Abstract

Holoprosencephaly is a brain abnormality resulting from an incomplete cleavage of the prosencephalon during early embryogenesis. Uncontrolled maternal diabetes mellitus is one of the strongest human teratogens and the most well-known cause of holoprosencephaly. We report a rare case of holoprosencephaly in a male fetus in a preexisting diabetic pregnancy. The diagnosis of the holoprosencephaly was established by ultrasound scan and MRI data. 15. Fertility-sparing surgery: a hopeful strategy for young women with cancer.

Abstract

Fertility preservation in cancer patients is currently based on either assisted reproductive technology or fertility-sparing surgery. Loss of fertility may be caused by excisional surgery associated with an adnexal or uterine pathology or secondary to gonadal insufficiency caused by chemotherapy or radiation. The counseling of these patients is very important, being carried out jointly by the oncologist, gynecologist, and reproductive medicine specialist. Reproductive surgery usually requires avoiding laparotomy to significantly reduce the formation of adhesions and trauma or tissue damage. This is done using standard laparoscopic surgery or robotic surgery (computer-assisted laparoscopy), a method increasingly used and accessible to all specialists who want to maintain the fertility of their patients with various oncological diseases. Abstract

The number of primiparous pregnant women, over the age of 35, has increased considerably in the past few years, because of the elevated level of education and also because the multitude of contraceptive methods. Such a patient suits the category of elderly primiparous and it represents a real obstetrical challenge, due to the numerous complications related to pregnancy and the immediate postpartum, as well as those related to the neonatal prognosis. We present a 35 old years patient, second gestation, primiparous, a partially monitored pregnancy, known with essential hypertension grade III ESC / ESA, insufficiently controlled therapeutically, with target organ damage (exudative retinal detachment, hypertensive choroidopathy), morbid obesity and also with the suspicion of a renal tumor. The rate of complications occurring in the case of an elderly primiparous was increased in the present case. Obstetrical complications occurred: preeclampsia, HELLP (hemolysis, elevated liver enzymes, low platelet count) syndrome, placental abnormalities (placenta accreta), intrauterine growth restriction, chronic fetal distress and finally fetal death in utero. The evolution of the patient in postpartum was burdened by the decompensation of the pre-existing pathology. In conclusion, elderly primiparous is susceptible to obstetrical complications during pregnancy and childbirth, such as premature birth, fetal acute distress, which may result in fetal death, placental abruption, placental abnormalities, preeclampsia, eclampsia, therefore following up the pregnancy requires increased attention and is also a real challenge for the obstetrician.

Abstract

Preeclampsia is an important health problem with a higher prevalence in the adolescent population. Furthermore, preeclampsia causes adverse maternal and neonatal outcomes. Newborns can be affected by preeclampsia, resulting in lower birth weight or Apgar score, the need for neonatal intensive care, or prematurity. All these complications are also associated with adolescent pregnancies, and together with preeclampsia, it can determine poorer neonatal outcomes. The aim of the study was to compare the neonatal outcomes of adolescents and adults with preeclampsia. We analyzed data on all the newborns of adolescents with preeclampsia (n=12) who delivered at the Department of Obstetrics and Gynecology of University Emergency Hospital in Bucharest between January 1st, 2019, and December 31st, 2019 and compared it with data from 12 aleatory newborns of adults diagnosed with preeclampsia. The prevalence of preeclampsia was higher in the adolescent population compared with the adult one. The weight of newborns was lower among adolescents with preeclampsia. There were no significant differences in Apgar scores between the two groups. Preterm delivery was more frequent in adolescent patients with preeclampsia. Preeclampsia is an additional risk factor for adolescent pregnancy, but it is also a severe materno-fetal complication for this population. [ABSTRACT FROM AUTHOR]

Abstract

Breast carcinoma is the most important cause of death in women's oncologic pathology worldwide. Due to its high frequency, important advances in diagnostic and management of this entity were made. Because of its rarity (only 0.5-1% in all breast cancers), little is known about male breast cancer, and no specific male standardised protocol was made, treatment and follow-up being adopted from women breast cancer guidelines. We consider that this entity, with its hormone specific profile, is worth being considered for further studies, taking into account that male breast cancer has a far worse prognostic than female breast cancer.We present our four-year experience with male breast lesions in a retrospective review study, including gynecomastia and breast carcinoma cases.We identified 17 male breast lesions, 11 of them with a diagnosis of gynaecomastia, the rest being breast carcinomas. Four malignant lesions were encountered in the 5th, 6th and 7th decade, with two of them in young adults (35 and 40 years old). In a single case, two different histotypes of breast carcinoma were found. All of them were G2, with tumoral stages varying between IB to IV. Androgen receptors were expressed in each case, in different percentages.Male breast carcinoma is a rare entity with a poor prognosis, a diffuse and profound invasion of adjacent structures, and a high expression of hormone receptors, and with no histologic relation to gynaecomastia, considering that no associated benign lesions were found.
