Adrenocorticotropic hormone (ACTH)-secreting pituitary adenoma is the most commoncause of endogenous Cushing’s disease. Renal cell carcinoma (RCC) is the mostcommon renal cancer and there may be many endocrine abnormalitiesassociated with this cancer. Cushing’s syndrome can be observed in patients whoare diagnosed with renal cell carcinoma, although Cushing's disease is veryrare . The diagnosis of Cushing's disease is made by the evaluation of clinicalsymptoms, endocrinological tests and radiological findings. The most effectivetreatment for Cushing's disease is adenomal resection, but the chance of curewith resection is lower in macroadenomas. Radiotherapy, medical treatments(such as bromocriptine, cyproheptadine, ketaconazole, pasireotide) or bilateraladrenalectomy may be considered in cases of not cured in the postoperativeperiod. In this report, we present a patient with a diagnosis of Cushing’sdisease and renal cell carcinoma., Hipofizbezinden adrenokortikotropik hormon (ACTH) salgılayan adenom, endojen Cushing hastalığının ensık görülen nedenidir. Renal hücreli karsinom (RCC) en sık görülen böbrekkanseridir ve bu kanser ile ilişkili birçok endokrinolojik anormallik olabilir.Renal hücreli karsinom tanısı alan hastalarda Cushing sendromu saptanabilirken Cushinghastalığı çok nadir görülür. Cushing hastalığının tanısı kliniksemptomlar, endokrinolojik testler ve radyolojik bulguların değerlendirilmesiile konur. Cushing hastalığında en etkili tedavi adenomun rezeksiyonudur amamakroadenomlarda rezeksiyon ile kür şansı daha düşüktür. Postoperatif dönemdekür sağlanamamış olgularda tedavi seçenekleri arasında radyoterapi, medikaltedaviler (bromokriptin, siproheptadin, ketakonazol, pasireotide gibi) veyabilateral adrenalektomi düşünülebilir. Burada Cushinghastalığı ile birlikte renal hücreli karsinom tanısı konulan bir hastasunulacaktır.

Diferansiye tiroid karsinomları (DTK) tiroid foliküler epitelyal hücrelerden gelişen ve tüm tiroid karsinomlarının %90’ını oluşturan kanserlerdir. Çalışmamızda iyot refrakter metastatik DTK’larda sorafenib tedavisinin etkinliği ve güvenliğinin değerlendirilmesi planlandı. Çalışmamıza merkezimizde iyot refrakter metastatik DTK nedeniyle sorafenib tedavisi alan 6 hasta dahil edildi. Sorafenib tedavisi sonrası izlemde hastalarda en sık cilt döküntüleri olmak üzere, hipertansiyon, yüksek ventrikül hızlı atriyal fibrilasyon ve özefajit gibi ilaca bağlı yan etkiler gözlendi. Cilt lezyonları tüm hastalarda tedavinin 1. ayında ve hafif şiddette gelişmiş olup, ilaç doz azaltımı sonrası semptomlar 1 ay içinde düzeldi. Bir hasta perikardiyal effüzyon gelişimi ve iki hasta akciğer metastazlarında progresyon sonrası eksitus oldu. Tedavi sonrası 3 hastada progresif hastalık, 1 hastada stabil hastalık ve 1 hastada parsiyel yanıt saptandı. Sorafenib tedavisinin etkinliği konvansiyonel kemoterapilere göre daha yüksek olmakla birlikte, gelişebilecek yan etkiler nedeniyle deneyimli merkezlerce uygulanmalı, hastalar yan etkiler konusunda bilgilendirilmelidir. Six patients who have received sorafenib treatment due to refractory metastatic DTC were included in the study. During follow-up skin rashes were the most frequent adverse effect after sorafenib treatment followed by hypertension, high ventricular rate atrial fibrillation and esophagitis. Skin lesions were mild and occurred in the first month of the treatment, after dose reduction symptoms resolved within 1 month. One patient had died of pericardial effusion and two patients had died of progressive pulmonary metastasis. Progressive disease was observed in 3 patients, stable disease in 1 patient and partial response in 1 patient after treatment.Although the efficacy of sorafenib is higher than the conventional chemotherapeutics, it should be used in experienced centers because of the possible side effects that can develope and patients should be informed about those side effects.

Symptoms in primary adrenal insufficiency depend on glucocorticoid and mineralocorticoid deficiency. Because the symptoms are nonspecific, the diagnosis is often delayed and patients may present with adrenal crisis. The appropriate dose of glucocorticoid and mineralocorticoid replacement should be performed. The goal of treatment of patients with primary adrenal insufficiency is to restore normal well-being, stable weight, normal sexual function, and physical activity. Patients with primary adrenal insufficiency are at risk of life-threatening adrenal crises. For this reason, in cases of stress such as surgery, trauma and infections, glucocorticoid doses should be increased and the patients should be informed about the circumstances in which they will increase the dose. In follow-up, evaluation of clinical findings in terms of under or overtreatment is important. For this reason, it is recommended that these patients must be followed up at experienced centers., Primer adrenal yetmezlikte semptomlar glukokortikoid ve mineralokortikoid eksikliğine bağlıdır. Semptomların nonspesifik olması nedeniyle tanı sıklıkla gecikir ve hastalar adrenal kriz ile presente olabilir. Tedavide uygun dozda glukokortikoid ve mineralokortikoid replasmanı yapılmalıdır. Amaç normal iyilik halinin sağlanması, kilonun korunması, normal seksüel fonksiyonların ve fiziksel aktivitelerin sürdürülebilmesidir. Primer adrenal yetmezliğin en önemli sebebinin otoimmün olması nedeniyle bu hastalarda diğer otoimmun hastalıkların değerlendirilmesi önem taşımaktadır. Primer adrenal yetmezlikli hastalarda hayatı tehdit edici adrenal kriz riski bulunmaktadır. Bu nedenle cerrahi, travma ve enfeksiyon gibi stres durumlarında glukokortikoid dozlarında artış yapılmalı ve hastalar doz artışı yapacakları durumlar hakkında bilgilendirilmelidir. Hastaların eksik ya da aşırı doz replasmanları açısından klinik bulgularının değerlendirilmesi izlemlerinde çok önemlidir. Bu nedenle bu hastaların, hastalık hakkında bilgili ve deneyimli merkezlerde takip edilmesi önerilmektedir.

Our aim was to investigate the relationship between lower urinary tract symptoms (LUTS) in terms of vascular risk factors include diabetes. In a prospective study, a total of 116 men aged 40 years or more who presented to the outpatient clinics of urology or endocrinology between January 2012 and April 2013 were included. After receiving a detailed medical history, fasting blood glucose, serum lipids including total cholesterol, HDL and triglyceride, HbA1c, creatinine, total testosterone and total prostate-specific antigen were measured. Urinalysis and uroflowmetry were done. Postvoiding residual urine and prostate volume were measured by suprapubic ultrasonography. International Prostate Symptom Score (IPSS) of the patients were determined. Existence of vascular risk factors including hypertension, hyperlipidemia, diabetes mellitus, coronary artery disease and obesity as well as age, body mass index (BMI), weight, waist circumference and body fat percentage values were recorded. Of the patients, 41 (35.3%) had hypertension, 54 (46.6%) dyslipidemia, 68 (58.6%) diabetes mellitus, 34 (29.3%) coronary artery disease and 39 (33.6%) obesity. When the patients were grouped according to the presence of the risk factors (no risk [14 patients], mild to moderate [1-2 risk factor] [65 patients] and severe [3 or more risk factors] [37 patients]), there was no a significant difference among the groups in terms of IPSS (p = 0.76).
