1. Pre-operative bariatric patient characteristics driving hiatal hernia repair decision by operating surgeons

Abstract

Background: Hiatal hernia (HH) is routinely reported in 40% of bariatric surgery patients. Left unrepaired, HH can lead to post-surgical reflux, regurgitation, and vomiting. Objectives: We hypothesize that patients with pre-operative reflux symptoms and a higher body mass index (BMI) will receive hiatal hernia repairs (HHR) more often. The study aim was to analyze the variables that drive HHR decision by operating surgeons. Methods: The records of 551 patients who underwent endoscopy in preparation for bariatric surgery were analyzed. Prevalence of HH was derived based on esophagogastroduodenoscopy (EGD) findings performed by a bariatric surgeon during patients’ bariatric surgery. The relationship between categorical participant attributes was calculated using a significance level of 0.05. Results: The groups consisted of 295 Roux-en-Y gastric bypass (RYGB) and 264 sleeve gastrectomy (SG) patients with preoperative HH identified in 310 patients. SG and a decreased BMI were significant for receiving a HHR. Type II diabetes (T2D), duodenitis found on EGD and pathology report, esophagitis, and Roux-en-Y gastric bypass (RYGB) were significant for not receiving a HHR. Only duodenitis, RYGB, and SG were found to be significant factors after multivariate analysis. Conclusions: While some pre-operative patient characteristics may not impact a surgeon's HHR decision in the bariatric population, our study suggests that duodenitis, SG, and RYGB may influence a surgeon's HHR decision.

2. Observation of continuum radiations from the Cluster fleet: first results from direction finding

Abstract

The Cluster fleet offers the first possibility of comparing non-thermal terrestrial continuum radiation from similarly equipped nearby observation points. A very rich data set has already been acquired on the Cluster polar orbit, between 4 and 19 Earth radii geocentric distances, and preliminary analysis has been carried out on these emissions. We focus in this paper on direction finding performed from all four spacecraft as a means to locate the position of the sources of this continuum radiation. Directions are derived from spin modulation properties, under the usual analysis assumptions of the wave vector of the radiation lying in the plane containing the spin axis and the antenna position at electric field minimum. All the spin axes of the four Cluster spacecraft are aligned perpendicular to the ecliptic, thus the aligned spacecraft antenna spin planes provide redundant 2-D views of the propagation path of the radiation and source location. Convincing 2-D triangulation results have been obtained in the vicinity of the source region. In addition, the out of spin plane component of the wave vector reveals itself to a certain extent through directivity characteristics compared at different distances of the spin plane to the ecliptic. The four case events studied (two of them taken near apogee, the other two near perigee) have confirmed general properties derived from previous observations: trapping in the lower frequency range, radiation escaping into the magnetosheath region in the higher frequency range. All propagation directions are compatible with source positions in the plasmapause region, however, at a significant distance from the equator in one case. Our observations have also revealed new properties, like the importance of small-scale density irregularities in the local amplification of continuum radiation. We conclude that more detailed generation and propagation models are needed to fit the observations.

Abstract

Killian-Jameson Diverticulum (KGD) is similar to Zenker's diverticulum(ZD) in respect to underlying pathology of protrusion of esophageal mucosa and vague foregut symptoms including dysphagia. However, KGD is distinctly different from ZD due to location of diverticulum, incidence, and preferred surgical approach. Methods We present a case of a patient who had previously undergone a transcervical diverticulectomy for JKD. He initially had symptomatic relief but eight months later developed a recurrence of his diverticulum with dysphagia. A per-oral endoscopic myotomy (POEM) was then performed. A video of this endoscopic intervention can be seen in this submission, along with appropriate commentary. Results The patient had resolution of his dysphagia and he recovered without complication. Conclusion This novel approach to recurrent Killian-Jameson Diverticulum after the more standard open transcervical approach should be considered for this challenging clinical scenario. The basic principles of an endoscopic myotomy can be applied similarly to such a diverticulum. Abstract

The high-spin states of the neutron-rich Ga81, with three valence protons outside a Ni78 core, were measured. The measurement involved prompt γ-ray spectroscopy of fission fragments isotopically identified using the combination of the variable mode spectrometer (VAMOS++) and the advanced gamma tracking array (AGATA). The new γ-ray transitions, observed in coincidence with Ga81 ions, and the corresponding level scheme do not confirm the high-spin levels reported earlier. The newly observed high-spin states in Ga81 are interpreted using the results of state-of-the-art large-scale shell model (LSSM) calculations. The lower excitation energy levels are understood as resulting from the recoupling of three valence protons to the closed doubly magic core, while the highest excitation energy levels correspond to excitations of the magic N=50 neutron core. These results support the doubly magic character of Ni78 and the persistence of the N=50 shell closure but also highlight the presence of strong proton-neutron correlations associated with the promotion of neutrons across the magic N=50 shell gap, only few nucleons away from Ni78.

Abstract

Relatively neutron-rich thorium isotopes lie at the heart of a nuclear region of nuclei exhibiting octupole correlation effects. The detailed level structure of Th223 has been investigated in measurements of γ radiation following the fusion-evaporation channel of the Pb208(O18,3n)Th223 reaction at 85 MeV beam energy. The level structure has been extended up to spin 49/2, and 33 new γ rays have been added using triple-γ coincidence data. The spins and parities of the newly observed states have been confirmed by angular distribution ratios. In addition to the two known yrast bands based on a K=5/2 configuration, a non-yrast band has been established up to spin 35/2. We interpret this new structure as based on the same configuration as the yrast band in Th221 having dominant K=1/2 contribution. At the highest spin a backbending occurs around a rotational frequency of ℏω=0.23 MeV, very close to the one predicted in Th222, where a sharp transition to a reflection-symmetric shape is expected.

Abstract

High-spin states in $^{139}\mathrm{Ce}$ have been populated using the $^{130}\mathrm{Te}(^{14}\mathrm{C},5n)$ reaction. The level scheme has been extended to higher spins, including a new band of dipole transitions. The parity of several states has been changed from negative to positive, mainly based on the comparison with the level structure of the core nucleus $^{140}\mathrm{Ce}$ and the results of a realistic shell-model calculation. The dipole band is interpreted as a magnetic rotation band with $\ensuremath{\pi}{h}_{11/2}^{2}\ensuremath{\bigotimes}\ensuremath{\nu}{h}_{11/2}^{\ensuremath{-}1}$ configuration built on small deformation axial shape with $({\ensuremath{\varepsilon}}_{2}=0.12,\ensuremath{\gamma}={0}^{\ensuremath{\circ}})$.

Abstract

The population of the high-spin isomers in ${}^{139}$Nd and ${}^{140}$Nd was investigated using the ${}^{96}$Zr(${}^{48}$Ca,$xn$) reaction and the JUROGAM + RITU + GREAT setup employing the recoil decay tagging technique. Three transitions feeding the 23/2${}^{+}$ isomer in ${}^{139}$Nd and two transitions feeding the 20${}^{+}$ isomer in ${}^{140}$Nd were identified. The newly observed transitions allowed the excitation energy of the isomer to be established in ${}^{139}$Nd and to assign configurations to the states and bands deexcited by the observed transitions in both nuclei.

Abstract

Expérience GANIL E521aS/SPIRAL; International audience; The unbound nucleus $^{18}$Na, the intermediate nucleus in the two-proton radioactivity of $^{19}$Mg, was studied by the measurement of the resonant elastic scattering reaction $^{17}$Ne(p,$^{17}$Ne)p performed at 4 A.MeV. Spectroscopic properties of the low-lying states were obtained in a R-matrix analysis of the excitation function. Using these new results, we show that the lifetime of the $^{19}$Mg radioactivity can be understood assuming a sequential emission of two protons via low energy tails of $^{18}$Na resonances.

Abstract

The unbound nucleus 18 Na , the intermediate nucleus in the two-proton radioactivity of 19 Mg , is studied through the resonant elastic scattering 17 Ne ( p ,17 Ne ) p . The spectroscopic information obtained in this experiment is discussed and put in perspective with previous measurements and the structure of the mirror nucleus 18 N .

Abstract

The 8Li+4He elastic scattering excitation function was measured by using the inverse kinematic thick target method. The 8Li beam was provided by the LNS radioactive facility EXCYT at an energy of 30 MeV and it was delivered into a large scattering chamber filled with 4He gas. The detection system was made by three silicon telescopes and one MCP. This last detector was used to measure the number of incident particles as well as the time of flight allowing for the discrimination between elastic and inelastic scattering. The setup and the experimental technique will be described in details and the results, as well as the comparison of the data with an R-matrix calculation, will be shown and discussed.

Abstract

We present a study of the excitation spectra and of the spectral functions for the elementary excitations in superfluid neutron star matter below the crust. We restrict the analysis to the proton and electron components within the Random Phase Approximation generalized to the superfluid case. The excitation spectrum shows the interplay of these two components and their relevance for the excitation modes. The results are relevant for neutrino physics and thermodynamical processes in neutron stars.

Abstract

The 8Li elastic scattering on 4He in inverse kinematics has been studied in order to search for 8Li-α cluster configurations in excited states of 12B. The experimental technique, the inverse kinematic thick target method (TTIK), involved a 8Li beam provided by the EXCYT facility at an initial energy of Eb=30.6 MeV which is continuously degraded down to zero in the passage through a helium extended thick target. After the interaction, the recoil alpha particles were detected by using four ΔE-E silicon detector telescopes. The explored 8Li-α centre-of-mass energy ranges from ECM=2.7 MeV up to ECM=9.6 MeV. A micro-channel plate, placed before the gas chamber, is used to measure the time interval between the entrance of the 8Li into the chamber and the arrival of the recoil alpha particle on the detectors. This "time interval" measurement improves the TTIK technique allowing the discrimination of elastic from inelastic events. In this paper, the experimental technique will be described and the preliminary results for the elastic cross section will be shown.

13. Exploring the paradigm of robotic surgery and its contribution to the growth of surgical volume.

Abstract

Background: Robotic surgery is an appealing option for both surgeons and patients. The question around the introduction of new surgical technology, such as robotics, with the potential link to increased procedure-specific volume has not been addressed. We hypothesize that hospital adoption of robotic technology increases the total volume of specific procedures as compared to nonrobotic hospitals., Methods: The 2010-2020 Florida Agency for Health Care Administration Inpatient database was queried for open, laparoscopic, and robotic colectomy, lobectomy, gastric bypass, and antireflux procedures. International Classification of Diseases, 9th and 10th Revisions, codes were used. Difference in difference method was used to evaluate the impact of robotics on total procedure-specific volume of robotic hospitals versus nonrobotic hospitals before and after adopting robotic technology. Incident rate ratios from the difference in difference analysis determined the significance of adding robotics. Patient demographics were evaluated using χ 2 test., Results: A total of 291,826 procedures were performed at 217 hospitals, 151 with robotic capabilities. Robotic hospitals experienced a 37% increase in surgical volume due to robotic technology (incident rate ratio 1.37, P < .05), which was consistent for each surgery except antireflux procedures (incident rate ratio 0.95). Robotic procedures had significantly higher charges for medical/surgical supplies; however, the mean length of stay for robotic procedures was significantly shorter than that of laparoscopic and open cases., Conclusion: Hospital adoption of robotic technology significantly increases surgical volume for select procedures. 14. Narrative review of management controversies for paraesophageal hernia.

Abstract

Objective: To review management controversies in paraesophageal hernia and options for surgical repair., Background: Paraesophageal hernia is an increasingly common problem. There are controversies over whether and when paraesophageal hernias should be surgically repaired. In addition, if these hernias are to be repaired, the method of repair, need for mesh reinforcement, need for fundoplication, and need for gastropexy are not uniformly accepted., Methods: Recent literature was reviewed on need for repair, approach (open, laparoscopic or robotic surgery), method of repair (primary suture, use of relaxing incisions, use of mesh reinforcement), materials and configuration of mesh reinforcement, need and type of fundoplication, and need for gastropexy, with emphasis on surgical outcomes., Conclusions: The extant literature suggests that paraesophageal hernia should be approached in a patient-centered, precision medicine manner. In general, hernia reduction, sac excision and primary suture approximation of the hiatal crura are mandatory. Use of mesh should be based on individual risk factors; if mesh is used, biological meshes appear to have a more favorable safety profile, with the "reverse C" or keyhole configuration allowing for increase in crural tensile strength at it most vulnerable areas. Use and choice of fundoplication or magnetic sphincter augmentation should be based on individual considerations. Abstract

Background/aim: Immunotherapy combined with surgery is associated with better survival than surgery alone in patients with advanced melanoma. This study examined the utilization of immunotherapy in relation to population characteristics and the associated survival benefit., Materials and Methods: This was a retrospective cohort study utilizing the US National Cancer Database. The study population included 6,165 adult patients (≥18 years) with stage III cutaneous melanoma (median follow-up=32 months)., Results: A total of 1,854 patients underwent immunotherapy in addition to surgery, which was associated with a survival benefit over surgery alone (hazard ratio(HR)=0.66, 95% confidence interval(CI)=0.56-0.77, p<0.001). Older age, presence of comorbidities, Medicaid/Medicare insurance, and living in a community with lower average education level were associated with less immunotherapy utilization (all p<0.05). No statistically significant racial disparity in immunotherapy usage was found (p=0.07)., Conclusion: Compared to other demographic factors, insurance status was associated with the greatest disparities in immunotherapy utilization and mortality for patients who underwent surgery for advanced melanoma., (Copyright© 2018, International Institute of Anticancer Research (Dr. George J.
