Funding Information: Finally, Dr Garralda highlighted progress made by Cancer Core Europe (CCE), an independent consortium of major cancer centres in Europe that have worked together to create a multi‐site European Cancer Institute, where joint translation and clinical research is performed, with a special focus on next‐generation clinical trials and precision medicine. The creation of large, shared databases through CCE is expected to accelerate cancer diagnosis, clinical decisions, and outcomes research throughout Europe, even when small subsets of cancer patients are targeted. The Basket of Baskets (BoB) multimodal clinical trial design and the CCE Molecular Tumour Board exemplify how the above can be achieved [ 22,23 ] and have further developed with the recent funding from the EU's Horizon 2020 research and innovation programme to build data‐rich clinical trials (DART). The DART project was launched in February 2021, with the aim to develop digital tools for trial management and clinical decision‐making. DART is expected to improve clinical trial methodology through the introduction of new statistical designs. Moreover, it aims at integrating more accurate imaging and molecular biomarkers of treatment responses into a new generation of clinical trials, while also ensuring patient empowerment and involvement or engagement (Table 2 ). European cancer research stakeholders met in October 2022 in Heidelberg, Germany, at the 5th Gago conference on European Cancer Policy, to discuss the current cancer research and cancer care policy landscape in Europe. Meeting participants highlighted gaps in the existing European programmes focusing on cancer research, including Europe's Beating Cancer Plan (EBCP), the Mission on Cancer (MoC), Understanding Cancer (UNCAN.eu), and the joint action CRANE, and put forward the next priorities, in the form of the Heidelberg Manifesto for cancer research. This meeting report presents all discussions that shed light on how infrastructures can be effectively shaped for translational, prevention, clinical and outcomes cancer research, with a focus on implementation and sustainability and while engaging patients and the public. In addition, we summarize recommendations on how to introduce frameworks for the digitalization of European cancer research. Finally, we discuss what structures, commitment, and resources are needed to establish a collaborative cancer research environment in Europe to achieve the scale required for innovation. publishersversion epub_ahead_of_print

Purpose: Inmate oncologic patients' rates increased drastically worldwide. Elderly, limited exercise, unhealthy diet, hepatitis, HIV+ status, tobacco and alcohol use, constitute the main cancer risk factors. We present an outline of practical oncological management and ethical thinking, in the specific environment of a detention facility. Methods: PubMed, Cochrane Database of Controlled Trials, SCOPUS and grey literature were extensively searched upto October 2021. Ιncarcerated oncologic patients experiencevarious everyday challenges:their confinement in high securityfacilities, the lack of access to critical care and related ethicaldilemmas inherent to the context of a correctional facility. Results: The detention facilities may be inadequate in providing early cancer diagnosis and appropriate care mainlydue to a lack of specialized personnel, b) in-house or in external specialized cancer hospitals, care variability (e.g. admissions in small local or regional hospitals), c) delays inproviding access and d) gatekeeper systems. There is a paucity of administration of a) systemic therapy(chemotherapy, targeted drug therapy etc), b) radiotherapy, c)palliative care, and d) enrollment in clinical trials. Conclusions: Correctional facilities must encourage teamwork between healthcare and correctional professionals inorder to improve the provided anticancer care.

To protect cancer patients from COVID-19 exposure, prioritization strategies are being implemented at global level. Measures include use of telehealth services, deferring elective surgeries, delaying non life-saving therapies, interrupting maintenance and supportive care regimens and suspending screening and regular follow-up visits. Nonetheless, the risk of infection may not always outweigh oncology treatment benefit. Lives of most oncology patients depend on their ability to receive medical, surgical and radiotherapy care. Postponing screening, follow-up and radical surgeries increase patients’ risk of developing metastatic disease. A viral pandemic lasts long time and exhibits seasonal and geographical variations. Though vaccines will be available only in the 2021, a global, aggressive, all-embracing and protracted slowdown of oncologic activities will severely jeopardize patients’ outcomes. A present international oncologists’ panel, ECPC and FAVO, strongly suggest that Hospital measures in a specific geographical area/Nation should be in line with the local epidemic, and restrictions adopted should be adapted and stratified over time.

Introduction. Financial toxicity (FT) is a well-recognized problem in oncology. US-based studies have shown that: (a) cancer patients have a 2.7 times risk of bankruptcy; (b) patients who declare bankruptcy have a 79% greater hazard of death; (c) financial burden significantly impairs quality of life (QoL), and (d) reduces compliance and adherence to treatment prescriptions. The aim of the project is to develop and validate a patient-reported-outcome (PRO) measure to assess FT of cancer patients in Italy, where, despite the universal health coverage given by the national health service, FT is an emerging issue.\ud Methods and analysis. Our hypothesis is that a specific FT measure, which considers the relevant socio-cultural context and health care system, would allow to understand the main determinants of cancer-related financial toxicity in Italy and to contrast them. According to the International Society for Pharmaco-economics and Outcomes Research (ISPOR) guidelines on PROs, the project will include the following steps: (1) concept elicitation (from focus groups with patients and caregivers; literature; oncologists; nurses) and analysis, creating a coding library; (2) item generation (using a format that includes a question and a response on a 4-point Likert scale) and analysis through patients’ cognitive interviews of item importance within different coding categories to produce the draft instrument; (3) factor analysis and internal validation (with Cronbach’s alpha and test-retest for reliability) to produce the final instrument; (4) external validation with QoL anchors and depression scales. The use of the FT measure in prospective trials is also planned. \ud Ethics and dissemination. The protocol is approved by the ethical committees of all the participating centres. The project will tentatively produce a validated tool by the spring 2021.The project might also represent a model and the basis for future cooperation with other European countries, with different health care systems and socio-economic conditions.

Malnutrition is a frequent problem in patients with cancer, which leads to prolonged hospitalisation, a higher degree of treatment-related toxicity, reduced response to cancer treatment, impaired quality of life and a worse prognosis. The attitude towards this issue varies considerably among oncologists, and many malnourished patients still receive inadequate nutritional support. This could be related to the insufficient awareness of nutritional problems among healthcare professionals and the lack of structured collaboration between oncologists and clinical nutritionists. Recently, the Italian Society of Medical Oncology (AIOM), the Italian Society of Artificial Nutrition and Metabolism (SINPE) and the Italian Federation of Volunteer-based Cancer Organisations (FAVO) have initiated a structured collaborative project named ‘integrating nutritional therapy in oncology (INTO)’, with the aim to increase the awareness of nutritional issues among oncologists and, consequently, improve the nutritional care of patients with cancer. Ship structural design is done today resorting to powerful numerical tools. Ship structures come out to be optimal regarding load and failure mechanisms; in spite of this they come to lose that residual capability which in past gave a valuable benefit to overcome certain undesirable side effects. Such effects are not acceptable when ship’s structures are prone to develop high levels of vibrations excited by propulsion plant or other sources. In this frame, the paper investigates a river ship case, where preliminary calculations of structural dynamic response to vibrations induced by on board sources gain a relevant role in defining the minimum thickness of plates and supporting structures’ topology. River ship hulls have indeed light structures due to low demanding environments, which lead to an increase of vibration effects on accommodation decks. The approach here discussed is based on direct calculations of the hull’s structures dynamic response, performed in the frequency range where first harmonic frequencies of main excitation sources play a relevant role in inducing local and global vibration effects. The critical aspect of interaction between structure and fluid will be dealt with by an implicit modeling of the surrounding water around the hull. The aim of the study is to give the reader basic tools to implement, in the ship’s hull preliminary design stage, free and forced vibration analyses in order to support decision making about the hull scantling when vibration effects are not negligible. Actualmente el diseño estructural de buques se lleva a cabo utilizando poderosas herramientas numéricas. De este modo, mientras las estructuras del buque se optimizan para afrontar cargas y mecanismos de colapso, por el contrario han perdido esa capacidad residual que en el pasado les daba un valioso beneficio para superar ciertos indeseables efectos secundarios. Dichos efectos no son aceptables si las estructuras de los buques son propensas a desarrollar altos niveles de vibraciones excitadas por el sistema propulsivo u otras fuentes. En este marco, el artículo investiga el caso de un barco fluvial, donde los cálculos preliminares de la respuesta dinámica a las vibraciones inducidas en las estructuras por fuentes a bordo toman un papel importante en la definición de los espesores mínimos de las planchas y la topología de las estructuras. En efecto los cascos de los buques fluviales tienen estructuras ligeras debido a un entorno poco exigente, que conlleva al aumento de las vibraciones en los puentes habitables. El método tratado en el artículo se basa en el cálculo directo de la respuesta dinámica de las estructuras del casco, realizado en el rango de frecuencia donde las primeras frecuencias armónicas de las principales fuentes de excitación juegan un papel relevante en inducir efectos locales y globales de vibración. El aspecto crítico de la interacción entre la estructura y el fluido será tratado con la modelación implícita del agua circundante al casco. El objetivo del estudio es brindar herramientas básicas para implementar, durante el diseño preliminar del casco, análisis de vibraciones libres y forzadas para respaldar la toma de decisiones respecto al escantillonado cuando los efectos de las vibraciones no son despreciables.

Relevant information on cancer is available, but its organisation and harmonisation is necessary to make it usable for health planners, doctors, patients and other stakeholders. - Cancer registries should be at the core of a European Cancer Information System (ECIS), providing basic cancer indicators on incidence, survival, prevalence and patterns of care but these data need to be systematically linked to clinical, socioeconomic and population data. - The first step towards an ECIS, the harmonisation of incidence and survival data, has been taken during EPAAC in order to update European cancer data and construct a common database computing incidence, survival and prevalence data. - Specific conditions for data use still need to be considered, regulating confidentiality and ownership.
