Background: Patients with chronic diseases such as chronic kidney disease (CKD) have been reported to have more adverse outcomes during the coronavirus disease 2019 (COVID-19) pandemic. There are insuficient data on the outcomes of patients with primary glomerular diseases (PGD) afer COVID-19. Methods: We designed a national multicenter observational study that included adult patients with biopsy-proven PGD who survived COVID-19. A control group was created from the same centers, including PGD patients without COVID-19. The clinical and laboratory data of the patients at baseline, frst, and third months afer COVID-19 were recorded. Results: A total of 129 patients from 21 centers were included (COVID-19 group, n = 77). Baseline characteristics were almost similar except the ratio of active disease in the non-COVID-19 group was signifcantly higher than in the COVID-19 group. No patients died during the frst and third months. Respiratory symptoms were signifcantly higher in the COVID-19 group than in the non-COVID-19 group in the frst month (7.8% vs. 0%, P = .039). All other follow-up outcomes, including initiation of chronic kidney replacement therapy and initiation of new immunosuppressive treatment, and the laboratory data were not diferent between the groups in the frst and third months. Conclusion: Primary glomerular disease patients in the post-COVID-19 period had more respiratory symptoms than non-COVID-19 PGD patients, but outcomes, including death and initiation of kidney replacement therapy, were not diferent in the frst and third months post COVID-19. There is growing evidence that chronic kidney disease (CKD) is an independent risk factor for cognitive impairment, especially due to vascular damage, blood–brain barrier disruption and uremic toxins. Given the presence of multiple comorbidities, the medication regimen of CKD patients often becomes very complex. Several medications such as psychotropic agents, drugs with anticholinergic properties, GABAergic drugs, opioids, corticosteroids, antibiotics and others have been linked to negative effects on cognition. These drugs are frequently included in the treatment regimen of CKD patients. The first review of this series described how CKD could represent a risk factor for adverse drug reactions affecting the central nervous system. This second review will describe some of the most common medications associated with cognitive impairment (in the general population and in CKD) and describe their effects. [ABSTRACT FROM AUTHOR]

Objective: Variations in care at national or global level may have an impact on the prognosis of patients on chronic hemodialysis. We aimed to describe regional differences in all-cause mortality or cardiovascular morbidity in chronic hemodialysis patients in Turkey. Methods: We enrolled 2461 patients who were initiated chronic hemodialysis in 93 centers in Turkey between January 27, 2017, and February 09, 2018. We included 2-year follow-up data of 1877 patients in this prospective study. The primary outcome, the rate of composite endpoint of all-cause mortality or cardiovascular morbidity, was compared between geographical regions. Secondary outcomes were the rates of hospitalization and infections. Results: In total, 552 patients (29.4%) developed the primary outcome. The highest and lowest rates of primary outcome occurred in the Mediterranean (34.5%) and Southeastern (26.5%) & Central Anatolian regions (26.5%), respectively, with no significant differences across regions (P = .82). Hospitalization events were detected in 377 patients (20.1%). The highest rate of hospitalization was detected in the Black Sea region (33.8%), and the lowest (7.6%) in the Southeastern region. The regions did not differ in hospitalization rates (P = .88). Infections occurred in 11.3% (n = 212) of the patients. The highest and lowest rates of infections occurred in the Aegean (18.2%) and the Southeastern (2.9%) regions, respectively. We detected significant difference between geographic regions (P = .02). Conclusions: Our study showed that almost 3 in every 10 chronic hemodialysis patients reached the primary endpoint of all-cause mortality/cardiovascular morbidity during the 2 years of follow-up. The occurrence of this outcome does not seem to exhibit geographical variation across the country.

13. COVID-19 Infection in Peritoneal Dialysis Patients: A Comparative Outcome Study with Patients on Hemodialysis and Patients without Kidney Disease

Objectives: There is limited data about coronavirus disease-19 (COVID-19) characteristics and results in peritoneal dialysis (PD) patients. This study aimed to investigate the characteristics and outcomes among PD patients and compare them with matched hemodialysis (HD) patients and a control group without kidney disease. Methods: We included 18 PD patients and consecutive age- and gender-matched 18 HD and 18 patients without kidney disease (control group) registered into the Turkish Society of Nephrology database including 1301 COVID-19 patients. We compared demographic, clinical, radiological, laboratory data, and outcomes namely intensive care unit (ICU) admission, mechanical ventilation, mortality, and composite outcome (death and/or ICU admission). Results: ICU admission, mechanical ventilation, and mortality rates in PD patients (27.8%, 22.2%, and 22.2%, respectively) and the HD group (16.7%, 11.1%, and 16.7%, respectively) were higher than the control group (11.1%, 11.1%, and 5.6%, respectively), but intergroup comparison did not reveal difference. A total of 11 (20.3%) patients had composite outcome (6 PD patients, 3 HD patients, and 2 patients in the control group). In Cox regression analysis, higher age and higher CRP level were related to increased risk of composite outcome. Adjusted rate of composite outcome in PD group was significantly higher than the control group (P =.050). This rate was similar in HD and control groups (P =.30). Conclusions: Combined in-hospital mortality and/or ICU admission of PD patients with COVID-19 was significantly higher than the control patients. There is a need for careful surveillance of PD patients for infection signs and prompt treatment of COVID-19.

- Abstract
Background: we aimed to present the demographic characteristics, clinical presentation, and outcomes of our multicenter cohort of adult KTx recipients with COVID-19.Methods: we conducted a multicenter, retrospective study using data of patients hospitalized for COVID-19 collected from 34 centers in Turkey. Demographic characteristics, clinical findings, laboratory parameters (hemogram, CRP, AST, ALT, LDH, and ferritin) at admission and follow-up, and treatment strategies were reviewed. Predictors of poor clinical outcomes were analyzed. The primary outcomes were in-hospital mortality and the need for ICU admission. The secondary outcome was composite in-hospital mortality and/or ICU admission.Results: one hundred nine patients (male/female: 63/46, mean age: 48.412.4years) were included in the study. Acute kidney injury (AKI) developed in 46 (42.2%) patients, and 4 (3.7%) of the patients required renal replacement therapy (RRT). A total of 22 (20.2%) patients were admitted in the ICU, and 19 (17.4%) patients required invasive mechanical ventilation. 14 (12.8%) of the patients died. Patients who were admitted in the ICU were significantly older (age over 60 years) (38.1% vs 14.9%, p=0.016). 23 (21.1%) patients reached to composite outcome and these patients were significantly older (age over 60years) (39.1% vs. 13.9%; p=0.004), and had lower serum albumin (3.4g/dl [2.9-3.8] vs. 3.8g/dl [3.5-4.1], p=0.002), higher serum ferritin (679 mu g/L [184-2260] vs. 331 mu g/L [128-839], p=0.048), and lower lymphocyte counts (700/mu l [460-950] vs. 860 /mu l [545-1385], p=0.018). Multivariable analysis identified presence of ischemic heart disease and initial serum creatinine levels as independent risk factors for mortality, whereas age over 60 years and initial serum creatinine levels were independently associated with ICU admission. Beyond that, COVID-19 or severe acute respiratory syndrome (SARS) due to infection may directly affect kidney functions. This broad spectrum of COVID-19 influence on kidney patients and kidney functions obviously necessitate an up to date management policy for nephrological care. This review overviews and purifies recently published literature in a question to answer format for the practicing nephrologists that will often encounter COVID-19 and kidney related cases during the pandemic times.

- Abstract
Background: Chronic kidney disease (CKD) and immunosuppression, such as in renal transplantation (RT), stand as one of the established potential risk factors for severe coronavirus disease 2019 (COVID-19). Case morbidity and mortality rates for any type of infection have always been much higher in CKD, haemodialysis (HD) and RT patients than in the general population. A large study comparing COVID-19 outcome in moderate to advanced CKD (Stages 3-5), HD and RT patients with a control group of patients is still lacking. Methods: We conducted a multicentre, retrospective, observational study, involving hospitalized adult patients with COVID-19 from 47 centres in Turkey. Patients with CKD Stages 3-5, chronic HD and RT were compared with patients who had COVID-19 but no kidney disease. Demographics, comorbidities, medications, laboratory tests, COVID-19 treatments and outcome [in-hospital mortality and combined in-hospital outcome mortality or admission to the intensive care unit (ICU)] were compared. Results: A total of 1210 patients were included [median age, 61 (quartile 1-quartile 3 48-71) years, female 551 (45.5%)] composed of four groups: control (n = 450), HD (n = 390), RT (n = 81) and CKD (n = 289). The ICU admission rate was 266/1210 (22.0%). A total of 172/1210 (14.2%) patients died. The ICU admission and in-hospital mortality rates in the CKD group [114/289 (39.4%); 95% confidence interval (CI) 33.9-45.2; and 82/289 (28.4%); 95% CI 23.9-34.5)] were significantly higher than the other groups: HD = 99/390 (25.4%; 95% CI 21.3-29.9; P < 0.001) and 63/390 (16.2%; 95% CI 13.0-20.4; P < 0.001); RT = 17/81 (21.0%; 95% CI 13.2-30.8; P = 0.002) and 9/81 (11.1%; 95% CI 5.7-19.5; P = 0.001); and control = 36/450 (8.0%; 95% CI 5.8-10.8; P < 0.001) and 18/450 (4%; 95% CI 2.5-6.2; P < 0.001). 21. Effects of Interdialytic Weight Gain and Salt Intake on Serum Vascular Endothelial Growth Factor-C (VEGF-C) Levels in Hemodialysis Patients.

Objective: Sodium may accumulate in subcutaneous tissue without any osmotic (non-osmotic) effect. There are insufficient data on non-osmotic sodium mechanisms in hemodialysis patients. We aimed to investigate the relationship between serum vascular endothelial growth factor-C levels, interdialytic weight gain, and salt intake in hemodialysis patients. Methods: The study included 20 anuric hemodialysis patients (11/9; female/male) with stable interdialytic weight gain in the last 3 months. Pre- and post-dialysis sodium and vascular endothelial growth factor-C levels were studied in 2 consecutive hemodialysis sessions. Fluid body components were determined by body composition monitor. The formulas derived from serum sodium and body fluids were used to calculate the interdialytic salt intake. The determinants of serum vascular endothelial growth factor-C levels were analyzed. Results: Salt intake in the interdialytic period was 10.36 ± 5.16 g/day and interdialytic weight gain 3.4% (0.53-6.98). While no correlation was found between salt intake and vascular endothelial growth factor-C levels in the correlation analysis, a moderate correlation was found between interdialytic weight gain and pre-and post-dialysis vascular endothelial growth factor-C levels (r: 0.453; P = .045 and r: 0.454; P = .044, respectively). In the partial correlation analysis performed by controlling the salt intake effect, an increase in this correlation was detected (r: 0.489; P = .34 and r: .525; P = .018, respectively). There was no relationship between age, gender, hemodialysis vintage, and vascular endothelial growth factor-C levels. Conclusion: A moderate correlation was detected between interdialytic weight gain and serum vascular endothelial growth factor-C levels and it may be a clinical reflection of long-term volume and salt loading. However, more extensive studies are needed to clarify this relationship and determine the factors affecting serum vascular endothelial growth factorC levels in hemodialysis patients. Yüksek Lisans Tezi, Bu çalışmanın amacı; okul yöneticilerinin kişilik özelliklerinin duygusal zekâları ve öz-yeterlikleri açısından incelenmesidir. Araştırmanın çalışma grubunu 2017-2018 eğitim öğretim yılında Konya ilinde çalışan 384 okul yöneticisi oluşturmaktadır. Çalışma grubundaki okul yöneticilerinin %17,7'si kadın, %82,3'ü erkektir; %86,7'si lisans, %13,3 yüksek lisans ve doktora mezunudur. Araştırmada okul yöneticilerinin kişilik özellikleri algı düzeylerini belirlemek amacıyla "Sıfatlara Dayalı Kişilik Testi" ölçeğinin Çeliköz ve Şeker Sır'ın (2016) Doğrulayıcı Faktör Analizi (DFA) ile test ederek geçerlik, güvenirlik çalışmasını yaptıkları ve 40 maddeden 27 maddeye indirdikleri Beş Faktörlü Kişilik Ölçeği kullanılmıştır. Okul yöneticilerinin duygusal zekâ algı düzeylerini ölçmek için Tatar, Tok ve Saltukoğlu (2011) tarafından gözden geçirilerek üç faktörlü bir yapıda Türkçe'ye uyarlaması yapılan Schutte Duygusal Zekâ Ölçeği (1998) kullanılmıştır. Öz-yeterlik algılarını ölçmek amacıyla Tschannen-Moran ve Gareis (2004) tarafından geliştirilen ve Türkçe'ye uyarlaması Negiş-Işık ve Derinbay (2015) tarafından yapılan 18 madde ifadesi bulunan tek faktörlü Yönetici Öz-yeterlik Ölçeğinden yararlanılmıştır. Araştırmada okul yöneticilerinin kişilik özellikleri, duygusal zekaları ve öz-yeterlikleri arasındaki ilişki pearson momentler çarpımı korelasyon katsayısı tekniği ile test edilmiştir. Okul yöneticilerinin kişilik özelliklerini öz-yeterlikleri ile duygusal zekanın yordama düzeyi regresyon analizi ile sınanmıştır. Okul yöneticilerinin cinsiyet ve öğrenim düzeyi değişkenlerine göre kişilik özelliklerine ilişkin puan ortalamaları arasında farklılaşmanın olup olmadığına ilişkin t-testi sonuçları karşılaştırılmıştır. Araştırma sonuçlarına göre; okul yöneticilerinin kişilik özelliklerinin alt boyut puanları ile duygusal zekanın alt boyut puanları arasında ve her iki değişkenin öz-yeterlik puanları arasında pozitif yönlü anlamlı ilişkiler olduğu görülmüştür., The purpose of this study is to explore the personality traits of school administrators in terms of emotional intelligence and self-efficacy. The study group consists of 384 school administrators working in the center of Konya in the 2017-2018 academic year. 17,7% of the school administrators in the study group were female and 82,3% were male, 86,7% of school administrators have a bachelor's degree, 13,3% have master degree and doctorate degree. Five Factor Personality Scale, its Confirmatory Factor Analysis (CFA) was tested to validity and reliability by Çeliköz and Şeker Sır (2016) as "Personality Test Based Adjectives" and reduced the scale from 40 to 27 items, used to determine the perception levels of the personality traits of school administrators in the study. The Schutte Emotional Intelligence Scale (2002), reviewed and adapted to Turkish in a three-factor structure by Tatar, Tok and Saltukoğlu (2011), was used to determine the emotional intelligence of school administrators. With the intention of measuring the perceptions of self-efficacy, it was utilised from the scale, which was developed by Tschannen-Moran and Gareis (2004) and adapted to Turkish and consisted of 18 items as a single-factor Administrator Self-Efficacy Scale by Negiş-Işık and Derinbay (2015). In the study, the relationship between the personality traits, emotional intelligence and self-efficacy of school administrators was analyzed with the pearson moment product correlation coefficient technique. The predictive level of emotional intelligence, self-efficacy and the personality traits of school administrators was tested by regression analysis. According to the variables of gender and education levels of school administrators, the t-test results were compared to whether differences exist between the mean scores in terms of the personality traits. According to the research findings; there was found a significant positive relationship between the scores of sub-dimensions of the personality traits and emotional intelligence of school administrators as well as between both of variables and self-efficacy scores.

Coronavirus disease 2019 (COVID-19) has been accepted as a global pandemic, and poses a greater risk to the elderly and those with comorbidities. Comorbid diseases (particularly end-stage kidney disease with hemodialysis) and impaired immunity place patients in the high-risk group for COVID-19. In recent studies, it was also mentioned that exaggerated inflammation and a cytokine storm were the underlying causes related to the high mortality in COVID-19 patients. Currently, treatment modalities to balance the immune system of such vulnerable patient groups are essential, to protect them from the disease. Several vitamins (like vitamins C, D, and E), trace elements like zinc, and probiotics have been proposed as immune boosters to protect and combat infectious conditions. It is well known that these vitamins and elements are insufficient in hemodialysis patients. In this review, we aimed to evaluate the immune-boosting mechanisms of vitamins C, D, E, zinc, and probiotics, the studies related to their beneficial effects against infections, and their possible benefits for hemodialysis patients during the COVID-19 pandemic. Objective: Several studies have shown that local renin–angiotensin system (RAS) activity in the kidneys may play a role in the pathogenesis of hypertension and kidney damage in patients with chronic kidney disease. In this study, we aimed to investigate the effect of local RAS activity on hypertension and residual renal function (RRF) in patients undergoing peritoneal dialysis (PD). Materials and Methods: Fifty patients with residual urine undergoing PD were included in the study. They were divided into the hypertensive (n=30) and non-hypertensive (n=20) groups. The urine angiotensinogen-to-creatinine ratio, which is an indicator of local RAS activity, was compared between the two groups. Factors affecting this ratio were also investigated. Results: There was no significant difference in the mean urine angiotensinogen-to-creatinine ratios between the two groups. A correlation analysis revealed that the urine angiotensinogen-to-creatinine ratio had a significant negative correlation with RRF determined by 24-hour creatinine excretion (r=−0.391, p=0.005). There was a positive correlation between the urine angiotensinogen-to-creatinine ratio with proteinuria (r=0.289, p=0.04) and negative correlation with serum albumin (r=−0.280, p=0.049). However, we could not find any association between the urine angiotensinogen-to-creatinine ratio and blood pressure values. Conclusion: Local RAS activation in the kidney reflected by urinary angiotensinogen is associated with RRF and proteinuria in patients undergoing PD; however, high blood pressure was not correlated with urinary angiotensinogen levels. Prematürite retinopatisi (PR) gelişmekte olan ülkelerde yenidoğan bakım hizmeti olanaklarının artmasıyla birlikte çocukluk çağı körlük nedenlerinde önemli bir yer tutmaktadır. Bilinen en önemli iki risk faktörü; düşük doğum ağırlığı ve gestasyonel haftanın küçük olmasıdır. Riskli bebeklerin tamamının uygun tarama programları ile tespit edilip, takip programına alınması gerekmektedir. Tedavide uygun vakalarda ilk tercih periferal retinal ablasyondur. Cerrahi tedavi fonksiyonel sonuçları yüz güldürücü değildir. Son zamanlarda hastalığın tedavisinde tek başına veya periferal retinal ablasyonla kombine intravitreal antivasküler endotelyal büyüme faktörü (anti-VEGF) enjeksiyon sonuçları umut vericidir. Retinopathy of prematurity is one of the most significant reasons of the childhood blindness in developing countries due to the rising numbers of preterm deliveries and improved neonatal care. The most important risk factors for PR are low birth weight and low gestational age. It is necessary for risky babies to be recognized by a screening program and to be followed by a trace program for PR. Peripheral retinal ablation is applied for therapy of PR. The functional results of surgical therapy are not promising. The results of intravitreal anti-VEGF injections alone or combined with peripheral retinal ablation become gradually encouranging.

Kaposi's sarcoma (KS) is an unusual tumor principally affecting the skin of the lower extremities. Although the association between KS and renal transplant has been well documented, there are a few KS cases in the literature associated with membranoproliferative glomerulonephritis or other glomerular diseases. This report presents a patient with membranoproliferative glomerulonephritis (MPGN) who developed KS following treatment with long-term medium dose glucocorticoid and short-term additional immunosuppressives. The KS cases associated with glomerulonephritis are also reviewed. KS is a rare complication in glomerular diseases that may (or may not) be related to immunosuppression. Hence, immunosuppression treatment should be carefully planned in glomerulonephritis treatment and avoided if they are not essentially necessary.

Purpose: To investigate the effect of bevacizurnab on corneal neovascularization in an alkali bum-induced model of corneal angiogenesis. Material and Method: Twenty-four Wistar albino rats were used in our study. After chemical cauterization of the cornea, the rats were divided randomly into four groups. Group 1 (control group) received artificial tears twice a day, group 2 received topical fluorometholone acetate twice a day, in group 3, a single dose of bevacizumab (2.5 mg) was administered by a subconjunctival injection, and group 4 received topical bevacizumab 5mg/mi twice a day. Three weeks later, the rat corneas were evaluated by slit-lamp biomicroscopy and corneal photographs were taken with a digital camera, followed by sacrifice of the subjects. The proportional area of vascularized cornea, length of the longest neovascular sprout, corneal oedema and corneal opacity score were assessed. Result The analysis of digital photographs showed less corneal neovascularization, corneal oedema, corneal opacity score and shorter length of the longest neovascular sprout in the three drug groups than in the control one (p<0.05). The area of corneal neovascularization in groups 3 and 4 was less than in group 2 (p=0.035 and p=0.027, respectively). corneal neovascularization, corneal oederna and corneal opacity did not differ significantly between the subconjunctival and topical bevacizumab groups. However, statistically significant decrease was observed in the length of the longest neovascular sprout in the topical bevacizumab group (p=0.029). Discussion: Subconjunctival/topical bevacizumab treatment is an effective method in reducing corneal neovascularization. However, we observed that topical bevacizumab is more efficient than subconjunctival bevacizumab and fluorometholone acetate in preventing corneal neovascularization. [ABSTRACT FROM AUTHOR]

Meme kanseri, kadınlarda en sık rastlanan ve en sık ölüm sebebi olan kanserdir. Çalışmamızın amacı ünitemize başvuran invaziv meme kanserli hastalardaki prognostik faktörlerin sağkalıma etkisini saptamaktır.Ekim 2002 - Ekim 2013 arasında ünitemize başvuran invaziv meme kanseri tanısı almış 717 hastanın verileri retrospektif olarak incelendi. Hastaların tanı tarihi, yaşı, cinsiyeti, aile öyküsü, menopozal durumu; tümörün histolojik alt tipi, gradı, hormon reseptör profili (ER, PR), Cerb-B2 durumu, LVI, PNI varlığı; tümör çapı, metastatik LN sayısı ve evresi, varsa ilk progresyon tarihi, hasta yaşıyor ise son kontrol zamanı ve eksitus ise eksitus zamanı ve nedeni kaydedildi. Hastaların 714’ü kadın 3'ü erkekti. Ortalama yaş 53,1 (24-85) idi. Progresyonsuz ve genel sağkalımı etkileyen faktörler tek değişkenli ve çok değişkenli sağkalım analizleri ile değerlendirildi.Tüm hastaların ortalama takip süresi 53,5 ay (0,8-295) idi. Takipte 125 hastada (% 17,4) progresyon gelişirken progresyona kadar geçen ortalama süre 44,3 ay (5,6-167,2), ortalama progresyonsuz sağkalım süresi 49,2 ay (0,8-295) idi. Beş ve on yıllık progresyonsuz sağkalım oranları sırasıyla % 78 ve % 61 idi. Hastalardan 107 tanesinin (% 14,9) meme kanserine bağlı öldüğü saptandı. Ortalama genel sağkalım süresi 67,1 ay (2,1-380,6); 5 ve 10 yıllık genel sağkalım oranları sırasıyla % 86 ve % 70 olarak belirlendi. The aim of this study is to determine the prognostic factors that effect progression free and overall survival in breast cancer patients in our unit. Datas of 717 invasive breast cancer patients, entered our unit between October 2002 and October 2013, were examined retrospectively. Diagnosis date, age, sex, family history, menopausal status; histologic type and grade, hormone receptor status (ER, PR), Cerb-B2 status, LVI, PNI, tumor size, stage, metastatic LN, first progression date (if present), if the patient lives last visit date and if the patient died death time and death cause were noted. Seven hundred and fourteen patient were women, 3 patients were men. Mean age was 53,1 (24-85). Prognostic factors that effect progression free and overall survival were analyzed in univariate and multivariate survival analysis. Mean follow-up time was 53,5 month (0,8-295). During follow-up 125 patient (% 17,7) were progressed. Mean time to progression was 44,3 month (5,6-167,2) and mean progression free survival time was 49,2 month (0,8-295). Five and ten year progression free survival rates were % 78 and % 61 respectively. One hundred and seven patients (% 14,9) died due to breast cancer. Mean overall survival time was 67,1 (2,1-380,6) month. Five and ten year overall survival rates were % 86 and % 70 respectively. In univariate analysis ALN status, tumor size, stage, histological grade, ER, Cerb-B2 and LVI were associated with progression free survival; ALN status, tumor size, stage, histological grade, histological type, ER, Cerb-B2, LVI and PNI were associated with overall survival. Background: we aimed to present the demographic characteristics, clinical presentation, and outcomes of our multicenter cohort of adult KTx recipients with COVID-19.Methods: we conducted a multicenter, retrospective study using data of patients hospitalized for COVID-19 collected from 34 centers in Turkey. Demographic characteristics, clinical findings, laboratory parameters (hemogram, CRP, AST, ALT, LDH, and ferritin) at admission and follow-up, and treatment strategies were reviewed. Predictors of poor clinical outcomes were analyzed. The primary outcomes were in-hospital mortality and the need for ICU admission. The secondary outcome was composite in-hospital mortality and/or ICU admission.Results: one hundred nine patients (male/female: 63/46, mean age: 48.412.4years) were included in the study. Acute kidney injury (AKI) developed in 46 (42.2%) patients, and 4 (3.7%) of the patients required renal replacement therapy (RRT). A total of 22 (20.2%) patients were admitted in the ICU, and 19 (17.4%) patients required invasive mechanical ventilation. 14 (12.8%) of the patients died. Patients who were admitted in the ICU were significantly older (age over 60 years) (38.1% vs 14.9%, p=0.016). 23 (21.1%) patients reached to composite outcome and these patients were significantly older (age over 60years) (39.1% vs. 13.9%; p=0.004), and had lower serum albumin (3.4g/dl [2.9-3.8] vs. 3.8g/dl [3.5-4.1], p=0.002), higher serum ferritin (679 mu g/L [184-2260] vs. 331 mu g/L [128-839], p=0.048), and lower lymphocyte counts (700/mu l [460-950] vs. 860 /mu l [545-1385], p=0.018). Multivariable analysis identified presence of ischemic heart disease and initial serum creatinine levels as independent risk factors for mortality, whereas age over 60 years and initial serum creatinine levels were independently associated with ICU admission. On analysis for predicting secondary outcome, age above 60 and initial lymphocyte count were found to be independent variables in multivariable analysis. Conclusion: over the age of 60, ischemic heart disease, lymphopenia, poor graft function were independent risk factors for severe COVID-19 in this patient group. Transplants from living donors constitute a significant part of the kidney transplants performed today. This study aimed to examine the risk of developing chronic kidney diease (CKD) in long-term follow-up of kidney donors, and the related factors. Three hundred and thirty-eight donors were included in the study. The median age of the donors was 47 (39-54 years) and 64.2% of the donors were female. CKD development was observed in 64 (18,9%) donors during the median follow-up of 71 months. Stage 3 CKD has developed in 50 donors and stage 1-2 CKD has developed in 14 donors. None of the donors developed stage 4 or 5 CKD. Çalışmaya 100 canlı böbrek vericisi (Yaş ortalaması 52.46±10.38, 65 kadın) dahil edildi. Böbrek hacimleri nefrektomi öncesi çekilen abdomen bilgisayarlı tomografiler ve son poliklinik kontrolü sırasında yapılan abdomen manyetik rezonans görüntülemeler kullanılarak hesaplandı. Health literacy (HL) is inadequate for most of chronic kidney patients. The level of HL may help to improve disease management, thus QOL. In this study, the impact of diabetes mellitus on HL and QOL indices was investigated in hemodialysis patients. In this descriptive study, in 14 different (4 public, 10 private) dialysis centers data we collected through face-to-face interviews from 223 cases (91 women and 132 men) with diabetes and 223 (91 women and 132 men) without diabetes. Amaç: Strabismik, anizometropik ve mikst ambliyopların OKT(Optik Koherens Tomografi) ile makula, koroid ve retina sinir lifi tabakası (RSLT) kalınlığı ile kontrast duyarlılık fonksiyonları (KDF) değerlendirerek ambliyopi grupları arasında retinanın yapısal ve fonksiyonel değişikliklerin saptanması amaçlandı.Gereç ve Yöntem: Strabismik ambliyop 20, anizometropik ambliyop 20, mikst ambliyop 20 hasta ve bu hastaların diğer iyi gözleri ile 20 sağlıklı birey çalışmaya alındı. Tüm olgularda tam bir oftalmolojik muayeneye ek olarak optik koherens tomografi (OKT) ile santral makula, koroid ve retina sinir lifi tabakası kalınlığı ölçümü ile kontrast duyarlılık testi yapıldı. Bulgular: Her üç ambliyop grubun santral makula kalınlığı, subfoveal, nazal ve temporal koroidal kalınlıkları iyi gözlere ve kontrol grubuna göre anlamlı bir fark yoktu(p>0.05). Ambliyop gruplar RSLT kalınlığı açısından kendi aralarında ve kontrol grubuna göre dört kadranda da bir anlamlı fark yoktu(p>0.05). Fakat iyi gözleriyle karşılaştırıldığında ise; temporal RSL kalınlığı anizometropik ve mikst ambliyoplarda daha ince, alt RSL kalınlığı ise anizometropik ambliyoplarda daha kalındı(p0.05). Piperasilin-Tazobaktam ve Vankomisin Antibiyoterapilerinin Birlikte Kullanımının Nefrotoksik Etkilerinin İncelenmesi-Hacettepe Üniversitesi Tıp Fakültesi İç Hastalıkları Uzmanlık Tezi, Ankara 2014. Amaç: Hastane ilişkili enfeksiyonların tedavisinde etkin bir şema olan piperasilin-tazobaktam artı vankomisin kombinasyonuna bağlı akut böbrek hasarı gelişme riskinin değerlendirilmesi. Gereç ve Yöntem: 1 Ocak 2012 ile 31 Aralık 2013 tarihleri arasında, tek başına piperasilin-tazobaktam veya vankomisin alanlar ile piperasilin-tazobaktam artı vankomisin ve meropenem artı vankomisin kombinasyonunu alan toplam 402 hastanın kayıtları retrospektif incelendi. Yoğun bakım ünitelerinde izlenenler, bazal kreatinin düzeyi 2 mg/dL'nin üstünde olanlar, rutin hemodiyaliz ya da periton diyalizi programında olanlar, septik şokta olanlar ile hamileler çalışmaya dahil edilmedi. Antibiyotik tedavisi sırasında akut böbrek hasarı varlığı Acute Kidney Injury Network (AKIN) kriterleri kullanılarak kaydedildi. Nazolakrimal kanal tıkanıklığı (NLKT), tüm yaş gruplarında çeşitli sebeplere bağlı olarak gelişen, lakrimal drenaj sisteminin önemli bir patolojisidir. NLKT?nin erişkin yaş grubunda temel tedavisi cerrahidir. Cerrahide eksternal DSR, endoskopik DSR ve lazer cerrahisi gibi yöntemler uygulanmaktadır. Bu retrospektif çalışmada eksternal DSR ile bikanaliküler silikon tüp implantasyonlu Transkanaliküler Multidiyod Lazer (ETDL) DSR sonuçlarının karşılaştırılması amaçlandı. Çalışmaya eksternal DSR uygulanmış 30 hasta ile ETDL DSR uygulanmış 30 hasta alındı. Eksternal DSR uygulanan hastaların 23?ü (%76.7) kadın ve 7?si (%23.3) erkek, yaş ortalaması 47.33±12.44 yıl idi. ETDL DSR grubunun ise 18?i (%60) kadın ve 12?si (%40) erkek ve yaş ortalaması 46.2±19.4 yıl idi. Ayrıca eksternal DSR grubunun 13?ü (% 43.3) sağ; 17 ?si (%56.7) sol, ETDL-DSR grubunun ise 16?sı (%53.3) sağ, 14? ü (%46.7) sol göz idi. İki grup arasında yaş, cinsiyet ve lateralite açısından istatistiksel olarak anlamlı bir fark yoktu (p>0.05). Her iki grubun en az 12 ay takibi sonunda eksternal DSR grubunda başarı oranı %96.7 (30 hastanın 29?u) iken, ETDL-DSR grubunda %90 (30 hastanın 27?si) idi ve iki grubun başarı oranları arasında istatiksel olarak anlamlı fark yoktu (p>0.05).Sonuç olarak her iki grup arasında başarı oranı açısından istatistiksel olarak anlamlı bir fark olmamasına karşın, eksternal DSR, NLKT tedavisinde halen temel cerrahi yöntem olarak güncelliğini korumaktadır. Nasolacrimal Canal Obstruction (NLCO) is an important pathology of lacrimal drainage system which may be seen in all ages with different causes. Treatment of NLCO in adult patients depend on surgery. Surgical options include external dacriocystorhinostomy (DCR), endoscopic DCR and laser surgery. The aim of this retrospective study is to compare the results of external DCR and Transcanalicular Multidiode Laser DCR (ETDL-DCR) with implantation of bicanalicular silicon tube.30 patients who received the treatment of external DCR and who received the treatment of ETDL-DCR were included in this study. Of the 30 patients who received the treatment of external DCR 23 (%76.7) were female and 7 (%23.3) were male. And the average age was 47.33±12.44. Of the 30 patients who received the treatment of ETDL- DCR 18 (%60.0) were female and 12 (%40.0) were male. And the average age was 46.2±19.4. Of the patients who received external DCR 13 (%43.3) were operated from the right eye, 17 (%56.7) were operated from the left eye. Of the patients who received ETDL- DCR 16 (%53.3) were operated from the right eye, 14 (%46.7) were operated from the left eye. There was no significant statistical difference between the two groups concerning the age, sexuality and laterality (p>0.05). Both of the groups were followed at least 12 months. And success ratio in external DCR was %96.7 and %90 in ETDL-DCR. But there was no significant statistical difference between the two groups in terms of success ratio (p>0.05).In conclusion besides not having any statistical difference between two groups, external DCR still is the most common surgical treatment method in NLCO.
