Abstract

Quantum mechanically, photoionization can be fully described by the complex photoionization amplitudes that describe the transition between the ground state and the continuum state. Knowledge of the value of the phase of these amplitudes has been a central interest in photoionization studies and newly developing attosecond science, since the phase can reveal important information about phenomena such as electron correlation. We present a new attosecond-precision interferometric method of angle-resolved measurement for the phase of the photoionization amplitudes, using two phase-locked Extreme Ultraviolet pulses of frequency $\omega$ and $2\omega$, from a Free-Electron Laser. Phase differences $\Delta \tilde \eta$ between one- and two-photon ionization channels, averaged over multiple wave packets, are extracted for neon $2p$ electrons as a function of emission angle at photoelectron energies 7.9, 10.2, and 16.6 eV. $\Delta \tilde \eta$ is nearly constant for emission parallel to the electric vector but increases at 10.2 eV for emission perpendicular to the electric vector. We model our observations with both perturbation and \textit{ab initio} theory, and find excellent agreement. In the existing method for attosecond measurement, Reconstruction of Attosecond Beating By Interference of Two-photon Transitions (RABBITT), a phase difference between two-photon pathways involving absorption and emission of an infrared photon is extracted. Our method can be used for extraction of a phase difference between single-photon and two-photon pathways and provides a new tool for attosecond science, which is complementary to RABBITT.

Abstract

The hitherto unexplored two-photon doubly-excited states [Ne$^{*}$($2p^{-1}3s$)]$_{2}$ were experimentally identified using the seeded, fully coherent, intense extreme ultraviolet free-electron laser FERMI. These states undergo ultrafast interatomic Coulombic decay (ICD) which predominantly produces singly-ionized dimers. In order to obtain the rate of ICD, the resulting yield of Ne$_{2}^{+}$ ions was recorded as a function of delay between the XUV pump and UV probe laser pulses. The extracted lifetimes of the long-lived doubly-excited states, 390 (-130 / +450} fs, and of the short-lived ones, less than 150~fs, are in good agreement with \emph{ab initio} quantum mechanical calculations.

Abstract

X-ray free-electron lasers (FELs), which amplify light emitted by a relativistic electron beam, are extending nonlinear optical techniques to shorter wavelengths, adding element specificity by exciting and probing electronic transitions from core levels. These techniques would benefit tremendously from having a stable FEL source, generating spectrally pure and wavelength-tunable pulses. We show that such requirements can be met by operating the FEL in the so-called echo-enabled harmonic generation (EEHG) configuration. Here, two external conventional lasers are used to precisely tailor the longitudinal phase space of the electron beam before emission of X-rays. We demonstrate high-gain EEHG lasing producing stable, intense, nearly fully coherent pulses at wavelengths as short as 5.9 nm (~211 eV) at the FERMI FEL user facility. Low sensitivity to electron-beam imperfections and observation of stable, narrow-band, coherent emission down to 2.6 nm (~474 eV) make the technique a prime candidate for generating laser-like pulses in the X-ray spectral region, opening the door to multidimensional coherent spectroscopies at short wavelengths.

4. The ISOBATA Project: A FAIR-Compliant Approach to Bathymetric Data Collection for Seabed Coverage in the Antarctic Region

Abstract

The ISOBatA project, funded by the Italian National Antarctic Research Program (PNRA), aims to optimize underway bathymetric data collection in the Antarctic region. This initiative supports the Seabed2030 and the International Bathymetric Chart of the Southern Ocean (IBCSO) projects by allocating dedicated ship time of the R/V Laura Bassi to navigate (1) along designated corridors in uncharted regions of the Southern Ocean, including the Emerald Fracture Zone and the Macquarie Triple Junction, and (2) among yearly monitored locations of the Ross Sea. ISOBatA emphasizes the reuse of existing datasets using the IBCSO coverage map and metadata from prior PNRA expeditions. It also aims to establish best practices and workflows for high-quality bathymetric data acquisition, processing, analysis, and archiving. Bathymetric data collected over the last three PNRA expeditions were integrated with other datasets (e.g., magnetometric, sub-bottom profiling, and oceanographic data) to create maps and validate the approach. Significant discrepancies (up to 1200 meters) were found along the Emerald Fracture Zone in an area where the IBCSO contains only predicted satellite bathymetry, highlighting gaps in our understanding of geodynamic processes. In the Ross Sea, various submarine landforms were detected, revealing the extent of processes shaping the recent evolution of the Antarctic margin (from bottom currents to ice retreat during the last deglaciation). The ISOBatA experience, dataset and findings underscore the importance of collaboration among institutions and research groups to share data and resources and how the application of FAIR principles and practices to underway data collection is critical to expand seafloor coverage, especially in remote areas like the Antarctic region.

We have examined the transmission of soft X-ray pulses from the FERMI free electron laser through carbon films of varying thickness, quantifying nonlinear effects of pulses above and below the carbon K-edge. At typical of soft X-ray free electron laser intensities, pulses exhibit linear absorption at photon energies above and below the K-edge, ∼308 and ∼260 eV, respectively; whereas two-photon absorption becomes significant slightly below the K-edge, ∼284.2 eV. The measured two-photon absorption cross section at 284.18 eV (∼6 × 10−48 cm4 s) is 7 orders of magnitude above what is expected from a simple theory based on hydrogen-like atoms - a result of resonance effects.

Abstract

XUV and X-ray Free Electron Lasers (FELs) produce short wavelength pulses with high intensity, ultrashort duration, well-defined polarization and transverse coherence, and have been utilised for many experiments previously possible at long wavelengths only: multiphoton ionization, pumping an atomic laser, and four-wave mixing spectroscopy. However one important optical technique, coherent control, has not yet been demonstrated, because Self- Amplified Spontaneous Emission FELs have limited longitudinal coherence. Single-colour pulses from the FERMI seeded FEL are longitudinally coherent, and two-colour emission is predicted to be coherent. Here we demonstrate the phase correlation of two colours, and manipulate it to control an experiment. Light of wavelengths 63.0 and 31.5 nm ionized neon, and the asymmetry of the photoelectron angular distribution was controlled by adjusting the phase, with temporal resolution 3 attoseconds. This opens the door to new shortwavelength coherent control experiments with ultrahigh time resolution and chemical sensitivity.

Abstract

Nonlinear optical processes at soft x-ray wavelengths have remained largely unexplored due to the lack of available light sources with the requisite intensity and coherence. Here we report the observation of soft x-ray second harmonic generation near the carbon K edge (∼284  eV) in graphite thin films generated by high intensity, coherent soft x-ray pulses at the FERMI free electron laser. Our experimental results and accompanying first-principles theoretical analysis highlight the effect of resonant enhancement above the carbon K edge and show the technique to be interfacially sensitive in a centrosymmetric sample with second harmonic intensity arising primarily from the first atomic layer at the open surface. This technique and the associated theoretical framework demonstrate the ability to selectively probe interfaces, including those that are buried, with elemental specificity, providing a new tool for a range of scientific problems.

10. VUV and X-ray coherent light with tunable polarization from single-pass free-electron lasers

Abstract

Tunable polarization over a wide spectral range is a required feature of light sources employed to investigate the properties of local symmetry in both condensed and low-density matter. Among new-generation sources, free-electron lasers possess a unique combination of very attractive features, as they allow to generate powerful and coherent ultra-short optical pulses in the VUV and X-ray spectral range. However, the question remains open about the possibility to freely vary the light polarization of a free-electron laser, when the latter is operated in the so-called nonlinear harmonic-generation regime. In such configuration, one collects the harmonics of the free-electron laser fundamental emission, gaining access to the shortest possible wavelengths the device can generate. In this letter we provide the first experimental characterization of the polarization of the harmonic light produced by a free-electron laser and we demonstrate a method to obtain tunable polarization in the VUV and X-ray spectral range. Experimental results are successfully compared to those obtained using a theoretical model based on the paraxial solution of Maxwell's equations. Our findings can be expected to have a deep impact on the design and realization of experiments requiring full control of light polarization to explore the symmetry properties of matter samples.

Abstract

Morphobathymetry, a versatile and invaluable hydrographic resource, enriches scientific investigations and deepens our understanding of the Antarctic region across a range of scientific disciplines. Its use benefits scientific disciplines such as geophysics and geodynamics, integrating with other fields of research, revealing complex relationships between the seabed and oceanographic and biological phenomena. The interconnections between seafloor shape, ocean-atmosphere dynamics, and biogeochemical interactions unveil indeed a new dimension of understanding. Despite data quality does not often meet IHO standards, often due to vessel speed, and the challenges of collecting high-quality data during transits, we emphasize the value of these efforts. ISOBATA's data from Laura Bassi's recent Antarctic expeditions, although insufficient for a comprehensive mapping, effectively fills critical bathymetric gaps in areas typified by remarkable and previously unknown tectonic features. The ISOBatA project, funded by the Italian National Antarctic Research Program (PNRA), spanned a two-year period with the aim of optimizing data collection on board the Italian icebreaker Laura Bassi during its transits in the Southern Ocean. The project focused on the acquisition of acoustic and magnetometric data, with an emphasis on morphobathymetry. ISOBatA used ship time to reduce the vessel’s speed and travel along preferential corridors designated to survey unexplored areas. Additionally, ISOBATA extensively leveraged metadata from previous PNRA expeditions and the IBCSO coverage map, enhancing research efficiency and increasing the amount, quality, and scientific value of gathered data. Abstract

The experience of the PNRA ISOBatA (Italian Southern Ocean Bathymetry from consistent exploitation of opportunistic seafloor datasets in Antarctic region and surrounding areas) project, over the past two years, has shown the importance of collecting underway bathymetric data by exploiting the transit time between the Italian Mario Zucchelli Station in the Ross Sea and New Zealand, during Antarctic expeditions. Mapping the Antarctic seafloor is a challenging goal in terms of critically advancing our knowledge of seabed morphology, especially when strategically coupled with field observations. High-resolution mapping of the Southern Ocean seafloor may offer valuable insights into tectonic and volcanic processes of plate boundaries in this remote environment. The ISOBatA project collected in particular underway bathymetric data on board the Italian R/V Laura Bassi at the Macquarie Triple Junction and crossing the Emerald Fracture Zone, where also magnetic data were obtained. From a methodological point of view, the ISOBatA project developed dedicated procedures to ensure robust multibeam data quality during the acquisition, processing and archiving phases, to also provide a data sharing policy according to FAIR principles. The expeditions in the Southern Ocean revealed critical problems mainly related to environmental conditions associated to the working area (i.e.: ice-populated waters and/or adverse weather conditions) but at the same time promoted the implementation of appropriate best practices and workflows, also defined according to the geomorphological context of the working sectors. Our main objective is to promote common and shared procedures for the management of multibeam bathymetric data acquisition during transit times, and to support the current scientific effort that all interested countries are undertaking to achieve a systematic exploration of the seafloor, especially for remote areas such as the Arctic, the Southern or the equatorial Atlantic Oceans.

Abstract

Background & Aims: Split liver transplant(ation) (SLT) is still considered a challenging procedure that is by no means widely accepted. We aimed to present data on 25-year trends in SLT in Italy, and to investigate if, and to what extent, outcomes have improved nationwide during this time. Methods: The study included all consecutive SLTs performed from May 1993 to December 2019, divided into three consecutive periods: 1993–2005, 2006–2014, and 2015–2019, which match changes in national allocation policies. Primary outcomes were patient and graft survival, and the relative impact of each study period. Results: SLT accounted for 8.9% of all liver transplants performed in Italy. A total of 1,715 in situ split liver grafts were included in the analysis: 868 left lateral segments (LLSs) and 847 extended right grafts (ERGs). A significant improvement in patient and graft survival (p <0.001) was observed with ERGs over the three periods. Predictors of graft survival were cold ischaemia time (CIT) <6 h (p = 0.009), UNOS status 2b (p <0.001), UNOS status 3 (p = 0.009), and transplant centre volumes: 25–50 cases vs. <25 cases (p = 0.003). Patient survival was significantly higher with LLS grafts in period 2 vs. period 1 (p = 0.008). No significant improvement in graft survival was seen over the three periods, where predictors of graft survival were CIT <6 h (p = 0.007), CIT <6 h vs. ≥10 h (p = 0.019), UNOS status 2b (p = 0.038), and UNOS status 3 (p = 0.009). Retransplantation was a risk factor in split liver graft recipients, with significantly worse graft and patient survival for both types of graft (p <0.001). Conclusions: Our analysis showed Italian SLT outcomes to have improved over the last 25 years. These results could help to dispel reservations regarding the use of this procedure. Abstract

In this note we describe a conceptual design of a part of the electron beam delivery system for FERMI@Elettra free electron laser (FEL) located between the end of the linac and the entrance to the FEL. This part includes the emittance diagnostic section, the electron beam switchyard for two FELs called spreader and matching sections. The design meets various constrains imposed by the existing and planned building boundaries, desire for utilization of existing equipment and demands for various diagnostic instruments.

Abstract

We describe the machine layout and major performance parameters for the FERMI FEL project funded for construction at Sincrotrone Trieste, Italy, within the next five years. The project will be the first user facility based on seeded harmonic cascade FELs, providing controlled, high peak-power pulses. With a high-brightness rf photocathode gun, and using the existing 1.2 GeV S-band linac, the facility will provide tunable output over a range from ~;100 nm to ~;10 nm, with pulse duration from 40 fs to ~; 1 ps, peak power ~;GW, and with fully variable output polarization. Initially, two FEL cascades are planned; a single-stage harmonic generation to operate > 40 nm, and a two stage cascade operating from ~;40 nm to ~;10 nm or shorter wavelength. The output is spatially and temporally coherent, with peak power in the GW range. Lasers provide modulation to the electron beam, as well as driving the photocathode and other systems, and the facility will integrate laser systems with the accelerator infrastructure, including a state-of-the-art optical timing system providing synchronization of rf signals, lasers, and x-ray pulses. Major systems and overall facility layout are described, and key performance parameters summarized.

Abstract

Transits during oceanographic expeditions constitute a potential huge amount of acquired bathymetric data that could be systematically integrated to increase the knowledge on submarine morphology, especially for planned surveys in the equatorial Atlantic, the Arctic, the Indian and the Southern Oceans. The recent PNRA ISOBatA project aims to efficiently exploit seafloor datasets collected during transfer times within the Antarctic region and the Ross Sea. Along the route from New Zealand to the Italian Mario Zucchelli Station in the Ross Sea, the Emerald Fracture Zone and the Macquarie Triple Junction, located in the SW Pacific Ocean, represent two selected areas to test the strength of transit acquisition in remote areas, normally affected by adverse weather conditions. The ISOBatA project has the main purpose to contribute to the mapping of Antarctic waters developing best practices and dedicated workflows to implement QA in multibeam data acquisition procedures during transit times, as well as in the processing, analysis and archiving of data and metadata. The ISOBatA experience in the Southern Ocean suggests there are several critical issues associated with collection of multibeam data in remote and ice-infested waters. Operating procedures need more standardization, to avoid the acquisition of redundant data along common routes and unreliable data. Our work aims to open a discussion to address the need for standardization in data acquisition during transit times, which should include priority in accordance with the geomorphological/geographical nature of the working areas. The integration of bathymetric data acquired during research vessel transfers to remote regions could imply a common international effort for a systematic exploration of the seafloor, sharing coverage in real time to avoid redundancy.

18. Successful Transplant of a Liver Graft After Giant Hepatic Artery Aneurysm Resection and Reconstruction

Abstract

The shortage of organs has pushed transplant surgeons to accept liver grafts with extended criteria, but severe vascular abnormalities may still discourage the use of otherwise acceptable organs. We report herein the case of a liver graft with a 64-mm aneurysm of the proper hepatic artery extended to the origin of the right and left hepatic branches. The graft was deemed unsuitable for transplant by all other centers in the region. However, liver function tests were normal, and there was no evidence of compromised arterial supply. At back table, we resected the aneurysm and anastomosed the right and left hepatic arteries to a vascular graft obtained from the distal tract of the donor's superior mesenteric artery. After portal reperfusion, we anastomosed the mesenteric graft to the recipient's hepatic artery at the origin of the gastroduodenal artery. The postoperative course and the subsequent 6-month follow-up were uneventful. In conclusion, the presence of a hepatic artery aneurysm should not be an absolute contraindication to the use of a liver graft. The present case emphasizes the possibility to utilize an organ that would have been otherwise discarded.

Abstract

The Ionian slopes of the Calabrian continental margin have been affected by mass movements of varying style, scale and age. Here we present examples of seabed and subsurface features identified from recently acquired multibeam morpho-bathymetric data and sub-bottom profiles, being interpreted in the framework of the MAGIC project. Four different types of mass wasting phenomena are recognized: mass transport complexes (MTCs) within intra-slope basins, isolated slide scars on slopes, possible features of gravity sliding above Messinian salt and headwall scarps in canyons. An improved understanding of past and present processes of mass failure on the Calabrian margin is strategic for assessments of the geohazards they may represent.

Abstract

Data management in the marine sciences faces the complex issue of addressing contrasting cognitive models across scientific domains, meaning the various representations that different communities build for overlapping areas of interest. Each of these communities constructs an identity on a specific and personal set of inherited backgrounds, practices, and tacit knowledge, which are mirrored in how they understand the targets of their studies. From a practical point of view, this can result in mis-linking observations and usage. This paper reports on the work done within the EU FP7 SeaDataNet project to tackle such problems through an integrated discovery and data access paradigm. It is based on a flexible metadata model that allows researchers to link domain-specific metadata profiles encoded using SensorML OGC standard, the general discovery framework based on an ISO 19115/19139 profile called CDI, and the data through a common hub based on the Observations and Measurements OGC standard.

Abstract

34-year-old man with chronic renal and pancreas failure in complicated diabetic disease received a kidney-pancreas transplantation. On the 32nd postoperative day, an acute kidney rejection occurred and resolved with OKT3 therapy. The patient also presented refractory urinary infection by E. Fecalis and M. Morganii, and a focal bronchopneumonia in the right-basal lobe resolved with elective chemotherapy. During the 50th post-operative day, an intense soft tissue inflammation localized in the first left metatarsal-phalangeal articulation occurred (Figure 1) followed by an abscess with a cutaneous fistula and extension to the almost totality of foot area. The radiological exam revealed a small osteo-lacunar image localized in the proximal phalanx head of the first finger foot. From the cultural examination of the purulent material, N. Asteroides was identified. An amoxicillin-based treatment was started and continued for three months, with the complete resolution of infection This case is reported for its rarity in our casuistry, and for its difficult differential diagnosis with other potentially serious infections.

Abstract

The study of contourite drifts is an increasingly used tool for understanding the climate history of the oceans. In this paper we analyse two contourite drifts along the continental margin west of Spitsbergen, just south of the Fram Strait where significant water mass exchanges impact the Arctic climate. We detail the internal geometry and the morphologic characteristics of the two drifts on the base of multichannel seismic reflection data, sub-bottom profiles and bathymetry. These mounded features, that we propose to name Isfjorden and Bellsund drifts, are located on the continental slope between 1200 and 1800 m depth, whereas the upper slope is characterized by reduced- or non-deposition. The more distinct Isfjorden Drift is about 25 km wide and 45 km long, and over 200 ms TWT thick. We revise the 13 years-long time series of velocity, temperature, and salinity obtained from a mooring array across the Fram Strait. Two distinct current cores are visible in the long-term average. The shallower current core has an average northward velocity of about 20 cm/s, while the deeper bottom current core at about 1450 m depth has an average northward velocity of about 9 cm/s. We consider Norwegian Sea Deep Water episodically ventilated by relatively dense and turbid shelf water from the Barents Sea responsible for the accumulation of the contourites. The onset of the drift growth west of Spitsbergen is inferred to be about 1.3 Ma and related to the Early Pleistocene glacial expansion recorded in the area. The lack of mounded contouritic deposits on the continental slope of the Storfjorden is related to consecutive erosion by glacigenic debris flows. The Isfjorden and Bellsund drifts are inferred to contain the record of the regional palaeoceanography and glacial history and may constitute an excellent target of future scientific drilling.

32. Fine mapping of a replication origin of human DNA.

Abstract

A highly sensitive procedure was developed for the identification of the origin of bidirectional DNA synthesis in single-copy replicons of mammalian cells. The method, which does not require cell synchronization or permeabilization, entails the absolute quantification, by a competitive PCR procedure in newly synthesized DNA samples, of the abundance of neighboring DNA fragments distributed along a given genomic region. This procedure was utilized for mapping the start site of DNA replication in a 13.7-kb region of human chromosome 19 coding for lamin B2, which is replicated immediately after the onset of S phase in HL-60 cells. Within this region, DNA replication initiates in a 474-bp area corresponding to the 3' noncoding end of the lamin B2 gene and the nontranscribed spacer between this gene and the 5' end of another highly transcribed one. This localization was obtained both in aphidicolin-synchronized and in exponentially growing HL-60 cells.

Abstract

By the use of a highly sensitive mapping procedure allowing the identification of the start sites of DNA replication in single-copy genomic regions of untreated, exponentially growing cultured cells (M. Giacca, L. Zentilin, P. Norio, S. Diviacco, D. Dimitrova, G. Contreas, G. Biamonti, G. Perini, F. Weighardt, S. Riva, and A. Falaschi, Proc. Natl. Acad. Sci. USA 91:7119-7123, 1994), the pattern of DNA replication of the Chinese hamster dihydrofolate reductase (DHFR) gene domain was investigated. The method entails the purification of short stretches of nascent DNA issuing from DNA replication origin regions and quantification, within this sample, of the abundance of different adjacent segments by competitive PCR. Distribution of marker abundance peaks around the site from which newly synthesized DNA had emanated. The results obtained by analysis of the genomic region downstream of the DHFR single-copy gene in asynchronous cultures of hamster CHO K1 cells are consistent with the presence of a single start site for DNA replication, located approximately 17 kb downstream of the gene. This site is coincident with the one detected by other studies using different techniques in CHO cell lines containing an amplified DHFR gene domain.

Abstract

Large networks of fixed or mobile sensors based on Internet of Things technologies generate a large amount of data, that requires specific methods for preprocessing, averaging, interpolation and graphic representation. In this, bidimensional scalar data are taken into account; we consider the issues of window averaging, interpolation, and convex hull definition. The considered scenario is a fully automated, near real-time acquisition and visualisation system. The aim of this work is to compare the quality and the computational cost of various interpolation methods, while highlighting the possible risks behind the need to introduce experts' choices in some of them. In this, some algorithmic optimisations and comparisons are presented, based on both simulated and real data. We present some software solutions for the problems of window averaging and buffering of the convex hull and some tests on interpolation methods like Inverse Distance Weighting and Natural Neighbours to determine which is the most appropriate for our real-time measurements. Abstract

Within the United Nations Sustainable Development 2030 agenda, sustainable growth in the marine and maritime sector needs sea water quality monitoring. This is a very demanding and expensive task which results in the sea being largely undersampled. MaDCrow is a research and development project supported by the European Regional Development Fund, that involves citizens as data collectors while aiming to improve public environmental awareness and participation in scientific research. Its goal is to create an innovative technological infrastructure for real-time acquisition, integration and access of data, thus generating knowledge on sea water quality and marine ecosystem of the Gulf of Trieste. Data acquisition is based on an autonomous and removable device, developed within the project, that can be deployed on any small size sailing boat, recreational vessel, or fishing boat. The device holds low-cost sensors to measure pH, temperature, dissolved oxygen and salinity and the hardware and software to acquire, georeference and transmit the environmental data without interfering with the activities of the boats. In this work we analyze the use, capabilities and advantages of low-cost sensors but also their limitations, comparing, with a special focus on pH, their performances with those of the traditional ones. Applying the paradigm in a highly anthropized area such as the Gulf of Trieste, which is characterized also by a very high spatial and temporal variability of environments, we point out that this new approach allows to monitor sea water quality and highlight local anomalies with a resolution and spatial and temporal coverage that was not achievable with previous procedures, but yet at very low costs. Once received, data are then processed and submitted to a mediation flow that contextualizes and disseminates them for public use on a website. The final products have been customized to reach stakeholders such as tourists, fishermen and policy makers. The availability of information understandable to everyone, while fostering environmental awareness, stimulates, at the same time, involvement and participation of citizen scientists in the initiative. In the future, while committing to enlarge the number of participants, we will extend the analysis also toward other types of sensors.

The shortage of organs has pushed transplant surgeons to accept liver grafts with extended criteria, but severe vascular abnormalities may still discourage the use of otherwise acceptable organs. We report herein the case of a liver graft with a 64-mm aneurysm of the proper hepatic artery extended to the origin of the right and left hepatic branches. The graft was deemed unsuitable for transplant by all other centers in the region. However, liver function tests were normal, and there was no evidence of compromised arterial supply. At back table, we resected the aneurysm and anastomosed the right and left hepatic arteries to a vascular graft obtained from the distal tract of the donor's superior mesenteric artery. After portal reperfusion, we anastomosed the mesenteric graft to the recipient's hepatic artery at the origin of the gastroduodenal artery. The postoperative course and the subsequent 6-month follow-up were uneventful. In conclusion, the presence of a hepatic artery aneurysm should not be an absolute contraindication to the use of a liver graft. The present case emphasizes the possibility to utilize an organ that would have been otherwise discarded.

Abstract

Aim: To compare survival and recurrence after laparoscopic liver resection (LLR) and laparoscopic radiofrequency ablation (LRFA) for the treatment of small hepatocellular carcinoma (HCC)., Methods: Between June 1, 2005 and November 30, 2010, 46 patients (62.26 ± 8.55 years old; female/male: 12/34) treated for small HCC were enrolled following strict criteria. Patients with better liver function and larger tumors were referred for LLR ( n = 24), while those with poorer liver function and multiple tumors were referred for LRFA ( n = 22), and they were then followed for similar durations (44.74 ± 21.3 mo for LLR vs 40.27 ± 30.8 mo for LRFA)., Results: The LLR and LRFA groups were homogeneous with regard to age, sex, etiology of liver cirrhosis, and AFP levels. The overall survival (OS) and disease-free survival (DFS) probability was 0.354 and 0.260, respectively. A significantly higher OS was observed in the LLR group (LLR: 0.442; LRFA: 0.261; P = 0.048), whereas no statistical difference was found for DFS (LLR: 0.206; LRFA: 0.286; P = 0.205). In the LRFA group was treated a greater number of nodules (LLR: 1.41 ± 0.77; LRFA: 2.72 ± 1.54; P < 0.001). Cox regression analysis found the number of intraoperative HCC nodules as the unique variable statistically significant for OS (hazard ratio: 2.225; P < 0.001). The rank-hazard plot showed a steeper increase of relative hazard for intraoperative nodules > 2., Conclusion: Our preliminary results confirm the superiority of hepatic resection on thermoablation in the treatment of small HCC in selected patients, when both approaches are made laparoscopically. LLR showed better results compared to LRFA in terms of OS. Abstract

Background: The aim of this study was to compare the effectiveness of laparoscopic liver resection (LLR) and laparoscopic radiofrequency ablation (LRFA) in the treatment of small nodular hepatocellular carcinoma (HCC)., Patients and Methods: We enrolled 50 cirrhotic patients with similar baseline characteristics that underwent LLR (n = 26) or LRFA (n = 24), in both cases with intraoperative ultrasonography. Operative and peri-operative data were retrospectively evaluated., Results: LLR included anatomic resection in eight cases and non-anatomic resection in 18. In LRFA patients, a thermoablation of 62 nodules was achieved. Between LLR and LRFA groups, a significant difference was found both for median diameters of treated HCC nodules (30 vs. 17.1 mm; P < 0.001) and the number of treated nodules/patient (1.29 ± 0.62 vs. 2.65 ± 1.55; P < 0.001). A conversion to laparotomy occurred in two LLR patient (7.7%) for bleeding. No deaths occurred in both groups. Morbidity rates were 26.9% in the LLR group versus 16.6% in the LRFA group (P = 0.501). Hospital stay in the LLR and LRFA group was 8.30 ± 6.52 and 6.52 ± 2.69 days, respectively (P = 0.022). The surgical margin was free of tumour cells in all LLR patients, with a margin <5 mm in only one case. In the LRFA group, a complete response was achieved in 90.3% of thermoablated HCC nodules at the 1-month post-treatment computed tomography evaluation., Conclusions: LLR for small peripheral HCC in patients with chronic liver disease represents a valid alternative to LRFA in terms of patient toleration, surgical outcome of the procedure, and short-term morbidity.

44. Two-stage liver transplantation with temporary porto-middle hepatic vein shunt.

Abstract

Two-stage liver transplantation (LT) has been reported for cases of fulminant liver failure that can lead to toxic hepatic syndrome, or massive hemorrhages resulting in uncontrollable bleeding. Technically, the first stage of the procedure consists of a total hepatectomy with preservation of the recipient's inferior vena cava (IVC), followed by the creation of a temporary end-to-side porto-caval shunt (TPCS). The second stage consists of removing the TPCS and implanting a liver graft when one becomes available. We report a case of a two-stage total hepatectomy and LT in which a temporary end-to-end anastomosis between the portal vein and the middle hepatic vein (TPMHV) was performed as an alternative to the classic end-to-end TPCS. The creation of a TPMHV proved technically feasible and showed some advantages compared to the standard TPCS. In cases in which a two-stage LT with side-to-side caval reconstruction is utilized, TPMHV can be considered as a safe and effective alternative to standard TPCS.
