The effects of COVID-19 remained significant on the social and academic well-being of university students. This raised various mental and psychological challenges for the students, which had the potential to deteriorate their academic engagement and performance in online learning environments. The emotion regulation theory hypothesizes that various cognitive skills may assist people in coping with such types of social and psychological challenges. Cognitive reappraisal is a strategy used at the individual level to regularize thought processes in stressful situations. This study aimed to investigate the influence of cognitive reappraisal “as a mediator” on the relationships between COVID-19 related stressors and university students’ engagement in an online learning environment. Three hundred and one students selected from eight public sector universities in Pakistan participated in a survey. Three instruments, including the COVID-19 Related Stressors, Cognitive Reappraisal Questionnaire, and Student Engagement Dimensions, were used to collect data. Mediation analysis confirmed the application of emotion regulation theory in the context of relationships between COVID-19 related stressors and students’ engagement in online learning environments. The study has significant implications for promoting students’ well-being in online setups during stressful situations.

4. Bovine lactoferrin alleviates aflatoxin B1 induced hepatic and renal injury in broilers by mediating Nrf2 signaling pathway

ABSTRACT: Aflatoxin B1 (AFB1) a mycotoxin found in chicken feed that possess a global hazard to poultry health. However different potent compounds like bovine lactoferrin (bLF) may prove to be protective effects against AFB1. This study aims to explore the protective effect of bLF against AFB1-induced injury in the liver and kidney in broiler. For this purpose, 600 broilers chicks were randomly alienated into 5 groups (n = 120 each): negative control; positive control (3 mg/kg AFB1), and bLF high, medium, and low dosage groups (600 mg/kg, 300 mg/kg, and 150 mg/kg, respectively). The results highlight that AFB1 toxicity in birds exhibited low feed intake, reduction in weight gain, and a decrease in FCR while, bLF regulated these adverse effects. Meanwhile, AFB1 group showed higher levels of alanine transaminase (ALT) and aspartate aminotransferase (AST) and lower levels of superoxide dismutase (SOD) and glutathione (GSHpx) in liver, while urea and creatinine were decline in kidney. Supplementation with bLF effectively controlled these biomarkers and control the negative effects of toxicity. Furthermore, hematoxylin and eosin (H&E) staining exhibited normal morphological structures within liver and kidney in the bLF treated groups, while degenerative changes were observed in AFB1 group. Similarly, bLF, decreased oxidative stress and thus prevented apoptosis in the liver and kidney cells of the birds. Whereas, mRNA level of mitochondrial apoptosis related gene including Bcl-2 (Bak and Bax), caspase-3 and caspase-9 was upregulated, while bcl2 gene were downregulated in AFB1 group. Dietary supplementation of bLF effectively normalizes the expression of these genes. AFB1 exposed birds shown to decrease gene expression level of the crucial component of Nrf2 pathway, responsible to regulate antioxidant defense. Interestingly, bLF reverse these detrimental effects of and restore the normal expression levels of Nrf2 pathway. Conclusively, our findings demonstrate that bLF mitigates the detrimental effects of AFB1, besides regulation of the apoptosis-related genes via mitochondrial pathways. These findings validate that the bLF (600 mg/kg) could be used as protective agent against AFB1-induced liver and kidney damage.

5. BSCL2 Gene Mutation Causing Progressive Encephalopathy with or without Lipodystrophy, A Case Report from Pakistan

Celia’s encephalopathy or Progressive encephalopathy with or without lipodystrophy (PELD) is a rare childhood neurodegenerative syndrome, that is autosomal recessive mendelian trait, It is Seipin protein associated encephalopathy and results in regression of developmental mile stones. We report a case with global developmental delay and hypertonia, hyperreflexia, recurrent infections and dyskinesia. The diagnosis was done by exome sequencing revealing mutation in the BSCL2 gene on chromosome 11q13. The cases was very rare and found to mimic other diseases that is why patient was investigated and reported. The study was done to identify report and manage such cases appropriately. The patient was investigated in OPD in 2021. Whole exome sequencing (WES) was done for diagnosis after taking appropriate blood sample. A protein change p.(trp323leufs*15) was identified due to homozygous frameshift elongation likely pathogenic on chromosome 11 in BSCL gene.p. (trp323leufs*15) creates a shift in reading frame starting at codon 323. The new frame ends in stop codon after 15 proteins downstream. This new variant is absent in genomAD and local database. This variant has also not been reported yet in clinVar. This variant is classified as likely pathogenic class 2 based on ACMG recommendations. Any patient was favoring symptoms and features should be investigated with whole genome sequencing to confirm the diagnosis.

Abstract Trauma scientists have raised the alarm about the devastating consequences of the Ukraine war on mental health. We examined how higher education students—as indirect victims—coped with this conflict and how they emotionally reacted during 2022. We involved 2314 students from 16 countries through an online survey. A structural equation model indicated significant relations between war-related worry about military and macroeconomics domains and two coping strategies (opposition, support giving), in turn significantly linked with six emotions. The model was strongly invariant across gender, study field, and geographic area. The most frequent emotions were anger and anxiety, followed by two future-centred emotions (hopelessness and hope). Emotions were more frequent for females and students of the countries geographically close to the war region. Our findings call for evidence-based policy recommendations to be implemented by institutions to combat the negative short and long-term psychological sequelae of being witnesses of armed conflicts.

10. Gaps between Acquired and Required Teacher Education Graduate Attributes: Does Accreditation Influence in Pakistan?

Purpose: The major purpose of the present study was to identify the gaps between acquired and required graduate attributes of pass out graduates of teacher education programs in Pakistan and investigate the influence of program accreditation on these gaps. Research Method: A quantitative research method was used to achieve the research objectives. 160 graduates of the accredited and nonaccredited master level teacher education programs associated with 24 different universities in Punjab and Islamabad Capital Territory participated in the study. A questionnaire based on national professional standards for teachers (NPSTs) in Pakistan was developed to collect the data. Findings: The study demonstrates that the participants' acquired attributes do not match the required attributes of their job. The gaps between acquired and required graduates' attributes are comparatively smaller among the graduates of accredited programs. The study supports the positive effects of accreditation on the output of teacher education programs. Implications for Research and Practice: The findings of this study strengthen the idea that a well framed accreditation mechanism positively influences the products of professional programs. Further research may be conducted on making teacher education accreditation programs more effective in Pakistan with respect to the graduate attributes.

11. Antibacterial and in vivo toxicological studies of Bi2O3/CuO/GO nanocomposite synthesized via cost effective methods

Abstract In this research work, Bi2O3, Bi2O3/GO and Bi2O3/CuO/GO nanocomposites have been synthesized via an eco-friendly green synthesis technique, solgel route and co-precipitation method respectively for the assessment of antibacterial activity as well as in vivo toxicity. The XRD patterns confirm the formation of Bi2O3, Bi2O3/GO and Bi2O3/CuO/GO nanocomposites showing monoclinic structures. Crystallite size and lattice strain are calculated by Scherrer equation, Scherrer plot and Willimson Hall plot methods. Average crystallite size measured for Bi2O3, Bi2O3/GO and Bi2O3/CuO/GO nanocomposites by Scherrer equation, Scherrer plot and WH-plot methods are (5.1, 13.9, 11.5)nm, (5.4, 14.2, 11.3)nm and (5.2, 13.5, 12.0)nm respectively. Optical properties such as absorption peaks and band-gap energies are studied by UV–vis spectroscopy. The FTIR peaks at 513 cm−1, 553 cm−1 and 855 cm−1 confirms the successful synthesis of Bi2O3, Bi2O3/GO and Bi2O3/CuO/GO nanocomposites. The antibacterial activity of synthesized Bi2O3, Bi2O3/GO and Bi2O3/CuO/GO nanocomposites is examined against two gram-negative (Escherichia coli and pseudomonas) as well as gram-positive bacteria (Bacillus cereus and Staphylococcus aureus) at dose 25 mg/kg and 40 mg/kg by disk diffusion technique. Zone of inhibition for Bi2O3, Bi2O3/GO and Bi2O3/CuO/GO at dose 40 mg/kg against E. coli (gram − ve) are 12 mm, 17 mm and 18 mm respectively and against Pseudomonas (gram − ve) are 28 mm, 19 mm and 21 mm respectively. While the zone of inhibition for Bi2O3/GO and Bi2O3/CuO/GO at dose 40 mg/kg against B. cereus (gram + ve) are 8 mm and 8.5 mm respectively and against S. aureus (gram + ve) are 5 mm and 10.5 mm respectively. These amazing results reveal that Bi2O3, Bi2O3/GO and Bi2O3/CuO/GO nanocomposite as a kind of antibacterial content, have enormous potential for biomedical applications. In addition, the in vivo toxicity of synthesized Bi2O3/CuO/GO nanocomposite is investigated on Swiss Albino mice at dose of 20 mg/kg by evaluating immune response, hematology and biochemistry at the time period of 2, 7, 14 and 30 days. No severe damage is observed in mice during whole treatment. The p value calculated by statistical analysis of hematological and biochemistry tests is nonsignificant which ensures that synthesized nanocomposites are safe and non-toxic as they do not affect mice significantly. This study proves that Bi2O3/CuO/GO nanocomposites are biocompatible and can be explored further for different biomedical applications.

Objective: To assess the acceptance of the COVID-19 vaccine among medical and non-medical undergraduate students and compare the COVID-19 vaccine receptivity differences between medical and non-medical undergraduate students. Study Design: Cross-sectional study. Place and Duration of Study: Bahria University Medical & Dental College, and Bahria University Karachi Campus Pakistan, from Mar to Jun 2021. Methodology: In this study, a total of 392 participants, were selected by non-probability convenience sampling. Students aged 19-26 years were enrolled. Participants were required to sign a voluntary consent form prior to participation. The questionnaire included 21 questions inclusive of demographic characteristics, insight regarding COVID-19 disease, prior EPI vaccine exposure, attitude towards COVID-19 vaccine, including the importance of vaccine to end the pandemic, acceptance of vaccine along with reasons and non-acceptance and sent by email as a Google form to participants. Results: There were 171(43.6%) males and females 218(55.6%). The mean age of participants was 20.7±2.29 years. 217(55.4%) students were from the MBBS programs, and 175(44.6%) were from non-MBBS programs. COVID-19 vaccine acceptability was better amongst medical students, with 182(83.9%) considering it safe for use in contrast to 5(2.8%) non-medical students who considered it safe. Conclusion: The comparison of knowledge and attitudes towards accepting the COVID-19 vaccine between medical students and non-medical students revealed better acceptance amongst undergraduate medical students than non-medical undergraduate students.

14. Emotional Intelligence as a Determinant of Readiness for Online Learning

- Abstract
Students' performance in online learning environments is associated with their readiness to adopt a digital learning approach. Traditional concept of readiness for online learning is connected with students' competencies of using technology for learning purposes. We in this research, however, investigated psychometric aspects of students' preparedness for online learning. Purpose of the activity was to examine students' emotional intelligence as a determinant of their readiness for online learning. 432 learners enrolled in distance learning programs participated in the study. Wong and Law emotional intelligence scale (WLEIS) and online learning readiness scale (OLRS) were adapted to collect the required data. Regression coefficient portrays a large predictive contribution of students' emotional intelligence in explaining variance in their readiness for e-learning.

The market for nanoparticles has grown significantly over the past few decades due to a number of unique qualities, including antibacterial capabilities. It is still unclear how nanoparticle toxicity works. In order to ascertain the toxicity of synthetic cobalt iron oxide (CoFe2O4) nanoparticles (CIONPs) in rabbits, this study was carried out. Sixteen rabbits in total were purchased from the neighborhood market and divided into two groups (A and B), each of which contained eight rabbits. The CIONPs were synthesized by the co-precipitation method. Crystallinity and phase identification were confirmed by X-ray diffraction (XRD). The average size of the nanoparticles (13.2 nm) was calculated by Scherrer formula (Dhkl = 0.9 λ/β cos θ) and confirmed by TEM images. The saturation magnetization, 50.1 emug−1, was measured by vibrating sample magnetometer (VSM). CIONPs were investigated as contrast agents (CA) for magnetic resonance images (MRI). The relaxivity (r = 1/T) of the MRI was also investigated at a field strength of 0.35 T (Tesla), and the ratio r2/r1 for the CIONPs contrast agent was 6.63. The CIONPs were administrated intravenously into the rabbits through the ear vein. Blood was collected at days 5 and 10 post-exposure for hematological and serum biochemistry analyses. The intensities of the signal experienced by CA with CIONPs were 1427 for the liver and 1702 for the spleen. The treated group showed significantly lower hematological parameters, but significantly higher total white blood cell counts and neutrophils. The results of the serum biochemistry analyses showed significantly higher and lower quantities of different serum biochemical parameters in the treated rabbits at day 10 of the trial. At the microscopic level, different histological ailments were observed in the visceral organs of treated rabbits, including the liver, kidneys, spleen, heart, and brain. In conclusion, the results revealed that cobalt iron oxide (CoFe2O4) nanoparticles induced toxicity via alterations in multiple tissues of rabbits.

Texture analysis is an outstanding and fundamental task being used in many medical and computer vision applications. Malfunctioning of the human liver upsets almost all the other organs in the human body. Usually liver infections are difficult to analyze because of inconclusive side effects. More often, the liver could be confronting critically but it may not be significantly unveiled. The main objective of this research work is to provide some standard liver diagnostic measures to minimize the risk factors, as better diagnosis is essential requirement in radiology. The Computerized Tomography (CT) contributes important information to the clinical evaluation of diffuse liver diseases. Haralick texture parameters have been computed on the selected Regions of Interest. B11 is used for discrimination and interpretation of normal and cirrhotic liver diseases. Normal and diseased Liver CT images were collected from Bahawal Victoria Hospital. Normal and cirrhotic liver samples of clinically verified patients were obtained and total 900 Regions of Interest (ROIs) were taken from the selected data. Training of the classes was next step after texture parameter computation. In this work, supervised classification method was used to classify the selected images. In this way, the classes were trained in a supervised manner. The maximum accuracy obtained during this research work was 100%, linear dimensionality was 1 and the linear separability was 0.99%. Results of this research work suggested that texture parameters have high degree of reliability to automatically discriminate similar tissue textures, when regions are obvious. This framework separates benign from malignant liver tumors with moderately high precision and is therefore link up the psychophysics with machine vision to outline, recognize, categorize or discriminate textures.

Due to depletion of fossil fuel and their environmental effect, renewable resources are the prime interest in which fuel cell technology competes with fossil fuels but their ORR at cathode restrict its efficiency. Perovskite type orthorhombic structure of LaFeO3 incorporated GO has ability to use as catalyst in low and high temperature fuel cells having non-agglomerated particles in nanometers range (20- 100), high temperature stability, low overpotential of about 0.6V, ECSA of about 13.22 m2g-1 and current density of about 0.012 mAcm-1. Results open further research directions in supercapacitors, batteries, biological fuel cells, and applications which requires high temperature stability.

In the translational arm of the ATAC (anastrozole or tamoxifen alone or combined) trial (TransATAC) we have assessed individual and multiparameter biomarkers for their prediction of overall and distant recurrence. None of the biomarkers identified differential benefit for anastrozole versus tamoxifen. Each of ER, PgR, HER2 and Ki67 was associated with risk of recurrence. A combination of these to create a single predictor IHC4 was as informative as the 21-gene recurrence score (RS). Purpose We recently reported that the mRNA-based, 21-gene Genomic Health recurrence score (GHI-RS) provided additional prognostic information regarding distant recurrence beyond that obtained from classical clinicopathologic factors (age, nodal status, tumor size, grade, endocrine treatment) in women with early breast cancer, confirming earlier reports. The aim of this article is to determine how much of this information is contained in standard immunohistochemical (IHC) markers. Patients and Methods The primary cohort comprised 1,125 estrogen receptor–positive (ER-positive) patients from the Arimidex, Tamoxifen, Alone or in Combination (ATAC) trial who did not receive adjuvant chemotherapy, had the GHI-RS computed, and had adequate tissue for the four IHC measurements: ER, progesterone receptor (PgR), human epidermal growth factor receptor 2 (HER2), and Ki-67. Distant recurrence was the primary end point, and proportional hazards models were used with sample splitting to control for overfitting. A prognostic model that used classical variables and the four IHC markers (IHC4 score) was created and assessed in a separate cohort of 786 patients. Results All four IHC markers provided independent prognostic information in the presence of classical variables. In sample-splitting analyses, the information in the IHC4 score was found to be similar to that in the GHI-RS, and little additional prognostic value was seen in the combined use of both scores. The prognostic value of the IHC4 score was further validated in the second separate cohort. Conclusion This study suggests that the amount of prognostic information contained in four widely performed IHC assays is similar to that in the GHI-RS. Objective: To characterize human liver tissues by demonstrating the ability of machine vision, and to propose a new auto-generated report based on texture analysis that may work with co-occurrence matrix statistics. Method: The retrospective study was conducted at Bahawal Victoria Hospital (BVH), Bahawalpur, Pakistan, and comprised clinically verified computed tomography imaging data between October 2018 and September 2020. The image samples and related data were used to segregate classes 1-4. Appropriate image classes belonging to the same disease were trained to confirm the abnormalities in liver tissues using supervised learning methods, principal component analysis, linear discriminant analysis, and non-linear discriminant analysis. Robust and reliable texture features were investigated by generating testing classes. Overall performance of the presented machine vision approach was analyzed using four parameters; precision, recall/sensitivity, F1-score, and accuracy. Statistical analysis was done using B11 software. Results: There were 312 image samples from 71 patients; 51(71.8%) males and 20(28.2%) females. Among the patients, 19(26.7%) had abscess, 15(21.1%) had metastatic disease, 23(32.4%) had tumour necrosis, 6(8.5%) had vascular disorder, and 8(11.3%) were normal. Principal component analysis, linear discriminant analysis, and non-linear discriminant analysis showed high >97.86% values, but the discrimination rate was 100% for class 4. Conclusion: Abnormalities in the human liver could be discriminated and diagnosed by texture analysis techniques using second-order statistics that may assist the radiologist and medical physicists in predicting the severity and proliferation of abnormalities in liver diseases. - Abstract
The current study aims to investigate the possibilities of developing reflective thinking among learners through distance education programs. The case of Allama Iqbal Open University (AIOU) Islamabad, Pakistan is examined to achieve this task. The study is based on Mezirow's theory of reflective thinking, which divides thinking in four categories. The "Questionnaire of Reflective Thinking" ("QRT") developed by Kember et al. (2000) based on Mezirow (1991) was administered to 450 AIOU students. The thinking and learning practices of students governed by their habitual actions, understanding, reflection, and critical reflection are statistically examined to meet the research objectives. Findings reveal that AIOU teacher education programs have a stronger tendency to promote understanding and reflective thinking among learners. The need to integrate content for developing critical reflection among learners is highlighted. The study also discovers the significant impacts of students' job status and previous qualifications on their thinking patterns.

28. Socio-Economic Affects of Floods on Female Teachers in Jampur (Pakistan)

Women are major affected segment of society in any disaster in under developed countries. Floods of 2010, in Pakistan, affected more than 17 million people. Ultimately, it created several social, psychological and financial problems for affected females. The current paper aimed to study the socio-economic affects of floods on female teachers of private schools in tehsil headquarters Jampur. There were sixty two registered and non-registered private schools in the city in which five hundreds and seventy nine female teachers were working. Eight schools were selected as cluster to collect the data. Totally sample was comprised fifty eight female teachers. Semi structure interview and focus group discussion techniques were used to gather the data. Collected data were analyzed use thematic analysis approach. Findings indicated that female school teachers working in private school of city Jampur were badly affected in floods 2010. They had to face several social and financial problems. There was much space to improve relief and aid activities in the affected areas. Separate health facilities for women were also needed. The paper recommends better planning and its transparent and fair implementations in next rainy season.

29. Parents' Attitude toward Daughters' Education in Tribal Area of Dera Ghazi Khan (Pakistan)

The paper aimed to investigate the parents' attitudes toward their daughters' education in tribal areas of district Dera Ghazi Khan (Pakistan). To achieve the objectives four research questions were established. Focus of the questions was to examine the significance of girls' education for tribal parents. Existing and expected role of tribal parents as well as contribution from government and community for girls' education was also aimed to explore in research questions. Sample comprised thirty parents and five teachers/educational workers. The data was collected through semi-structured interviews and was analyzed using content analysis approach. The findings disclosed the parents' positive perceptions toward their daughters' education but at the same time severe scarcity of human and physical infrastructure for girls' education was also presented in the area. The paper recommended several empirical steps to overcome these problems including provision of new school locations and ensuring the availability of school buildings, supporting infrastructure and teachers for already functioning schools in the area. Financial aid for poor students was also proposed in the study.

Neoadjuvant systemic therapy (NAST) provides the unique opportunity to assess response to treatment after months rather than years of follow-up. However, significant variability exists in methods of pathologic assessment of response to NAST, and thus its interpretation for subsequent clinical decisions. Our international multidisciplinary working group was convened by the Breast International Group-North American Breast Cancer Group (BIG-NABCG) collaboration and tasked to recommend practical methods for standardized evaluation of the post-NAST surgical breast cancer specimen for clinical trials that promote accurate and reliable designation of pathologic complete response (pCR) and meaningful characterization of residual disease. Recommendations include multidisciplinary communication; clinical marking of the tumor site (clips); and radiologic, photographic, or pictorial imaging of the sliced specimen, to map the tissue sections and reconcile macroscopic and microscopic findings. The information required to define pCR (ypT0/is ypN0 or ypT0 yp N0), residual ypT and ypN stage using the current AJCC/UICC system, and the Residual Cancer Burden system were recommended for quantification of residual disease in clinical trials.

Abstract Background Many interventions to motivate community health workers to perform better rely on financial incentives, even though it is not clear that monetary gain is the main motivational driver. In Pakistan, Lady Health Workers (LHW) are responsible for delivering community level primary healthcare, focusing on rural and urban slum populations. There is interest in introducing large-scale interventions to motivate LHW to be more actively involved in improving tuberculosis case-finding, which is low in Pakistan. Methods Our study investigated how to most effectively motivate LHW to engage more actively in tuberculosis case-finding. The study was embedded within a pilot intervention that provided financial and other incentives to LHW who refer the highest number of tuberculosis cases in three districts in Sindh province. We conducted semi-structured interviews with 20 LHW and 12 health programme managers and analysed these using a framework categorising internal and external sources of motivation. Results Internal drivers of motivation, such as religious rewards and social recognition, were salient in our study setting. While monetary gain was identified as a motivator by all interviewees, programme managers expressed concerns about financial sustainability, and LHW indicated that financial incentives were less important than other sources of motivation. LHW emphasised that they typically used financial incentives provided to cover patient transport costs to health facilities, and therefore financial incentives were usually not perceived as rewards for their performance. Conclusions This study indicated that interventions in addition to, or instead of, financial incentives could be used to increase LHW engagement in tuberculosis case-finding. Our finding about the strong role of internal motivation (intrinsic, religious) in Pakistan suggests that developing context-specific strategies that tap into internal motivation could allow infectious disease control programmes to improve engagement of community health workers without being dependent on funding for financial incentives.

The objective of this study is to the assess toxicity of superparamagnetic nanoparticles within normal and diabetic human blood groups using complete blood count (CBC). For this purpose, iron oxide nanoparticles (IONPs) were synthesized by co-precipitation method. Samples were characterized by XRD, SEM, EDS, VSM and CBC. XRD confirmed the cubic structure of Fe3O4 with Miller indices (2 0 0), (2 2 0), (3 1 1), (4 0 0), (4 4 0) and average size of magnetic nanoparticles was calculated about 11.13 nm diameter. The morphology of Fe3O4 nanoparticles was investigated by scanning electron microscope (SEM). SEM images of magnetite were found partially smooth. Spectra of EDX depicted the Fe, O and Cl elements in IONP. Magnetic properties were examined by vibrating sampling magnetometer (VSM). The blood toxicity was reported by blood contents in normal and diabetic blood samples for specified intervals using CBC technique. One sample test of variance of hemoglobin, erythrocytes, leucocytes and thrombocytes showed the significant difference (p > 0.05) among 0 hrs to 72 hrs for all normal and diabetic blood samples. This study concluded that presence of iron oxides nanoparticles of size 11.13 nm in human blood induces reactive oxygen species which cause the cell death. Hemoglobin was highly affected content of blood than erythrocytes, leucocytes by toxic effects of IONPs. Reduction of platelets among all content of blood groups posed another signature of this hematology study. Toxic spectrum of IONPs must be considered before the application of MRI contrast agents and drug delivery.

Objective: To evaluate the frequency, common presentation, anti-snake venom (ASV) related management and outcome of snake bite cases in admitted children at a tertiary care hospital. Study Design: Case-series study. Setting: Department of Pediatric, Dera Ghazi Khan Medical College (DGKMC) & Hospital. Period: 1st March 2021 to 31st August 2021. Material & Methods: During the study period, all children presenting with snake bite were included. All cases were observed for their presentation, classified according to signs and symptoms, and managed through ASV as needed. Results: During the study period, a total of 52 patients of snake bite admitted in paediatric medical ward of DGKMC & Hospital. There were 37 (71.2%) male and 15 (28.8%) female. Overall, mean age of the children was 6.1±3.1 years. Maximum number of snake bite cases were reported during May, June and July as 15 (28.8%), 13 (25.0%) and 8 (15.4%) respectively. In the presentation, 29 (55.8%) patients presented with local symptoms only while remaining 23 (44.2%) presented with local as well as systemic symptoms. In the management, 3 (5.8%) cases did not require any ASV vial, 35 (67.3%) needed initial doses only. Fourteen (26.9%) cases required initial as well as maintenance doses. In terms of outcome, 49 (94.2%) children were discharged successfully, 1 (1.9%) expired and 2 (3.8%) got left against medical advice (LAMA). Conclusion: Snake bite was a common pediatric medical emergency at our institute. Most children presented with hemotoxic symptoms and needed ASV for resolution of symptoms. Prognosis is good with timely and judicious management of snake bite.

Celia's encephalopathy or Progressive encephalopathy with or without lipodystrophy (PELD) is a rare childhood neurodegenerative syndrome, that is autosomal recessive mendelian trait, It is Seipin protein associated encephalopathy and results in regression of developmental mile stones. We report a case with global developmental delay and hypertonia, hyperreflexia, recurrent infections and dyskinesia. The diagnosis was done by exome sequencing revealing mutation in the BSCL2 gene on chromosome 11q13. The cases was very rare and found to mimic other diseases that is why patient was investigated and reported. The study was done to identify report and manage such cases appropriately. The patient was investigated in OPD in 2021. Whole exome sequencing (WES) was done for diagnosis after taking appropriate blood sample. A protein change p.(trp323leufs*15) was identified due to homozygous frameshift elongation likely pathogenic on chromosome 11 in BSCL gene.p. (trp323leufs*15) creates a shift in reading frame starting at codon 323. The new frame ends in stop codon after 15 proteins downstream. This new variant is absent in genomAD and local database. This variant has also not been reported yet in clinVar. This variant is classified as likely pathogenic class 2 based on ACMG recommendations. Any patient was favoring symptoms and features should be investigated with whole genome sequencing to confirm the diagnosis. [ABSTRACT FROM AUTHOR]

Aromatase inhibitors (AIs) reduce breast cancer recurrence and prolong survival, but up to 30% of patients exhibit recurrence. Using a genome-wide association study of patients entered on MA.27, a phase III randomized trial of anastrozole versus exemestane, we identified a single nucleotide polymorphism (SNP) in CUB And Sushi multiple domains 1 (CSMD1) associated with breast cancer–free interval, with the variant allele associated with fewer distant recurrences. Mechanistically, CSMD1 regulates CYP19 expression in an SNP- and drug-dependent fashion, and this regulation is different among 3 AIs: anastrozole, exemestane, and letrozole. Overexpression of CSMD1 sensitized AI-resistant cells to anastrozole but not to the other 2 AIs. The SNP in CSMD1 that was associated with increased CSMD1 and CYP19 expression levels increased anastrozole sensitivity, but not letrozole or exemestane sensitivity. Anastrozole degrades estrogen receptor α (ERα), especially in the presence of estradiol (E2). ER+ breast cancer organoids and AI- or fulvestrant-resistant breast cancer cells were more sensitive to anastrozole plus E2 than to AI alone. Our findings suggest that the CSMD1 SNP might help to predict AI response, and anastrozole plus E2 serves as a potential new therapeutic strategy for patients with AI- or fulvestrant-resistant breast cancers.

Liver disease is one of the most prominent causes of the increase in the death rate worldwide. These death rates can be reduced by early liver diagnosis. Computed tomography (CT) is a method for the analysis of liver images in clinical practice. To analyze a large number of liver images, radiologists face problems that sometimes lead to the wrong classifications of liver diseases, eventually resulting in severe conditions, such as liver cancer. Thus, a machine-learning-based method is needed to classify such problems based on their texture features. This paper suggests two different kinds of algorithms to address this challenging task of liver disease classification. Our first method, which is based on conventional machine learning, uses texture features for classification. This method uses conventional machine learning through automated texture analysis and supervised machine learning methods. For this purpose, 3000 clinically verified CT image samples were obtained from 71 patients. Appropriate image classes belonging to the same disease were trained to confirm the abnormalities in liver tissues by using supervised learning methods. Our proposed method correctly quantified asymmetric patterns in CT images using machine learning. We evaluated the effectiveness of the feature vector with the K Nearest Neighbor (KNN), Naive Bayes (NB), Support Vector Machine (SVM), and Random Forest (RF) classifiers. The second algorithm proposes a semantic segmentation model for liver disease identification. Our model is based on semantic image segmentation (SIS) using a convolutional neural network (CNN). The model encodes high-density maps through a specific guided attention method. The trained model classifies CT images into five different categories of various diseases. The compelling results obtained confirm the effectiveness of the proposed model. The study concludes that abnormalities in the human liver could be discriminated and diagnosed by texture analysis techniques, which may also assist radiologists and medical physicists in predicting the severity and proliferation of abnormalities in liver diseases.

41. Acceptance of COVID-19 Vaccine Among Undergraduate Students

Objective: To assess the acceptance of the COVID-19 vaccine among medical and non-medical undergraduate students and compare the COVID-19 vaccine receptivity differences between medical and non-medical undergraduate students. Study Design: Cross-sectional study. Place and Duration of Study: Bahria University Medical & Dental College, and Bahria University Karachi Campus Pakistan, from Mar to Jun 2021. Methodology: In this study, a total of 392 participants, were selected by non-probability convenience sampling. Students aged 19-26 years were enrolled. Participants were required to sign a voluntary consent form prior to participation. The questionnaire included 21 questions inclusive of demographic characteristics, insight regarding COVID-19 disease, prior EPI vaccine exposure, attitude towards COVID-19 vaccine, including the importance of vaccine to end the pandemic, acceptance of vaccine along with reasons and non-acceptance and sent by email as a Google form to participants. Results: There were 171(43.6%) males and females 218(55.6%). The mean age of participants was 20.7±2.29 years. 217(55.4%) students were from the MBBS programs, and 175(44.6%) were from non-MBBS programs. COVID-19 vaccine acceptability was better amongst medical students, with 182(83.9%) considering it safe for use in contrast to 5(2.8%) non-medical students who considered it safe. Conclusion: The comparison of knowledge and attitudes towards accepting the COVID-19 vaccine between medical students and non-medical students revealed better acceptance amongst undergraduate medical students than non-medical undergraduate students.

Objectives: To determine the frequency of sustained ventricular arrhythmias in Non-ST segment elevation myocardial infarction (NSTEMI) patients. Methodology: This is a Cross sectional study was done at Chaudhry Pervaiz Elahi Institute of Cardiology, Multan Pakistan from 10th August 2018 to 9th February 2019. We included 170 patients fulfilling the inclusion criteria with diagnosis of NSTEMI presented in department of cardiac emergency were selected. Informed consent was taken. The data was collected on prepared proforma. Results: In our study mean age of patients was 50.90+7.25 years. There were 139 (81.76%) male patients and 31 (18.24%) female patients. There were 58 (34.12%) patients who were having diabetes mellitus, 68 (40.0%) patient’s having hypertension, 71 (41.76%) patients were smokers, 15 (8.82%) patients were having hypercholesterolemia, and 18 (10.59%) patients who were having positive family history of coronary artery disease (CAD). Sustained ventricular arrhythmias (VA) occurred in 9 (5.29%) patients of non-STEMI. Conclusion: The incidence of Sustained ventricular arrhythmias in patients of non-ST elevation myocardial infarction (NSTEMI) is 5.29%. The occurrence of such events remains difficult to predict. Cardiac monitoring should be done in all patients to monitor occurrence of such Sustained ventricular arrhythmias in these patients.

Purpose: The information and communication technology (ICT) in the twenty-first century has shifted the focus of English as a second language (ESL) practitioners away from the traditional and learner-centered pedagogy that was prevalent in the past in which focus of education was more learner-oriented. The purpose of this research is to evaluate the influence that information and communication technology (ICT) has on the writing abilities of graduate level students. Design/Methodology/Approach: To collect data for this purpose, the 60 respondents i.e., 30 were from BSCS 1st & 30 from SE 1st were chosen from NUML, Multan by using Krejcie & Morgan (1970) method for sample selection. Both groups of students, i.e., one group was taught through using traditional methods and others were taught by using ICT methodologies for a period of 20 days. A test has been carried out that consisted of an essay-style question, and the students have been asked to compose an essay on the subject that has been provided to them. Findings: The The outcomes of the test were recorded and evaluated by using Corder's (1970) error analysis for posterity. Results of both groups were analyzed and compared. Implications/Originality/Value: It was concluded through the facts that the use of ICT in English language instruction has significantly improved the scores of the students. Hence, it was proved through statistical analysis that information and communication technology has a major impact in improving the writing abilities of ESL students. [ABSTRACT FROM AUTHOR]

- Abstract
Religion is the main component of the Pakistani Education system. It is important to understand the role of schooling in religious socialization of the students. The study aimed to compare the role of public and private schooling in religious socialization of secondary school students. Quantitative research design was followed to achieve the objectives. Researchers developed a 36 items questionnaire to collect data from secondary school students enrolled in public and private secondary schools. The questionnaire consisted of 6 factors including curriculum, teacher, co-curricular activities, school management, class fellows and parents-teacher interaction. The share of the participants was equal for male, female and rural, urban students. Descriptive and inferential statistics were used to analyze the data. The results showed that public and private secondary schools played an important role in promoting religious socialization among students. However, there was a significant difference in the perceptions of male and female secondary school students about their religious socialization at schools.

- Abstract
Religious socialization is an important debate in the field of social sciences and education psychology. Religion is an integral part of Pakistani society because Pakistan is an idealistic state. Therefore, it is important to study the factors that promote religious socialization among secondary school students. It gave clear dimensions about the emerging social trends in religious societies. This study aimed to critically analyze the factors promoting religious socialization and compare them. These factors include family, peers, school, government, religious places, mass media, and social media. For this analysis, a quantitative approach was used with a descriptive research design. The review of the related literature enables researchers to develop a questionnaire comprising 38 items. The population for the study was from secondary schools of district Jhang. The study consisted of 600 respondents from different schools with an equal ratio of male and female schools as well as rural and urban schools. The percentages were collected, tabulated, and compared. Results suggested that family was the most significant agent that promoted religious socialization among students at secondary schools. The independent sample t-test was used to compare the school and gender-wise religious socialization at secondary schools. The results of the present study show that there are many factors that play an important role in promoting religious socialization among students at secondary schools.
