Abstract

For the Laplacian of an $n$-Riemannian manifold $X$, the Weyl law states that the $k$-th eigenvalue is asymptotically proportional to $(k/V)^{2/n}$, where $V$ is the volume of $X$. We show that this result can be derived via physical considerations by demanding that the gravitational potential for a compactification on $X$ behaves in the expected $(4+n)$-dimensional way at short distances. In simple product compactifications, when particle motion on $X$ is ergodic, for large $k$ the eigenfunctions oscillate around a constant, and the argument is relatively straightforward. The Weyl law thus allows to reconstruct the four-dimensional Planck mass from the asymptotics of the masses of the spin 2 Kaluza--Klein modes. For warped compactifications, a puzzle appears: the Weyl law still depends on the ordinary volume $V$, while the Planck mass famously depends on a weighted volume obtained as an integral of the warping function. We resolve this tension by arguing that in the ergodic case the eigenfunctions oscillate now around a power of the warping function rather than around a constant, a property that we call weighted quantum ergodicity. This has implications for the problem of gravity localization, which we discuss. We show that for spaces with D$p$-brane singularities the spectrum is discrete only for $p =6,7,8$, and for these cases we rigorously prove the Weyl law by applying modern techniques from RCD theory., Comment: 35 pages, 1 figure

Abstract

Many of the most sensitive physics experiments searching for rare events, like neutrinoless double beta ($0\nu\beta\beta$) decay and dark matter interactions, rely on cryogenic macro-calorimeters operating at the mK-scale. Located underground at the Gran Sasso National Laboratory (LNGS), in central Italy, CUORE (Cryogenic Underground Observatory for Rare Events) is one of the leading experiments for the search of $0\nu\beta\beta$ decay, implementing the low-temperature calorimetric technology. We present a novel multi-detector analysis to correlate environmental phenomena with the low-frequency noise of low-temperature calorimeters. Indeed, the correlation of marine and seismic data with data from a pair of CUORE detectors indicates that cryogenic detectors are sensitive not only to intense vibrations generated by earthquakes, but also to the much fainter vibrations induced by marine microseisms in the Mediterranean Sea due to the motion of sea waves. Proving that cryogenic macro-calorimeters are sensitive to such environmental sources of noise opens the possibility of studying their impact on the detectors physics-case sensitivity. Moreover, this study could pave the road for technology developments dedicated to the mitigation of the noise induced by marine microseisms, from which the entire community of cryogenic calorimeters can benefit.

Abstract

We propose an algorithm which builds a concrete dual for large-radius 3d de Sitter with a timelike York boundary for both gravity and bulk effective fields. This generalizes the solvable $T\bar T+\Lambda_2$ deformation, whose finite real spectrum accounts for the refined Gibbons-Hawking entropy as a microstate count while reproducing the radial static patch geometry. The required generalization to produce approximately local boundary conditions for bulk quantum fields requires a scheme for defining double-trace operators dual to deformed boundary conditions to realize the finite timelike boundary, valid at finite N. By starting with a small stint of a pure $T\bar T$ trajectory, the theory becomes finite, enabling well-defined subtractions to define the double-trace deformation so as to match the large-N prescription of Hartman, Kruthoff, Shaghoulian, and Tajdini to good approximation. We incorporate the matter effecting an uplift from negative to positive cosmological constant, and analyze the effect of matter on the energy spectrum of the theory arising from time-dependent bulk excitations. This validates the cosmic horizon $dS_3$ microstate count for large-radius $dS_3$ holography, embedding $T\bar T+\Lambda_2$ concretely into a larger theory consistent with bulk locality for matter fields. Abstract

Magnetic frustration is a route that can lead to the emergence of novel ground states, including spin liquids and spin ices. Such frustration can be introduced through either the geometry of lattice structures or by incompatible exchange interactions. Identifying suitable strategies to control the degree of magnetic frustration in real systems is an active field of research. In this study, we devise a design principle for the tuning of frustrated magnetism on the square lattice through the manipulation of nearest (NN) and next-nearest neighbor (NNN) antiferromagnetic (AF) exchange interactions. By studying the magnon excitations in epitaxially-strained La$_2$NiO$_4$ films using resonant inelastic x-ray scattering (RIXS) we show that, in contrast to the cuprates, the dispersion peaks at the AF zone boundary. This indicates the presence of an AF-NNN spin interaction. Using first principles simulations and an effective spin-model, we demonstrate the AF-NNN coupling to be a consequence of the two-orbital nature of La$_2$NiO$_4$. Our results demonstrate that compressive strain can enhance this coupling, providing a design principle for the tunability of frustrated magnetism on a square lattice.

Abstract

In models with extra dimensions, matter particles can be easily localized to a 'brane world', but gravitational attraction tends to spread out in the extra dimensions unless they are small. Strong warping gradients can help localize gravity closer to the brane. In this note we give a mathematically rigorous proof that the internal wave-function of the massless graviton is constant as an eigenfunction of the weighted Laplacian, and hence is a power of the warping as a bound state in an analogue Schr\"odinger potential. This holds even in presence of singularities induced by thin branes. We also reassess the status of AdS vacuum solutions where the graviton is massive. We prove a bound on scale separation for such models, as an application of our recent results on KK masses. We also use them to estimate the scale at which gravity is localized, without having to compute the spectrum explicitly. For example, we point out that localization can be obtained at least up to the cosmological scale in string/M-theory solutions with infinite-volume Riemann surfaces; and in a known class of N = 4 models, when the number of NS5- and D5-branes is roughly equal., Comment: 43 pages, 2 figures

Abstract

We develop the theory of Energy Conserving Descent (ECD) and introduce ECDSep, a gradient-based optimization algorithm able to tackle convex and non-convex optimization problems. The method is based on the novel ECD framework of optimization as physical evolution of a suitable chaotic energy-conserving dynamical system, enabling analytic control of the distribution of results - dominated at low loss - even for generic high-dimensional problems with no symmetries. Compared to previous realizations of this idea, we exploit the theoretical control to improve both the dynamics and chaos-inducing elements, enhancing performance while simplifying the hyper-parameter tuning of the optimization algorithm targeted to different classes of problems. We empirically compare with popular optimization methods such as SGD, Adam and AdamW on a wide range of machine learning problems, finding competitive or improved performance compared to the best among them on each task. Abstract

We develop Microcanonical Hamiltonian Monte Carlo (MCHMC), a class of models which follow a fixed energy Hamiltonian dynamics, in contrast to Hamiltonian Monte Carlo (HMC), which follows canonical distribution with different energy levels. MCHMC tunes the Hamiltonian function such that the marginal of the uniform distribution on the constant-energy-surface over the momentum variables gives the desired target distribution. We show that MCHMC requires occasional energy conserving billiard-like momentum bounces for ergodicity, analogous to momentum resampling in HMC. We generalize the concept of bounces to a continuous version with partial direction preserving bounces at every step, which gives an energy conserving underdamped Langevin-like dynamics with non-Gaussian noise (MCLMC). MCHMC and MCLMC exhibit favorable scalings with condition number and dimensionality. We develop an efficient hyperparameter tuning scheme that achieves high performance and consistently outperforms NUTS HMC on several standard benchmark problems, in some cases by more than an order of magnitude., Comment: 34 pages, 11 figures

Abstract

We prove an equivalence between the classical equations of motion governing vacuum gravity compactifications (and more general warped-product spacetimes) and a concavity property of entropy under time evolution. This is obtained by linking the theory of optimal transport to the Raychaudhuri equation in the internal space, where the warp factor introduces effective notions of curvature and (negative) internal dimension. When the Reduced Energy Condition is satisfied, concavity can be characterized in terms of the cosmological constant $\Lambda$; as a consequence, the masses of the spin-two Kaluza-Klein fields obey bounds in terms of $\Lambda$ alone. We show that some Cheeger bounds on the KK spectrum hold even without assuming synthetic Ricci lower bounds, in the large class of infinitesimally Hilbertian metric measure spaces, which includes D-brane and O-plane singularities. Abstract

We introduce a novel framework for optimization based on energy-conserving Hamiltonian dynamics in a strongly mixing (chaotic) regime and establish its key properties analytically and numerically. The prototype is a discretization of Born-Infeld dynamics, with a squared relativistic speed limit depending on the objective function. This class of frictionless, energy-conserving optimizers proceeds unobstructed until slowing naturally near the minimal loss, which dominates the phase space volume of the system. Building from studies of chaotic systems such as dynamical billiards, we formulate a specific algorithm with good performance on machine learning and PDE-solving tasks, including generalization. 12. Non-supersymmetric AdS$_6$ and the swampland

Abstract

We discuss infinite families of non-supersymmetric AdS$_6$ solutions in Type IIB string theory. They are siblings of supersymmetric solutions which are associated with $(p,q)$ 5-brane webs and holographically dual to 5d SCFTs engineered by those brane webs. The non-supersymmetric backgrounds carry identical 5-brane charges and are connected to the supersymmetric ones by RG flows. We study the stability of the non-supersymmetric solutions, identifying perturbative and non-perturbative decay channels for all the backgrounds explicitly available. We also identify likely decay mechanisms for solutions that have not been constructed explicitly but may be expected to exist based on brane web considerations. Finally, we exclude scale separation by constructing universal spin 2 modes with masses comparable to the mass-scale of the cosmological constant., Comment: 49 pages, 9 figures

Abstract

We consider gravity compactifications whose internal space consists of small bridges connecting larger manifolds, possibly noncompact. We prove that, under rather general assumptions, this leads to a massive spin-two field with very small mass. The argument involves a recently-noticed relation to Bakry--\'Emery geometry, a version of the so-called Cheeger constant, and the theory of synthetic Ricci lower bounds. The latter technique allows generalizations to non-smooth spaces such as those with D-brane singularities. For AdS$_d$ vacua with a bridge admitting an AdS$_{d+1}$ interpretation, the holographic dual is a CFT$_d$ with two CFT$_{d-1}$ boundaries. The ratio of their degrees of freedom gives the graviton mass, generalizing results obtained by Bachas and Lavdas for $d=4$. We also prove new bounds on the higher eigenvalues. These are in agreement with the spin-two swampland conjecture in the regime where the background is scale-separated; in the opposite regime we provide examples where they are in naive tension with it., Comment: 61 pages, 4 figures

Abstract

Multi-orbital physics in quasi-two-dimensional electron gases (q2DEGs) triggers unique phenomena not observed in bulk materials, such as unconventional superconductivity and magnetism. Here, we investigate the mechanism of orbital selective switching of the spin-polarization in the oxide q2DEG formed at the (001) interface between the LaAlO$_{3}$, EuTiO$_{3}$ and SrTiO$_{3}$ band insulators. By using density functional theory calculations, transport, magnetic and x-ray spectroscopy measurements, we find that the filling of titanium-bands with 3d$_{xz,yz}$ orbital character in the EuTiO3 layer and at the interface with SrTiO$_{3}$ induces an antiferromagnetic to ferromagnetic switching of the exchange interaction between Eu-4f$^{7}$ magnetic moments. The results explain the observation of the carrier density dependent ferromagnetic correlations and anomalous Hall effect in this q2DEG, and demonstrate how combined theoretical and experimental approaches can lead to a deeper understanding of novel electronic phases and serve as a guide for the materials design for advanced electronic applications.

16. Hyperbolic compactification of M-theory and de Sitter quantum gravity

Abstract

We present a mechanism for accelerated expansion of the universe in the generic case of negative-curvature compactifications of M-theory, with minimal ingredients. M-theory on a hyperbolic manifold with small closed geodesics supporting Casimir energy -- along with a single classical source (7-form flux) -- contains an immediate 3-term structure for volume stabilization at positive potential energy. Hyperbolic manifolds are well-studied mathematically, with an important rigidity property at fixed volume. They and their Dehn fillings to more general Einstein spaces exhibit explicit discrete parameters that yield small closed geodesics supporting Casimir energy. The off-shell effective potential derived by M. Douglas incorporates the warped product structure via the constraints of general relativity, screening negative energy. Analyzing the fields sourced by the localized Casimir energy and the available discrete choices of manifolds and fluxes, we find a regime where the net curvature, Casimir energy, and flux compete at large radius and stabilize the volume. Further metric and form field deformations are highly constrained by hyperbolic rigidity and warping effects, leading to calculations giving strong indications of a positive Hessian, and residual tadpoles are small. We test this via explicit back reacted solutions and perturbations in patches including the Dehn filling regions, initiate a neural network study of further aspects of the internal fields, and derive a Maldacena-Nunez style no-go theorem for Anti-de Sitter extrema. A simple generalization incorporating 4-form flux produces axion monodromy inflation. In a broad class of gravity theories, the equations of motion for vacuum compactifications give a curvature bound on the Ricci tensor minus a multiple of the Hessian of the warping function. Using results in so-called Bakry--\'Emery geometry, we put rigorous general bounds on the KK scale in gravity compactifications in terms of the reduced Planck mass or the internal diameter. We reexamine in this light the local behavior in type IIA for the class of supersymmetric solutions most promising for scale separation. Abstract

The field of oxide spintronics can strongly benefit from the establishment of robust ferromagnetic insulators with near room-temperature Curie temperature. Here we investigate the structural, electronic, and magnetic properties of atomically-precise epitaxially-strained thin films of the double perovskite La$_2$NiMnO$_6$ (LNMO) grown by off-axis radio-frequency magnetron sputtering. We find that the films retain both a strong insulating behavior and a bulk-like Curie temperature in the order of 280 K, nearly independently from epitaxial strain conditions. These results suggest a prospective implementation of LNMO films in multi-layer device architectures where a high-temperature ferromagnetic insulating state is a prerequisite., Comment: 15 pages, 4 figures

Abstract

Computational complexity is a new quantum information concept that may play an important role in holography and in understanding the physics of the black hole interior. We consider quantum computational complexity for $n$ qubits using Nielsen's geometrical approach. We investigate a choice of penalties which, compared to previous definitions, increases in a more progressive way with the number of qubits simultaneously entangled by a given operation. This choice turns out to be free from singularities. We also analyze the relation between operator and state complexities, framing the discussion with the language of Riemannian submersions. This provides a direct relation between geodesics and curvatures in the unitaries and the states spaces, which we also exploit to give a closed-form expression for the metric on the states in terms of the one for the operators. We analyze the stability of four-dimensional de Sitter vacua constructed by compactifying massive Type IIA supergravity in the presence of two O8 sources [1]. When embedded in String Theory the first source has a clear interpretation as an O8$_-$ plane, but the second one could correspond to either an O8$_+$ plane or to an O8$_-$ plane with 16 D8-branes on top. We find that this latter solution has a tachyonic instability, corresponding to the D8-branes moving away from the O8$_-$ plane. Abstract

Studies on oxide quasi-two dimensional electron gas (q2DEG) have been a playground for the discovery of novel and sometimes unexpected phenomena, like the reported magnetism at the surface and at the interface between LaAlO$_{3}$ and SrTiO$_{3}$ non-magnetic materials. However, magnetism in this system is weak and there are evidences of a not intrinsic origin. Here, by using in-situ high-resolution angle resolved photoemission we demonstrate that ferromagnetic EuTiO$_{3}$, the magnetic counterpart of SrTiO$_{3}$ in the bulk, hosts a q2DEG at its (001) surface. This is confirmed by density functional theory calculations with Hubbard U terms in the presence of oxygen divacancies in various configurations, all of them leading to a spin-polarized q2DEG related to the ferromagnetic order of Eu-4f magnetic moments. 22. On AdS$_7$ stability

Abstract

AdS$_7$ supersymmetric solutions in type IIA have been classified, and they are infinitely many. Moreover, every such solution has a non-supersymmetric sister. In this paper, we study the perturbative and non-perturbative stability of these non-supersymmetric solutions, focusing on cases without orientifolds. Perturbatively, we first look at the KK spectrum of spin-2 excitations. This does not exhibit instabilities, but it does show that there is no separation of scales for either the BPS and the non-BPS case, thus proving for supersymmetric AdS$_7$ a well-known recent conjecture. We then use 7d gauged supergravity and a brane polarization computation to access part of the spectrum of KK scalars. The result signals an instability for all non-supersymmetric solutions except those that have a single D8 on each side. We finally look at non-perturbative instabilities, and find that NS5 bubbles make these remaining solutions decay., Comment: 1+42 pages, 3 figures

In previous work, we found ten-dimensional solutions to the supergravity equations of motion with a dS$_4$ factor and O8-planes. We generalize this analysis and obtain other solutions in the same spirit, with an O8$_+$ and an O6$_-$. We examine our original solutions in more detail, focusing in particular on the O8$_-$ singularities and on the issues created by their boundary conditions. 24. Classical de Sitter Solutions of Ten-Dimensional Supergravity

Abstract

We find four-dimensional de Sitter compactifications of type IIA supergravity by directly solving the ten-dimensional equations of motion. In the simplest examples, the internal space has the topology of a circle times an Einstein manifold of negative curvature. An orientifold acts on the circle with two fixed loci, at which an O8$_-$ and an O8$_+$ plane sit. These orientifold planes are fully backreacted and localized. While the solutions are numerical, the charge and tension of the orientifold planes can be verified analytically. Our solutions have moduli at tree level and can be made parametrically weakly-coupled and weakly-curved. Abstract

We find non-supersymmetric AdS$_8$ solutions of type IIA supergravity. The internal space is topologically an $S^2$ with a U(1) isometry. The only non-zero flux is $F_0$; an O8 sourcing it is present at the equator of the $S^2$. The warping function and dilaton are non-constant. It is also possible to add D8-branes on top of the O8. Possible destabilizing brane bubbles (whose presence would be suggested by the weak-gravity conjecture) are either absent or collapsing. Our solutions are candidate holographic duals to unitary interacting CFTs in seven dimensions with exceptional global symmetry. Abstract

A notable class of superconformal theories (SCFTs) in six dimensions is parameterized by an integer $N$, an ADE group $G$, and two nilpotent elements $\mu_\mathrm{L,R}$ in $G$. Nilpotent elements have a natural partial ordering, which has been conjectured to coincide with the hierarchy of renormalization-group flows among the SCFTs. In this paper we test this conjecture for $G=\mathrm{SU}(k)$, where AdS$_7$ duals exist in IIA. We work with a seven-dimensional gauged supergravity, consisting of the gravity multiplet and two $\mathrm{SU}(k)$ non-Abelian vector multiplets. We show that this theory has many supersymmetric AdS$_7$ vacua, determined by two nilpotent elements, which are naturally interpreted as IIA AdS$_7$ solutions. The BPS equations for domain walls connecting two such vacua can be solved analytically, up to a Nahm equation with certain boundary conditions. The latter admit a solution connecting two vacua if and only if the corresponding nilpotent elements are related by the natural partial ordering, in agreement with the field theory conjecture., Comment: 33 pages, 2 figures

Abstract

Charge density waves are a common occurrence in all families of high critical temperature superconducting cuprates. Although consistently observed in the underdoped region of the phase diagram and at relatively low temperatures, it is still unclear to what extent they influence the unusual properties of these systems. Using resonant x-ray scattering we carefully determined the temperature dependence of charge density modulations in (Y,Nd)Ba$_2$Cu$_3$O$_{7-{\delta}}$ for three doping levels. We discovered short-range dynamical charge density fluctuations besides the previously known quasi-critical charge density waves. They persist up to well above the pseudogap temperature T*, are characterized by energies of few meV and pervade a large area of the phase diagram, so that they can play a key role in shaping the peculiar normal-state properties of cuprates., Comment: 34 pages, 4 figures, 11 supplementary figures

Abstract

We used x-ray absorption spectroscopy to study the orbital symmetry and the energy band splitting of (111) LaAlO${_3}$/SrTiO${_3}$ and LaAlO${_3}$/EuTiO${_3}$/SrTiO${_3}$ heterostructures, hosting a quasi two-dimensional electron system (q2DES), and of a Ti-terminated (111) SrTiO${_3}$ single crystal, also known to form a q2DES at its surface. We demonstrate that the bulk tetragonal Ti-3d D${_4}$${_h}$ crystal field is turned into trigonal D${_3}$${_d}$ crystal field in all cases. The symmetry adapted a${_1}$${_g}$ and e${^\pi_g}$ orbitals are non-degenerate in energy and their splitting, \Delta, is positive at the bare STO surface but negative in the heterostructures, where the a${_1}$${_g}$ orbital is lowest in energy. These results demonstrate that the interfacial symmetry breaking induced by epitaxial engineering of oxide interfaces has a dramatic effect on their electronic properties, and it can be used to manipulate the ground state of the q2DES., Comment: 6 pages article, plus 5 pages supplementary information

Abstract

The geometry of the ${\cal N} = 3$, SO(4)--invariant, AdS$_4$ solution of massive type IIA supergravity that uplifts from the ${\cal N} = 3 $ vacuum of $D=4$ ${\cal N} = 8$ dyonic ISO(7) supergravity is investigated. Firstly, a $D=4$, SO(4)--invariant restricted duality hierarchy is constructed and used to uplift the entire, dynamical SO(4)--invariant sector to massive type IIA. The resulting consistent uplift formulae are used to obtain a new local expression for the ${\cal N} = 3 $ AdS$_4$ solution in massive IIA and analyse its geometry. Locally, the internal $S^6$ geometry corresponds to a warped fibration of $S^2$ and a hemisphere of $S^4$. This can be regarded as a warped generalisation of the usual twistor fibration geometry. Finally, the triplet of Killing spinors corresponding to the ${\cal N}=3$ solution are constructed and shown to obey the massive type IIA Killing spinor equations., Comment: 43 pages. v2: References added and minor typos corrected. The clinical spectrum of SSc is wide and its manifestations may lead to severe morbidity and mortality, in addition to a great impact on patients’ quality of life. Due to the multifaceted clinical manifestations of SSc, its management requires a combined expertise of different medical specialists to guarantee an adequate disease control and prevent organ complications. Multi-disciplinary teams (MDT), which are composed by physicians and other specialized health professionals, represent therefore a key element for the comprehensive management of SSc patients. Moreover, MTD can improve communication and patients’ empowerment while the presence of dedicated nurses can help patients to ask questions about their condition. The scope of this narrative review is to analyse the available evidences regarding the role of MDT in the management of SSc patients, and how this holistic approach may improve different disease domains and the overall prognosis. MDT regarding the cardiovascular and lung complication are the more represented in literature, given the great impact in prognosis. Nonetheless, MDT have been shown to be fundamental also in other disease domains as they can intercept early manifestations, thus stratifying patients based on the individual risks in order to personalize patients’ follow-up. MDTs may also minimize the treatment delay, enabling fast-track specialist referral. Abstract

Charge modulations are considered as a leading competitor of high-temperature superconductivity in the underdoped cuprates, and their relationship to Fermi surface reconstructions and to the pseudogap state is an important subject of current research. Overdoped cuprates, on the other hand, are widely regarded as conventional Fermi liquids without collective electronic order. For the overdoped (Bi,Pb)2.12Sr1.88CuO6+{\delta} (Bi2201) high-temperature superconductor, here we report resonant x-ray scattering measurements revealing incommensurate charge order reflections, with correlation lengths of 40-60 lattice units, that persist up to at least 250K. Charge order is markedly more robust in the overdoped than underdoped regime but the incommensurate wave vectors follow a common trend; moreover it coexists with a single, unreconstructed Fermi surface, without pseudogap or nesting features, as determined from angle-resolved photoemission spectroscopy. This re-entrant charge order is reproduced by model calculations that consider a strong van Hove singularity within a Fermi liquid framework., Comment: 11 pages, 5 figures

Abstract

In high Tc superconductors the magnetic and electronic properties are determined by the probability that valence electrons virtually jump from site to site in the CuO2 planes, a mechanism opposed by on-site Coulomb repulsion and favored by hopping integrals. The spatial extent of the latter is related to transport properties, including superconductivity, and to the dispersion relation of spin excitations (magnons). Here, for three antiferromagnetic parent compounds (single-layer Bi2Sr0.99La1.1CuO6+delta, double-layer Nd1.2Ba1.8Cu3O6 and infinite-layer CaCuO2) differing by the number of apical atoms, we compare the magnetic spectra measured by resonant inelastic x-ray scattering over a significant portion of the reciprocal space and with unprecedented accuracy. We observe that the absence of apical oxygens increases the in-plane hopping range and, in CaCuO2, it leads to a genuine 3D exchange-bond network. These results establish a corresponding relation between the exchange interactions and the crystal structure, and provide fresh insight into the materials dependence of the superconducting transition temperature., Comment: 9 pages, 4 figures, 1 Table, 42 references

Abstract

Large ring-laser gyroscopes are capable of measuring angular rotations with a precision well below fractions of $prad/s$, not far from $10^{-14}$ $rad/s$, the accuracy required for General Relativity tests, this is what the GINGER (Gyroscope-IN-GEneral-Relativity) experiment is aiming for. These features do not guarantee the possibility of measuring the General Relativity Lense--Thirring effect, that manifests itself as a tiny ($\approx 10^{-9} \times \Omega_E$) perturbation of the Earth rotation rate. An underground location being in principle less affected by external local disturbances represents a good candidate for housing such a challenging experiment. GINGERino is a test apparatus to investigate the residual local disturbances in the most inner part of the underground international laboratory of the GranSasso (LNGS). It consists of a square ring laser with a $3.6$ m side. The instrument has been tailored to be the larger allowed by the particular location inside the laboratory. Its main objective is to measure the very low frequency rotational motions, in order to prove that LNGS is a suitable location for very low noise measurements and, possibly, General Relativity tests. Aside this main goal, GINGERino will provide unique data for geodesy and geophysics. Its installation has been completed during 2015. Since then, several long set of data have been collected, and the apparatus has been continuously running unattended for more than one week. The typical power spectrum sensitivity was a few $ 10^{-10} rad/s/\sqrt(Hz)$, with integration time not longer than tens of seconds. Typical clinical features include macrothrombocytopenia, bleeding and elevated vitamin B12 levels, while bone marrow fibrosis and splenomegaly may develop during disease progression. Recently, the involvement of other blood lineages has been highlighted, revealing the role of NBEAL2 outside the megakaryocyte-platelet axis. Low leukocyte counts, decreased neutrophil granulation and impaired neutrophil extracellular trap formation represent prominent findings in GPS patients, reflecting deranged innate immunity and associated with an increased susceptibility to infection. In addition, low numbers and impaired degranulation of NK cells have been demonstrated in animal models. Autoimmune diseases involving different organs and a spectrum of autoantibodies are present in a substantial proportion of GPS patients, expanding the syndromic spectrum of this disorder and pointing to dysregulation of the adaptive immune response. Low-grade inflammation, as evidenced by elevation of liver-derived acute-phase reactants, is another previously unrecognized feature of GPS which may contribute to disease manifestations. Abstract

Aims: A set of indicators to assess the quality of care for patients hospitalized for heart failure was developed by an expert working group of the Italian Health Ministry. Because a better performance profile measured using these indicators does not necessarily translate to better outcomes, a study to validate these indicators through their relationship with measurable clinical outcomes and healthcare costs supported by the Italian National Health System was carried out. Methods and results: Residents of four Italian regions (Lombardy, Marche, Lazio, and Sicily) who were newly hospitalized for heart failure (irrespective of stage and New York Heart Association class) during 2014–2015 entered in the cohort and followed up until 2019. Adherence to evidence-based recommendations [i.e. renin–angiotensin–aldosterone system (RAS) inhibitors, beta-blockers, mineralocorticoid receptor antagonists (MRAs), and echocardiograms (ECCs)] experienced during the first year after index discharge was assessed. Composite clinical outcomes (cardiovascular hospital admissions and all-cause mortality) and healthcare costs (hospitalizations, drugs, and outpatient services) were assessed during the follow-up. The restricted mean survival time at 5 years (denoted as the number of months free from clinical outcomes), the hazard of clinical outcomes (according to the Cox model), and average annual healthcare cost (expressed in euros per person-year) were compared between adherent and non-adherent patients. A non-parametric bootstrap method based on 1000 resamples was used to account for uncertainty in cost-effectiveness estimates. A total of 41 406 patients were included in this study (46.3% males, mean age 76.9 ± 9.4 years). Adherence to RAS inhibitors, beta-blockers, MRAs, and ECCs were 64%, 57%, 62%, and 20% among the cohort members, respectively. Abstract

Duchenne muscular dystrophy (DMD) is a neuromuscular condition characterized by muscle weakness. The Performance of upper limb (PUL) test is designed to evaluate upper limb function in DMD patients across three domains. The aim of this study is to identify frequently lost or gained PUL 2.0 abilities at distinct functional stages in DMD patients. This retrospective study analyzed prospectively collected data on 24-month PUL 2.0 changes related to ambulatory function. Ambulant patients were categorized based on initial 6MWT distance, non-ambulant patients by time since ambulation loss. Each PUL 2.0 item was classified as shift up, no change, or shift down. The study's cohort incuded 274 patients, with 626 paired evaluations at the 24-month mark. Among these, 55.1 % had activity loss, while 29.1 % had gains. Ambulant patients showed the lowest loss rates, mainly in the shoulder domain. The highest loss rate was in the shoulder domain in the transitioning subgroup and in elbow and distal domains in the non-ambulant patients. Younger ambulant patients demonstrated multiple gains, whereas in the other functional subgroups there were fewer gains, mostly tied to singular activities. Our findings highlight divergent upper limb domain progression, partly linked to functional status and baseline function.

Abstract

We studied the structural, magnetic and transport properties of LaAlO3/EuTiO3/SrTiO3 heterostructures grown by Pulsed Laser Deposition. The samples have been characterized in-situ by electron diffraction and scanning probe mi-croscopy and ex-situ by transport measurements and x-ray absorption spectroscopy. LaAlO3/EuTiO3/SrTiO3 films show a ferromagnetic transition at T<7.5 K, related to the ordering of Eu2+ spins, even in samples characterized by just two EuTiO3 unit cells. A finite metallic conductivity is observed only in the case of samples composed by one or two EuTiO3 unit cells and, simultaneously, by a LaAlO3 thickness equal or above 4 unit cells. The role of ferromagnetic EuTiO3 on the transport properties of delta-doped LaAlO3/EuTiO3/SrTiO3 is critically discussed.

Abstract

Background: Hypertension management in older patients represents a challenge, particularly when hospitalized. Objective: The objective of this study is to investigate the determinants and related outcomes of antihypertensive drug prescription in a cohort of older hospitalized patients. Methods: A total of 5671 patients from REPOSI (a prospective multicentre observational register of older Italian in-patients from internal medicine or geriatric wards) were considered; 4377 (77.2%) were hypertensive. Minimum treatment (MT) for hypertension was defined according to the 2018 ESC guidelines [an angiotensin-converting-enzyme-inhibitor (ACE-I) or an angiotensin-receptor-blocker (ARB) with a calcium-channel-blocker (CCB) and/or a thiazide diuretic; if >80 years old, an ACE-I or ARB or CCB or thiazide diuretic]. Determinants of MT discontinuation at discharge were assessed. Study outcomes were any cause rehospitalization/all cause death, all-cause death, cardiovascular (CV) hospitalization/death, CV death, non-CV death, evaluated according to the presence of MT at discharge. Results: Hypertensive patients were older than normotensives, with a more impaired functional status, higher burden of comorbidity and polypharmacy. A total of 2233 patients were on MT at admission, 1766 were on MT at discharge. Discontinuation of MT was associated with the presence of comorbidities (lower odds for diabetes, higher odds for chronic kidney disease and dementia). An adjusted multivariable logistic regression analysis showed that MT for hypertension at discharge was associated with lower risk of all-cause death, all-cause death/hospitalization, CV death, CV death/hospitalization and non-CV death. Conclusions: Guidelines-suggested MT for hypertension at discharge is associated with a lower risk of adverse clinical outcomes. Nevertheless, changes in antihypertensive treatment still occur in a significant proportion of older hospitalized patients.

Abstract

Background and objective: Chronic obstructive pulmonary disease (COPD) and heart failure (HF) mutually increase the risk of being present in the same patient, especially if older. Whether or not this coexistence may be associated with a worse prognosis is debated. Therefore, employing data derived from the REPOSI register, we evaluated the clinical features and outcomes in a population of elderly patients admitted to internal medicine wards and having COPD, HF or COPD + HF. Methods: We measured socio-demographic and anthropometric characteristics, severity and prevalence of comorbidities, clinical and laboratory features during hospitalization, mood disorders, functional independence, drug prescriptions and discharge destination. The primary study outcome was the risk of death. Results: We considered 2,343 elderly hospitalized patients (median age 81 years), of whom 1,154 (49%) had COPD, 813 (35%) HF, and 376 (16%) COPD + HF. Patients with COPD + HF had different characteristics than those with COPD or HF, such as a higher prevalence of previous hospitalizations, comorbidities (especially chronic kidney disease), higher respiratory rate at admission and number of prescribed drugs. Patients with COPD + HF (hazard ratio HR 1.74, 95% confidence intervals CI 1.16–2.61) and patients with dementia (HR 1.75, 95% CI 1.06–2.90) had a higher risk of death at one year. The Kaplan–Meier curves showed a higher mortality risk in the group of patients with COPD + HF for all causes (p = 0.010), respiratory causes (p = 0.006), cardiovascular causes (p = 0.046) and respiratory plus cardiovascular causes (p = 0.009). Conclusion: In this real-life cohort of hospitalized elderly patients, the coexistence of COPD and HF significantly worsened prognosis at one year. This finding may help to better define the care needs of this population.

Abstract

Background: Active relapsing–remitting (RR) and secondary progressive (SP) multiple sclerosis (MS) are currently defined as “relapsing MS” (RMS). The aim of this cross-sectional study was to assess drivers of treatment switches due to clinical relapses in a population of RMS patients collected in the Italian MS and Related Disorders Register (I-MS&RD). Methods: RRMS and SPMS patients with at least one relapse in a time window of 2 years before of data extraction were defined as RMS. Factors associated with disease-modifying therapy (DMT) switching due to clinical activity were assessed through multivariable logistic regression models in which treatment exposure was included as the last recorded DMT and the last DMT’s class [moderate-efficacy (ME), high-efficacy (HE) DMTs and anti-CD20 drugs]. Results: A cohort of 4739 RMS patients (4161 RRMS, 578 SPMS) was extracted from the I-MS&RD. A total of 2694 patients switching DMTs due to relapses were identified. Switchers were significantly (p < 0.0001) younger, less disabled, more frequently affected by an RR disease course in comparison to non-switcher patients. The multivariable logistic regression models showed that Alemtuzumab (OR 0.08, 95% CI 0.02–0.37), Natalizumab (0.48, 0.30–0.76), Ocrelizumab (0.1, 0.02–0.45) and Rituximab (0.23, 0.06–0.82) exposure was a protective factor against treatment switch due to relapses. Moreover, the use of HE DMTs (0.43, 0.31–0.59), especially anti-CD20 drugs (0.14, 0.05–0.37), resulted to be a protective factor against treatment switch due to relapses in comparison with ME DMTs. Conclusions: More than 50% of RMS switched therapy due to disease activity. HE DMTs, especially anti-CD20 drugs, significantly reduce the risk of treatment switch.

Abstract

Introduction: Over the years, disease registers have been increasingly considered a source of reliable and valuable population studies. However, the validity and reliability of data from registers may be limited by missing data, selection bias or data quality not adequately evaluated or checked. This study reports the analysis of the consistency and completeness of the data in the Italian Multiple Sclerosis and Related Disorders Register. Methods: The Register collects, through a standardized Web-based Application, unique patients. Data are exported bimonthly and evaluated to assess the updating and completeness, and to check the quality and consistency. Eight clinical indicators are evaluated. Results: The Register counts 77,628 patients registered by 126 centres. The number of centres has increased over time, as their capacity to collect patients. The percentages of updated patients (with at least one visit in the last 24 months) have increased from 33% (enrolment period 2000–2015) to 60% (enrolment period 2016–2022). In the cohort of patients registered after 2016, there were ≥ 75% updated patients in 30% of the small centres (33), in 9% of the medium centres (11), and in all the large centres (2). Clinical indicators show significant improvement for the active patients, expanded disability status scale every 6 months or once every 12 months, visits every 6 months, first visit within 1 year and MRI every 12 months. Conclusions: Data from disease registers provide guidance for evidence-based health policies and research, so methods and strategies ensuring their quality and reliability are crucial and have several potential applications.

Abstract

In models with extra dimensions, matter particles can be easily localized to a ‘brane world’, but gravitational attraction tends to spread out in the extra dimensions unless they are small. Strong warping gradients can help localize gravity closer to the brane. In this note we give a mathematically rigorous proof that the internal wave-function of the massless graviton is constant as an eigenfunction of the weighted Laplacian, and hence is a power of the warping as a bound state in an analogue Schrödinger potential. This holds even in presence of singularities induced by thin branes. We also reassess the status of AdS vacuum solutions where the graviton is massive. We prove a bound on scale separation for such models, as an application of our recent results on KK masses. We also use them to estimate the scale at which gravity is localized, without having to compute the spectrum explicitly. For example, we point out that localization can be obtained at least up to the cosmological scale in string/M-theory solutions with infinite-volume Riemann surfaces; and in a known class of N = 4 models, when the number of NS5- and D5-branes is roughly equal.

Abstract

Objective To describe demographic, clinical and laboratory features of systemic sclerosis sine scleroderma (ssSSc) in a large multicentre systemic sclerosis (SSc) cohort. Methods Data involving 1808 SSc patients from Italian Systemic sclerosis PRogression INvestiGation registry were collected. The ssSSc was defined by the absence of any cutaneous sclerosis and/or puffy fingers. Clinical and serological features of ssSSc were compared with limited cutaneous (lcSSc) and diffuse cutaneous (dcSSc) subsets. Results Among patients with SSc, only 61 (3.4%) were classified as having ssSSc (F/M=19/1). Time from Raynaud's phenomenon (RP) onset to diagnosis was longer in ssSSc (3 years, IQR 1-16.5) than lcSSc (2 years, IQR 0-7), and dcSSc (1 year, IQR 0-3) (p<0.001). Clinical ssSSc phenotype was comparable to lcSSc, except for digital pitting scars (DPS) (19.7% vs 42%, p=0.01), but significantly milder than dcSSc, particularly for digital ulcers (DU) (6.6% vs 35.7%, p<0.001), oesophagus (46.2% vs 63.5%, p=0.009), lung (mean diffusion capacity for carbon monoxide 72.2±19.6 vs 62.4±22.8, p=0.009; mean forced vital capacity 105.6±21.7 vs 89.2±20.9, p<0.001) and major videocapillaroscopic alterations (late pattern 8.6% vs 47.6%, p<0.001). Moreover, in ssSSc the percentages of anticentromere and antitopoisomerase were comparable to lcSSc (40% and 18.3% vs 36.7% and 26.6%), but divergent respect to dcSSc (8.6% and 67.4%, p<0.001). Conclusion The ssSSc is a quite rare disease variant characterised by clinico-serological features comparable to lcSSc, but significantly different from dcSSc. Overall, longer RP duration, low percentages of DPS and peripheral microvascular abnormalities, and increased anti-centromere seropositivity distinguish ssSSc. Further investigations based on national registries might provide useful insights on the actual relevance of the ssSSc within the scleroderma spectrum.

Abstract

On July 7, 2023, at 1:21 am, a fire was declared in a retirement home in Milan, Italy. The number of casualties (n = 87) according to the Simple Triage and Rapid Treatment (START) triage system was categorized as 65 green, 14 yellow, 2 red, and 6 black; 75% were women, and the mean age was 85.1 years (± 9). Most patients were unable to walk. A total of 30 basic life support (BLS) ambulances, 3 advanced cardiac life support (ACLS) teams on fast cars, 2 buses, and 1 coordination team were deployed. A scoop and run approach was adopted with patients being transported to 15 health care facilities. The event was terminated at 5:43 am. Though the local mass casualty incident (MCI) response plan was correctly applied, the evacuation of the building was difficult due to the age and comorbidities of the patients. START failed to correctly identify patients categorized as minor. Communication problems arose on site that led to the late evacuation of critical patients.

Abstract

We studied the ferroelectric and ferromagnetic properties of compressive strained and unstrained BiMnO3 thin films grown by rf-magnetron sputtering. BiMnO3 samples exhibit a 2D cube-on-cube growth mode and a pseudo-cubic struc-ture up to a thickness of 15 nm and of 25 nm when deposited on (001) SrTiO3 and (110) DyScO3, respectively. Above these thicknesses we observe a switching to a 3D island growth and a simultaneous structural change to a monoclinic structure characterized by a (00l) orientation of the monoclinic unit cell. While ferromagnetism is observed below Tc = 100 K for all samples, signatures of room temperature ferroelectricity were found only in the pseudo-cubic ultra-thin films, indicating a correlation between electronic and structural orders., Comment: 4 pages, 4 figures
