Aim – to study the integral scale for assessing enteral insufficiency in patients with acute pancreatitis during the phases of enzyme toxemia and purulent-destructive complications; to develop a strategy for choosing a method for enteral insufficiency correction. Material and methods. We have analyzed the treatment outcomes in 232 patients with acute pancreatitis admitted to the surgical department of the Samara Regional Clinical Hospital n. a. V.D. Seredavin for the period from 2013 to 2019. Two study groups were formed: retrospective and prospective. The retrospective (control) group included 175 patients whose treatment outcomes were assessed retrospectively for the parameters of the enteral insufficiency syndrome and measures were suggested for its differentiated correction. The prospective (intervention) group included 57 patients who were treated using the developed principles of enteral insufficiency correction. Results. The occurrence of I and II degrees of enteral insufficiency differed significantly depending on the phase of acute pancreatitis. When using complex correction of enteral insufficiency according to the proposed scheme, in patients in the phase of enzyme toxemia the I degree enteral insufficiency was prevalent, the patients in the purulent-destructive phase were more likely to have II degree enteral insufficiency. Conclusion. The timely inclusion of enteral insufficiency correction into the complex of therapeutic measures for pancreatitis may improve the treatment outcome in patients with moderate and severe phases of acute pancreatitis.

Optical coherence tomography (OCT) is a non-invasive imaging technique widely used in ophthalmology for visualizing retinal layers, aiding in the early detection and monitoring of retinal diseases. OCT is useful for detecting diseases such as age-related macular degeneration (AMD) and diabetic macular edema (DME), which affect millions of people globally. Over the past decade, the area of application of artificial intelligence (AI), particularly deep learning (DL), has significantly increased. The number of medical applications is also rising, with solutions from other domains being increasingly applied to OCT. The segmentation of biomarkers is an essential problem that can enhance the quality of retinal disease diagnostics. For 3D OCT scans, AI is beneficial since manual segmentation is very labor-intensive. In this paper, we employ the new SAM 2 and MedSAM 2 for the segmentation of OCT volumes for two open-source datasets, comparing their performance with the traditional U-Net. The model achieved an overall Dice score of 0.913 and 0.902 for macular holes (MH) and intraretinal cysts (IRC) on OIMHS and 0.888 and 0.909 for intraretinal fluid (IRF) and pigment epithelial detachment (PED) on the AROI dataset, respectively.

Electroretinography (ERG) is a non-invasive method of assessing retinal function by recording the retina's response to a brief flash of light. This study focused on optimizing the ERG waveform signal classification by utilizing Short-Time Fourier Transform (STFT) spectrogram preprocessing with a machine learning (ML) decision system. Several window functions of different sizes and window overlaps were compared to enhance feature extraction concerning specific ML algorithms. The obtained spectrograms were employed to train deep learning models alongside manual feature extraction for more classical ML models. Our findings demonstrated the superiority of utilizing the Visual Transformer architecture with a Hamming window function, showcasing its advantage in ERG signal classification. Also, as a result, we recommend the RF algorithm for scenarios necessitating manual feature extraction, particularly with the Boxcar (rectangular) or Bartlett window functions. By elucidating the optimal methodologies for feature extraction and classification, this study contributes to advancing the diagnostic capabilities of ERG analysis in clinical settings.

Visual electrophysiology is often used clinically to determine the functional changes associated with retinal or neurological conditions. The full-field flash electroretinogram (ERG) assesses the global contribution of the outer and inner retinal layers initiated by the rods and cone pathways depending on the state of retinal adaptation. Within clinical centers, reference normative data are used to compare clinical cases that may be rare or underpowered within a specific demographic. To bolster either the reference dataset or the case dataset, the application of synthetic ERG waveforms may offer benefits to disease classification and case-control studies. In this study and as a proof of concept, artificial intelligence (AI) to generate synthetic signals using generative adversarial networks is deployed to upscale male participants within an ISCEV reference dataset containing 68 participants, with waveforms from the right and left eye. Random forest classifiers further improved classification for sex within the group from a balanced accuracy of 0.72-0.83 with the added synthetic male waveforms. Optical coherence tomography (OCT) is a non-invasive imaging technique with extensive clinical applications in ophthalmology. OCT enables the visualization of the retinal layers, playing a vital role in the early detection and monitoring of retinal diseases. OCT uses the principle of light wave interference to create detailed images of the retinal microstructures, making it a valuable tool for diagnosing ocular conditions. This work presents an open-access OCT dataset (OCTDL) comprising over 2000 OCT images labeled according to disease group and retinal pathology. The dataset consists of OCT records of patients with Age-related Macular Degeneration (AMD), Diabetic Macular Edema (DME), Epiretinal Membrane (ERM), Retinal Artery Occlusion (RAO), Retinal Vein Occlusion (RVO), and Vitreomacular Interface Disease (VID). The images were acquired with an Optovue Avanti RTVue XR using raster scanning protocols with dynamic scan length and image resolution. Each retinal b-scan was acquired by centering on the fovea and interpreted and cataloged by an experienced retinal specialist. The electroretinogram (ERG) is a clinical test that records the retina's electrical response to light. Analysis of the ERG signal offers a promising way to study different retinal diseases and disorders. Machine learning-based methods are expected to play a pivotal role in achieving the goals of retinal diagnostics and treatment control. This study aims to improve the classification accuracy of the previous work using the combination of three optimal mother wavelet functions. We apply Continuous Wavelet Transform (CWT) on a dataset of mixed pediatric and adult ERG signals and show the possibility of simultaneous analysis of the signals. The modern Visual Transformer-based architectures are tested on a time-frequency representation of the signals. The method provides 88% classification accuracy for Maximum 2.0 ERG, 85% for Scotopic 2.0, and 91% for Photopic 2.0 protocols, which on average improves the result by 7.6% compared to previous work.

The continuous advancements in healthcare technology have empowered the discovery, diagnosis, and prediction of diseases, revolutionizing the field. Artificial intelligence (AI) is expected to play a pivotal role in achieving the goals of precision medicine, particularly in disease prevention, detection, and personalized treatment. This study aims to determine the optimal combination of the mother wavelet and AI model for the analysis of pediatric electroretinogram (ERG) signals. The dataset, consisting of signals and corresponding diagnoses, undergoes Continuous Wavelet Transform (CWT) using commonly used wavelets to obtain a time-frequency representation. Wavelet images were used for the training of five widely used deep learning models: VGG-11, ResNet-50, DensNet-121, ResNext-50, and Vision Transformer, to evaluate their accuracy in classifying healthy and unhealthy patients. The findings demonstrate that the combination of Ricker Wavelet and Vision Transformer consistently yields the highest median accuracy values for ERG analysis, as evidenced by the upper and lower quartile values. The median balanced accuracy of the obtained combination of the three considered types of ERG signals in the article are 0.83, 0.85, and 0.88. However, other wavelet types also achieved high accuracy levels, indicating the importance of carefully selecting the mother wavelet for accurate classification. The study provides valuable insights into the effectiveness of different combinations of wavelets and models in classifying ERG wavelet scalograms.

Background: The electroretinogram is a clinical test used to assess the function of the photoreceptors and retinal circuits of various cells in the eye, with the recorded waveform being the result of the summated response of neural generators across the retina., Methods: The present investigation involved an analysis of the electroretinogram waveform in both the time and time-frequency domains through the utilization of the discrete wavelet transform and continuous wavelet transform techniques. The primary aim of this study was to monitor and evaluate the effects of treatment in a New Zealand rabbit model of endophthalmitis via electroretinogram waveform analysis and to compare these with normal human electroretinograms., Results: The wavelet scalograms were analyzed using various mother wavelets, including the Daubechies, Ricker, Wavelet Biorthogonal 3.1 (bior3.1), Morlet, Haar, and Gaussian wavelets. Distinctive variances were identified in the wavelet scalograms between rabbit and human electroretinograms. The wavelet scalograms in the rabbit model of endophthalmitis showed recovery with treatment in parallel with the time-domain features., Conclusions: The study compared adult, child, and rabbit electroretinogram responses using DWT and CWT, finding that adult signals had higher power than child signals, and that rabbit signals showed differences in the a-wave and b-wave depending on the type of response tested, while the Haar wavelet was found to be superior in visualizing frequency components in electrophysiological signals for following the treatment of endophthalmitis and may give additional outcome measures for the management of retinal disease.
