1. Mucormycosis of the paranasal sinuses in a patient with acute myeloid leukemia

Abstract

Introduction. Invasive fungal infection is among the leading causes of morbidity, mortality, and economic burden for patients with acute leukemia after induction of chemotherapy. In the past few decades, the incidence of invasive fungal infection has increased dramatically. Its management has been further complicated by the increasing frequency of infection by non-Aspergillus molds (e.g. Mucorales). Neutropenic patients are at a high risk of developing an invasive mucormycosis with fulminant course and high mortality rate (35-100%). Case Outline. We are presenting the case of a 72-year-old male with an acute monoblastic leukemia. The patient was treated during five days with hydroxycarbamide 2 × 500 mg/day, followed by cytarabine 2 × 20 mg/sc over the next 10 days. He developed febrile neutropenia, headache, and edema of the right orbital region of the face. Computed tomography of the sinuses revealed shadow in sinuses with thickening of mucosa of the right paranasal sinuses. Lavage and aspirate from the sinuses revealed Rhizopus oryzae. Mucormycosis was successfully treated with amphotericin B (5 mg/kg/day) followed by ketoconazole (400 mg/day). Two months later the patient died from primary disease. Conclusion. In patients with acute leukemia who developed aplasia, febrile neutropenia, and pain in paranasal sinuses, fungal infection should be taken into consideration. New and non-invasive methods for taking samples from sinuses should be standardized in order to establish an early and accurate diagnosis of mucormycosis with the source in paranasal sinuses, and to start early treatment by a proper antifungal drug. Clear communication between physician and mycologist is critical to ensure proper and timely sampling of lavage and aspirate from sinuses and correct specimen processing when mucormycosis is suspected clinically. 2. Squeezing Bose-Bose liquid drops

Abstract

We explore ultradilute Bose-Bose liquid droplets squeezed by an external harmonic potential in one spatial direction. Our theoretical study is based on a functional that is built using quantum Monte Carlo results of the bulk phase and incorporates finite-range effects. A characteristic feature of these drops is the existence of a critical atom number, that is the minimum number of particles to have a many-body bound state. We report results on the critical atom numbers for different magnetic fields and applying confinement strengths towards a quasi-two-dimensional setup. In the regime where the local density approximation is expected to be valid, we find that the critical atom number decreases linearly with the harmonic oscillator length of the confining potential. With the largest squeezing explored in our work, we predict stable drops at the level of one thousand atoms. Our functional reduces the critical numbers for any confinement and applied magnetic field with respect to the estimations based on the Lee-Huang-Yang model. We observe saturated drops when the number of atoms in the drop is much larger than the critical value, their central density being higher for the quantum Monte Carlo functional than for the Lee-Huang-Yang one., Comment: 7 pages, 6 figures

3. Spectroscopic substellar initial mass function of NGC 2244

Abstract

We aim at characterizing the low-mass (sub)stellar population of the central portion (2.4 pc$^2$) of the $\sim$2 Myr old cluster NGC 2244 using near infrared spectroscopy. By studying this cluster, characterized by a low stellar density and numerous OB stars, we aim at exploring the effect that OB stars may have on the production of BDs. We obtain near infrared HK spectroscopy of 85 faint candidate members of NGC 2244. We derive the spectral type and extinction by comparison with spectral templates. We evaluate cluster membership using three gravity-sensitive spectral indices based on the shape of the $H$-band. Furthermore, we evaluate the infrared excess from Spitzer of all the candidate members of the cluster. Finally, we estimate the mass of all the candidate members of the cluster and derive the initial mass function, star-to-BD number ratio and disk fraction. The initial mass function is well represented by a power law ($dN/dM \propto M^{-\alpha}$) below 0.4 $M_\odot$, with a slope $\alpha$ = 0.7-1.1 depending on the fitted mass range. We calculate a star-to-BD number ratio of 2.2-2.8. We find the low-mass population of NGC 2244 to be consistent with nearby star-forming regions, although it is at the high-end of BD production. We find BDs in NGC 2244 to be on average closer to OB stars than to low-mass stars, which could potentially be the first evidence of OB stars affecting the formation of BDs. We find a disk fraction of all the members with spectral type later than K0 of 39$\pm$9% which is lower than typical values found in nearby star-forming regions of similar ages., Comment: Accepted by A&A, KMOS spectroscopic data will be made public on Vizier upon publication

Abstract

Pancreatic beta cells are coupled excitable oscillators that synchronize their activity via different communication pathways. Their oscillatory activity manifests itself on multiple timescales and consists of bursting electrical activity, subsequent oscillations in the intracellular Ca2+, as well as oscillations in metabolism and exocytosis. The coordination of the intricate activity on the multicellular level plays a key role in the regulation of physiological pulsatile insulin secretion and is incompletely understood. In this contribution, we investigate theoretically the principles that give rise to the synchronized activity of beta cell populations by building up a phenomenological multicellular model that incorporates the basic features of beta cell dynamics. Specifically, the model is composed of coupled slow and fast oscillatory units that reflect metabolic processes and electrical activity, respectively. Using a realistic description of the intercellular interactions, we study how the combination of electrical and metabolic coupling generates collective rhythmicity and shapes functional beta cell networks. It turns out that while electrical coupling solely can synchronize the responses, the addition of metabolic interactions further enhances coordination, the spatial range of interactions, increases the number of connections in the functional beta cell networks, and ensures a better consistency with experimental findings. Moreover, our computational results provide additional insights into the relationship between beta cell heterogeneity, their activity profiles, and functional connectivity, supplementing thereby recent experimental results on endocrine networks.

Abstract

Allergic fungal sinusitis (AFS) is a chronic non­invasive disease. Hypersensitive immune response is usually initiated by allergens of filamentous fungi Aspergillus, Penicillium, Cladosporium, Fusarium, Bipolaris, Curvularia and Alternaria. AFS is a clinical and immune analogue of the allergic bronchopulmonary aspergillosis (ABPA) as the sinus exudate resembles that of the bronchoalveolar lavage (BAL) in ABPA. Patients with AFS are usually immunocompetent, atopic and males. The most common symptoms are headache, fullness in the paranasal sinuses, and difficult breathing through the nose. Clinically, there is a chronic mucosal inflammation and histopathologic finding shows allergic mucin and eosinophils. Specific staining methods, Gomori’s Methenamine Silver (GMS) or periodic acid­Schiff (PAS), are used for microscopic visualisation of hyphae, which are, in addition to the isolated fungi, most reliable evidence of AFS. Computerized tomography (CT) of paranasal sinuses shows the areas of hyperdensity. In cases where AFS is complicated by the erosion of bone tissue, discontinuation of the sinus bone wall can be seen. Significant laboratory finding, which correlate highly with the AFS, are high immunoglobulin E (IgE) antibodies specific for fungi, detected by the skin prick test or in serum. Treatment is often surgical, and after removal of the allergic mucin, therapy involves oral and nasal corticosteroids, immunotherapy and locally applied antimycotics (with verified fungal etiology). During treatment, the total/specific IgE is monitored - concentration increases with the development of AFS, and decreases during the improvement process. Knowledge of the pathophysiological mechanisms of AFS is scarce, and represents the focus of further research in order to define an optimal diagnostic and therapeutic approach. 10. Revisiting bright delta Scuti stars and their period-luminosity relation with TESS and Gaia DR3

Abstract

We have used NASA's TESS mission to study catalogued delta Scuti stars. We examined TESS light curves for 434 stars, including many for which few previous observations exist. We found that 62 are not delta Scuti pulsators, with most instead showing variability from binarity. For the 372 delta Scuti stars, we provide a catalogue of the period and amplitude of the dominant pulsation mode. Using Gaia DR3 parallaxes, we place the stars in the period-luminosity diagram and confirm previous findings that most stars lie on a ridge that corresponds to pulsation in the fundamental radial mode, and that many others fall on a second ridge that is a factor two shorter in period. This second ridge is seen more clearly than before, thanks to the revised periods and distances. We demonstrate the value of the period-luminosity diagram in distinguishing delta Scuti stars from short-period RR Lyrae stars, and we find several new examples of high-frequency delta Scuti stars with regular sequences of overtone modes, including XX Pyx and 29 Cyg. Finally, we revisit the sample of delta Scuti stars observed by Kepler and show that they follow a tight period-density relation, with a pulsation constant for the fundamental mode of Q = 0.0315 d., Comment: accepted by MNRAS

Abstract

ObjectivesTo provide a basis for clinical management decisions in Purpureocillium lilacinum infection.MethodsUnpublished cases of invasive P. lilacinum infection from the FungiScope® registry and all cases reported in the literature were analysed.ResultsWe identified 101 cases with invasive P. lilacinum infection. Main predisposing factors were haematological and oncological diseases in 31 cases (30.7%), steroid treatment in 27 cases (26.7%), solid organ transplant in 26 cases (25.7%), and diabetes mellitus in 19 cases (18.8%). The most prevalent infection sites were skin (n = 37/101, 36.6%) and lungs (n = 26/101, 25.7%). Dissemination occurred in 22 cases (21.8%). Pain and fever were the most frequent symptoms (n = 40/101, 39.6% and n = 34/101, 33.7%, respectively). Diagnosis was established by culture in 98 cases (97.0%). P. lilacinum caused breakthrough infection in 10 patients (9.9%). Clinical isolates were frequently resistant to amphotericin B, whereas posaconazole and voriconazole showed good in vitro activity. Susceptibility to echinocandins varied considerably. Systemic antifungal treatment was administered in 90 patients (89.1%). Frequently employed antifungals were voriconazole in 51 (56.7%) and itraconazole in 26 patients (28.9%). Abstract

5G is finally here. Initial deployments are already operational in several major cities and first 5G-capable devices are being released. Though it is not limited only to millimeter wave deployments, the main promise of 5G lies in the utilization of the high bandwidth available at high frequencies. However, high-frequency deployments are coverage-limited and require denser placement of base stations, which can increase the cost significantly. One of the main contributing factors to the cost is fiber deployment. Integrated access backhauling (IAB), where part of the wireless spectrum is used for the backhaul connection of base stations instead of fiber, is an attractive solution that could make dense deployments economically viable. With this main objective, 3GPP is in the process of standardizing multi-hop IAB networks. 29. Global optimization using Gaussian Processes to estimate biological parameters from image data

Abstract

Parameter estimation is a major challenge in computational modeling of biological processes. This is especially the case in image-based modeling where the inherently quantitative output of the model is measured against image data, which is typically noisy and non-quantitative. In addition, these models can have a high computational cost, limiting the number of feasible simulations, and therefore rendering most traditional parameter estimation methods unsuitable. In this paper, we present a pipeline that uses Gaussian process learning to estimate biological parameters from noisy, non-quantitative image data when the model has a high computational cost. This approach is first successfully tested on a parametric function with the goal of retrieving the original parameters. We then apply it to estimating parameters in a biological setting by fitting artificial in-situ hybridization (ISH) data of the developing murine limb bud. We expect that this method will be of use in a variety of modeling scenarios where quantitative data is missing and the use of standard parameter estimation approaches in biological modeling is prohibited by the computational cost of the model.

Abstract

Morphogenesis, the process by which an adult organism emerges from a single cell, has fascinated humans for a long time. Modelling this process can provide novel insights into development and the principles that orchestrate the developmental processes. This chapter focusses on the mathematical description and numerical simulation of developmental processes. In particular, we discuss the mathematical representation of morphogen and tissue dynamics on static and grow- ing domains, as well as the corresponding tissue mechanics. In addition, we give an overview of numerical methods that are routinely used to solve the resulting systems of partial differential equations. These include the finite element method and the Lattice Boltzmann method for the discretisation as well as the arbitrary Lagrangian-Eulerian method and the Diffuse-Domain method to numerically treat deforming domains.

35. Usage, knowledge and attitudes towards electronic cigarettes use among nursing students in Croatia: a cross-sectional study.

Abstract

Background: Evidence shows harmful effects of e-cigarettes on health. There is limited data on the use of e-cigarettes among nursing students. This study aimed to investigate the usage, knowledge, and attitudes towards e-cigarettes among nursing students in Croatia, as well as their views on the role of nurses in counseling patients and assisting with smoking cessation. Methods: This cross-sectional study was conducted via an online questionnaire among nursing students at 10 universities in Croatia in March/April 2024. Students were asked questions about socio-demographics, knowledge and attitudes towards e-cigarettes and their views on the role of nurses in counseling patients and assisting with smoking cessation. Results: The study included 1,039 participants; 89% were women, 43% were smokers; 53% used e-cigarettes and 76% used them for recreational purposes. 21% of participants agreed that e-cigarettes are an effective method for quitting smoking. More than half indicated that e-cigarettes could encourage non-smokers to start smoking. The majority (60%) reported that they had never received education on smoking cessation at university. Furthermore, 66% stated that they did not feel confident advising smokers about e-cigarettes. The majority (70%) considered physicians to be the most knowledgeable health experts to advise and educate users of e-cigarettes. There were no significant differences in attitudes between smokers and non-smokers on the use of e-cigarettes. Participants demonstrated insufficient knowledge about e-cigarettes. Non-smokers had more positive attitudes towards the role of nurses in combating smoking compared to smokers. Participants who had received education on smoking cessation were more willing to advise smokers and had clearer views on the risks and the importance of the nurse’s role in the quitting process compared to those who had not received such education. Smoking status and age were significant predictors of counseling confidence. Conclusion: The study found a high prevalence of smoking and e-cigarette use among Croatian nursing students, along with limited knowledge and generally negative attitudes towards e-cigarettes. It highlighted gaps in education regarding the health risks of e-cigarettes and the lack of practical skills needed to help smokers quit, suggesting that nursing programs should update their curricula to better prepare students in these areas. 36. Dental Antimicrobial Stewardship: Developing a Mobile Application for Rational Antibiotic Prescribing to Tackle Misdiagnosis.

Abstract

Background/Objectives: Inexperienced dentists and dental students are especially prone to misdiagnosis, and this represents a huge problem regarding antimicrobial stewardship. We aimed to develop a mobile app for rational antibiotic prescribing in dentistry based on local–systemic symptoms and patient factors, rather than solely on diagnosis, to tackle misdiagnosis. Methods: The study involved 64 participants, 50 of which were third-year dental students attending a pharmacology course focusing on antimicrobials, comprising lectures and practical sessions without (noAPP group, n = 22) or with (APP group n = 28) the assistance of a mobile application. The other 14 participants were practicing dentists who decided to register and use the application. All registered users of the application were asked to take a feedback survey, while learning outcomes were evaluated via a pharmacology quiz. Results: A decision tree was used for application development. In total, 76 impressions were collected on the application. The majority of the impressions were related to odontogenic–endodontic infections. Multiple linear regression analysis did not reveal differences in survey responses between practicing dentists and undergraduate students in the feedback survey responses. There was a significant difference in the mean pharmacology test scores between the noAPP and APP groups (5.50 ± 1.80 vs. 7.21 ± 1.03, p = 0.0001). Conclusions: The dentalantibiotic.com application was developed to support rational antibiotic prescribing, in view of tackling misdiagnosis, among inexperienced dentists, as well as to assist in undergraduates' pharmacology learning, and the current study shows its large impact as an educational tool. Abstract

Lider cheese is a commercial full- fat semi-hard cheese produced from pasteurized goat`s milk in mountain area of Nikšić, Montenegro. The aim of this paper is to characterize the change of protein and fatty acid profiles, mineral content and antioxidant potential during the period of one month of its ripening. Proteolysis was monitored using electrophoresis of total extracts and water-soluble fractions under reducing conditions. Fatty acid profiles were determined by gas chromatography whereas the mineral content of cheese at different stage of ripening was determined by Inductively coupled plasma - optical emission spectrometry (ICP-OES) for macroelements and Inductively coupled plasma mass spectrometry (ICP-MS) for microelements, respectively. One month of ripening of Lider cheese is characterized by a relatively slow proteolysis which can be attributed to intensive lose of moisture, by the increase in desirable fatty acid content and consequently by the improvement of fatty acid healthy indices. After 30 days of ripening, mainly due to the degradation of αs -CN, the WSN/ TN and TCA/TN contents increased from 4.69 % to 6.06 % and 1.15 % to 1.90 %, respectively. One- month-old cheese had improved indexes of lipid quality as well as improved reducing power and ability to scavenge free radicals. Ripening did not affect mineral content of cheese if it is expressed on dry matter basis. According to obtained results, the optimal ripening period of Lider cheese is 15 days. Abstract

Airbags are subject to strict quality control in order to ensure passengers safety. The quality of fabric and sewing thread influence the final product and therefore, sewing defects must be early and accurately detected, in order to remove the item from production. Airbag seams assembly can take various forms, using linear and circle primitives, with threads of different colors and length densities, creating lockstitch or double threads chainstitch. The paper presents a framework for the automatic detection of defects occurring during the airbag sewing stage. Types of defects as skipped stitch, missed stitch or superimposed seam for lockstitch and two threads chainstitch are detected and marked. Using image processing methods, the proposed framework follows the seams path and determines if a color pattern of the considered stitches is valid.
