Abstract

A solution to the quantum Zermelo problem for control Hamiltonians with general energy resource bounds is provided. Interestingly, the energy resource of the control Hamiltonian and the control time define a pair of conjugate variables that minimize the energy-time uncertainty relation. The resulting control protocol is applied to a single qubit as well as to a two-interacting qubit system represented by a Heisenberg spin dimer. For these low-dimensional systems, it is found that physically realizable control Hamiltonians exist only for certain, quantized, energy resources., Comment: 13 pages, 1 figure

Abstract

Reaction paths and classical and quantum trajectories are studied within a generalized Hamilton-Jacobi framework, which allows to put on equal footing topology and dynamics in chemical reactivity problems. In doing so, we show how high-dimensional problems could be dealt with by means of Caratheodory plots or how trajectory-based quantum-classical analyses reveal unexpected discrepancies. As a working model, we consider the reaction dynamics associated with a Mueller-Brown potential energy surface, where we focus on the relationship between reaction paths and trajectories as well as on reaction probability calculations from classical and quantum trajectories., Comment: 22 pages, 5 figures, 1 table

Abstract

Fragmentation is a common feature of rockfall that exerts a strong control on the trajectories of the generated blocks, the impact energies, and the runout. In this paper, we present a set of four real-scale rockfall tests aimed at studying the fragmentation of the rocky blocks, from the global design of the field procedure to the data analysis and the main results. A total of 124 limestone, dacite, or granite blocks ranging between 0.2 and 5 m3 were dropped from different heights (8.5 to 23.6 m) onto four slopes with different shapes (single or double bench) and slope angles (42º to 71º). The characteristics of the blocks, in particular the size, surface texture and joint condition, were measured before the drops. The trajectories of the blocks and both the initial and the impact velocities were tracked and recorded by means of three high-speed video cameras. A total of 200 block-to-ground impacts have been studied. On average, 40% of the blocks broke upon impact on the slope or on the ground, making it necessary to measure the fragments. The initial and final sizes of the blocks/fragments were measured by hand with tape, though photogrammetric techniques (UAV and terrestrial) were also used for comparison purposes. The information gathered during the field tests provides a deep insight into the fragmentation processes. On the one hand, the high-resolution slow-motion videos help to describe when and how the block breakage takes place and the spatial distribution of the pieces. On the other hand, it is possible to compute the block trajectories, the velocities, and the energy losses using videogrammetry. The results include, for instance, a block average fragmentation of 54% and 14% for the limestone and granitoids, respectively; the systematic inventory of the size fragments, which may be used for fitting the power law distributions; and after each breakage, the total angle of aperture occupied by the fragments has been measured, with values in the range

Abstract

PURPOSE Operable triple-negative breast cancers (TNBCs) have a higher risk of relapse than non-TNBCs with standard therapy. The GEICAM/2003-11_CIBOMA/2004-01 trial explored extended adjuvant capecitabine after completion of standard chemotherapy in patients with early TNBC. PATIENTS AND METHODS Eligible patients were those with operable, node-positive-or node negative with tumor 1 cm or greater-TNBC, with prior anthracycline- and/or taxane-containing chemotherapy. After central confirmation of TNBC status by immunohistochemistry, patients were randomly assigned to either capecitabine or observation. Stratification factors included institution, prior taxane-based therapy, involved axillary lymph nodes, and centrally determined phenotype (basal v nonbasal, according to cytokeratins 5/6 and/or epidermal growth factor receptor positivity by immunohistochemistry). The primary objective was to compare disease-free survival (DFS) between both arms. RESULTS Eight hundred seventy-six patients were randomly assigned to capecitabine (n = 448) or observation (n = 428). Median age was 49 years, 55.9% were lymph node negative, 73.9% had a basal phenotype, and 67.5% received previous anthracyclines plus taxanes. Median length of follow-up was 7.3 years. DFS was not significantly prolonged with capecitabine versus observation [hazard ratio (HR), 0.82; 95% CI, 0.63 to 1.06; P = .136]. In a preplanned subgroup analysis, nonbasal patients seemed to derive benefit from the addition of capecitabine with a DFS HR of 0.53 versus 0.94 in those with basal phenotype (interaction test P = .0694) and an HR for overall survival of 0.42 versus 1.23 in basal phenotype (interaction test P = .0052). Tolerance of capecitabine was as expected, with 75.2% of patients completing the planned 8 cycles. CONCLUSION This study failed to show a statistically significant increase in DFS by adding extended capecitabine to standard chemotherapy in patients with early TNBC. In a preplanned subset analysis, patients with nonbasal phenotype seemed to obtain benefit with capecitabine, although this will require additional validation. Abstract

The formation of stable organic biradicals by a deprotonation process is reported for a series of conjugated heterocycles that share a Ph-N(H)-2-thiazole structural motif. We characterise the paramagnetic electronic ground state by means of continuous-wave and pulse EPR. We propose a simple valence bond mechanism for a deprotonation-induced formation of paramagnetic organic molecules, based on the interplay between the electronegativity of heteroatomic groups and the recovery of aromaticity to stabilise the biradical species. The Ph-N(H)-2-thiazole motif is found in a variety of biologically active molecules, exemplified here with the anticancer drug Dasatinib, and our results suggest a radical-based mechanism for the protein kinase inhibition activity of the drug. The existence of this structure-property relationship for an elementary chemical motif suggests that biradical species may be more prevalent than previously thought and have an important role in bioorganic chemistry.

10. Broad consensus on the optimal sequence for the systemic treatment of metastatic breast cancer: results from a survey of Spanish medical oncologists

Abstract

Objective: The aim of this survey conducted by 20 leading Spanish oncologists was to analyze the concurrence between Spanish clinical practice and the recently published definition of the optimal sequence for the systemic treatment of metastatic breast cancer (MBC) according to patient profiles. Methods: A self-administered questionnaire was developed, divided into five sections comprising 34 specific questions related to sequential treatments, plus three additional general questions. Respondents were asked to justify negative answers. Participants were recruited randomly by invitation out of a total of 619 oncologists. The questionnaire was sent and collected via e-mail between October 2015 and May 2016. A total of 191 completed questionnaires were received. Results: Overall, 70% of oncologists would keep the three patient profiles exactly as proposed (hormone receptor-positive and HER2-negative, HER2-positive, and triple negative breast cancer). Affirmative answers to questions regarding treatment sequences for these patient profiles (1-34) ranged from 77.8-99.5%, with an average of 90.9% of oncologists being in agreement with the recommended sequential treatments. The lowest degree of consensus was observed for endocrine treatments in pre-menopausal women and for chemotherapy options in hormone-resistant patients, whilst the highest degree of consensus was reached for targeted therapies in HER2-positive patients and for endocrine therapy in post-menopausal women. In their comments, participants revealed a number of economic constraints that prevented them from implementing some of the best treatment options. Conclusions: In conclusion, despite the complexity of MBC treatment, there is general agreement on the optimal treatment sequences.

Abstract

Recently, two poisoning episodes produced by Jimson weed (Datura stramonium) consumption have been observed in dairy cattle farms from Catalonia. The plant was mistakenly gathered, in both cases, together with corn used for animal feed. The set of symptoms were slight, basically gastrointestinal disorders and decrease of milk production, and animal recovery was observed as soon as the contaminated feed was removed. This problematic situation could be increasing in prevalence due to environmental and economic factors., Recientemente hemos observado dos intoxicaciones en ganado vacuno lechero, en granjas de Cataluña, debidas a la ingestión de estramonio (Datura stramonium). La planta había sido en ambos casos erróneamente recolectada junto con el maíz empleado para la alimentación de los animales. El cuadro fue leve y, básicamente, cursó con problemas digestivos y descenso en la producción lechera, y remitió con la retirada del alimento contaminado. Debido a factores ambientales y económicos, el problema podría estar en aumento.

34. Reaction Modes of Carbonyl Oxide, Dioxirane, and Methylenebis(oxy) with Ethylene:  A New Reaction Mechanism

Abstract

The reaction modes of the three isomers, carbonyl oxide (A), dioxirane (B), and methylenebis(oxy) (C), with ethylene (E) were investigated by using CASPT2, CCSD(T), and B3LYP with a 6-311+G(2d,2p) basis set. Contrary to general expectations, A prefers to react with E via a [4+2] cycloaddition reaction (activation enthalpy ΔH#(298) = 1.0 kcal/mol) to yield 1,2-dioxolane (MA2) (reaction enthalpy, −65.0 kcal/mol) rather than via an epoxidation reaction (ΔH#(298) = 11.3 kcal/mol) leading to oxirane and formaldehyde. Epoxidation by B is slower in view of an activation enthalpy of 13.7 kcal/mol; however, it should proceed stereochemically similar as in the case of A. Biradicals C and E can undergo a pseudo-[4+2] cycloaddition reaction (ΔH#(298) = 1.8 kcal/mol), which leads to 1,3-dioxolane (MC2) in a strongly exothermic reaction (ΔHR(298) = −84.5 kcal/mol). The results obtained lead to a mechanistic reconsideration of epoxidation reactions observed in the course of the ozonolysis. They also provide a general basis for comparing reactions of 1,3-dipolar compounds and 1,3-biradicals with unsaturated compounds.

Abstract

The classical reaction path Hamiltonian formulation of Miller, Handy, and Adams is reformulated using a linear expansion of the gradient in internal coordinates. It leads to a correspondence between the arc length, s, along the intrinsic reaction coordinate, and the whole set of internal coordinates and, furthermore, to a dynamical equation for s, a second-order Bernoulli-type equation, which is analytically solvable inside the validity range of the quadratic expansion of the potential. Therefore, by virtue of the above correspondence, the time dependence of the whole set of internal coordinates is easily recovered, by means of a few functional and overlap evaluations. It thus enhances the computational performance of the overall direct dynamics method. The unimolecular 1,2 hydrogen migration, between the (corresponding) carbene and ethyne oxide, is considered as example for illustrative purposes.

36. Theoretical Investigation of the Low-Lying Electronic States of Dioxirane:  Ring Opening to Dioxymethane and Dissociation into CO<INF>2</INF> and H<INF>2</INF>

Abstract

The low-lying electronic states of dioxirane (1), their ring opening to dioxymethane (2), and the dissociation of 2 into CO2 and H2 have been investigated by means of CASSCF and MRD-CI+Q quantum chemistry calculations. The ground state of 1 is a singlet with 4π electrons, 11A1(4π), while the ground state of 2 is a 2π-electron singlet, 11A1(2π), lying 5.8 kcal/mol higher than 1 in energy. A 0 K activation energy of 21.4 kcal/mol is predicted for the thermal ring opening of 1 into 2, which takes place via a transition structure approximately corresponding to the crossing between the lower 1A1(4π) and 1A1(2π) states of both molecules. Twelve excited states have been calculated for 1 with vertical excitation energies ranging from 3.07 to 13.11 eV. The energy ordering of these states changes dramatically upon relaxation of the molecular geometries. The optimum geometries of these excited states show an ∠OCO in the 106.3−120.1° range, so they should be considered as excited states of 2. Minimum energy points of the intersection seam between the 11A2(3π)/11B1(3π), 11B1(3π)/1A1(2π), 11A2(3π)/1A1(4π), and 11B1(3π)/1A1(4π) potential energy surfaces have been located in an ∠OCO range of 91.0−104.6°. The photochemical ring opening of 1 into 2 may occur through vertical excitation to either the 11B1(3π) or 11A2(3π) states of 1 and subsequent radiationless decay to ground-state 2 via minimum energy intersection points on the potential energy surfaces of the appropriate states. The dissociation of ground-state 2 into CO2 and H2 is predicted to be exothermic by 105.2 kcal/mol with a 0 K activation energy of 3.2 kcal/mol, while the dissociations of the first four excited states of 2 are all predicted to be endothermic.

Abstract

[Background] Breast cancer is the most common malignancy and the second leading cause of cancer-related mortality in Spanish women. Ribociclib in combination with endocrine therapy (ET) has shown superiority in prolonging survival in patients with hormone receptor-positive (HR+), human epidermal growth factor receptor 2-negative (HER2−) advanced breast cancer (ABC) vs. ET alone., [Methods] CompLEEment-1 is a single-arm, open-label phase 3b trial evaluating ribociclib plus letrozole in a broad population of patients with HR+, HER2– ABC. The primary endpoints were safety and tolerability. Here we report data for Spanish patients enrolled in CompLEEment-1., [Results] A total of 526 patients were evaluated (median follow-up: 26.97 months). Baseline characteristics showed a diverse population with a median age of 54 years. At study entry, 56.5% of patients had visceral metastases and 8.7% had received prior chemotherapy for advanced disease. Rates of all-grade and Grade ≥3 adverse events (AEs) were 99.0% and 76.2%, respectively; 21.3% of patients experienced a serious AE, and 15.8% of AEs led to treatment discontinuation. AEs of special interest of neutropenia, increased alanine aminotransferase, increased aspartate aminotransferase and QTcF prolongation occurred in 77.8%, 14.8%, 11.4% and 4.0% of patients, respectively. Patients aged >70 years experienced increased rates of all-grade and Grade ≥3 neutropenia and anemia. Abstract

Breast cancer is one of the most prevalent tumors among women. Its prognosis and treatment outcomes depend on factors related to tumor cell biology. However, recent studies have revealed the critical role of the tumor microenvironment (TME) in the development, progression, and treatment response of breast cancer. In this review, we explore the different components of the TME and their relevance as prognostic and predictive biomarkers in breast cancer. In addition, techniques for assessing the tumor microenvironment, such as immunohistochemistry or gene expression profiling, and their clinical utility in therapeutic decision-making are examined. Finally, therapeutic strategies targeting the TME are reviewed, highlighting their potential clinical benefits. Overall, this review emphasizes the importance of the TME in breast cancer and its potential as a clinical tool for better patient stratification and the design of personalized therapies.

Abstract

High efficiency and safety are critical factors in ensuring the optimal performance and reliability of systems and equipment across various industries. Fault monitoring (FM) techniques play a pivotal role in this regard by continuously monitoring system performance and identifying the presence of faults or abnormalities. However, traditional FM methods face limitations in fully capturing the complex interactions within a system and providing real-time monitoring capabilities. To overcome these challenges, Digital Twin (DT) technology has emerged as a promising solution to enhance existing FM practices. By creating a virtual replica or digital copy of a physical equipment or system, DT offers the potential to revolutionize fault monitoring approaches. This paper aims to explore and discuss the diverse range of predictive methods utilized in DT and their implementations in FM across industries. Furthermore, it will showcase successful implementations of DT in FM across a wide array of industries, including manufacturing, energy, transportation, and healthcare. The utilization of DT in FM enables a comprehensive understanding of system behavior and performance by leveraging real-time data, advanced analytics, and machine learning algorithms. By integrating physical and virtual components, DT facilitates the monitoring and prediction of faults, providing valuable insights into the system's health and enabling proactive maintenance and decision making.

Abstract

By the end of 2020, there were more than 8 million women alive who had received a breast cancer diagnosis in the previous 5 years, making it the most prevalent neoplasia in the world. About 70% of breast-cancer cases present positivity for estrogen and/or progesterone receptors and a lack of HER-2 overexpression. Endocrine therapy has traditionally been the standard of care for ER-positive and HER-2-negative metastatic breast cancer. In the last 8 years, the advent of CDK4/6 inhibitors has shown that adding them to endocrine therapy doubles PFS. As a result, this combination has become the gold standard in this setting. Three CDK4/6 inhibitors have been approved by the EMA and the FDA: abemaciclib, palbociclib, and ribociclib. They all have the same indications, and it is at each physician's discretion to choose one or the other. The aim of our study was to perform a comparative efficacy analysis of the three CDK4/6i using real-world data. We selected patients diagnosed with endocrine-receptor-positive and HER2-negative breast cancer who were treated with all three CDK4/6i as first-line therapy at a reference center. After 42 months of retrospective follow up, abemaciclib was associated with a significant benefit in terms of progression-free survival in endocrine-resistant patients and in the population without visceral involvement. In our real-world cohort, we found no other statistically significant differences among the three CDK4/6 inhibitors.

Abstract

The NOA (Oncological Nutrition in Andalusia) project analyses the degree of integration and areas of improvement in implementing nutritional support in the care plans of cancer patients in Andalusia. The aim was to analyse nutritional interventions for better care of cancer patients and for the improvement of the management of malnutrition in cancer. A prospective evaluation of the implementation of two areas of improvement in nutrition was conducted in three hospitals. Data were collected from each hospital over a six-month period using an online platform. A standardised care plan was designed for hospitals in Andalusia, in which proposed improvements were devised and prioritised, selecting nutritional screening in oncology services and the participation of the Nutrition Support Team (NST) on the tumour boards, as well as the assessment of the patients presented at these sessions. Our results indicated an increase in the number of medical records with nutritional evaluation results six months later, regardless of the type of tumour or hospitalisation; and there was greater participation of the NST on the tumour boards, mainly for head and neck and oesophagogastric cases. Solutions for improvement have been pinpointed and implemented that have positively impacted the nutritional care plan in the course of oncological disease.

Abstract

Background: The ability to diagnose preeclampsia clinically is suboptimal. Our objective was to validate a novel multianalyte assay and characterize its performance, when intended for use as an aid to rule-out preeclampsia., Methods: Prospective, multicenter cohort study of pregnant individuals presenting between 28 0/7 and 36 6/7 weeks' with preeclampsia-associated signs and symptoms. Individuals not diagnosed with preeclampsia after baseline evaluation were enrolled in the study cohort, with those who later developed preeclampsia, classified as cases and compared with a negative control group who did not develop preeclampsia. Individuals with assay values at time of enrollment ≥0.0325, determined using a previously developed algorithm, considered at risk. The primary analysis was the time to develop preeclampsia assessed using a multivariate Cox regression model., Results: One thousand thirty-six pregnant individuals were enrolled in the study cohort with an incidence of preeclampsia of 30.3% (27.6%-33.2%). The time to develop preeclampsia was shorter for those with an at-risk compared with negative assay result (log-rank P <0.0001; adjusted hazard ratio of 4.81 [3.69-6.27, P <0.0001]). The performance metrics for the assay to rule-out preeclampsia within 7 days of enrollment showed a sensitivity 76.4% (67.5%-83.5%), negative predictive value 95.0% (92.8%-96.6%), and negative likelihood ratio 0.46 (0.32-0.65). Assay performance improved if delivery occurred <37 weeks and for individuals enrolled between 28 and 35 weeks., Conclusions: We confirmed that a novel multianalyte assay was associated with the time to develop preeclampsia and has a moderate sensitivity and negative likelihood ratio but high negative predictive value when assessed as an aid to rule out preeclampsia within 7 days of enrollment., Registration: The study was registered on Clinicaltrials.gov (Identifier NCT02780414).
