Search

The factors that were crucial for the construction of administrative buildings in the regional capitals of the Czech Republic are subject to examination in this article. One primary question is whether the development of office construction reflects the qualitative importance of the cities, or whether there are some other regularities in the spatial distribution of construction. To identify the key factors, controlled interviews with experts professionally involved in the construction of administrative buildings were carried out, and these data were then extended as part of a large-scale questionnaire survey with other experts on the issue. The results have confirmed the dominant position of the capital city of Prague in terms of its qualitative importance, as the remaining regional capitals have less than one-tenth of the volume of modern office building areas. The greatest differences in the construction of administrative buildings have been noted in Brno and Ostrava, despite the fact that they exhibit similar characteristics when considered in the light of respondent-determined factors.

Introduction: Combined immuno-oncology (IO) regimens are the cornerstone of the current front-line systemic therapy for metastatic renal cell carcinoma (mRCC). Despite the fact that combined IO regimens show high efficacy, they are often accompanied by a wide spectrum of immune-related adverse effects (irAEs). Case Presentation: We describe a case of rare irAEs manifested as giant cell temporal arteritis (GCA) followed by severe encephalopathy occurring after continuing immunotherapy in a 66-year-old man with mRCC receiving a combination of ipilimumab and nivolumab in the first line of systemic therapy. GCA occurred 4 months after the initiation of IO and responded promptly to the low-dose prednisone therapy. Four months after the continuation of nivolumab maintenance, the patient was hospitalized due to severe irAE encephalopathy which presented as psycho-behavioral abnormalities and progressive cognitive decline. He was treated with high-dose methylprednisolone which led to complete resolution of the symptoms and IO was permanently discontinued. The patient achieved a durable partial response. Conclusion: Both GCA and the subsequent encephalopathy in our patient responded well to the corticosteroid therapy, leading to the complete resolution of the symptoms and the patient achieved a durable partial response. Although the risk of severe neurologic irAEs affecting the central nervous system induced by IO re-administration, following previous discontinuation due to irAE, is not well-defined because of their rarity, this case highlights the need for caution, particularly in cases with a history of previous irAE-associated GCA. Aim: Percutaneous transthoracic needle biopsy (PTNB), an alternative to bronchoscopic confirmation of lung lesions, is today being associated with a risk of pneumothorax and hemorrhage. Further, there are no data on the possible risk of malignant disease spreading to the pleura at the site of the PTNB. Previous studies have dealt with this risk in stage I non-small cell lung cancer only. The aim of this study was thus to assess the risk of pleural recurrence for all types of lung lesions. Secondary objectives included assessment of diagnostic yield and safety with respect to the incidence of pneumothorax and hemorrhage., Methods: Clinical data of all patients from the University Hospital in Pilsen who had undergone PTNB of lung lesions between 1.1.2018 and 31.12.2022 were included in this retrospective study., Results: Following PTNB, ipsilateral pleural effusion occurred in 4.8% of patients without prior pleural infiltration. The effusion was confirmed as malignant in one patient (0.7%). Diagnostic yield of the method was 86.6%. We recorded pneumothorax or hemorrhage in the lung parenchyma or pleural space requiring medical intervention in 3.4% and 1.1% of patients, respectively., Conclusion: In our study, percutaneous transthoracic needle biopsy of lung lesions showed high sensitivity and low degree of acute complications requiring an invasive solution. The risk of pleural recurrence after a biopsy was very low. Background/aim: Circulating tumour DNA (ctDNA) represents an emerging biomarker in non-small cell lung cancer (NSCLC). We focused on the combination of ctDNA and positron emission tomography/computed tomography (PET/CT) in the follow-up monitoring of advanced-stage NSCLC patients treated with chemotherapy., Patients and Methods: Eighty-four patients were enrolled in this study. 18 F-fluorodeoxyglucose PET/CT and ctDNA assessments were performed at baseline and after two cycles of chemotherapy (follow-up)., Results: There was a correlation of ctDNA with metabolic tumour volume (MTV), total lesion glycolysis (TLG), and iodine concentration (IC) at baseline (p=0.001, p=0.001, p=0.003) and at follow-up (p=0.006, p=0.002, p=0.001). The objective response was associated with follow-up ctDNA (p<0.001) and the change of all PET/CT parameters. ROC analyses showed that the combination of follow-up ctDNA with changes in SUVmax is very promising for the estimation of objective response and progression-free survival., Conclusion: The combination of ctDNA assessment with PET/CT is a promising approach for the follow-up monitoring of therapy response and prognosis estimation of advanced-stage NSCLC patients., (Copyright © 2022, International Institute of Anticancer Research (Dr. George J. Aim: To introduce the possible benefits of the positron-emission tomography (PET)/computed tomography (CT) with 18 F-3'-deoxy-3'-fluorothymidine ( 18 F-FLT) in patients with orofacial carcinomas and its impact to patient management., Materials and Methods: Thirty-six patients with orofacial squamous cell carcinomas underwent 18 F-FLT-PET/CT during radiotherapy. Examinations were performed after administration of 18 F-FLT (1.8 MBq/kg) including full-diagnostic CT. Analysis of the radiotherapy effect was performed with possible prospect of repeated and focused irradiation., Results: Complete absence of 18 F-FLT uptake was found in 20 patients, thus complete response to radiotherapy was reported. Persistence of focal 18 F-FLT uptake was observed in 16 patients; in 11 patients, the measured activity was only mild. In five patients, a higher level of 18 F-FLT uptake was measured and additional irradiation was performed in defined regions. Repeated follow-up proved complete regression 18 F-FLT uptake., Conclusion: It was possible to assess the effect of radiotherapy with the use of 18 F-FLT-PET/CT and findings are suitable for radiation dose-escalation planning., (Copyright© 2018, International Institute of Anticancer Research (Dr. George J. Aim: to evaluate the performance of N-staging assessment in clinically-proven T1 breast carcinoma by high-resolution 18 F-fluorodeoxyglucose-positron-emission tomography/computed tomography ( 18 F-FDG-PET/CT) using time-of-flight with point-spread function reconstruction., Patients and Methods: In 30 women with clinically proven T1 breast carcinoma, imaging before surgery was performed using 18 F-FDG-PET/CT. The results of PET/CT in detection of lymph node metastases were compared with those obtained after pathological investigation of axillary biopsy. A four-ring PET subsystem with image reconstruction using time-of-flight and point-spread function was used with the radiopharmaceutical dose reduction to 2.5 MBq/kg., Results: Axillary lymph node metastasis was confirmed by histology in 13 patients, but metastasis was suspected based on PET/CT in 12 of those patients, the absence of metastasis was surgically confirmed in 17 women, 15 of which were suspected based on PET/CT. The sensitivity for detection of axillary lymph node metastasis was 93.3%, with a specificity of 88.2% in the whole patient cohort. Additionally, distant metastatic spread was found in 13.3% of patients., Conclusion: The reconstruction of PET images with time-of-flight and point-spread function enabled the improvement of diagnostic performance in N-staging of breast carcinoma, even when the dose of radiopharmaceutical was reduced to 2.5 MBq/kg., (Copyright© 2018, International Institute of Anticancer Research (Dr. George J. Aim: To evaluate our own experience with 18 F- fluoromethylcholine-(FCH)-positron-emission tomography combined with magnetic resonance imaging (PET/MRI) in restaging of patients with prostatic carcinoma and elevated serum prostate-specific antigen (PSA) level., Patients and Methods: The analysis was performed on a sample of 100 male patients who underwent 18 F-FCH-PET/MRI, with a mean age of 63.2 years (range=47-78 years). The imaging was performed using an integrated PET/MRI hybrid system after intravenous application of 18 F-FCH at a dose of 1.25 MBq/kg. The number and sites of pathological accumulation of FCH related to local recurrence, nodal spread and skeletal metastases were compared to corresponding MRI findings; furthermore, the relation of PSA level and presence of FCH accumulation in tumorous tissue was assessed; finally we correlated the findings of different sites of metastatic involvement., Results: In patients with a PSA level up to 2 ng/ml, accumulation in tumorous tissue of local recurrence or metastases was found in 83.33% in cases of biochemical relapse, and in patients with PSA level above 5 ng/ml in 100% of cases. In general, we found any finding explained rise of PSA level in 94% of patients., Conclusion: 18 F-FCH-PET/MRI using an integrated system with 1,25 MBq/kg dosing of FCH is a valuable tool in evaluation of restaging in patients with prostatic carcinoma, with high detection rate even in those with a low serum PSA level., (Copyright© 2018, International Institute of Anticancer Research (Dr. George J. Aim: The aim of the study was to assess possible correlation of fluorogeoxyglucose (FDG) uptake and iodine-related attenuation values derived from positron-emission tomography/computed tomography (PET/CT) using single-source dual-energy CT scan (DE-CT) in non-small cell lung cancer (NSCLC). Materials and Methods: Forty-eight patients with histologically-proven NSCLC underwent 18 F-FDG-PET/CT within their staging process. PET/CT included single-source DE-CT in late post-contrast phase. Direct comparison of PET and DE-related values was performed. A sub-study regarding different histological types and various thresholds for quantification of volume metabolic values was also performed. Results: A strong correlation was found of metabolic tumor volume and total lesion glycolysis with total iodine content using Pearson correlation analysis (r=0.965-0.983; p<0.0001) with various thresholds for FDG lesion segmentation. The strongest correlations with iodine content were reached using 10% threshold for segmentation. Only a weak correlation was found between iodine content and the maximal standard uptake value. A significant difference between adenocarcinomas and other histological subtypes was found for selected parameters of metabolic PET and DE-CT data. Conclusion: Our study demonstrated a strong correlation of the iodine content calculated from single-source DE-CT with volumetric FDG parameters in NSCLC. without a significant effect of the threshold value for FDG lesion segmentation., (Copyright© 2018, International Institute of Anticancer Research (Dr. George J. Background/aim: The aim of this study was to evaluate the possibility to discriminate the blood clot from tumorous tissue in the assessment of transitional cell carcinoma (TCC), when CT data are used only from single-phase dual-energy CT (DECT)., Materials and Methods: A total of 18 patients were included into the retrospective analysis of the prospectively collected data (the average age was 67.3 years, range=62-79 years, 12 males, 6 females). DECT was performed in all patients after intravenous administration of the iodinated contrast material. DECT analysis using three-material decomposition algorithm was used to discriminate blood and enhancing tumorous tissue. In all patients, TCC was detected and subsequently kidney surgery or endoscopy was performed within the next two weeks. The findings were compared with DECT results., Results: In our cohort of patients we found 9 TCC of the renal pelvis or ureter and 7 TCC of the urinary bladder. The accuracy of the TCC detection was 94% (17/18). The presence of coagulated blood within the urinary collecting system together with a tumor was detected in 7 CT examinations. The blood coagulum without a confirmed malignant tumor was found in one case with false positive DECT finding. In other 6 cases bleeding was confirmed during surgery or endoscopy (accuracy 87%)., Conclusion: The dual-energy data analysis enables discrimination of a blood clot from tumorous tissue. Background/aim: Hepatic epithelioid haemangio-endothelioma (HEHE) is a very rare malignant tumor of vascular origin and uncertain biological behaviour that is difficult to diagnose using preoperative radiology diagnostic techniques., Patients and Methods: The authors present here two patients with HEHE of different extent. The first patient had a generalised form of disease, with involvement of the liver, lungs and bones and was treated with a combination of bevacizumab and capecitabine. The second patient had a localised form of disease involving the liver and this was resolved using a combination of liver resection and radiofrequency ablation. In both patients, the radiology work-up before surgery was non-specific and metastases of another malignant process were considered. The definitive histological diagnosis was made by the pathologist on the basis of immunohistochemical analysis that demonstrated the presence of CD31, CD34 and calmodulin-binding transcription activator 1 (CAMTA 1)., Results: Both patients remain in an overall good condition 27 and 5 months respectively following treatment for HEHE., Conclusion: Preoperative radiological diagnosis of HEHE is difficult and immunohistochemical examination of the tumor tissue sample remains the key diagnostic tool. Background: Patients with sensitive EGFR mutations are already being treated with first and second generation tyrosine kinase inhibitors (TKIs). However, resistance to these drugs occurs over time, and over half of all cases is caused by a mutation (T790M) in the EGFR kinase domain. Osimertinib offers a new treatment option that overcomes this problem. Unfortunately, resistance to this drug also develops after several months of treatment and is caused by another mutation (C797S) in EGFR., Case Report: Our case report provides evidence for the progressive development of EGFR-TKI resistance in a patient with a deletion of exon 19 in the EGFR gene. First, based on a mutation (T790M) identified after afatinib treatment and a subsequent mutation (C797S) mutation identified after osimertinib treatment. We mention overcoming this resistance (C797S) mutation by using 4th generation EGFR-TKI and other alternative procedures (chemotherapy, immunotherapy, and combinations of older EGFR-TKI generations). We also mention a rare case of peritoneal metastasis that occurred after previous treatment with osimertinib that we attempted to ameliorate by using erlotinib because the impaired condition of the patient did not allow treatment by chemotherapy. There are documented cases in which erlotinib has been successfully given to patients with peritoneal metastases and patients with the EGFR mutation C797S following progression to afatinib. This was not the case in our patient, probably because of the remaining EGFR mutation T790M., Conclusion: In our case report, erlotinib did not show efficacy after progression to osimetinib. Nowadays, chemotherapy is the only possible treatment in patients with good a performance status. Aim: The purpose of the present study was to evaluate the possibility of detection, staging and differentiation assessment of hepatocellular carcinoma (HCC) using a combination of dual-phase computed tomography (CT)-angiography in the arterial and portal phase with positron emission tomography (PET) imaging using (18)F-fluorodeoxyglucose ((18)FDG)., Patients and Methods: From a set of 10,000 patients who underwent (18)FDG-PET/CT, we examined a total of 65 patients (52 males, 13 females; mean age=61.7 years, ranging from 35-82 years) with HCC. The imaging included CT data acquisition after intravenous application of iodinated contrast material in arterial and portal phases, allowed to obtain data in CT angiography quality. Histological diagnosis of the resection sample (21), biopsy (37) or necropsy (7), including the evaluation of the hepatocytary origin of the tumor and the grade of its differentiation, was determined in all patients., Results: The most sensitive sign in the detection of HCC was the alternative presence of hypervascularity or hyperaccumulation of (18)F FDG that reached 93.8%. The high level of (18)F-FDG accumulation showed sensitivity of 84.1% and specificity of 75.0% for distinguishing between well- and poorly differentiated HCC., Conclusion: The combination of the dual-phase CT angiography with (18)FDG PET helps in the assessment of staging and differentiation of HCC and has an important role in treatment decision-making., (Copyright© 2015 International Institute of Anticancer Research (Dr. John G. Aim: To evaluate the proliferation activity in gliomas using 18F-fluorothymidine (18F-FLT)-positron emission tomography/computed tomography (PET/CT)., Patients and Methods: Samples of 26 tumors were analyzed (mean age=51.6; range=26-72 years; 16 males, 10 females). All examinations were performed using a PET/CT scanner equipped with lutetium oxyorthosilicate (LSO) detectors. All data were acquired with a delay of 15 min, following intravenous application of 18F-FLT (dosed 2 MBq/kg of body weight). The PET/CT contained CT after intravenous application of iodinated contrast agent and high-resolution brain PET acquired during 15 min in one position. PET/CT was performed before confirmation of the histological diagnosis and the level of 18F-FLT accumulation was compared to the grading of the tumor evaluated using immunohistochemistry staining of Ki-67. Samples were obtained by stereotactic biopsy (5×) or surgical resection (21×)., Results: Five tumors of grade IV, 7 tumors of grade III and 14 tumors of grade II were found. Pre-bioptical discrimination between high-grade and low-grade tumors reached accuracy 92.3% (24/26), sensitivity 92.3% (12/13) and specificity 92.9 (13/14). The mean maximum standardized uptake value (SUVmax) in high-grade tumors was 2.23, significantly different from low-grade tumors (mean SUVmax 0.61, T=7.803, p<0.0001)., Conclusion: 18F-FLT-PET/CT enables to estimate the proliferation activity of glioma before biopsy., (Copyright© 2015 International Institute of Anticancer Research (Dr. John G.
