Favipiravir, an antiviral agent originally used for influenza infections, has become popular due to its beneficial signals in coronavirus disease. It is currently used in some countries within COVID-19 treatment protocols. This is an initial report of favipiravir-related fluorescence observed in three healthcare providers working in the same ward in our hospital. All three individuals had been diagnosed with COVID-19 two months earlier and were treated with favipiravir. None of the three individuals received hydroxychloroquine or tetracyclines. Wood’s light examination led to an incidental discovery of favipiravir-induced fluorescence involving the sclera, nails, and teeth. In all patients, white linear, square, and band-like specks of fluorescence were noticed on the sclera of both eyes, some teeth, and the proximal part of all fingernails and toenails. Exposure of the eyes to the Wood’s light was for a brief duration of 3 to 5 seconds during examination and photodocumentation. Favipiravir might cause bright white fluorescence of nails, sclera, and teeth, detectable by Wood’s light even two months after its cessation.

Idiopathic cutaneous angiosarcoma (CA) of the head and neck is a distinct subtype of angiosarcoma most commonly presenting as a single or multiple purple, bruise-like patches that arise de novo and enlarge over several months. In clinical practice, both misdiagnosis and delayed diagnosis are frequently encountered. Here, we present a case of idiopathic CA on the scalp with invasion to the cranium in a patient with breast cancer metastatic to the brain. The patient was initially misdiagnosed and mistreated with herpes zoster and breast cancer metastatic to the skin, which led to a delayed diagnosis by two months until dermatologic evaluation. The diagnosis was then firmly established as CA based on consistent clinical and histological features. Since the tumor was inoperable, radiotherapy and chemotherapy were been considered as the appropriate adjuvant modes of therapy. Despite an initial favorable response, the disease demonstrated a rapidly progressive course and the patient succumbed to the disease within six months. This report briefly reviews the clinical and histological portrait and management options for this aggressive tumor.

Bu araştırma ile okul öncesi öğretmenlerinin sınıf yönetimi stratejilerini kullanma sıklığına ilişkin görüşleri ile öğretmenlerin uyguladıkları sınıf yönetimi stratejileri arasındaki uygunluk durumlarının belirlenmesi amaçlanmıştır. Bu çalışma karma araştırma yöntem desenlerinden yakınsayan paralel desen kullanılarak, Ankara ilinde bulunan Milli Eğitim Bakanlığı'na bağlı bağımsız anaokullarında ve ilkokula bağlı anasınıflarında 2015-2016 yılları arasında çalışan gönüllü 72 okul öncesi öğretmeni ile yürütülmüştür. Çalışma grubunda bulunan öğretmenlerden, `Gönüllü Katılım Belgesi` ile sınıf içerisinde gözlem yapabilmek için gerekli izinler alınmıştır. Çalışmanın nicel boyutuna ilişkin veriler; `Kişisel Bilgi Formu`, `Sınıf Yönetiminde Öğretmen Stratejileri Envanteri Özel Öğretim Teknikleri Ölçeği`nden elde edilmiştir. Çalışmanın diğer boyutunda ise bütün öğretmenlerin sınıfında tam gün gözlem sürecinde yapılan anektod kayıtların yanında bu süreçte elde edilen bilgiler çerçevesinde doldurulan `Öğretmen Stratejileri Uygulama Formu` aracılığı ile nitel verilere ulaşılmıştır. Verilerin analizinde betimsel istatistikler, tek yönlü varyans analizi, Scheffe Testi, Kruskal Wallis H Testi, Mann Whitney U Testi ile Bonferroni düzeltmesi kullanılmıştır. Veriler incelendiğinde öğretmenlerin sınıf yönetimi stratejilerinde baskılayıcı teknikleri daha çok kullandıklarının saptanmasının yanı sıra geliştirici stratejilere de süreç içerisinde yer verdikleri belirlenmiştir. Bir diğer bulguda ise okul öncesi öğretmenlerinin geliştirici ve baskılayıcı özel öğretim tekniklerini kullanma düzeylerinin sınıf yönetimi stratejilerini uygulama düzeylerini genel olarak değiştirmediği görülmüştür. Buna rağmen öğretmenlerin geliştirici teknikleri kullanmaları sonucunda etkinlikler arası geçişlerde daha az zaman kaybı yaşadıkları ve istendik davranışları daha fazla destekledikleri bulunmuştur. Bir başka bulguda ise geçişlerde sakin kalan öğretmenlerin baskılayıcı stratejileri kullandıkları tespit edilmiştir. Çalışmanın nitel bulgularında ise öğretmenlerin sınıf ortamına, davranış edinimine, duygu düzenlemesi ve problem çözmeye, disiplin sağlamaya ilişkin stratejileri etkili ve aktif biçimde kullanamadıkları belirlenmiştir. Eğitim akışı ve geçişlere, etkinlik sürecine ilişkin sınıf yönetimi stratejileri aktif olarak kullanılsa da öğretmenlerin desteklenmeye ihtiyaç duydukları; bireysel öğretime, iletişim ve ilişkilerin düzenlenmesine ilişkin stratejileri genel olarak aktif biçimde kullandıkları sonucuna ulaşılmıştır. This study aims to find out the conformity between the pre-school teachers opinions on the frequency of using classroom management strategies and the strategies that teachers actually adopt. The research is mixed method research and carried out by applying the convergent parallel design with 72 volunteer pre-school teachers who work for independent pre-schools which serve under The Turkish Ministry of Education and for nursery schools which are connected to primary schools in Ankara between the years 2015 and 2016. The necessary permission for in-class observation is obtained from teachers in the study group via `Letter of Voluntary Participation`. The data related to the quantitative aspect of the study were obtained via `Personal Information Form` and `The Scale of Special Teaching Methods Inventory of Teacher Strategies in Classroom Management`. In the other aspect of the study, both the anecdote recordings made through a whole-day observation in all teachers' classes and `Teacher Strategy Adaptation From` which was filled according to the information got from the observations led to the collection of qualitative data. While the data are analyzed, descriptive statistics, One Way Analysis of Variance, Scheffe Test, Kruskal Wallis H Test, Mann Whitney U Test and Bonferroni Correction were used. When the data were analyzed, it was concluded that teachers use repressive strategies more common and constructive strategies within the process. Other aspects of the findings show that the level of use of repressive and constructive special teaching methods of pre-school teachers has no effect on the level of the adaptation of classroom management strategies. Nevertheless, it was concluded that teachers lose less time in transitions to following activities and promote terminal behavior more when they make use of constructive strategies. Another finding suggests that teachers who remain calm during transitions use repressive strategies. Qualitative findings of the study show that teachers cannot use the strategies related to classroom environment, behavior acquisition, emotion regulation, problem solving and discipline effectively and actively. It was concluded that teachers can use strategies related to individual teaching, communication and interaction regulation. Although the classroom management strategies related to transitions and the activity process are used actively, it was concluded that teachers need support in the field to improve. Background and Design: The development and maturation of B lymphocytes ensue within the bone marrow independent of the antigen. While going through a complex maturation process within the bone marrow, these cells express surface antigen receptors and eventually achieve functional and phenotypic maturity. Mature naive B cells express IgM and IgD molecules as surface antigen receptors. After completion of the maturation process within the bone marrow, mature naive B cells migrate to peripheric lymphoid organs expecting to meet their specific antigen. Humoral immune response is initiated within the peripheric lymphoid organs when mature naive B cells meet their specific antigen. The antigen receptors of mature naive B cells may directly recognize and be activated with polysaccharide, lipid, glycolipid and nucleic acid antigens and with tiny soluble molecules. However, recognition of and activation by protein antigens requires CD4+ T cell help. The activation of B cell is a complex procedure consisting of sequential steps. For protein antigens these steps involve: 1. Recognition of antigens by B cells and intracellular signaling cascade 2. Antigen presentation by B cells to Th cells 3. B cell activation with the help of Th cells 4. B cell differentiation (development of antibody releasing effector B cells, heavy chain isotype/ class switching, affinity maturation, development of memory B cells) 5. Bacillus thermocatenulatus lipazı (BTL2) yüksek tribütirin seçiciliği olan thermokararlı bir lipazdır ve yapısı aktif- monomer (PDB ID: 2w22) ve inaktif-dimer (PDB ID: 1KU0) olarak atomistik detayda çözülmüştür. Burada, BTL2'nin protein mühendisliği ile ilgili iki vaka çalışması rapor edilmiştir. Protein mühendisliğinin rutin metotlarından olan yönlendirilmiş mutagenez, heterolog protein ekpressiyonu ve purifikasyon, bu tezdeki BTL2 ve mutantları için yapılmıştır. İlk olarak, kısa zincir boyundaki sübstratlara karşı olan aktivasyon bariyerini düşürmek için BTL2'nin akılcı dizaynı gerçekleştirilmiştir. Sonuçlar L360F mutantının BTL2'deki aktivasyonu bariyerini C4 için düşüren ve C8 için yükselten bir mutasyon olduğunu doğrulamıştır. İkinci olarak, yarı-akılcı dizayn kullanarak BTL2'nin kapak kısmında yer alan iki triptofan'nın (W212, W235) etkilerini incelemek için uygunlanmıştır. Özellikle W212A, thermokararlılık ve agregasyon açısından ciddi bir düşüş ve çinko toleransı da bir artış göstermiştir. Bu açıdan, BTL2'nin agregasyonun çinko bağlanma bölgesi üzerinden gerçekleştiğini ve W212 rezidüsünün bu mekanizma için sekonder ama elzem bir rolu olduğu öngörülmektedir. Bacillus thermocatenulatus lipase (BTL2) is a thermostable enzyme with distinct tributyrin selectivity and currently two different resolved structures are publicly available representing the; a) active-monomer form (PDB ID: 2w22) and b) inactive-dimer form (PDB ID: 1KU0). Using the information gained from these two structures, here we report the resultsof two studies related to the protein engineering of BTL2 aiming to improve its chainlength selectivity and to assess the impact of the lid tryptophans. For both of these studies, the experimental approach was to generate specific BTL2 mutants for which site-directed mutagenesis, heterologous protein expression and purification methodologies were used.Inthe first study, a rational design of BTL2 was carried out to lower the activation barrier of hydrolysis of short chain substrates. The results indicate that the mutant L360F optimizes the physical and chemical interactions in BTL2 to lower the activation barrier for C4 and elevate it for C8 hydrolysis. For the second case, a semi-rational design was implemented to investigate the impacts of the two tryptophans (W212, W235) in the lid of BTL2. W235A affected tributyrin selectivity while W212A mutation is resulted in loss of the thermostability and aggregation tendency but also in the improvement of zinc tolerance in BTL2. For this, it is envisioned that the aggregation of BTL2 is mediated through a zinc domain and W212 is a secondary but vital residue for this action. 156

Bu tez çalışmasında, yapay zekâ tekniklerinden olan Uzman Sistemlerin, su kalite kontrol problemlerinin çözümündeki kullanımı incelenmiştir. Uzman Sistemlerle, kalite kontrol faaliyetleri daha etkin uygulanmakta ve kontrol süreleri minimize edilebilmektedir. Bu çalışmada, Melen Nehri üzerindeki 5 ayrı noktadan Ekim 2001-Eylül 2002 tarihleri arasında alınan su numunelerinde yapılan ölçümler sonucunda ortaya çıkan veriler kullanılarak, Su Kirliliği Kontrol Yönetmeliği'ndeki Kıta İçi Su Kaynaklarının sınıflarına ve Uzman Sistem yaklaşımına göre su kalite sınıfları belirlenmiştir. Bu yaklaşımda, su kalite sınıfları, kirliliği yaratan parametreler ve kirletici kaynakların neler olduğu, uzman bilgisine dayanarak tahmin edilmiştir. In this thesis, usage of Expert Systems, which is a type of artificial intelligence technique, is investigated in to solve water quality control problems. By using Expert Systems, quality control processes can be applied more efficiently and control process can be minimized. In this study, water quality classes have been determined by using the outputs of measurements of water samples taken from five different points of River Melen between October 2001-September 2002 in accordance with the ?Categories of the Continental Water Sources in Water Pollution Control Regularitons of Turkey?. In this approach, water quality classes, the parameters causing of the pollution and what the sources of the pollution are were estimated on the basis of expert knowledge.

For this reason, the determination o f pollution level and quality of water for several usage, the monitoring of the variation of quality depends on time, the controlling, the protecting, the rebalancing and the efficient managing of the most important and valuable groundwater have a great importance. This study carried out determination of pollution potential of groundwater used for several purposes and consists of investigation of 19 wells, which have been researching by State Hydraulic Works First Regional Head Office in Bursa. The result of physical, Chemical and microbiological analyses of 19 wells are evaluated between 1990 and 1999, and it is tried to determination of pollution potential of underground water.

Umbilical venous catheters (UVCs) have become a part of routine perinatal care. In the case of its misplacement, extravasation into liver parenchyma might be observed and unusual findings might be detected and a suspicion of tumoral lesions emerges during the ultrasound examination. To avoid the unnecessary liver biopsies and catastrophic complications of UVC misplacement in the pediatric population, clinicians and radiologists must be familiar with the radiological findings. Though small simple renal cysts in an adult over 30-40 years of age are not too unusual, however, if the same cysts are seen in a child, and especially if there are additional findings, then several diagnostic possibilities may come to mind. The role of ultrasound, together with the help of intravenous contrast agents and Doppler mode, are very critical in describing the morphologic features and follow-up of the complex or multiple and bilateral renal cysts. These sonographic signs are occasionally specific for diagnosis, but in many cases sonographic clues should be evalu-ated together with the other genetic and clinical data to reach diagnosis. The first part of this pictorial essay included the introduction into the subject and the classification of non-genetic cystic renal diseases. Çalışmamızın amacı meme MRG'de kullandığımız BI-RADS kategori 3 kriterleri göz önüne alındığında, kategori 3 lezyonların saptanma sıklığını ve bu lezyonlardaki malignite oranını belirlemektedir. Ayrıca bu lezyonlarda kısa aralıklı takip sonuçlarımızı vermek ve takiplere uyumu değerlendirmek yan hedeflerimiz arasındadır.Haziran 2014 ile Şubat 2018 tarihleri arasında meme MRG yapılmış 913 hasta retrospektif olarak tarandı. Bunların 187'sinin BI-RADS kategori 3 olarak değerlendirildiği görüldü. Hastaların demografik bilgilerine, radyoloji raporlarına, biyopsi işlem raporlarına, biyopsi sonuçlarına ve varsa operasyon raporlarına hastane bilgi sistemleri ve bölümümüzde bulunan MRG görüntü arşivi taranarak ulaşıldı. Çalışmaya en az 2 yıllık takipleri olan ya da histopatolojik sonucu bulunan hastalar dahil edildi. Kategori 3 lezyonların özellikleri ve kullanım sıklıkları belirlendi. MRG bulgularına eğer varsa karşılık gelen mamografi ve USG bulguları araştırıldı. Hastalara altı ay sonra, birinci yılında ve ikinci yılında olmak üzere üç kez takip önerilmiş olup eğer MRG'de saptanan lezyon USG veya mamografi ile izlenebiliyorsa takiplere bu yöntemlerle devam edilmesi önerildi. Takip protokolümüze uyum ve malignite oranları hesaplandı. Toplam 913 hastanın 187'si (%20,5) BI-RADS kategori 3 olarak değerlendirilmiştir. Kategori 3 olarak değerlendirilen ve takiplere alınan 69 hastadaki 132 lezyonun %77,3'ü kitle, %11,4'ü kitlesel olmayan kontrast enhansmanı, %4,5'i foküs ve %6,9'u lenf nodu şeklindeydi. Toplamda yedi hastaya biyopsi yapıldı ve lineer/ duktal kontrast enhansmanı bulunan bir hastada sonuç malign geldi (invaziv duktal karsinom, T1N0M0). Toplam 132 lezyondaNÖD %99,2(%95 CI:%98,38 ile %100,09 arasında), malignite oranı ise tek lezyon ile %0,8'dir (%95 CI: %0,15 ile %1,85 arasında). Çalışmamıza dahil edilen toplam 69 hastadan %89,9'u önerdiğimiz iki yıllık takip protokolüne en az iki veya daha fazla sayıda gelerek yeterli uyum sağlamıştır.Sonuç olarak; meme MRG'de kategori 3 lezyonlar için malignite oranımız MG/US'deki kategori 3 ile takip edilen lezyonlarda beklenen malignite oranı ile benzerdir. Ayrıca negatif öngörü değerimiz yüksektir. Bu nedenle meme MRG'de saptanan lezyonlar için uyguladığımız kategori 3 kriterlerimizin uygun olduğunu ve kategori 3 tanımını karşıladığını düşünmekteyiz. Ancak mamografi ve ultrasonografi incelemelerine kıyasla meme MRG'de BI-RADS kategori 3 lezyonların morfolojik ve kinetik özellikleri, görülme sıklıkları, malignite oranları, uyulması gereken takip aralığı hakkında literatürde sınırlı sayıda veri bulunmakta olup daha geniş çaplı ve çok merkezli çalışmalara ihtiyaç vardır.Anahtar kelimeler: Meme, MRG, BI-RADS kategori 3. Our aim is to determine the frequency of BI-RADS category 3 lesions detected on MRG and analyze the malignancy rate in these lesions diagnosed under our criteria. Also to present MRI findings and to evaluate the patient compliance to our short-term follow-up protocol are our secondary purposes.We conducted this study retrospectively and after the permission of our ethic committee (Number: 70904504/115, Board Code: 2012-KAEK-20 / Date: 12.03.2018). From our PACS and hospital information systems (MEDI-HASTA 16.43 1997-2014, MIA-MED 1.0.1.3181 2014-2016, Siemens Leonardo Workstation 2009, SECTRA PACS IDS7 18.2.14.4021 2016), we found 913 patients undergone dynamic breast MRI between June 2014 and February 2018. Of these, 187 women had BI-RADS category 3 assessment. We excluded the patients who are not completing or participating to our follow-up protocol and whose biopsy results are not available. Finally 69 patients with 132 category 3 lesions, whose follow-up results and/or annual reports are available, composed our study group. We evaluated MRI findings comparing by mammography and US findings in these patients. Our short-term follow-up protocol on MRI is composed of three examinations at 6-month intervals for first year, and final examination at 24thmonth. Besides, we recommend age –appropriate screenings for the patients whose lesion is stabile after two years. If the category 3 lesions on MRI is seen also on mammography and/or US, we recommend to go on follow-ups with these modalities instead of MRI. The detection frequency, the malignancy rate, negative predictive value, the compliance with our follow-up protocol considering 95% CI were analyzed.The detection frequency on MRI for BI-RADS category 3 lesions was 20.5%. The distribution of MRI findings was as follows: 77.3% mass, 11.4% nonmasslike contrast enhancement, 4.5% focus, 6.9% lymph node. In total, seven lesions underwent core or excisional biopsy, which we recommend biopsy four of them because of interval progression during follow-ups. Biopsy yielded invasive ductal carcinoma in only one lesion (T1N0M0) showing ductal contrast enhancement on MRI. Our NPV was 99,2% (95% CI: from 98,38% to 100,09%). The malignancy rate was calculated as 0.8% (95% CI: from 0.15% to 1.85%). Of 69 patients, 89% of them complied our follow-up protocol by attending at least twice.As a conclusion, malignancy rate with a 0.8% rate is very low in BI-RADS category 3 lesions detected on MRI similar to low malignancy rate provided with mammography and ultrasound. Çalışmamızda, bölümümüze ikinci trimestr rutin obstetrik USG amacıyla yönlendirilen 20-24 haftalık gebelerde fetal kalbin sağ ve sol ventrikül genişlikleri, sağ ve sol atirum genişlikleri, sağ ve sol ventrikül duvar kalınlıkları, ventrikül genişlik oranı, atrium genişlik oranı, ventrikül duvar kaınlık oranı, kalp çevresi, toraks çevresi ve kardiyotorasik oranının normal değerlerini ve nomogramlarını saptamayı amaçladık.Aralık 2015-Haziran 2016 tarihleri arasında ikinci trimestr obstetrik organ taraması amacıyla bölümümüze refere edilen ardışık 371 gebe prospektif olarak değerlendirildi. Çalışmaya dahil edilen fetuslara detaylı kardiyak USG yapıldı. Memenin kompleks kistik lezyonları malignite açısından kuşkulu lezyonlardır. Bu çalışmamızda bölümümüzde saptanan ve patolojik sonucu olan kompleks meme kistlerinin radyolojik bulgularını ve patolojik sonuçlarını gözden geçirdik. Kompleks kistik lezyonların maligniteyi öngören ve öne çıkaran mamografik ve sonografik bulgularını saptamayı amaçladık.Ocak 2004 - Aralık 2012 tarihleri arasında ince iğne aspirasyon biyopsisi, sonografi ya da mamografi eşliğinde işaretleme sonrası eksizyonel biyopsi, sonografi eşliğinde kor/vakum biyopsi yapılan 1578 hastanın kayıtları incelendi. Sonografisinde kalın duvar ve/veya kalın septa, solid komponent (mural nodül ve/veya büyük oranda kistik solid komponent) ya da birden fazla sonografik özelliği birlikte içeren (çoklu sonografik bulgu) kistler kompleks kist olarak tanımlandı. 61 hastadaki toplam 66 kompleks kistik lezyon çalışmaya dahil edildi. Her bir sonografik ve mamografik bulgu için pozitif prediktif değer (PPD%) hesaplandı. Malignite ile bağlantılı sonografik ve mamografik bulguların karşılaştırılması Pearson Ki-kare testi ve Fishers? Exact test ile yapıldı. P değeri 0.05?ten küçük olan tüm sonuçlar istatistiksel olarak anlamlı sayıldı.Toplam 66 kompleks meme kistinin 28?inde (%42) kalın duvar ve/veya kalın septa 38?inde (%58) solid komponent vardı. Mamografide en sık bulgu kitle opasitesi olup 6 lezyona mikrokalsifikasyon eşlik etmekteydi. Toplamda 66 lezyonun 56?sı (%85) benign, 10?u (%15) malign histopatolojik sonuç aldı. Malign sonuçların 4?ü invaziv duktal karsinom, 4?ü intraduktal papiller karsinom ve 2?si müsinöz karsinomdu. En sık benign sonuçlar fibrokistik değişiklikler, yağ nekrozu ve apselerdi. Sonografik bulgulara göre PPD (%) kalın duvar ve/veya kalın septa için 4, solid komponent için 24 olarak bulundu. Bu iki sonografik özellik açısından p değeri 0.036 olarak bulundu. Mamografik bulgulara göre PPD (%) kitle opasitesi için 27, kitle opasitesi ve mikrokalsifikasyon birlikteliği için 33?tür. In our study, we reviewed the radiological findings and pathological evaluations of complex breast cysts that were detected by our department and had pathology reports. We aimed to determine the mammographic and sonographic features of complex cystic lesions that predicted and favored malignancy.Records of 1578 patients that underwent fine needle aspiration biopsy, excisional biopsy after either sonography or mammography guided wire localization, or core/vacuum biopsy under sonography between January 2004 - December 2012 were examined. The cysts that demonstrated thick wall and/or thick septae, solid component (mural nodule and/or a predominantly cystic solid component) or more than one sonographic feature combined (multiple sonographic finding) were defined as complex cysts. A total of 66 complex cystic lesions in 61 patients were included in the study. Positive predictive value (%PPD) was calculated for each sonographic and mammographic finding. The analysis of sonographic and mammographic findings in relation to malignancy was done with Pearson chi-square test and Fishers? Exact test. All results of a p value of less than 0.05 were considered statistically significant.Out of a total of 66 complex breast cysts, 28 (42%) had thick walls and/or septae; and 38 (58%) had solid components. The most common finding in mammography was mass with well-defined margins. Six masses seen on mammography had microcalcification additionally. Histopathological evaluation of 66 lesions revealed 56 (85%) benign and 10 (15%) malign lesions. Malign lesions included 4 invasive ductal carcinomas, 4 intraductal papillary carcinomas and 2 mucinous carcinomas. The most common benign results were fibrocystic change, fat necrosis and abscesses. PPD (%) for sonographic findings was 4 for thick wall and/or septae and 24 for solid component. The p value for these two sonographic features was 0.036. PPD (%) for mammographic findings was 27 for mass, and 33 for the combination of mass and microcalcification. The p value for mammographic features was calculated to be 0.133.According to our study, even though the likelihood of malignancy is relatively higher in complex cysts with solid components compared to other complex cysts, no specific finding has been found that would indicate malignancy in complex cysts.
