About a third of babies are delivered by Cesarean section. There has been an increase in maternal deaths during labor due to complications with subsequent births after a C-section. Therefore, there is a clinical motivation to reduce the C-section rate. Current techniques are, however, inefficient at determining fetal distress leading to a high false positive rate for complications and ultimately a C-section. For the current study, Monte Carlo simulations were used to calculate the amount of signal received on a model of a pregnant mother, as well as, the percent of the signal that comes from the fetal layer. Models with and without a 1 mm amniotic fluid were compared and showed differing trends.

Non-invasive transabdominal fetal pulse oximetry (TFO) can provide physicians with a better metric of fetal well-being than currently-used fetal monitoring methods. There are two major challenges in this light-based measurement modality. One is in detecting the weak fetal signal at the surface of the maternal abdomen, and the other is in recovering the fetal signal from the diffuse reflectance measured, which contains a mixture of information about both maternal and fetal tissue. In this paper, we describe the TFO system we developed and evaluate its ability to recover the fetal signal in a preclinical setting. In particular, we assess its capability to measure the fetal signal over several thicknesses of maternal tissue and gauge the effectiveness of different approaches in removing the maternal influence from the mixed signal. Our results show that our TFO system, built using commodity low-cost components, can measure the highly-attenuated fetal signal through maternal tissue as thick as 5 cm. Furthermore, we determine that adaptive noise cancellation techniques are the most effective for separating the fetal signal from the maternally-influenced noise. These findings strongly support our TFO system's ability to perform well on a variety of different pregnant body types.

Transabdominal fetal pulse oximetry is an approach to measure oxygen saturation of the unborn child non-invasively. The principle of pulse oximetry is applied to the abdomen of a pregnant woman, such that the measured signal includes both, the maternal and the fetal pulse curve. One of the major challenges is to extract the shape of the fetal pulse curve from the mixed signal for computation of the oxygen saturation. In this paper we analyze the principle kind of connection of the fetal and maternal pulse curves in the measured signal. A time varying finite element model is used to rebuild the basic measurement environment, including a bulk tissue and two independently pulsing arteries to model the fetal and maternal blood circuit. The distribution of the light fluence rate in the model is computed by applying diffusion equation. From the detectors we extracted the time dependent fluence rate and analyzed the signal regarding its components. The frequency spectra of the signals show peaks at the fetal and maternal basic frequencies. Additional signal components are visible in the spectra, indicating multiplicative coupling of the fetal and maternal pulse curves. We conclude that the underlying signal model of algorithms for robust extraction of the shape of the fetal pulse curve, have to consider additive and multiplicative signal coupling.

OBJECTIVE: To assess the association of cervical effacement with the rate of intrapartum cervical change among nulliparous women. METHODS: We conducted a secondary analysis of a prospective trial of intrapartum fetal pulse oximetry. For women who had vaginal deliveries, interval-censored regression was used to estimate the time to dilate at 1-cm intervals. For each given centimeter of progressive cervical dilation, women were divided into those who had achieved 100% cervical effacement and those who had not. The analysis was performed separately for women in spontaneous labor and those who were given oxytocin. RESULTS: A total of 3,902 women were included in this analysis, 1,466 (38%) who underwent labor induction, 1,948 (50%) who underwent labor augmentation (combined for the analysis), and 488 (13%) who labored spontaneously. Transabdominal fetal pulse oximetry is a method to monitor the oxygen supply of the unborn child non-invasively. Due to the measurement setup, the received signal of the detector is composed of photons coding purely maternal and photons coding mixed fetal-maternal information. To analyze the wellbeing of the fetus, the fetal signal is extracted from the mixed component. In this paper we assess source-detector configurations, such that the mixed fetal-maternal components of the acquired signals are maximized. Monte-Carlo method is used to simulate light propagation and photon distribution in tissue. We use a plane layer and a spherical layer geometry to model the abdomen of a pregnant woman. From the simulations we extracted the fluence at the detector side for several source-detector distances and analyzed the ratio of the mixed fluence component to total fluence. Our simulations showed that the power of the mixed component depends on the source-detector distance as expected. Further we were able to visualize hot spot areas in the spherical layer model where the mixed fluence ratio reaches the highest level. The results are of high importance for sensor design considering signal composition and quality for non-invasive fetal pulse oximetry.

The vast majority of pregnant women in the United States are subjected to electronic fetal heart monitoring during labor. There is limited evidence to support its benefit compared with intermittent auscultation. In addition, there is significant variability in interpretation and its false positive rate is high. The latter may have contributed to the rise in operative deliveries. In order to address the critical need for better approaches to intrapartum monitoring, the MFMU Network has completed 2 large multi-site randomized trials, one to evaluate fetal pulse oximetry and the other to evaluate fetal ECG ST segment analysis (STAN). Both of these technologies had been approved for clinical use in the U.S. based on prior smaller trials. These technologies were evaluated in laboring women near term and their primary outcomes were overall cesarean delivery for the oximetry trial and a composite adverse neonatal outcome for STAN. Both trials failed to show a benefit of the technology, neither in the rates of operative deliveries nor in the rates of adverse neonatal outcomes. The experience with these trials, summarized in this report, highlights the need for rigorous evidence before introduction of new technology into clinical practice and provide a blueprint for future such trials to address the need for better intrapartum monitoring approaches.

OBJECTIVE: To investigate the role of fetal pulse oximetry on cesarean section rates and neonatal outcomes in pregnant women with an indication of induction of labour due to intrauterine growth restriction. MATERIAL-METHOD: Pregnant women who have been planned induction of labour with 50 mcg sublingually misoprostol due to intrauterine growth restriction were assigned into two groups. 46 women in the first group were assessed with fetal heart rate monitoring alone during labour and a cesarean section due to fetal distress was performed according to fetal heart rate monitoring (group 1). 45 women in the second group were assessed with fetal pulse oximetry plus fetal heart rate monitoring and a cesarean section due to fetal distress was performed according to fetal oxygen saturations recorded by fetal pulse oximetry (group 2). Induction to delivery interval, intrapartum variables, Apgar scores, cord blood gas analysis and early neonatal outcomes were compared in between the groups. ACOG also suggests, however, that "this technique, while occa- sionally helpful, is used uncommonly in current obstetric practice." 1 In this 2-part review, we will focus on intrapartum evaluation of fetal well being and critically examine the useful- ness of fetal scalp sampling (FSS) in com- parison to other available methods of intrapartum fetal assessment, including fetal scalp stimulation, vibroacoustic stim- ulation, fetal pulse oximetry, and scalp blood lactate concentration. We will also define those settings in which we believe the risk and/or expense of FSS evaluation is warranted.

Pulse oximetry is a technique for estimating arterial oxygen saturation continuously and non-invasively. Reflectance pulse oximetry might become useful for monitoring the fetus during labour but it is much more susceptible to all kinds of physiological variations than the well-established transmission pulse oximetry for neonatal or adult monitoring. This review focuses on the accuracy of reflectance pulse oximetry. Results of human, animal, in vitro and theoretical models indicate that factors such as; blood volume fraction differences, haematocrit, and blood flow differences are major sources for inaccurate pulse oximetry readings in the fetal arterial oxygen saturation range of 10-80%. These factors cannot be overcome by systems using two wavelengths sensors with the 660/890 or 940 nm combination. Reported precision values (S.D. of difference between pulse oximeter and blood sample saturation) range between 2.5 and 12.9% for various 660 nm sensors. Most sensors were tested only once with a limited number of animals. A new 735/890 nm sensor (Nellcor Puritan Bennett) demonstrates a promising accuracy (precision around 5%) in two studies. Various other sensors have also been developed, but are not or scarcely evaluated. Without thorough establishment of the reliability of this technique, clinical fetal oxygen saturation data are still of limited value.

Τα τελευταία χρόνια τόσο σε διεθνές όσο και σε εθνικό επίπεδο έχει δοθεί βαρύτητα στην ανίχνευση της εμβρυϊκής υποξίας κατά τη διάρκεια της κύησης και του τοκετού, καθώς είναι σαφής η άμεση συσχέτισή της με βραχυπρόθεσμες και απώτερες επιπλοκές του νεογνού. Η επιτυχής έκβαση ενός τοκετού εξαρτάται κυρίως από τον έγκαιρο εντοπισμό της δημιουργίας της εμβρυϊκής υποξίας και η ανάγκη για την υλοποίηση υπολογιστικών συστημάτων για την έγκαιρη διάγνωσή της είναι συνεχής και αυξανόμενη. Στα πλαίσια της διδακτορικής διατριβής σχεδιάστηκε και αναπτύχθηκε ένα πρωτότυπο σύστημα διάγνωσης της πρόωρης οξέωσης του εμβρύου κατά τη διάρκεια του τοκετού, το οποίο βασίστηκε στην επεξεργασία του εμβρυϊκού καρδιακού ρυθμού και στην ανάλυση της εμβρυϊκής παλμικής οξυμετρίας. Στόχος του συγκεκριμένου συστήματος είναι να αποτελέσει ένα βοηθητικό σύστημα διάγνωσης της εμβρυϊκής υποξίας και να συμβάλει στη λήψη αποφάσεων σχετικά με το χρόνο αποπεράτωσης του τοκετού, με σκοπό την πρόληψη άμεσων και απώτερων σοβαρών νεογνικών επιπλοκών. Ειδικότερα, η υλοποίηση του προτεινόμενου συστήματος βασίστηκε στην εφαρμογή του συνεχούς μετασχηματισμού κυματιδίων και της προσαρμοστικής προσέγγισης με τη χρήση του αλγορίθμου matching pursuit για την ανάδειξη της «κρυμμένης» πληροφορίας που μεταφέρει ο εμβρυϊκός καρδιακός ρυθμός στις πολύ χαμηλές συχνότητες. Συνδυάζοντας τα αποτελέσματα των παραπάνω τεχνικών επεξεργασίας, και ιδιαίτερα του αλγορίθμου matching pursuit, με τον προσδιορισμό του συνολικού χρόνου χαμηλού κορεσμού του αρτηριακού εμβρυϊκού αίματος, υλοποιήθηκε, με τη χρήση ενός διαμεριστικού αλγορίθμου, το προτεινόμενο σύστημα. In the last few years the research community has given great attention to the detection of antepartum and intrapartum fetal hypoxia, given its direct impact on both short- and long-term neonatal morbidity and mortality. The successful completion of labor depends mainly on the prompt identification of fetal hypoxia. The development of computational systems for the early diagnosis of restricted fetal oxygen supply is therefore of critical importance. This thesis involves the design and development of an innovative system for the early detection of acidosis, which was based on the fetal heart rate processing and fetal pulse oximetry analysis. The aim of the system is to comprise a computer-aided diagnostic system of fetal hypoxia and to contribute to the decision making regarding the labor completion time, in order to prevent short- and long-term neonatal complications. Specifically, the development of the system was based on the implementation of continuous wavelet transform and adaptive approximation using the matching pursuit algorithm, in order to reveal the “hidden” information conveyed in the very low frequency range of the fetal heart rate. The system involves the combination of the results of the above-mentioned processing techniques, and especially of the matching pursuit algorithm, along with the calculation of the duration of fetal arterial low oxygen saturation, applying a commonly used clustering algorithm. The proposed system is an innovative and promising approach towards the early diagnosis of fetal hypoxia, given its high specificity and positive predictive value, thus effectively addressing the major drawback of clinical cardiotocography.

To evaluate the accuracy of intrapartum foetal pulse oximetry (SO(2POX)) using reusable sensors and the effect of a sensor performance test on data quality. Furthermore, to assess the sensor-related costs by using reusable sensors and sensor performance test.36 reusable sensors were used for SO(2POX) during labour of 289 term foetuses. A sensor performance test device assessing the emitter and receiver capability and the firmness of attachment of the sensors had been developed and used in the last 134 measurements before each resterilisation. Oxygen saturation (SaO(2)) at birth was measured spectrophotometrically after cord blood sampling. The accuracy of SO(2POX) was evaluated by analysing its relationship to SaO(2). The valid SO(2POX) data, as confirmed by subsequent sensor test in the second group, was considered comparable with those with single sensor use. Sensor-related average cost (sensors, test device and sterilisation) of such measurements was compared with that of single sensor use.Eight sensors failed performance test despite valid pulse oximetry signal output during their last measurements. There were significant overall linear correlations between SO(2POX) and SaO(2) (r=0.45, P0.0001). Separate analyses of regression in the group without sensor performance testing showed an r(2) of 0.41, whereas in the group with subsequent sensor performance testing, the r(2) was 0.52 (P0.05). By reusing the sensors, the sensor-related cost per valid measurement was $18.9 and 71% lower compared to single use of sensors ($65).Pulse oximetry may reflect fetal oxygen saturation. Data quality may be compromised by insufficient sensor performance, even though the reflection signal quality is acceptable. If sensor performance is tested before each measurement, reusable sensors may reduce the costs of fetal pulse oximetry.

Objetivo: estudar a correlação entre a saturação de oxigênio fetal medida pela oximetria de pulso durante o segundo período do parto e o pH da artéria umbilical ao nascimento. Pacientes e Métodos: a saturação de oxigênio fetal (FSpO2) foi monitorizada pela oximetria de pulso durante o segundo período do trabalho de parto em 64 gestações simples a termo com apresentação de vértice. O sangue dos vasos umbilicais foi obtido imediatamente após o nascimento para subseqüente medida dos gases e do pH venoso e arterial. Todos os fetos mantiveram a FSpO2 > ou = 30% durante o primeiro período do parto, até o início do segundo período. Objetivo: estudar a sensibilidade e a especificidade dos valores de saturação de oxigênio fetal (SpO2) e padrões da freqüência cardíaca fetal (FCF) durante o parto, no prognóstico de fetos acidóticos ao nascimento. Pacientes e Métodos: os valores fetais de SpO2 foram obtidos pela técnica da oximetria de pulso. Um valor de SpO2 > ou = 30% foi considerado normal, e 10 minutos no intervalo entre contrações, foi considerado anormal. A SpO2 fetal e os traçados de FCF foram obtidos continuamente no primeiro e segundo períodos do parto. A classificação utilizada para a FCF foi a do NICHD19. Resultados: um total de 72 casos foram estudados. A sensibilidade e especificidade com base na SpO2 fetal foram respectivamente de 61,5% e 96,6%, ao passo que a sensibilidade e especificidade baseadas nos padrões de FCF foram respectivamente 69,2% e 66,1%. Os valores preditivos positivo e negativo em função da SpO2 fetal foram respectivamente 80% e 91,9%; em função dos padrões de FCF foram respectivamente 31% e 90,7%. Conclusões: uma boa especificidade da SpO2 para o prognóstico de recém-nascidos acidóticos foi encontrada, se comparada com a especificidade dos padrões de FCF, ao passo que a sensibilidade foi relativamente baixa para os dois métodos. Entretanto, o número de fetos acidóticos é muito pequeno para conclusões. Objetivos: estudar os níveis de saturação de oxigênio fetal (SpO2) durante o trabalho de parto pela técnica da oximetria de pulso e sua relação com o pH da artéria umbilical (AU). Pacientes e Métodos: a SpO2 fetal foi medida durante o parto por meio da técnica da oximetria de pulso em 50 casos. Comparou-se a média dos valores de SpO2 entre os dois períodos do trabalho de parto, sendo o primeiro subdividido em fases, segundo a dilatação cervical ( ou = 7,20 e ou = 30,0%. Objective : To determine if oxygen saturation measurement with pulse oximetry (Sp o 2 ) in combination with cardiotocograghy (CTG), improves the assessment of the intrapartum fetal condition. Study Design : Four expert obstetricians individually evaluated 119 cases that were monitored during labor: during the first session the CTG data were available, and in the second session CTG and Sp o 2 data were evaluated. They were instructed to indicate the need for intervention and to estimate the umbilical artery pH. Results : In the non-acidotic group (umbilical artery pH≥7.15, n =112) the average(±S.D.) number of interventions decreased from 27(±17) to 16(±9) when Sp o 2 was available. This reduction in number of interventions resulted in an significantly increased specificity for two referees. In the acidotic group ( n =7) the average number of interventions also decreased, from 6(±2) to 4(±2), and as a consequence the sensitivity decreased. The pH estimate based on CTG+Sp o 2 was higher in both acidotic and non-acidotic fetuses than the estimated pH based on CTG alone. Conclusion : In this study all referees intervened less frequently when Sp o 2 was used as an adjunct to CTG. This resulted in fewer unnecessary operative interventions, but may also lead to unidentified fetal acidosis. The number of acidotic newborns ( n =7) was too small, however, to draw definite conclusions. Larger studies should address the efficacy of Sp o 2 in detecting fetal compromise before clinical use can be advocated. © 1997 Elsevier Science Ireland Ltd.

Objective The purpose of this study was to assess maternal and perinatal outcomes as a function of second-stage labor duration. Study Design We assessed outcomes in nulliparous laboring women who were enrolled in a trial of fetal pulse oximetry. Results Of 5341 participants, 4126 women reached the second stage of labor. As the duration of the second stage increased, spontaneous vaginal delivery rates declined, from 85% when the duration was Conclusion The second stage of labor does not need to be terminated for duration alone.
