Peritoneal carcinomatosis is the most frequent site of metastases in patients with gastric cancer. Current standard treatment is palliative systemic chemotherapy with very poor prognosis. Cytoreductive surgery (CRS) combined with hyperthermic intraperitoneal chemotherapy (HIPEC) resulted in long-term benefits in selected patients. Among patients with peritoneal carcinomatosis, a distinctive subset is oligometastatic disease which is characterized by low metastatic burden. Pressurized intraperitoneal aerosol chemotherapy (PIPAC) is a recent technique of intraperitoneal chemotherapy used in combination with systemic chemotherapy with promising results.

3. Steady-State Initialization of Object-Oriented Advanced Thermal Power Generation System Models with Application to the Case of the SOS-CO2 Cycle

The forthcoming energy transition calls for a new generation of thermal power generation systems with low- or zero-emission and highly flexible operation. Dynamic modelling and simulation is a key enabling factor in this field, as controlling such plants is a difficult task for which there is no previous experience and very short design times are expected. The steady-state initialization of those dynamic models is an essential step in the design process, but is unfortunately a difficult task which involves the numerical solution of large systems of nonlinear equations with iterative Newton methods, which is often prone to numerical failures. In this work, several strategies and methodologies are discussed to successfully achieve steady-state initialization of first-principles equation-based, object-oriented models of advanced thermal power generation systems. These are presented in the context of the Modelica modelling language, but could be applied to other equation-based, object-oriented modelling and simulation environments. Finally, the successful application of such strategies and methodologies to the SOS-CO2 advanced power generation system is presented., Comment: Submitted to Simulation Modelling Practice and Theory

4. Multi-rate Runge-Kutta methods: stability analysis and applications

We present an approach for the efficient implementation of self-adjusting multi-rate Runge-Kutta methods and we extend the previously available stability analyses of these methods to the case of an arbitrary number of sub-steps for the active components. We propose a physically motivated model problem that can be used to assess the stability of different multi-rate versions of standard Runge-Kutta methods and the impact of different interpolation methods for the latent variables. Finally, we present the results of several numerical experiments, performed with implementations of the proposed methods in the framework of the \textit{OpenModelica} open-source modelling and simulation software, which demonstrate the efficiency gains deriving from the use of the proposed multi-rate approach for physical modelling problems with multiple time scales.

Equation-based modelling is a powerful approach to tame the complexity of large-scale simulation problems. Equation-based tools automatically translate models into imperative languages. When confronted with nowadays' problems, however, well assessed model translation techniques exhibit scalability issues, that are particularly severe when models contain very large arrays. In fact, such models can be made very compact by enclosing equations into looping constructs, but reflecting the same compactness into the translated imperative code is not trivial. In this paper, we face this issue by concentrating on a key step of equations-to-code translation, the equation/variable matching. We first show that an efficient translation of models with (large) arrays needs awareness of their presence, by defining a figure of merit to measure how much the looping constructs are preserved along the translation. We then show that the said figure of merit allows to define an optimal array-aware matching, and as our main result, that the so stated optimal array-aware matching problem is NP-complete. As an additional result, we propose a heuristic algorithm capable of performing array-aware matching in polynomial time. The proposed algorithm can be proficiently used by model translator developers in the implementation of efficient tools for large-scale system simulation.

8. Can the COVID-19 epidemic be controlled on the basis of daily test reports?

This paper studies if and to which extent COVID-19 epidemics can be controlled by authorities taking decisions on public health measures on the basis of daily reports of swab test results, active cases and total cases. A suitably simplified process model is derived to support the controllability analysis, highlighting the presence of very significant time delay; the model is validated with data from several outbreaks. The analysis shows that suppression strategies can be effective if strong enough and enacted early on. It also shows how mitigation strategies can fail because of the combination of delay, unstable dynamics, and uncertainty in the feedback loop; approximate conditions based on the theory of limitation of linear control are given for feedback control to be feasible., Comment: 6 pages double-column + 2 pages of supplementary material. 6-page paper published on IEEE Control Systems Letters which has strict page limit and does not accept supplementary material. 9. On the choice of initial guesses for the Newton-Raphson algorithm

The initialization of equation-based differential-algebraic system models, and more in general the solution of many engineering and scientific problems, require the solution of systems of nonlinear equations. Newton-Raphson's method is widely used for this purpose; it is very efficient in the computation of the solution if the initial guess is close enough to it, but it can fail otherwise. In this paper, several criteria are introduced to analyze the influence of the initial guess on the evolution of Newton-Raphson's algorithm and to identify which initial guesses need to be improved in case of convergence failure. In particular, indicators based on first and second derivatives of the residual function are introduced, whose values allow to assess how much the initial guess of each variable can be responsible for the convergence failure. 11. A self adjusting multirate algorithm based on the TR-BDF2 method

We propose a self adjusting multirate method based on the TR-BDF2 solver. The potential advantages of using TR-BDF2 as the key component of a multirate framework are highlighted. A linear stability analysis of the resulting approach is presented and the stability features of the resulting algorithm are analysed. The analysis framework is completely general and allows to study along the same lines the stability of self adjusting multirate methods based on a generic one step solver. A number of numerical experiments demonstrate the efficiency and accuracy of the resulting approach also the time discretization of hyperbolic partial differential equations.

- Abstract
BACKGROUND: The SAfety and FEasibility of standard EVAR outside the instruction for use (SAFE-EVAR) Study was designed to define the attitude of Italian vascular surgeons towards the use of standard endovascular repair (EVAR) for infrarenal abdominal aortic aneurysm (AAA) outside the instruction for use (IFU) through a Delphi consensus endorsed by the Italian Society of Vascular and Endovascular Surgery (Società Italiana di Chirurgia Vascolare ed Endovascolare - SI CVE). METHODS : A questionnaire consisting of 26 statements was developed, validated by an 18-member Advisory Board, and then sent to 600 Italian vascular surgeons. The Delphi process was structured in three subsequent rounds which took place between April and June 2023. In the first two rounds, respondents could indicate one of the following five degrees of agreement: 1) strongly agree; 2) partially agree; 3) neither agree nor disagree; 4) partially disagree; 5) strongly disagree; while in the third round only three different choices were proposed: 1) agree; 2) neither agree nor disagree; 3) disagree. We considered the consensus reached when ≥70% of respondents agreed on one of the options. After the conclusion of each round, a report describing the percentage distribution of the answers was sent to all the participants. RESULTS: Two-hundred-forty-four (40.6%) Italian Vascular Surgeons agreed to participate the first round of the Delphi Consensus; the second and the third rounds of the Delphi collected 230 responders (94.3% of the first-round responders). Four statements (15.4%) reached a consensus in the first rounds. Among the 22 remaining statements, one more consensus (3.8%) was achieved in the second round. Finally, seven more statements (26.9%) reached a consensus in the simplified last round. Globally, a consensus was reached for almost half of the proposed statements (46.1%). Introduction: Vascular access devices (VADs), namely peripheral VADs (PVADs) and central venous VADs (CVADs), are crucial in both intensive care unit (ICU) and non-ICU settings. However, VAD placement carries risks, notably catheter-related bloodstream infections (CRBSIs). Candida spp. is a common pathogen in CRBSIs, yet its clinical and microbiological characteristics, especially in non-ICU settings, are underexplored. Methods: We conducted a monocentric, retrospective observational study at Luigi Sacco Hospital from 1 May 2021 to 1 September 2023. We reviewed medical records of non-ICU adult patients with CVADs and PVADs. Data on demographics, clinical and laboratory results, VAD placement, and CRBSI occurrences were collected. Statistical analysis compared Candida spp. CRBSI and bacterial CRBSI groups. Results: Out of 1802 VAD placements in 1518 patients, 54 cases of CRBSI were identified, and Candida spp. was isolated in 30.9% of episodes. The prevalence of CRBSI was 3.05%, with Candida spp. accounting for 0.94%. Incidence rates were 2.35 per 1000 catheter days for CRBSI, with Candida albicans and Candida non-albicans at 0.47 and 0.26 per 1000 catheter days, respectively—patients with Candida spp. CRBSI had more frequent SARS-CoV-2 infection, COVID-19 pneumonia, and hypoalbuminemia. Conclusions: During the COVID-19 pandemic, Candida spp. was a notable cause of CRBSIs in our center, underscoring the importance of considering Candida spp. in suspected CRBSI cases, including those in non-ICU settings and in those with PVADs. - Abstract
The components exposed to the highest temperature and heat fluxes in Solar Tower plants are the tubes of the panels composing the central receiver. Their structural integrity is weakened due to fatigue and creep phenomena, likely worsened by a corrosive environment, and their solar power harvesting performance suffers from diminished absorptance of their coating. This paper describes a 2D dynamic thermal model of the tubes of a central receiver, developed following an Object-Oriented modelling paradigm, and applies the model to assess the impact of degradation phenomena such as corrosion, coating deterioration, and soiling of heliostats. The model is verified against steady-state data available in literature both on overall receiver metal maximum temperature, HTF outlet temperature and absorbed heat, and on detailed circumferential analysis of the metal external temperature for a specific tube. The dynamic behavior of the receiver is also assessed, with focus on the impact of cloud events and the induced thermal transients.

- Abstract
Cyber-Physical Systems are ever-increasing in complexity and new methods and tools for developing them are needed. To support these highly dynamic systems, increasing the flexibility of the modeling languages is desirable. This paper proposes and examines a Modelica language extension to support dynamic overconstrained graphs with reconfiguration at runtime. Two applications of this new feature are also discussed: synchronous AC power systems and incompressible fluid networks. Reported findings suggest that supporting dynamic overconstrained graphs might yield performance benefits and provide the possibility of simulating systems that can not currently be simulated in existing Modelica tools.

- Abstract
Background: The aim of this study was to compare the 2-year outcomes of de novo versus postendovascular lesion treatment of femoropopliteal occlusions included in a national, multicenter, observational, prospective registry based on the treatment of critical Limb-threatening IschaeMia with infragenicular Bypass adopting in situ SAphenous VEin technique (LIMBSAVE) registry.Methods: From January 2018 to December 2019, 541 patients from43 centers have been enrolled in the LIMBSAVE registry. Of these patients, 460 were included in the present study: 341 (74.1%) with de novo lesions (DN group) and 119 (25.9%) with postendovascular treatment lesions (PE group). Initial outcome measures were assessed at 30 days after treatment. Furthermore, at the 2-year follow-up, the estimated outcomes of primary patency, primary-assisted patency, secondary patency, and limb salvage were analyzed with Kaplan-Meier curves and compared between groups with the log-rank test.Results: Both groups were homogeneous in terms of demographic data, preoperative risk factors, and clinical presentation. However, compared with DN group, more patients in PE group had a great saphenous vein diameter of less than 3 mm (11.1% vs 21%; P = .007). Intraoperatively, both groups showed similar distal anastomosis sites: below-the-knee popliteal artery (63% DN group, 66.4% PE group) and tibial vessel (37% DN group, 33.6% PE group) (P = .3). The overall mean duration of follow-up was 11.6 months (range, 1-24 months). At the 2-year follow-up, there were no differences between the two groups in terms of primary patency (66.3% DN group vs 74.1% PE group; P = .9), primary-assisted patency (78.2% DN group vs 79.5% PE group; P = .2), secondary patency (85.1% DN group vs 91.4% PE group; P = .2), and limb salvage (95.2% DN group vs 95.1% PE group; P = .9).Conclusions: The LIMBSAVE registry did not show a worsening of overall patency and limb salvages rates at the 2-year follow-up in patients undergoing in situ saphenous

Evidence against the use of prophylactic drain after gastrectomy are increasing and ERAS guidelines suggest the benefit of drain avoidance. Nevertheless, it is unclear whether this practice is still widespread. We conducted a survey among Italian surgeons through the Italian Gastric Cancer Research Group and the Polispecialistic Society of Young Surgeons, aiming to understand the current use of prophylactic drain. A 28-item questionnaire-based survey was developed to analyze the current practice and the individual opinion about the use of prophylactic drain after gastrectomy. Groups based on age, experience and unit volume were separately analyzed. Response of 104 surgeons from 73 surgical units were collected. A standardized ERAS protocol for gastrectomy was applied by 42% of the respondents. Most of the surgeons, regardless of age, experience, or unit volume, declared to routinely place one or more drain after gastrectomy. Only 2 (1.9%) and 7 surgeons (6.7%) belonging to high volume units, do not routinely place drains after total and subtotal gastrectomy, respectively. More than 60% of the participants remove the drain on postoperative day 4–6 after performing an assessment of the anastomosis integrity. Interestingly, less than half of the surgeons believe that drain is the main tool for leak management, and this percentage further drops among younger surgeons. On the other hand, drain’s role seems to be more defined for duodenal stump leak treatment, with almost 50% of the surgeons recognizing its importance. Routine use of prophylactic drain after gastrectomy is still a widespread practice even if younger surgeons are more persuaded that it could not be advantageous.

Simple Summary: Mini-invasive surgery represents an interesting yet challenging technical evolution for treating peritoneal metastases. This retrospective study aims to present the experience of the Italian Peritoneal Surface Malignancies Oncoteam with laparoscopic cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC), including a detailed description of the technique and a systematic review of the literature. The study shows the feasibility and safety of laparoscopic CRS-HIPEC and its association with favorable outcomes in properly selected patients. Cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) has gained increasing acceptance in clinical practice. Performing CRS and HIPEC laparoscopically represents a challenging and intriguing technical evolution. However, the experiences are limited, and the evidence is low. This retrospective analysis was performed on patients treated with laparoscopic CRS-HIPEC within the Italian Peritoneal Surface Malignancies Oncoteam. Clinical, perioperative, and follow-up data were extracted and collected on prospectively maintained databases. We added a systematic review according to the PRISMA method for English-language articles through April 2022 using the keywords laparoscopic, hyperthermic, HIPEC, and chemotherapy. From 2016 to 2022, fourteen patients were treated with Lap-CRS-HIPEC with curative intent within the Italian centers. No conversion to open was observed. The median duration of surgery was 487.5 min. The median Peritoneal Cancer Index (PCI) was 3, and complete cytoreduction was achieved in all patients. Two patients (14.3%) had major postoperative complications, one requiring reintervention. After a median follow-up of 16.9 months, eleven patients were alive without disease (78.6%), two patients developed recurrence (14.3%), and one patient died for unrelated causes (7.1%). The literature review confirmed these results. In conclusion, current evidence shows that Lap-CRS-HIPEC is feasible, safe, and associated with a favorable outcome in selected patients. ObjectivesGuidelines recommend open bypass surgery for long occlusions of infrainguinal arteries. In situ saphenous vein bypass is a standardized technique. The aim of this study was to report preliminary six-month outcomes of a national, multicenter, observational, prospective registry based on the examination of treatment of critical Limb IscheMia with infragenicular Bypass adopting the in situ SAphenous VEin technique (LIMBSAVE).MethodsFrom January 2018 until October 2019, 428 patients from 41 centers were enrolled in the LIMBSAVE registry. Data were prospectively collected in a dedicated database, including demographics, preoperative risk factors, clinical and diagnostic preoperative assessments, intraoperative measures (including safety and effectiveness of the valvulotome during the surgical procedures), and 30-day follow-up data. Furthermore, estimated six-month outcomes according to Kaplan-Meier curves in terms of primary patency, primary assisted patency, secondary patency, and limb salvage were evaluated.ResultsPatients were predominantly male (n = 332, 77.6%) with a mean age of 73.3 years (range 39-95). Technical success, defined as bypass pulse after use of the valvulotome, was obtained in all cases. The proximal anastomosis could be reached by the valvulotome in all cases. The mean number of valvulotome uses was 2.5 (range 1-5). No vein perforation was reported. In nine cases (2.1%), a vein lesion with intramural hemorrhage occurred. The mean length of hospital stay was 11.1 days (range 1-60). At 30-day follow-up, the overall bypass patency rate was 97.4%, and the rate of open or endo reinterventions for failing bypass was 5.4%. Background and aim of the work: since 2003, a series of so called ‘micro-posterior’ approaches have been developed in the orthopaedic surgery to perform total hip replacement. These techniques present several theoretical advantages compared to the classic postero-lateral approach: reduction in blood loss, post-operative pain, and length of stay but also improving functional outcomes. In our hypothesis these goals could also be obtained in patients with femoral neck fractures, especially in the elderly with several comorbidities. Methods: In our series we performed 50 consecutive cemented hemiarthroplasties through SuperPATH approach. At the latest follow-up (FU) 41 patients were included in the study. Clinical and radiological evaluation was performed before the surgery and at the latest FU using VAS score, Harris Hip Score (HHS) and conventional AP and LL X-rays. Results: We didn’t report any intra-operative complication (i.e. periprosthetic fractures, vasculo-nervous injuries or dislocations) or any case with a dysmetria greater than 1 cm. Mean blood transfusion units were 0.8 during surgery (0 – 4) and 1,6 after surgery (0-4). We reported a mean length of stay of 9.7 days (range 7-15) mainly due to our patients’ high comorbidity rate. At the last one-year FU no cases of dislocation, infection and clinical or radiographical signs of prosthetic loosening were recorded. All our patients returned to the same activity level before femoral fracture occurred, according to HHS. Conclusion: The SuperPATH is a real minimally invasive approach with a low rate of intra- and post-operative complications. It is related to a reduced dislocation rate and potentially to a reduced infection rate compared to the conventional surgical approaches, allowing, furthermore, a faster functional recovery. - Abstract
OpenModelica is a unique large-scale integrated open-source Modelica- and FMI-based modeling, simulation, optimization, model-based analysis and development environment. Moreover, the OpenModelica environment provides a number of facilities such as debugging; optimization; visualization and 3D animation; web-based model editing and simulation; scripting from Modelica, Python, Julia, and Matlab; efficient simulation and co-simulation of FMI-based models; compilation for embedded systems; Modelica-UML integration; requirement verification; and generation of parallel code for multi-core architectures. The environment is based on the equation-based object-oriented Modelica language and currently uses the MetaModelica extended version of Modelica for its model compiler implementation. - Abstract
The most solar-specific components in a Concentrated Solar Power (CSP) plant are the solar field and the receiver. The latter is exposed to extreme working conditions as it withstands high heat fluxes and temperatures in a corrosive environment during usual and transient conditions. It is then of paramount importance to assess the thermal behavior of such a fundamental component of the plant. The object-oriented modelling paradigm enables precise dynamic analysis of physical systems through the solution of sets of equations derived following an acausal approach. This guarantees modularity and description of interactions through physical connection equations with no causality specification. The application of the object-oriented modelling technique on the external receiver of a CSP tower plant is described in detail in this paper, proving its potentialities and modelling capabilities. The model is first verified against data available in literature and then applied for the analysis of dynamic operations, including typical issues of CSP plants as the impact of clouds and heliostats soiling.

The main results of the simulation study of a double-flash geothermal plant, including a steam-C[O.sub.2] reboiler-based separation process, are presented. Dynamic models of two-phase, [H.sub.2]-C[O.sub.2] components are derived, and their use for dynamic simulation and operating point optimization is discussed. Control system issues are then considered. On one hand, the simulator is used as an aid to design and test standard level and pressure control loops, whose actual performance would otherwise be difficult to predict due to the high degree of interconnection of the plant components. On the other hand, the control concept for the innovative part of the plant is discussed, recognizing that the control problem and the operating point optimization problems have to be tackled simultaneously. A simple, yet effective, solution for this problem is proposed. Index Terms--Geothermal power generation, optimization methods, power generation control, power plant simulation, process control.

- Abstract
The equation-based object-oriented Modelica language allows easy composition of models from components. It is very easy to create very large parametrized models using component arrays of models. Current open-source and commercial Modelica tools can with ease handle models with a hundred thousand equations and a thousand states. However, when the system size goes above half a million (or more) equations the tools begin to have problems with scalability. This paper presents the new frontend of the OpenModelica compiler, designed with scalability in mind. The new OpenModelica frontend can handle much larger systems than the current one with better time and memory performance. The new frontend was validated against large models from the ScalableTestSuite library and Modelica Standard Library, with good results.
