Abstract

Aminopropyl modified mesoporous SiO2 nanoparticles, MCM-41 type, have been synthesized by the co-condensation method from tetraethylorthosilicate (TEOS) and aminopropyltriethoxysilane (APTES). By means of modifying TEOS/APTES ratio we have carried out an in-depth characterization of the nanoparticles as a function of APTES content. Surface charge and nanoparticles morphology were strongly influenced by the amount of APTES and particles changed from hexagonal to bean-like morphology insofar APTES increased. Besides, the porous structure was also affected, showing a contraction of the lattice parameter and pore size, while increasing the wall thickness. These results bring about new insights about the nanoparticles formation during the co-condensation process. The model proposed herein considers that different interactions stablished between TEOS and APTES with the structure directing agent have consequences on pore size, wall thickness and particle morphology. Finally, APTES is an excellent linker to covalently attach active targeting agents such as folate groups. We have hypothesized that APTES could also play a role in the biological behavior of the nanoparticles. So, the internalization efficiency of the nanoparticles has been tested with cancerous LNCaP and non-cancerous preosteoblast-like MC3T3-E1 cells. The results indicate a cooperative effect between aminopropylsilane presence and folic acid, only for the cancerous LNCaP cell line.

2. Ultrasound-mediated Cavitation-enhanced extravasation of mesoporous silica nanoparticles for controlled-release drug delivery

Abstract

Mesoporous silica nanoparticles have been reported as suitable drug carriers, but their successful delivery to target tissues following systemic administration remains a challenge. In the present work, ultrasound-induced inertial cavitation was evaluated as a mechanism to promote their extravasation in a flow-through tissue-mimicking agarose phantom. Two different ultrasound frequencies, 0.5 or 1.6 MHz, with pressures in the range 0.5-4 MPa were used to drive cavitation activity which was detected in real time. The optimal ultrasound conditions identified were employed to deliver dye-loaded nanoparticles as a model for drug-loaded nanocarriers, with the level of extravasation evaluated by fluorescence microscopy. The same nanoparticles were then co-injected with submicrometric polymeric cavitation nuclei as a means to promote cavitation activity and decrease the required in-situ acoustic pressure required to attain extravasation. The overall cavitation energy and penetration of the combination was compared to mesoporous silica nanoparticles alone. The results of the present work suggest that combining mesoporous silica nanocarriers and submcrometric cavitation nuclei may help enhance the extravasation of the nanocarrier, thus enabling subsequent sustained drug release to happen from those particles already embedded in the tumour tissue., Comment: 27 pages, 6 figures

Abstract

3D porous scaffolds based on agarose and nanocrystalline apatite, two structural components that act as a temporary mineralized extracellular matrix, were prepared by the GELPOR3D method. This shaping technology allows the introduction of thermally-labile molecules within the scaffolds during the fabrication procedure. An angiogenic protein, Vascular Endothelial Growth Factor, and an antibiotic, cephalexin, loaded in mesoporous silica nanoparticles, were included to design multifunctional scaffolds for bone reconstruction. The dual release of both molecules showed a pro-angiogenic behaviour in chicken embryos grown ex ovo, while, at the same time providing an antibiotic local concentration capable of inhibiting Staphylococcus aureus bacterial growth. In this sense, different release patterns, monitored by UV-spectroscopy, could be tailored as a function of the cephalexin loading strategy. The scaffold surface was characterized by a high hydrophilicity, as determined by contact angle measurements, that facilitated the adhesion and proliferation of preosteoblastic cells., Comment: 29 pages, 10 figures

4. Suicide-Gene Transfection of Tumor-tropic Placental Stem Cells employing Ultrasound-Responsive Nanoparticles

Abstract

A Trojan-horse strategy for cancer therapy employing tumor-tropic mesenchymal stem cells transfected with a non-viral nanovector is here presented. In this sense, ultrasound-responsive mesoporous silica nanoparticles were coated with a polycation (using two different molecular weights), providing them with gene transfection capabilities that were evaluated using two different plasmids. First, the expression of Green Fluorescent Protein was analyzed in Decidua-derived Mesenchymal Stem Cells after incubation with the silica nanoparticles. The most successful nanoparticle was then employed to induce the expression of two suicide genes: cytosine deaminase and uracil phosphoribosyl transferase, which allow the cells to convert a non-toxic pro-drug (5-fluorocytosine) into a toxic drug (5-Fluorouridine monophosphate). The effect of the production of the toxic final product was also evaluated in a cancer cell line (NMU cells) co-cultured with the transfected vehicle cells, Decidua-derived Mesenchymal Stem Cells., Comment: 24 pages, 6 figures

Abstract

Reported COVID-19 deaths in Germany are relatively low as compared to many European countries. Among the several explanations proposed, an early and large testing of the population was put forward. Most current debates on COVID-19 focus on the differences among countries, but little attention has been given to regional differences and diet. The low-death rate European countries (e.g. Austria, Baltic States, Czech Republic, Finland, Norway, Poland, Slovakia) have used different quarantine and/or confinement times and methods and none have performed as many early tests as Germany. Among other factors that may be significant are the dietary habits. It seems that some foods largely used in these countries may reduce angiotensin-converting enzyme activity or are anti-oxidants. Among the many possible areas of research, it might be important to understand diet and angiotensin-converting enzyme-2 (ACE2) levels in populations with different COVID-19 death rates since dietary interventions may be of great benefit.

Abstract

Allergic Rhinitis and its Impact on Asthma (ARIA) has evolved from a guideline by using the best approach to integrated care pathways using mobile technology in patients with allergic rhinitis (AR) and asthma multimorbidity. The proposed next phase of ARIA is change management, with the aim of providing an active and healthy life to patients with rhinitis and to those with asthma multimorbidity across the lifecycle irrespective of their sex or socioeconomic status to reduce health and social inequities incurred by the disease. ARIA has followed the 8-step model of Kotter to assess and implement the effect of rhinitis on asthma multimorbidity and to propose multimorbid guidelines. A second change management strategy is proposed by ARIA Phase 4 to increase self-medication and shared decision making in rhinitis and asthma multimorbidity. An innovation of ARIA has been the development and validation of information technology evidence-based tools (Mobile Airways Sentinel Network [MASK]) that can inform patient decisions on the basis of a self-care plan proposed by the health care professional.

Abstract

Background: Mobile technology may help to better understand the adherence to treatment. MASK-rhinitis (Mobile Airways Sentinel NetworK for allergic rhinitis) is a patient-centred ICT system. A mobile phone app (the Allergy Diary) central to MASK is available in 22 countries. Objectives: To assess the adherence to treatment in allergic rhinitis patients using the Allergy Diary App. Methods: An observational cross-sectional study was carried out on all users who filled in the Allergy Diary from 1 January 2016 to 1 August 2017. Secondary adherence was assessed by using the modified Medication Possession Ratio (MPR) and the Proportion of days covered (PDC) approach. Results: A total of 12143 users were registered. A total of 6949 users reported at least one VAS data recording. Among them, 1887 users reported >= 7 VAS data. About 1195 subjects were included in the analysis of adherence. One hundred and thirty-six (11.28%) users were adherent (MPR >= 70% and PDC <= 1.25), 51 (4.23%) were partly adherent (MPR >= 70% and PDC = 1.50) and 176 (14.60%) were switchers. On the other hand, 832 (69.05%) users were non-adherent to medications (MPR <70%). Of those, the largest group was non-adherent to medications and the time interval was increased in 442 (36.68%) users. Conclusion and clinical relevance: Adherence to treatment is low. The relative efficacy of continuous vs on-demand treatment for allergic rhinitis symptoms is still a matter of debate. This study shows an approach for measuring retrospective adherence based on a mobile app. This also represents a novel approach for analysing medication-taking behaviour in a real-world setting.

Abstract

AimsMobile Airways Sentinel NetworK (MASK) belongs to the Fondation Partenariale MACVIA-LR of Montpellier, France and aims to provide an active and healthy life to rhinitis sufferers and to those with asthma multimorbidity across the life cycle, whatever their gender or socio-economic status, in order to reduce health and social inequities incurred by the disease and to improve the digital transformation of health and care. The ultimate goal is to change the management strategy in chronic diseases.MethodsMASK implements ICT technologies for individualized and predictive medicine to develop novel care pathways by a multi-disciplinary group centred around the patients.StakeholdersInclude patients, health care professionals (pharmacists and physicians), authorities, patient's associations, private and public sectors.ResultsMASK is deployed in 23 countries and 17 languages. 26,000 users have registered.EU grants (2018)MASK is participating in EU projects (POLLAR: impact of air POLLution in Asthma and Rhinitis, EIT Health, DigitalHealthEurope, Euriphi and Vigour).Lessons learnt(i) Adherence to treatment is the major problem of allergic disease, (ii) Self-management strategies should be considerably expanded (behavioural), (iii) Change management is essential in allergic diseases, (iv) Education strategies should be reconsidered using a patient-centred approach and (v) Lessons learnt for allergic diseases can be expanded to chronic diseases.

Abstract

BackgroundMultimorbidity in allergic airway diseases is well known, but no data exist about the daily dynamics of symptoms and their impact on work. To better understand this, we aimed to assess the presence and control of daily allergic multimorbidity (asthma, conjunctivitis, rhinitis) and its impact on work productivity using a mobile technology, the Allergy Diary. MethodsWe undertook a 1-year prospective observational study in which 4 210 users and 32585days were monitored in 19 countries. Five visual analogue scales (VAS) assessed the daily burden of the disease (i.e., global evaluation, nose, eyes, asthma and work). Visual analogue scale levels <20/100 were categorized as "Low" burden and VAS levels 50/100 as "High" burden. ResultsVisual analogue scales global measured levels assessing the global control of the allergic disease were significantly associated with allergic multimorbidity. Eight hypothesis-driven patterns were defined based on "Low" and "High" VAS levels. There were <0.2% days of Rhinitis Low and Asthma High or Conjunctivitis High patterns. There were 5.9% days with a Rhinitis HighAsthma Low pattern. There were 1.7% days with a Rhinitis HighAsthma HighConjunctivitis Lowpattern. A novel Rhinitis HighAsthma HighConjunctivitis High pattern was identified in 2.9% days and had the greatest impact on uncontrolled VAS global measured and impaired work productivity. Work productivity was significantly correlated with VAS global measured levels. ConclusionsIn a novel approach examining daily symptoms with mobile technology, we found considerable intra-individual variability of allergic multimorbidity including a previously unrecognized extreme pattern of uncontrolled multimorbidity.

Abstract

The overarching goals of the European Innovation Partnership on Active and Healthy Ageing (EIP on AHA) are to enable European citizens to lead healthy, active and independent lives whilst ageing. The EIP on AHA includes 74 Reference Sites. The aim of this study was to transfer innovation from an app developed by the MACVIA-France EIP on AHA reference site (Allergy Diary) to other reference sites. The phenotypic characteristics of rhinitis and asthma multimorbidity in adults and the elderly will be compared using validated information and communication technology (ICT) tools (i.e. the Allergy Diary and CARAT: Control of Allergic Rhinitis and Asthma Test) in 22 Reference Sites or regions across Europe. This will improve the understanding, assessment of burden, diagnosis and management of rhinitis in the elderly by comparison with an adult population. Specific objectives will be: (i) to assess the percentage of adults and elderly who are able to use the Allergy Diary, (ii) to study the phenotypic characteristics and treatment over a 1-year period of rhinitis and asthma multimorbidity at baseline (cross-sectional study) and (iii) to follow-up using visual analogue scale (VAS). This part of the study may provide some insight into the differences between the elderly and adults in terms of response to treatment and practice. Finally (iv) work productivity will be examined in adults.

Abstract

Mobile technology has been used to appraise allergic rhinitis control, but more data are needed. To better assess the importance of mobile technologies in rhinitis control, the ARIA (Allergic Rhinitis and its Impact on Asthma) score ranging from 0 to 4 of the Allergy Diary was compared with EQ-5D (EuroQuol) and WPAI-AS (Work Productivity and Activity Impairment in allergy) in 1288 users in 18 countries. This study showed that quality-of-life data (EQ-5D visual analogue scale and WPA-IS Question 9) are similar in users without rhinitis and in those with mild rhinitis (scores 0-2). Users with a score of 3 or 4 had a significant impairment in quality-of-life questionnaires.

Abstract

NaFePO4 is known as a promising intercalation cathode material for sodium-ion batteries. During the electrochemical reaction, an intermediate phase forms with a composition close to Na approximate to 2/3FePO4, whose crystal structure is defined with a supercell that results from both Na/vacancy and charge ordering. In this work, we present a detailed study of this superstructure through synchrotron powder X-ray diffraction and electron microscopy studies.

Introducción: A pesar del gran avance que ha experimentado la cirugía laparóscopica en los últimos años, no ha sido hasta hace dos, cuando el Programa Nacional de la Especialidad en Cirugía General y Digestivo se ha hecho eco de esta evolución y en su formato ha especificado el número y el tipo de intervenciones que debería efectuar un residente durante su período de formación para adquirir la destreza quirúrgica necesaria en el dominio de esta nueva técnica quirúrgica. Objetivos: Evaluar la adecuación de la docencia en cirugía laparóscopica en nuestro hospital en relación al Programa Nacional de la Especialidad y al programa específico del Hospital Ramón y Cajal, examinando el número de cirugías laparoscópicas en las que el residente ha participado tanto como cirujano como ayudante durante el período comprendido entre 2000-2005. Resultados: La primera cirugía laparóscopica que realiza el residente de cirugía general es la colecistectomía, y ésta se realiza en general en el 2º año de residencia. Un residente de 5º año, cuando termina su formación, ha realizado una media de 49 colecistecomías laparoscópicas como primer cirujano y ha ayudado a una media de 56,4 cirugías (incluyendo cirugía laparóscopica avanzada). Conclusión: No existe consenso en la literatura acerca de cuál es el número ni el tipo de cirugías laparoscópicas que debe realizar el residente tras acabar su período de formación. En base al Programa Nacional de la Especialidad y al del Hospital Ramón y Cajal, pensamos que nuestros residentes reciben una correcta formación en cirugía laparóscopica.

Presentamos el caso clínico de una mujer de 54 años que consulta por una gran tumoración infraclavicular de dos años de evolución. Los resultados de la punción-aspiración de la lesión son compatibles con un tumor de células granulares. La lesión es extirpada y el estudio histológico confirma el diagnóstico. Abstract

Introduccion: Aunque el cancer de mama metastatiza con mayor frecuencia en ganglios linfaticos, hueso, pulmon e higado, tambien se puede extender hacia el tracto gastrointestinal, peritoneo y organos ginecologicos. Material y Metodo: Describimos tres casos de carcinomatosis peritoneal secundaria a la diseminacion metastasica de un carcinoma lobulillar infiltrante de mama. En los tres casos el diagnostico se hizo varios anos despues del descubrimiento del tumor primario y en dos de ellos se observa remision de la enfermedad al ano y cuatro anos respectivamente tras la administracion de quimio y hormonoterapia postoperatoria. Conclusion: La carcinomatosis peritoneal en cancer de mama es poco frecuente, pero cuando aparece casi siempre es secundaria a un carcinoma lobulillar infiltrante con receptores hormonales positivos. Los sintomas son inespecificos y normalmente existe un intervalo largo desde el diagnostico inicial del tumor, de ahi que en muchos casos sea dificil diferenciar entre un tumor primario de la cavidad peritoneal y la presencia de metastasis de un carcinoma de mama, no solo a nivel clinico, sino tambien histologico. El diagnostico resulta fundamental para el oncologo, pues con tratamiento quimio y hormo-noterapico se han descrito remisiones parciales o completas del tumor durante periodos prolongados de tiempo

Material and methods: We describe three cases of peritoneal carcinomatosis secondary to metastatic dissemination of an infiltrating lobular breast cancer. In the three cases diagnosis was realized many years after the delivering of the primary tumour and in two cases we observed tumour remission at one and four years respectively after administration of chemotherapy and hormonal therapy postoperative. Conclusion: Peritoneal carcinomatosis in breast cancer is rare, but when appears it is usually secondary to infiltrating lobular breast cancer with positive hormonal receptors. The symptoms are unspecific and normally exists a long term after the initial diagnosis of the tumour, therefore in many cases it is difficult of differentiate between a primary tumour of peritoneal cavity and metastases of a breast cancer. The diagnosis is fundamental, because with the chemotherapy and the hormonal therapy, partial or complete tumour remissions, have been described during long time

Abstract

LiFePO4 has been under intense scrutiny over the past decade because it stands as an attractive positive electrode material for the next generation of Li-ion batteries to power electric vehicles and hybrid electric vehicles, hence the importance of its thermal behavior. The reactivity of LiFePO4 with air at moderate temperatures is shown to be dependent on its particle size. For nanosized materials, a progressive displacement of Fe from the core structure leading to a composite made of nanosize Fe 2O3 and highly defective, oxidized LixFe yPO4 compositions, among which the 'ideal' formula LiFe2/3PO4. Herein we report, from both temperature-controlled X-ray diffraction and electronic diffraction microscopy, that these off-stoichiometry olivine-type compounds show a defect ordering resulting in the formation of a superstructure. Such a finding shows striking similarities with the temperature-driven oxidation of fayalite Fe 2SiO4 (another olivine) to structurally defective laihunite, reported in the literature three decades ago. © 2010 American Chemical Society.

Abstract

The thermal behavior under air of LiFePO4-based powders was investigated through the combination of several techniques such as temperature-controlled X-ray diffraction, thermogravimetric analysis and Mössbauer and NMR spectroscopies. The reactivity with air at moderate temperatures depends on the particle size and leads to progressive displacement of Fe from the core structure yielding nano-size Fe2O3 and highly defective, oxidized LixFeyPO4 compositions whose unit-cell volume decreases dramatically when the temperature is raised between 400 and 600 K. The novel LiFePO4-like compositions display new electrochemical reactivity when used as positive electrodes in Li batteries. Several redox phenomena between 3.4 V and 2.7 V vs. Li were discovered and followed by in-situ X-ray diffraction, which revealed two distinct solid solution domains associated with highly anisotropic variations of the unit-cell constants. © 2009 The Royal Society of Chemistry.

Abstract

Nanometric cobalt oxide Co3O4 powders made of 4 nm isotropic particles, directly precipitated from Co2+ aqueous solutions under alkaline and oxidizing conditions, are found to exhibit abnormal X-ray diffraction intensities, mainly in (111) and (220) reflections, because of a significant amount of displaced Co atoms from ideal 8a and 16d positions to interstitial 48f and 16c sites. Upon heating, the Co/O stoichiometry is maintained and the delocalized atoms progressively migrate to stable positions through empty neighboring sites. Despite the presence of such defects, the order of reaction (n ̃ 1) and the activation energy (Ea ̃ 60 ± 5 kJ/mol) versus the decomposition of diluted solutions of hydrogen peroxide solutions is found to be similar to bulk nonfaulted Co3O4 materials, but the intrinsic rate constants k20 °C are found to be proportionally enhanced by both the defect and microstrains levels. Last, a careful selection of monolithic samples, so as to keep away the catalytic influence of the intercrystallites reactive grainboundaries, has enabled us to precisely study and isolate the role of the structural characteristics on the catalytic activity of Co3O4 toward H2O 2 decomposition. The catalytic activity of such divided oxides toward this reaction is also put forward as a fast selection tool for catalysts in Li-oxygen secondary cells. © 2009 American Chemical Society.

Abstract

Classical electrodes for Li-ion technology operate by either single-phase or two-phase Li insertion/de-insertion processes, with single-phase mechanisms presenting some intrinsic advantages with respect to various storage applications. We report the feasibility to drive the well-established two-phase room-temperature insertion process in LiFePO"4 electrodes into a single-phase one by modifying the materials particle size and ion ordering. Electrodes made of LiFePO"4 nanoparticles (40nm) formed by a low-temperature precipitation process exhibit sloping voltage charge/discharge curves, characteristic of a single-phase behaviour. The presence of defects and cation vacancies, as deduced by chemical/physical analytical techniques, is crucial in accounting for our results. Whereas the interdependency of particle size, composition and structure complicate the theorists attempts to model phase stability in nanoscale materials, it provides new opportunities for chemists and electrochemists because numerous electrode materials could exhibit a similar behaviour at the nanoscale once their syntheses have been correctly worked out. © 2008 Macmillan Publishers Limited. Abstract

The recent infatuation for LiFePO4 as positive electrode material in Li-ion batteries has prompted a renewed interest in olivine-type structures, with a view to enhance their conduction properties. We show that the dual substitution of Li for Fe and of P for Si in the olivine LiFePO 4 phase leads to a complete solid solution Li1-xFe 1+xP1-xSixO4 as deduced from combined X-ray diffraction, Mössbauer, and NMR experiments. Our findings challenge the common belief that the anionic network cannot be substituted. Moreover, it is found that such a substitution promotes Li intersite mixing between the olivine M1 and M2 sites. Such mixing, together with the worsening of the conducting properties of the dually substituted samples, is believed to be responsible for the poor electrochemical performances of the member's series. Beyond x = 0.20, the samples were electrochemically inactive. While the current materials are disappointing application-wise, such a study provides clues to the rich chemistry remaining to be unveiled with olivine-type structures in particular and polyanionic compounds in general. © 2008 American Chemical Society.

Abstract: A systematic study of the Li–Si–(O)–N system is presented. The synthetic conditions to prepare Li2SiN2, Li5SiN3, Li18Si3N10, Li21Si3N11 and Li7SiN3O are described and the structure of the last two compounds has been solved for the first time. While Li21Si3N11 crystallizes as a superstructure of the anti-fluorite structure with Li and Si ordering, Li7SiN3O exhibits the anti-fluorite structure with both anion and cation disorder.
