosimilars or vice versa. To investigate a moderate association (incidence rate ratio 2) between biological drugs approved for IMIDs and safety events of interest, such as optic neuritis (lowest background incidence rate 10.4/100,000 PYs) or severe infection (highest background incidence rate 4312/100,000 PYs), a total of 43,311 PYs and 104 PYs of exposure to individual biological drugs, respectively, would be required. As such, using this network, of 15 individual biological drugs approved for IMIDs, the association with those adverse events could be investigated for four (27%) and 14 (93%), respectively. Conclusion The VALORE project multi-database network has access to data on more than 140,000 biological drug users (and > 0.5 million PYs) from 13 Italian regions during the years 2010–2019, which will be further expanded with the inclusion of data from other regions and more recent calendar years. Overall, the cumulated amount of person-time of exposure to biological drugs approved for IMIDs provides enough statistical power to investigate weak/moderate associations of almost all individual compounds and the most relevant safety outcomes. Moreover, this network may offer the opportunity to investigate the interchangeability of originator and biosimilars of several TNFα inhibitors in different therapeutic areas in real-world settings. Abstract

A previous Delphi survey from the Rational Use of Analgesics (RUA) project involving Italian palliative care specialists revealed some discrepancies between current guidelines and clinical practice with a lack of consensus on items regarding the use of strong opioids in treating cancer pain. Those results represented the basis for a new Delphi study addressing a better approach to pain treatment in patients with cancer. The study consisted of a two-round multidisciplinary Delphi study. Specialists rated their agreement with a set of 17 statements using a 5-point Likert scale (0 = totally disagree and 4 = totally agree). Consensus on a statement was achieved if the median consensus score (MCS) (expressed as value at which at least 50% of participants agreed) was at least 4 and the interquartile range (IQR) was 3–4. This survey included input from 186 palliative care specialists representing all Italian territory. Consensus was reached on seven statements. More than 70% of participants agreed with the use of low dose of strong opioids in moderate pain treatment and valued transdermal route as an effective option when the oral route is not available. There was strong consensus on the importance of knowing opioid pharmacokinetics for therapy personalization and on identifying immediate-release opioids as key for tailoring therapy to patients’ needs. Limited agreement was reached on items regarding breakthrough pain and the management of opioid-induced bowel dysfunction. These findings may assist clinicians in applying clinical evidence to routine care settings and call for a reappraisal of current pain treatment recommendations with the final aim of optimizing the clinical use of strong opioids in patients with cancer.

Abstract

This article presents the main results of the validation phase of the PRENOLIN project. PRENOLIN is an international benchmark on 1D nonlinear (NL) site-response analysis. This project involved 19 teams with 23 different codes tested. It was divided into two phases; with the first phase verifying the numerical solution of these codes on idealized soil profiles using simple signals and real seismic records. The second phase described in this article referred to code validation for the analysis of real instrumented sites. This validation phase was performed on two sites (KSRH10 and Sendai) of the Japanese strong-motion networks KiK-net and Port and Airport Research Institute (PARI), respectively, with a pair of accelerometers at surface and depth. Extensive additional site characterizations were performed at both sites involving in situ and laboratory measurements of the soil properties. At each site, sets of input motions were selected to represent different peak ground acceleration (PGA) and frequency content. It was found that the code-to-code variability given by the standard deviation of the computed surface-response spectra is around 0.1 (in log10 scale) regardless of the site and input motions. This indicates a quite large influence of the numerical methods on site-effect assessment and more generally on seismic hazard. Besides, it was observed that sitespecific measurements are of primary importance for defining the input data in siteresponse analysis. The NL parameters obtained from the laboratory measurements should be compared with curves coming from the literature. Finally, the lessons learned from this exercise are synthesized, resulting also in a few recommendations for future benchmarking studies, and the use of 1D NL, total stress site-response analysis. © 2018, Seismological Society of America. Abstract

Wave barriers (open or in-filled trenches) are often used in engineering practice to reduce ground vibrations caused by industrial activities or transportation systems. Typically, most of the energy is transmitted in the form of Rayleigh waves so that information related to their propagation can be very useful in the design process. This technical note presents a case study in which both active and passive surface wave tests have been used in the design of a vibration mitigation barrier. It is shown that several pieces of information can be inferred from these tests for specific applications: source characterization in terms of dominant frequencies and direction of propagation; dominant wavelength for the preliminary design; and soil characterization for the construction of a numerical model aimed at analyzing the performance of the wave barrier. Abstract

The Central Italy earthquake sequence nominally began on 24 August 2016 with a M6.1 event on a normal fault that produced devastating effects in the town of Amatrice and several nearby villages and hamlets. A major international response was undertaken to record the effects of this disaster, including surface faulting, ground motions, landslides, and damage patterns to structures. This work targeted the development of high-value case histories useful to future research. Subsequent events in October 2016 exacerbated the damage in previously affected areas and caused damage to new areas in the north, particularly the relatively large town of Norcia. Additional reconnaissance after a M6.5 event on 30 October 2016 documented and mapped several large landslide features and increased damage states for structures in villages and hamlets throughout the region. This paper provides an overview of the reconnaissance activities undertaken to document and map these and other effects, and highlights valuable lessons learned regarding faulting and ground motions, engineering effects, and emergency response to this disaster.
