Loss of muscle mass is associated with increased risk of morbidity and mortality in hospitalized patients. Uncertainties of treatment efficiency by short‐term artificial nutrition remain, specifically improvement of protein balance in skeletal muscles. In this study, algorithmic microarray analysis was applied to map cellular changes related to muscle protein metabolism in human skeletal muscle tissue during provision of overnight preoperative total parenteral nutrition (TPN). Twenty‐two patients (11/group) scheduled for upper GI surgery due to malignant or benign disease received a continuous peripheral all‐in‐one TPN infusion (30 kcal/kg/day, 0.16 gN/kg/day) or saline infusion for 12 h prior operation. Biopsies from the rectus abdominis muscle were taken at the start of operation for isolation of muscle RNA. The effects of EGFR and COX-2 protein overexpression on clinical outcomes in pancreatic ductal adenocarcinoma (PDAC) patients remains unclear. Therefore, the aim of the present study was to evaluate the protein expression of epithelial growth factor receptor (EGFR) and cyclooxygenase-2 (COX-2) in tumor cells in surgically resected PDAC, in comparison with clinicopathological characteristics and clinical outcomes. Immunohistochemical staining of formalin-fixed paraffin-embedded tissue derived from surgically resected tumors was performed. Tissue slides were evaluated for membrane wild-type EGFR and cytoplasmic COX-2 staining using a histoscore system. Statistical associations between EGFR and COX-2 staining and clinicopathological characteristics were examined to predict survival. In a cohort of 32 resected PDAC patients, high EGFR protein expression in tumor cells was significantly associated with shorter median overall survival (7.9 vs. 39.2 months, P=0.0038). The corresponding hazard ratio (HR) for patients with high EGFR protein expression in tumor cells was 3.12 [95% confidence interval (CI): 1.39–7.00, P=0.006]. COX-2 protein expression was not associated with survival (22.6 vs. 24.5 months P=0.60; HR 1.22 95% CI: 0.59–2.51, P=0.60). Following multivariate Cox regression analysis, high EGFR protein expression in tumor cells (P=0.043) remained as significant independent prognostic factor for survival. In conclusion, high wild-type EGFR protein expression, but not COX-2 protein expression, in tumor cells is a prognostic factor for reduced overall survival following pancreatic tumor resection, supporting a role for EGFR in identifying resected patients that may benefit from EGFR-targeted therapy. Circulating tumor cells (CTCs) are able to predict outcome in patients with breast, colon and prostate cancer and appear to be promising biomarkers of pancreatic carcinoma. The aim of the present study was to demonstrate a statistically significant portal-arterial difference of CTCs during curative resection of periampullary cancer. A commercially available instrument (IsofluxR) was used to quantify blood content of CTC in 10 patients with periampullary cancer according to preoperative diagnostics. Portal and arterial blood samples (~8 ml each) were simultaneously collected intra-operatively following surgical dissection prior to division of the pancreas for tumor removal. Quantitative CTC analyses were performed according to standardized protocols for immune-magnetic enrichment of CTC. Flow cytometry was applied for qualitative evaluations of various CTC markers in 7 patients. There was a statistically significant difference in the number of CTCs collected in the portal blood [58±14 cells per 100 ml; mean ± standard error (SE)] vs. arterial blood [24±7 cells per 100 ml (SE), P<0.025]. A fractional uptake of ≥40% across liver and lung compartments of assumed malignant CTC was estimated to correspond to the appearance of ~410 tumor cells per minute during pancreatic resections based on estimated hepatic blood flow, measured tumor cell mass and tumor cell proliferation activity. Complications in the collection of portal blood were not observed. A significant uptake across liver or lung compartments of potentially malignant tumor CTCs from periampullary carcinoma may represent a model to capture, define and characterize cell clones with metastatic potential in liver and lung tissues following surgical resection. 8. Searching for an optimal balance : Dual career experiences of Swedish adolescent athletes

Objectives: The paper presents a national level Swedish project aimed at examining adolescent student-athletes' dual career experiences (including sport, studies, and private life) during their first year at national elite sport schools (Swedish abbreviation RIGs will be used) with a particular focus on development of their athletic and student identities. The developmental model of transitions faced by athletes (Wylleman & Lavallee, 2004) and the athletic career transition model (Stambulova, 2003) served as underlying frameworks. Design: A longitudinal mixed-method research design was used with autumn-to-spring quantitative and qualitative parts. Method: Sixteen year old student-athletes, representing 27 sports and 33 RIGs (n = 261 in the first and n = 250 in the second measurement), completed three quantitative instruments. Additionally, in-depth interviews were conducted with 10 participants. Results: Results revealed (a) significant changes in the participants' transition/adaptation variables from the first to the second measurement accompanied by rather high perceived quality of adjustment at RIG both at the beginning and at the end of the educational year; (b) significant contributions of the transition variables to the perceived quality of adjustment with personal resources as a key predictor; (c) significantly higher athletic than student identity in both quantitative measurements, but with inter- and intra-individual differences with regard to balancing the two shown by the qualitative data. Conclusions: The study contributes to deeper understanding of dual career experiences of Swedish adolescent athletes; the authors provide recommendations for psychological dual career support services at RIGs and outline future research in the Swedish dual career model. © 2014 Elsevier Ltd., Funding: The Swedish Sports Confederation (grant #700/10-51)

This bachelor thesis brings up a discussion about museums availability and access to thearchaeological display objects towards blind and visually impaired visitors. The study phasewill take start in analyzing four museums in south Sweden, all which have exhibitions aboutthe prehistory.Visually impaired and blind visitors don’t have a lot to go on at the current exhibitions aboutprehistory. This is because of the lack of access and availability when it comes to their specialneed. To raise awareness to this problem I took out a survey for the museums to answer. I alsovisited two of the exhibitions to make my own opinion of the museums public areas; such asthe entrance, hallway and the exhibition area. Things I looked out for were Braille, audioguides and objects to touch.The sense to touch is of most importance for visually impaired and blind to create an image ofan object. Because of this I discuss the ability for an exhibition where most of the objects areexposed to all visitors to be touched. An exposure of archaeological object to benefit needsfor visually impaired is an important discussion towards the management of thearchaeological display objects, which leads the study towards replicas and experimentalarchaeology.The conclusion was that it is quite possible to design and customize an archaeologicalexhibition for the visually impaired by factors like; reconstructed objects, route marker fororientation, Braille and audio guides with well rendered image descriptions.

The biggest housing operator is in a place where they must tear down a lot of buildings because they do not have enough tenants. But what do people really want in an apartment and in the surroundings? What is needed to make it attractive? That is something that this study will discuss and in the end it will lead to a proposal of an area that is suitable for a new housing and a few proposals to make the existing apartments more attractive.

Syftet med denna kvalitativa litteratur och intervjustudie var att undersöka pedagogens roll i relationsskapande möten för barn i behov av särskilt stöd i förskoleklass, i synnerhet med tanke på barn som har ett utagerande beteende. De frågeställningar som låg till grund för denna studie var; Vilka pedagogiska strategier kan bidra till att öka graden av inklusion av barn som har ett utagerande beteende i både didaktiska situationer och i socialt samspel? och: Vilken är pedagogens roll i relationsskapande möten och hur blir detta synligt i arbetet med barn som har ett utagerande beteende? För att få svar på dessa frågeställningar valde jag att först undersöka relevanta internationella forskningsartiklar, leta strategier och presentera de mest frekventa strategierna i en tabell. Då detta resultat kändes statiskt och lämnade flera frågor obesvarade gjordes valet att intervjua en yrkesverksam förskollärare i förskoleklass. Intervjufrågorna baserades på resultatet av litteraturstudien, egna reflektioner och på föreliggande studies teoretiska inramning, det vill säga Martin Bubers relationsfilosofi. Resultatet av delstudie I visar att det finns en mängd pedagogiska strategier som kan bidra till att öka graden av inklusion av barn med ett utagerande beteende. De mest frekventa strategierna utifrån forskningsartiklarna är; Pedagogens förhållningssätt; beröm, uppmuntran och positivt bemötande. Att ha en trygg och tydlig struktur på miljö, uppgifter, förväntningar och på dagen. Skapa en tillåtande och accepterande atmosfär där olikheter ses som tillgångar och arbeta för att skapa goda relationer. Uppmuntra till diskussion, interaktion och samlärande. Undvik särskiljande lösningar som små undervisningsgrupper. Anpassa alla aktiviteter efter individuella behov, förutsättningar och intressen. All träning i sociala förmågor bör sammanflätas med vardagliga aktiviteter samt att ha en god samverkan med föräldrar, kollegor och andra instanser. Resultatet av delstudie II visar att pedagogens roll för att skapa inkluderande relationsskapande möten är betydelsefull. Pedagogens roll är att ge hela barnet möjligheter till lärande och utveckling genom att se, bekräfta, uppmuntra, stödja och lyfta fram det som är enastående och unikt hos varje individ. En medveten pedagog har en förmåga att ta tillvara på relationsskapande möten för att bygga förtroende, tillit och för att skapa en förstående och accepterande atmosfär där olikheter ses som tillgångar. Pedagogens roll är att ha en tilltro till barnets förmågor och ta tillvara på dessa. Arbetssätt och förhållningssätt i detta perspektiv anpassas efter barnens olika förutsättningar, behov och intressen. The purpose of this qualitative literature and interview study was to investigate the role of teacher in relationship-buildingsessions with children in need of special needs support in pre-school,particularly with respect to children who have a disruptive behavior. Theissues underlying this study were: Whatpedagogical approaches can help to increase the degree of inclusion of childrenwith a disruptive behavior in both didactic situations and in socialinteraction? and: What is theteacher's role in relational meetings and how this will be visible in the workwith children who have disruptive behavior? To answer these questions, Ichose to first examine relevant international research articles, searchstrategies and present the most frequent strategies in a table. Since thisresult seemed static and left several questions unanswered I chose to interviewa professional teacher in preschool class. The interview questions were basedon the results of the literature review, reflections and this present study theoreticalframe, namely Martin Buber's relational philosophy. The first study shows thatthere are a variety of teacher strategies that can help to increase the degreeof inclusion of children with disruptive behavior. The most frequent strategiesbased on the research articles are; teachers attitude and approach; praise,encouragement and positive feedback. Have a safe and clear structure of theenvironment, tasks, and expectations of the day. Create a permissive andaccepting atmosphere where diversity is seen as an asset and develop goodrelations. Encourage discussion, interaction and co-operative learning. Avoidexcluding solutions such as small teaching groups. Adapt all activities toindividual needs, abilities and interests. All training in social skills shouldbe integrated with everyday activities. Have a good cooperation with parents,colleagues and other agencies. The results of the second study show that theteacher´s role in creating inclusive relational meetings is important. Theteacher's role is to give the child opportunities for learning and developmentthrough to really see, acknowledge, encourage, support and highlight what isoutstanding and unique in each individual. A conscious teacher has the abilityto take advantage of the relationship-building sessions to build confidence,trust and to create an understanding and accepting atmosphere where differencesare seen as assets. The teacher's role is also to have a belief in each child'sabilities and take advantage of these abilities. Work methods and teachersattitudes in this perspective are tailored to meet children's differentcircumstances, needs and interests.

15. How to Become a Winner in the Long-Run? Dual Career Experiences of Swedish Adolescent Athletes

- Abstract
This presentation will focus on a national level Swedish project aimed at examining adolescent athletes’ transition to, and adaptation at, national elite sport schools (NESS). In this study transitional issues (e.g., demands, resources, coping strategies related to sport, studies and private life) and athletes’ personal attributes (e.g., athletic and student identities) were addressed from a holistic lifespan perspective using longitudinal mix-method (quantitative and qualitative) research design. The instruments used in the two quantitative measurements (autumn and spring) included: the Dual Career Survey (Engström & Stambulova, 2010), the Athletic Identity Measurement Scale (Brewer, Van Raalte, & Linder, 1993), the Student Identity Measurement Scale (Engström & Stambulova, 2010), the Task & Ego Orientation in Sport Questionnaire (Duda, 1989), and the Basic Needs Satisfaction Scale (Chen et al., in press). Participants (main sample) were athletes of 15-16 years old, representing two genders, 27 sports, and 33 NESSs across the country with 261 who took part in the first measurement and 250 who took part in the second measurement. In addition, in-depth interviews were conducted twice a year with 10 participants from the main sample. The results confirmed that starting at NESS meant for student-athletes entering a complicated transition embracing all major spheres of their life. At the beginning of the year the participants underestimated demands of studying, doing sports and living at NESS and overestimated (based on previous relevant experiences) their readiness to pursue dual career at NESS. Later, during the year more awareness and understanding of the reality came, and led to their self-re-evaluation of the adaptation process and outcomes. 17. Dual career for student-athletes: A longitudinal study of adaptation during the first six months at the sport gymnasium in Sweden

The purpose of this study was to gain understanding of the student-athletes’ transition to a sport gymnasium (sport high school). The objectives were: (1) to explore the Dual Career Survey and Student Identity Measurement Scale, (2) to examine the dynamics during the first six month at the sport gymnasium in student-athletes (a) transition variables, (b) personal variables, (c) importance/satisfaction with various spheres of life variables, and (3) to explore how these variables contribute to the quality of the student-athletes’ adjustment during their first six months at the sport gymnasium. This study had a quantitative longitudinal design with two measurements made three and six months into the first year at the sport gymnasium. The participants (N=91) were student-athletes at Malmö Sport gymnasium in Sweden. Four instruments were used: Dual Career Survey, Student Identity Measurement Scale, Athletic Identity Measurement Scale and The Task and Ego Orientation in Sport Questionnaire. The results showed a statistically significant increase in perceived transition demands associated with decreases in adjustment to the dual career and life satisfaction between the first and the second measurements. Predictors for a quality of adjustment included: personal recourse as a positive predictor, current need for help/support as a negative predictor (at the first measurement), and support in sport as a positive predictor (at the second measurement). Positive predictors were also athletic identity at both measurements and student identity at the first measurement. Importance of, and satisfaction with, different spheres of life were positive predictors at both measurements. The results are discussed in relation to theoretical frameworks and previous research.

18. Long-term outcome of microscopic esophagitis in chronic GERD patients treated with esomeprazole or laparoscopic antireflux surgery in the LOTUS trial

Udgivelsesdato: 2010-May, OBJECTIVES: Gastroesophageal reflux disease (GERD)-associated changes in esophageal histology have been reported mainly after short-term medical antireflux therapy, and few individual lesions have been examined. We report detailed histological findings from the LOTUS study, at baseline and at 1 and 3 years after laparoscopic antireflux surgery (LARS) or esomeprazole treatment in patients with chronic GERD. METHODS: LOTUS is a long-term, open, parallel-group, multicenter, randomized, controlled trial conducted in 11 European countries that compared LARS (n=248) with esomeprazole 20-40 mg daily (n=266). Biopsies from the distal esophagus 2 cm above the Z-line and at the Z-line were taken at baseline, and 1 and 3 years. The following lesions were assessed: basal cell hyperplasia (BCH), papillary elongation (PE), intercellular space dilatations (ISDs), intraepithelial eosinophils (EOSs), neutrophils, and necrosis/erosion. A severity score (SS, range 0-2) was calculated by taking the average score of all assessable lesions. RESULTS: All lesions were more severe on Z-line biopsies than at 2 cm, and almost all improved significantly from baseline to 1 and 3 years. The average SS (from 2 cm to Z-line) changed from 0.95 to 0.57 (1 year) and to 0.49 (3 years) on esomeprazole, and from 0.91 to 0.56 (1 year) and to 0.52 (3 years) after LARS (P<0.001 for both treatments at 1 and 3 years, with no significant difference between treatments). The proportions of patients with severe histological changes decreased from approximately 50% at baseline to 11% at 3 years. CONCLUSIONS: Both continuous esomeprazole treatment and laparoscopic fundoplication are associated with significant and similar overall improvement in microscopic esophagitis after 1 year that is maintained at 3 years.

Consumers are often exposed to brands and their persuasive message. The marketers want the consumers to create attitudes toward their brands. Mostly they take the explicit attitudes into consideration, and neglect the implicit ones. The purpose of this study was to find out if there was a relation between the explicit and the implicit attitudes toward different types of brands and if these could predict behaviour. The participants in the experiment were 60 students at the University of Lund. The data were collected through a questionnaire dealing with explicit attitudes toward brands and actual behaviour. There was also a computerised test (IAT) measuring their implicit attitudes toward brands. The result showed a significant positive correlation between explicit and implicit attitudes (r =. 462, p<. 001) and also between these two attitudes and behaviour. Thus, the participants had a preference for strong brands, both explicit and implicit and these attitudes both predicted consumer behaviour.
