Abstract

Background/Objectives: The textbook outcome (TO) is a composite tool introduced to uniform surgical units and regulate surgical quality and outcomes. A patient is considered TO only if all predetermined items are met. In colorectal surgery, TO represents a new tool that can achieve important results given the prevalence of colorectal cancers. However, at present, there is a lack of uniformity in the TO's definition. This study utilized the Delphi process to define an Italian version of the TO in colorectal cancer. Methods: The survey consisted of two rounds of online questionnaires submitted to an expert panel in colorectal oncological surgery, renowned academic surgeons, who had attended multiple scientific conferences and who were authors of papers on this specific topic. Five main topics with 26 questions were investigated through an online modified Delphi method. Items with almost 75% agreement achieved consensus. Results: Twenty-eight Italian experts were selected and participated in the two rounds. The Italian version of the textbook outcome in colorectal surgery was defined as the presence of 90-day postoperative survival, negative margins and at least 12 lymph nodes, a minimally invasive approach, ostomy fashioning if preoperatively planned, postoperative complication < Clavien-Dindo 3b, at least 10 ERAS items, no readmission, proper CHT and RT regimens, complete colonoscopy after or before surgery and Tumor Board Evaluation. Conclusions: The textbook outcome in colorectal cancer patients is a quality instrument providing a complete overview of the care of such patients, from diagnosis to treatment. We hereby propose an Italian version of the TO with outcomes chosen by an expert panel.

Abstract

Background: The impact of method of anastomosis and minimally invasive surgical technique on surgical and clinical outcomes after right hemicolectomy is uncertain. The aim of the MIRCAST study was to compare intracorporeal and extracorporeal anastomosis (ICA and ECA respectively), each using either a laparoscopic approach or robot-assisted surgery during right hemicolectomies for benign or malignant tumours. Methods: This was an international, multicentre, prospective, observational, monitored, non-randomized, parallel, four-cohort study (laparoscopic ECA; laparoscopic ICA; robot-assisted ECA; robot-assisted ICA). High-volume surgeons (at least 30 minimally invasive right colectomy procedures/year) from 59 hospitals across 12 European countries treated patients over a 3-year interval The primary composite endpoint was 30-day success, defined by two measures of efficacy-absence of surgical wound infection and of any major complication within the first 30 days after surgery. Secondary outcomes were: overall complications, conversion rate, duration of operation, and number of lymph nodes harvested. Propensity score analysis was used for comparison of ICA with ECA, and robot-assisted surgery with laparoscopy. Results: Some 1320 patients were included in an intention-to-treat analysis (laparoscopic ECA, 555; laparoscopic ICA, 356; robot-assisted ECA, 88; robot-assisted ICA, 321). No differences in the co-primary endpoint at 30 days after surgery were observed between cohorts (7.2 and 7.6 per cent in ECA and ICA groups respectively; 7.8 and 6.6 per cent in laparoscopic and robot-assisted groups). Lower overall complication rates were observed after ICA, specifically less ileus, and nausea and vomiting after robot-assisted procedures. Conclusion: No difference in the composite outcome of surgical wound infections and severe postoperative complications was found between intracorporeal versus extracorporeal anastomosis or laparoscopy versus robot-assisted surgery.

Abstract

The focus on the development of technical and tactical skills and on the enjoyment and involvement of students has grown in the recent years. It has been shown how schools fail to provide adequate support to the development of such abilities during the teaching-learning processes. The principal aim of this study was to estimate the effects of a Tactical Game Model (TGM) on tactical skills (in relation to an invasion game: football and a net game: volleyball) and on the affective domain of learning in primary-school children. The sample of participants included 70 primary-school pupils attending the fourth grade (mean age: 8,9 years). The duration of the protocol was 22 weeks (2 lessons per week). In order to analyse the impact on tactical skills and affective domain of learning, the Team Sport Assessment Procedure (TSAP) and the Physical Activity Enjoyment Scale (PACES-IT) were used. Repeated Measure ANOVA (RM-ANOVA) on TSAP variables revealed a significant model regarding the effect of the TGM on the development of tactical skills [football: F=12,2 (p, Posljednjih je godina sve veći fokus na razvoju tehničkih i taktičkih vještina te na zadovoljstvu učenika i njihovoj želji za sudjelovanjem. Dokazano je kako škole ne pružaju odgovarajuću potporu razvoju takvih sposobnosti tijekom odgojno-obrazovnog procesa. Glavni cilj ovog istraživanja bio je procijeniti utjecaj Modela taktike igre (TGM) na taktičke vještine (u odnosu na invazivne sportske igre: nogomet i sportske igre koje uključuju mrežu: odbojka) te na afektivno područje učenja kod djece osnovnoškolskog uzrasta. Abstract

The traditional teaching methods linked to motor education and based on the canonical transmission of knowledge and learning are now outdated. In fact, we could say that we emphasize all forms of learning that have a dynamic and concrete development in which all children can have a role as a 'protagonist'. The school increasingly conditioned by continuous social changes, cannot be lived only as a place of duplication and transmission of knowledge, but can be lived and organized as a space of creativity, fun and emotion as well as imagination also motor. "This teaching practice is often hampered by the school organization of places and times and by the training of teachers' skills" (Lipoma, 2014, p.201). The environment, therefore, must be organized and structured according to the needs of children and their age groups, full of materials, large and small tools where it is possible, however, above all to make concrete experiences experiencing successes and failures in a safe environment. Another fundamental element that distinguishes the success of motor learning is the time dedicated to the movement that lately is reduced and of little quality. “The time devoted to recreational activities has decreased and this is found not only in the school but also in moments of extra-school leisure where you notice the increase of games "sedentary" to the detriment of those engines" (Casolo, 2019, p.495).

Abstract

The technological innovations of the last decade have caused countless changes to everyone's lifestyle, especially in the number of hours spent in front of a screen. These generational differences are even more marked if the new generations of students are taken into consideration, in fact, even at the school level, technological innovations have brought changes. Just as lifestyle and teaching have changed, what is considered as the "needs" of pupils and how they live and relate both in formal and informal environments have also changed. Considering the emergency period caused by the COVID-19 pandemic, greater attention has been paid to the importance of exercising and everyone has also re-evaluated the beauty and benefits of exercising outdoors. Outdoor Movement Education in this sense can be a key that allows the new generations to experience the emotions and sensations that can be felt during outdoor moving activities. This work is a reflection on how technology can be a tool to increase the benefits in Outdoor Movement Education considering children and youth with special needs. The results of this work have shown how the use of technology in Outdoor Movement Education brings numerous advantages both under the didactic profile (i.e., life skills) and with regard to the cognitive (i.e., attention and concentration), motor (i.e., motor skills, fundamental movement skills) and affective (i.e., enjoyment) domains of learning respectively.
