Abstract

Radiation damage tests in hydrogenated amorphous silicon (a-Si:H) flexible flux and dose-measuring devices have been performed with a 3 MeV proton beam, to evaluate combined displacement and total ionizing dose damage. The tested devices had two different configurations and thicknesses. The first device was a 2 um thick n-i-p diode having a 5 mm x 5 mm area. The second device was a 5 um thick charge selective contact detector having the same area. Both the devices were deposited on a flexible polyimide substrate and were irradiated up to the fluence of 1016 neq/cm2. The response to different proton fluxes has been measured before irradiation and after irradiation at 1016 neq/cm2 for charge-selective contacts and n-i-p devices. The effect of annealing for partial performance recovery at 100{\deg}C for 12 hours was also studied and a final characterization on annealed devices was performed. Abstract

The Laser Interferometer Space Antenna (LISA) of the European Space Agency (ESA) will be the first low-frequency gravitational-wave observatory orbiting the Sun at 1 AU. The LISA Pathfinder (LPF) mission, aiming at testing the instruments to be located on board the LISA spacecraft (S/C), hosted, among the others, fluxgate magnetometers and a particle detector as parts of a diagnostics subsystem. These instruments allowed us to estimate the magnetic and Coulomb spurious forces acting on the test masses that constitute the mirrors of the interferometer. With these instruments, we also had the possibility to study the galactic cosmic-ray short term-term variations as a function of the particle energy and the associated interplanetary disturbances. Platform magnetometers and particle detectors will also be placed on board each LISA S/C. This work reports on an empirical method that allowed us to disentangle the interplanetary and onboard-generated components of the magnetic field by using the LPF magnetometer measurements. Moreover, we estimate the number and fluence of solar energetic particle events expected to be observed with the ESA Next Generation Radiation Monitor during the mission lifetime. An additional cosmic-ray detector, similar to that designed for LPF, in combination with magnetometers, would permit to observe the evolution of recurrent and non-recurrent galactic cosmic-ray variations and associated increases of the Interplanetary Magnetic Field at the transit of high-speed solar wind streams and interplanetary counterparts of coronal mass ejections. The diagnostics subsystem of LISA makes this mission also a natural multi-point observatory for space weather science investigations.

Abstract

Hydrogenated amorphous silicon (a-Si:H) is a material having an intrinsically high radiation hardness that can be deposited on flexible substrates like Polyimide. For these properties a-Si:H can be used for the production of flexible sensors. a-Si:H sensors can be successfully utilized in dosimetry, beam monitoring for particle physics (x-ray, electron, gamma-ray and proton detection) and radiotherapy, radiation flux measurement for space applications (study of solar energetic particles and stellar events) and neutron flux measurements. In this paper we have studied the dosimetric x-ray response of n-i-p diodes deposited on Polyimide. We measured the linearity of the photocurrent response to x-rays versus dose-rate from which we have extracted the dosimetric x-ray sensitivity at various bias voltages. In particular low bias voltage operation has been studied to assess the high energy efficiency of these kind of sensor. A measurement of stability of x-ray response versus time has been shown. The effect of detectors annealing has been studied. Operation under bending at various bending radii is also shown.

Abstract

Context. The Metis coronagraph is one of the remote-sensing instruments of the ESA/NASA Solar Orbiter mission. Metis is aimed at the study of the solar atmosphere and solar wind by simultaneously acquiring images of the solar corona at two different wavelengths; visible-light (VL) within a band ranging from 580 nm to 640 nm, and in the HI Ly-alpha 121.6 +/- 10 nm ultraviolet (UV) light. The visible-light channel includes a polarimeter with electro-optically modulating Liquid Crystal Variable Retarders (LCVRs) to measure the linearly polarized brightness of the K-corona to derive the electron density. Aims. In this paper, we present the first in-flight validation results of the Metis polarimetric channel together with a comparison to the on-ground calibrations. It is the validation of the first use in deep space (with hard radiation environment) of an electro-optical device: a liquid crystal-based polarimeter. Methods. We used the orientation of the K-corona's linear polarization vector during the spacecraft roll maneuvers for the in-flight calibration. Results. The first in-flight validation of the Metis coronagraph on-board Solar Orbiter shows a good agreement with the on-ground measurements. It confirms the expected visible-light channel polarimetric performance. A final comparison between the first pB obtained by Metis with the polarized brightness (pB) obtained by the space-based coronagraph LASCO and the ground-based coronagraph KCor shows the consistency of the Metis calibrated results., Comment: 8 pages, 13 figures, 3 tables, paper

Abstract

The HASPIDE (Hydrogenated Amorphous Silicon PIxels DEtectors) project aims at the development of thin hydrogenated amorphous silicon (a-Si:H) detectors on flexible substrates (mostly Polyimide) for beam monitoring, neutron detection and space applications. Since a-Si:H is a material with superior radiation hardness, the benefit for the above-mentioned applications can be appreciated mostly in radiation harsh environments. Furthermore, the possibility to deposit this material on flexible substrates like Polyimide (PI), polyethylene naphthalate (PEN) or polyethylene terephthalate (PET) facilitates the usage of these detectors in medical dosimetry, beam flux and beam profile measurements. Particularly interesting is its use when positioned directly on the flange of the vacuum-to-air separation interface in a beam line, as well as other applications where a thin self-standing radiation flux detector is envisaged. In this paper, the HASPIDE project will be described and some preliminary results on PI and glass substrates will be reported.

Holmes and argues that his understudied novel, Ethiopia, The Land of Promise (1917), represents an important chapter in the history of Afrofuturism and American speculative fiction. The literary critical relevance of Ethiopia emerges forcefully from Holmes’s intertextual dialogue with other utopian and science fiction authors, such as Martin R. Delany, Sutton E. Griggs, Pauline E. Hopkins, Frances E.W. Harper, Edward Bellamy, and W.E.B. Du Bois. Holmes’s critique of Jim Crow segregation enables the articulation of a “distinctly revolutionary” project for African American futurity.

Abstract

The investigation of the wind in the solar corona initiated with the observations of the resonantly scattered UV emission of the coronal plasma obtained with UVCS-SOHO, designed to measure the wind outflow speed by applying the Doppler dimming diagnostics. Metis on Solar Orbiter complements the UVCS spectroscopic observations, performed during solar activity cycle 23, by simultaneously imaging the polarized visible light and the HI Ly-alpha corona in order to obtain high-spatial and temporal resolution maps of the outward velocity of the continuously expanding solar atmosphere. The Metis observations, on May 15, 2020, provide the first HI Ly-alpha images of the extended corona and the first instantaneous map of the speed of the coronal plasma outflows during the minimum of solar activity and allow us to identify the layer where the slow wind flow is observed. The polarized visible light (580-640 nm), and the UV HI Ly-alpha (121.6 nm) coronal emissions, obtained with the two Metis channels, are combined in order to measure the dimming of the UV emission relative to a static corona. This effect is caused by the outward motion of the coronal plasma along the direction of incidence of the chromospheric photons on the coronal neutral hydrogen. The plasma outflow velocity is then derived as a function of the measured Doppler dimming. The static corona UV emission is simulated on the basis of the plasma electron density inferred from the polarized visible light. This study leads to the identification, in the velocity maps of the solar corona, of the high-density layer about +/-10 deg wide, centered on the extension of a quiet equatorial streamer present at the East limb where the slowest wind flows at about (160 +/- 18) km/s from 4 Rs to 6 Rs. Beyond the boundaries of the high-density layer, the wind velocity rapidly increases, marking the transition between slow and fast wind in the corona.

Abstract

The Metis coronagraph is one of the remote sensing instruments hosted on board the ESA/NASA Solar Orbiter mission. Metis is devoted to carry out the first simultaneous imaging of the solar corona in both visible light (VL) and ultraviolet (UV). High-energy particles penetrate spacecraft materials and may limit the performance of on-board instruments. A study of galactic cosmic-ray (GCR) tracks observed in the first VL images gathered by Metis during the commissioning phase for a total of 60 seconds of exposure time is presented here. A similar analysis is planned for the UV channel. A prediction of the GCR flux up to hundreds of GeV is made here for the first part of the Solar Orbiter mission to study the Metis coronagraph performance. GCR model predictions are compared to observations gathered on board Solar Orbiter by the EPD/HET experiment in the range 10 MeV-100 MeV in the summer 2020 and with previous measurements. Estimated cosmic-ray fluxes above 70 MeV n$^{-1}$ have been also parameterized and used for Monte Carlo simulations aiming at reproducing the cosmic-ray track observations in the Metis coronagraph VL images. The same parameterizations can also be used to study the performance of other detectors. By comparing observations of cosmic-ray tracks in the Metis VL images with FLUKA Monte Carlo simulations of cosmic-ray interactions in the VL detector, it is found that cosmic rays fire a fraction of the order of 10$^{-4}$ of the whole image pixel sample. Therefore, cosmic rays do not affect sensibly the quality of Metis VL images. It is also found that the overall efficiency for cosmic-ray identification in the Metis VL images is approximately equal to the contribution of Z$>$2 particles. Abstract

Non-recurrent short term variations of the galactic cosmic-ray (GCR) flux above 70 MeV n$^{-1}$ were observed between 2016 February 18 and 2017 July 3 aboard the European Space Agency LISA Pathfinder (LPF) mission orbiting around the Lagrange point L1 at 1.5$\times$10$^6$ km from Earth. The energy dependence of three Forbush decreases (FDs) is studied and reported here. A comparison of these observations with others carried out in space down to the energy of a few tens of MeV n$^{-1}$ shows that the same GCR flux parameterization applies to events of different intensity during the main phase. FD observations in L1 with LPF and geomagnetic storm occurrence is also presented. Finally, the characteristics of GCR flux non-recurrent variations (peaks and depressions) of duration $<$ 2 days and their association with interplanetary structures are investigated. It is found that, most likely, plasma compression regions between subsequent corotating high-speed streams cause peaks, while heliospheric current sheet crossing cause the majority of the depressions.

Abstract

Galactic cosmic-ray (GCR) energy spectra observed in the inner heliosphere are modulated by the solar activity, the solar polarity and structures of solar and interplanetary origin. A high counting rate particle detector (PD) aboard LISA Pathfinder (LPF), meant for subsystems diagnostics, was devoted to the measurement of galactic cosmic-ray and solar energetic particle integral fluxes above 70 MeV n$^{-1}$ up to 6500 counts s$^{-1}$. PD data were gathered with a sampling time of 15 s. Characteristics and energy-dependence of GCR flux recurrent depressions and of a Forbush decrease dated August 2, 2016 are reported here. The capability of interplanetary missions, carrying PDs for instrument performance purposes, in monitoring the passage of interplanetary coronal mass ejections (ICMEs) is also discussed.

Abstract

Background: Lockdown measures during the SARS-CoV-2 pandemic determined radical changes to behavioral and social habits, that were reflected by a reduction in the transmission of respiratory pathogens and in anthropogenic atmospheric emissions. Objective: This ecological study aims to provide a descriptive evaluation on how restrictive measures during the SARS-CoV-2 pandemic impacted Pediatric Emergency Department (PED) referrals for asthma exacerbations, and their potentially associated environmental triggers in Bologna, a densely populated urban area in Northern Italy. Methods: Files of children evaluated for acute asthma during 2015 to 2020 at the PED of Sant'Orsola University Hospital of Bologna were retrospectively reviewed. Historical daily concentration records of particulate (PM2.5, PM10) and gaseous (NO2, C6H6) air pollutants, and pollen were concurrently evaluated, including specific PM chemical tracers for traffic-related air pollution (TRAP). Results: In 2020, asthma-related PED referrals decreased compared to referral rates of the previous 5 years (p < 0.01). This effect was particularly marked during the first lockdown period (March to May), when the drastic drop in PED referrals was associated with a reduction of high-priority cases up to 85% and by 54%, on average. A concomitant reduction in the concentrations of traffic-related air pollutants was observed in the range of 40%–60% (p < 0.01). Conclusions: The lower rate of asthma exacerbations in childhood was in this study paralleled with reduced TRAP levels during the pandemic. Synergic interactions of the multiple consequences of lockdowns likely contributed to the reduced exacerbations, including decreased exposure to ambient pollutants and fewer respiratory infections, identified as the most important factor in the literature.

Abstract

Background There is mounting evidence on the existence of a Pediatric Inflammatory Multisystem Syndrome-temporally associated to SARS-CoV-2 infection (PIMS-TS), sharing similarities with Kawasaki Disease (KD). The main outcome of the study were to better characterize the clinical features and the treatment response of PIMS-TS and to explore its relationship with KD determining whether KD and PIMS are two distinct entities. Methods The Rheumatology Study Group of the Italian Pediatric Society launched a survey to enroll patients diagnosed with KD (Kawasaki Disease Group – KDG) or KD-like (Kawacovid Group - KCG) disease between February 1st 2020, and May 31st 2020. Demographic, clinical, laboratory data, treatment information, and patients’ outcome were collected in an online anonymized database (RedCAP®). Relationship between clinical presentation and SARS-CoV-2 infection was also taken into account. Moreover, clinical characteristics of KDG during SARS-CoV-2 epidemic (KDG-CoV2) were compared to Kawasaki Disease patients (KDG-Historical) seen in three different Italian tertiary pediatric hospitals (Institute for Maternal and Child Health, IRCCS “Burlo Garofolo”, Trieste; AOU Meyer, Florence; IRCCS Istituto Giannina Gaslini, Genoa) from January 1st 2000 to December 31st 2019. Chi square test or exact Fisher test and non-parametric Wilcoxon Mann-Whitney test were used to study differences between two groups. Results One-hundred-forty-nine cases were enrolled, (96 KDG and 53 KCG). KCG children were significantly older and presented more frequently from gastrointestinal and respiratory involvement. Cardiac involvement was more common in KCG, with 60,4% of patients with myocarditis. 37,8% of patients among KCG presented hypotension/non-cardiogenic shock. Coronary artery abnormalities (CAA) were more common in the KDG. The risk of ICU admission were higher in KCG. Lymphopenia, higher CRP levels, elevated ferritin and troponin-T characterized KCG. KDG received more frequently immunoglobulins (IVIG) and acetylsalicylic acid (ASA) (81,3% vs 66%; p = 0.04 and 71,9% vs 43,4%; p = 0.001 respectively) as KCG more often received glucocorticoids (56,6% vs 14,6%; p Conclusion Our study suggests that SARS-CoV-2 infection might determine two distinct inflammatory diseases in children: KD and PIMS-TS. Older age at onset and clinical peculiarities like the occurrence of myocarditis characterize this multi-inflammatory syndrome. Our patients had an optimal response to treatments and a good outcome, with few complications and no deaths.

Abstract

BackgroundEvidence is urgently needed to support treatment decisions for children with multisystem inflammatory syndrome (MIS-C) associated with severe acute respiratory syndrome coronavirus 2.MethodsWe performed an international observational cohort study of clinical and outcome data regarding suspected MIS-C that had been uploaded by physicians onto a Web-based database. We used inverse-probability weighting and generalized linear models to evaluate intravenous immune globulin (IVIG) as a reference, as compared with IVIG plus glucocorticoids and glucocorticoids alone. There were two primary outcomes: the first was a composite of inotropic support or mechanical ventilation by day 2 or later or death; the second was a reduction in disease severity on an ordinal scale by day 2. Secondary outcomes included treatment escalation and the time until a reduction in organ failure and inflammation.ResultsData were available regarding the course of treatment for 614 children from 32 countries from June 2020 through February 2021; 490 met the World Health Organization criteria for MIS-C. Of the 614 children with suspected MIS-C, 246 received primary treatment with IVIG alone, 208 with IVIG plus glucocorticoids, and 99 with glucocorticoids alone; 22 children received other treatment combinations, including biologic agents, and 39 received no immunomodulatory therapy. Receipt of inotropic or ventilatory support or death occurred in 56 patients who received IVIG plus glucocorticoids (adjusted odds ratio for the comparison with IVIG alone, 0.77; 95% confidence interval [CI], 0.33 to 1.82) and in 17 patients who received glucocorticoids alone (adjusted odds ratio, 0.54; 95% CI, 0.22 to 1.33). The adjusted odds ratios for a reduction in disease severity were similar in the two groups, as compared with IVIG alone (0.90 for IVIG plus glucocorticoids and 0.93 for glucocorticoids alone). The time until a reduction in disease severity was similar in the three groups.ConclusionsWe found no evidence that recovery from MIS-C differed after primary treatment with IVIG alone, IVIG plus glucocorticoids, or glucocorticoids alone, although significant differences may emerge as more data accrue. Abstract

In a single-site study (San Diego, CA, USA), we previously showed that Kawasaki Disease (KD) cases cluster temporally in bursts of approximately 7 days. These clusters occurred more often than would be expected at random even after accounting for long-term trends and seasonality. This finding raised the question of whether other locations around the world experience similar temporal clusters of KD that might offer clues to disease etiology. Here we combine data from San Diego and nine additional sites around the world with hospitals that care for large numbers of KD patients, as well as two multi-hospital catchment regions. We found that across these sites, KD cases clustered at short time scales and there were anomalously long quiet periods with no cases. Both of these phenomena occurred more often than would be expected given local trends and seasonality. Additionally, we found unusually frequent temporal overlaps of KD clusters and quiet periods between pairs of sites. These findings suggest that regional and planetary range environmental influences create periods of higher or lower exposure to KD triggers that may offer clues to the etiology of KD.

Abstract

Metis is a multi-wavelength coronagraph onboard the European Space Agency (ESA) Solar Orbiter mission. The instrument features an innovative instrument design conceived for simultaneously imaging the Sun's corona in the visible and ultraviolet range. The Metis visible channel employs broad-band, polarized imaging of the visible K-corona, while the UV one uses narrow-band imaging at the HI Ly 􀄮, i.e. 121.6 nm. During the commissioning different acquisitions and activities, performed with both the Metis channels, have been carried out with the aim to check the functioning and the performance of the instrument. In particular, specific observations of stars have been devised to assess the optical alignment of the telescope and to derive the instrument optical parameters such as focal length, PSF and possibly check the optical distortion and the vignetting function. In this paper, the preliminary results obtained for the PSF of both channels and the determination of the scale for the visible channel will be described and discussed. The in-flight obtained data will be compared to those obtained on-ground during the calibration campaign.

Abstract

Context. The Metis coronagraph on board Solar Orbiter offers a new view of coronal mass ejections (CMEs), observing them for the first time with simultaneous images acquired with a broad-band filter in the visible-light interval and with a narrow-band filter around the H I Ly-alpha line at 121.567 nm, the so-called Metis UV channel. Aims. We show the first Metis observations of a CME, obtained on 16 and 17 January 2021. The event was also observed by the EUI/FSI imager on board Solar Orbiter, as well as by other space-based coronagraphs, such as STEREO-A/COR2 and SOHO/LASCO/C2, whose images are combined here with Metis data. Methods. Different images are analysed here to reconstruct the 3D orientation of the expanding CME flux rope using the graduated cylindrical shell model. This also allows us to identify the possible location of the source region. Measurements of the CME kinematics allow us to quantify the expected Doppler dimming in the Ly-alpha channel. Results. Observations show that most CME features seen in the visible-light images are also seen in the Ly-alpha images, although some features in the latter channel appear more structured than their visible-light counterparts. We estimated the expansion velocity of this event to be below 140 km s(-1). Hence, these observations can be understood by assuming that Doppler dimming effects do not strongly reduce the Ly-alpha emission from the CME. These velocities are comparable with or smaller than the radial velocities inferred from the same data in a similar coronal structure on the east side of the Sun. Conclusions. The first observations by Metis of a CME demonstrate the capability of the instrument to provide valuable and novel information on the structure and dynamics of these coronal events. Considering also its diagnostics capabilities regarding the conditions of the ambient corona, Metis promises to significantly advance our knowledge of such phenomena.

Abstract

BACKGROUND Evidence is urgently needed to support treatment decisions for children with multisystem inflammatory syndrome (MIS-C) associated with severe acute respiratory syndrome coronavirus 2. METHODS We performed an international observational cohort study of clinical and outcome data regarding suspected MIS-C that had been uploaded by physicians onto a Web-based database. We used inverse-probability weighting and generalized linear models to evaluate intravenous immune globulin (IVIG) as a reference, as compared with IVIG plus glucocorticoids and glucocorticoids alone. There were two primary outcomes: the first was a composite of inotropic support or mechanical ventilation by day 2 or later or death; the second was a reduction in disease severity on an ordinal scale by day 2. Secondary outcomes included treatment escalation and the time until a reduction in organ failure and inflammation. RESULTS Data were available regarding the course of treatment for 614 children from 32 countries from June 2020 through February 2021; 490 met the World Health Organization criteria for MIS-C. Of the 614 children with suspected MIS-C, 246 received primary treatment with IVIG alone, 208 with IVIG plus glucocorticoids, and 99 with glucocorticoids alone; 22 children received other treatment combinations, including biologic agents, and 39 received no immunomodulatory therapy. Receipt of inotropic or ventilatory support or death occurred in 56 patients who received IVIG plus glucocorticoids (adjusted odds ratio for the comparison with IVIG alone, 0.77; 95% confidence interval [CI], 0.33 to 1.82) and in 17 patients who received glucocorticoids alone (adjusted odds ratio, 0.54; 95% CI, 0.22 to 1.33). The adjusted odds ratios for a reduction in disease severity were similar in the two groups, as compared with IVIG alone (0.90 for IVIG plus glucocorticoids and 0.93 for glucocorticoids alone). The time until a reduction in disease severity was similar in the three groups. CONCLUSIONS We found no evidence that recovery from MIS-C differed after primary treatment with IVIG alone, IVIG plus glucocorticoids, or glucocorticoids alone, although significant differences may emerge as more data accrue. Abstract

In this work, we present an investigation of the wind in the solar corona that has been initiated by observations of the resonantly scattered ultraviolet emission of the coronal plasma obtained with UVCS-SOHO, designed to measure the wind outflow speed by applying Doppler dimming diagnostics. Metis on Solar Orbiter complements the UVCS spectroscopic observations that were performed during solar activity cycle 23 by simultaneously imaging the polarized visible light and the H I Lyman-alpha corona in order to obtain high spatial and temporal resolution maps of the outward velocity of the continuously expanding solar atmosphere. The Metis observations, taken on May 15, 2020, provide the first H I Lyman-alpha images of the extended corona and the first instantaneous map of the speed of the coronal plasma outflows during the minimum of solar activity and allow us to identify the layer where the slow wind flow is observed. The polarized visible light (580-640 nm) and the ultraviolet H I Ly alpha (121.6 nm) coronal emissions, obtained with the two Metis channels, were combined in order to measure the dimming of the UV emission relative to a static corona. This effect is caused by the outward motion of the coronal plasma along the direction of incidence of the chromospheric photons on the coronal neutral hydrogen. The plasma outflow velocity was then derived as a function of the measured Doppler dimming. The static corona UV emission was simulated on the basis of the plasma electron density inferred from the polarized visible light. This study leads to the identification, in the velocity maps of the solar corona, of the high-density layer about +/- 10 degrees wide, centered on the extension of a quiet equatorial streamer present at the east limb - the coronal origin of the heliospheric current sheet - where the slowest wind flows at about 160 +/- 18 km s(-1) from 4 R-circle dot to 6 R-circle dot. Beyond the boundaries of the high-density layer, the wind velocity rapidly increases, marking the transition between slow and fast wind in the corona.

Abstract

Background: There is mounting evidence on the existence of a Pediatric Inflammatory Multisystem Syndrome temporally associated to SARS-CoV-2 infection (PIMS-TS), sharing similarities with Kawasaki Disease (KD). The main outcome of the study were to better characterize the clinical features and the treatment response of PIMS-TS and to explore its relationship with KD determining whether KD and PIMS are two distinct entities. Methods: The Rheumatology Study Group of the Italian Pediatric Society launched a survey to enroll patients diagnosed with KD (Kawasaki Disease Group – KDG) or KD-like (Kawacovid Group - KCG) disease between February 1st 2020, and May 31st 2020. Demographic, clinical, laboratory data, treatment information, and patients’ outcome were collected in an online anonymized database (RedCAP®). Relationship between clinical presentation and SARSCoV-2 infection was also taken into account. Moreover, clinical characteristics of KDG during SARS-CoV-2 epidemic (KDG-CoV2) were compared to Kawasaki Disease patients (KDG-Historical) seen in three different Italian tertiary pediatric hospitals (Institute for Maternal and Child Health, IRCCS “Burlo Garofolo”, Trieste; AOU Meyer, Florence;IRCCS Istituto Giannina Gaslini, Genoa) from January 1st 2000 to December 31st 2019. Chi square test or exact Fisher test and non-parametric Wilcoxon Mann-Whitney test were used to study differences between two groups. Results: One-hundred-forty-nine cases were enrolled, (96 KDG and 53 KCG). KCG children were significantly older and presented more frequently from gastrointestinal and respiratory involvement. Cardiac involvement was more common in KCG, with 60,4% of patients with myocarditis. 37,8% of patients among KCG presented hypotension/ non-cardiogenic shock. Coronary artery abnormalities (CAA) were more common in the KDG. The risk of ICU admission were higher in KCG. Lymphopenia, higher CRP levels, elevated ferritin and troponin-T characterized KCG. Abstract

BACKGROUND: Lung ultrasound (LUS) is a useful tool for assessing lung status in critically ill patients, but is not widely applied in intensive care units (ICUs). The aim of this study was to investigate the use of LUS in Italian ICUs. METHODS: A questionnaire was distributed to 116 ICU representatives belonging to the Italian GiViTI network, participating in a national project on continuous quality of care assessment. The answers to the questionnaire were analyzed with the data collected in the course of said project. RESULTS: 87 ICUs took part in the study. LUS was performed in 94.3% of them, generally by an internal intensivist. The mean number of tests per bed per week was 0.97 (SD, 1.0). A standardized reporting format was present in only 11% of ICUs. LUS was generally used to investigate known or suspected diseases and as a screening tool in only 10%. The number of testes per bed per week was inversely correlated to the number of beds per physician (P<0.001) and to the provider performing the LUS (P<0.001), but not to other structural, organizational or performance data. CONCLUSIONS: LUS is performed in the majority of ICUs and is generally used to investigate known or suspected diseases. Frequency of use appears to be low and related to selected organizational factors. There is generally no standardized format for reporting LUS findings.

Abstract

The prompt availability of reliable epidemiological information on emerging pandemics is crucial for public health policy-makers. Early in 2013, a possible new H1N1 epidemic notified by an intensive care unit (ICU) to GiViTI, the Italian ICU network, prompted the re-activation of the real-time monitoring system developed during the 2009-2010 pandemic. Based on data from 216 ICUs, we were able to detect and monitor an outbreak of severe H1N1 infection, and to compare the situation with previous years. The timely and correct assessment of the severity of an epidemic can be obtained by investigating ICU admissions, especially when historical comparisons can be made.

Abstract

INTRODUZIONE: La nuova politica europea per la salute “Health 2020” identifica l’assistenza primaria quale asse portante per migliorare in modo significativo la salute e il benessere delle popolazioni. La regione Emilia-Romagna, in coerenza con l’impostazione europea ha emanato nel 2010 le indicazioni per la realizzazione e l’organizzazione funzionale delle case della Salute (CdS, DGR n.291/2010). L’Azienda Sanitaria Locale (AUSL) di Parma ha avviato una programmazione specifica che prevede la realizzazione di 26 CdS.METODI: La seguente analisi riassume lo stato di implementazione delle CdS nell’ AUSL di Parma, descrive i professionisti che vi operano e propone un modello per l’assessment della qualità, efficacia ed efficienza delle CdS come modello organizzativo innovativo di erogazione dell’assistenza primaria.RISULTATI: Distribuite nei quattro Distretti dell’ AUSL di Parma vi sono, ad oggi, 16 CdS funzionanti, di cui 6 di tipologia piccola, 4 Media e 6 di tipologia Grande, corrispondenti al 27% delle CdS funzionanti sull’intero territorio regionale. La media della popolazione assistita per ciascuna tipologia è: 10519 per le CdS grandi, 8182 per le medie e 5029 per le piccole (range: 3240- 13573). Nelle CdS vi operano 89 medici di Medicina Generale (MMG) e 9 Pediatri di Libera Scelta (PdLS), corrispondenti, rispettivamente, al 30,8% e 15% del totale dei MMG e PdLS dell’AUSL. Si è recentemente avviato un progetto di ricerca in collaborazione con l’Università di Parma e con la Thomas Jefferson University (USA) all’interno del quale si sono definiti le popolazioni e sottopopolazioni oggetto dello studio, i comparatori, gli indicatori di processo e gli outcomes clinici attraverso i quali analizzare e monitorare nel tempo la qualità, l’efficacia ed l’efficienza delle CdS rispetto ad altri modelli di erogazione dell’assistenza territoriale.CONCLUSIONI: A tre anni dall’avvio del progetto, il modello delle CdS si va consolidando nel territorio dell’AUSL di Parma con progressivo aumento delle CdS funzionanti, dei servizi sanitari offerti, delle attività di formazione rivolte al personale sanitario e di educazione sanitaria ed empowerment dei pazienti. Parallelamente, una rinnovata collaborazione tra AUSL e Università permetterà di costruire solide evidenze scientifiche utili ai fini del progresso della ricerca nell’ambito dell’epidemiologia dei servizi ma anche della diffusione delle buone pratiche e dell’estensione del modello CdS sul territorio nazionale.

Abstract

BACKGROUND The number of elderly patients undergoing major surgical interventions and then needing admission to intensive care unit (ICU) grows steadily. We investigated this issue in a cohort of 232,278 patients admitted in five years (2011-2015) to 163 Italian general ICUs. METHODS Surgical patients older than 75 registered in the GiViTI MargheritaPROSAFE project were analyzed. The impact on hospital mortality of important chronic conditions (severe COPD, NYHA class IV, dementia, end-stage renal disease, cirrhosis with portal hypertension) was investigated with two prognostic models developed yearly on patients staying in the ICU less or more than 24 hours. RESULTS 44,551 elderly patients (19.2%) underwent emergency (47.3%) or elective surgery (52.7%). At least one severe comorbidity was present in 14.6% of them, yielding a higher hospital mortality (32.4%, vs. 21.1% without severe comorbidity). In the models for patients staying in the ICU 24 hours or more, cirrhosis, NYHA class IV, and severe COPD were constant independent predictors of death (adjusted odds ratios [ORs] range 1.67-1.97, 1.54-1.91, and 1.34-1.50, respectively), while dementia was statistically significant in four out of five models (adjusted ORs 1.23-1.28). End-stage renal disease, instead, never resulted to be an independent prognostic factor. For patients staying in the ICU less than 24 hours, chronic comorbidities were only occasionally independent predictors of death. CONCLUSIONS Our study confirms that elderly surgical patients represent a relevant part of all ICUs admissions. About one of seven bear at least one severe chronic comorbidity, that, excluding end-stage renal disease, are all strong independent predictors of hospital death.

Abstract

Takotsubo cardiomyopathy, or broken heart syndrome, is characterized by transient left ventricular dysfunction associated to chest pain, elevation of cardiac enzymes, and electrocardiographic changes, mimicking an acute coronary syndrome, especially in older women after a physical or emotional stress. It is extremely infrequent in children as well as after infective stress. We described a celiac 4-year-old girl, following a gluten-free diet, who developed features of cardiac failure few days after episodes of acute diarrhea with fever. The patient was treated with oral anticongestive therapy and intravenous immunoglobulins, and she had a dramatic and rapid improvement; echocardiographic features normalized in 48hours. © 2012 Wiley Periodicals, Inc.

Abstract

On december 12th 2011 Università Bocconi hosted a consensus conference on primary care. Th emost relevant stakeholders of the sector were among the participants to the meeting, many of them being also the authors of this paper. The conference aimed at reflecting on the state of the art of of primary care in Italy, with a specific focus on the role played by general practitioners. On the basis of the collection and analysis of the available scientific literature and their experiences, the conference participants have worked out a diagnosis of the current Italian system of general practice and elaborated on the degree of implementation of the reforms that have involved the sector. This paper summarises the main reflections and conclusions of that day.
