Introduction: The Three Dimensional Fast Imaging Employing Steady state Acquisition (3D FIESTA) has higher spatial resolution between the Cerebrospinal Fluid (CSF) and cranial nerves with accurate identification of Cerebellopontine Angle (CPA) and internal auditory canal tumours and takes shorter acquisition imaging time than conventional Magnetic Resonance Imaging (MRI) scan. Aim: To evaluate the efficacy of 3D FIESTA imaging as a screening tool for CPA lesions, hence to depict the fine anatomy of the cisternal and canalicular segments of the facial nerve and vestibulocochlear nerves in order to elucidate the aetiopathogenesis of unexplained inner ear symptoms. Materials and Methods: The present study was a hospital based cross-sectional study which was done in Department of Radiodiagnosis, Chettinad Hospital and Research Institute, Chennai, Tamil Nadu, India. The study was conducted on 30 patients, who were referred for MRI Brain to the department and diagnosed with cerebellopontine angle lesion from August 2018 to October 2019. A 1.5 Tesla, MRI scanner was used to scan all patients with a 8 channel Neurovascular (NV) radiofrequency coil. Along with routine conventional MRI sequences, 3D FIESTA sequence was also performed. All the data was collected and analysed by Statistical Package for Social Sciences (SPSS) software version 23.0. Data for descriptive statistics i.e. frequency and percentage analysis, mean±Standard Deviation (SD), sensitivity, specificity, Positive Predictive Value (PPV) and Negative Predictive Value (NPV) were calculated. Results: On Histopathological Examination (HPE), 63.3% were schwannoma, 16.7% meningioma, 10% epidermoid cyst and 3.3% intracanalicular lipoma. In 6.7% of patients, imaging features were in favour of CPA arachnoid cyst. The size of the intracanalicular part of tumour was underestimated in T2 weighted images (T2WI). 3D FIESTA gave a better estimated tumour area, even though slightly less but almost equivalent to that in post-contrast imaging. In this study, post-contrast imaging was considered as the gold standard. It was proven that conventional sequences like T2WI showed a sensitivity of 85.71% and specificity of 100% whereas 3D FIESTA sequence showed 100% sensitivity and specificity in assessing the CPA tumour extent and cranial nerve involvement. Conclusion: 3D FIESTA imaging is a sensitive technique for the diagnosis of retrocochlear and CPA lesions without contrast administration. 3D FIESTA imaging can be considered as a useful screening tool for patients presenting with inner ear symptoms.

Introduction: A subgingivally extending proximal carious defect can be adequately restored with an adhesive indirect aesthetic restoration only when the proximal margins can be raised coronal to the Cementoenamel Junction (CEJ) using various Margin Elevation Materials (MEM). This approach is commonly referred to as the Proximal Box Elevation (PBE) technique. However, there is a lack of evidence in the literature regarding the quality of the marginal seal at the root dentine- MEM interface when using PBE technique. Aim: The purpose of this systematic review was to summarise the available evidence on the the quality of the marginal seal at the root dentine- MEM when using the PBE technique during the placement of adhesive indirect aesthetic restorations. Materials and Methods: Comprehensive electronic search was performed in PubMed Central, Cochrane Library and Google Scholar from their inception to January 2021 at the Department of Conservative Dentistry and Endodontics, Sri Ramachandra Institute of Higher Education and Research (SRIHER) between May 2020 to January 2021. Full text articles published in english language which have performed an indirect restoration with and without PBE technique were included in the review. A total of 249 articles were screened initially in the review. The main keywords used were: “PBE”, “Cervical Margin Relocation (CMR)”, “Deep Margin Elevation (DME)” and “coronal margin relocation”. The review protocol followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The risk of bias assessment of the included studies was performed independently. Results: A total of 9 in-vitro studies were included in the present systematic review. Resin based composites of different viscosities seemed to perform adequately as MEM. It may be prudent to perform the margin elevation in multiple layers. Self etch adhesives had a better marginal seal than the total etch adhesives. Regarding the effect of indirect restoration on the marginal seal at the root dentine- MEM interface, there was no conclusive evidence. PBE may provide a congenial environment for predictable and efficient bonding of indirect adhesive aesthetic restorations. Conclusion: Based on the moderate quality of evidence from the included articles in this systematic review, it can be concluded that while performing a PBE technique, the quality of marginal seal at root dentine- MEM interface is satisfactory under in-vitro conditions.

Introduction: People who use drugs in community settings are at risk of a fatal overdose, which can be mitigated by naloxone administered via bystanders. In this study we sought to investigate methods of estimating and tracking opioid overdose reversals by community members with take-home naloxone (THN) to coalesce possible ways of characterizing THN reach with a metric that is useful for guiding both distribution of naloxone and advocacy of its benefits.Methods: We conducted a scoping review of published literature on PubMed on August 15, 2022, using PRISMA-ScR protocol, for articles discussing methods to estimate THN reversals in the community. The following search terms were used: naloxone AND (“take home” OR kit OR “community distribution” OR “naloxone distribution”). We used backwards citation searching to potentially find additional studies. Overdose education and naloxone distribution program-based studies that analyzed only single programs were excluded.Results: The database search captured 614 studies, of which 14 studies were relevant. Backwards citation searching of 765 references did not reveal additional relevant studies. Of the 14 relevant studies, 11 were mathematical models. Ten used Markov models, and one used a system dynamics model. Of the remaining three articles, one was a meta-analysis, and two used spatial analysis. Studies ranged in year of publication from 2013–2022 with mathematical modeling increasing in use over time. Only spatial analysis was used with a focus on characterizing local naloxone use at the level of a specific city.Conclusion: Of existing methods to estimate bystander administration of THN, mathematical models are most common, particularly Markov models. System dynamics modeling, meta-analysis, and spatial analysis have also been used. All methods are heavily dependent upon overdose education and naloxone distribution program data published in the literature or available as ongoing surveillance data. Overall, there is a paucity of literature describing methods of estimation and even fewer with methods applied to a local focus that would allow for more targeted distribution of naloxone.

Chronic low back pain (cLBP) is the most prevalent chronic pain condition. There are no treatments that haven been found to directly assuage evoked cLBP. To this extent, mindfulness-meditation is a promising pain therapy. Yet, it is unclear if meditation can be utilized to directly attenuate evoked chronic pain through endogenous opioids. A double-blind, randomized, and placebo-controlled clinical trial with a drug crossover design examined if mindfulness-meditation, as compared to sham mindfulness-meditation, attenuated straight leg-raise test evoked chronic pain during intravenous (0.15 mg/kg bolus + 0.15 mg/kg/hour maintenance) naloxone (opioid antagonist) and placebo-saline infusion. Fifty-nine individuals with cLBP (mean age = 46 years; 30 females) completed all study procedures. After the pre-intervention pain testing session, patients were randomized to a four-session (20-min/session) mindfulness (n = 30) or sham mindfulness-meditation (n = 29) intervention. After the interventions, mindfulness and sham mindfulness-meditation were associated with significant reductions in back pain during saline and naloxone infusion when compared to rest (non-meditation) in response to the cLBP-evoking straight leg-raise test. These results indicate that meditation directly reduces evoked chronic pain through non-opioidergic processes. Importantly, after the interventions, the mindfulness group reported significantly lower straight leg-raise induced pain than the sham mindfulness-meditation group during rest (non-meditation) and meditation. Mindfulness and sham mindfulness-meditation training was also associated with significantly lower Brief Pain Inventory severity and interference scores. The pain-relieving effects of mindfulness meditation were more pronounced than a robust sham-mindfulness meditation intervention, suggesting that non-reactive appraisal processes may be uniquely associated with improvements in chronic low-back pain.Trial Registration: ClinicalTrials.gov identifier: NCT04034004.

Diabetes mellitus is the most common metabolic disease causing microvascular, macrovascular, neuropathic, and nephropathic complications. Diabetic foot ulcer (DFU) is the frequently occurring clinical complication due to associated neuropathy, peripheral arterial disease leading to threatened amputation if untreated. Various treatments and preventive management techniques are currently in practice to lessen the morbidity of these complications. Homeopathy is proven to be the successful treatment in chronic diseases, especially DFUs as evidenced by the previous case reports and literature. The document includes a case report where a long-standing diabetic foot ulcer (DFU) was completely healed over a few months using the homeopathic remedy Lachesis. This treatment not only healed the ulcer but also prevented further complications and the need for toe amputation, leading to an overall improvement in health as evaluated by the modified Naranjo criteria.

Abstract Tuberculosis (TB) remains a major global health concern, with drug-resistant strains posing a significant challenge to effective treatment. Bacteriophage (phage) therapy has emerged as a potential alternative to combat antibiotic resistance. In this study, we investigated the efficacy of widely used mycobacteriophages (D29, TM4, DS6A) against Mycobacterium tuberculosis (M. tuberculosis) under pathophysiological conditions associated with TB, such as low pH and hypoxia. We found that even at low multiplicity of infection (MOI), mycobacteriophages effectively infected M. tuberculosis, got rapidly amplified, and lysed M. tuberculosis, demonstrating their potential as therapeutic agents. Furthermore, we observed a novel phage tolerance mechanism with bacteria forming aggregates after several days of phage treatment. These aggregates were enriched with biofilm components and metabolically active bacteria. However, no phage tolerance was observed upon treatment with the three-phage mixture, highlighting the dynamic interplay between phages and bacteria and emphasizing the importance of phage cocktails. We also observed that phages were effective in lysing bacteria even under low pH and low oxygen concentrations as well as antibiotic-resistant bacteria. Our results provide key insights into phage infection of slow-growing bacteria and suggest that mycobacteriophages can effectively eliminate M. tuberculosis in complex pathophysiological environments like hypoxia and acidic pH. These results can aid in developing targeted phage-based therapies to combat antibiotic-resistant mycobacterial infections.

Abstract A recent global health crisis, COVID-19 is a significant global health crisis that has profoundly affected lifestyles. The detection of such diseases from similar thoracic anomalies using medical images is a challenging task. Thus, the requirement of an end-to-end automated system is vastly necessary in clinical treatments. In this way, the work proposes a Squeeze-and-Excitation Attention-based ResNet50 (SEA-ResNet50) model for detecting COVID-19 utilizing chest X-ray data. Here, the idea lies in improving the residual units of ResNet50 using the squeeze-and-excitation attention mechanism. For further enhancement, the Ranger optimizer and adaptive Mish activation function are employed to improve the feature learning of the SEA-ResNet50 model. For evaluation, two publicly available COVID-19 radiographic datasets are utilized. The chest X-ray input images are augmented during experimentation for robust evaluation against four output classes namely normal, pneumonia, lung opacity, and COVID-19. Then a comparative study is done for the SEA-ResNet50 model against VGG-16, Xception, ResNet18, ResNet50, and DenseNet121 architectures. The proposed framework of SEA-ResNet50 together with the Ranger optimizer and adaptive Mish activation provided maximum classification accuracies of 98.38% (multiclass) and 99.29% (binary classification) as compared with the existing CNN architectures. The proposed method achieved the highest Kappa validation scores of 0.975 (multiclass) and 0.98 (binary classification) over others. Furthermore, the visualization of the saliency maps of the abnormal regions is represented using the explainable artificial intelligence (XAI) model, thereby enhancing interpretability in disease diagnosis.

Abstract Breast cancer has become the leading cause of cancer mortality among women worldwide. The timely diagnosis of such cancer is always in demand among researchers. This research pours light on improving the design of computer-aided detection (CAD) for earlier breast cancer classification. Meanwhile, the design of CAD tools using deep learning is becoming popular and robust in biomedical classification systems. However, deep learning gives inadequate performance when used for multilabel classification problems, especially if the dataset has an uneven distribution of output targets. And this problem is prevalent in publicly available breast cancer datasets. To overcome this, the paper integrates the learning and discrimination ability of multiple convolution neural networks such as VGG16, VGG19, ResNet50, and DenseNet121 architectures for breast cancer classification. Accordingly, the approach of fusion of hybrid deep features (FHDF) is proposed to capture more potential information and attain improved classification performance. This way, the research utilizes digital mammogram images for earlier breast tumor detection. The proposed approach is evaluated on three public breast cancer datasets: mammographic image analysis society (MIAS), curated breast imaging subset of digital database for screening mammography (CBIS-DDSM), and INbreast databases. The attained results are then compared with base convolutional neural networks (CNN) architectures and the late fusion approach. For MIAS, CBIS-DDSM, and INbreast datasets, the proposed FHDF approach provides maximum performance of 98.706%, 97.734%, and 98.834% of accuracy in classifying three classes of breast cancer severities.

Abstract Studies on the neural correlates of navigation in 3D environments are plagued by several issues that need to be solved. For example, experimental studies show markedly different place cell responses in rats and bats, both navigating in 3D environments. In this study, we focus on modelling the spatial cells in rodents in a 3D environment. We propose a deep autoencoder network to model the place and grid cells in a simulated agent navigating in a 3D environment. The input layer to the autoencoder network model is the HD layer, which encodes the agent’s HD in terms of azimuth (θ) and pitch angles (ϕ). The output of this layer is given as input to the Path Integration (PI) layer, which computes displacement in all the preferred directions. The bottleneck layer of the autoencoder model encodes the spatial cell-like responses. Both grid cell and place cell-like responses are observed. The proposed model is verified using two experimental studies with two 3D environments. This model paves the way for a holistic approach using deep neural networks to model spatial cells in 3D navigation.

Bitterness in citrus juice is a common quality issue in the fruit-based beverage industry. Naringin, the dominant flavonoid responsible for bitterness in citrus fruit juice, can be mitigated by the enzyme naringinase. This study focuses on the isolation of novel naringinase-producing bacterium from mandarin orange peel, followed by the production and partial purification of naringinase. The bacterial strain exhibiting the highest enzyme production potential was identified as Bacillus megaterium AULS 1. The effects of carbon source (naringin) concentration, inducer (naringenin), pH, and temperature on naringinase production were investigated. Optimized conditions yielded a maximum naringinase production of 386.43 U mL–1. The produced enzyme was partially purified using ammonium sulfate precipitation and dialysis. The effects of pH, temperature, and metal ion concentration on the activity of the partially purified enzyme were evaluated, resulting in a highest activity of naringinase of 539.25 U mL–1 under optimized conditions. The debittering activity of the partially purified enzyme was 45.78 %. These findings demonstrate that Bacillus megaterium AULS 1 has the potential for naringinase production, and can be utilized for debittering citrus juices.

Abstract Predictors of rebound after correction of coronal plane deformities using temporary hemiepiphysiodesis (TH) are not well defined. The following research questions were tested: (1) Is the dynamic knee joint load useful to improve rebound prediction accuracy? (2) Does a large initial deformity play a critical role in rebound development? (3) Are BMI and a young age risk factors for rebound? Fifty children and adolescents with idiopathic knee valgus malalignment were included. A deviation of the mechanical femorotibial angle (MFA) of ≥ 3° into valgus between explantation and the one-year follow-up period was chosen to classify a rebound. A rebound was detected in 22 of the 50 patients (44%). Two predictors of rebound were identified: 1. reduced peak lateral knee joint contact force in the first half of the stance phase at the time of explantation (72.7% prediction); 2. minor initial deformity according to the MFA (70.5% prediction). The best prediction (75%) was obtained by including both parameters in the binary logistic regression method. A TH should not be advised in patients with a minor initial deformity of the leg axis. Dynamic knee joint loading using gait analysis and musculoskeletal modeling can be used to determine the optimum time to remove the plates.

H syndrome is a rare autosomal recessive genodermatosis characterized by cutaneous manifestations and other systemic involvement. It presents with features like hyperpigmentation, hypertrichosis, hepatosplenomegaly, hearing loss, hallux valgus, hyperglycemia, low height, hypogonadism, and heart anomalies. Without a high index of suspicion, it is often misdiagnosed with other sclerodermatous cutaneous disorders. This case report highlights the clinical variability of H syndrome. An 8-year-old female child presented with hyperpigmented, hypertrichotic indurated patches with hyperglycemia and short stature. Histopathology showed dense lymphohistiocytic infiltrate and fibrosis in the dermis and subcutaneous tissue. The patient lacked features like heart anomalies, hearing loss, and hallux valgus, suggesting forme fruste presentation of H syndrome. An overlap between features of H syndrome and PHID syndrome is also suspected because of atypical presentation.

Filariasis is a very important public health problem endemic in tropical and subtropical countries. It is a debilitating disease with significant socioeconomic burden. In 2016, lymphatic filariasis was successfully eliminated from the Maldives, which was certified by WHO. We report a case of filariasis detected by a rapid antigen test while undergoing a screening program conducted by Health Protection Agency, Maldives in December 2023. The patient was asymptomatic and showed no peripheral blood eosinophilia. Conventional blood smears were negative. Nevertheless, buffy coat smear served as a promising test in detection of parasite, thus confirming the re-emergence of the disease in the country. Therefore, our study highlight the significance of buffy coat smears in detection of hemoparasites. We also focus on the importance of surveillance system in order to sustain the disease elimination goal that was already achieved.

Abstract African American (AA) women are twice as likely to develop triple-negative breast cancer (TNBC) as women of European descent. Additionally, AA women with TNBC present a much more aggressive disease course than their European American (EA) counterparts. Thus, there is an unmet clinical need to identify race-specific biomarkers and improve survival outcomes in AA patients with TNBC. The minus-end directed microtubule motor protein kinesin family member C1 (KIFC1) promotes centrosome clustering and chromosomal instability and is often overexpressed in TNBC. Previous findings suggest that KIFC1 plays a role in cell proliferation and migration in TNBC cells from AAs and that the levels of nuclear KIFC1 (nKIFC1) are particularly high in AA patients with TNBC. The nuclear localization of KIFC1 in interphase may underlie its previously unrecognized race-specific association. In this study, we found that in TNBC cells derived from AAs, nKIFC1 interacted with the tumor suppressor myosin heavy chain 9 (MYH9) over EA cells. Treatment of AA TNBC cells with commercial inhibitors of KIFC1 and MYH9 disrupted the interaction between KIFC1 and MYH9. To characterize the racial differences in the KIFC1-MYH9-MYC axis in TNBC, we established homozygous KIFC1 knockout (KO) TNBC cell lines. KIFC1 KO significantly inhibited proliferation, migration, and invasion in AA TNBC cells but not in EA TNBC cells. RNA sequencing analysis showed significant downregulation of genes involved in cell migration, invasion, and metastasis upon KIFC1 KO in TNBC cell lines from AAs compared to those from EAs. These data indicate that mechanistically, the role of nKIFC1 in driving TNBC progression and metastasis is stronger in AA patients than in EA patients, and that KIFC1 may be a critical therapeutic target for AA patients with TNBC. The study revealed that major factors influencing the social media's effectiveness in digital communication are Content, Customer Interaction and Social Media metrics. As a result of comprehensive analysis, this research paper not only provides valuable insights into the current state of social media's effective-ness in digital communication but also offers practical recommendations for businesses looking to optimize their social media strategies. In an era where data reigns supreme, this research findings serve as a guide for harnessing the power of social media as a dynamic and influential tool in the digital communication arsenal.
