Abstract

The rise of artificial intelligence (AI) applications in healthcare provides new possibilities for personalized health management. AI-based fitness applications are becoming more common, facilitating the opportunity for individualised exercise prescription. However, the use of AI carries the risk of inadequate expert supervision, and the efficacy and validity of such applications have not been thoroughly investigated, particularly in the context of diverse health conditions. The aim of the study was to critically assess the efficacy of exercise prescriptions generated by OpenAI’s Generative Pre-Trained Transformer 4 (GPT-4) model for five example patient profiles with diverse health conditions and fitness goals. Our focus was to assess the model’s ability to generate exercise prescriptions based on a singular, initial interaction, akin to a typical user experience. The evaluation was conducted by leading experts in the field of exercise prescription. Five distinct scenarios were formulated, each representing a hypothetical individual with a specific health condition and fitness objective. Upon receiving details of each individual, the GPT-4 model was tasked with generating a 30-day exercise program. These AI-derived exercise programs were subsequently subjected to a thorough evaluation by experts in exercise prescription. The evaluation encompassed adherence to established principles of frequency, intensity, time, and exercise type; integration of perceived exertion levels; consideration for medication intake and the respective medical condition; and the extent of program individualization tailored to each hypothetical profile. The AI model could create general safety-conscious exercise programs for various scenarios. This study assessed the relationship between lung function parameters against polysomnography (PSG) and continuous positive airway pressure (CPAP) adherence data amongst an Aboriginal Australian population.Methods: Patients who undertook both a diagnostic PSG and spirometry were included. Restrictive, obstructive, and mixed impairments were assessed via global lung function initiative (GLI-2012, ATS/ERS) criteria/guidelines. PSG and CPAP data were evaluated between patients with or without spirometry impairments.Results: Of the total 771 patients, 248 had PSG and spirometry data available (52% female, 44% remote residents, 78% obese). The majority (89%) had OSA (51% severe), 95 (38%) were observed to have a restrictive impairment, and 31 (13%) had an obstructive or mixed impairment on spirometry. Compared to patients with no spirometric impairment, those with restrictive or obstructive/mixed impairments demonstrated significantly lower sleep efficiency (median 84% vs 79% and 78%), higher apnoea-hypopnea index (AHI) during rapid eye movement (REM) sleep (median 32 vs 52 and 55 events/hour), reduced REM oxygen saturation (SpO2) (median 94.0% vs 92.0% and 92.5%) and reduced adherence to CPAP therapy (median 39% vs 22% and 17%). Differences in sleep efficiency, REM AHI, and NREM SpO2 held for patients with obstructive/mixed impairments in multivariate modelling.Conclusion: Aboriginal Australian patients with OSA have a higher concurrent lung function’ impairment. Spirometric impairment appears to negatively influence sleep efficiency, nocturnal SpO2 and CPAP adherence. Abstract

Artificial intelligence chatbots may fundamentally transform academic research, automate mundane tasks, and enhance productivity. However, the integration of artificial intelligence chatbots (AIc) is impeded by a complex stigma deeply rooted in individuals’ misconceptions and apprehension, including concerns about academic integrity, job displacement, data privacy, and algorithmic bias. The aim of this study was to scrutinize the origins and impacts of the stigma associated with artificial intelligence chatbots within the realm of academic research and to propose strategies to mitigate such stigmas. This study draws parallels between the reception of artificial intelligence chatbots and previous transformative technologies, presenting case studies illustrating the spectrum of responses to the integration of artificial intelligence chatbots into academic research. This study identifies the need for a shift in mindset from perceiving artificial intelligence chatbots as threats to recognizing them as facilitators of efficiency and innovation. It also underscores the importance of understanding these models as tools that aid researchers but do not replace the need for human expertise and judgment. We further highlighted the role of education, transparency, regulation, and ethical guidelines in overcoming the stigma associated with artificial intelligence chatbots. Given how adaptable people are, the surrounding stigma will likely fade with time. We support a cooperative strategy with continuing education and discussion to maximize the benefits of artificial intelligence chatbots while minimizing their drawbacks, hopefully paving the way for their ethical and successful application in scholarly research.

Abstract

Wet-cupping therapy (WCT) is one of the oldest known medical techniques, used as a traditional and complementary therapy with a wide application all around the world for general health. Research on the effects of WCT on sports performance are sparse and inconsistent. Thus, we aimed to explore the effects of WCT on repeated sprint ability, wellness, and exertion in young active males. Forty-nine active adult males (age: [28 ​± ​5] years; body height [177 ​± ​8] cm; body mass: [79 ​± ​7] kg; body mass index: [25.4 ​± ​1.8] kg/m2) were selected for the study. The participants performed a running-based sprint test on two separate occasions (Control and Post-WCT). WCT was performed 24 ​h before the testing session. They completed the Hooper questionnaire to assess their well-being (i.e., sleep, stress, fatigue, and soreness) before each session. The rating of perceived exertion (RPE) was collected after each testing session. A higher maximum power (p ​< ​0.05, effect size [ES] ​= ​0.6), mean power (p ​< ​0.01, ES ​= ​0.5) and minimum power (p ​< ​0.01, ES ​= ​0.6) were recorded post-WCT as compared to Control session along with a better perceived sleep (p ​< ​0.01, ES ​= ​0.85). Perceived stress (p ​< ​0.01, ES ​= ​0.6) and RPE (p ​< ​0.001; ES ​= ​1.1) were lower during the post-WCT compared to the Control session. The present findings demonstrated that WCT moderately enhanced repeated sprint ability and had positive effects on perceived sleep, stress, and exertion. WCT may be an effective ergogenic aid to improve repeated sprint ability and general well-being in young adult males. Future large-scale multicentric clinical studies are paramount to confirm the results of our study.

Abstract

This short review aimed at (i) providing an update on the health benefits associated with melatonin supplementation, while (ii) considering future potential research directions concerning melatonin supplementation use relative to Coronavirus disease of 2019 (COVID-19). A narrative review of the literature was undertaken to ascertain the effect of exogenous melatonin administration on humans. Night-time melatonin administration has a positive impact on human physiology and mental health. Indeed, melatonin (i) modulates the circadian components of the sleep-wake cycle; (ii) improves sleep efficiency and mood status; (iii) improves insulin sensitivity; and (iv) reduces inflammatory markers and oxidative stress. Melatonin has also remarkable neuroprotective and cardioprotective effects and may therefore prevent deterioration caused by COVID-19. We suggest that melatonin could be used as a potential therapy in the post-COVID-19 syndrome, and therefore call for action the research community to investigate on the potential use of exogenous melatonin to enhance the quality of life in patients with post-COVID-19 syndrome., EXCLI Journal; 22:Doc173; ISSN 1611-2156

Objective In a convenience sample of athletes, we conducted a survey of COVID-19-mediated lockdown (termed ‘lockdown’ from this point forward) effects on: (i) circadian rhythms; (ii) sleep; (iii) eating; and (iv) training behaviors. Résumé Introduction. La détermination du profil des patients COVID-19 ayant une basse saturation pulsée de l’hémoglobine en oxygène(SpO2) aidera les cliniciens à identifier ceux ayant des facteurs de mauvais pronostic. Identifier et comparer les données cliniques, biologiques et radiologiques des patients Algériens hospitalisés pour COVID-19 et repartis selon le niveau de la SpO2 à l’admission mesurée au repos et à l’air ambiant. Méthodes. Une étude prospective était menée auprès des patients Algériens hospitalisés pour COVID-19 (période: 19 mars au 30 avril 2020). Les caractéristiques générales et les données cliniques, biologiques, radiologiques des patients étaient déterminées. Résultats. 86 patients étaient inclus dans l’étude [G1:SpO2 >95% (n=51) et G2: SpO2 ≤95% (n=35)]. Comparativement au G1, le G2 était plus âgé (48±14 contre 61±12 ans, p=0,0001), incluait plus de patients âgés ≥ 50 ans (37,2 contre 80,0%, p=0,0001), hypertendus (21,6 contre 45,7%, p=0,0180), cancéreux (0,0 contre 14,3%, p=0,0054), anémiques (25,6 contre 56,3%, p=0,0069), ayant une hyperleucocytose (4,7 contre 21,9%, p=0,0236), un syndrome inflammatoire biologique (82,5 contre 100%, p=0,0142), une hyper-urémie (7,0 contre 37,5%, p=0,0185), une hyper-créatininémie (4,7 contre 18,8%, p=0,0356), un dommage tissulaire (41,0 contre 66,7%, p=0,0341), des signes radiologiques en verre dépolie en plage (52,0 contre 71,4%, p=0,0397), des condensations en bande (30,0 contre 54,3%, p=0,0244), une extension sévère (2,0 contre 25,7%, p=0,0008), et incluait moins de patients qui se plaignaient de diarrhée (49,0 contre 22,9%, p=0,0145), ayant un signe radiologique en verre dépoli nodulaire (66,0 contre 40,0%, p=0,0177) et une extension légère (78,0 contre 40,0%, p=0,0004). Conclusion. Les critères associés à une SpO2 basse chez les patients COVID-19 hospitalisés étaient un âge avancé, la présence d’antécédents d’hypertension-artérielle et de cancer, des fréquences élevées de certaines anomalies biologiques ou des signes radiologiques. Le symptôme diarrhée, l’aspect radiologique en verre dépoli nodulaire et une extension légère des lésions radiologiques apparaissent comme des éléments protecteurs.
