1. Challenges and opportunities in pediatric residency: an analysis of the increasing number of residents in Italy

Abstract

Abstract Background Pediatric residency in Italy has undergone significant changes in recent years, with a notable increase in the number of pediatric residents. Exploring the implications of this increase, highlighting disparities in training opportunities, and discussing the broader socio-economic impacts on pediatric healthcare, represent a crucial consideration for the healthcare system in the future. Main Body The Italian National Association of Pediatric Residents (“Osservatorio Nazionale Specializzandi in Pediatria”, ONSP) conducted an extensive survey among pediatric residents to assess the current state of pediatric residency. Key findings indicate that 50% of respondents believe the number of residents is excessive for the available training opportunities, leading to concerns about the quality of education and hands-on experience. Despite the increased number of residents, the workload has increased, but up to one-third of residents feel that the autonomy provided by their programs is insufficient. Significant disparities in training quality were found across different regions, with notable shortages in neonatology and pediatric emergency departments. Research opportunities are also limited, with only 17% of residents finding the time allocated to research satisfactory, especially in central and southern Italy. Conclusion The increase in pediatric residents presents both challenges and opportunities. Addressing these challenges through strategic reforms, such as implementing standardized national curricula, investing in training resources and mobility programs, and enhancing research opportunities, is crucial for the future of pediatric residency in Italy. Ensuring high-quality training for all residents is an ethical and practical obligation that will significantly impact pediatric healthcare.

Abstract

To answer NASA’s call for a sensitive X-ray observatory in the 2030s, we present the High Energy X-ray Probe (HEX-P) mission concept. HEX-P is designed to provide the required capabilities to explore current scientific questions and make new discoveries with a broadband X-ray observatory that simultaneously measures sources from 0.2 to 80 keV. HEX-P’s main scientific goals include: 1) understand the growth of supermassive black holes and how they drive galaxy evolution; 2) explore the lower mass populations of white dwarfs, neutron stars, and stellar-mass black holes in the nearby universe; 3) explain the physics of the mysterious corona, the luminous plasma close to the central engine of accreting compact objects that dominates cosmic X-ray emission; and 4) find the sources of the highest energy particles in the Galaxy. These goals motivate a sensitive, broadband X-ray observatory with imaging, spectroscopic, and timing capabilities, ensuring a versatile platform to serve a broad General Observer (GO) and Guest Investigator (GI) community. In this paper, we present an overview of these mission goals, which have been extensively discussed in a collection of more than a dozen papers that are part of this Research Topic volume. The proposed investigations will address key questions in all three science themes highlighted by Astro2020, including their associated priority areas. HEX-P will extend the capabilities of the most sensitive low- and high-energy X-ray satellites currently in orbit and will complement existing and planned high-energy, time-domain, and multi-messenger facilities in the next decade.

Abstract

IntroductionThe use of robotic systems in the surgical domain has become groundbreaking for patients and surgeons in the last decades. While the annual number of robotic surgical procedures continues to increase rapidly, it is essential to provide the surgeon with innovative training courses along with the standard specialization path. To this end, simulators play a fundamental role. Currently, the high cost of the leading VR simulators limits their accessibility to educational institutions. The challenge lies in balancing high-fidelity simulation with cost-effectiveness; however, few cost-effective options exist for robotic surgery training.MethodsThis paper proposes the design, development and user-centered usability study of an affordable user interface to control a surgical robot simulator. It consists of a cart equipped with two haptic interfaces, a VR visor and two pedals. The simulations were created using Unity, which offers versatility for expanding the simulator to more complex scenes. An intuitive teleoperation control of the simulated robotic instruments is achieved through a high-level control strategy.Results and DiscussionIts affordability and resemblance to real surgeon consoles make it ideal for implementing robotic surgery training programs in medical schools, enhancing accessibility to a broader audience. This is demonstrated by the results of an usability study involving expert surgeons who use surgical robots regularly, expert surgeons without robotic surgery experience, and a control group. The results of the study, which was based on a traditional Peg-board exercise and Camera Control task, demonstrate the simulator’s high usability and intuitive control across diverse user groups, including those with limited experience. This offers evidence that this affordable system is a promising solution for expanding robotic surgery training.

Abstract

Febrile seizures (FS) can be frightening for parents, even though they are usually harmless. Various questionnaires have been used to assess parental reactions and awareness about FS, revealing insufficient knowledge. Studies have shown that educational interventions significantly reduce parental concerns, improve knowledge, and promote better first-aid measures. Providing clear information and emotional support to parents is important to reduce their concerns and improve FS management. Healthcare providers should give comprehensive information about FS, including the risk of recurrence, and provide clear instructions on their management. The economic impact of FS includes direct and indirect costs. Studies have shown a decrease of hospitalizations and associated costs due to improved clinical adherence to guidelines, which also reduces the inappropriate use of healthcare resources. This systematic review provides a comprehensive overview of the existing literature on parental anxiety and education about FS, as well as their economic impact, aiming at identifying areas for improvement in the management of FS and providing valuable insights for healthcare providers and policymakers to better address the non-clinical burden of this condition.

Abstract

According to the DSM-5, neurodevelopmental disorders represent a group of heterogeneous conditions, with onset during the developmental period, characterized by an alteration of communication and social skills, learning, adaptive behavior, executive functions, and psychomotor skills. These deficits determine an impairment of personal, social, scholastic, or occupational functioning. Neurodevelopmental disorders are characterized by an increased incidence and a multifactorial etiology, including genetic and environmental components. Data largely explain the role of genetic and environmental factors, also through epigenetic modifications such as DNA methylation and miRNA. Despite genetic factors, nutritional factors also play a significant role in the pathophysiology of these disorders, both in the prenatal and postnatal period, underscoring that the control of modifiable factors could decrease the incidence of neurodevelopmental disorders. The preventive role of nutrition is widely studied as regards many chronic diseases, such as diabetes, hypertension, and cancer, but actually we also know the effects of nutrition on embryonic brain development and the influence of prenatal and preconceptional nutrition in predisposition to various pathologies. These factors are not limited only to a correct caloric intake and a good BMI, but rather to an adequate and balanced intake of macro and micronutrients, the type of diet, and other elements such as exposure to heavy metals. This review represents an analysis of the literature as regards the physiopathological mechanisms by which food influences our state of health, especially in the age of development (from birth to adolescence), through prenatal and preconceptional changes, underlying how controlling these nutritional factors should improve mothers’ nutritional state to significantly reduce the risk of neurodevelopmental disorders in offspring. We searched key words such as “maternal nutrition and neurodevelopmental disorders” on Pubmed and Google Scholar, selecting the main reviews and excluding individual cases. Therefore, nutrigenetics and nutrigenomics teach us the importance of personalized nutrition for good health. So future perspectives may include well-established reference values in order to determine the correct nutritional intake of mothers through food and integration.

Autonomic symptoms may be local and general clinical manifestations of both epilepsy and migraine caused by the dysfunction of brain areas best known as the central autonomic network. Despite their prevalence, autonomic signs are often misdiagnosed and their treatment is undervalued. This review aims to describe the autonomic manifestations reported during seizures and migraineur attacks according to their presentation, focusing on the role of the central autonomic network (CAN) and on the parasympathetic outflow that often-induced cranial autonomic symptoms (CAS) during migraineur attacks. Further, our purpose is to analyze the pathophysiological meanings and whether their presence influences the prognosis and therapy of these disorders.

Abstract

Accretion is a universal astrophysical process that plays a key role in cosmic history, from the epoch of reionization to galaxy and stellar formation and evolution. Accreting stellar-mass black holes in X-ray binaries are one of the best laboratories to study the accretion process and probe strong gravity—and most importantly, to measure the angular momentum, or spin, of black holes, and its role as a powering mechanism for relativistic astrophysical phenomena. Comprehensive characterization of the disk-corona system of accreting black holes, and their co-evolution, is fundamental to measurements of black hole spin. Here, we use simulated data to demonstrate how key unanswered questions in the study of accreting stellar-mass black holes will be addressed by the High Energy X-ray Probe (HEX-P). HEX-P is a probe-class mission concept that will combine high spatial resolution X-ray imaging and broad spectral coverage (0.2–80 keV) with a sensitivity superior to current facilities (including XMM-Newton and NuSTAR) to enable revolutionary new insights into a variety of important astrophysical problems. We illustrate the capability of HEX-P to: 1) measure the evolving structures of black hole binary accretion flows down to low (≲ 0.1%) Eddington-scaled luminosities via detailed X-ray reflection spectroscopy; 2) provide unprecedented spectral observations of the coronal plasma, probing its elusive geometry and energetics; 3) perform detailed broadband studies of stellar mass black holes in nearby galaxies, thus expanding the repertoire of sources we can use to study accretion physics and determine the fundamental nature of black holes; and 4) act as a complementary observatory to a range of future ground and space-based astronomical observatories, thus providing key spectral measurements of the multi-component emission from the inner accretion flows of black hole X-ray binaries.

Abstract

Using data from the Neutron star Interior Composition ExploreR observatory, we identified a permanent spin frequency decrease of Δ ν = −(1.04 ± 0.07) × 10 ^−7 Hz around MJD 60132 in the rotation-powered pulsar PSR B0540-69, which exhibits a periodic signal at a frequency of ν ∼ 19.6 Hz. This points to an antiglitch event, a sudden decrease of the pulsar’s rotational frequency without any major alteration in the pulse profile or any significant increase of the pulsed flux. Additionally, no burst activity was observed in association with the antiglitch. To date, observations of the few known antiglitches have been made in magnetars or accreting pulsars. This is the first antiglitch detected in a rotation-powered pulsar. Given its radiatively quiet nature, this antiglitch is possibly of internal origin. Therefore, we tentatively frame this event within a proposed mechanism for antiglitches where the partial “evaporation” of the superfluid component leads to an increase in the normal component’s moment of inertia and a decrease in the superfluid one.

Abstract

Since their first discovery in the late 1960s, gamma-ray bursts have attracted an exponentially growing interest from the international community due to their central role in the most highly debated open questions of the modern research of astronomy, astrophysics, cosmology, and fundamental physics. These range from the intimate nuclear composition of high-density material within the core of ultra-dense neuron stars, to stellar evolution via the collapse of massive stars, the production and propagation of gravitational waves, as well as the exploration of the early universe by unveiling the first stars and galaxies (assessing also their evolution and cosmic re-ionization). GRBs in the past ∼50 years have stimulated the development of cutting-edge technological instruments for observations of high-energy celestial sources from space, leading to the launch and successful operations of many different scientific missions (several of them still in data-taking mode currently). In this review, we provide a brief description of the GRB-dedicated missions from space being designed and developed for the future. The list of these projects, not meant to be exhaustive, shall serve as a reference to interested readers to understand what is likely to come next to lead the further development of GRB research and the associated phenomenology.

Abstract

The pathophysiological mechanisms underlying migraine are more difficult to investigate in children than in the adult population. Abnormal cortical excitability turns out to be one of the most peculiar aspects of migraine, accounting for the manifestations of migraine attacks. Recently, visual cortical excitability has been explored effectively in adult migraineurs with a technique based on cross-modal audio-visual illusions (with sound-induced flash illusions (SIFIs) being reduced in migraineurs compared to non-migraineur subjects). On such a basis, in this study, we investigated visual cortical excitability in children with migraine using SIFIs using combinations of visual and sound stimuli presented randomly. We evaluated 26 children with migraine without aura and 16 healthy children. Migraineurs did not differ from the age-matched healthy subjects regarding fission or fusion illusions but perceived more flashes in trials of multiple flashes with or without beeps. The higher number of SIFIs in migraineur children compared to adults may be due to a greater propensity of visual stimulation to be driven by auditory stimuli (i.e., acoustic dominance). The increased ability to perceive flashes reveals a hyperfunctional visual cortex, demonstrating that the use of SIFIs is a valid tool for assessing visual cortical responsiveness even in pediatric migraine.

11. Timing and Spectral Analysis of HMXB 4U 1700-37 Observed with Insight-HXMT

Abstract

We report timing and spectral studies of the high-mass X-ray binary 4U 1700-37 using Insight-HXMT observations carried out in 2020 during its out-of-eclipse state. We found significant variations in flux on a timescale of kilo-seconds, while the hardness (count rate ratio between 10–30 keV and 2–10 keV) remains relatively stable. No evident pulsations were found over a frequency range of 10 ^−3 –2000 Hz. During the spectral analysis, for the first time, we took the configuration of different Insight-HXMT detectors’ orientations into account, which allows us to obtain reliable results even if stable contamination exists in the field of view. We found that the spectrum could be well described by some phenomenological models that are commonly used in accreting pulsars (e.g., a power law with a high energy cutoff) in the energy range of 2–100 keV. We found hints of cyclotron absorption features around ∼16 keV or/and ∼50 keV.

12. Annihilating dark matter search with 12 years of Fermi LAT data in nearby galaxy clusters

Abstract

Galaxy clusters contain an abundance of dark matter making them attractive laboratories for indirect DM searches. This work details a search for signals of pair annihilation from WIMP dark matter in the GeV gamma ray regime. We perform this search in five low $z$ and high galactic latitude galaxy clusters (Centaurus, Coma, Virgo, Perseus and Fornax); using nearly 12 years of Fermi/LAT data. Through the non-detection of this characteristic signal, we derive constraints on the annihilation cross-section of DM pair annihilation into the $b\overline{b}$, $W^+W^-$ and $\gamma\gamma$ channels. The limits obtained are of a comparable magnitude to those of recently derived from Fermi/LAT observations of dwarf spheroidal galaxies.

Abstract

IntroductionStroke-like syndrome (SLS) is a rare subacute neurological complication of intrathecal or high-dose (≥500 mg) Methotrexate (MTX) administration. Its clinical features, evoking acute cerebral ischaemia with fluctuating course symptoms and a possible spontaneous resolution, have elicited interest among the scientific community. However, many issues are still open on the underlying pathogenesis, clinical, and therapeutic management and long-term outcome.Materials and MethodsWe retrospectively analyzed clinical, radiological and laboratory records of all patients diagnosed with SLS between 2011 and 2021 at 4 National referral centers for Pediatric Onco-Hematology. Patients with a latency period that was longer than 3 weeks between the last MTX administration of MTX and SLS onset were excluded from the analysis, as were those with unclear etiologies. We assessed symptom severity using a dedicated arbitrary scoring system. Eleven patients were included in the study.ResultsThe underlying disease was acute lymphoblastic leukemia type B in 10/11 patients, while fibroblastic osteosarcoma was present in a single subject. The median age at diagnosis was 11 years (range 4–34), and 64% of the patients were women. Symptoms occurred after a mean of 9.45 days (± 0.75) since the last MTX administration and lasted between 1 and 96 h. Clinical features included hemiplegia and/or cranial nerves palsy, paraesthesia, movement or speech disorders, and seizure. All patients underwent neuroimaging studies (CT and/or MRI) and EEG. The scoring system revealed an average of 4.9 points (± 2.3), with a median of 5 points (maximum 20 points). We detected a linear correlation between the severity of the disease and age in male patients.ConclusionsSLS is a rare, well-characterized complication of MTX administration. Despite the small sample, we have been able to confirm some of the previous findings in literature. We also identified a linear correlation between age and severity of the disease, which could improve the future clinical management.

Objective In this review, we present a synthesis of the updated evidence on PTSD rates in parents of children with severe diseases. We also aim to try to understand if research in this field has been refined over time with the long-term intent to better face the new challenges of Covid-19 in the paediatric field. Data sources The PubMed database was searched. Study selection Studies were included if they assessed PTSD in parents of children diagnosed with physical illnesses. Data extraction Of 240 studies, 4 were included. Results Analysis of the 4 studies revealed 2 studies with PTSD rates around 20% and in line with previous best-evidence. All 4 studies tried to provide more data on fathers, however, all the studies present the lack of a control group. Limitations The limited number of studies, which also differ widely in the methodology used. Conclusions Methodological errors evidenced in all the 4 studies limit their reliability, making the understanding of the paediatric caregiver’s concern regarding PTSD still difficult. More sound research is needed.

Abstract

Using observations of X-ray pulsar Hercules X-1 by the Imaging X-ray Polarimetry Explorer we report a highly significant (>17σ) detection of the polarization signal from an accreting neutron star. The observed degree of linear polarization of ~10% is far below theoretical expectations for this object, and stays low throughout the spin cycle of the pulsar. Both the degree and angle of polarization exhibit variability with the pulse phase, allowing us to measure the pulsar spin position angle 57(2) deg and the magnetic obliquity 12(4) deg, which is an essential step towards detailed modelling of the intrinsic emission of X-ray pulsars. Combining our results with the optical polarimetric data, we find that the spin axis of the neutron star and the angular momentum of the binary orbit are misaligned by at least ~20 deg, which is a strong argument in support of the models explaining the stability of the observed superorbital variability with the precession of the neutron star.

Aim and Methods: In this interview with ChatGPT, we explore the potential impact of AI on future pediatric research. Our discussion covers a range of topics, including the potential positive effects of AI, such as improved clinical decision-making, enhanced medical education, faster drug development, and better research outcomes. We also examine potential negative effects, such as bias and fairness concerns, safety and security issues, overreliance on technology, and ethical considerations. Conclusions: While AI continues to advance, it is crucial to remain vigilant about the possible risks and limitations of these technologies and to consider the implications of these technologies and their use in the medical field. The development of AI language models represents a significant advancement in the field of artificial intelligence and has the potential to revolutionize daily clinical practice in every branch of medicine, both surgical and clinical. Ethical and social implications must also be considered to ensure that these technologies are used in a responsible and beneficial manner.

Abstract

Background. The orofacial pain syndromes (OFPs) are a heterogeneous group of syndromes characterized by painful attacks involving the orofacial structures. They may be summarily subdivided into two great categories: (1) orofacial pain mainly attributed to dental disorders such as dentoalveolar and myofascial orofacial pain or temporomandibular joint (TM) pain; (2) orofacial pain mainly attributed to non-dental pain as neuralgias, facial localization of primary headaches or idiopathic orofacial pain. The second group is uncommon, often described by single case reports, can often show overlapping symptoms with the first group, and represents a clinical challenge, carrying the risk of undervaluation and possibly invasive odontoiatric treatment. We aimed to describe a clinical pediatric series of non-dental orofacial pain and better to underline some topographic and clinical features associated with them. We retrospectively collected the data of children admitted to our headache centers (Bari, Palermo, Torino) from 2017 to 2021. Our inclusion criterion was the presence of non-dental orofacial pain following the topographic criteria of 3° International Classification of Headache Disorders (ICHD-3), and exclusion criteria included the pain syndromes attributed to the dental disorders and pain syndromes due to the secondary etiologies Results. Our sample comprised 43 subjects (23/20 M/F, in the range of ages 5–17). We classified them int: 23 primary headaches involving the facial territory during attacks, 2 facial trigeminal autonomic cephalalgias, 1 facial primary stabbing headache, 1 facial linear headache, 6 trochlear migraines, 1 orbital migraine 3 red ear syndrome and 6 atypical facial pain. All patients described debilitating pain for intensity (moderate/severe), 31 children had episodic attacks, and 12 had continuous pain. Almost all received drugs for acute treatment (less than 50% were satisfied), and some received non-pharmacological treatment associated with drug therapy Conclusion. Although rare OFP can occur in pediatric age, it can be debilitating if unrecognized and untreated, affecting the psychophysical well-being of young patients. We highlight the specific characteristics of the disorder for a more correct and earlier identification during the diagnostic process, already difficult in pediatric age, and to define the approach and possible treatment to prevent negative outcomes in adulthood.

Abstract

The physiological mechanisms underlying the complex interplay between life stressors and metabolic factors is receiving growing interest and is being analyzed as one of the many factors contributing to depressive illness. The brain histaminergic system modulates neuronal activity extensively and we demonstrated that its integrity is necessary for peripheral signals such as the bioactive lipid mediator oleoylethanolamide (OEA) to exert its central actions. Here, we investigated the role of brain histamine and its interaction with OEA in response to chronic social defeat stress (CSDS), a preclinical protocol widely used to study physio-pathological mechanisms underlying symptoms observed in depression. Both histidine decarboxylase null (HDC−/-) and HDC+/+ mice were subjected to CSDS for 21 days and treated with either OEA or vehicle daily, starting 10 days after CSDS initiation, until sacrifice. Undisturbed mice served as controls. To test the hypothesis of a histamine-OEA interplay on behavioral responses affected by chronic stress, tests encompassing the social, ethological and memory domains were used. CSDS caused cognitive and social behavior impairments in both genotypes, however, only stressed HDC+/+ mice responded to the beneficial effects of OEA. To detect subtle behavioral features, an advanced multivariate approach known as T-pattern analysis was used. It revealed unexpected differences of the organization of behavioral sequences during mice social interaction between the two genotypes. These data confirm the centrality of the neurotransmitter histamine as a modulator of complex behavioral responses and directly implicate OEA as a protective agent against social stress consequences in a histamine dependent fashion.

Abstract

In this work, using Neutron star Interior Composition Explorer and Insight-Hard X-ray Modulation Telescope observations, we present a study of the broad-band spectral and timing evolution of the source throughout the first reflare, which occurred about 4-month after the major outburst. Our findings suggest that during the reflare, below a critical luminosity Lcrit ∼ 2.5 × 1036 (D/2.2 kpc)2 erg s−1, the scale of the corona shrinks in the radial direction, whereas the inner radius of the disc does not change considerably; however, the inner radius of the disc starts to move inward when the source exceeds the critical luminosity. We conclude that at low luminosity the increase in accretion rate only heats up the inner zone of the accretion disc without the transfer of angular momentum which occurs above a certain luminosity.

Abstract

The mission concept THESEUS (Transient High Energy Sky and Early Universe Surveyor) aims at exploiting Gamma-Ray Bursts (GRB) to explore the early Universe, as well as becoming a cornerstone of multi-messenger and time-domain astrophysics. To achieve these goals, a key feature is the capability to survey the soft X-ray transient sky and to detect the faint and soft GRB population so far poorly explored. Among the expected transients there will be high-redshift GRBs, nearby low-luminosity, X-ray Flashes and short GRBs. Our understanding of the physics governing the GRB prompt emission will benefit from the 0.3 keV–10 MeV simultaneous observations for an unprecedented large number of hundreds of events per year. In particular the mission will provide the identification, accurate sky localisation and characterization of electromagnetic counterparts to sources of gravitational wave and neutrino sources, which will be routinely detected during the 2030s by the upgraded second generation and third generation Gravitational Wave (GW) interferometers and next generation neutrino detectors.

Abstract

High mass X-ray binaries are among the brightest X-ray sources in the Milky Way, as well as in nearby Galaxies. Thanks to their highly variable emissions and complex phenomenology, they have attracted the interest of the high energy astrophysical community since the dawn of X-ray Astronomy. In more recent years, they have challenged our comprehension of physical processes in many more energy bands, ranging from the infrared to very high energies. In this review, we provide a broad but concise summary of the physical processes dominating the emission from high mass X-ray binaries across virtually the whole electromagnetic spectrum. These comprise the interaction of stellar winds with the high gravitational and magnetic fields of compact objects, the behaviour of matter under extreme magnetic and gravity conditions, and the perturbation of the massive star evolutionary processes. We highlight the role of the INTEGRAL mission in the discovery of many of the most interesting objects in the high mass X-ray binary class and its contribution in reviving the interest for these sources over the past two decades. We show how the INTEGRAL discoveries have not only contributed to significantly increase the number of high mass X-ray binaries known, thus advancing our understanding of the population as a whole, but also have opened new windows of investigation that stimulated the multi-wavelength approach nowadays common in most astrophysical research fields. We conclude the review by providing an overview of future facilities being planned from the X-ray to the very high energy domain that will hopefully help us in finding an answer to the many questions left open after more than 18 years of INTEGRAL scientific observations.

Abstract

The first step in a behavioral study is represented by the organization of a suitable ethogram, that is, a formal description of individual components of the behavior. Then, each component of such a behavioral repertoire can be quantified (e.g., how many times it occurs, its duration, percent distribution, latency, etc.). However, it is our contention that the possibility to describe the behavior of a living being by means of hundreds or even thousands of numbers concerning isolated components, disjointed from the comprehensive behavioral architecture, does not imply the possibility to use those numbers to reconstruct the meaning of behavior. Such a level of comprehension requires a qualitative approach based on the analysis of behavioral structure and its underlying dynamics. By means of synergic utilization of quantitative and qualitative data a more complete description of a given behavior becomes available. In present study we discuss results obtained from observations of feeding behavior in two groups of male Wistar rats: a control group, under standard diet, and a second group, under hyperglycidic one. Results have been presented both in terms of quantitative evaluations and in terms of structural/qualitative ones, the latter obtained by means of T-pattern detection and analysis. As to quantitative results, mean durations showed a significant reduction of Walking and Feeding and an increase of Hind-Paw Licking and Body Grooming; concerning mean occurrences, a significant increase of Front-Paw Licking, Hind-Paw Licking, and Body Grooming was present; percent distributions showed significant reductions for Walking and Feeding and a significant increase for all grooming activities. As to qualitative assessments, T-pattern analysis unveiled a clear-cut behavioral reorganization induced by the hyperglycidic diet. If on the one hand, 50 different T-patterns were detected in subjects under standard diet, on the other hand, 703 different T-patterns were discovered in animals under hyperglycidic treatment, with a highly significant increase of mean lengths and a significant reduction of mean occurrences of T-patterns. Synergic evaluation of results in terms of quantitative and qualitative aspects shows, in rats fed with hyperglycidic diet, an increased anxiety condition, likely dependent on food-related stimuli and suggestive of a pervasive craving-related behavior.

Abstract

PSR B1259-63 is a gamma-ray binary system hosting a radio pulsar orbiting around a O9.5Ve star, LS 2883, with a period of ∼3.4 years. The interaction of the pulsar wind with the LS 2883 outflow leads to unpulsed broadband emission in the radio, X-ray, GeV, and TeV domains. One of the most unusual features of the system is an outburst of GeV energies around the periastron, during which the energy release substantially exceeds the spin down luminosity under the assumption of the isotropic emission. In this paper, we present the first results of a recent multi-wavelength campaign (radio, optical, and X-ray bands) accompanied by the analysis of publicly available GeV Fermi/LAT data. The campaign covered a period of more than 100 days around the 2021 periastron and revealed substantial differences from previously observed passages. We report a major delay of the GeV flare, weaker X-ray flux during the peaks, which are typically attributed to the times when the pulsar crosses the disk, and the appearance of a third X-ray peak never observed before. We argue that these features are consistent with the emission cone model proposed by us previously, in the case of a sparser and clumpier disk of the Be star.

Abstract

Background: Despite its high prevalence, the clinical course of pediatric migraine has not been fully understood, and previous studies present conflicting results. We present here the findings of a 10-year follow-up study involving children with severe migraine pain admitted to our emergency department. Furthermore, all studies were carried out on selected outpatient clinical case studies. Our aim was to evaluate a population of migraine children admitted to an emergency department because of increased severity or frequency of pain or even because of very anxious parents concerning their child’s headache in order to describe their long-term outcomes, whether it differed from that of outpatient populations and to identify possible predictors of prognosis. Methods: We recruited 80 subjects with migraine headaches (mean age 8 years with a range of 4–14 years, 50% females), attending the baseline examination of a population admitted for a headache to the Emergency Department in the first half year of 2012. Of the 80 subjects, 48 (60%) were eligible for follow-up in 2022. We included in our study only patients diagnosed with migraine, according to the diagnostic criteria of the International Classification of Headache Disorders. All were contacted by telephone, and a semi-structured questionnaire was provided to them by email. The association between several possible prognostic factors (gender, familiar neurologic disorders, prenatal and perinatal disorders, social activities, sleep disorders, etc.) and the long-term persistence of migraine headaches were explored using logistic regression analysis. Results: Of 48 subjects with migraine headaches at baseline, 31 (65%) had persistent migraine, and 17 (35%) experienced remission. The preliminary results showed that the presence of neurologic disorders in parents (p < 0.01—odds ratio 9.34 (2.53–41.64) and sleep disorders (p < 0.01—odds ratio 13.18 (2.25–252.74) significantly predicted the 10-year persistence of migraine headaches, while the other considered predictors were found not to influence prognosis. Conclusions: To our knowledge, this was the first study conducted on a selected pediatric population upon admission to the emergency room. Our study suggests that a population of pediatric migraine selected for admission to the emergency department also shows a favorable long-term prognosis, like the studies conducted in the outpatient sample. Familial neurological comorbidity and sleep disorders were unfavorable factors for predicting good outcomes.

Abstract

Background Lafora disease (LD) is a neurodegenerative disorder featuring action and stimulus-sensitive myoclonus, epilepsy, and cognitive deterioration. Mutations in the EPM2A/EPM2B genes classically prove causative for the disease in most cases. Since full-field electroretinogram (ffERG) may reveal early-stage changes in a wide spectrum of diseases, we aimed to evaluate retinal cones and rods dysfunction in a cohort of Italian LD patients. Methods Patients with genetically confirmed LD were recruited and subjected to ffERG analysis following the International Society for Clinical Electrophysiology of Vision (ISCEV) protocol. Results Six patients aged between 13 and 26 years (mean 19.5 years) were included. The mean age at disease onset was 12.5 years with a mean disease duration of 7 years. The ffERG analysis revealed a global mild to severe generalized cones dysfunction in all patients. Linear correlation was identified between disease stage and the degree of cones and rods dysfunction, as well as between the type of mutation and the cones and rods dysfunction. Conclusions This study brings further evidence of early retinal alterations in LD patients. The cones and rods dysfunction grade is related to disease duration. The ffERG is an important tool to determine the disease stage, allowing to evaluate either natural or treatment-related disease progression in a minimally invasive way.

Abstract

Electroencephalography (EEG) is pivotal in the clinical assessment of epilepsy, and sleep is known to improve the diagnostic yield of its recording. Sleep-EEG recording is generally reached by either partial deprivation or by administration of sleep-inducing agents, although it is still not achieved in a considerable percentage of patients. We conducted a double-blind placebo-controlled study, involving a hundred patients between 1 and 6 years old, randomized into two groups: Group 1 received liposomal melatonin (melatosome) whereas Group 2 received a placebo. Sleep latency (SL), defined as the time span between the onset of a well-established posterior dominant rhythm, considered as a frequency of 3 to 4 Hz, increasing to 4–5 Hz by the age of 6 months, to 5–7 Hz by 12 months, and finally to 8 Hz by 3 years, and the first EEG sleep figures detected, were measured for each patient. A significant difference in SL was observed (10.8 ± 5 vs. 18.1 ± 13.4 min, p-value = 0.002). Within each group, no differences in sleep latency were detected between genders. Furthermore, no difference in EEG abnormality detection was observed between the two groups. Our study confirmed the efficacy and safety of melatonin administration in sleep induction. Nonetheless, liposomal melatonin presents a greater bioavailability, ensuring a faster effect and allowing lower dosages. Such results, never before reported in the literature, suggest that the routine employment of melatonin might improve clinical practice in neurophysiology, reducing unsuccessful recordings.

Abstract

Soft protons are a potential threat for X-ray missions using grazing incidence optics, as once focused onto the detectors they can contribute to increase the background and possibly induce radiation damage as well. The assessment of these undesired effects is especially relevant for the future ESA X-ray mission Athena, due to its large collecting area. To prevent degradation of the instrumental performance, which ultimately could compromise some of the scientific goals of the mission, the adoption of ad-hoc magnetic diverters is envisaged. Dedicated laboratory measurements are fundamental to understand the mechanisms of proton forward scattering, validate the application of the existing physical models to the Athena case and support the design of the diverters. In this paper we report on scattering efficiency measurements of soft protons impinging at grazing incidence onto a Silicon Pore Optics sample, conducted in the framework of the EXACRAD project. Measurements were taken at two different energies, ~470 keV and ~170 keV, and at four different scattering angles between 0.6 deg and 1.2 deg. The results are generally consistent with previous measurements conducted on eROSITA mirror samples, and as expected the peak of the scattering efficiency is found around the angle of specular reflection., 16 pages, 11 figures; accepted for publication on Experimental Astronomy

Abstract

In this White Paper we present the potential of the Enhanced X-ray Timing and Polarimetry (eXTP) mission for determining the nature of dense matter; neutron star cores host an extreme density regime which cannot be replicated in a terrestrial laboratory. The tightest statistical constraints on the dense matter equation of state will come from pulse profile modelling of accretion-powered pulsars, burst oscillation sources, and rotation-powered pulsars. Additional constraints will derive from spin measurements, burst spectra, and properties of the accretion flows in the vicinity of the neutron star. Under development by an international Consortium led by the Institute of High Energy Physics of the Chinese Academy of Science, the eXTP mission is expected to be launched in the mid 2020s.

Abstract

HERMES (High Energy Rapid Modular Ensemble of Satellites) is a space-borne mission based on a constellation of nano-satellites flying in a low-Earth orbit (LEO). The six 3U CubeSat buses host new miniaturized instruments hosting a hybrid Silicon Drift Detector/GAGG:Ce scintillator photodetector system sensitive to X-rays and gamma-rays. HERMES will probe the temporal emission of bright high-energy transients such as Gamma-Ray Bursts (GRBs), ensuring a fast transient localization (with arcmin-level accuracy) in a field of view of several steradians exploiting the triangulation technique. With a foreseen launch date in late 2023, HERMES transient monitoring represents a keystone capability to complement the next generation of gravitational wave experiments. Moreover, the HERMES constellation will operate in conjunction with the Space Industry Responsive Intelligent Thermal (SpIRIT) 6U CubeSat, to be launched in early 2023. SpIRIT is an Australian-Italian mission for high-energy astrophysics that will carry in a Sun-synchronous orbit (SSO) an actively cooled HERMES detector system payload. Temporal pattern analysis is an advanced multivariate technique able to investigate the structure of behavior by unveiling the existence of statistically significant constraints among the interval length separating events in sequence. If on the one hand, such an approach allows investigating the behavioral response to pain in its most intimate and inner features, on the other hand, due to the meaning of the studies on pain, it is of relevant importance that the results utilize intuitive and easily comprehensible ways of representation. The aim of this paper is to show various procedures useful to represent the results originating from the multivariate T-pattern analysis of the behavioral response to pain in Wistar rats tested in a hot-plate and IP injected morphine or saline as a control.

Abstract

Ultra-light primordial black holes (PBHs) in the mass range of 10$^{16}$ - 10$^{22}$ g are allowed by current observations to constitute a significant fraction, if not all, of the dark matter in the Universe. In this work, we present limits on ultra-light, non-rotating PBHs which arise from the non-detection of the Hawking radiation signals from such objects in the keV-MeV energy band. Namely, we consider observations from the current-generation missions XMM-Newton and INTEGRAL/SPI and discuss the observational perspectives of the future missions Athena, eXTP, and THESEUS for PBH searches. Based on 3.4 Msec total exposure time XMM-Newton observations of Draco dwarf spheroidal galaxy, we conclude that PBH with masses $\lesssim 10^{16}$ g can not make all dark matter at 95% confidence level. Our ON-OFF-type analysis of $>100$ Msec of INTEGRAL/SPI data on the Milky Way halo puts significantly stronger constraints. Only $\lesssim 10$% dark matter can be presented by PBHs with masses $\lesssim 3\cdot 10^{16}$ g while the majority of dark matter can not be represented by PBHs lighter than $7\cdot 10^{16}$ g at 95% confidence level. We discuss the strong impact of systematic uncertainty related to the variations of instrumental and astrophysical INTEGRAL/SPI background on the derived results and estimate its level. We also show that future large-field-of-view missions such as THESEUS/X-GIS will be able to improve the constraints by a factor of 10-100 depending on the level of control under the systematics of these instruments., To match accepted for publication in Phys. Abstract

Sydenham’s chorea (SC) is a post-streptococcal autoimmune disorder of the central nervous system, and it is a major criterium for the diagnosis of acute rheumatic fever (ARF). SC typically improves in 12–15 weeks, but patients can be affected for years by persistence and recurrencies of both neurological and neuropsychiatric symptoms. We enrolled 48 patients with a previous diagnosis of ARF, with or without SC, in a national multicenter prospective study, to evaluate the presence of neuropsychiatric symptoms several years after SC’s onset. Our population was divided in a SC group (n = 21), consisting of patients who had SC, and a nSC group (n = 27), consisting of patients who had ARF without SC. Both groups were evaluated by the administration of 8 different neuropsychiatric tests. The Work and Social Adjustment Scale (WSAS) showed significantly (p = 0.021) higher alterations in the SC group than in the nSC group. Furthermore, 60.4% (n = 29) of the overall population experienced neuropsychiatric symptoms other than choreic movements at diagnosis and this finding was significantly more common (p = 0.00) in SC patients (95.2%) than in nSC patients (33.3%). The other neuropsychiatric tests also produced significant results, indicating that SC can exert a strong psychopathological impact on patients even years after its onset.

Abstract

Using observations of X-ray pulsar Her X-1 by the Imaging X-ray Polarimetry Explorer, we report on a highly significant detection of the polarization signal from an accreting neutron star. The observed degree of the linear polarization of ∼ 10% is found to be far below theoretical expectations for this object, and stays low throughout the spin cycle of the pulsar. Both the polarization degree and the angle exhibit variability with pulse phase, which allowed us to measure the pulsar spin position angle and magnetic obliquity of the neutron star, which is an essential step towards detailed modeling of the intrinsic emission of X-ray pulsars. Combining our results with the optical polarimetric data, we find that the spin axis of the neutron star and the angular momentum of the binary orbit are misaligned by at least ∼20 deg, which is a strong argument in support of the models explaining stability of the observed super-orbital variability with the precession of the neutron star.

Abstract

We study the 2018 outburst of Aql X-1 via the monitor of all sky X-ray image (MAXI) data. We show that the outburst starting in February 2018 is a member of short-low class in the frame of outburst duration and the peak count rate although the outburst morphology is slightly different from the other fast-rise-exponential-decay (FRED) type outbursts with a milder rising stage. We study the partial accretion in the weak propeller stage of Aql X-1 via the MAXI data of the 2018 outburst. We report on the spectral analysis of 3 observations of Aquila X-1 obtained by Insight – hard X-ray modulation telescope (Insight-HXMT) during the late decay stage of the 2018 outburst. We discuss that the data taken by Insight-HXMT is just after the transition to the weak propeller stage. Our analysis shows the necessity of a comptonization component to take into account the existence of an electron cloud resulting photons partly up-scattered.

Abstract

To constrain the equation of state of cold dense matter, astrophysical measurements are essential. These are mostly based on observations of neutron stars in the X-ray band, and, more recently, also on gravitational wave observations. Of particular interest are observations of unusually heavy or light neutron stars which extend the range of central densities probed by observations and thus permit testing nuclear physics predictions over a wider parameter space. Here we report on the analysis of such a star, a central compact object within the supernova remnant HESS J1731-347. We estimate the mass and radius of the neutron star to be M = 0.77(20)Msun and R = 10.4(8) km, respectively, based on modeling of the X-ray spectrum and a robust distance estimate from Gaia observations. Our estimate implies that this object is either the lightest neutron star known, or a "strange star” with a more exotic equation of state. Adopting a standard neutron star matter hypothesis significantly constrains the corresponding equations of state.

Abstract

Highly extended gamma-ray emission around the Geminga pulsar was discovered by Milagro and verified by HAWC. Despite many observations with Imaging Atmospheric Cherenkov Telescopes (IACTs), detection of gamma-ray emission on angular scales exceeding the IACT field-of-view has proven challenging. Recent developments in analysis techniques have enabled the detection of significant emission around Geminga in archival data with H.E.S.S.. In 2019, further data on the Geminga region were obtained with an adapted observation strategy. Following the announcement of the detection of significant TeV emission around Geminga in archival data, in this contribution we present the detection in an independent dataset. Abstract

The realtime follow-up of neutrino events is a promising approach to search for astrophysical neutrino sources. It has so far provided compelling evidence for a neutrino point source: the flaring gamma-ray blazar TXS 0506+056 observed in coincidence with the high-energy neutrino IceCube-170922A detected by IceCube. The detection of very-high-energy gamma rays (VHE, $\mathrm{E} > 100\,\mathrm{GeV}$) from this source helped establish the coincidence and constrained the modeling of the blazar emission at the time of the IceCube event. The four major imaging atmospheric Cherenkov telescope arrays (IACTs) - FACT, H.E.S.S., MAGIC, and VERITAS - operate an active follow-up program of target-of-opportunity observations of neutrino alerts sent by IceCube. This program has two main components. One are the observations of known gamma-ray sources around which a cluster of candidate neutrino events has been identified by IceCube (Gamma-ray Follow-Up, GFU). Second one is the follow-up of single high-energy neutrino candidate events of potential astrophysical origin such as IceCube-170922A. GFU has been recently upgraded by IceCube in collaboration with the IACT groups. Abstract

The Large Area Detector (LAD) is the high-throughput, spectral-timing instrument onboard the eXTP mission, a flagship mission of the Chinese Academy of Sciences and the China National Space Administration, with a large European participation coordinated by Italy and Spain. The eXTP mission is currently performing its phase B study, with a target launch at the end-2027. The eXTP scientific payload includes four instruments (SFA, PFA, LAD and WFM) offering unprecedented simultaneous wide-band X-ray timing and polarimetry sensitivity. The LAD instrument is based on the design originally proposed for the LOFT mission. It envisages a deployed 3.2 m2 effective area in the 2-30 keV energy range, achieved through the technology of the large-area Silicon Drift Detectors - offering a spectral resolution of up to 200 eV FWHM at 6 keV - and of capillary plate collimators - limiting the field of view to about 1 degree. In this paper we will provide an overview of the LAD instrument design, its current status of development and anticipated performance.

Abstract

Background: Poirier–Bienvenu Neurodevelopmental Syndrome (POBINDS) is a rare disease linked to mutations of the CSNK2B gene, which encodes for a subunit of caseinkinase CK2 involved in neuronal growth and synaptic transmission. Its main features include early-onset epilepsy and intellectual disability. Despite the lack of cases described, it appears that POBINDS could manifest with a wide range of phenotypes, possibly related to the different mutations of CSNK2B. Methods: Our multicentric, retrospective study recruited nine patients with POBINDS, detected using next-generation sequencing panels and whole-exome sequencing. Clinical, laboratory, and neuroimaging data were reported for each patient in order to assess the severity of phenotype, and eventually, a correlation with the type of CSNK2B mutation. Results: We reported nine unrelated patients with heterozygous de novo mutations of the CSNK2B gene. All cases presented epilepsy, and eight patients were associated with a different degree of intellectual disability. Other features detected included endocrinological and vascular abnormalities and dysmorphisms. Genetic analysis revealed six new variants of CSNK2B that have not been reported previously. Conclusion: Although it was not possible to assess a genotype–phenotype correlation in our patients, our research further expands the phenotype spectrum of POBINDS patients, identifying new mutations occurring in the CSNK2B gene.

Abstract

Primordial Black Holes are expected to be formed in the early Universe by the gravitational collapse of overdense regions, among other mechanisms. They are also expected to loose their mass over time by the Hawking radiation process. As the rates of this radiation increase with temperature, the PBH evaporation should result in a violent explosion. The current upper limits on explosion rates are on the order of 10$^{4}$ − 10$^{5}$pc$^{-3}$yr$^{-1}$. In this contribution we'll present the results of a search for TeV γ-ray burst within timescale of few seconds, using nearly 5000 hours of H.E.S.S. data. The search algorithm and statistical estimation strategy will be presented as well as cosmological implications of this measurement.

Abstract

Extreme high synchrotron peaked blazars (EHBLs) are amongst the most powerful accelerators found in nature. Usually the synchrotron peak frequency of an EHBL is above $10^{17}\,$Hz, i.e., lies in the range of medium to hard X-rays making them ideal sources to study particle acceleration and radiative processes. EHBL objects are commonly observed at energies beyond several TeV, making them also powerful probes of gamma-ray absorption in the intergalactic medium. During the last decade, several attempts have been made to increase the number of EHBL detected at TeV energies and probe their spectral characteristics. Here we report new detections of EHBLs in the TeV energy regime, each at a redshift of less than 0.2, by the High Energy Stereoscopic System (H.E.S.S.). Also, we report on X-ray observations of these EHBLs candidates with Swift XRT. In conjunction with the very high energy observations, this allows us to probe the radiation mechanisms and the underlying particle acceleration processes., Proceedings of the 37th International Cosmic Ray Conference (ICRC 2021)

Abstract

While the youngest known supernova remnants (SNRs), such as Cassiopeia A (Cas A), have been proven to be able to accelerate cosmic rays (CRs) only up to ∼ 10$^{14}$ eV at their present evolutionary stages, recent studies have shown that particle energies larger than a few PeV (10$^{15}$ eV) could be reached during the early stages of a core-collapse Supernova (cc-SN), when the high-velocity forward shock expands into the dense circumstellar medium (CSM) shaped by the stellar progenitor wind. Such environments, in particular the type IIn SNe whose progenitors may exhibit mass loss rates as high as 10$^{-2}$ M$_{\bigodot}$ yr$^{-1}$, could thus lead to γ-ray emission from $\pi ^{0}$ decay in hadronic interactions, potentially detectable with current Cherenkov telescopes at very-high energies. Such a detection would provide direct evidence for efficient acceleration of CR protons/nuclei in supernovae, and hence new insights on the long-standing issue of the origin of Galactic Cosmic Rays. In that context, the High Energy Stereoscopic System (H.E.S.S.) has been carrying out a Target of Opportunity program since 2016 to search for such an early very-high-energy γ-ray emission towards nearby core-collapse supernovae and supernova candidates (up to ∼ 10 Mpc), within a few weeks after discovery. After giving an overview of this H.E.S.S. Target of Opportunity program, we present the results obtained from the July 2019 observations towards the transient AT2019krl, originally classified as a type IIn supernova, which occurred in the galaxy M74 at ∼ 9.8 Mpc. Although its nature still remains unclear, the derived H.E.S.S. constraints on this transient are placed in the general context of the expected VHE γ-ray emission from core-collapse supernovae.

Context. In July 2015, the high-mass X-ray binary V0332+53 underwent a giant outburst, a decade after the previous one. V0332+53 hosts a strongly magnetized neutron star. During the 2004-2005 outburst, an anti-correlation between the centroid energy of its fundamental cyclotron resonance scattering features (CRSFs) and the X-ray luminosity was observed. Aims. The long ( 100 d) and bright (Lx approx. 10(exp 38) ergs(exp -1)) 2015 outburst offered the opportunity to study during another outburst the unique properties of the fundamental CRSF and its dependence on the X-ray luminosity. Methods. The source was observed by the INTEGRAL satellite for approximately 330 ks. We exploit the spectral resolution at high energies of the SPectrometer on INTEGRAL (SPI) and the Joint European X-ray Monitors to characterize its spectral properties, focusing in particular on the CRSF-luminosity dependence. We complement the data of the 2015 outburst with those collected by SPI in 2004-2005 and left unpublished so far. Results. We find a highly significant anti-correlation of the centroid energy of the fundamental CRSF and the 3-100 keV luminosity of E1 alpha -0:095(8)L(sub 37) keV. This trend is observed for both outbursts. We confirm the correlation between the width of the fundamental CRSF and the X-ray luminosity previously found in the JEM-X and IBIS dataset of the 2004-2005 outburst. By exploiting the RXTE/ASM and Swift/BAT monitoring data we also report on the detection of a approximately 34 d modulation superimposed on the mean profiles and roughly consistent with the orbital period of the pulsar. We discuss possible interpretations of such variability.

Abstract

Background and aims Recent studies have showed that in emergency department (ED) pediatric admissions for headache are increasing in the last years. However Covid-19 pandemic may have changed the use of health services for several reasons. Aim of this study is to analyze the rates of admission for pediatric headaches in ED before and during Covid19 Pandemic. Methods we have collected retrospectively the records of children (range of age 5–14) admitted on ED in 2012, 2019 and 2020. We selected the records including Headache and Headache associated to other symptoms (vomit, fever, dizziness, etc.), collecting further the use of computed tomography (CT) and neurological consultation. Results In 2012, 2019 and 2020 the cephalalgic children admitted to ED were respectively 313/18806 (1.66%), 407/15605 (2.61%) and 234/ 9630 (2.43%). The admission rates for headaches shows highly significant differences between 2012 and biennial 2019/2020. There are no differences in use of CT and neurological consultations. The only difference in access was the initial drop in the first months of lockdown (2020/80% vs 2019/50%). Conclusions Our data support the increase of admission for headache to the pediatric ED in the last ten years. However the Covid-19 pandemic has not increased the admission rate compared to 2019 neither the use of CT or neurological consultations. The fear of using EDs was no changed for headache compared to other pediatric alarm symptoms.

Abstract

Motivated by the alleged detection of a cyclotron resonance scattering feature in the X-ray spectrum of the Be-transient KS 1947+300 with NuSTAR, not observed with other observatories, we have revisited the available observations of the source to clarify the origin of the reported feature. We focus on the analysis of observations obtained with the BeppoSAX observatory during the 2001 outburst, and by NuSTAR in 2013, when the discovery of the line was reported. We find that in both cases, the source broad-band X-ray continuum can be well described with a two-component Comptonization model, which does not require an additional absorption-like feature. In addition, we show that low-significance residuals mimicking the line, which appear using different continuum models, are consistent with known calibration uncertainties. We therefore conclude that the reported cyclotron line detection is largely questionable, and thus the question on the strength of the source magnetic field remains open. We briefly discuss the origin of the observed broad-band continuum in the context of the pulse-phase-resolved analysis conducted for BeppoSAX data and speculate that the soft component may be associated with the emission from polar areas of the pulsar irradiated by the accretion column.

Abstract

The evidences for the influence of thermonuclear (type-I) X-ray bursts upon the surrounding environments in neutron star low-mass X-ray binaries (LMXB) were detected previously via spectral and timing analyses. Benefitting from a broad energy coverage of Insight-HXMT, we analyze one photospheric radius expansion (PRE) burst, and find an emission excess at soft X-rays. Our spectral analysis shows that, such an excess is not likely relevant to the disk reflection induced by the burst emission and can be attributed to an enhanced pre-burst/persistent emission. We find that the burst and enhanced persistent emissions sum up to exceed Eddington luminosity by ∼ 40 percentages. We speculate that the enhanced emission is from a region beyond the PRE radius, or through the Comptonization of the corona.
