Functional gastrointestinal disorders (FGIDs) are common and cause significant morbidity. Psychiatric comorbidities associated with FGIDs include anxiety and depression. However, little is understood about the factors that modulate this association. The aim of this study was to examine the association between FGIDs and depression and anxiety, and to determine the covariates influencing this association in a New Zealand cohort.The Christchurch IBS cOhort to investigate Mechanisms FOr gut Relief and improved Transit (COMFORT) study is an observational case-control study that recruited FGID cases and healthy controls between 2016 and 2018. In addition to the collection of a wide range of biological samples, participants completed questionnaires concerning socioeconomic status, physical activity, smoking, alcohol intake, anxiety, and depression (the latter two measured using the Hospital Anxiety and Depression Score [HADS]). A multivariate analysis was performed using the significant covariates from the univariate analyses to test whether their effect was independently significant on anxiety and depression.A total of 315 participants (57 with diarrhea-predominant IBS (IBS-D), 30 with constipation-predominant IBS (IBS-C), 41 with mixed-IBS (IBS-M), 16 with functional diarrhea (FD), 42 with functional constipation (FC), and 129 controls); mean age 53 years (range 18-70 years), 221 (70%) female) completed the questionnaires. Anxiety (odds ratio [OR] 2.85 [95% confidence interval [CI] 1.64-4.94, p 0.01) and depression (OR 3.40 [95% CI 1.35-8.55, p = 0.01]) were strongly associated with FGID cases versus controls. Lower economic living status (p 0.01) was an independent covariate associated with depression, while lower economic living status (p 0.005) and abdominal pain (p = 0.005) were both independently associated with anxiety.In addition to the established associations between FGIDs and anxiety and depression, we have shown that the economic standard of living, pain, and IBS phenotype are significant independent covariates. This study demonstrates the range of lifestyle and demographic factors that modulate morbidity associated with FGIDs and may provide targets for intervention.

Systemic lupus erythematosus (SLE) is a systemic, chronic, inflammatory, autoimmune disease characterized by multi-organ and - system involvement such as the kidneys and hematologic and central nervous systems. The clinical presentation of the disease is extremely variable. Systemic findings, nephritis, neuropsychiatric disease, and cytopenia are more common in children than in adults at presentation. Chorea typically manifests with jerky, involuntary, and purposeless movements, involving the face, four limbs, and neck. In addition, patients suffer from dysarthria, gait disturbance, and neuropsychiatric symptoms. Chorea can be the first finding of SLE during childhood. Herein we reported a 13-year female patient who presented with movement and speech disorders and who was diagnosed chorea-related SLE.

Objective: This study aims to evaluate the effectiveness of local hyaluronic acid (HA) administration to surgically remove impacted third molar sockets and measure pain, swelling, and trismus. Materials and Methods: The study included a total of 25 healthy patients aged 18-29 years with asymptomatic bilaterally impacted lower third molars. All cases have been performed under local anesthesia. In the study group, 0.8% HA (Gengigel®) was applied in the postextraction sockets of the right third molars and in the control group nothing was applied to the extraction sockets of the left third molars. Postoperative pain, trismus, and swelling were evaluated on the 1st, 3rd, and 7th postoperative days. Results: No difference was determined between groups in facial swelling and maximum mouth opening. However, the amount of pain significantly reduced in HA groups according to visual analog scale (P = 0.001). Conclusion: The results of this study showed that HA can produce an analgesic action in postextraction sockets after surgical removal of impacted teeth and therefore it has a clinical benefit to reduce usage of nonsteroidal anti-inflammatory drugs after dentoalveolar surgery.Keywords: Hyaluronic acid, pain, surgery, third molar

Contains fulltext : 136496.pdf (Publisher’s version ) (Open Access) OBJECTIVE: The objective of the study was to understand the extent to which financial incentives such as Payment by Results and other payment mechanisms motivate kidney centres in England to change their practices. DESIGN: The study followed a qualitative design. Data collection involved 32 in-depth semistructured interviews with healthcare professionals and managers, focusing on their subjective experience of payment structures. PARTICIPANTS: Participants were kidney healthcare professionals, clinical directors, kidney centre managers and finance managers. Healthcare commissioners from different parts of England were also interviewed. SETTING: Participants worked at five kidney centres from across England. The selection was based on the prevalence of home haemodialysis, ranging from low (8%) prevalence, with at least one centre in each one of these categories at the time of selection. RESULTS: While the tariff for home haemodialysis is not a clear incentive for its adoption due to uncertainty about operational costs, Commissioning for Quality and Innovation (CQUIN) targets and the Best Practice Tariff for vascular access were seen by our case study centres as a motivator to change practices. CONCLUSIONS: The impact of financial incentives designed at a policy level is influenced by the understanding of cost and benefits at the local operational level. In a situation where costs are unclear, incentives which are based on the improvement of profit margins have a smaller impact than incentives which provide an additional direct payment, even if this extra financial support is relatively small.

ADP-ribosylation factor-related protein (ARP) is a membrane-associated GTPase with remote similarity to the family of ADP-ribosylation factors (ARF). In a yeast two-hybrid screen designed to identify proteins interacting with ARP, we isolated a partial cDNA of the ARF-specific guanine nucleotide exchange factor mSec7-1/cytohesin encoding its N terminus and most of the Sec7 domain (codons 1-200). ARP and ARP-Q79L (GTPase-negative ARP) exhibited a higher affinity to mSec7-1-(1-200) than ARP-T31N (nucleotide exchange-defective ARP) in the two-hybrid assay. Similarly, full-length [35S]mSec7-1/cytohesin was specifically adsorbed to glutathione-Sepharose loaded with glutathione S-transferase (GST)-ARP-Q79L, GST-ARP, or GST-ARP-T31N, the latter exhibiting the lowest binding affinity. Overexpression of ARP-Q79L, but not of ARP-T31N, in COS-7 cells reduced the fluorescence from co-expressed green fluorescent protein fused with mSec7-1/cytohesin or mSec7-2/ARNO in plasma membranes as detected by deconvolution microscopy. Recombinant ARP and ARP-Q79L, but not ARP-T31N, inhibited the phospholipase D (PLD) activity stimulated by mSec7-2/ARNO and ARF in a system of isolated membranes. Furthermore, transfection of HEK-293 cells with ARP or ARP-Q79L, but not ARP-T31N, inhibited the muscarinic acetylcholine receptor-3 induced PLD stimulation and translocation of ARF from cytosol to membranes. These data suggest that the GTP-bound form of ARP specifically binds mSec7-1/cytohesin, and that ARP may be involved in a pathway inhibiting the ARF-controlled activity of PLD.

Neuronal death affects selectively granule cell precursors of the cerebellum and the dopaminergic neurons of midbrain in the weaver mutant mouse. The weaver phenotype is associated with a missense mutation in the gene coding for the GIRK2 potassium channel, which results in chronic depolarization. Using DNA gel electrophoresis, electron microscopy (EM), the in situ end-labeling (ISEL) technique at the light and EM level, and immunohistochemistry for apoptosis-related proteins c-Jun and proliferating cell nuclear antigen (PCNA), we have investigated the mechanisms of cell death in cerebellum and substantia nigra. Between postnatal day P1 and P21, in the external germinal layer of the cerebellum, most degenerating granule cell precursors were found to aggregate to form clusters. Degenerating cells exhibited strong nuclear staining for ISEL, c-Jun, and PCNA and had a typical apoptotic morphology by EM. Increased c-Jun and ISEL staining were also occasionally seen in Purkinje cells. Between P14 and P21, when dopaminergic neurons start to degenerate, staining for ISEL, c-Jun, and PCNA in weaver substantia nigra was the same as in controls. By EM, however, we found only in weaver mice numerous dopaminergic cells that showed extensive vacuolar and autophagic changes of cytoplasm, preservation of membrane and organelle integrity, and absence of chromatin condensation or DNA fragmentation by EM-ISEL. The combination of vacuolar and autophagic changes identifies a novel type of non-necrotic, nonapoptotic cell death. After biochemical analysis of DNA, a clear-cut laddering, suggestive of oligonucleosomal fragmentation, was present in samples from weaver cerebellum. Cell death diversity appears to be influenced by specific features of target cells. These findings may be relevant for understanding the mechanisms of cell death in neurodegenerative diseases.

Tez 12-16 yaş grubundaki çocukları bedensel , duygusal , zihinsel gelişimleri içermektedir. Teorik ve uygulamalı olmak üzere iki bölümden oluşmaktadır. Teorik bölümde çocukların bedensel ve fiziksel gelişimi araştırılmış ve ortaya konulmuştur. Uygulamalı bölümde ise çocuklar ile yapılan anket çalışması ve çocuklara yönelik kitaplarla ilgili araştırmalar yapılmıştır. Çocuğun duygu ve düşünceleri esas alınarak , yeterli ilgi, sevgi ve güven ortamında büyümelerinin önemi ortaya konulmuştur. Bir birey olaray çocuğun , toplumun diğer üyeleri arasındaki önemi esas olarak ebeveynlerden beklentileri ortaya konulmuştur. Yaptıkları resimler incelenerek çocuğun resim ile olan yakınlığı anlatılmak istenmiştir. Çocuk zihinsel , bedensel,duygusal ve sosyal gereksinimleri tamamlamak isteyen bir kişidir. Bu nedenle çocuğa gösterilen sevgi, şevkat ve güven duygusu çocuğu olumlu yönde etkileyecektir. Onlara sunacağımız , gereksinmelerini karşılayacağımız her olumlu davranışın , sonucu en iyi şekilde almamızdaki önemine bağlı kalarak, illüstrasyonun çocuk kitaplarındaki önemi ve özelliklerinin neler olacağı belirtilmeye çalışılmıştır. Birinci Bölüm'de Çocuk Gelişimi önemi çocukların karakterleri ile birlikte yaptıkları resimler ele alınmış , ilkokul dönemi ile okul öncesi dönem kıyaslanarak çocuğun geçirdiği evreler anlatılmıştır. İkinci Bölüm'de çocuğun yaratıcı eylem içerisinde bulunması yaptığı resimlerde gösterdiği tipler , genel özellikleri anlatılarak resmin çocuğun yaşamındaki önemi vurgulanmıştır. Üçüncü Bölümde çocuk kitapları ve özelliklerine değinilerek kitabın,herkesin yaşamında değerli olduğu gibi çocuğun yaşamında da etkili olduğu , içeriğinin biçimi ve kahramanlarıyla önemi anlatılmıştır. Öykü kitaplarındaki görsel göstergelerin illüstrasyon , renk,yazı ve grafik tasarımlarıyla birt bütün olduğu verilmeye çalışmmıştır. Dördüncü Bölümde illüstrasyonun önemi ve Türkiye'de geçirdiği evreler anlatılarak hikayelerde ve bütün kitaplarda afişlerde önemli olduğu anlatılmıştır. Beşinci Bölüm tezin uygulamalı bölümü olmuştur ki , burada çocuklara yönelik yapılan anketler, önemi ve sonuçları yer alır. Kitabın , çocuğun zihin geliştirmesindeki önemi, istekleri belirlenerek arzuları doğrultusunda hikaye kitabı oluşturulmuştur. Kitap çocuğun zihin geliştirilmesinde yararlı bir araçtır.

Tez (yüksek lisans) –Ondokuz Mayıs Üniversitesi, 1998 Libra Kayıt No: 31645 İnsanın yaşamsal etkinliklerinde istikrarı sağlamak için oluşturduğu bir araç olan takvim; zaman'ın değerlendirilmesine getirdiği düzen açısından, kendine bağlı kalınarak yaşanması zorunluluğu ile bugün sosyal hayatın vazgeçilmez bir parçasıdır. Toplumumuzun değişik kesimlerinin farklı beklentilerini karşılamak üzere gerçekleştirilmiş takvim tasarımları, niteliksel açıdan önemleri ile orantılı bir düzey göstermemektedir. Bu genel durumun nedenleri ile ilgisinde irdelenmesi araştırmaya değer görülmüştür. Çalışmada, tasarım sürecinin -belirleyici olarakalgılanabilir türden- girdileri arasındaki ilişkilerdüzenini çözümleme amaçlanmaktadır. Takvimin pratik işlevi yanında, kullanımı ile zaman içinde oluşan kültürel ve estetik bir nesne olma özelliği de vardır.Tasarım üzerinde bunların belirleyici etkisi olduğu gibi, tanıtım işlevi (reklam), müşteri beklentileri, hedef kitlenin kültürel yapısı, estetik yargıları ve alışkanlıkları da birer belirleyicidir. Bu çalışmada belirleyiciler ve etkileri karşılaştırmalı örneklerle açımlanmış; aralarındaki ilgiler ortaya konmaya çalışılmıştır. Aynı zamanda öneriler, gerekçeli açıklamaları olan bir dizi takvim serisiyle, uygulamalı olarak da örneklenmiştir. Bu örneklemeler için tür olarak özellikle yaygın kullanımı ve diğer takvim türlerine oranla tasarımcıya sağladığı yüzey genişliği açısından yeni kullanım ve beğeni ölçütleri iletini olarak daha etkiliği olacağına inandığımız duvar takvimleri tercih edilmiştir.
