Aim and Background: A noticeable gap in the literature is observed in factors associated with the occurrence of respiratory illness among people working in garment industries. This study aims to investigate whether spirometry screening is warranted for individuals employed within the garment industry. Subjects and Methods: This is an observational cross-sectional pilot study conducted in asymptomatic workers of various sectors in a garment industry using spirometry to check for the presence of small airway obstruction. Duration of work along with the nature of work was taken into consideration to see if any correlation exists with the small airway obstruction. Results: A random sample of 40 asymptomatic individuals was drawn consisting of 16 males and 24 females with 47.5% of them working for around 5–10 years. Ninety-five percentage showed no evidence of small airway obstruction and only 5% showed small airway obstruction showing that no correlation exists between the duration of work and the development of small airway obstruction in the garment industry. Conclusion: There appears to be no significant link between the duration of employment in the garment industry and the development of small airway obstruction as compared to the textile/cotton industry, yet annual screening could be integrated into routine health assessments.

Background: The leather industry stands as one of India’s burgeoning sectors, boasting significant manpower. Workers in the finishing sector, particularly those involved in polishing, face heightened risks due to exposure to formaldehyde, aniline, and resins, which can lead to respiratory complications. Aims and Objectives: The aims and objectives of the study are to determine the small airway obstruction in asymptomatic finishing sector workers of a leather factory. Materials and Methods: The current study was a cross-sectional study conducted in a leather factory located in Chennai, Tamil Nadu. A random sample of 60 workers was drawn from different sectors of a leather factory. Basic demographic details, smoking history, nature of work, exposure to irritants along with duration, and respiratory symptoms were collected. All workers were subjected to spirometry as per ATS guidelines and the results were analyzed. Association between length (years) of exposure, respiratory symptoms, and spirometry impairment among the workers were analysed. Results: Out of 60 samples, 38 were exposed to irritants and 22 were not exposed to irritants. Out of 38, 25 had small airway disease, seven had large airway obstruction, one had a restrictive pattern, and five had normal spirometry. 42.1% of them were asymptomatic. Conclusion: Small airway obstruction is notably prevalent, especially among asymptomatic workers in the polishing sector. Prolonged exposure is associated with the development of large airway obstructive disease. This study underscores the importance of continuous monitoring for airway diseases in occupationally exposed workers, advocating for timely preventive and protective measures to curb disease progression.

The knowledge of the tracheobronchial tree is fundamental for a bronchoscopist, especially for diagnostic, therapeutic, and surgical options that are under consideration. Among all lobes, the right upper lobe (RUL) has a relatively high incidence of tumor and airspace diseases. Most bronchial anomalies are on the right side. Bifurcation is the most common anatomical variation. Quadrification has a very low prevalence. We report the case of a 54-year-old male who came with complaints of hemoptysis and showed a quadrivial type of bronchial division of RUL during a bronchoscopic examination after which three-dimensional computed tomography bronchography showed a supernumerary lateral or axillary segment bronchus. Awareness of this variant may explain otherwise confusing findings and its clinical implication is important for every pulmonologist.

Fowler calls as a Bad Smell which is the symbols of potential problem in the code that may necessitate a refactoring. Inexperienced software felt challenging to resolve the bad smells even with the help of refactoring tools. The intention is that the developers don't know where to invoke the refactoring tools and how to choose the refactoring tools for identifying various code smells. For this projected a framework to perform refactoring promptly. By this the developer may refactor the bad smells and resolves them promptly. Refactoring is a well-established practice that aims at improving the internal structure of a software system without changing its external behavior. Existing literature provides evidence of how and why developers perform refactoring in practice. In this proposed work to continue on this line of research by support the MICA-GRDFC technique is more capable and operative than state of the art refactoring techniques practices in 200 open source systems. The treatment of malignant superior vena cava syndrome (SVCS) revolves around radiotherapy and chemotherapy to relieve symptoms with surgery being ruled out due to the advanced stage of malignancy. Primary placement of endovascular stent for palliation of malignant SVCS is not commonly reported in the literature. Here, we present two cases of malignant superior vena cava syndrome with successful relief of symptoms after the placement of endo vascular stent.

A handover mechanism in a vehicular network can link a mobile vehicle to the Internet. Nonetheless, Vehicular Ad hoc Networks (VANETs) have a tendency to change their network topology frequently because of the mobility of vehicles. Since there will be many handovers, network performance will be negatively affected. A new architecture called Software Defined Network (SDN) can be used to make the vehicular network more efficient by making network devices that connect to each other programmable. This research proposes a seamless handover system in which the Software-Defined Networking (SDN) and Media-Independent-Handover (MIH) Framework are used to implement dynamic topology changes in VANETs. VANET as well as Internet of Things (IoT) plays a good role on the handover scheme. IoT gives a better results in the traffic management, security and connectivity. In order to demonstrate the dominance of our proposed model, network simulations are carried out from a variety of angles and perspectives. The results reveal that when a handover occurs, our solution can greatly enhance network performance. The performance can be evaluated in the form handover delay, handover failure ratio, throughput and packet loss with 5%, 40%, 10% and 2% improved with the existing methods.

Abstract Background Intra-regional cultural and linguistic differences are common in low- and middle-income countries. To sensitise undergraduate medical students to the social and contextual determinants of health to achieve the ‘health for all’ goal, these countries must focus on innovative teaching methods. The early introduction of a Community Orientation Program (COP) as a Community-based Medical Education (CBME) method could be a game changing strategy. In this paper the methods, evaluation, and implication of the COP in an Indian setting are described. Methods The curriculum of the COP was developed based on the analysis, design, development, implementation, and evaluation (ADDIE) model for educational intervention. In this learner-centric and supervised educational program, the key aim was to focus on developing students’ communication skills, observation power and enhancing their motivation for learning through collaborative learning. To meet the objectives of the COP, a situated learning model under the constructivism theory was adopted. Results Between 2016 and 2019, 557 students were trained through the COP by visiting more than 1300 households in ten villages. To supplement the students’ observations in the community, more than 150 small group discussions, a health education programme for the community and summary presentations were conducted. The students’ feedback indicated the need to improve the clinical examinations demonstration quality and increase the number of instruments for clinical examinations. More than 80% of students felt that the program would assist them to improve their communication skills, their understanding of the various socio-demographic factors associated with the common diseases, and it will enable them to respect the local culture during their clinical practice. Conclusions Early initiation of the COP as a CBME method in the undergraduate medical curriculum in an Indian setting has shown promising results. Further evidence is required to adopt such a program routinely for under-graduate medical teaching in the low- and middle- income settings.

A mobile terminal will expect a number of handoffs within its call duration. In the event of a mobile call, when a mobile node moves from one cell to another, it should connect to another access point within its range. In case there is a lack of support of its own network, it must changeover to another base station. In the event of moving on to another network, quality of service parameters need to be considered. In our study we have used the Markov decision process approach for a seamless handoff as it gives the optimum results for selecting a network when compared to other multiple attribute decision making processes. We have used the network cost function for selecting the network for handoff and the connection reward function, which is based on the values of the quality of service parameters. We have also examined the constant bit rate and transmission control protocol packet delivery ratio. We used the policy iteration algorithm for determining the optimal policy. Intravenous location of lobular capillary haemangioma/ pyogenic granuloma is rare with less than 50 cases reported. It is extremely unusual to get a metastatic carcinoma within an intravascular lobular capillary haemangioma (IVLCH). To our knowledge, there are no reported cases of intravenous lobular capillary haemangioma harbouring a metastatic carcinoma. Here we discuss a case of metastatic prostatic carcinoma within an IVLCH along with a review of literature of intravenous pyogenic granuloma and analysis of the possible aetiological factors for the development of lobular capillary haemangioma in our patient.

Background: There is dearth of literature on asthma-chronic obstructive pulmonary disease overlap syndrome (ACOS) in India. The aim was to compare clinical characteristics between patients with ACOS and non-ACOS COPD and to identify clinical predictors of ACOS in patients with COPD. Methods: We conducted a retrospective study by reviewing data collected from patients performing spirometry at our hospital. Those with postbronchodilator FEV1/FVC
