Abstract

This article discusses the progression of parapsoriasis, a group of diseases that includes pityriasis lichenoides (PL), small-plaque parapsoriasis (SPP), and large-plaque parapsoriasis (LPP), to mycosis fungoides (MF), a type of cutaneous T cell lymphoma (CTCL). While PL rarely progresses to MF, SPP and LPP have occasionally been observed to progress to MF. The article presents a case study of a Japanese man with LPP who eventually developed MF, and analyzes the clonality of T cell receptors (TCRs) in skin samples over time. The study suggests that TCR clonality may be a key factor in differentiating parapsoriasis from MF, but further research is needed to validate these findings. [Extracted from the article]

Abstract

Objectives Evaluate effects of long-term bimekizumab treatment on patient-reported outcome (PRO) measures, symptoms and the impact of PsA on patients. Methods Patients with active PsA were enrolled into BE ACTIVE, a 48-week randomised controlled trial (NCT02969525). After Week 48, patients could enter a 104-week open-label extension (NCT03347110), receiving bimekizumab 160 mg every four weeks. PRO measures assessed included arthritis pain visual analogue scale (VAS), PsA Impact of Disease (PsAID)-9, 36-Item Short Form Survey (SF-36) and HAQ-Disability Index (HAQ-DI). Results were analysed as mean (S.E.M.) changes from baseline (CfB) from Week 0 to the end of the open-label extension (3 years) and as percentage of patients reaching patient-acceptable symptom state (PASS) for global impact (PsAID-9 total score ≤4) and normal function (HAQ-DI total score <0.5). Non-responder imputation was applied to missing binary outcomes. Results In 206 patients (mean age 49.3 years, 51.0% male), completion rate was high; 161 (78.2%) patients completed Week 152. Bimekizumab treatment was associated with long-term sustained improvements in pain [arthritis pain VAS CfB; Week 48: −29.9 (1.9); Week 152: −32.0 (1.9)] and fatigue [PsAID-9 fatigue CfB; −2.4 (0.2); −2.7 (0.2)]. High percentages of patients achieved acceptable symptom state (PsAID-9 PASS: 75.2%; 65.0%) and normalised function (HAQ-DI <0.5: 49.0%; 46.1%). Improvements in patient global assessment and SF-36 Physical Component Summary were also sustained. Conclusions Bimekizumab treatment was associated with long-term sustained improvements in pain and fatigue, reducing overall impact of PsA on patients. 28. Case of carbon-ion radiation recall mucositis associated with anti-programmed cell death 1 treatment for paranasal sinuses melanoma.

Abstract

Since the treatment for mucosal melanoma often accompanies surgical difficulty, carbon-ion radiotherapy in combination with immune check point inhibitors (ICI) against programmed cell death 1(PD-1) and/or cytotoxic T-lymphocyte-associated protein 4 (CTLA-4) is a favorable choice. Radiation recall henomenon (RRP) is represented by an inflammatory reaction within previously irradiated fields,such as mucositis, dermatitis, and pneumonitis, in association with the administration of chemotherapeutic agents.1 Here, we report a case of carbon-ion radiation recall mucositis associated with nivolumab and pembrolizumab for melanoma located in the paranasal sinuses. A 71-year-old Japanese woman was referred to us because of cheek pain and bloody rhinorrhea. Computed tomography revealed a tumor in the left paranasal sinuses without metastasis.Histopathological examinations showed that atypical cells proliferated in the mucosa (Figure 1a). The tumor cells were positive for Melan-A and HMB-45 (Figure 1b), consistent with mucosal melanoma. As the tumor was unresectable, it was treated with carbon-ion radiotherapy (Figure 1c), and local resolution was achieved. However, bilateral adrenal metastases were observed 1 year later without local recurrence,and then nivolumab as anti-PD-1 antibody was administrated. At 1 week after the first administration of nivolumab, the patient complained of left oral pain. Physical examination revealed erythema and swelling on the left upper lip (Figure 1d), and erosive ulcers on the left side of the palatal, buccal, and nasal mucosa (Figure 1e), although histopathological examination was not performed in accordance with the patient’s wishes. The symptoms were resolved by discontinuation of nivolumab. Despite the change from nivolumab to pembrolizumab as another anti-PD-1 antibody, mucositis recurred 1 week after the administration of pembrolizumab (Figure 1f).
