PurposeTo investigate the characteristics and rate of central visual field loss after optic disc hemorrhage (DH).DesignProspective cohort study.MethodsThree hundred forty-three eyes of 220 subjects who had ≥3 years of follow-up with a minimum of 5 visits with 10-2 and 24-2 visual field (VF) were recruited. Rates of 10-2 mean deviation (MD) loss in each hemifield and predefined zones were compared using linear mixed-effects models in DH and non-DH eyes. Clustered pointwise regression analysis was also used to define central VF progressors and compared with 24-2 VF loss using guided progression analysis.ResultsThirty-nine eyes with DH and 304 eyes without DH had a mean follow-up of 5.2 years. Eyes with DH had rates of 10-2 MD loss that were 3 times faster than non-DH eyes (mean difference -0.36 dB/year [95% confidence interval 0.54-0.18]; P < .001) and were 3.7 times more likely to progress (P=.002). A larger proportion of glaucomatous eyes showed central VF progression rather than peripheral VF progression in the DH group (30.8% vs. 20.5%) compared with the non-DH group (10.9% vs. 9.2%). In early glaucoma, the rate of 10-2 MD loss was 5.5 times faster in DH eyes than in non-DH eyes (P < .001). Superonasal and superotemporal central VF regions progressed more rapidly than other regions, especially in DH eyes.ConclusionCentral VF loss is accelerated in glaucoma eyes with DH and it corresponds topographically to the DH location. In patients with glaucoma with DH, one should consider supplementing 10-2 VFs with 24-2 VFS to monitor the disease.

Central visual field (VF) progression could directly threaten patientss visual function compared to glaucomatous damage. This study was designed to investigate visual field (VF) progression pattern and associated risk factors including optical coherence topography angiographic (OCT-A) findings in glaucoma patients with initial paracentral scotoma. This prospective, observational study included 122 eyes presenting as initial paracentral scotomas with serial 24-2 and 10-2 VF tests at the glaucoma clinic of Seoul St Mary's Hospital between November 2017 and August 2020. The participants underwent at least 5 serial VF exams and OCT-A at baseline. Numerical values of the initial and final 10-2 VF tests were averaged for each VF test point using the total deviation map. Innermost 10-2 VF progression was defined as three or more new contiguous points at the central 12 points on 10-2 VF. Other clinical characteristics were collected including history of disc hemorrhage and vessel density (VD) was measured from OCT-A images. Linear regression analysis was performed to obtain the change of mean deviation and a cut-off for progression was defined for both 24-2 and 10-2 VFs. The average total deviation maps of the initial 10-2 VF tests shows initial paracentral scotoma located in the superior region in an arcuate pattern that was deep in the 4°–6° region above fixation. This arcuate pattern was more broadly located in the 4°–10° region in the primary open-angle glaucoma (POAG) group, while it was closer to fixation in 0°–4° region in the normal-tension glaucoma (NTG) group. The final average map shows deepening of scotomas in the 4°–10° region in POAG, which deepened closer to the region of fixation in NTG. The diagnosis of NTG (β 1.892; 95% CI 1.225–2.516; P = 0.035) and lower choroidal VD in the peripapillary atrophy (PPA) region (β 0.985; 95% CI 0.975 to 0.995; P = 0.022) were significantly related to innermost 10-2 VF progression. Initial paracentral scotomas in NTG tended to progress closer to the region of fixation, which should be monitored closely. Important progression risk factors related to paracentral scotoma near the fixation were the diagnosis of NTG and reduced choroidal VD in the β-zone PPA region using OCT-A. We should consider vascular risk factors in NTG patients presenting with initial paracentral scotoma to avoid vision threatening progression of glaucoma.

Purpose: To evaluate the effectiveness of a new modification of the vacuum-perimeter test (VPT) by V.V. Volkov in the early diagnosis of glaucoma in comparison with the results of the original test and its known modification — vacuum-compression automated test (VCAT) in an open comparative clinical research. Patients and Methods. The study involved 26 patients (47 eyes) (average age 52.9 ± 8.5 years) with suspected open-angle glaucoma (OAG). Patients were divided into three groups: 1-st — 19 eyes with preglaucoma, 2-nd — 18 eyes with the early stage of OAG, 3-rd — 10 eyes of five patients with unconfirmed glaucoma. The control group (4-th) included 20 eyes of 10 healthy people (average age 56.4 ± 4.4 years). Along with the standard ophthalmological examination, all patients underwent perimetry on Humphrey Visual Field Analyzer II 745i (Germany-USA), our modification of Frequency Doubling Technology (FDT) perimetry, evaluation of the optic nerve head (ONH) on Heidelberg Retina Tomograph (HRT 3, Germany) and three stress tests: VPT, VCAT and our modification — vacuum-contrast-frequency test (VCFT). VCFT was created on the base of FDT perimetry with the aim of increasing the sensitivity of the original test (VPT) for detection of preglaucoma through the use of non-standard stimulus and increasing the number of investigated points of central visual field. Results. According to the National classification (1975) the diagnosis of preglaucoma was determined on the basis of a positive result of at least one of three stress tests, taking into account the risk factors for the development of glaucoma in patients. The early stage of OAG was established on the base of International standards for structural and functional assessment of the ONH (2003). The specificity of all three compared stress tests was 100 %, which confirmed their negative results in all patients (10 eyes) from the 3-rd group. However, the sensitivity of VCFT (75 %) was significantly higher than VPT (21.05 %) and VCAT (21.05 %), which is due, we believe, to the specific nature of VCFT stimulus. Conclusion. According to the obtained data, the developed new modification of VPT — VCFT on the specificity of the results was not worse, and the sensitivity was better than the original VPT and VCAT in the diagnosis of preglaucoma. VCFT is quickly performed and comfortable for patients.

This study assessed axial length and choroidal thickness changes following short-term peripheral myopic defocus in normal adult subjects. Twenty subjects underwent defocus sessions by viewing a full-field projected movie 4 m away for 4 h in the morning, while wearing spectacle lenses, corrected for distance vision in both eyes. The right eye, serving as the test eye, was peripherally defocused using a Fresnel lens overlay of + 3.50 D with a central clear aperture of 11.5 mm (correlating to a clear central visual field of approximately 23°), while the left eye served as the control (with no Fresnel lens overlay). A subset of 10 subjects from the same cohort also underwent additional defocus sessions with + 5.00 D of peripheral defocus. Axial length was measured and radial sub-foveal choroidal scans were obtained before and after the defocus sessions. PurposeTo examine whether glaucomatous central visual field abnormalities can be more effectively detected using a qualitative, expert evaluation of the 10-2 test compared with the topographically corresponding central 12 locations of the 24-2 test (C24-2).DesignCross-sectional study.MethodsEyes with a glaucomatous optic nerve appearance or ocular hypertension (n=523) and healthy eyes (n=107) were included as cases and control subjects, respectively. The 10-2 and C24-2 visual field results of all eyes were graded by 4 glaucoma specialists for the probability that central visual field abnormalities were present.ResultsThe sensitivity of the 10-2 and C24-2 tests for detecting the cases at 95% specificity were not significantly different (e.g., 32.2% and 31.4%, respectively, for grader 1, P=.87; all graders P ≥ .25). At 95% specificity, the pattern standard deviation values from these tests had a similar sensitivity to the qualitative evaluation for the C24-2 test for all graders (P ≥ .083), but it had a significantly higher sensitivity than the qualitative evaluation for the 10-2 test for 3 graders (P ≤ .016).ConclusionsThe similarity in performance of the 10-2 and C24-2 test suggests that the increased sampling density of the former does not significantly improve the detection of central visual field abnormalities, even when based on expert assessment. These findings should not be taken to mean that the 10-2 test is not useful, but it underscores the need for its utility to be clearly established before incorporating it as routine glaucoma standard of care.

Background Existing evidence suggests that visual field defect in eyes with glaucoma significantly varies between individuals. The following study compared the central visual field defects with the peripheral visual field defects in patients with suspect glaucoma and primary open-angle glaucoma (POAG) and investigated whether using the central visual field test alone could result in loss of clinically valuable information. Methods In this prospective observational study, 167 eyes from 89 patients with suspect glaucoma or POAG were first examined with static automated perimetry (SAP), followed by a peripheral visual field test on Octopus 900 perimeter (Haag-Streit, Koeniz, Switzerland). The peripheral visual field test was performed by “Auto Kinetic Perimetry” program, in which Goldmann III4e stimuli randomly moved along 16 vectors at a constant angular velocity of 5 deg/s. Results Glaucomatous peripheral visual field defects were seen in 18% of the eyes with a normal central visual field. In addition, 86% of glaucoma patients with moderate-to-severe central visual field defects had corresponding peripheral visual field defects in the form of localized or diffuse depression of the isopters. Furthermore, a moderate correlation was found between the central and peripheral visual fields. The median test duration was 71 s for the peripheral test and 803 s for the central test (p Conclusions Our study demonstrated the diversity of glaucomatous visual field defects, as well as the possibility of losing the clinically valuable information due to focusing on the central visual field test alone. The peripheral kinetic perimetry is clinically feasible to complement the central static perimetry for a comprehensive assessment of visual function in glaucoma patients.

The discovery of melanopsin as a third type of retinal photoreceptor, contributing to both perceptual vision and reflex light responses, represents a new opportunity to optimise the design of artificial light sources for practical applications and to generate experimental stimuli. In the case of emissive displays, multiprimary designs incorporating a cyan primary could be used to allow melanopic radiance to be controlled independent of colour and luminance. Here we explore the performance a five-primary (violet, cyan, green, yellow, red) display device and find an anomaly in colour appearance when the cyan primary is employed. The anomaly took the form of a reddish/pinkish tinge in the central visual field, consistent with descriptions of Maxwell’s spot. This effect was apparent in some full colour images and in uniform discs over a range of chromaticities. Its appearance in coloured discs correlated with differences in calculated colour coordinate between central and peripheral vision. A simulation indicated that inclusion of any primary with predominant output in the 470–500 nm range has the potential to produce such a discrepancy in central vs peripheral appearance. Applying an additional constraint in colour processing to reproduce naturally occurring differences in central vs peripheral colour coordinate eliminated appearance of the spot and produced acceptable colour images.

People’s confusion in interpreting the gender of the toilet sign has caused a hesitation to the people in determining the toilet’s cubicle. In this study, the toilet’s pictorial sign were examined and depicted as stylized forms of men and women. The aim of this research is to collect the data of the people’s eye gaze movement in order to identify the toilet’s sign system. Other related data were also collected through literature to obtain the theories that support the study. Three volunteers were participated in this study. In the first experiment, they had to identify 6 signs with different shapes presented at the same location with their central visual field using an “Eye-tracking” device without being told in advance. In the second experiment, they had to mention the type of gender in the three pairs of 6 toilet signs displayed. The result of the study indicated a difference between the pattern of human’s eyes movements in observing the sign unconsciously and the zoning at the middle represented as the “body” was the most viewed part. This research is expected to provide informations and references for academics, designers and governments in order to create sign system and universal design services.

Purpose To evaluate the central visual field by microperimetry (MP), in early glaucoma. Methods Consecutive perimetrically experienced patients with a single nasal step or arcuate scotoma and 14 control eyes underwent MP. Retinal sensitivity on MP was mapped for frequency and depth of loss in the central 10° around fixation. Results Twenty-one eyes had a single nasal step and 19 eyes with single arcuate scotoma on standard automated perimetry (SAP), with central 10° being normal on 30-2 and 10-2 perimetry. The average mean sensitivity on MP, in glaucomatous and control eyes was 11.8 ± 3.9 dB and 16.6 ± 1.2 dB, respectively, P = 0.0004. The average mean defect on MP-1, in glaucomatous and control eyes was -6.5 ± 2.0 dB and -3.0 ± 1.2 Db, respectively, P = 0.05. The corresponding retinal hemisphere showed significant defects in MP. In eyes with single nasal steps, an absolute scotoma was seen in 14-28% of eyes 8-10° off fixation, moderate to mild defects were seen in 10-52% eyes, and 10% eyes showed involvement up to 4° from the fixation. Eyes with arcuate scotoma had an absolute scotoma on MP in 95% of eyes, 6-10° from fixation, with extension up to 2° from fixation in 21%. In glaucomatous eyes, the normal hemisphere on SAP showed a mild defect on MP in 43%. Control eyes did not show any defect in SAP or MP. Conclusion A significant loss of central retinal sensitivity is recorded on MP in early glaucomatous neuropathy as compared to SAP. Paramacular absolute defects were seen at 6-10° from fixation.

Purpose Damage of retinal representation of the visual field affects its local features and the spared, unaffected parts. Measurements of visual deficiencies in ophthalmological patients are separated for central (shape) or peripheral (motion and space perception) properties, and acuity tasks rely on stationary stimuli. We explored the benefit of measuring shape and motion perception simultaneously using a new motion-based acuity task. Methods Eight healthy control subjects, three patients with retinitis pigmentosa (RP; tunnel vision), and 2 patients with Stargardt disease (STGD) juvenile macular degeneration were included. To model the peripheral loss, we narrowed the visual field in controls to 10 degrees. Negative and positive contrast of motion signals were tested in random-dot kinematograms (RDKs), where shapes were separated from the background by the motion of dots based on coherence, direction, or velocity. The task was to distinguish a circle from an ellipse. The difficulty of the task increased as ellipse became more circular until reaching the acuity limit. Results High velocity, negative contrast was more difficult for all, and for patients with STGD, it was too difficult to participate. A slower velocity improved acuity for all participants. Conclusions Proposed acuity testing not only allows for the full assessment of vision but also advances the capability of standard testing with the potential to detect spare visual functions. Translational Relevance The motion-based acuity task might be a practical tool for assessing vision loss and revealing undetected, undamaged, or strengthened properties of the injured visual system by standard testing, as suggested here for two patients with STGD and three patients with RP.

To compare the ability of 24-2C and 10-2 test grids in measuring visual field global indices, identifying central visual field defects, and facilitating macular structure-function analysis with OCT scans in glaucoma and glaucoma suspect patients.Prospective, cross-sectional study.One eye from 131 glaucoma and 57 glaucoma suspect patients recruited from a referral-only, university-based glaucoma clinic.Each subject underwent perimetric testing using 24-2C SITA-Faster and 10-2 SITA-Fast in random order, and Cirrus OCT macular imaging (Ganglion Cell Analysis) for structure-function correlations.Visual field global indices (mean deviation, pattern standard deviation, binarized "cluster" pass/fail, and central mean sensitivity), number and proportion of visual field defects, and structure-function concordance with the Cirrus OCT deviation map following visual field location displacement for correspondence with underlying retinal ganglion cell position.Global indices (mean deviation, pattern standard deviation, and central mean sensitivity) were similar between both grids. The 10-2 detected more defects compared with the 24-2C (P0.0001 for all patients, P = 0.006 for glaucoma patients). This was preserved when analyzing the proportion of defects in the central visual field for all patients (P = 0.02) but was not significantly different for glaucoma patients (P = 0.051). The 10-2 identified more central "clusters" of 2+ contiguous points of deficit (P0.0001). Structure-function comparisons performed at locations where visual field and OCT test locations were colocalized revealed greater concordance of structural and functional deficits using the 10-2 (P0.0001). The 10-2 took a median of 201 seconds, and the 24-2C took a median of 154 seconds, corresponding to the different thresholding algorithms.The 24-2C and 10-2 test grids return similar global indices of visual field performance and proportionally similar amounts of central visual field loss. The additional points in the 10-2 grid return more "clusters" of defects and a greater rate of structure-function concordance compared with the 24-2C test grid. Thus, the 24-2C can identify the presence of a clustered central visual field defect using similar probability criteria, whereas the 10-2 may be more useful in comprehensively characterizing the defect and predicting central visual function.

Context dependent top-down modulation in visual processing has been a topic of wide interest. Recent findings on context dependent modulation, combined with the tools available to investigate network mechanisms in the mouse, make the mouse primary visual cortex an ideal system to investigate context-dependent modulation. However, the distribution of inputs to V1 from across the brain is still relatively unknown. In this study, we investigate inputs to V1 by injecting cholera toxin B subunit (CTB), a retrograde tracer, across the extent of V1. To identify CTB labelled cell bodies and quantify their distribution across various brain regions, we developed a software pipeline that maps each labelled cell body to its corresponding brain region. We found over fourteen brain regions that provided inputs to V1. Higher visual areas (HVAs) provided the most inputs to V1, followed by the retrosplenial, cingulate, and other sensory cortices. As our injections spanned a range of coordinates along the mediolateral axis of V1, we asked if there was any topographic organisation of inputs to V1: do particular areas project preferentially to specific regions of V1. Based on the distribution of inputs from different HVAs, injection sites broadly clustered into two groups, consistent with a retinotopic separation into sites within the central visual field and the peripheral visual field. Furthermore, the number of cells detected in HVAs was correlated to the azimuthal retinotopic location of each injection site. This topographic organization of feedback projections along the medio-lateral axis of V1 suggests that V1 cells representing peripheral vs central visual fields are differentially modulated by HVAs, which may have an ethological relevance for a navigating animal.

PURPOSE: To investigate central visual field (VF) defects among four phenotypes of glaucomatous optic discs. DESIGN: Cross-sectional study. METHODS: Optic disc phenotypes were determined in eyes with definite or suspected glaucoma that had a 24–2 VF with mean deviation (MD) better than −12 dB and a 10–2 VF. 10–2 VFs were classified as abnormal based on a cluster criterion. Additionally, the average of the total deviation values at each 10–2 test point was compared by optic disc phenotype. RESULTS: Four glaucomatous optic disc phenotypes were identified in 448 eyes of 309 patients: focal ischemic (FI)(n=121), generalized cup enlargement (GE)(n=109), myopic glaucoma (MY)(n=66), and senile sclerotic (SS)(n=152). Background Glaucoma is a multifactorial chronic optic neuropathy characterized by typical optic nerve morphological changes and alterations of the visual field (VF). Delayed detection and treatment of glaucoma might result in irreversible blindness. Structural damage precedes the functional damage. Objective The objective of this study was to correlate between macular thickness (MT) affection and central VF changes in patients with primary open-angle glaucoma (POAG). Patients and methods This is a prospective cross-sectional observational study that included 51 eyes diagnosed with POAG. They were evaluated by spectral domain optical coherence tomography, which was compared with several VF parameters, including mean deviation and loss variance. Results A significant correlation was found between VF parameters and decrease in MT (mean deviation: r=−0.506; P=0.000, loss variance: r=−0.492; P=0.000). BACKGROUND/OBJECTIVES: To identify risk factors for glaucoma-related central visual field (VF) deterioration after vitrectomy with internal limiting membrane (ILM) peeling for epiretinal membrane (ERM). SUBJECTS/METHODS: A prospective cohort study consisting of cases with or without glaucoma (33 eyes of 33 patients in each group) who underwent vitrectomy with ILM peeling for ERM. Humphrey 10–2 VFs and ganglion cell complex (GCC) thickness were measured at baseline and about 3, 6, and 12 months postoperatively. Longitudinal changes in VF indices and factors associated with their postoperative changes were investigated using mixed-effects models, as was sectorwise total deviation (TD) analysis using six sectors consisting of outer/inner arcuate and cecocentral sectors in each hemifield. Central visual field (CVF) loss is important in maintaining vision-related quality of life in eyes with open-angle glaucoma (OAG). The present study investigated whether nasalized location of central retinal vessel trunk (CRVT) at baseline is associated with rapid rate of CVF loss in early-stage OAG eyes. This study included 76 OAG eyes with high nasalization CRVT [HNL] group and 75 OAG eyes with low nasalization CRVT [LNL] group matched for glaucoma severity at baseline that showed progressive visual field (VF) loss. The rates of mean threshold changes at various regions were compared in the two groups using a linear mixed model. Clinical variables associated with rapid rate of CVF progression were also identified using a linear mixed model. Purpose A method of evaluating central visual field (VF) progression in eyes with retinitis pigmentosa (RP) has still to be established. We previously reported the potential merit of applying a binomial test to pointwise linear regression (binomial PLR) in glaucoma progression. In the current study, we investigated the usefulness of binomial PLR in eyes with RP. Methods A series of 10 VFs (VF 1–10, Humphrey field analyzer, 10-2 test) from 196 eyes of 103 patients with RP were collected retrospectively. The PLR was performed by regressing the total deviation of all test points with the complete series of 10 VFs. The accuracy (positive predictive value, negative predictive value, and false-positive rate) and the time required to detect VF progression with shorter VF series (from VF 1–5 to VF 1–9) were compared across the binomial PLR, a permutation analysis of PLR (PoPLR), and a mean deviation (MD) trend analysis. Results In evaluating VF progression, the binomial PLR was comparable with the PoPLR and MD trend analyses in its positive predictive value (0.55 to 0.95), negative predictive value (0.67 to 0.92), and false-positive rate (0.01 to 0.05). The binomial PLR required significantly less time to detect VF progression (5.0 ± 2.0 years) than the PoPLR and MD trend analyses (P < 0.01, P < 0.001, respectively). Conclusions The application of a binomial PLR achieved reliable and earlier detection of central VF progression in eyes with RP. Translational Relevance A binomial PLR was useful in assessing VF progression in RP.

Sound is a vital cue in helping hearing people orient their gaze and attention towards events outside their central line of sight, especially in the far periphery, where vision is poor. Without sound cues, deaf individuals must rely on vision as an ‘early warning system’ for peripheral events, and in fact numerous behavioural studies demonstrate that deaf adults have superior visual sensitivity, particularly to far peripheral stimuli. We asked whether an increased demand on peripheral vision throughout development might be reflected in early visual brain structures. Using functional magnetic resonance imaging (fMRI), we mapped visual field representations in 16 early, profoundly deaf adults and 16 hearing age-matched controls. To target the far periphery, we used wide-field retinotopic mapping stimuli to map visual field eccentricity out to 72°, well beyond conventional mapping studies. Deaf individuals exhibited a larger representation of the far peripheral visual field in both the primary visual cortex and the lateral geniculate nucleus of the thalamus. Importantly, this was not due to a total expansion of the visual map, as there was no difference between groups in overall size of either structure, but a smaller representation of the central visual field in the deaf group, suggesting a redistribution of neural resources. Here, we demonstrate for the first time that the demands placed on vision due to lifelong hearing loss can sculpt visual maps at the first level of inputs from the retina, increasing neural resources for processing stimuli in the far peripheral visual field.

Aging and central vision loss are associated with cortical atrophies, but little is known about the relationship between cortical thinning and the underlying cellular structure. We compared the macro- and micro-structure of the cortical gray and superficial white matter of 38 patients with juvenile (JMD) or age-related (AMD) macular degeneration and 38 healthy humans (19-84 years) by multimodal MRI including diffusion-tensor imaging (DTI). A factor analysis showed that cortical thickness, tissue-dependent measures, and DTI-based measures were sensitive to distinct components of brain structure. Age-related cortical thinning and increased diffusion were observed across most of the cortex, but increased T1-weighted intensities (frontal), reduced T2-weighted intensities (occipital), and reduced anisotropy (medial) were limited to confined cortical regions. Vision loss was associated with cortical thinning and enhanced diffusion in the gray matter (less in the white matter) of the occipital central visual field representation. Moreover, AMD (but not JMD) patients showed enhanced diffusion in lateral occipito-temporal cortex and cortical thinning in the posterior cingulum. These findings demonstrate that changes in brain structure are best quantified by multimodal imaging. They further suggest that age-related brain atrophies (cortical thinning) reflect diverse micro-structural etiologies. Moreover, juvenile and age-related macular degeneration are associated with distinct patterns of micro-structural alterations.

Standard automated perimetry (SAP), the most common form of perimetry used in clinical practice, is associated with high test variability, impacting clinical decision making and efficiency. Contrast sensitivity isocontours (CSIs) may reduce test variability in SAP by identifying regions of the visual field with statistically similar patterns of change that can be analysed collectively and allow a point (disease)-to-CSI (normal) comparison in disease assessment as opposed to a point (disease)-to-point (normal) comparison. CSIs in the central visual field however have limited applicability as they have only been described using visual field test patterns with low, 6° spatial sampling. In this study, CSIs were determined within the central 20° visual field using the 10-2 test grid paradigm of the Humphrey Field Analyzer which has a high 2° sampling frequency. The number of CSIs detected in the central 20° visual field was greater than previously reported with low spatial sampling and stimulus size dependent: 6 CSIs for GI, 4 CSIs for GII and GIII, and 3 CSIs for GIV and GV. CSI number and distribution were preserved with age. Purpose To quantify the central visual field (VF) loss patterns in glaucoma using artificial intelligence. Design Retrospective study. Participants VFs of 8712 patients with 13 951 Humphrey 10-2 test results from 13 951 eyes for cross-sectional analyses, and 824 patients with at least 5 reliable 10-2 test results at 6-month intervals or more from 1191 eyes for longitudinal analyses. Methods Total deviation values were used to determine the central VF patterns using the most recent 10-2 test results. A 24-2 VF within a 3-month window of the 10-2 tests was used to stage eyes into mild, moderate, or severe functional loss using the Hodapp-Anderson-Parrish scale at baseline. Archetypal analysis was applied to determine the central VF patterns. Cross-validation was performed to determine the optimal number of patterns. Stepwise regression was applied to select the optimal feature combination of global indices, average baseline decomposition coefficients from central VFs archetypes, and other factors to predict central VF mean deviation (MD) slope based on the Bayesian information criterion (BIC). Main Outcome Measures The central VF patterns stratified by severity stage based on 24-2 test results and a model to predict the central VF MD change over time using baseline test results. Results From cross-sectional analysis, 17 distinct central VF patterns were determined for the 13 951 eyes across the spectrum of disease severity. These central VF patterns could be divided into isolated superior loss, isolated inferior loss, diffuse loss, and other loss patterns. Notably, 4 of the 5 patterns of diffuse VF loss preserved the less vulnerable inferotemporal zone, whereas they lost most of the remaining more vulnerable zone described by the Hood model. Inclusion of coefficients from central VF archetypical patterns strongly improved the prediction of central VF MD slope (BIC decrease, 35; BIC decrease of >6 indicating strong prediction improvement) than using only the global indices of 2 baseline VF results. Eyes with baseline VF results with more superonasal and inferonasal loss were more likely to show worsening MD over time. Conclusions We quantified central VF patterns in glaucoma, which were used to improve the prediction of central VF worsening compared with using only global indices.

Humans actively sample their environment with saccadic eye movements to bring relevant information into high-acuity foveal vision. Despite being lower in resolution, peripheral information is also available prior to each saccade. How pre-saccadic extrafoveal preview of a visual object influences its post-saccadic processing is still an unanswered question. Here, we investigated this question by simultaneously recording behavior and fixation-related brain potentials while human subjects made saccades to face stimuli. We manipulated the relationship between pre-saccadic “previews” and post-saccadic images to explicitly isolate the influences of the former. Subjects performed a gender discrimination task on a newly foveated face under three preview conditions: phase-scrambled face, incongruent face (different identity from the foveated face), and congruent face (same identity). As expected, reaction times were faster after a congruent-face preview compared to the phase-scrambled and incongruent conditions. Importantly, a face preview (either incongruent or congruent) resulted in a strong reduction of post-saccadic neural responses. Specifically, we analyzed the classic face-selective N170 component at occipito-temporal EEG electrodes, which was still present in our experiments with active looking. We found that this component was strongly attenuated for face preview conditions compared to scrambled conditions. This large and long-lasting decrease in evoked activity is consistent with an active prediction mechanism influencing category-specific neural processing at the start of a new fixation. These findings constrain theories of visual stability and show that the extrafoveal preview methodology can be a useful tool to investigate its underlying mechanisms.Significance StatementNeural correlates of object recognition have traditionally been studied by flashing stimuli to the central visual field. This procedure differs in fundamental ways from natural vision, where viewers actively sample the environment with eye movements and also obtain a low-resolution preview of soon-to-be-fixated objects. Here we show that the N170, a classic electrophysiological marker of the structural processing of faces, also occurs during a more natural viewing condition but is massively reduced due to extrafoveal preprocessing (preview benefit). Our results therefore highlight the importance of peripheral vision during trans-saccadic processing in building a coherent and stable representation of the world around us.

In our environment our senses are bombarded with a myriad of signals, only a subset of which is relevant for our goals. Using sub-millimeter-resolution fMRI at 7T we resolved BOLD-response and activation patterns across cortical depth in early sensory cortices to auditory, visual and audiovisual stimuli under auditory or visual attention. In visual cortices, auditory stimulation induced widespread inhibition irrespective of attention, whereas auditory relative to visual attention suppressed mainly central visual field representations. In auditory cortices, visual stimulation suppressed activations, but amplified responses to concurrent auditory stimuli, in a patchy topography. Critically, multisensory interactions in auditory cortices were stronger in deeper laminae, while attentional influences were greatest at the surface. These distinct depth-dependent profiles suggest that multisensory and attentional mechanisms regulate sensory processing via partly distinct circuitries. Our findings are crucial for understanding how the brain regulates information flow across senses to interact with our complex multisensory world.

Central visual field (CVF) loss has been linked to poorer vision-related quality of life in eyes with open-angle glaucoma (OAG) and can occur even in early-stage OAG eyes. The present study investigated whether the detection of choroidal microvasculature dropout (CMvD) during follow-up, which may be a sign of perfusion deficiency in the optic nerve head, is associated with rapid rate of CVF loss in early-stage OAG eyes. This study included 44 Korean OAG eyes with CMvD, identified by optical coherence tomography angiography, and 44 Korean OAG eyes without CMvD matched for age and severity of visual field loss at initial presentation. The rates of mean threshold changes in global and CVF were compared in eyes with and without CMvD using a linear mixed model. Clinical variables associated with rapid rate of CVF progression were also identified using a linear mixed model. Purpose. To evaluate clinically activated retinal astrocytes and Müller cells (ARAM) regarding retinal sensitivity and retinal nerve fiber layer (RNFL) thickness in primary open-angle glaucoma (POAG). Methods. Central visual field (VF; i.e., retinal sensitivity) was measured with a custom-made macular pattern by microperimetry and correlated with the presence (ARAM+) or absence (ARAM−) of ARAM on red-free fundus photography and with the corresponding RNFL by optical coherence tomography (OCT). Results. In the eyes of POAG patients, ARAM+ had overall a significantly lower retinal sensitivity (ARAM+: 7.34 dB, ARAM−: 11.9 dB; p<0.001) and lower RNFL thickness in the inferior peripapillary quadrants compared to ARAM− (RNFL superior: ARAM+ 74.2 μm, ARAM− 77.5 μm; RNFL temporal: ARAM+ 46.8 μm, ARAM− 53.0 μm, p<0.001; and RNFL inferior: ARAM+ 63.2 μm, ARAM− 73.1 μm, p<0.001). Within the same eye, ARAM+ showed a lower retinal sensitivity compared to ARAM− ([ARAM− (11.13 dB)] − [ARAM+ (9.56 dB) = 1.57 dB; p=0.25). The proportion of ARAM+ per eye correlated strongly with reduced retinal light sensitivity (p=0.02), corresponding lower peripapillary RNFL thickness (p=0.02), and lower RNFL temporal quadrant thickness (p<0.01), but not with greater age (p=0.45). Conclusion. ARAM was more frequently identified in the eyes with a lower retinal sensitivity and peripapillary RNFL thickness and may be a clinical sign in the macula for an advanced stage of POAG.

Purpose To investigate the use of ganglion cell inner plexiform layer (GC-IPL) thickness, as measured by spectral domain optical coherence tomography, to detect central visual field (VF) progression. Methods This study included 384 eyes from 384 patients (219 preperimetric and 165 perimetric glaucomatous eyes; average follow-up, 4.3 years). Photographic assessment of retinal nerve fiber layer (RNFL) and serial VF analysis were performed to detect glaucoma progression in the central (within 10°) area. Study inclusion required at least five serial spectral domain optical coherence tomography exams at different visits. The long-term test-retest variability of average GC-IPL thicknesses was calculated in 110 stable preperimetric glaucomatous eyes. The sensitivity and specificity of GC-IPL measurements for the detection of central VF progression were calculated in an event-based analysis using the calculated variability as a cut-off and were compared with those of central RNFL photographic assessment. Results The intersession test-retest variability, defined as the 95% confidence interval, was 1.76 µm for average GC-IPL thickness. The sensitivity and specificity of the average GC-IPL thickness for detecting central VF progression were 60.7% and 78.9%, respectively. Among six sectors, the inferonasal GC-IPL sector showed the highest sensitivity (53.6%). The sensitivity of the ≥1 sector GC-IPL to detect central VF progression was significantly higher than that of central RNFL photographic progression (p = 0.013). Other GC-IPL parameters showed comparable sensitivity and specificity to detect central VF progression compared with RNFL photographic progression. Conclusions Serial GC-IPL measurements show comparable performance in the detection of central glaucomatous VF progression to RNFL photographic assessment.

Population receptive field (pRF) analysis is a popular method to infer spatial selectivity of voxels in visual cortex. However, it remains largely untested how stable pRF estimates are over time. Here we measured the intersession reliability of pRF parameter estimates for the central visual field and near periphery, using a combined wedge and ring stimulus containing natural images. Sixteen healthy human participants completed two scanning sessions separated by 10–114 days. Individual participants showed very similar visual field maps for V1-V4 on both sessions. Intersession reliability for eccentricity and polar angle estimates was close to ceiling for most visual field maps (r>.8 for V1-3). PRF size and cortical magnification (CMF) estimates showed strong but lower overall intersession reliability (r≈.4–.6). Group level results for pRF size and CMF were highly similar between sessions. Additional control experiments confirmed that reliability does not depend on the carrier stimulus used and that reliability for pRF size and CMF is high for sessions acquired on the same day (r>.6). Our results demonstrate that pRF mapping is highly reliable across sessions., Highlights • We assess the intersession reliability of population receptive field (pRF) estimates. • Reliability of visual field position estimates in V1-3 is excellent (r >.8). • Reliability of pRF size and local cortical magnification factor are good (r≈.3-.6), especially when tested on the same day (r>.6). • Natural images are an effective stimulus for visual field mapping.

Purpose: Perimetry is increasingly being used to measure sensitivity at central visual field locations. For many tasks, the central (0°, 0°) location is functionally the most important, however threshold estimates at this location may be affected by masking by the nearby spatial structure of the fixation target. We investigated this effect. Methods: First we retrospectively analysed microperimetry (MAIA-2; CenterVue, Padova, Italy) data from 60 healthy subjects, tested on a custom grid with 1° central spacing. We compared sensitivity at (0°, 0°) to the mean sensitivity at the eight adjacent locations. We then prospectively tested 15 further healthy subjects on the same instrument using a cross-shaped test pattern with 1° spacing. Testing was carried out with and without the central fixation target, and sensitivity estimates at (0°, 0°) were compared. We also compared sensitivity at (0°, 0°) to the mean of the adjacent four locations in each condition. Three subjects undertook 10 repeated tests with the fixation target in place to assess within-subject variability of the effect. Results: In the retrospective analysis, central sensitivity was median 2.8 dB lower (95% range 0.1–8.8 dB lower, p < 0.0001) than the mean of the adjacent locations. In the prospective study, central sensitivity was median 2.0 dB lower with the fixation target vs without (95% range 0.4–4.7 dB lower, p = 0.0011). With the fixation target in place central sensitivity was median 2.5 dB lower than mean sensitivity of adjacent locations (95% range 0.8–4.2 dB lower, p = 0.0007), whilst without the fixation target there was no difference (mean 0.4 dB lower, S.D. 0.9 dB, p = 0.15). These differences could not be explained by reduced fixation stability. Mean within subject standard deviation in the difference between central and adjacent locations' sensitivity was 1.84 dB for the repeated tests. Conclusions: Perimetric sensitivity estimates from the central (0°, 0°) location are, on-average, reduced by 2 to 3 dB, corresponding to a 60–100% increase in stimulus luminance at threshold. This effect can be explained by masking by the nearby fixation target. The considerable within- and between-subject variability in magnitude, and the unknown effects of disease may hamper attempts to compensate threshold estimates for this effect. Clinicians should interpret central perimetric sensitivity estimates with caution, especially in patients with reduced sensitivity due to disease.

PURPOSE The purpose of this study was to characterize the extent to which central visual field (VF) loss reflects peripheral VF loss in patients with varying degrees of glaucoma severity. METHODS A total of 232 patients with glaucoma or suspect glaucoma completed static central VF testing using the 24-2 pattern and peripheral VF testing using the suprathreshold 30-60 pattern. Points from 24-2 tests were reclassified as normal/abnormal based on pattern deviation values. RESULTS Strong positive correlations (r ≥ 0.7) were observed between the proportion of abnormal central and peripheral points for the full VF, superior hemifield, and inferior hemifield, although the percentage of total central and peripheral abnormal points differed by ≥10% in 45% of eyes. In eyes with an average of 10%-40% abnormal points in the central and peripheral VFs, 12.0% more abnormal peripheral points were noted compared with the percentage of abnormal central points (P < 0.001; SD, 16.7%; range, 61% more to 37% less). In eyes with an average of 60%-90% abnormal points in the central and peripheral VFs, 16.4% fewer abnormal peripheral points were noted compared with the percentage of abnormal central points (P = 0.04; SD, 20.9%; range, 19% more to 49% less). CONCLUSIONS Central 24-2 testing generally reflects the extent of damage to the more peripheral VF in glaucoma, although significant disagreement exists for individual eyes. Further work is needed to determine whether integration of peripheral test points can improve detection of true VF loss in early glaucoma or be useful in monitoring progressive glaucomatous damage to areas of preserved VF in advanced glaucoma.

The ability to detect visual targets in complex background varies across individuals and are affected by factors such as stimulus saliency and top-down attention. Here, we investigated how the saliency of visual background (naturalistic cartoon video vs. blank screen) and top-down attention (single vs. dual tasks) separately affect individual ability to detect visual targets. Behaviorally, we found that target detection accuracy decreased and reaction time elongated when the background was salient or during dual tasking. The EEG response to visual background was recorded using a novel stimulus tagging technique. This response was strongest in occipital electrodes and was sensitive to background saliency but not dual tasking. In contrast, the event-related potential (ERP) evoked by the visual target was strongest in central electrodes, and was affected by both background saliency and dual tasking. With a cartoon background, the EEG responses to visual targets, presented in the central visual field, and the EEG responses to peripheral visual background could both predict individual target detection performance. When these two responses were combined, better prediction was achieved. These results suggest that neural processing of visual targets and background jointly contribute to individual visual target detection performance.

Posterior vitreous detachment (PVD) is commonly seen in the aging eye [1]. Although the majority of PVDs occur without complication, a small proportion can be associated with excessive traction on the macula, resulting in retinal anatomic changes. These eyes are characterized as having vitreomacular traction (VMT). VMT can lead to functional changes such as metamorphopsia and blurring of visual acuity with central visual field defects.

IMPORTANCE: Although the central visual field (VF) in end-stage glaucoma may substantially vary among patients, structure-function studies and quality-of-life assessments are impeded by the lack of appropriate characterization of end-stage VF loss. OBJECTIVE: To provide a quantitative characterization and classification of central VF loss in end-stage glaucoma. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study collected data from 5 US glaucoma services from June 1, 1999, through October 1, 2014. A total of 2912 reliable 10-2 VFs of 1103 eyes from 1010 patients measured after end-stage 24-2 VFs with a mean deviation (MD) of −22 dB or less were included in the analysis. Data were analyzed from March 28, 2018, through May 23, 2019. MAIN OUTCOMES AND MEASURES: Central VF patterns were determined by an artificial intelligence algorithm termed archetypal analysis. Longitudinal analyses were performed to investigate whether the development of central VF defect mostly affects specific vulnerability zones. RESULTS: Among the 1103 patients with the most recent VFs, mean (SD) age was 70.4 (14.3) years; mean (SD) 10-2 MD, −21.5 (5.6) dB. Fourteen central VF patterns were determined, including the most common temporal sparing patterns (304 [27.5%]), followed by mostly nasal loss (280 [25.4%]), hemifield loss (169 [15.3%]), central island (120 [10.9%]), total loss (91 [8.3%]), nearly intact field (56 [5.1%]), inferonasal quadrant sparing (42 [3.8%]), and nearly total loss (41 [3.7%]). Location-specific median total deviation analyses partitioned the central VF into a more vulnerable superonasal zone and a less vulnerable inferotemporal zone. At 1-year and 2-year follow-up, new defects mostly occurred in the more vulnerable zone. Purpose To compare the performance of the pattern standard deviation (PSD) values derived from the central 12 locations of the 24-2 visual field test (C24-2) to the entire 10-2 test for detecting central visual field abnormalities in eyes with, suspected of having, or at risk of having glaucoma. Design Cross-sectional case-control study. Methods Eyes with, suspected of having, or at risk of having glaucoma, based on masked grading of optic disc stereophotographs and/or ocular hypertension (intraocular pressure ≥ 22 mm Hg) were included as cases (n = 523). Eyes from healthy participants were included as controls (n = 107) to allow the 2 tests to be compared at matched specificities. The sensitivity to detect cases at 95% specificity using PSD values derived from the entire 10-2 test and C24-2 were compared. Results The sensitivity of the 10-2 and C24-2 PSD values was not significantly different between the 10-2 and C24-2 at matched specificities (35.9% and 35.4% respectively; P = .900). There was also a substantial agreement between the cases detected by both methods (kappa = 0.80 ± 0.04), and a very strong association between the PSD values from the 2 methods (R2 = 0.91). Conclusions 10-2 and 24-2 tests identified a similar number of eyes with, suspected of having, or at risk of having glaucoma as having central visual field abnormalities using PSD values. These findings do not mean that 10-2 tests are not useful, but highlight the need for further studies to determine the potential advantages of 10-2 tests through equivalent comparisons against 24-2 tests to ensure appropriate recommendations are made about its incorporation into the glaucoma standard of care.

Background: To characterize the axial and off-axis refraction across four meridians of the retina in myopic eyes before and after Orthokeratology (OK) and LASIK surgery. Methods: Sixty right eyes with a spherical equivalent (M) between − 0.75 to − 5.25 D (cylinder 0.05) from the post-treatment RPRE within 30° and 20° of the central visual field after LASIK and OK, respectively. These results demonstrated that the treatment zone warrants an optimal central field of vision. Conclusions: The present study gives an overview of RPRE after refractive corneal reshaping treatments (OK and LASIK) across vertical, horizontal and two oblique meridians together. Purpose. To describe a case of optic disk pit (ODP) with sudden central visual field scotoma. Methods. A 49-year-old woman presented, reporting sudden painless central visual field loss 3 months prior to presentation. Neuroophthalmologic, systematic, and laboratory evaluation and full imaging processes were performed. Results. Fundoscopy and color photography demonstrated an optic disk pit inferotemporally. Perimetry identified central visual field horizontal scotoma. OCT revealed absence of serous retinal detachment, but disclosed inner retina thinning corresponding to the area of the visual field loss. Fluorescein angiography demonstrated delay in the cilioretinal arteries and also disclosed a relative delay in the perfusion of an arterial branch off the inferior retinal arcade. Clinical and laboratory evaluations were negative for any related pathology. Conclusion. Sudden central visual field scotoma in patients with ODP may be associated with delayed vascular filling of CRA and retinal arterioles within the optic disc anomaly region.

Purpose - to compare the ease of use, the comfort for persons to be tested, the examination rate, as well as the variability of repeated results obtained using four methods of computerized perimetry. Materials and methods. This clinical study included three groups of patients with open-angle glaucoma (OAG). The 1st group included patients with OAG stage I, the 2nd group - with OAG stage II, the 3rd group - with OAG stage III. The control group included healthy individuals. All tested persons underwent examinations by 4 computerized methods (HFA II, Tomey AP-1000, Pericom, and the FDT-perimetry modification developed at the Ophthalmology Department of the Military Medical Academy). Results. FDT-perimetry appeared to be the shortest, easiest test and most comfortable for tested persons. Perimetry using Tomey AP-1000, Pericom and HFA II was more time-consuming and more difficult to perform. Repeated results of all four methods were better than the first one due to the “learning curve” effect, and showed different variability. Conclusion. To obtain reliable computerized perimetry results, taking into account the possible “learning curve” effect, we recommend repeating the perimetric test at least 2-3 times at same conditions. It is important for the selected perimetric test to be easy to perform, comfortable for persons to be tested, and quite fast to perform.

Microperimetry, also referred to as fundus perimetry or fundus-driven perimetry, enables simultaneous acquisition of visual sensitivity and eye movement data. We present sensitivity data collected from 60 participants with normal vision using gaze-contingent perimetry. A custom designed spatially dense test grid was used to collect data across the visual field within 13° of fixation. These data are supplemental to a study in which we demonstrated a spatial interpolation method that facilitates comparison of acquired data from any set of spatial locations to normative data and thus screening of individuals with both normal and non-foveal fixation “Methods for normative data comparison in gaze-contigent microperimetry” (Denniss and Astle, 2016) [1].
