2. An evaluation of a mobile telehealth intervention for people with insulin-requiring diabetes

Abstract

This thesis describes the evaluation of a mobile telehealth intervention for people with diabetes. In line with the gaps in the literature identified in a systematic review of mobile telehealth interventions in diabetes, the aims of this thesis were to test the effectiveness of a mobile telehealth intervention, to examine the mechanisms of action of the intervention, to identify predictors of benefit and of telehealth usage, to explore patients’ perspectives on telehealth and to assess intervention fidelity. A randomized controlled trial comparing standard care to standard care supplemented with mobile telehealth was conducted and included 81 participants with insulin-requiring and poorly controlled diabetes. Participants completed assessments at baseline, 3 months and 9 months. Outcomes included glycaemic control (HbA1c), self-management behaviours, quality of life and health care utilization. Efficacy beliefs and personal models of diabetes were assessed as process variables. In addition, mobile telehealth participants completed assessments of acceptability of telehealth, perceived adequacy of training and self-efficacy to use telehealth. Semi-structured interviews conducted with intervention group participants having completed the study explored patients’ perceptions and experiences using mobile telehealth. Results indicated the intervention had no effect on study outcomes. Efficacy beliefs in the intervention group were found to improve significantly compared to the control group and there was some evidence in mediation analyses to suggest these changes mediated improvements in study outcomes. Some evidence also suggested that improvements in efficacy beliefs were predicted by clinical variables. In terms of patients’ usage of telehealth, weekly frequency of self-monitoring of blood glucose and dietary behaviours at baseline were the strongest predictors, but results also suggested that acceptability, self-efficacy to use telehealth, perceived adequacy of training, and the number of contacts with telehealth providers were related to telehealth usage. The qualitative interviews highlighted features of the mobile telehealth equipment that were particularly popular. They also provided valuable information on the perceived effects of mobile telehealth and on key areas of improvement for the successful implementation of telehealth in routine practice. Data collected on the delivery of the intervention suggested it was unlikely to have been delivered faithfully.

Abstract

Mountain lakes are often situated in protected natural areas, a feature that leads to their role as sentinels of global environmental change. Despite variations in latitude, mountain lakes share many features, including their location in catchments with steep topographic gradients, cold temperatures, high incident solar and ultraviolet radiation (UVR), and prolonged ice and snow cover. These characteristics, in turn, affect mountain lake ecosystem structure, diversity, and productivity. The lakes themselves are mostly small, and up until recently, have been characterized as oligotrophic. This paper provides a review and update of the growing body of research that shows that sediments in remote mountain lakes archive regional and global environmental changes, including those linked to climate change, altered biogeochemical cycles, and changes in dust composition and deposition, atmospheric fertilization, and biological manipulations. These archives provide an important record of global environmental change that pre-dates typical monitoring windows. Paleolimnological research at strategically selected lakes has increased our knowledge of interactions among multiple stressors and their synergistic effects on lake systems. Lakes from transectsacross steep climate (i.e., temperature and effective moisture) gradients in mountain regions show how environmental change alters lakes in close proximity, but at differing climate starting points. Such research in particular highlights the impacts of melting glaciers on mountain lakes. The addition of new proxies, including DNA-based techniques and advanced stable isotopic analyses, provides a gateway to addressing novel research questions about global environmental change. Recent advances in remote sensing and continuous, high-frequency, limnological measurements will improve spatial and temporal resolution and help to add records to spatial gaps including tropical and southern latitudes. Mountain lake records provide a unique opportunity for global scale assessments that provide knowledge necessary to protect the Earth system.

Abstract

Excited states in the neutron-deficient nucleus 161 72Hf89 have been populated using the 118Sn(48Ti,5n) 161Hf and 110Pd(56Fe,5n) 161Hf fusion-evaporation reactions at 240 and 270 MeV, respectively. The level scheme for 161Hf has been extended with the observation of new band structures and an I π = (13/2+) isomeric state with a half-life of 4.8(2) μs has been identified. The decay path from this isomer to the (7/2−) ground state is established. The yrast band, based on the (13/2+) isomeric state, is extended up to (73/2+) and side band structures are identified up to (69/2−) and (55/2−). Quasiparticle assignments for these rotational structures are made on the basis of their alignment properties and Woods-Saxon cranked shell-model calculations. peerReviewed

Abstract

Summary: Background: Minimal hepatic encephalopathy (HE) and HE grade 1 (HE1) according to the West Haven criteria have recently been grouped as one entity named—covert HE‐ (CHE). Data regarding the impact of CHE on health‐related quality of life (HRQoL) and sleep quality are controversial. Aim: First, to determine whether CHE affects HRQoL and sleep quality of cirrhotic patients and second, whether minimal HE (MHE) and HE1 affect HRQoL and sleep quality to a comparable extent. Methods: A total of 145 consecutive cirrhotic patients were enrolled. HE1 was diagnosed clinically according to the West Haven criteria. Critical flicker frequency and the Psychometric Hepatic Encephalopathy Score were used to detect MHE. Chronic Liver Disease Questionnaire (CLDQ) was used to assess HRQoL and Pittsburgh Sleep Quality Index (PSQI) was applied to assess sleep quality. Results: Covert HE was detected in 59 (40.7%) patients (MHE: n = 40; HE1: n = 19). Multivariate analysis identified CHE (P < 0.001) and female gender (P = 0.006) as independent predictors of reduced HRQoL (CLDQ total score). CHE (P = 0.021), low haemoglobin (P = 0.024) and female gender (P = 0.003) were identified as independent predictors of poor sleep quality (PSQI total score). Results of CLDQ and PSQI were comparable in patients with HE1 and MHE (CLDQ: 4.6 ± 0.9 vs 4.5 ± 1.2, P = 0.907; PSQI: 11.3 ± 3.8 vs 9.9 ± 5.0, P = 0.3). Conclusion: Covert HE was associated with impaired HRQoL and sleep quality. MHE and HE1 affected both outcomes to a comparable extent supporting the use of CHE as a clinically useful term for patients with both entities of HE in clinical practice. [ABSTRACT FROM AUTHOR]

Abstract

A classic region of band termination at high spin occurs in rare-earth nuclei with around ten valence nucleons above the 146Gd closed core. Results are presented here for such non-collective oblate (γ = 60°) terminating states in odd-Z 155Ho, odd-odd 156Ho, and even-even 156Er, where they are compared with neighbouring nuclei. In addition to these particularly favoured states, the occurrence of collective triaxial strongly deformed (TSD) bands, bypassing the terminating states and extending to over 65ħ, is reviewed.
