Abstract

Teres major injuries are rare and are generally treated conservatively, except in high-performance athletes. This report describes a case of traumatic rupture of the teres major at its myotendinous junction in a professional gymnast. The patient underwent surgical treatment 10 days after the injury. Six months post-surgery, the patient achieved complete recovery of the range of motion and strength, returning to the pre-injury performance level, guided by physiotherapy rehabilitation. This is the first documented case in the literature of surgical treatment of this injury in a professional gymnast. The main lesson from this case is that early surgical repair in elite athletes can result in excellent functional outcomes and allow return to sport at the pre-injury performance level. Abstract

This study presented a pioneering investigation of the changes in the magnetic resonance imaging images of pectoralis major muscle (PMM) tendon rupture. In all, 26 men were evaluated with acute total PMM rupture (<3 months since injury) with a mean age of 37.3 years (SD = 9.7 years) and 10 control patients with a mean age of 32.6 years (SD = 4.2 years). The evaluation of the tendon PMM injuries was based on the magnetic resonance imaging exam and the histological analysis. The magnetic resonance imaging of the surgically showed two (7.1%) contralateral sides were normal, 16 (57.1%) showed superior tendinopathy, and 10 (35.7%) had total tendinopathy. Inferior tendinopathy was not observed. The tendon histology revealed degenerative changes in 16 (66.7%) fragments, with 12 (50.0%) considered as mild (<25%), and four considered as (16.7%) high (>50.0%) tendinopathy. Total acute rupture of the PMM tendon among weightlifters might be associated with tendinous degeneration prior to injury. [ABSTRACT FROM AUTHOR]

Abstract

To compare outcomes between autologous fascia lata and autologous hamstring grafts for chronic pectoralis major muscle (PMM) rupture repair, and perform histological, and imaging analyses. Forty male patients with chronic PMM ruptures (time since injury ranging from >3 months to 5 years) and a mean age of 37.3 years (SD = 9.7 years) were evaluated. One group (20 patients) received an autologous semitendinosus graft, and another group (20 patients) received an autologous fascia lata graft for PMM reconstruction. These patients with fascia lata grafts by Bak 2criterium 60% of the patients presented excellent results, 20% presented good results, 15% presented fair results, and 5% presented poor results. In the hamstring group 65% of the patients presented excellent results, 30% presented good results, and 5% presented fair results. Abstract

Introdução: A participação de pacientes com câncer de mama em programas de atividade física está associada á redução de mortalidade e recidiva, melhora da qualidade de vida e redução dos sintomas relacionados ao tratamento. Apesar dessas evidências, aproximadamente dois terços das pacientes são insuficientemente ativas. Assim, programas educativos e ações na comunidade encorajam sobreviventes de câncer de mama no aumento do nível de atividade física. Objetivos: Promover uma experiência prática de atividade física em pacientes tratadas por câncer de mama e avaliar os motivos que impedem a prática de atividade física nessa população. Métodos: Evento de extensão cadastrado sob o número PROEX 16866 a ser realizado anualmente, organizado por médicos, fisioterapeutas, profissionais de educação física, psicólogos e nutricionistas. Consiste em: práticas de atividade física supervisionada e palestra sobre a importância da prática de atividade física regular. Como instrumentos de avaliação foram utilizados questionários de avaliação dos fatores limitantes a prática de atividade física e de função dos membros superiores. Projeto aprovado sob o número CAAE 36724320.4.0000.5505. Resultados: A taxa de adesão ao evento foi de 75%. 64% das participantes apresentavam sobrepeso ou obesidade. Os principais motivos relatados para a não realização da atividade física foram: falta de tempo (55%) e cansaço (33%). Pacientes sedentárias (11%) apresentaram piores valores de função dos membros superiores quando comparados às fisicamente ativas. Conclusão: Falta de tempo é o principal motivo que limita a atividade física em pacientes com câncer de mama. Ações na comunidade permitem a conscientização e promoção do exercício físico nessa população.

Abstract

Pectoralis major muscle tendon ruptures associated with physical activity or effort are no longer uncommon in the medical literature. Treatment has also evolved significantly in the last 20 years. However, simultaneous bilateral rupture has only been described in a few cases. This article reports three cases with simultaneous bilateral rupture and describes the examinations and treatment performed. Bilateral lesions, although infrequent, also require early diagnosis and treatment in the acute phase. Abstract

Background. Whole-body vibration (WBV) has proven to be an efficient tool in improving the physical qualities of the neuromuscular system, such as increasing muscle strength, performance, joint stability, electromyographic signal, proprioception, among others. Its use in the upper limbs and joint instabilities has shown good immediate results in a single session. The aim of this study was to analyze the acute effects of the use of mechanical vibration on the muscle strength of subjects with shoulder instability. Methods. 10 athletes in the process of post-surgical rehabilitation of shoulder instability were volunteers in this study. Muscle strength was tested in the internal (IR) and external (ER) rotation movements in the neutral position using isokinetic dynamometry at two angular velocities - 60 and 180º/s in three moments: before, immediately after, and 10 minutes after the application of WBV. The WBV was applied only once, using a frequency of 30 Hz, with the patient assuming the arm flexion position on the platform during 5 series of 30 seconds of stimulation. Results. The limb with shoulder instability benefited in both movements - ER and IR - and in both angular velocities. The limb without instability, on the other hand, had increased strength only in the RI movements at 60º/s and RE at 180º/s. Conclusions. It is concluded that a single session of WBV applied directly to the upper limbs is capable of acutely improving muscle strength in both limbs in subjects with shoulder instability, with the affected limb being the most benefited.
