History of Present Illness: A 57-year-old woman with history of bone disease presented with a 3-day history of cough with thick yellow phlegm and progressive shortness of breath. No fever, chest pain or abdominal pain was noted. In the emergency department, she had SpO2 of 55% on room air, and then 90% on 15L NRB. - Abstract
The tokamak à configuration variable (TCV) continues to leverage its unique shaping capabilities, flexible heating systems and modern control system to address critical issues in preparation for ITER and a fusion power plant. For the 2019-20 campaign its configurational flexibility has been enhanced with the installation of removable divertor gas baffles, its diagnostic capabilities with an extensive set of upgrades and its heating systems with new dual frequency gyrotrons. The gas baffles reduce coupling between the divertor and the main chamber and allow for detailed investigations on the role of fuelling in general and, together with upgraded boundary diagnostics, test divertor and edge models in particular. The increased heating capabilities broaden the operational regime to include Te/Ti ∼1 and have stimulated refocussing studies from L-mode to H-mode across a range of research topics. ITER baseline parameters were reached in type-I ELMy H-modes and alternative regimes with 'small' (or no) ELMs explored. Most prominently, negative triangularity was investigated in detail and confirmed as an attractive scenario with H-mode level core confinement but an L-mode edge. Emphasis was also placed on control, where an increased number of observers, actuators and control solutions became available and are now integrated into a generic control framework as will be needed in future devices. The quantity and quality of results of the 2019-20 TCV campaign are a testament to its successful integration within the European research effort alongside a vibrant domestic programme and international collaborations.

Organic Electrochemical Transistors (OECTs) are suitable for developing ultra-sensitive bioelectronic sensors. In the organic electrochemical transistors architecture, the source-drain channel is made of a conductive polymer film either cast from a formulated dispersion or electrodeposited from a monomer solution. The commercial poly(3,4-ethylenedioxidethiophene)/poly(styrene sulfonate) (PEDOT:PSS) water dispersion is the workhorse of organic bioelectronics for its high conductance, low impact and ease of processability. In this study, a hybrid organic electrochemical transistors channel fabrication strategy is presented, where electrochemical deposition of a PEDOT/X (with X indicating the counterion) is performed on a dispersion-cast PEDOT:PSS film. Six different counterions where used: X = PSS, Nafion, Hyaluronate, Dextran sulfate, Dexamethasone phosphate and tauroursodeoxycholic acid, each potentially endowing organic electrochemical transistors with additional functions such as ion exchange and pharmacological activity upon release of X. The PEDOT/X-PEDOT:PSS bilayers were characterized by means of electrochemical impedance spectroscopy (EIS), atomic force microscopy (AFM), X-ray photoelectron spectroscopy (XPS) and focused ion beam tomography combined with scanning electron microscopy (FIB-SEM). In addition, their respective organic electrochemical transistorss were characterized and compared to PEDOT:PSS organic electrochemical transistors. Our results show that the hybrid bilayer strategy is viable to fabricate multifunctional organic electrochemical transistorss with biologically-relevant function, thereby retaining the outstanding figures of merit of commercial PEDOT:PSS.

To evaluate the effects of rehabilitation (physical and cognitive) treatments on the diagnosis severity and Disability Rating Scale (DRS) scores, adjusted for a number of potential confounders measured at baseline, in a large cohort of patients with disorders of consciousness across time.An observational, longitudinal (2 evaluations), multicenter project was made in 90 Italian centers.Patients (N=364) with a diagnosis of disorders of consciousness.Primary outcome was the severity of diagnosis, expressed on an ordinal scale (OtherMCSVSdeath). In the Other group were included patients who emerged from an MCS and recovered consciousness. The secondary outcome was the DRS score (range of 0-30 with 30 being the worst value). The DRS is a tool used to define the level of residual disability, commonly used to classify the level of functional impairment in patients with acquired brain injury. Both outcomes were measured for each wave.A total of 364 subjects having a complete set of demographic, clinical, and pharmacologic data were included in the propensity score (PS) analysis. Results showed that the rehabilitation treatments (physical and cognitive) reduced the clinical worsening over time in both severity diagnosis and DRS (around 6.5 points) in patients with disorders of consciousness across different propensity score strategies (ie, PS matching, PS adjustment, and PS-weighted procedures). In addition, cognitive protocols seem to be limited to patients with a median value of DRS=23.Our propensity score analysis suggests that rehabilitation treatment protocols seem effective and should be applied to a broader spectrum of patients with disorders of consciousness.

- Abstract
Introduction: On February 20, 2020, a severe case of pneumonia due to SARS-CoV-2 was diagnosed in northern Italy (Lombardy). Some studies have identified obesity as a risk factor for severe disease in patients with COVID-19. The purpose of this study was to investigate the incidence of SARS-CoV-2 infection and its severity in patients who have undergone bariatric surgery. Material and Methods: During the lockdown period (until May 2020), we contacted operated patients by phone and social networks (e.g., Facebook) to maintain constant contact with them; in addition, we gave the patients a dedicated phone number at which to call us for emergencies. We produced telemedicine and educational videos for obese and bariatric patients, and we submitted a questionnaire to patients who had undergone bariatric surgery in the past. Results: A total of 2145 patients (313 male; 1832 female) replied to the questionnaire. Mean presurgical BMI: 44.5 ± 6.8 kg/m2. Mean age: 44.0 ± 10.0 year. Mean BMI after surgery: 29.3 ± 5.5 kg/m2 (p < 0.05). From February to May 2020, 8.4% of patients reported that they suffered from at least one symptom among those identified as related to SARS-CoV-2 infection. Thirteen patients (0.6%) tested positive for COVID-19. Six patients (0.3%) were admitted to the COVID Department, and 2 patients (0.1%) were admitted to the ICU. Conclusions: Although the reported rates of symptoms and fever were high, only 0.6% of patients tested positive for COVID-19. Among more than 2000 patients who underwent bariatric surgery analyzed in this study, only 0.1% needed ICU admission.

- Abstract
The tokamak à configuration variable (TCV) continues to leverage its unique shaping capabilities, flexible heating systems and modern control system to address critical issues in preparation for ITER and a fusion power plant. For the 2019-20 campaign its configurational flexibility has been enhanced with the installation of removable divertor gas baffles, its diagnostic capabilities with an extensive set of upgrades and its heating systems with new dual frequency gyrotrons. The gas baffles reduce coupling between the divertor and the main chamber and allow for detailed investigations on the role of fuelling in general and, together with upgraded boundary diagnostics, test divertor and edge models in particular. The increased heating capabilities broaden the operational regime to include Te/Ti ∼1 and have stimulated refocussing studies from L-mode to H-mode across a range of research topics. ITER baseline parameters were reached in type-I ELMy H-modes and alternative regimes with 'small' (or no) ELMs explored. Most prominently, negative triangularity was investigated in detail and confirmed as an attractive scenario with H-mode level core confinement but an L-mode edge. Emphasis was also placed on control, where an increased number of observers, actuators and control solutions became available and are now integrated into a generic control framework as will be needed in future devices. The quantity and quality of results of the 2019-20 TCV campaign are a testament to its successful integration within the European research effort alongside a vibrant domestic programme and international collaborations.

SARS-CoV-2 (Severe Acute Respiratory Syndrome Coronavirus 2) has been identified in China as responsible for viral pneumonia, now called COVID-19 (Coronavirus Disease 2019). Patients infected can develop common symptoms like cough and sore throat, and, in severe cases, acute respiratory syndrome and even death. To optimize the available resources, it is necessary to identify in advance the subjects that will develop a more serious illness, therefore requiring intensive care.The neutrophil / lymphocyte ratio (NLR) parameter, resulting from the blood count, could be a significant marker for the diagnosis and management of risk stratification.A retrospective, single-center case-control observational study was conducted. The differential cell count of leukocytes, the NLR and the clinical course of patients hospitalized in intensive care with COVID-19 were analyzed, comparing them with other patients (COVID-19 and non-COVID-19) and healthy individuals selected among workers of the Teaching Hospital Policlinico Umberto I in Rome.370 patients (145 cases and 225 controls) were included in the case-control study, 211 males (57%) and 159 females (43%). The average age of the population was 63 years (SD 16.35). In the group of cases, out of 145 patients, 57 deaths and 88 survivors were recorded, with a lethality rate of 39.3%. The group of cases has an NLR of 7.83 (SD = 8.07), a much higher value than the control group where an NLR of 2.58 was recorded (SD = 1.93) (p0.001). The Neutrophils / Lymphocytes ratio may prove to be a diagnostic factor for COVID-19, an NLR3.68 revealed an OR 10.84 (95% CI = 6.47 - 18.13) (p0.005).The value of NLR considered together with the age variable allows a risk stratification and allows the development of diagnostic and treatment protocols for patients affected by COVID-19. A high neutrophil to lymphocyte ratio suggests worse survival. Risk stratification and management help alleviate the shortage of medical resources and reduce the mortality of critically ill patients.

Objective To investigate the clinical manifestations at diagnosis and during follow-up in patients with 22q11.2 deletion syndrome to better define the natural history of the disease. Study design A retrospective and prospective multicenter study was conducted with 228 patients in the context of the Italian Network for Primary Immunodeficiencies. Clinical diagnosis was confirmed by cytogenetic or molecular analysis. Results The cohort consisted of 112 males and 116 females; median age at diagnosis was 4 months (range 0 to 36 years 10 months). The diagnosis was made before 2 years of age in 71% of patients, predominantly related to the presence of heart anomalies and neonatal hypocalcemia. In patients diagnosed after 2 years of age, clinical features such as speech and language impairment, developmental delay, minor cardiac defects, recurrent infections, and facial features were the main elements leading to diagnosis. During follow-up (available for 172 patients), the frequency of autoimmune manifestations ( P = .015) and speech disorders ( P = .002) increased. After a median follow-up of 43 months, the survival probability was 0.92 at 15 years from diagnosis. Conclusions Our data show a delay in the diagnosis of 22q11.2 deletion syndrome with noncardiac symptoms. This study provides guidelines for pediatricians and specialists for early identification of cases that can be confirmed by genetic testing, which would permit the provision of appropriate clinical management.

Salmonellais a bacterium usually associated with mild gastroenteritis in chickens, but strains of the serotype Gallinarum can result in 2 severe systemic chicken diseases: fowl typhoid (by biovar Gallinarum) and pullorum disease (by biovar Pullorum). These diseases can be controlled by immunization with a specific vaccine (SG 9R). The aim of this study was to characterize recent SalmonellaGallinarum isolates by molecular analyses, as well as detect field and vaccine strains in clinical and environmental samples from poultry flocks. Field isolates of S.Gallinarum (n = 18) and other serotypes (n = 117) were obtained from Brazilian poultry farms between 2012 and 2015. In addition, clinical and environmental samples of poultry flocks (vaccinated or not) with fowl typhoid were obtained in a 3-year period (2013 to 2015). The results demonstrated that specific PCR targeted on speCand glgCgenes and PCR-restriction fragment length polymorphisms based on fliCand sefAgenes detected all the 18 isolates, 10 identified as field isolates of biovar Gallinarum and 8 as biovar Pullorum. All isolates of other serotypes presented negative results. In addition, PCR tests were performed directly from the pre-enrichment broth of the bacteriological procedures. Vaccine SG 9R and field S.Gallinarum strains were detected in samples with (n = 19) and without (n = 59) vaccination. These results demonstrate the use of PCR tests for detection of S. Gallinarum in clinical and environmental Brazilian poultry samples.

Prosthodontic management of palatal defects is fundamental to improve patient’s life undergoing to a maxillary surgical treatment. A lot of maxillary defects are a direct consequence of surgical treatment of malformations, neoplasms or trauma. The obturators are prosthesis used to close palatal defects after maxillectomy, to restore masticatory function and to improve speech. The primary goals of the obturator prosthesis are to preserve the remaining teeth and tissue and to provide comfort, function, and aesthetics to the patients. Different materials and retention methods are a characteristic of new types of obturators.

- Abstract
[EN] Flumequine is a fluoroquinolone derivative used in food-producing species to control systemic infections caused by susceptible microorganisms, in particular Gram negative species such as Escherichia coli, Salmonella spp. and Pasteurella spp. Our study was carried out in order to evaluate the distribution and residue depletion of flumequine in rabbits. Tissue distribution was defined administering a single oral dose of 15 mg of flumequine per kg body weight. Residue depletion was determined administering the drug via drinking water at the ranging dose of 15 mg per kg body weight for 5 days. The tissue concentrations were quantified using a HPLC method, with a quantification limit of 25 mg.kg-1 for muscle, fat and lung and of 50 mg.kg-1 for liver and kidney. The experimental results show that in rabbits flumequine reaches effective tissue concentrations rapidly after oral treatment. At the moment of sacrifice (withdrawal time 0 hours) the residue depletion study showed the highest concentrations in the kidney and the liver (2064 with SD 1571 and 388 with SD 225 mg.kg-1, respectively), while in the other tissues analysed (muscle, fat and lung) the residues were much lower (27 with SD 30, 38 with SD 12, 60 with SD 34 mg.kg-1 in muscle, fat and lung, respectively). The residue concentrations decrease quickly and fall below the maximum residual limits, as defined by the European Authorities (200, 250, 500 and 1000 mg.kg-1 for muscle, fat, liver and kidney, respectively), within 24 hours from the cessation of medication. Considering the tissue concentrations observed after the repeated administration it can be concluded that at the dose employed (15 mg.kg-1) potentially effective drug concentrations are recorded only in the liver and the kidney.
