Objective Pneumoperitoneum can cause disruption in splanchnic perfusion. This study aims to investigate effects of pneumoperitoneum on splanchnic oxygenation during abdominal laparoscopic surgery in paediatric patients with NIRS (Near-Infrared Spectroscopy). Material and methods A total of 45 patients between 1 and 4 years of age with ASA physical status I–II and scheduled to undergo abdominal laparoscopic surgery under general anaesthesia were enrolled in this prospective, observational surgery. No premedication was used. Standard monitoring and regional splanchnic saturation (rSPcO2), regional cerebral oxygen saturation (rScO2) with NIRS were established before anaesthesia. Anaesthesia was induced with an inhalational agent and maintained with an oxygen/air mixture and sevoflurane. Peripheral oxygen saturation (SPO2), rSPcO2, rScO2, heart rate (HR), mean arterial pressure (MAP), end tidal CO2 (Et-CO2), and insufflation pressure (IP) were continuously monitored during administration of anaesthesia and recorded. After intubation (baseline T0); before CO2 insufflation induced pneumoperitoneum (PP) (T1); CO2 PP insufflation (T2); 5 minutes after PP insufflation (T3); 10 minutes after PP insufflation (T4); 15 minutes after PP insufflation (T5); 20 minutes after PP insufflation (T6); 30 minutes after PP insufflation (T7), 60 minutes after PP insufflation (T8), and after desufflation (T9). Bradycardia and hypotension were recorded. Paracetamol IV 10 mg kg−1 was applied for post-operative analgesia. p

Ovarian cysts are one of the most common pathologies in the infancy period. Approximately 15% of intraabdominal masses seen in this period are genital origined and one-third of these masses are ovarian cysts. The incidence of ovarian cysts has been reported as 1 in 2500 live births. The most common complications in ovarian cysts are torsion and bleeding. Fetal ovarian cysts usually regress spontaneously in complicated cysts; laparoscopic or open surgery can be applied. A female infant with a diagnosis of intraabdominal cystic mass, suspicion of duodenal atresia, and mesenteric cyst made based on a prenatal ultrasonographic imaging was hospitalized in the neonatal intensive care unit for examination and treatment. The abdominal ultrasonography revealed a cystic lesion in the lower left quadrant. The patient was evaluated with abdominal CT for detailed evaluation 1 day later and revealed a cystic lesion in the right upper quadrant. The displacement of the cystic mass conducted the differential diagnosis in favor of mesenteric cyst and ovarian torsion. Laparoscopic surgery was performed for examination and treatment. It was observed that the left ovary was torsioned and displaced. Ovarian cysts are the most frequently observed masses among prenatal cystic masses in female fetuses. It should be kept in mind that cystic masses detected in the prenatal period may often be ovarian cysts and that these cysts can be displaced in the abdomen while torsion, and a treatment plan should be determined according to the clinical findings of the patient. 9. Retrospective analysis of patients who develop cancer after renal transplantation: A 12-year experience in a single center

Objective: Kidney transplantation is the best treatment option for patients with end-stage renal disease. However, renal transplant recipients have been shown to have a higher risk of cancer than the general population. This has been attributed to malignancies associated with immunosuppression in these patients. In our study, we aimed to investigate the incidence of post-transplant malignancy, duration of development and risk factors in kidney transplant recipients.Method: 1029 patients who underwent kidney transplantation in our organ transplant clinic between January 2006 and December 2018 were analyzed retrospectively through their demographic data, immunosuppressive protocols, post-transplant cancer types, and cancer onset files.Results: Cancer developed in a total of 21 recipients (2%). The donor of 89.5% of the cases was alive. There was a history of smoking in 64.3% of the cases, and a family history of cancer in 45.5%. The most common type of cancer was skin cancer (n: 4). Basal cell Ca in 2 cases with skin cancer, Kaposi sarcoma in 1 case, and Merkel Cell Ca in 1 case (Table 2). Two of our patients with lung cancer had a history of smoking. The family history of our 2 cases with gastric cancer was positive. However, no statistically significant difference was found between family history and smoking and cancer incidence (p=0.576)Conclusions: As transplant recipients continue to live longer with improved outcomes, cancer will increase as a cause of morbidity and mortality in this population. More research is needed to understand whether tumors arising in this population are affected by the patient's immunosuppression.

Bu çalışmada, Mezitler ve çevresinde vadi içi ve ormanlık alanlarda belirlenen istasyonlarda ağaç, çalı, kaya ve toprak gibi substratlardan alınan liken örneklerinin çeşitli tayin anahtarları yardımı ile tür tanımlamaları yapılarak bölgenin liken çeşitliliğinin belirlenmesi amaçlanmıştır. 07.08.2018-01.08.2019 tarihleri arasında 39 lokaliteden 414 liken örneği toplanmış ve teşhis edilen örneklerin Ascomycota‟ya ait 43 cins altında 76 liken taksonu olduğu belirlenmiştir. Tespit edilen taksonlardan 4‟ünün Türkiye için yeni kayıt, 6‟sının ise Bursa için yeni kayıt olduğu saptanmıştır. Türkiye için yeni kayıt olan türler; Arthonia arthonioides (Ach.) A.L. Sm. 1911, Lecanora helicopsis (Walenb.) Ach. 1814, Pseudosagedia borreri (Trevis.) Hafellner ve Kalb 1995, Verrucaria cernaensis Zschacke 1927. Bursa için yeni kayıt olan türler; Farnoldia jurana (Schaer.) Hertel 1983, Lecidea helvola (Körb. ex Hellb.) Th. Fr. 1874, Lecidella euphorea (Flörke) Kremp. 1861, Lepraria caesioalba (B. de Lesd.) J.R. Laundon 1992, Verrucaria elaeomelaena (A. Massal.) Arnold 1868, Verrucaria viridula (Schrad.) Ach. 1803. In this study, it was aimed to determine the lichen diversity of the region by making species identifications with the help of various determination keys of lichen samples taken from substrates such as trees, shrubs, rocks and soil at stations determined in the valley and forest areas in and around Mezitler. Between 07.08.2018 and 01.08.2019, 414 lichen samples were collected from 39 localities and it was determined that the identified samples were 76 lichen taxa under 43 genera belonging to Ascomycota. It was determined that 4 of the identified taxa were new records for Turkey and 6 were new records for Bursa. Species that are new records for Turkey; Arthonia arthonioides (Ach.) A.L. Sm. 1911, Lecanora helicopsis (Walenb.) Ach. 1814, Pseudosagedia borreri (Trevis.) Hafellner & Kalb 1995, Verrucaria cernaensis Zschacke 1927. OBJECTIVE To investigate the clinical outcomes of patients who underwent cardioversion compared with those who did not have cardioverson in a large dataset of patients with recent onset non-valvular atrial fibrillation. DESIGN Observational study using prospectively collected registry data (Global Anticoagulant Registry in the FIELD-AF-GARFIELD-AF). SETTING 1317 participating sites in 35 countries. PARTICIPANTS 52 057 patients aged 18 years and older with newly diagnosed atrial fibrillation (up to six weeks' duration) and at least one investigator determined stroke risk factor. MAIN OUTCOME MEASURES Comparisons were made between patients who received cardioversion and those who had no cardioversion at baseline, and between patients who received direct current cardioversion and those who had pharmacological cardioversion. Overlap propensity weighting with Cox proportional hazards models was used to evaluate the effect of cardioversion on clinical endpoints (all cause mortality, non-haemorrhagic stroke or systemic embolism, and major bleeding), adjusting for baseline risk and patient selection. RESULTS 44 201 patients were included in the analysis comparing cardioversion and no cardioversion, and of these, 6595 (14.9%) underwent cardioversion at baseline. The propensity score weighted hazard ratio for all cause mortality in the cardioversion group was 0.74 (95% confidence interval 0.63 to 0.86) from baseline to one year follow-up and 0.77 (0.64 to 0.93) from one year to two year follow-up. Of the 6595 patients who had cardioversion at baseline, 299 had a follow-up cardioversion more than 48 days after enrolment. 7175 patients were assessed in the analysis comparing type of cardioversion: 2427 (33.8%) received pharmacological cardioversion and 4748 (66.2%) had direct current cardioversion. During one year follow-up, event rates (per 100 patient years) for all cause mortality in patients who received direct current and pharmacological cardioversion were 1.36 (1.13 to 1.64) and 1.70 (1.35 to 2.14), respectively. CONCLUSION In this large dataset of patients with recent onset non-valvular atrial fibrillation, a small proportion were treated with cardioversion. Direct current cardioversion was performed twice as often as pharmacological cardioversion, and there appeared to be no major difference in outcome events for these two cardioversion modalities. For the overall cardioversion group, after adjustments for confounders, a significantly lower risk of mortality was found in patients who received early cardioversion compared with those who did not receive early cardioversion. 13. The Cause of an Unusual Abdominal Pain in Children: Splenic Torsiyon - Three Case Report.

Splenic torsion is a rare cause of abdominal pain that may develop due to laxity or absence of the ligaments that stabilize the spleen. A torsioned spleen may present with an acute abdomen clinically and may require urgent surgical intervention. We aimed to discuss three pediatric cases who applied to our clinic with acute abdomen symptoms after splenic torsion and their treatment approaches. Case 1: A 10-year-old female patient presented with complaints of progressive abdominal pain and non-bilious vomiting. On examination, there was abdominal tenderness and palpable fullness in the left lower quadrant. Imaging methods were compatible with splenic torsion. Laparoscopic splenectomy was performed. Case 2: A 4-year-old girl presented with complaints of abdominal pain and non-bilious vomiting. On examination, diffuse tenderness in the abdomen and defense-rebound were positive. Imaging methods were compatible with splenic torsion. On exploration, it was observed that the spleen was torsioned in different directions around the double pedicle. Splenectomy was performed. Case 3: A 5-year-old male patient was operated in another hospital with the complaint of abdominal pain, with the diagnosis of acute appendicitis, with open surgery. However, there was no improvement in the patient's symptoms after surgery. The patient was consulted to our clinic on the 5th post-operative day. Imaging methods were found to be compatible with splenic torsion. Laparoscopic splenectomy was performed. In the pediatric population, splenic torsion can cause acute or chronic abdominal pain of unknown cause. Splenopexy should be the first goal of treatment in torsion, but splenectomy is the only treatment option in complicated cases and laparoscopy can be used safely even in complicated cases. Aims:\ud The safety and efficacy of the novel selective cardiac myosin activator, omecamtiv mecarbil, in patients with heart failure with reduced ejection fraction (HFrEF) is tested in the Global Approach to Lowering Adverse Cardiac outcomes Through Improving Contractility in Heart Failure (GALACTIC‐HF) trial. Here we describe the baseline characteristics of participants in GALACTIC‐HF and how these compare with other contemporary trials.\ud \ud Methods and Results:\ud Adults with established HFrEF, New York Heart Association functional class (NYHA) ≥ II, EF ≤35%, elevated natriuretic peptides and either current hospitalization for HF or history of hospitalization/ emergency department visit for HF within a year were randomized to either placebo or omecamtiv mecarbil (pharmacokinetic‐guided dosing: 25, 37.5 or 50 mg bid). 8256 patients [male (79%), non‐white (22%), mean age 65 years] were enrolled with a mean EF 27%, ischemic etiology in 54%, NYHA II 53% and III/IV 47%, and median NT‐proBNP 1971 pg/mL. HF therapies at baseline were among the most effectively employed in contemporary HF trials. 15. Can Percutaneous Internal Ring Suturing Procedure Take the Place of Open Method in the Surgical Treatment of Girls with Inguinal Hernia?

Objectives: Percutaneous internal ring suturing (PIRS) method for inguinal hernia (IH) repair in girls is increasingly used in clinics especially experienced in minimal invasive surgery. We aimed to evaluate and compare our results of laparoscopic PIRS and the open procedure for IH repair in girls in our series. Methods: We retrospectively evaluated female patients in our pediatric surgery clinic who underwent IH surgery between 2012 and 2017 and results were assessed statistically. Results: We identified 293 girls operated in our clinic with IH. In 164 of them, PIRS procedure (group 1) was performed; 73 had right, 26 had left, and 65 (39.6%) had bilateral IH. Among these 65 patients, 16 had only right and 11 had only left IH according to preoperative examination, which turned out to be bilateral during laparoscopy. In the open surgery group (Group II), there were 129 patients. Seventy-eight patients had right, 38 had left, and 13 (10%) had bilateral IH. Sliding fallo-pian tubes were present in seven of Group I versus 21 of group II (p<0.001), while sliding ovaries were present in seven of Group I versus 16 of Group II (p=0.015). Recurrence was observed in only one patient in Group I (p>0.05). Conclusion: The higher rate of bilaterality is still the problem for laparoscopic procedures. Despite over-diagnosis, no more complications were not detected due to PIRS. High ratio of sliding tubes and ovaries can be due to traction for preparing the sac in open surgery. Hematoma can be a serious problem to complete PIRS procedure. Preferred technique from the surgeon's point of view is in favor of PIRS procedure. In addition, PIRS pro-cedure can be more protective for internal genitalia, but more studies with higher numbers and longer follow-up period are needed. [ABSTRACT FROM AUTHOR]

Behçet Hastalığı (BH), kronik, alevlenme ve iyilik dönemleri ile seyreden, arteryel ve venöz tipte, değişik çap ve yerleşimdeki tüm damarları tutabilen sistemik bir vaskülit olarak tanımlanmaktadır. BH'nın etyolojisi tam olarak aydınlatılamamış olsa da genetik, immünolojik, çevresel faktörler, geçirilmiş enfeksiyonlar, damar endotel patolojileri vepıhtılaşma bozuklukları sorumlu tutulmaktadır. Günümüzde hala BH'nın kesin bir tedavisi yoktur. Ayrıca progresyonu ve terapötik yanıtı tam olarak tahmin edebilen spesifik biyobelirteçler mevcut değildir. BH'nin patogenezinde rol alan hücresel ve moleküler mekanizmaların daha iyi anlaşılması hastalığın tedavisinde yeni stratejilerin geliştirilmesine olanak sağlayacaktır. Son yıllarda, inflamasyonda ve immün sistem hücrelerinin fonksiyonlarının düzenlenmesinde Follistatin-like protein 1 (FSTL-1)'in önemli bir rolü olduğunu ortaya koyan birçok çalışma yapılmıştır. FSTL-1'in immün sistemdeki rolü tam olarak anlaşılamamış olsa da FSTL-1 için hem pro-inflamatuar hem de anti-inflamatuar fonksiyonlar bildirilmiştir. FSTL-1'in insan inflamatuar hastalıklarındaki olası rolü, ilk olarakRomatoid Artrit'li(RA) hastaların sinovyal dokusundan izole edilerek gösterilmiştir. Ayrıca RA, ülseratif kolit, sistemik lupus eritematozus, sjogren sendromu, sistemik skleroz ve polimiyozit/dermatomiyozit gibi çeşitli otoimmün hastalıklarda serum FSTL-1 seviyeleri önemli ölçüde yüksek bulunmuştur.Makrofajlarda ve fibroblastlarda FSTL-1'in aşırı ekspresyonu, IL-1beta, TNF-alfa ve IL-6 dahil olmak üzere pro-inflamatuar sitokinlerin aktivitesini arttırmış ve hayvan deney modellerindenormal farelerde ciddi artrite neden olmuştur. FSTL-1 nötralizasyonunun, kollajen doku hastalığı ilişkili artrit (CİA) fare modellerinin artritik eklemlerinde IFN-gamave kemokin ligand 10 üretimini engelleyerek artriti iyileştirdiği gösterilmiştir.Bu çalışmamızda; birçok inflamatuar hastalığın patogenezinde rolü olduğu çeşitli çalışmalarla orta konmuş, mezenkimal kökenli bir glikoprotein olan FSTL-1'in, inflamatuar bir vaskülit olan BH'nda serum düzeylerini saptamak ve hastalık aktivasyonu ile olan ilişkisini araştırmayı amaçladık.Çalışmamıza Dicle Üniversitesi Tıp Fakültesi Fiziksel Tıp ve Rehabilitasyon Anabilim Dalı polikliniklerine, Haziran-Eylül 2019 tarihleri arasında başvuran 1990 Uluslararası Behçet Hastalığı Çalışma Grubu Kriterlerini karşılayan yeni tanı ve takipli 45 hasta dahil edildi. Item does not contain fulltext BACKGROUND: Many patients with atrial fibrillation have concomitant coronary artery disease with or without acute coronary syndromes and are in need of additional antithrombotic therapy. There are few data on the long-term clinical outcome of atrial fibrillation patients with a history of acute coronary syndrome. This is a 2-year study of atrial fibrillation patients with or without a history of acute coronary syndromes. METHODS: Adults with newly diagnosed atrial fibrillation and >/=1 investigator-defined stroke risk factor were enrolled in GARFIELD-AF between March 2010 and September 2015. The association between prior acute coronary syndromes and long-term outcomes was determined using a Cox proportional hazards model, adjusting for baseline risk factors, oral anticoagulation (OAC) +/- antiplatelet (AP) therapy, and usual care. RESULTS: Of 39,679 patients, 10.5% had a history of acute coronary syndromes. At 2-year follow-up, patients with prior acute coronary syndromes had higher adjusted risks of stroke/systemic embolism (hazard ratio [HR] 1.39; 95% confidence interval [CI], 1.08-1.78), major bleeding (HR 1.30; 95% CI, 0.95 -1.79), all-cause mortality (HR 1.34; 95% CI, 1.21 -1.49), cardiovascular mortality (HR 1.85; 95% CI, 1.51-2.26), and new acute coronary syndromes (HR 3.42; 95% CI, 2.62-4.45). Comparing antithrombotic therapy in the acute coronary syndromes vs no acute coronary syndromes groups, most patients received OAC +/- AP: 60.8% vs 66.1%, but AP therapy was more likely in the acute coronary syndromes group (68.1% vs 32.9%), either alone (34.9% vs 20.8%) or with OAC (33.2% vs 12.1%). Overall, 17.8% in the acute coronary syndromes group received dual AP therapy with (5.3%) or without OAC (12.5%). Among patients with moderate/high risk for stroke/systemic embolism, fewer in the acute coronary syndromes group received OAC with or without AP therapy (Congestive heart failure, Hypertension, Age 75 years, Diabetes mellitus, prior Stroke, TIA, or thromboembolism, Vascular disease, Age 65-74 years, Sex category [CHA2DS2-VASc] 2: 52.1% vs 64.6%; CHA2DS2-VASc >/=3: 62.0% vs 70.7%), and the majority with a Hypertension (uncontrolled systolic blood pressure >160 mm Hg), Abnormal renal or liver function, previous Stroke, Bleeding history or predisposition, Labile international normalized ratios, Elderly, and concomitant Drugs or alcohol excess (HAS-BLED) score >/=3 were on AP therapy (83.8% vs 65.5%). CONCLUSIONS: In GARFIELD-AF, previous acute coronary syndromes are associated with worse 2-year outcomes and a greater likelihood of under-treatment with OAC, while two-thirds of patients receive AP therapy. Major bleeding was more common with previous acute coronary syndromes, even after adjusting for all risk factors.

Bu araştırmada lise son sınıf öğrencilerinin üstbiliş becerileri ve akademik erteleme davranışları arasındaki ilişkiler öğrencilere ait bazı değişkenler açısından incelenmiştir. Çalışma ilişkisel tarama modeline uygun olarak tasarlanmıştır. Araştırmanın evreni 2017-2018 eğitim öğretim yılı içerisinde İç Anadolu Bölgesindeki bir il merkezinde bulunan ortaöğretim kurumlarının (liselerin) son sınıf öğrencilerinden oluşmaktadır. Araştırma örneklemi kolayda örnekleme yöntemi yardımı ile seçilmiş ve araştırmaya gönüllü olarak katılmış 492 lise son sınıf öğrencisinden oluşmaktadır. Araştırma verileri araştırmacı tarafından hazırlanan bir kişisel bilgi formu, `Üstbiliş Becerileri Ölçeği` ve `Akademik Erteleme Ölçeği` ile toplanmıştır. Veriler SPSS 22.00 paket programı kullanılarak çözümlenmiştir. Çözümlemede frekans ve yüzde, madde ortalamaları, Kolmogrov-Smirnov Testi, Kruskal Wallis-H testi, Mann-Whitney U Testi ve Spearman Brown Sıra Farkları Korelasyon Analizi tekniklerinden yararlanılmıştır. Araştırma sonuçlarına göre lise son sınıf öğrencilerinin üstbiliş becerilerini kullanma düzeyleri ile akademik erteleme davranışı gösterme düzeylerinin genel olarak orta düzeyde olduğu tespit edilmiştir. Üstbiliş beceri puanları öğrencilerin cinsiyet ve anne-baba eğitim düzeyi değişkenine göre değişmezken, ailenin sosyo-ekonomik düzeyi ve devam edilen lise türüne göre anlamlı farklılıklar arz etmektedir. Akademik erteleme puanları ailenin sosyo-ekonomik düzeyi, annenin eğitim düzeyi ve babanın eğitim düzeyi değişkenine göre değişmezken; öğrencilerin cinsiyet ve devam ettikleri lise türü değişkenine göre anlamlı farklılıklar arz etmektedir. Öğrencilerin üstbiliş puanları ile akademik erteleme davranışı puanları ortalamaları arasında .18 düzeyinde pozitif yönlü düşük bir ilişki olduğu tespit edilmiştir. Araştırma sonuçları ilgili literatür temelinde tartışılmış ve bulgulara dayalı bazı öneriler sunulmuştur.Araştırma sonuçlarına göre lise son sınıf öğrencilerinin üstbiliş becerilerini kullanma düzeyleri ile akademik erteleme davranışı gösterme düzeylerinin genel olarak orta düzeyde olduğu tespit edilmiştir.Anahtar Kelimeler: Akademik erteleme, lise son sınıf, üstbiliş In this survey,it was anayzed the relation between metacognition abilities and academic procrastianation behaviors of senior high school students in terms of several other factors.This survey has been designed as a correlational survey.The sampling of the survey concists of the senior high school students in a city of Central Anatolia Region.The sampling of the survey contains 492 volunteer students chosen with the help of convenience sampling method.The data of the survey was gotten via academic procrastianation and metacognition ability scale prepared by surveyor.The data were analyzed using SPSS 22.00 version software.During the analysis,frequency and percentage tables,means of item scores,Kolmogorov-Smirnov test,Mann-Whitney U test,Kruskal-Wallis H test and Sperman Correlational Analysis were used. According to results of the survey,there is an intermidiate relation between metacognition abilities and academic behaviors of senior high school students. The points of metacognition ability weren't affected from gender and educational level of the students' parents;however were affected from socioeconomic level of the parents and type of the high school. The points of academic procrastianation weren't affected from socioeconomic level of the parents,educational level of mother and father;however were affected from gender of the students and the type of high school. At the end of the survey,it was detected .18 level positively but a poor relation between the points of metacognition ability and academic procrastianation. Objective: Cardiac resynchronization therapy (CRT) has been shown to reduce mortality in selected patients with heart failure with reduced ejection fraction (HFrEF). CRT Survey-II was a snapshot survey to assess current clinical practice with regard to CRT. Herein, we aimed to compare Turkish data with other countries of European Society of Cardiology (ESC). Methods: The survey was conducted between October 2015 and December 2016 in 42 ESC member countries. All consecutive patients who underwent a de novo CRT implantation or a CRT upgrade were eligible. Results: A total of 288 centers included 11,088 patients. From Turkey, 16 centers recruited 424 patients representing 12.9% of all implantations. Compared to the entire cohort, Turkish patients were younger with a lower proportion of men and a higher proportion with ischemic etiology. Electrocardiography (ECG) showed sinus rhythm in 81.5%, a QRS duration of <130 ms in 10.1%, and ≥150 ms in 63.8% of patients. Left bundle branch block (LBBB) was more common. Median left ventricular ejection fraction (LVEF) was 25%, lower than in the overall ESC cohort, but NYHA class was more often II. Most common indication for CRT implantation was HF with a wide QRS (70.8%). Almost 98.3% of devices implanted were CRT-D, in contrast to the overall cohort. Fluoroscopy time was longer, but duration of overall procedure was shorter. LV lead implantation was unsuccessful in 2.6% patients. Periprocedural complication rate was 6.3%. The most common complication was bleeding. Remote monitoring was less utilized. Conclusion: These are the first observational data reflecting the current CRT practice in Turkey and comparing it with other countries of Europe. Bilişsel süreci kontrol edebilen, düzenleyebilen ve değerlendirebilen faaliyet, üstbiliş olarak ifade edilmektedir. "Bilmeyi bilme", "bilişe ilişkin biliş" veya "düşünmeyi düşünme" gibi deyimler de üstbiliş için kullanılmaktadır. Öğrenmede veya problem çözmede hangi stratejilerin ne zaman ve nasıl kullanılacağı da bu kavramla ilişkilidir. Eğitim alanındaki gelişmelerle birlikte üstbiliş kavramı da büyük bir önem kazanmıştır. Öğrencilerin aileleriyle olan ilişkileri ve üstbiliş becerileri arasında nasıl bir ilişki bulunduğunun belirlenmesi de merak konusudur. Bu çalışmada, 8. sınıf öğrencilerinin aile ilişkileri ile üstbiliş becerileri arasındaki ilişkiler ele alınmıştır. Araştırma tarama modelinde ilişkisel desene göre tasarlanmıştır. Araştırmanın çalışma grubunu Türkiye'nin Güneydoğu Anadolu Bölgesi'nde yer alan bir il merkezinde kolayda örneklemi yöntemiyle belirlenen üç ortaokuldaki 180 öğrenci oluşturmaktadır. Araştırma verileri, "Aile İlişkileri Ölçeği" ve "Yürütücü Biliş Becerileri Ölçeği" ile toplanmıştır. Analiz sürecinde, veri dağılımına uygun istatistiki teknikler kullanılarak, ikili ve çoklu karşılaştırmalar yapılmıştır. Bulgular öğrencilerdeki engelleyici aile algısı ile üstbiliş puanları arasında .-37,6 düzeyinde anlamlı negatif korelasyon olduğunu buna karşılık, aile desteği algısı ile üstbiliş puanları arasında .24,4 anlamlı pozitif korelasyon bulunduğunu göstermiştir. Araştırma sonuçları ilgili literatür temelinde tartışılmış ve bulgulara dayalı bazı öneriler sunulmuştur Activity that can control, regulate and evaluate the cognitive process is expressed as metacognition. Statements such as "knowing not knowing", "cognition about cognition" or "thinking about thinking" are also used for metacognition. Which strategies and when and how we use them to learn or solve problems are also related to this concept. Parallel to the developments in the field of education, metacognition has gained a great importance. It is also a matter of curiosity to determine the relationships between students' family relationships and metacognitive levels. In this study, it was tried to determine the relationships between family relations and metacognitive skills of 8th grade students. The research is designed according to relational survey model. Working group of the study sample as determined by the method easy in a city center located in the Southeastern Anatolia Region of Turkey is comprised of 180 students in three secondary schools. The data were collected by the "Family Relations Scale" and "Executive Scale of Cognition Skills". In the analysis process, binary and multiple comparisons were made using statistical techniques appropriate for data distribution. There was a significant negative correlation between obstructive family perception and metacognitive scores in bullying students, -37.6, whereas there was a significant positive correlation between family support perception and metacognitive scores .24,4. The results of the research are discussed on the basis of the related literature and some suggestions based on the findings are presented

Objective: Periesophageal vagal plexus injury is a complication of cryoablation for atrial fibrillation (AF). The aim of this study is to investigate the effect of cryoablation on esophageal functions and to determine the relationship between esophageal temperature and esophageal motility. Methods: Twenty patients with symptomatic paroxysmal AF who underwent cryoablation were included in this study. The lowest cryoballoon temperature for each pulmonary vein (PV) was recorded. Esophageal temperature was measured using an esophageal probe during each cryoapplication. Esophageal manometry was performed before the procedure and one day after the procedure for each patient in order to assess the esophageal functions. Results: During the procedure, the highest esophageal temperature change was found in the left-side PVs in 13 patients (65%) and in the rightside PVs in seven patients (35%). No correlation was found between the lowest cryoballoon temperature and esophageal temperature change (r=0.22, p=0.05). It was detected that the lower esophageal sphincter pressure and esophageal contraction amplitude pressure decreased after the procedure (before: 19.7±9.3 mm Hg, after: 14.3±4.9 mm Hg, p=0.001; before: 84.5±28.3 mm Hg, after: 72.7±34.3 mm Hg, p=0.005, respectively). Five patients (25%) developed gastrointestinal symptoms after the procedure. Conclusion: During cryoablation, esophageal temperature measurement can be performed to reduce the probability of esophageal injury. Cryoablation affects esophageal motility, and esophageal manometry can be performed to detect esophageal motility impairments in patients with gastrointestinal symptoms. [ABSTRACT FROM AUTHOR]

Bu çalışmada, Türkiye’de 81 il arasında gelişmişlik sıralamasında son sıralarda yer alan Ardahan ilinin sosyo-ekonomik yapısı, nüfus, eğitim, sağlık, istihdam ve dış ticaret göstergeleri yardımıyla 2009-2013 dönemi incelenmiştir. Çalışmanın son kısmında ise ilin güçlü, zayıf yönleriyle, ilin gelişmesine yönelik fırsatlar belirtilerek mevcut sorunlar için çözüm önerilerinde bulunulmuştur., The aim of this study is to examine socio-economic structure, population, education, health, employment and foreign trade indicators of Ardahan which is one of the most less developed city in Turkey. For this purpose, the period of 2009-2013 is investigated. In the last section, weakness and strengths of Ardahan is discussed and some policy recommendations indicating opportunities for the development of the city are suggested.

Reel efektif döviz kuru ile dış ticaret hacmi arasındaki ilişki son yıllarda tartışı- lan önemli konulardan biridir. Bu çalışmada Türkiye'nin dış ticaret hacmi ile reel efektif döviz kuru arasındaki nedensellik ilişkisi araştırılmaya çalışılmış, 1997:1- 2014:12 dönemi için yapılan ampirik çalışmada sırasıyla; logaritması alınan se- riler mevsimsel etkilerden arındırıldıktan sonra ADF, Phillips-Perron ve KPSS bi- rim kök testleri uygulanarak, seriler durağan hale getirilmiştir. Granger neden- sellik analizi yapıldıktan sonra Koentegrasyon (eşbütünleşme) analizi uygulana- rak, dış ticaret hacmi ve reel efektif döviz kuru arasındaki nedensellik ilişki in- celenmiştir. Aynı zamanda reel efektif döviz kuru ile dış ticaret hacmi arasında eşbütünleşme ilişkisinin varlığı ortaya konulmuştur. Daha sonra uzun dönem eş- bütünleşme ve kısa dönem hata düzeltme modeli tahmin edilmiştir. Yapılan ana- lizler sonucunda elde edilen sonuçlara göre reel efektif döviz kurundan dış tica- ret hacmine doğru bir nedensellik ilişkisi olduğu, ancak dış ticaret hacminden reel efektif döviz kuruna doğru bir nedensellik ilişkisinin bulunmadığı tespit edilmiştir., The relationship between foreign trade and real exchange rate has been discus- sed in recent years. The purpose of this study is to assess the casual relationship between real exchange rate and foreign trade volume. In our empirical study per- formed using monthly data from January 1997 to December 2014, first, the loga- rithms of the series are clarified deseasonalised and then they have been tested with the ADF, Phillips-Perron and KPSS unit root tests. Granger casuality analy- sis has been made and using the cointegration analysis, the relationship betwe- en foreign trade volume and real exchange rate has been examined. Then the long-run and short-run error correction model was estimated . According to the result of analyses, a casual relationship has been found from real exchange to foreign trade volume however it has been determined that there is no casual re- lationship from foreign trade volume to real exchange rate.

Ruj numunelerinde (n=25) sürekli akış hidrür oluşturmalı atomik absorpsiyon spektrometri (SA-HO-AAS) kullanarak kurşun tayin edilmiştir. Hidrür oluşumu sırasında duyarlığı etkileyen asit derişimi, oksitleyici ve indirgeyici reaktif derişimleri, taşıyıcı azot gaz akış hızı, çözelti akış hızı optimize edilmiştir. Numuneler teflon kaplarda HNO3: HF (7:2) (h:h) ortamında 100 °C'de yıkılanarak hazırlanmıştır. 283,3 nm'de sulu ortamda kurşun standart çözeltisi için doğrusal kalibrasyon aralığı 2,5-100 μg/L'dir (R2= 0,9987). Doğrusal regresyon ile elde edilen kalibrasyon doğrusunun denklemi A= 0,0059 C + 0,0049 olarak bulunmuştur (A: absorbans, C: μg/L). SA-HO-AAS ile ruj numunelerinde kurşun derişimleri %95 güven sınırları içerisinde 1,68 ± 0,72 mg/kg aralığında bulunmuştur. Teşhis sınırı (3 x Sb/m) ve karakteristik derişim sırasıyla 0,51 μg/L ve 0,74 μg/L olarak bulunmuştur. Tayin Alt Sınırı (TAS veya LOQ) ise (10 x Sb /m) 1,70 μg/L olarak hesaplanmıştır. Yöntemin çalışılan koşullar altında tekrarlanabilirliği 20 μg/L Pb derişiminde ardarda 11 ölçümde %BSS (bağıl standart sapma) cinsinden 4,2 olarak bulunmuştur. Geri kazanım sonuçları %95 güven sınırları içinde 98,14 ± 1,88 olarak bulunmuştur. Lead was determined in lipstick samples using continuous flow hydride generation atomic absorption spectrometry (CF-HG-AAS). The parameters such as the acidity, the concentrations of oxidizing and reducing agent, the flow rate of carrier nitrogen gas and the solutions were optimized. The samples were digested with HNO3: HF (7:2) (h:h) at 100 °C in teflon beaker. Linear calibration range for the lead solution in aqueous media at 283,3 nm is 2,5-100 µg/L (R2= 0,9987). The equation of calibration curve which was obtained by using linear regression was found as A= 0,0059 C + 0,0049 (A: absorbance, C: concentration, µg/L). The concentration of lead in samples were found in the range of 1,68 ± 0,72 mg/kg within the confidence limits of 95%. The detection limit (3 x Sb/m) and characteristic sensitivity were found as 0,51 µg/L and 0,74 µg/L, respectively. Also, limit of quantitation (LOQ) was calculated as 1,70 µg/L (10 x Sb /m). of the measurements (n= 11) at 20 µg/L Pb level was 4,2 % RSD (relative standard deviation). The recovery was found as 98,14 ± 1,88 within the confidence limit of 95%. 64

Objective: The corner stone of atrial fibrillation therapy includes the prevention of stroke with less adverse effects. The Global Anticoagulant Registry in the FIELD-Atrial Fibrillation (GARFIELD-AF) study provided data to compare treatment strategies in Turkey with other populations and every-day practice of stroke prevention management with complications. Methods: GARFIELD-AF is a large-scale registry that enrolled 52,014 patients in five sequential cohorts at >1.000 centers in 35 countries. This study was initiated to track the evolution of global anticoagulation practice, and to study the impact of NOAC therapy in AF. A total of 756 patients from 17 enrolling sites in Turkey were in cohort 4 and 5. Treatment strategies at diagnosis initiated by CHA2DS2-VASc score, baseline characteristics of patients, treatment according to stroke and bleeding risk profiles, and INR values were analyzed in cohorts. Additionally, event rates during the first year follow up were evaluated. Results: AF patients in Turkey were mostly seen in young women. Stroke risk according to the CHADS2 score and CHA2DS2-VASc score compared with world data. The mean of risk score values, including HAS-BLED score were lower in Turkey than in the world data. The percentage of patients receiving FXa inhibitor with or without an antiplatelet usage was more than the other drug groups. All-cause mortality was higher in Turkey. Different form world data when HAS-BLED score was above 3, the therapy was mostly changed to antiplatelet drugs in Turkey. Conclusion: In addition to deficiencies in available treatment options, patient care and clinical outcomes of patients with AF, the data of GARFIELD- AF provide data from Turkey about therapeutic strategies and best practices. Son yıllarda dünya ekonomisinde küreselleşmenin etkisiyle ülkeler ve firmalar birbirine hızla entegre olmaya başlamıştır. Bu durum, yatırımcıların daha fazla kar elde etmek amacıyla sermayelerini bu amaca daha kolay ulaşabilecekleri piyasalara taşıma isteğini de beraberinde getirmiştir. Sermaye hareketliliğinde firma için önemli olan gireceği ülkenin risklerine hâkim olması, sermayeyi kabul eden ülke için önemli olan ise firmanın güvenilirliğidir. Bahsedilen bu problemin ortadan kalkması için bütün tarafların birbirlerinin mali durumlarına hâkim olması gerekmektedir. Bu gereklilik taraflar açısından maliyetli olduğundan devreye kredi derecelendirme kuruluşları girmektedir. Her ne kadar kredi derecelendirme kuruluşları verdikleri notların bir tavsiye değil görüş belirttiğini bildirseler de 1900'lü yıllardan itibaren ekonomik hayatın içerisinde olan bu kuruluşların verdikleri notlar dikkatle takip edilmektedir.Bu bağlamda bu çalışma, kredi derecelendirme kavramı ile ilgili genel bilgileri ve kredi notunun etki alanlarını açıkladıktan sonra, derecelendirme kuruluşlarının not verme kriterlerini Türkiye için incelemiş, söz konusu kriterlerde meydana gelen değişmenin kredi notu ile paralel olup olmadığını gözlemlemeye çalışmıştır.ANAHTAR KELĠMELER: Kredi Derecelendirme Kuruluşları, Kredi Notu, Yatırım, Türkiye During recent years with the effect of globalization in the world economy countries and firms started to integrated each other quickly. This situation brought about the desire of carrying capitals of investors to derive profit more to the markets where they could achieve this purpose. The important thing for the firm in capital movements is having full knowledge of countries risks and the important thing for the country that accepts capital is thrust worthiness of firms. The problem mentioned to be removed, all sides required to know each other's economic situation. When this necessity gets costly for sides, credit rating agencies becomes a part of activity. Credit rating agencies emphasize that their notes mean their opinions and theese notes are not recommendations for countries. Objective: The present study was designed to evaluate the characteristics of pulmonary hypertension (PH) and adult cardiology practice patterns for PH in our country. Methods: We evaluated preliminary survey data of 1501 patients with PH (females, 69%; age, 44.8±5.45) from 20 adult cardiology centers (AdCCs). Results: The average experience of AdCCs in diagnosing and treating patients with PH was 8.5±3.7 years. Pulmonary arterial hypertension (PAH) was the most frequent group (69%) followed by group 4 PH (19%), group 3 PH (8%), and combined pre- and post-capillary PH (4%). PAH associated with congenital heart disease (APAH-CHD) was the most frequent subgroup (47%) of PAH. Most of the patients' functional class (FC) at the time of diagnosis was III. The right heart catheterization (RHC) rate was 11.9±11.6 per month. Most frequently used vasoreactivity agent was intravenous adenosine (60%). All patients under targeted treatments were periodically for FC, six-minute walking test, and echo measures at 3-month intervals. AdCCs repeated RHC in case of clinical worsening (CW). The annual rate of hospitalization was 14.9±19.5. In-hospital use of intravenous iloprost reported from 16 AdCCs in CWs. Bosentan and ambrisentan, as monotreatment or combination treatment (CT), were noted in 845 and 28 patients, respectively, and inhaled iloprost, subcutaneous treprostinil, and intravenous epoprostenol were noted in 283, 30, and four patients, respectively. Bosentan was the first agent used for CT in all AdCCs and iloprost was the second. Routine use of antiaggregant, anticoagulant, and pneumococcal and influenza prophylaxis were restricted in only two AdCCs. Conclusion: Our nationwide data illustrate the current status of PH regarding clinical characteristics and practice patterns. [ABSTRACT FROM AUTHOR]

The significance of AtikŞikayet Defteri numbered 10 increases through the absence of mühimme defters refering to itsdate range.Considering its content, Atik Şikayet Defteri numbered 10 appears to be a mühimme ofcampaign though it is called as " Atik Şikayet Defteri". This research bears the very uniquefeature of being a first hand source in terms of denoting the campaign organizations.
